Abstract:
Se analizan tecno-tipológicamente las industrias líticas, mayoritariamente elaborada sobre cuarcitas, del yacimiento paleolítico al aire libre de Tarazona III (2.479 piezas líticas), formando parte de la Secuencia Paleolítica del Guadalquivir, que abarca el final del Pleistoceno Medio e inicios del Pleistoceno Superior (129 ka y <104 ka). Se identifican las cadenas operativas del yacimiento, desde la selección de materias primas hasta la fabricación de los útiles con percutor duro. Son cadenas dirigidas especialmente a la obtención de pequeñas lascas (<5 cm), lo que refuerza la selección del tamaño del canto rodado, con patrones de reducción de núcleos elementales, buena representación de los centrípetos y baja presencia del levallois. La serie de N3 es muy equilibrada y completa; y relativamente completas las de N1 y N2 lo que sugiere, o bien la preconfiguración de las cadenas operativa en otros lugares cercanos y el transporte de los productos de talla al yacimiento; o, en N1, el desplazamiento hidro-geomorfológico de las piezas de dimensiones más reducidas. La proporción útiles lasca/lasca es del 24% (N2) y 34,7% (N3), esto es, en los niveles posteriores a 104 ka BP. Esta proporción junto a la baja variabilidad de los tipos sobre lasca, la exigua o nula presencia de macroútiles, define la industria de Tarazona III, como uno de los yacimientos representativos del Paleolítico Medio Antiguo (PMA) del Valle del Guadalquivir.

Abstract:
Object Currently, reliable low-cost and noninvasive techniques to assess cerebral perfusion in the operating room are not available. The authors report on their first clinical experience with laser speckle contrast analysis (LASCA) as a complementary imaging tool for the noninvasive and dynamic assessment of cerebral blood flow (CBF) during neurovascular surgery. The purpose of this preliminary study was to address the general feasibility of LASCA in terms of handling and image quality and to provide an example of its clinical implications. Methods Laser speckle contrast analysis was performed in patients undergoing cerebral revascularization procedures for the treatment of hemodynamic compromise and complex aneurysms. The portable LASCA device was centered over the surgical field, and continuous 5-minute recordings of relative CBF were obtained. In the case of flow augmentation for hemodynamic compromise, CBF monitoring was performed before and after completion of the anastomosis. In the case of flow replacement for parent artery sacrifice, CBF monitoring was performed during consecutive 30-second test occlusions of the radial artery graft after proximal internal carotid artery sacrifice and the subsequent initiation of blood flow through the bypass. Results In all cases, the authors achieved good visualization of relative CBF in addition to flow imaging in both the bypass graft and the cortical vasculature. During a sudden CBF decrease after test occlusion of the radial artery graft and subsequent flow initiation through the bypass, LASCA allowed immediate visualization and measurement of relative CBF in excellent spatiotemporal resolution. Conclusions In this study LASCA offered noninvasive and rapid intraoperative assessment of relative CBF, which can be used for optimizing neurovascular procedures.

Abstract:
In this article, the method of analysis of GB-speckles (gene-based speckles) has been adapted to the problem of detecting the differences in a group of genes (usually 5–7), used in Multi Locus Sequence Typing (MLST). This method is based on s-LASCA imaging (spatial Laser Speckle Contrast Analysis) of virtual GB-speckle and on the technique of RGB coordinates for GB-speckles, processed by the s-LASCA method. A very high sensitivity and accuracy of the new method for detecting gene polymorphism as a great alternative to classical MLST has been demonstrated. The analysis of GB-speckles, obtained for the concatenated sequences of seven genes (gatA, gidA, enoA, fumC, hemN, hflX, oppA) of three different Chlamydia trachomatis strains (E/Bour, ST94; G/9301, ST95; G/11222, ST94) has been applied as the model. The high efficiency of usage of s-LASCA-imaging of GB-speckles has been shown. The data obtained represent a significant progress in digital biology as a whole and improvements in the bio-digitalization of bacterial DNA.

Abstract Objectives Diabetic neuropathic pain is associated with small fiber neuropathy. We aimed to assess the functionality of small fibers in patients with diabetes by using a practical method. Design Patients with impaired glucose tolerance (IGT), diabetic neuropathic pain (DNP), type II diabetes mellitus without neuropathic pain, and healthy control were included. Axon-reflex flare responses were induced by the intradermal application of capsaicin and histamine at the distal leg. The associated flare characteristics (flare areas and flare intensities) were recorded by using Laser Speckle Contrast Analysis (LASCA). The pain and itch responses were rated while performing LASCA. To verify the structural properties of the small fibers, proximal and distal skin biopsies were performed. Results DN4, MNSI, NRS, evoked-burning pain scores, and HbA1c levels were the highest in the DNP group. Compatible with length-dependent neuropathy, the distal skin PGP9.5-positive intraepidermal nerve fiber densities (IENFDs) were the lowest, whereas TRPV1-positive IENFDs were the highest in patients with DNP. The distal leg LASCA data showed hypo-functionality in both patients with IGT and DNP and association with disease severity. Conclusions There is an unmet need to practically assess the functionality of small fibers in patients with pain. In this study, a practical and objective method that does not need special expertise for the measurement of the functional properties of small fibers by using axon-flare responses is presented. The LASCA method could potentially facilitate a practical, quick (within 5 minutes), and very early diagnosis of small fiber hypo-functionality in both patients with IGT and DNP.

This indicates the considerable improvement in the present tools of bioinformatics. Methods: Colour s-LASCA imaging of virtual laser GB-speckles, a new method of high discrimination and typing of pathogenic viruses, has been developed. This method has been adapted to the detecting of natural mutations in nucleotide sequences, related to the spike glycoprotein (coding the gene «S») of SARS–CoV-2 gene as the molecular target. Results: The rate of the colouring images of virtual laser GB-speckles generated by s-LASCA can be described by the specific value of R. If the nucleotide sequences compared utilizing this approach the relevant images are completely identical, then the three components of the resulting colour image will be identical, and therefore the value of R will be equal to zero. However, if there are at least minimal differences in the matched nucleotide sequences, then the value of R will be positive. Conclusion: The high effectiveness of an application of the colour images of GB-speckles that were generated by s-LASCA- has been demonstrated for discrimination between different variants of the SARS–CoV-2 spike glycoprotein gene.

Abstract:
Background:Vasculopathy is a hallmark of systemic sclerosis (SSc). Laser speckle contrast analysis (LASCA) is a research tool to assess peripheral blood perfusion (PBP) (1). At this moment, its reliability has been attested in SSc patients, but its predictive value for future ischemic digital trophic lesions (DTL) is unknown (1).Objectives:To investigate in an unselected, prospective SSc cohort if baseline LASCA PBP measurements can discriminate between patients who will develop ischemic DTL (iDTL) and those who will not.Methods:Patients (fulfilling 2013 ACR/EULAR criteria and/or 2001 LeRoy and Medsger criteria) were recruited during the period of December 2017 to September 2018. LASCA was performed at baseline, in standardized conditions (1). Regions of interest (ROIs) (diameter 1 cm) were outlined at the 2nd-5thfingertip both volar and dorsal. The ‘average PBP’ of these ROIs was calculated (expressed in arbitrary perfusion units [PU]). A monthly telephone survey was conducted for 1 year to investigate DTL occurrence. DTL were considered ‘ischemic’ if not related to calcinosis. Logistic regression and ROC analysis were used to assess if average PBP is predictive of future iDTL.Results:Of the 106 patients with complete follow-up (92 women [86,8%]; 18 limited SSc [17,0%], 82 limited cutaneous SSc [77,4%], 6 diffuse cutaneous SSc [5,7%]), 29 patients (27,4%) had a DTL history. Forty-nine patients (46,2%) were on vasodilator therapy. Only 7 patients developed at least 1 iDTL during follow-up (6,6%) (Figure 1a). Performing univariate logistic regression (ULR), average PBP was not predictive for future iDTL (Table 1). Of note, analyzing only the patients not taking vasodilators, average PBP in the ‘iDTL group’ (n = 3) was median 46,8 PU (min. 45,6 - max. 68,8) vs. median 141,4 PU (min. 24,4 - max. 269,5) in the ‘no iDTL group’ (n = 54) (Figure 1b). Abstract:
Laser speckle is an interference effect, which can be observed either in free space or on the image plane of a diffuse object illuminated by coherent laser light. The possibility of using the spatial statistics of time-integrated laser speckle pattern for monitoring capillary blood flow in quasi real-time has been demonstrated recently. However, there are still several important unresolved issues. First, the validity of the fundamental laser speckle statistical techniques used still requires verification. Second, the existing algorithms and software need to be re-examined and improved for achieving a real-time solution and new algorithms and software need to be introduced with a higher statistical accuracy and higher spatial resolution. Finally, a comprehensive theory and model(s) are needed to guide any further development. This thesis begins by studying the laser and laser speckle concepts and principles. Then the original laser speckle contrast analysis (LASCA) model and algorithms have been evaluated and re-examined. A real-time solution has been presented by using improved computational algorithms. The study of LASCA theory starts by exploring the biomedical structure of skin and tissue optics. A more comprehensive LASCA theory has then been established, with which three new LASCA computational models have been developed, based on temporal speckle statistics, spatial speckle statistics and temporal-spatial speckle statistics, respectively. To obtain a real-time solution, a class of highly efficient algorithms has been designed and developed, based on the three new computational models. To implement these algorithms, the LASCA foundation classes library and its application software have been developed accordingly, with the characteristics of portability, reusability, and modularity. To build up an optimised LASCA system, the LASCA related experimental parameters have also been analysed and discussed in detail and solutions have been proposed. As a result, the enhanced LASCA theory, LASCA system design techniques, LASCA related algorithms and software should become valuable tools in' guiding and assisting a more extensive study on the LASCA techniques.

De hecho, las ventajas en resistencia a la abrasión y al rayado le permiten ser usado en pisos de polideportivos, sin necesidad de encerar o sin que se quiebre por tráfico pesado. El estudio de mercado permite inferir que el suministro de los insumos necesarios para la producción estaría garantizado.

Abstract:
Le virus Lassa entraine une fièvre hémorragique endémique en Afrique de l’Ouest et représente un problème de santé publique. Les connaissances sur la pathogénèse et les réponses immunitaires associées à la maladie sont partielles. Nous avons suivi les paramètres pathologiques, virologiques et immunologiques associés aux infections létales et non létales du LASV chez le singe cynomolgus. Le tableau clinique a été caractérisé par une dépression, une anorexie, une perte de poids et une asthénie chez les animaux survivants, tandis que ces mêmes symptômes ont été accompagnés de fièvre, de difficultés respiratoires et d’épistaxis chez les animaux infectés par une dose létale. Seuls ces derniers ont montré une perturbation des paramètres de coagulation, une rhabdomyolyse et une hausse des marqueurs de lésions rénales. Nous avons observé un tropisme radicalement différent en fonction de la sévérité de la maladie, avec une dissémination virale dans les organes plus importante et plus rapide chez les animaux décédés, la présence de particules infectieuses plus nombreuses et des modifications anatomopathologiques plus sévères. Une réponse immunitaire innée et adaptative précoce et puissante a été associée avec le contrôle de l’infection et la survie tandis que les infections fatales ont été caractérisées par une réponse inflammatoire ressemblant au choc septique, une défaillance de la réponse immunitaire ainsi qu’une réplication virale incontrôlée. Cette étude permet d’améliorer nos connaissances de la pathogénèse de la fièvre de Lassa et d’apporter des marqueurs d’infection prédictifs de la maladie
Lassa virus causes a hemorrhagic fever endemic in West Africa and represents a threat for civilians. The pathogenesis and the immune responses associated with the disease are poorly understood. We followed pathological, virological and immunological parameters associated with fatal and non-fatal Lassa virus infection in the cynomolgus monkey. The clinical picture was characterized by depression, anorexia, weight loss and asthenia in survivors whereas the same symptoms were supported by fever, respiratory difficulties and epistaxis in animals infected with the lethal dose. Only fatalities have shown coagulation parameters dysfunction, rhabdomyolysis and an increase of renal function markers. We observed a different viral tropism in a function of the disease severity, with viral dissemination in organs that was more important and faster in fatalities, the appearance of numerous infectious particles number and more severe pathologic changes. Early and robust innate and adaptive immune response has been associated with the control of infection and recovery whereas fatal infections were characterized by a sepsis like inflammatory response, defective immune response as well as uncontrolled viral replication. This study sheds light on the pathogenesis of Lassa fever and reveals infection markers predictive of the disease outcome

This thesis is about the process and practice of diagnosis and the implications of new diagnostic technologies in low resource settings. As a setting and a disease which has seen significant investment in diagnostics, Lassa fever in Sierra Leone has been selected as a case study to examine these themes. In this thesis, 'new diagnostic technologies' refers to laboratory-based diagnostics which are fast, reliable, accurate and can be used in low income settings. The starting point of this thesis is a narrative surrounding such technologies which suggests that they will revolutionise low income healthcare settings by allowing accurate scientific diagnosis in places where it was not possible before. Various perspectives on diagnosis are examined and some limitations are identified in relation to their accounts of diagnostic process, context, practice and technology. To explore the case, aspects of science and technology studies, the sociology of scientific knowledge and medical anthropology are combined. A multi-sited ethnography of Lassa fever diagnosis was conducted in three settings: a rural village, a laboratory and the wards of a hospital. Documents were reviewed and interviews conducted with key actors and ex-Lassa fever patients. Analysis focused on framings (partial and subjective interpretations), narratives (persuasive storylines which make use of particular framings) and practice in relation to Lassa fever and the development of technology for its diagnosis. Assumptions about the disease, diagnostics and the process of diagnosis are identified and the conclusion considers how they compare with practice in each setting. This thesis argues that diagnosis is a complex negotiated process and that new diagnostics represent only one aspect of that process. Thus, they are not a ‘silver bullet' to transform low resource healthcare contexts. In particular, ‘improved' diagnostics do not always have the expected impacts, sometimes even introducing complexity and uncertainty. In challenging narratives about diagnostics, this thesis provides an alternative, practice-based, approach to thinking about diagnostics and innovations in health systems; this approach acknowledges the importance, and complexity, of the diverse contexts which shape innovations and technology use.

Pro gradu -tutkimukseni ”Kunnioittakaamme lasta”. Lastenkasvatus Kotiliesi-lehden valossa vuosina 1923–1963, tarkoituksena on ollut valottaa lasten kasvatukseen liittyviä teemoja yhden aikansa merkittävimmän naistenlehden näkökulmasta. Olen käyttänyt lähteenäni suomalaisen Kotiliesi-lehden vuosikertoja vuoden 1923 tammikuusta, vuoden 1963 kesäkuun loppuun. Aikarajaukseni perustana on Alli Wiherheimon neljäkymmentä vuotta kestänyt ura lehden päätoimittajana. Kotilieden tavoitteena oli tarjota neuvoja perheenäidin tehtäviin, kuten lastenkasvatukseen liittyen, ja kohottaa kodissa tehty työ arvoiselleen paikalle yhteiskunnassa. Olen käyttänyt tutkimuksessani kvalitatiivista metodia. Olen pyrkinyt ymmärtämään tutkimaani ilmiötä aineistolähtöisesti, eli niiden ajatusten kautta, joita Kotiliedessä kasvatuksesta esitettiin. Olen antanut tilaa myös Kotiliedessä kirjoittaneille, ja nostanut heidän ajatuksiin ja kokemuksiaan esille. Tärkein tutkimuskysymykseni on ollut se, miten käsitys lasten kasvattamisesta ja siinä käytetyistä keinoista on muuttunut. Olen tarkastellut sitä, millaisena persoonana lapsi nähtiin ja miten häneen suhtauduttiin, sekä miten lapsen fyysiseen ja psyykkiseen kehitykseen yritettiin kasvatuksella vaikuttaa. Olen myös verrannut Kotiliedessä esiintyneitä kasvatusihanteita yleiseen vallalla olleeseen kasvatusihanteeseen. Näitä kysymyksiä olen tarkastellut lapsen kolmen elinpiirin näkökulmasta, jotka ovat nousseet esiin Kotiliedessä. Tärkein lapsen elinympäristö oli koti, toinen koulu, joka edustaa tutkimuksessani myös yhteiskuntaa yleensä, ja kolmas lapsen vapaa-aika ja harrastukset. Tärkein uutuus tutkimuksessani on se, että olen sitonut lasten kasvattamisessa tapahtuneet muutokset historialliseen kontekstiinsa. Olen tuonut esille, että taloudellisilla, poliittisilla ja kulttuurisilla muutoksilla on ollut vaikutuksensa myös lasten elämään. Olen käsitellyt kokonaisvaltaisesti kaikkia lapsen elämän osa-alueita kasvatuksen näkökulmasta. Lehdestä saamani kasvatusta käsittelevän aineiston perusteella olen pystynyt hahmottamaan kolme toisistaan erottuvaa ajanjaksoa. Ensimmäinen näistä oli 1920- ja 1930-lukujen kehollisuutta ja etupäässä fyysisestä terveyttä ihannoiva ajanjakso. Ihannelapsi oli vanhempiaan kunnioittava, terve ja liikkuva lapsi. 1940-luvulla korostui työtä tekevän ihmisen ihanne. Vaikea ja köyhä aika vaati kodin töiden osaamista. Kasvatuksessa tuli keskittyä fyysisen hyvinvoinnin lisäksi myös lapsen henkisen hyvinvoinnin edistämiseen. Sodan seurauksena avioerot ja naisten työssäkäynti lisääntyivät. Kotiliedessä korostettiin 1950-luvulta lähtien läheisten perhesuhteiden merkitystä. Perhekasvatusta tuli antaa sekä kotona että koulussa, jotta lapsista kasvaisi hyviä tulevaisuuden äitejä ja isiä. Psykologia tarjosi apukeinon lapsen sielunelämän ymmärtämiseen ja oikeaan kasvattamiseen. Iloinen ja onnellinen lapsi oli osoitus onnistuneesta kasvatustyöstä.
