Abstract:
This chapter focuses on the typical pain complaints and their appropriateness for sympathetic blockade and neurolysis. Anatomic considerations, block technique, associated risks, and evidence of a successful block are covered for the stellate ganglion block, T2 sympathetic block, thoracic splanchnic block, celiac plexus block, superior hypogastric plexus block, and ganglion of impar block. Sympathetic blockade is commonly used for visceral pain syndromes. Visceral pain syndromes typically are not responsive to neuraxial blocks as well as conventional rehabilitative and pharmacologic treatments. Spinal sympathetic techniques involve careful prevertebral needle placement, typically using fluoroscopic guidance. The proximity of major vessels near the target injection area is the primary risk of these techniques. In general, sympathetic blocks are non-diagnostic, but they can still help determine whether a sympathetically mediated pain condition may be present and if sympatholysis may be an effective treatment option.

This chapter provides the rationale for measuring and valuing health benefits for economic evaluation. It covers the limitation of using conventional patient-reported outcome measures of health (e.g. SF-36) in economic evaluation. It then introduces the quality-adjusted life year (QALY), which attempts to value the benefits of health care in terms of a measure that combines the impact on survival and quality of life using the numeraire of a year in full health. This chapter introduces the core building blocks of measuring and valuing health: describing health; techniques for valuing health; deciding who should value health; and taking account of social consideration in the aggregation of QALYs.

Given the prominence of the varieties of capitalism debate and its imprint from the turn of the twenty-first century onwards, this chapter is dedicated to introducing, reviewing, critiquing, and expanding Peter Hall and David Soskice’s paradigm of ‘Varieties of Capitalism’. We explore the impact the turn towards ‘varieties’ has had, examine how the discussion has reshaped the subfield as a whole, and propose fresh ways pointing forward. Recent strides made suggest taking ideas and discourse more seriously. The cultural turn, however, deserves much more serious consideration than it has received so far. Disassembling the building blocks of the various models and exploring the glue that keeps them attached is another research avenue.

Chapter 5 is devoted to the impact of memories of past movements (with an emphasis on transition) on the movements that follow. After singling out main events, places, symbols, and persons that condense the memory of past movements in each country, it analyzes the mnemonic activities of social movements. The chapter looks at the ways in which collective memory is constructed and strategically used by the memory agents—in this case, the anti-austerity social movements of the European south. The memory building blocks invoked by the movements of each country are identified, and then the specific mechanisms, tactics, and broader strategies they employed are examined. Recent anti-austerity protests represent a main empirical focus for the analysis.

Chapter 5 follows the investment chain from the ‘buy side’ of investment management into the ‘sell side’ of brokers and traders. It discusses the development of ‘dark pools’—private share-trading venues in which subscribers can bid to buy shares or offer to sell them without those bids or offers being visible to the market at large. Originally, access to dark pools was restricted to investment management firms, and the pools were intended to permit those firms to buy or sell large blocks of shares among themselves at low cost and without the ‘market impact’ of trading in the public markets. The history of dark pools, however, shows how hard it has been to cling to that vision in the face of investment chain entanglements. The entanglement on which the chapter focuses most relates to the ways of payments (colloquially known in the US as ‘soft dollars’) for sell-side research.

Blockade of the ganglion impar (also known as ganglion of Walther or sacrococcygeal ganglion) is indicated for evaluating and managing visceral or sympathetic-maintained pain in the coccygeal and perineal area. Ganglion impar neurolysis has been reported in the palliative treatment for malignancies of the pelvis with cancer pain in the perineal area. Ultrasound can be successively used to locate the sacrococcygeal joint (SCJ) and facilitate the performance of ganglion impar block. However, ultrasound can also complement fluoroscopy, as lateral fluoroscopy is still needed to establish safe depth and monitor the spread of the injectate, especially with neurolytic injections.
