A comparison of high-risk pregnant ( n = 55) and non-pregnant ( n = 598) women from Harlem, New York City on HIV-related drug and sexual risk behaviors was undertaken to identify appropriate prevention and intervention policy recommendations. Clients were recruited for an AIDS risk reduction research demonstration project and were either intravenous drug users (IVDUs) or sexual partners of IVDUs. There was a higher percentage of IVDUs among the non-pregnant women, and no significant differences were found between pregnant and non-pregnant IVDUs in terms of needle risk behaviors. The two groups were also similar in non-injected drug use behaviors. Pregnant women were significantly less likely to use condoms. Recommendations focused on the need for increased HIV risk reduction among high-risk women in general, and increased education efforts among pregnant high-risk women regarding condom use as protection from HIV and the potential consequences of maternal substance use for their children.

SynopsisThe diagnostic specificity and predictive utility of the classical prognostic indicators in schizophrenia were examined in psychotic patients enrolled in the Suffolk County Mental Health Project. First-admission psychotic patients with schizophrenia (N= 96), major depression (N= 42), and bipolar disorder (N= 64) drawn from 10 facilities in Suffolk County, New York, were assessed during their initial hospitalization and at 6–month follow-up. Longitudinal consensus diagnoses were determined after the 6-month interview. The diagnostic groups shared similar background characteristics, but schizophrenics had poorer pre-morbid adjustment, longer periods of psychosis before hospitalization and more negative symptoms initially. Except for rehospitalization, schizophrenics had the worst and bipolars the best functioning at follow-up. Among the classical prognostic indicators, the best predictor of 6-month outcome for each diagnostic group was premorbid functioning.

Abstract Background: Introduction: 1 in 6 Americans has or will have a sensory or communication disorder in their lifetime. In New York, approximately 12.6% of the population is affected by some degree of hearing loss or vestibular dysfunction. Osteopathic medical student auditory screenings have the potential to positively impact communities in screening for auditory disorders. The aim of this study is to argue for the importance of incorporating auditory screening programs into osteopathic student-run health fairs to positively impact underserved communities. Methods: Osteopathic medical students developed an auditory screening protocol and training session to detect auditory pathologies in health fair participants. Screenings included patient intake, hearing loss risk factor discussion questions, and audiological testing such as the whispered voice and turning fork tests. Participants of Touro College of Osteopathic Medicine’s fall and spring health fairs were invited to undergo the screening from September 2017 to March 2018 in New York City, New York. Results: During the inaugural health fairs, 28 participants were screened for auditory pathologies. 6 participants (21%) were referred for additional testing due to abnormal or inconclusive results. Median age of participants was 60 with 19 females and 9 males, IQR 5. Participants primarily self-identified as African American/Black (54%, n=15) or Hispanic (29%, n=8). Conclusion: The results of this pilot project underscore the benefits of effective auditory screening programs at osteopathic student-run health fairs, especially in urban, underserved communities.

7023 Background: Expanded Access (EA), also known as “compassionate use,” is a regulatory pathway in which a patient with an immediate life-threatening condition or disease can gain access to an investigational product for treatment when no satisfactory therapy is available. Oncology practices may lack the regulatory experience or administrative support to use EA. In response, FDA OCE launched Project Facilitate (PF), a call center to assist oncology healthcare providers requesting EA. An analysis of single-patient investigational new drugs (IND) was performed to assess the first 10 months of PF compared to the period prior to its launch. Methods: Preliminary data was extracted from the FDA’s central database that yielded 719 single-patient INDs between May 31-November 30, 2018 & 2019 in the Office of Oncologic Diseases (OOD). Data collected included IND receipt date, acknowledgment date, application status, drug name, underlying malignancy of patient, address of requesting physician, withdrawal date, and patient demographics. A manual review of INDs was performed to assess for actual processing dates and to capture demographics not captured by the database. A total of 28 INDs were excluded due to duplications, cancellation by Sponsor prior to issuance of FDA decision, or coding errors in the database. Industry denial explanations were reported by the provider by emails. Results: Data from 692 INDs were analyzed and 692 (100%) were granted safe to proceed. The median processing time was 1 day (mean=2) in 2018 and 1 day (mean= 1.5) in 2019. Our findings indicate that the volume of oncology EA requests increased by 76 (19%) in 2019 vs 2018. A total of 207 unique drugs were requested. Malignancies most frequently involved included: Acute myeloid leukemia (n = 84, 8.3%), soft tissue sarcoma (n = 77, 7.6%), and non-small cell lung cancer (n = 60, 5.9%). States with the highest requests included: California (n = 82, 11.8%), New York (n = 81, 11.7%), and Massachusetts (n = 42, 6.1%). A majority of requests were from major academic centers (77%). All denied requests (N = 9) by industry were due to company’s decision to not provide products outside of a clinical trial. Conclusions: The positive trends in decreased processing times and increased number of requests are consistent with OCE’s mission to improve efficiency of the EA program and ensure equitable access to all oncology patients. To extract favorable terms from local governments, developers can take advantage of intermunicipal competition and threaten to relocate or abandon a land development project. Some say that model of regional competition undercuts the economic, environmental, and societal benefits of development while others claim it boosts the efficiency of municipal operations. The authors examine the likelihood that land developers will push local governments to compete against each other for projects. Their study of New York municipalities ( N = 306) finds that one third of local governments have been threatened and that one third of those reacted by reducing burdens or increasing incentives for developers. Their logistic regression model reveals that more cooperative intermunicipal relations across a region decrease the chances of developers threatening to move projects. Interestingly, the model suggests that public participation increases the chances of a threat.

AbstractOutcome studies have established that successful community living is compromised in the population of individuals with traumatic brain injury and chronic behavioural difficulties along with a co-occurring diagnosis of substance abuse and/or mental health disorder. Two studies are presented. The first was aimed at describing long-term outcome of a sample of individuals (N = 51) served by the New York State Department of Health TBI Medicaid Waiver Program. Each of the participants was diagnosed with TBI plus either substance abuse or a mental health disorder, or both. Because of significant behavioural challenges, all of the participants were in a restrictive living setting the year before enrolment in the waiver program (e.g., nursing or correctional facility). Data on community living arrangement, self-reported community integration experiences, and costs are presented. Results indicate that most of the participants (41 of the 46 who were alive and living in state) continued to live in the community 8 to 9 years after commencement of community support services. The participants' community integration responses were generally positive and cost data demonstrate substantial savings to the state for this cohort. Comparing prewaiver costs in residential settings with most recent (2005) costs for community supports, there was an average daily cost savings of US$137 per person for the 1996 cohort and US$144 per person for the 1997 cohort. The second study explored the use of project-oriented interventions and supports in an agency that provides community support services to this dual diagnosis population. Project-oriented services are described as meeting many needs common to this dual-diagnosis population. Clinical staff (N = 11) and a sample of waiver participants (N = 7) were surveyed. Results suggest that the use of personally meaningful projects can become a clinical habit for staff and that projects are generally judged by participants to be a meaningful use of time, and significant in giving them an opportunity to play an expert role and to help others.

AbstractThe chapter outlines the cultural background for applying design strategies consistent with the challenge of circularity. The contribution focuses on ecological thinking as an effective design approach to produce and implement eco-innovative strategies able at facing environmental and societal challenges of our global age. Then the chapter depicts the Repair research experience in promoting a systemic design approach for recycling and reusing C&D waste as new, anthropogenic soils in peri-urban areas. According to the EEA Report n.6/2017, the chapter posits that the major environmental challenges of the present are not about single issues, such as waste reduction or soil-loss, rather they involve systemic change and design processes, linking together economy, social habits and technological responses. Therefore, the transition towards more sustainable urban metabolism deeply depends from creative visions by which breaking the circuit “take-make-dispose” and promote new—and somehow tentative—visions for implementing circularity at local and global scale. Further postulation in the paper is about assuming the concept of Anthropocene as theoretical ground for such eco-innovative design approach. The scientific evidence of living in human-dominated ecosystems makes designers towards a paradigm shift concerning the overcoming of the typical artificial/natural dichotomy by exploring the augmented opportunities in designing sustainable and resilient habitats thanks to a more collaborative, plural and innovative design approach: “What is important and significant here is how ecology and landscape architectural design might invent alternative forms of relationships between people, places and cosmos” (Corner, ‘Ecology and Landscape as agents of Creativity’, 1997, reprint in Reed &Lister (2018), Op. Cit., pp. 40–65, p. 42). Starting from these assumptions, the paper deepens the experience of collaborative design for implementing recycle and reuse of C&D waste for producing new technical soils, according to both the regulatory constraints (and potentials) and the site-specific features. The research goal is to provide new vegetated soils by waste thanks to an innovative design process based on both circular economy principles and collaborative knowledge production. Notably, the capacity of producing creative hybridization between biotic and abiotic component seems to be the new frontier in the field of technological design and material engineering. The term hypernatural, proposed by Blaine Brownell and Marc Swackhamer in 2015, introduces the idea of a co-evolutionary process between nature and science, looking at humans’ technological capacity as an effective opportunity for creating the conditions for making biotic ad abiotic systems working together: “The ultimate aim of technology is not antinatural: it is hypernatural” (Brownell & Swackhamer in Hyper-natural. Architecture’s new relationship with nature. Princeton Architectural Press, New York, p. 18, 2015). The chapter deals with the methodology applied for promoting a sort of protocological architecture (Burke, 2007), by which facilitating the C&D waste recycle and reuse within the construction sector, and notably into the landscape project. The research starts working under the H2020-Repair project, and it has developed within further research programs about C&D waste management in urban regeneration programs developed by the Department of Architecture of University of Naples Federico II.

This chapter analyzes how, from her time as a young performance poet in New York in the late 1960s to her current position as punk rock’s éminence grise, Patti Smith foregrounded the image of the poet as privileged seer. Simultaneously, Smith rejected stereotypically “feminine” personae emphatically both in terms of the content of her writing and in her very style when performing on stage. Much like Richard Hell’s response to the St. Mark’s scene, Smith developed vatic postures and made gender trouble within the context of her relationship to the Poetry Project. The Poetry Project proved a site in which Smith negotiated friendship literally and metaphorically as a way to establish herself in New York’s downtown scene, from which she launched herself into the world of corporate record labels and rock ‘n’ roll concert arenas. Smith’s friendship with Project-affiliated poets was equal parts target-based ingratiation and strategic distantiation verging at times into overt disrespect. This distantiation, performed fairly consistently in interviews during the early 1970s and re-invoked (if in a much-tempered version) in her memoir Just Kids (2010), successfully kept Smith from becoming fully absorbed into the Poetry Project scene.

As many of the poets considered here belonged to poetry ‘movements’ which constituted themselves through journals, this chapter is structured around those journals, considering the various ways in which small poetry periodicals used Barthes. Considering reviews, interviews, and manifestos, it offers a narrative of Barthes in notable magazines such as the New York-based L=A=N=G=U=A=G=E and its Canadian counterpart Open Letter, and smaller publications such as Bloomington, Indiana’s Wch Way. In this way, it includes the work of poets and critics whose engagement with Barthes is more tangential and bound up with the general project of a community of writers, such as Michael Palmer and Steve McCaffery.

All those gradual adjustments are manifested according to the demand, bring toward a less formal and more flexible spatial order, for which the traditional forms of the "static" city become the background of the "kinetic" landscape of informal cities. On the contrary of the formal processes of urban planning, informality process is configured as an organic development model and a flexible dynamic system opened to changes. The informal space is produced according to principles of spontaneity and self-organization. A consideration on the possibility to assume different approaches can be proposed. Those approaches should integrate in the design reasoning all the dynamics usually excluded by the discourse on the urban project, which processes can become catalysts to enrich the methods of planning and design of the urban space. Through the analysis of the case-study Previ Lima and the Living Room at the Border of St. Ysidro, the aim is to delineate in which way the contemporary architecture can absorb and metabolize these processes, triggering a different approach to a different method to intervene in the spaces of relationship among formal and informal. All these elements configure an urban nucleus that, due to its urban, architectural and landscape qualities, gives rise to one of the main tourist attractions of the region. However, the area described nowadays presents an important problem related to the use and habitability of public space, which is invaded by the presence of the private vehicle, even along the seaside, due to its touristic relevance. This article presents the results of an academic experience developed to study different possibilities of urban transformations for the municipality of Altea, taking as a project site the urban vacuum still conserved between the two situations previously described: the historical areas on the coastal elevations (Dalt) and new urban developments parallel to the seaside (Baix). This academic activity, performed by nearly 50 students from the University of Alicante, was developed in the context of the design course Urbanism 5 during the academic year 2015-16, thanks to the agreement signed between the Municipality of Altea and the University of Alicante. The digital transformation in the world of work has profound effects on the processes of career orientation and the transition between school and work. Together with international partners from Bulgaria, Germany, Greece, Spain, Italy and Hungary, a digital mentoring concept to secure the employability of young people has been or is being investigated in the three-year EU project "Career 4.0". The focus is on the further development of a personal development plan with the help of which the young people can reflect on their future employment opportunities. Compared to other teaching-learning situations, this is a learning process that is open to development without a predetermined true or false, as is usually the case with mathematical tasks, for example. This places special demands on the mentors when it comes to assessing which forms of feedback are particularly beneficial for the young people and which prove to be less beneficial.Within the framework of the EU project, empirical studies were carried out which came to the conclusion that the quality of the feedback that mentors give to mentees is assessed very dif-ferently by these groups of participants. The mentees see considerable potential for improve-ment when it comes to the quality of the feedback from the mentors. In contrast, the mentors themselves are not as critical of their activities in giving feedback. Over 60 mentees and over 30 mentors have participated in the empirical study so far.The starting point for the study is the meta-analysis of the research team around Hattie et al. (2016). They differentiate between the following forms of feedback: (1) task-related, (2) pro-cess-related, (3) self-regulation-related and (4) person- or self-related feedback. According to the evaluation of their meta-analysis, the second and third forms of feedback have the greatest effect on learning outcomes.Furthermore, scientific studies have shown that the acceptance of feedback depends on numerous influencing factors, which can be assigned to four areas: Characteristics of (1) the feedback message, (2) the feedback source, (3) the feedback recipient and (4) the feedback context. The effect of feedback can be related to three levels, following the psychology of lear-ning: (1) cognitive (e.g. closing competence gaps), (2) metacognitive (e.g. supporting self-assessment and self-awareness) and (3) motivational level (e.g. promoting readiness). How the feedback recipients (here: the young people) ultimately deal with the feedback also depends on their causal attribution, i.e. which reasons they see as causal for their progress or the failure of their actions. If, for example, they attribute their inadequate task performance to environmental factors, e.g. difficult and unfair tasks or disproportionate time pressure, or if they see the reasons in themselves, e.g. in their lack of commitment or insufficient skills, this has very different effects on the effects of the feedback. Among other things, this can lead to a "self-esteem distortion" if, for example, negative results are primarily attributed to external circumstances. The research project is also investigating the extent to which AI tools can help to make feed-back even more effective and efficient for learners. In order to provide IT and AI solutions (such as adaptive learning systems, learning analytics, intelligent CBR recommendation sys-tems) to support the giving of feedback, e.g. with the help of a learning platform, it is advantageous and necessary to make the feedback process transparent by using a process mo-delling approach and to work out individual process steps.Hattie, J. & Timperley, H.
