ENGLISH ABSTRACT: The topic of antiretroviral adherence remains a subject of continued importance, as it is associated with positive health outcomes amongst patients attending public healthcare facilities. Available literature on adherence behaviour mainly focuses on the psychological and behavioural barriers, while overlooking the multitude of structural barriers within the patient’s environment affecting the patient’s adherence to antiretroviral treatment and care. The present study provides a unique perspective on adherence behaviour amongst persons living with HIV and receiving antiretroviral treatment, as it identifies important structural barriers to clinical attendance and pill-taking. The sample for this study were selected from patients attending an infectious diseases clinic at a major peri-urban secondary hospital and receiving antiretroviral therapy, nurses and doctors providing health services to patients, and patient advocates providing psychosocial support to patients under the auspices of a local non-governmental organisation. The participants included in this study were selected by means of convenience sampling to participate either in semi structured interviews or focus group discussions. Participants were assured of the confidentiality of the process and their anonymity in both cases. Both semi structured interviews and focus groups were digitally recorded and transcribed after which transcriptions were entered into Atlas.ti for textual analysis. Transcriptions were thematically analysed according to the perceptions of various participants. The main themes that emerged from the present study included individual barriers, poverty-related barriers, institutionrelated barriers, and social and community-related barriers. The results of the present study were triangulated by considering the concurrences and discrepancies between the patients, clinicians and patient advocates on the main, and subthemes. These themes were then discussed according to Bronfenbrenner’s (1972) Ecological Systems Theory, which divided the main themes identified according to the different systems operating within the patient’s environment, i.e. the micro-, exo-, and macrosystem. The microsystem included both individual psychological and behavioural barriers and poverty-related barriers. Institutional barriers were considered within the exosystem of the patient’s ecological environment. And the social and community-related barriers were considered within the macrosystem of the patient’s ecological environment. The significance of this study lies in the identification of adherence behaviour as the product of the patient’s environment through the examination of triangulated data. Future research may include effective ways in which patients can be assisted in developing the necessary skills to cope with their environment and to enhance social support. The development of strategies to support newly-enrolled patients also still needs investigation.
AFRIKAANSE OPSOMMING: Volgehoue antiretrovirale behandeling bly ‘n onderwerp van voortdurende belang omdat dit geassosieer word met positiewe gesondheidsuitkomste onder pasiënte wat van openbare gesondheidsfasiliteite gebruik maak. Beskikbare literatuur oor volhoudings gedrag fokus grootliks op sielkundige en gedragshindernisse, terwyl veelvuldige strukturelehindernisse binne die pasiënt se omgewing steeds misgekyk word. Dié studie bied ‘n unieke perspektief op volhoudingsgedrag onder MIV-positiewe pasiënte wat tans antiretrovirale terapie ontvang, aangesien dit belangrike strukturele hindernisse tot kliniek bywoning en die neem van medikasie identifiseer. Dié steekproef sluit pasiënte in wat tans antiretrovirale terapie by ‘n aansteeklike siektes-kliniek by ‘n peri-stedelike sekondêre hospitaal ontvang. Dit sluit ook dokters en verpleegsters in wat gesondheidsdienste aan dié pasiënte verskaf, en pasiënt- advokate wat psigo-sosiale ondersteuning aan pasiënte verskaf onder die vaandel van ‘n plaaslike nieregerings organisasie. Dié deelnemers is deur middel van gerieflikheidssteekproef geselekteer om aan semi-gestruktureerde onderhoude of fokusgroepbesprekings deel te neem. Deelnemers van albei groepe is van hul anonimiteit en die vertroulikheid van die proses verseker. Beide die semi-gestruktureerde onderhoude en die fokusgroepbesprekings is digitaal opgeneem en transkripsies is daarvan gemaak, waarna die transkripsies in Atlas.ti gelaai is vir tekstuele analise. Transkripsies is tematies geanaliseer volgens die persepsies van die verskeie deelnemers. Die hooftemas wat na vore gekom het, sluit in individuele hindernisse, armoedeverwante hindernisse, institusieverwante hindernisse asook sosiale en gemeenskapsverwante hindernisse. Resultate van dié studie is getrianguleer deur die verskille en ooreenkomste te vind tussen pasiënte, klinici en pasiënt-advokate oor die hoof- en subtemas. Die hooftemas is toe volgens Bronfenbrenner (1972) se Ekologiese Sistemeteorie verdeel in die verskillende sisteme teenwoording in die pasiënt se omgewing, naamlik die mikro-, ekso-, en makrosisteem. Die mikrosisteem het individuele sielkundige en gedragshindernisse asook die armoedeverwante hindernisse ingesluit. Institusieverwante hindernisse is binne die eksosisteem van die pasiënt se ekologiese omgewing beskou en sosiale en gemeenskapsverwante hindernisse is beskou binne die makrosisteem van die pasiënt se ekologiese omgewing. Die belang van dié studie lê in die identifisering van volhoudingsgedrag as produk van die pasiënt se omgewing, soos beskou deur die Ekologiese Sistemeteorie. Toekomstige navorsing kan fokus op effektiewe maniere waarop pasiënte bygestaan kan word om die nodige vaardighede te ontwikkel om hul omgewing beter te kan hanteer en beskikbare sosiale ondersteuning te kan verbeter. Die ontwikkeling van strategieë om nuwe pasiënte by te staan, benodig ook verdere navorsing.

Abstract:
Increased enrollments of nontraditional students in U.S. higher education institutions have prompted many college and university administrators to consider student service programs. These programs ensure that support services are available to nontraditional students to cultivate healthy graduation rates among that student population. The purpose of this study was to discover factors that influence nontraditional students to become disengaged or be retained. The study was a qualitative case study with data collected from individual interviews with 10 nontraditional students participating in online and traditional onsite delivery systems at a private, nontraditional higher education institution in the western United States. The theoretical framework that guided this study was Knowles's andragogy theory. The research questions addressed students' perceptions of the role of persistence, barriers, self-direction, intrinsic motivation, delivery systems, learning modality, and academic and support services in their academic success. Data analysis was conducted to identify themes by coding the narrative responses and using member checks to validate data interpretations. Findings derived from the interviews indicated that students believed that they benefitted from a community of support within the university system. A professional development workshop was designed as a project to train university employees on how to create a professional learning community (PLC) to support students. This PLC was designed to promote positive social change by enhancing retention of adult students in academic programs until graduation and create an environment where people work together in a collaborative way within the university to ensure student success.

Abstract:
This research is in two phases. The aim of phase one was to explore the school experiences of learners identified as having literacy barriers to learning. Learners were in Year 8 (age 12-13) at the start of the research and Year 9 (age 13-14) by its conclusion. A case study design and the participatory method of ‘photovoice’ was used to elicit and foreground students’ views and experiences. To date there has been little research in this field using participatory methods. Thematic analysis was used to analyse the data. Findings suggest that the student photographers’ (N=4) were keen to share both their positive and negative experiences of school and in particular the importance they placed on relationships with peers and key staff. The aim of phase two was to use students’ photos, which were developed into individual ‘experience boards’, as a tool to facilitate staff reflections on practice. Three discussion groups (N=7) were held and data were analysed using thematic analysis. Findings were viewed and discussed using the framework of Maslow’s (1943) hierarchy of needs. Staff showed a high level of empathy and a desire to interpret and reflect on the photos. The constructs of feeling safe and secure were significant themes across all discussion groups. Furthermore, the lack of photos explicitly about learning challenged staff beliefs about student experiences and enabled them to acknowledge the importance the student photographers placed on these constructs. The use of photovoice stimulated reflection and is likely to have aided staff to better understand student experiences. There are implications for educational psychology practice in relation to taking a more holistic view of understanding and supporting learners with literacy difficulties and also the use of ‘photovoice’ to elicit and foreground student views and experiences as part of consultation or at a more systemic level.

Abstract:
As corporations have attained an increasing focus in the sustainability debate, so have the expectations of their sustainability performance. Rather than having a sole focus on an ad hoc mix of philanthropic actions, researchers now begin to call for businesses to recognize their role in broader societal sustainability challenges. This paper aims to explore the perceived drivers and barriers of corporate sustainability (CS) when a company seeks to take a societal perspective on its contributions to sustainable development. A case study has been conducted on a management consultancy firm to address this, with empirical data stemming from semi-structured interviews and a questionnaire. A systematic literature review was furthermore carried out to study the drivers and barriers identified in prior research, thus obtaining a methodological triangulation. The data was moreover analyzed with theoretical underpinnings from the field of transition management (TM). In doing so, it was possible to begin closing the gap between previous research on the drivers and barriers of CS and those that emerge when moving beyond traditional CS efforts. More specifically, a business transition management (BTM) framework was discussed in relation to the results from the case company, trying to assess whether the elements included align with the identified drivers; and to discuss whether the strategies within could combat the perceived barriers. When analyzing the empirical data and comparing it with the findings from the literature review, many similarities could be found. Client expectations, leadership, organizational culture, and communication were all essential aspects within the case company, and so were they in the literature review, thus, validating much of the findings in previous literature. A key difference, however, appears to be the complexities associated with these when taking a wider, societal perspective on sustainability. Relating this perspective to the activities carried out by employees is perceived to be difficult, and the business case is not fully recognized. It was suggested that aspects from the BTM framework could be utilized to combat the barriers associated with increasing complexities, such as the empowerment of visionary employees with a deeper insight into the complexities in question. The findings of this research endeavor have been complied in a proposed model to widening the perspective of CS efforts. Albeit not definite in its construct, it nonetheless begins to reflect on the interlinkages between TM and previous research on CS drivers and barriers. Additional drivers and barriers can be identified, and the differences between traditional CS initiatives and those that embrace a societal perspective should be further clarified. Still, this model provides a good starting point for further research on this phenomenon.
Allteftersom företag har fått en större roll i hållbarhetsdebatten så har också förväntningarna på deras hållbarhetsarbete ökat. I stället för att enbart fokusera på en ad hoc-blandning av filantropiska åtgärder– såsom slumpmässigt utvalda donationer, marknadsföring och svag hållbarhetsrapportering – börjar forskare nu uppmana företag att erkänna sin roll i bredare samhällsutmaningar för hållbarhet. Denna uppsats syftar till att utforska de upplevda drivkrafterna och barriärerna när ett företag försöker ta ett samhällsperspektiv på deras bidrag till hållbar utveckling. En fallstudie har genomförts på ett konsultföretag för att undersöka detta, med empiriska data från intervjuer och ett frågeformulär. En systematisk litteraturgranskning genomfördes dessutom för att studera de drivkrafter och barriärer som identifierats i tidigare forskning vilket medförde att metodologisk triangulering kunde uppnås. Vidare så analyserades empirin med underlag från Transition Management (TM). På så sätt kunde klyftan mellan dessa forskningsområden börja överbryggas. Mer specifikt så diskuterades ett ramverk för Business Transition Management (BTM) i förhållande till resultaten från fallföretaget, för att därigenom bedöma om detta överensstämmer med de identifierade drivkrafterna och för att diskutera om strategierna inom det ramverket skulle kunna bekämpa de upplevda hindren. Vid analysen av empirin så uppvisas många likheter med de drivande faktorerna och barriärerna som kan identifieras i litteraturöversikten. Kundernas förväntningar, ledarskap, organisationskultur och kommunikation visade sig vara viktiga aspekter inom fallföretaget, vilket de också var i litteraturgranskningen. Således valideras mycket av resultaten i tidigare litteratur. En skillnad verkar emellertid vara komplexiteten i samband med dessa när man tar ett bredare samhällsperspektiv på hållbarhet. Att relatera detta perspektiv till de anställdas projekt upplevs som svårt, och affärsmöjligheten med detta är ännu inte helt erkänt. Aspekter från BTM-ramverket antyds kunna användas för att bekämpa de hinder som är förknippade med den ökande komplexiteten, såsom att framhäva visionärer inom företaget. Slutsatserna av denna forskningsinsats har sammanställts i en föreslagen modell, där övergången till ett bredare hållbarhetsperspektiv presenteras. Trots att modellen i sig inte är vetenskapligt beprövad ännu, så bidrar den ändock med en reflektion kring sammanlänkningarna mellan TM och tidigare forskning om hållbarhetsarbetens drivande faktorer och barriärer. Ytterligare drivkrafter och barriärer kan identifieras, och skillnaderna mellan traditionella hållbarhetsinitiativ och de som omfattar ett samhällsperspektiv bör klargöras ytterligare. Trots detta så ger denna modell en bra utgångspunkt för ytterligare forskning om det undersökta fenomenet.

This research is concerned with understanding how organizational performance can be improved through effective knowledge transfer. In particular, the researcher is interested in organizational performance as it relates to those organizations that rely on core horizontal business processes, such as those found within a supply chain. Many organizations develop knowledge strategies to try and utilise existing knowledge within their organizations. However, this process tends to be developed from a top-down perspective, and deployed on an organization-wide basis. It is the researcher’s contention that this is not a suitable approach for developing an effective knowledge strategy for supply chain / complex organizations. It is the researcher’s belief that in order to develop and define a suitable method for knowledge strategy development and implementation, how knowledge and information are created and shared along core business processes must first be understood. To do this it is important to identify the barriers that impact knowledge transfer across an organization, and more specifically, along core business processes. By mapping a core IBM supply chain process, and identifying the employee work groups associated with the process (through social network analysis), the researcher was able to identify and assess knowledge transfer barriers, and how they impact along the IBM order flow process. Through a combination of qualitative and quantitative methods the researcher developed an emergent theory concerning how organizations should approach the identification and management of knowledge transfer barriers.

This thesis examined the question “What barriers are preventing Swedish companies from performing a structured conversion rate optimization process?”. As the purpose is to obtain an understanding of what is preventing companies from successfully execute conversion rate optimization (CRO). Given that CRO is an important part in most digital marketing activities. And despite increase in budget and importance in marketing, resource constraint continues to be the biggest obstacle.  The method employed to investigate this question was qualitative interviews with participants who worked with websites in seven different companies. An analysis was carried out, estimating the participating companies’ level of knowledge, overall structure, what to prioritize and current obstacles. It was established that the interviewees had several different areas of concern with regards to conversion rate optimization. Limited time, budget, priorities, knowledge, ownership, structured approach and interpreting data, were all treated in the analysis. A discussion was carried out to argument the definition of “biggest” barrier, as some barriers were more common than others but easier to overcome. Overall, these obstacles could all be traced back to barriers as prioritization, structure and ownership. The conclusion was that companies must have a more structured working process within the area of conversion rate optimization in order for this practice to be prioritized as a substantial part of companies online marketing activities.
Denna uppsats har undersökt frågan "Vilka hinder hindrar svenska företag från att genomföra en strukturerad konverteringsoptimerings process?". Med syfte att få en förståelse för vad som hindrar företagen att utföra konverteringsoptimering (CRO). Då CRO är en viktig del i de flesta digitala marknadsaktiviteter. Och trots ökad budget och kunskap inom marknadsföring är resursbegränsningen fortfarande det största hindret. Metoden som användes för att undersöka denna fråga var kvalitativa intervjuer med deltagare som arbetade med webbplatser inom sju olika företag. En analys genomfördes, som tog upp företags kunskapsnivå, övergripande struktur, prioriteringar och nuvarande hinder. Det fastställdes att intervjuarna hade flera olika problemområden med avseende om konverteringsoptimering. Begränsad tid, budget, prioriteringar, kunskap, ägandeskap, strukturerat abetsflöde och tolkning av data, behandlades alla i analysen. En diskussion gjordes för att argumentera för definitionen av "största" barriären, eftersom vissa hinder var vanligare än andra men lättare att övervinna. Sammantaget kan dessa hinder alltså spåras tillbaka till hinder som prioritering, struktur och ägande. Slutsatsen var att företagen måste ha en mer strukturerad arbetsprocess inom området för konverteringsoptimering för att denna disciplin ska prioriteras som en väsentlig del av företagets marknadsföringsaktiviteter online.

Adopting more sustainable consumption habits has been identified as a necessary step in the progression towards a sustainable society. In the area of sustainable consumption, personal food behaviour represents a strong leverage point. University students have been identified as a strategic audience; habits established during this transformative period can track forward into later life. This study seeks to identify the barriers inhibiting students from eating more sustainably. Perceived benefits of eating more sustainably, student food preferences, and student definitions of sustainable food are also identified. Focus groups, surveys, and interviews were carried out at universities in Europe, North America and Australia. Results show that perceived cost of sustainable food and a lack of knowledge, time and availability were ubiquitous barriers preventing students from adopting more sustainable eating habits. In addition to gathering the perceptions of others, the authors’ understanding of the challenges and benefits of eating more sustainably was augmented by a month-long self-study. Results from the self-study show that it is feasible to eat more sustainably without incurring additional costs. Recommendations informed by the focus groups, surveys, interviews and self-study are made to help students overcome the barriers to eating more sustainably.

Abstract:
Breast cancer is the most commonly diagnosed cancer in African-American (AA) women and is the second leading cause of cancer-related deaths in the United States among AA women ages 40 to 55 years of age. The 5-year breast cancer survival rates for AA women (78%) are lower than are those of Caucasian women (90%). The purpose of this project was to describe the benefits and barriers toward screening mammograms for breast cancer in AA women living in the Southside Health District in Virginia. The subscales of The Champion's Mammography Beliefs and Attitudes Questionnaire (MBAQ, 1999), which is based on the health belief model variables of perceived benefits and barriers, guided this project. A convenience sampling of AA women (n = 112) from 10 different churches in Brunswick and Mecklenburg County, Virginia completed Champion's MBAQ after participating in an educational program based on the Susan G. Komen's breast self-awareness messages. Descriptive analyzes revealed that 54% of the participants either agreed or strongly agreed with the perceived benefits to getting a screening mammogram, whereas 7% either agreed or strongly agreed with the perceived barriers to getting a screening mammogram. The results of this project are consistent with the literature and support the idea that it is imperative to educate AA women about screening mammograms in places where they socialize. This project contributed to social change in nursing practice by enhancing the awareness among AA women that early screening mammograms save lives.

Abstract:

The purpose of this study was to delve into the literacy instructional strategies of selected high-performing K-2 teachers in a Clark County, Nevada school district. The study assessed the efficacy of teachers using five core literacy components: phonemic awareness, phonics, vocabulary, fluency, and comprehension for student achievement. High performing teachers were defined as consistently demonstrating student performance gains of 25% in at least one of the five core literacy components over a 3-year period. The research question for this study was: What instructional strategies are used by selected high-performing K-2 teachers who work with diverse populations that have demonstrated a minimum gain of 25% in reading as measured by AIMSweb to develop (a) phonemic awareness, (b) phonics, (c) fluency, (d) vocabulary, and (e) comprehension?

The study used a qualitative method of data collection and analysis through in-depth teacher interviews, classroom observations, and district-wide data analysis. Interviews were open-ended and observations involved six teachers during their classroom literacy instructional time. Checklists were used to facilitate data collection during observations. Classroom pictures, teachers’ lesson plans, and AIMSweb data were used to support results.

The findings revealed that the most successful literacy strategies for teaching the five components of literacy were: phonemic awareness (word manipulation, word play, and word sort), phonics (word study, sound/spelling, and decoding/encoding), fluency (choral reading, repeated timed reading, partner/student-adult reading, and reading connected text with corrective feedback), vocabulary (explicit instruction on word meaning/independent word learning, direct instruction on new vocabulary, and context clues), and comprehension (predicting, inferring, making connections, using graphic organizers, and activating/building on prior knowledge). In addition, study conclusions revealed that the most successful strategies for teaching K-2 literacy included teacher collaboration and planning; strategic use of individual, small, and whole grouping; and integrating technology.

Several recommendations emerged from the study. Teachers need to be given ongoing professional development and resources for teaching the five component specific literacy strategies. Time needs to consistently be allotted for teachers to collaborate and plan for literacy instruction. Exploring diverse student populations, and their literacy acquisition needs, is vital to the education of youth. Placing emphasis on differentiated instruction, study replication, and analyzing literacy strategies and acquisition practices using varied methodologies will improve educational outcomes.

The process to successfully adapt to climate change for farmer communities’ in Vietnam faces a multitude of challenges.  This thesis proposes that institutional barriers to implementing policy related to climate change adaptation occur in key sectors related to farmer’s livelihoods at the local level, which are important to facilitating the adaptation process for farmer communities. It analyses the current state of the climate change policy landscape, investigates water, agricultural and disaster risk management sectors in Vietnam in the literature, and examines more closely how governmental institutions at the local level facilitate these in a field study of Bo De commune. The institutional barriers to successful adaptation to climate change reported at the local level in the field study are similar to the types of barriers identified across the literature, related to communication, technology, leadership facilitated by institutions. The results confirm that institutional barriers are occurring at the local level that are impeding the implementation process of key policies; this limits the ability of the farmer community to adapt successfully to climate change.

This qualitative study investigated the perceptions of exercise among individuals with fibromyalgia. The goal of this research was not to prove that exercise is good for people with fibromyalgia, but rather to gain an understanding of how health practitioners can help fibromyalgia patients adhere to regular physical activity. The research was conducted using three focus groups with people who are treated for fibromyalgia at Medical Consultants in Muncie, Indiana. The groups consisted of current exercisers and non-exercisers. Four themes emerged from the groups that the participants discussed. These themes were, reasons for exercising, barriers to exercise, desired characteristics of an exercise program, and key support personnel helping participants deal with their condition. The information contained within these themes can be used by health practitioners to develop exercise programs that entice fibromyalgia patients to adhere to a regular exercise program.

CLTs are made from renewable wood resources and have significant economic potential as a new value-added product for the United States. However, market penetration has been obstructed by product affordability and lack of availability for use. Previous studies and projects have surveyed opinions of designers and contractors about the adoption of CLTs. No previous study was found that surveyed cost estimators, who serve the essential function of creating economic comparisons of alternative materials in commercial construction. CLTs are not included in these current cost estimation tools and software packages which may be limiting the potential use of CLT in construction. The purpose of this study was to discover if cost estimation is being used to make structural decisions potentially affecting the marketability of CLT use in construction and building design because of the ability to estimate CLTs adequately. Through the use of a survey, the re-designing of a building, and discussions with subject matter experts, this study examined the knowledge level of cross-laminated timbers of under-surveyed building construction professions and the relationship between cost estimation and structural material choices. Their responses are demonstrating the need for better cost estimation tools for cross-laminated timbers such as inclusion in the Construction Specifications Institute's classification systems in order for CLTs to become a more competitive product. The study concluded that cost estimation is important for CLT market development, because it is being used extensively in the construction industry.

Cost estimation is performed early in the design process before the structural material has been chosen. However, making cost estimates of CLT materials early in the design process is not a practical solution at this point due to the lack of cost data available. As an alternative solution, this project developed a design tool that is meant to accelerate the design process and allow companies to approach suppliers for quotes, which require mostly complete designs. While this is not a complete solution, if designs are made faster and more effortless, they should also be a more affordable investment for clients. 5 | Page Building construction professionals perceived CLT construction as too expensive, unavailable to the consumer, or unwanted by the client. It was found that the lack of data, due primarily to the material being new to the US construction industry, was a significant barrier to CLT cost estimation. The custom design of many previous CLT projects, due to the lack of CLT construction in the current building codes, limits the collection of standard CLT construction data. There is also an issue with the discrete sizes of CLT panels limiting their competitiveness. These barriers were identified in this study, and further research is needed to develop complete solutions.

Cloud computing services have been supporting consumers’ enterprise demands for almost 20-years now and longer if conceptualized as a decoupled mainframe. Small companies and individuals with meager resources can gain and maintain the same class of service once only attainable by large companies that are well resources and funded. Additionally, the only impedance on a commercial company’s growth in the cloud are financial limitations or service limitations. This is not the case for the government as a consumer.

This praxis research effort focused on identifying the common barriers to cloud adoption, and the development of a data aggregating solution to better facilitate enterprise migrations to the cloud with the government as a consumer. The applied nature of this research enabled the integration of existing capabilities and standards into the proposed solution. Initial solutions development was based on the constructivist paradigm of qualitative methods, holding that knowledge is gained interactively. The follow on phase of research was quantitative in nature, following the positivism paradigm. The quantitative methods validated the initial solution and provided refinements.

Knowledge of cloud computing was gained through intrinsic subject matter expertise tempered with literature review and investigation. Statistical methods and tools were employed to provide engineering rigor to both validate and refine the proposed solution of the enterprise cloud management framework (ECMF).

The need for training may be a result of drives to improve quality, or might be needed as a response to human resource factors. In Libya the need for training is to develop the citizens as a whole in order to become a fully developed country. The main objective of this research was to identify barriers that may effect organisations and to highlight those that are particular to the Libyan context. This thesis identified and analysed the barriers, which can exist in Libyan public sector manufacturing organisations in particular using the Twisha Poultry Complex (TPC) as a case study. This research is original, and has not been previously undertaken. The project involved necessary in depth research to enable the development of a framework. The framework constructed, allows any organisation the facility in identifying barriers that exist and which could affect their employee training and development. This thesis concentrated on Health and Safety training as a lens. By investigating the existing Health and Safety training at TPC it was possible to gain an in depth understanding of barriers to training and development. Furthermore by using a theoretical framework, that lists areas and elements where barriers might occur, it was possible to highlight both the internal and external factors that produced barriers to training and development for the Twisha Poultry Complex. As a result of this study this researcher discovered a number of barriers to training and development that were particular to the Libyan context. It was discovered that there was a lack of skills, knowledge and the necessary attitudes to training and development. Also the selection processes in the country seem to be based on nepotism and there is a complete lack of any initial training for new recruits. Social and economic changes as well as the influence of tribe and family play an important part in people's attitudes to training. Whilst within the organisation investigated, there is a total absence of a training department and constraints are imposed, even on management training and development, this situation leads to the creation of barriers. The theoretical framework from this project can be used in further research in other organisations, especially in Libya, to investigate barriers to training.

The interaction between maintenance and quality, can lead to their integration with production. The second output of production is maintenance, whose output is increased production capacity. Both the production process and the quality of the maintenance work, which, in turn, affects equipment condition, affect the quality of final product. The thesis aims to identify the barriers and difficulties affecting quality in maintenance, within Libyan (public sector) manufacturing organisations. The specific objectives are derived to provide focus for the research activities, in order to fulfil the specific aim of the research in a structured and scientific manner. In order to achieve the thesis objectives, an empirically-based systems analysis of two case study organisations in the Libyan public manufacturing sector was carried out. Substantial field work was carried out using predominantly a qualitative approach. Qualitative data was collected by semi structured interview (from different levels of management and supervisors) to explore the quality management phenomena, and to provide a more holistic understanding through triangulation techniques of required and suitable data. This study has contributed to existing knowledge through getting an in-depth understanding of quality and maintenance issues. A specific definition of "quality in maintenance" was developed, common barriers of quality systems and the key factors of improving maintenance were summarised. Furthermore, by using the framework in figure 2.8 the enablers and inhibitors of quality in maintenance were demonstrated. This study is the first one that identified the unique barriers affecting quality in maintenance within the Libyan manufacturing organisations. These barriers are divided into three main kinds: the technical, economic barriers, managerial, organisational barriers, and cultural environmental barriers. The key findings of the research indicate that the case study organisations do not actively promote quality in the maintenance area that takes into consideration the influence of organisational, social, economic and political factors (change culture) on the quality of operations and performance inside the organisations.

This is a phenomenological study conceptualized in terms of social constructivism(Leatherman, 2007), Kurt Lewin‟s Field Theory(Neill, 2004) and ecological systems theory(Maddock, 2000).The literature review encompasses four focal points: conceptualisation of inclusive education, inclusive education in South Africa, policy implementation, and lessons learned from inclusive education implementation both internationally and in South Africa. A qualitative research method is employed in this study through the use of semistructured interviews and observations. The findings are analysed using content analysis. The key findings of this might help the teachers working in full service or mainstream schools to have a good understanding and a sense of what diversity and inclusive education entails. The findings might attempt to examine how educators manage inclusion in the classroom, their competencies and the strategies they need in order to be competent inclusive teachers. Recommendations are developed that point to how practices could be put in place to facilitate an effective implementation strategy for the development of an inclusive education system.

The purpose of the project was to identify barriers preventing men living in a rural setting, ages 19-60 from participating in routine preventative/primary care. Nationally and globally, men experience greater rates of morbidity and mortality due to chronic illness (Xu, Murphy, Kochanek, Bastian, & Arius, 2018). The literature suggests men do not routinely engage in primary care services (Centers for Disease Control and Prevention [CDC], 2015). The lack of participation leads to men experiencing poorer health outcomes (Banks & Baker, 2013; Baker & Shand, 2017; Pinkashov et al., 2013). Consistent and early access to primary care will help reduce the disparity and improve male health outcomes. The objectives of the practice improvement project (PIP) were: (1) identify actual and potential primary care uptake barriers of men aged 19-60 in a rural ND community; (2) discuss those barriers with rural ND healthcare clinic providers and clinical director and provide recommendations to improve men’s uptake in primary care services; and (3) measure effectiveness of providers implementing recommendations to reduce those barriers. Participants were recruited to voluntarily complete a 20-item questionnaire, free blood pressure screening, body fat analysis, and grip test. The survey results identified barriers in the areas of health literacy, confidentiality, empowerment, and self-efficacy. Through the utilization of a barrier screening survey, the coinvestigator was able to implement an educational presentation to the rural providers. The educational session provided interventional strategies for improving male utilization of primary care services. As result of the PIP, the rural healthcare organization was able to adopt two of the recommendations into practice. Additionally, the coinvestigator recommended incorporating body fat analysis and grip testing into the yearly examination. The screenings are relatively low-cost and non-invasive. Both screenings can offer the provider additional information regarding the overall health of their patients (Legrand et al., 2014; Park et al., 2019; Prasitsiriphon & Pothisiri, 2018). The additional health information can lead to more informed decision making and potentially aid in improved health outcomes for the patients.

This study explored the challenges minority students face in obtaining and understanding information when trying to apply for financial aid. One-to-one interviews, a focus group, and a review of documents were sources of data for the study. Themes emerging from the investigation were accessibility to financial aid information, dissemination of financial aid information, navigating the financial aid process, addressing institutional barriers to financial aid, and increase collaboration between high school and colleges/universities. Findings from the study included a need for earlier information to assist in navigating the financial aid process and a need for increased parental access to information. Recommendations for research include expanding the study to include quantitative research on the challenges facing minority students who apply for financial aid. Further research may be conducted on the community and non-profit organizations whose mission it is to help families navigate financial aid and college preparation.

In today's fast-paced, hyper-mediated society, the ability to balance accuracy and efficiency is essential. Media literacy educational programs have arisen to meet this need and proliferated in recent years. Although the practice of media literacy is thriving, its underlying mechanisms are poorly understood and evidence of effectiveness is mixed (e.g., Bergsma & Carney, 2008). A social psychological perspective has the potential to illuminate previously overlooked variables and inform research and practice in this growing field. In particular, whereas media literacy efforts typically emphasize thorough processing of media messages, dual-process theories of persuasion (e.g., Eagly & Chaiken, 1993; Petty & Cacioppo, 1986) suggest this is not always realistic. When motivation or ability is compromised, individuals default to a low-effort processing mode, relying on peripheral cues or heuristics rather than carefully evaluating message arguments. In this mode, media messages can persuade unconsciously. Using a dual-process approach, the present research investigated how specific barriers to motivation (perceptions of personal invulnerability) and processing ability (emotion-based advertising, environmental distractions) influence the processes of media literacy. Participants (N = 882) were randomly assigned to 16 conditions in a 2 [vulnerability: demonstrated, control] x 2 [distractions: present, absent] x 2(2) [ad type: informational, emotional; two replicates of each type] between-subjects nested design. The vulnerability manipulation increased distrust in the target ads and reduced their persuasiveness, not through the hypothesized mechanism of heightened perceptions of vulnerability, but mediated by increased counterarguing. Relative to informational ads, emotional ads were judged more persuasive, attractive, similar to personal experience, and elicited greater identification. However, they were also rated less trustworthy than informational ads, suggesting emotional advertising largely bypasses logical decision-making processes. Distraction reduced counterarguing only among those who found the ad relatively unpersuasive. The results of this research highlight the central role of trust in media literacy processes. Although individuals recognize and distrust emotional forms of advertising, they are nonetheless persuaded by such appeals. Given the sophistication of contemporary advertising techniques and the ubiquity of distractions, the present research suggests new approaches to media literacy are needed, specifically tailored to decision-making under these more realistic media-use conditions.

Abstract Currently, students with significant cognitive disabilities (SCD) are predominately exposed to a functional curriculum commonly delivered through behaviorists? methods (Keefe & Copeland, 2011). The most recent research has established that students with SCD who are presented with a high-quality comprehensive approach comparable to the best practices associated with general education practices can and do make positive gains in literacy skills (Bock, 2013; Browder, Ahlgrim-Delzell, Courtade, & Flowers, 2008; Erickson, Clendon, Abraham, Roy, & Van de Karr, 2005; Koppenhaver& Erickson, 2003,). The purpose of this phenomenological study was to examine potential barriers to implementing a comprehensive balanced literacy instructional program to students with SCD. The central question in this study was, what are the barriers to implementing a balanced literacy approach for students with significant cognitive disabilities? Three research questions guided the study, 1) What is the current knowledge base and understandings of administrators and teachers as it relates to teaching literacy skills to students with significant cognitive disabilities, how is this knowledge acquired, and how has it changed over time, 2) what are the perceived needs that need to be fulfilled in order for a systemic shift from a functional literacy curriculum to a balanced literacy curriculum approach to occur, and 3) how does the efficacy and beliefs held by teachers and administrators relate to the translation from research to practices as it corresponds to literacy instruction? Data was collected through interviews with administrators, special education teachers, and researchers. The analysis of the data from this study lead to the emergence of five key themes related to potential implementation barriers: acquisition of knowledge, current perspectives and understanding of literacy education, factors influencing curriculum decisions, high quality and relevant resources and supports, and systemic changes.

The aim of this research is to propose a sociotechnical framework to identify stakeholders involved in information systems and the benefit and barriers to the adoption of such systems. The proposed framework (BEBAF) would help to acknowledge the potential problematic areas for the implementation and adoption of information systems for each actor or social group and to be able to offer solutions based on the potential benefits for each of them. Precursors of BEBAF are social construction of technology (SCOT) and stakeholder identification process. Two case studies in the healthcare contexts, one in the UK and one in Spain, have been conducted in this dissertation using a qualitative approach to provide a rich picture of the influences on e-Health, and the users and organisational response to those influences. Both case studies aimed to support chronically ill patients at home or in nursing homes. BEBAF was then applied to both case studies. Among the most important implications are that those systems hold the promise for improving the quality of life of patients with chronic conditions, providing a better control over the disease. However, their impact on the organisational structures, the lack of funding and the difficulties of alignment of all the actors involved are relevant constraints to their adoption into the mainstream healthcare services. The main contributions of this thesis are: first, the definition and evaluation of a socio-technical framework to investigate IS adoption. Apart from the two closure mechanisms proposed by SCOT, BEBAF proposes a new closure mechanism by reinterpretation of benefits. In turn, the application of the framework has led to identify an extensive list of barriers and benefits for the adoption of e-Health systems with some suggested solutions. Another outcome is a comprehensive list of stakeholders involved in the adoption of such systems.

This dissertation includes two studies of rural age-related services in the state of Utah. The first study combines geospatial, demographic data (number of people over 55) and age-related services (hospitals, hospice providers, nursing homes, senior centers, and Areas Agencies on Aging) at the county level and census-tract level to determine localized differences in proportional access to age-related services. Higher and lower proportions are then predicted by contextual factors including rural/urban gradient, economic industry, and broadband access. Results demonstrate that broadband access was significantly associated with higher access to age-related services, but being a retirement destination (increase in people over 65 in county from 2000-2010) was associated with lower spatial access to age-related services. The second study involved in-depth interviews with stakeholders from communities across Utah, and qualitative analysis to identify specific barriers to age-related service access in their communities. This approach generated knowledge about challenges to accessing services and stakeholder-supported starting points and solutions for overcoming some of the identified barriers to age-related services in these communities. Taken together, the spatial data and responses from persons living and serving older adults in rural communities complement an understanding of facilitators and barriers to service access, paired with solutions.

Abstract:
Concern regarding sustainability and climate change is increasing, which is forcing countries world-wide to take action. The Swedish government has set a goal of fossil-free tra_c until 2030. Battery Electric Buses (BEB) might be one of the solutions needed in order to reach this goal. However, currently its prevalence is at an early stage.   The purpose of this study is to investigate how the technological transition towards BEBs in Sweden a_ects the public transport operators (PTOs). Moreover, to investigate how a third party service provider of Fleet Management System (FMS) services can support the PTOs in this transition.   The research has been carried out in co-operation with a PTO and a FMS service provider. The research contributes to their current understanding of how they will be a_ected by the emerging technological transition. This thesis also contributes with new empirical data of the technological transition towards electric vehicles within public bus transport, seen as a Large Technical System. Conceptually it contributes, by exploring how external companies can support the technological transition towards BEBs, with the application of Technological Transitions theory and the Multi Layer Perspective framework.   The methodology used is a case study of the technological transition towards BEBs in Sweden. Data was collected through twelve semi-structured interviews with researchers, PTOs, public transport authorities (PTA), a BEB manufacturer and a FMS-service company. Parallel to this a questionnaire was distributed to the twenty largest PTOs in Sweden. Moreover data was collected from company visits, pilot-project results and internal documentation.   Our findings show that there are thirteen perceived barriers present among the PTOs, in the process of BEB adoption. Six of these barriers relate to component aspects of BEBs, and seven relate to managerial aspects. Perceived barriers linked to component aspects of BEBs are; Variation in solutions and lack of technical standards, the Charging infrastructure, Shorter range or decreased load capacity, Unknown functionality in cold climate, Reliability and Durability. Perceived barriers linked to managerial aspects of BEBs are; Lack of knowledge and experience, Behavioral change, Economy, Maintenance, Ownership of infrastructure and buses, Business models and Varying requirements from PTAs. The barriers FMS-service providers can address are primarily, due to the technological nature of the services, present at niche level. PTOs together with FMS-service providers are encouraged to together strive towards gaining deeper knowledge about the new emerging echnologies. Through this, PTOs could be enabled to overcome the aforementioned barriers. Three reverse salients were also identi_ed, linked to the aforementioned barriers. If the everse salients are assessed, BEB acceptance among PTOs could be increased. The three identi_ed reverse salients are, the battery technology, the charging infrastructure and the contracts/ownership. The co-operation with the commissioning PTO and FMS-service provider has led to valuable access to Swedish public transport actors, and has aided in a deeper understanding of the phenomena. Although, this co-operation might have exposed us to a risk of being influenced.

The Student Growth Objectives are assessments created locally or by commercial educational organizations. The students’ scores from the Student Growth Objectives are included in teacher summative evaluation as one of the measures of teacher’s effectiveness. The Danielson Model for Teaching and Learning supports the idea that assessment literate teachers are highly effective. The purpose of this quantitative causal-comparative study was to identify New Jersey teachers’ competence in student educational assessments. Prizovskaya measured teachers’ assessment literacy level between different groups based on subject taught, years of experience, school assignment and educational degree attained. The data collection occurred via e-mail. Seven hundred ninety eight teachers received an Assessment Literacy Inventory survey developed by Mertler and Campbell. Eighty-two teachers fully completed the survey (N=82). The inferential analysis included an independent-sample t test, One-Way Analyses of Variances test, a post hoc, Tukey test and Welch and Brown-Forsythe tests. The results of this study indicated teachers’ overall score of 51% on entire instrument. The highest overall score of 61% was for Standard 1, Choosing Appropriate Assessment Methods (M = 0.61, SD = 0.23). The lowest overall score of 39% (M = 0.39 and SD = 0.24) was for Standard 2, Developing Appropriate Assessment Methods. The conclusion of this study was that New Jersey teachers demonstrated a low level of competence in student educational assessments. Frameworks seeking to explain teaching competency and development in higher education indicate that context and personal perspectives, as well as knowledge and action are crucial components in the understanding of how and why faculty teach as they do and how development may be encouraged and may be supported. This study sought to contribute to a deeper understanding of individual instructors' perceptions of context of higher education as it related to their teaching projects. This study investigated the daily pursuits and pre-occupations (teaching goals/projects) of an individual instructor, specifically, the instructional demands, departmental demands, the personal and professional pursuits of knowledge and the pursuits of pedagogical knowledge. More specifically, this study investigated perceived facilitators and barriers to the realization of individual teaching and other work projects.
Nineteen full-time faculty members in the Departments of Physiotherapy, Occupational therapy, Nursing, Social Work, Educational Psychology and Education at a large research and teaching university in Montreal, Quebec participated in this study. The instructors were asked to complete an adapted version of Little's (1983) Personal Project Analysis (P.P.A.) instrument, which is designed to elicit an instructor's current pre-occupations or projects in his or her current context. The instructors were asked to rate these projects (seven teaching projects and seven other work projects) using a Likert scale (0 to 10) on twenty-one empirically supported dimensions. These dimensions included the following: enjoyment, difficulty, control, initiative, stress, time pressure, outcome, self-identity, others' view, value congruency, challenge, commitment, competence, support, self-worth, fun, others' benefit, self-benefit, supportiveness of culture (departmental level), hindrance of culture (departmental level), and overall current satisfaction. Instructors were asked to assess their perceived conflicts between two of their teaching projects and two of their other work projects in addition to completing a demographic questionnaire.
The findings indicate that instructors identified five different types of daily pursuits that formed and defined their teaching context, as they perceived it. These five types of daily pursuits (projects) included: course planning and preparation projects; student investment, support and delegation of tasks to student projects; knowledge building and knowledge sharing projects; committees, faculty support and faculty teaching projects; and finally, teaching strategy projects. The instructors also identified five different types of daily pursuits that formed and defined their other work context. These included: publishing, conference presentation and research projects; grant proposals and funding projects; office organization projects; correspondence, university committees, outside mandates, departmental expectations and management of student and faculty projects; and finally, personal objectives and technical skill building projects.
P.P.A. enabled the researcher to identify on an individual instructor level the instructor's perceived facilitators and barriers to the successful completion of teaching and other work projects. Furthermore, P.P.A. as a faculty development instrument or as an alternative to semi-structured interview methods is supported by the findings.

My study is of the provision of career education, health education, personal development and student development programmes for students from Years 7 to 10 in ACT high schools. My purpose was to identify why these programmes are not available to all students and what can be done to make them available. The methods I used were an examination of a longitudinal case study of "Bellbird" High School in parallel with a survey of the current system perspective. Although the study focuses on the ACT in Australia, reference is made to the international literature, as well as local, regarding the attitudes of students, parents and teachers to career education, health education, personal development and student development programmes; data collection and interpretation; and the implementation of change. I have used data from three different survey instruments administered at "Bellbird" High School, at five year intervals, in 1978/79, 1984 and 1989. I have used another instrument at system level twice, in two consecutive years, in 1988 and 1989. Each of the instruments was developed for a specific purpose and not for gathering data for this study. All three surveys at "Bellbird" High School were designed to determine the attitudes of students, parents and teachers to aspects of the curriculum. They provided both quantitative and qualitative data. Basically, I have compared the 1979 and 1989 numerical data, and used the 1984 material for confirmation of significant issues. In addition to using qualitative data from these three instruments, I have also used comments from curriculum committee and School Board documents and evaluation reports from the Living Skills Programme. At system level, a questionnaire was designed to gather data about the provision of career education and health education in high schools in the ACT. This instrument, included questions about barriers to these programmes and strategies for overcoming the barriers. The findings were distributed to the schools. The procedure was based on the 'Research, Development and Diffusion' model. People associated with the successful implementation of the programmes under investigation were interviewed to find out what barriers they had faced, how they overcame them and what suggestions they had for overcoming other baniers to these programmes. Using the data CO-jointly with the literature and my own knowledge of cumculum implementation, I have proposed an action plan for "Bellbird" High School to extend the provision of its student development programme to all students. In conclusion, the suitability of the proposed action plan for use in other ACT high schools is appraised and general principles for the system are drawn out.

This study applied the stages of transformative learning to faculty perceptions and application of best practices to online learning. Research questions included: Can transformative learning theory constructs be used to identify potential barriers in faculty development and delivery of online instruction?; How does the stage of transformative learning of online faculty relate to their perceptions about online learning and their application of best practices?; Is there a correlation between stage of transformative learning and the amount of experience with online instruction a faculty member has? Principal component analysis and cluster analysis support a four-component solution. The four constructs equate to Mezirow's four stages of learning: transforming frames of reference through critical reflection of assumptions, validating contested beliefs through discourse, taking action on one's reflective insight, and critically assessing it. Multiple regression analyses were run to predict faculty perceptions on the identified components. Three of these were statistically significant based on years of experience teaching online, the number of professional development workshops taken on online teaching, or both. While the instrument appears to be a valid measurement of transformation of frame of reference, examination of previously contested beliefs, and critical assessment of action, further efforts will be needed before this is a fully validated instrument.

This dissertation examines vaccination rates for Human Papillomavirus (HPV) among college women 18-26 years of age who participated in the American College Health Association’s National College Assessment (ACHA-NCHA). Utilizing secondary data, this research sought to report HPV vaccination rates among a racially diverse population and to identify potential barriers to vaccination. The ACHA-NCHA survey provided a large sample size (N=68,193) in which to perform a binary logistic regression analysis. Demographic characteristics were analyzed as potential barriers to HPV vaccination. Additionally, lack of certain health behaviors were explored as potential barriers to HPV vaccination. In this study, White/non-Hispanic women had a higher HPV vaccination rate when compared to minority women. The binary regression analysis demonstrated that minority women were less likely to receive the HPV vaccine. Furthermore, it was determined that as the age of the respondents increased, the likelihood of receiving the vaccine decreased. Health behaviors that were predictive of receiving the HPV vaccine included receiving the Hepatitis B and Influenza vaccine, number of sexual partners and receiving sexually transmitted disease information. Women who received a gynecological exam were almost twice as likely to receive the vaccine, as were women who had parental health insurance coverage. One aim of The Affordable Care Act (2010) is to decrease disparities in health care. Drawing attention to potential barriers to HPV vaccination allows policy makers to make informed decisions regarding future activities to reduce disparities. Health promotion activities should be targeted to specific populations in an effort to increase HPV vaccination rates.

OBJECTIVES: To identify and describe practical incentives and barriers for community health center pharmacists in adopting a practice based research network (PBRN) that facilitates sustainable collaboration. METHODS: Directors of pharmacy at Community health centers listed as members of Arizona Association of Community Health Centers (AACHC), with on-site pharmacies, were contacted via telephone. During initial contact an IRB approved script was used to recruit the pharmacy director’s participation, at which time the subject’s disclaimer form was read and an appointment for a future phone interview was scheduled. Phone Interviews were conducted using a standardized questionnaire, and all results were manually recorded on a standardized data collection form. Data collected included, site specific information including the: educational background of the pharmacy director, and his or her perceived inducements and barriers to participating in a pharmacy based PBRN with the University of Arizona. RESULTS: Phone interviews were completed by 8 directors of pharmacy, 4 women (50%) and 4 men (50%). A total of 5 participants (62.5%) had a BS degree, 2 (25%) had PharmD degrees and 1 (12.5%) had both as BS and a PharmD degree. The mean length of time in current position was 5.56 yrs (SD= 4 yrs.). 75% of the participants indicated that they considered working with the University of Arizona (UofA) as an inducement, the same number of participants felt that their staff and practice as a whole would also consider it an inducement. Overall participants indicated that both their personal (75%) and staff‘s (87.5%) motivation to improve the pharmacy profession was considered an inducement, as well as their opportunity for professional growth (75%). All of the participants (100%) indicated they did not have adequate staffing to support research at this time and therefore felt it was a barrier to participation. When asked about resources as a whole, including staff, time and technology 87.5% of the participants felt this was a barrier. Other common barriers were; anticipated time requirements (75%), current schedule/time allowances (75%), staff’s outside commitments (75%). Out of the 8 participants only 2 (25%) are currently participating in PBRNS at this time, 3(37.5%) have research ideas that they are interested in working on, and 3(37.5%) indicated that they were not currently participating nor did they have any current interests. The major themes identified as inducements to participation were patient benefit, time/staffing involvement, and professional growth. CONCLUSIONS: The most common barriers to participating in a PBRN were: working with the UofA, motivation to improve the profession of pharmacy and the opportunity for professional growth. The most common inducements were staffing, current resources, anticipated time requirements, current schedules and outside commitments.

Objective: A significant number of children suffer from life-limiting illnesses, with many dying each year in the U.S. Services, beyond standard medical care, aimed at increasing overall quality of life for children with life-limiting illnesses, are greatly needed within and outside of our hospitals. Palliative care is conceptualized as treatment provided to relieve symptoms and improve quality of life. Despite the empirically supported benefits of providing pediatric palliative care, only a small percentage of pediatric patients actually receive these services. Thus, there is a great need for the development and provision of these services. The first aim of the current study was to compare results from Holtz Children’s Hospital (HCH) at the University of Miami with those from Lucile Packard Children's Hospital (LPCH) at Stanford University, where a survey assessing pediatric healthcare providers’ palliative care needs was originally developed. The second aim was the modification and further development of the measure, including an evaluation its psychometric properties. The third aim was to identify the educational and training needs of healthcare providers in providing pediatric palliative care. The fourth aim was to evaluate whether completing the survey improved awareness of a palliative care team. Method: Two hundred and twenty five healthcare providers at HCH completed the Survey on Pediatric Palliative Care for Healthcare Providers (SPPCHP). Participants were diverse with regards to ethnicity/race and profession. Results: Healthcare providers at both hospitals reported feeling inexperienced in similar aspects of delivering palliative care and desired further training and support. The SPPCHP demonstrated strong internal consistency and good construct validity, evidenced in factor analyses. Pediatric healthcare providers at HCH reported only feeling “somewhat” experienced in all aspects of pediatric palliative care and rated families’ inability to accept terminal diagnosis as the top barrier to providing this care; half of healthcare providers reported not receiving adequate support. Although referrals did not increase six months after study completion, awareness at the hospital was improved. Conclusions: Clinical implications include the need for systematic training and support in palliative care for a range of pediatric professionals, with an emphasis on addressing key barriers to care. Given the similarity of the findings at HCH and LPCH, these results appear generalizable to a variety of children’s hospitals.

The majority of US construction and demolition (C&D) wastes flows are currently landfilled, which represents a significant source of underutilized resources and environmental degradation. Growing public concerns have increased the demand for C&D waste product recovery firms to reduce the landfilling of C&D wastes. Despite C&D wastes being one of the US’s largest solid waste flows, there is a lack of collection and reporting on US construction sector wastes. This is disconcerting to waste product recovery firms who would benefit from C&D waste flow information to support their exploration of wants and uses of potential C&D wastes, development of competitive business strategies and operational planning. This thesis contended that the lack of information on the US construction sector’s C&D waste flows is a significant barrier to the optimal development of the C&D waste product recovery market. This thesis sought to address concerns over the collection and quality of information inherent in currently available C&D waste estimates for the US construction sector. The second chapter addressed the high levels of uncertainty and significant methodological shortfalls in existing national C&D waste estimates. A novel framework was developed to improve collection methods and the resolution of the data collected on C&D waste flows and composition. The framework included; measurement of data at the construction site, use of standardized reporting procedures, differentiation of renovation construction and renovation demolition wastes, accounting of construction materials stocked in buildings, characterization of materials at a product level and implementation of regulatory mechanisms. In the third chapter, a national stocks and C&D waste flows model was developed to determine structural wood product use, including softwood lumber, softwood plywood, OSB, glulam, I-joists and LVL, in US single family residences between 1950 and 2008. The results from this model demonstrated a product level of resolution in stocks and C&D waste flow estimates which revealed the variability of waste composition over time. The methodology used to develop the stock and flows model may also be applied to estimate other C&D waste products, provided sufficient data were available.

Coronary Heart Disease (CHD) is the single largest cause of death in the U.S., and is also the leading cause of death in Montana. In addition, the estimated direct and indirect costs of cardiovascular disease in the U.S. for 2007 were $431.8 billion. Anticipated barriers to health promotion behavior have been shown to affect intentions to engage in and execute the behaviors. Exploring individual\'s perception of the benefits of health promotion behaviors is equally important when addressing barriers to making lifestyle changes. CHD is strongly associated with modifiable risk factors including physical inactivity, poor diet, and tobacco use. Although CHD risk factors have been studied extensively, few studies have assessed individuals\' combined perceptions of barriers and benefits as reasons for non-adherence to recommended health promotion behaviors related to CHD risk and most of these studies included only women. The purpose of this research study was to examine perceived barriers and benefits to CHD risk modification in men and women living in a rural western state. Two survey tools were used to collect the data from a convenience sample of persons seen in the cardiac care areas at a local hospital in a rural western state. The findings of this study provided important information about the perceived barriers and benefits to CHD risk modification of persons previously diagnosed or at risk for CHD. Results from this study imply that in order to develop effective interventions, it is important to understand the individual and his or her unique characteristics including gender, socioeconomic status, and education level in relation to his or her perceived barriers and benefits to health promotion. Nurse practitioners and other primary care providers can incorporate this knowledge into future strategies to reduce or eliminate barriers, increase perceived benefits, and promote health promotion behaviors in individuals who are at risk for developing CHD. Advocating for health promotion behavior adoption and CHD risk modification offer a very important and practical tool for providers to help individuals address and lower risk factors as well as prevent CHD and treat individuals with established CHD.

Although clinical trials are essential for the development of cancer treatments, only approximately 3% of cancer patients in the U.S. participate in them. While 55% of these patients are enrolled in cancer clinical trials through community-based practices and around 80% of all cancer patients are seen at this type of practice, there is a lack of knowledge about the enrollment barriers at these sites. This study evaluates enrollment barriers at a community-based cancer clinic at the levels of the investigative site, healthcare provider, and patient. Barriers to enrollment and strategies to increase enrollment are evaluated through historical data analyses and results from a survey assessing the opinions of healthcare providers on enrollment and research practices.

Studies have reported that the most trusted health care relationship is that of the prenatal healthcare professionals (PHPs) and the prenatal patient. Yet many of the patient’s environmental health questions go unanswered for a variety of reasons. To better understand the situation, this research examines environmental health education practices of PHPs – obstetricians, prenatal nurses, family physicians, midwives and doulas - offered during the preconception and prenatal period. Specifically, this thesis discusses some of the PHP self-reported opportunities and barriers surrounding the provision of environmental health education (EHE). In person (n=17) and telephone (n=4) interviews were conducted with PHPs in the Ottawa Region. Additionally, a key informant within the Society of Obstetricians and Gynaecologists of Canada (SOGC) was asked specific questions about the association’s role in the development of standardized educational care guidelines. The results show that most PHPs have a limited knowledge of EHE and are reluctant to discuss it without access to more professional research. PHPs feel that there is little professional association support and that guidelines for adding EHE to their current prenatal care plans are non-existent. This study is one of the first studies that uniquely examined EHE in the prenatal period from perspective of prenatal healthcare professionals.

The aim of this essay has been to try out Soft Systems Methodology in development of a multidisciplinary framework to launch energy sustainability in Iranian industry sector. A petrochemical industry, one small sized company from the automobile industry and an energy service company are studied due to their diversity in conditions. The petrochemical industry is recognized to possibly have more gains in terms of improving energy consumption. Beyond SSM, the AHP model, MCIR framework, UML, and financial feasibility study methods are used in the development of CSEL framework. The AHP model applied in this study transforms qualitative data into quantitative decision making results. The MCIR framework classifies the barriers which have interrelated nature and identifies the gaps of energy sustainability in industry. NPV and SPB of financial analysis methods are applied in this framework and finally UML diagrams depict the information process in the latter subsystem of this framework.
O objectivo desta tese é definir uma metodologia multidisciplinar, usando Soft Systems Methodology, com vista a analisar o sector da Indústria no Irão do ponto de vista de lançamento de novos projectos de sustentabilidade energética. Para a metodologia proposta uma indústria petroquímica, uma pequena empresa do ramo automóvel e uma empresa que faz consultoria em energia são analisadas. A indústria petroquímica surge como a que potencialmente terá maiores ganhos do ponto de vista de poupança energética. Para além de SSM, o modelo AHP, a framework MCIR, diagramas UML e um estudo de viabilidade financeira são usados para o desenvolvimento da metodologia proposta. O modelo AHP usado neste estudo permite trabalhar dados qualitativos de uma forma quantitativa. A framework MCIR classifica as barreiras interrelacionadas e identifica as necessidades de sustentabilidade energética na indústria. Métodos de análise financeira são usados na metodologia proposta e, por último, diagramas UML mostram o processo de informação.

Abstract:
Black women mortality rates are perceived to be impacted by communication barriers, trust issues, and the lack of quality preventive health services. The purpose of this phenomenological study was to explore communication barriers and trust issues perceived by Black women when seeking preventive health services. HMB was used to identify public attitudes around receiving preventive health services and to construct each question based on perceived susceptibility and perceived severity of communication barriers and trust issues. An ecological model of the communication process was used as a framework to identify fundamental relationships between the Black female patients and health care providers. Data were collected using open-ended interview questions from Black women in public health and health care professions in southeast Texas (N=10). Results were coded and evaluated by thematic analysis. NVivo 10 software was used to store and manage data. Study findings showed 4 participants voiced their beliefs that their healthcare provider was somewhat apathetic when it came to addressing their health care needs, and 3 of the participants who visited a doctor's office within the last 12 months reportedly expressed having poor communication and trust issues with their health care provider. Emerged themes included lack of attentiveness from health care providers and lack of a comfortable atmosphere or bedside manner when receiving preventive health care services from their healthcare provider. This research has implications for social change if the health inequalities of Black women are identified and addressed, then Black women may have a reduction in health disparities when receiving preventive health services and an increase healthier outcomes.
