Abstract:
Purpose This study aims to explore the steps carried out in the implementation of Islamic Quality Management System (QMS) MS 1900 in Malacca zakat institution. Design/methodology/approach A case study strategy was used to explore the process of MS 1900 implementation at Malacca Zakat Center (MZC). Semi-structured interview with Top Management Officers, Managers, Shar’iah Officer, Quality Systems Officers and Auditors was used as main data collection method, and it was triangulated with data collected from documents and observations. The data were analysed by employing thematic analysis method. Findings The study found that there are 12 steps involved in MS 1900 implementation at MZC, namely, identifying gaps, understanding MS 1900, outlined quality policies and objectives, team work establishment, management representative establishment, management commitment, documentations, determining the quality scope system, implementing the MS 1900, internal audit, evaluating QMS performance and surveillance audit. All steps were carried out effectively with the cooperation of management and employees at MZC. Research limitations/implications The findings from this study can be employed by managers who intend to implement MS 1900 effectively in their organizations. Originality/value This study adds to the knowledge of the emergence of the first ever Islamic QMS MS 1900. It also contributes meticulous insights into the MS 1900 implementation particularly in the zakat centre.

Abstract:
Purpose This paper deals with the subject of “Lean Normalization.” The purpose of this paper is to propose a comprehensive methodology which combines the benefits of both International Organization for Standardization (ISO) standards and Lean Management. The application of this methodology leads manufacturing or service companies to certification without creating more documentation. It guarantees agility and flexibility in the day-to-day management of the company, and induces lower costs and reduced times. This methodology provides a new way of tackling the implementation of ISO standards in a company. Design/methodology/approach This methodology is based on the synergistic combination of both ISO standards (Quality Management System) and the key concepts of Lean Management. After a short presentation of ISO standards and Lean Management, the possibility of implementing a synergy between these two different approaches will be discussed. At this point, ISO 9001 standards will be focused on. The proposed methodology will be introduced step by step. Each step of the model will be formalized using the Structured Analysis and Design Technique graphic representation tool. Moreover, each step is associated with a key concept of Lean Management. In order to illustrate the proposed method, an example of implementation will also be presented. Findings To show the feasibility of this methodology, this paper finishes with a summary of the results achieved in a company and draws some interesting conclusions. Companies have the possibility of achieving certification in combination with a Lean Quality Management System (LQMS): a one-page quality manual, only ten records, no major non-conformities and no customer complaints over a two-year period. Research limitations/implications The methodology may have to be adapted to better suit the needs of companies seeking to implement the latest version of the ISO 9001 standards. Practical implications This paper presents the deployment of ISO 9001 standards whilst simultaneously respecting key Lean Management concepts. The practical results for enterprises are the implementation of an LQMS leading to a certification. Social implications This paper presents a new method combining both ISO standards and Lean Management concepts to achieve certification with an LQMS. Originality/value Due to its simplicity and the relevance of its steps, the proposed methodology can be applied to companies with a guaranteed success of certification.

Abstract:
This paper sets out to explore the degree of Quality Management System (QMS) implementation in current Malaysian construction industry. This research test these with a questionnaire based on nine major QMS aspects which are gathered from literature review. Data collected was analyzed using Mean and ANOVA. The findings indicate that QMS are implemented in moderate level with slight differences between mean values from various aspects. Besides, top management level respondents react more positive towards implementation of QMS in all aspects compared to middle management level and lower management respondents. Future research will benefit by using different QMS aspects for further improvement and replication to other industries.

It is ultimately focused on customer satisfaction which is a stringent factor for every business. Implementing and maintaining a QMS is a rather difficult, time consuming and expensive process which must be done with respect of many factors. The aim of this paper is to present a case study for implementing QMS ISO 9001 in a gamma irradiation treatment service provider. The research goals are the identification of key benefits, reasons, advantages, disadvantages, drawbacks etc for a successful QMS implementation and use. Finally, the expected results focus on creating a general framework for implementing an efficient QMS plan that can be easily adapted to other kind of services and markets.

There is a growing tendency in the application of the quality management system to the construction industry. Within this perspective, some quality management standards, like ISO 9001, are utilized to assure the quality in projects. Application of this system to water resources projects is also of importance since they are very large systems having various components for which quality management would improve the overall efficiency. The aim of this thesis is to examine the implementation of the quality management in the design and construction processes of a sample irrigation network in order to evaluate its benefits by the cost of quality, which is assumed to be the most effective performance measure. To this end, the prevention and appraisal costs and failure costs, which constitute the cost of quality,are identified separately and their interactions are investigated on a case study in which the causes of these failures are analyzed and quantified in the form of graphs.

Abstract:
In order to achieve accreditation for their services, pathology departments must meet international recognised standards and comply with national guidelines, issued by accrediting bodies such as Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency and the Human Tissue Authority in United Kingdom. Objectives: An evaluation of existing Quality Management System (QMS) and implementation of a quality standard and a centralised electronic document management software (Q-pulse) in a pathology department of a district hospital. Method: A service evaluation utilising an adapted version of the Context, Input, Process, and Product model, (with the addition of the implementation stage). This consisted of context evaluation (an evaluation of the department’s decentralised QMS), input evaluation (an assessment of the various types of electronic document management system (EDMS), implementation (the implementation of a centralised QMS and EDMS), progress evaluation (monitoring, documentation and assessment of the progress of the QMS) and finally impact evaluation (measuring the impact of the newly implemented system). Data collection included quality audits, non-participant observation and qualitative interviews. The implementation strategies involved technical support training and feedbacks. Results: The implementation of a continuous QMS and EDMS within the department, using the adapted CIPP model, increased the involvement of all staff in quality matters. It enabled the department to improve quality control processes, procedures and performance. Staff and quality teams now meet regularly and multidisciplinary workings groups have been put in place to continual improve the quality matters. The level of compliance and awareness to standards and guidelines has increased through education and training of staff in within the QMS, and there is an increase in top level management involvement in quality activities within the department. As a result of this evidence of compliance, the department’s Quality management system, was accorded, an unconditional accreditation, by the accrediting bodies. (Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency). Conclusion: The quality management system is functioning in all the departments. All staff involved in the evaluation processes, judged it to be useful for them and the department. Further research is required to establish the longevity of the implemented products and other modules within Q-Pulse.

A detailed framework is essential to facilitate quality system implementation. In this study, we have offered a cost effective do-it-yourself approach to quality management. We have proposed a quality system implementation framework for small-medium sized organizations to enable their transition from a no-quality system to an ISO 9001 quality management system. The proposed framework is validated using a case study of a small door manufacturing company. The findings reveal several setbacks experienced during quality system implementation and suggests means to overcome them using a proposed seven step framework. This study also advises an effective maintenance tool to facilitate continuous improvement in organizations after implementing a quality management system. The study results will be useful for quality practitioners, managers, consultants and engineers, especially in small companies and discloses several benefits that can be achieved by employing the proposed framework in any organization irrespective of its size and nature.

Abstract:

Active Pharmaceutical Ingredient (API) repackagers play a niche, yet critical role in the expansive pharmaceuticals industry. Many repackagers have sub-optimally implemented quality management systems (QMS) leading to sub-optimal performance and elevated risks. This study sets out to demonstrate a strategy to strengthen the case for quality for this industry sector. Dr. Deming's Plan-Do-Study-Act cycle serves as the study's framework, while Phil Crosby's Quality Is Free philosophy forms the theoretical basis. An archival measure of FDA Warning Letters concerning APIs has been performed to bring a considerable failure cost factor to the forefront. A statistical analysis has been performed on data available from the study's pilot firm to demonstrate relationships between quality-related and revenue performance indicators in order to present a bottom-line approach preferred by management. Results from the study indicate a linkage between QMS factors and business outcomes and call for improved understanding, increased support, and a push towards QMS maturation.

Esta dissertação propõe um processo de integração e implantação dos sistemas de gestão da qualidade, meio-ambiente e segurança e saúde ocupacional, os quais, atualmente, operam de forma isolada numa empresa. O negócio da empresa em questão é a produção de gases industriais e pretende que o processo de integração considere a cultura da organização e os esforços movidos nesta direção. A metodologia utilizada para a integração da gestão dos sistemas já é de uso e conhecimento das organizações e da academia e foi selecionada conforme equipe experiente e designada para este propósito, partindo do princípio que não se optaria pela reestruturação e sim pela adequação dos sistemas já existentes. São ainda comentados os pontos fracos identificados no programa de implantação dos sistemas isolados e que deveriam ser considerados quando da integração dos mesmos, assim como as necessidades que direcionam a integração e a expectativa com relação a responsividade, abrangência, resultados coletados, além da mudança cultural promovida. A estratégia é considerada diferencial na decisão da integração dos sistemas e a metodologia Seis Sigma a ferramenta que possibilita a mensuração de custos e a implantação da filosofia da melhoria contínua na busca em exceder as expectativas do cliente.
This dissertation proposes an integration process and the implementation of quality management systems, environmental and safety policies, and occupational health. These systems are presently operating separately in the company. The company´s core business is the production of industrial gases and its internal policies establish that the integration process should take into consideration the culture of the organization and also the efforts already taken to move in this direction. The methodology applied for the systems integration management is under knowledge domain and it is being used by organizations and by the academy. It was selected and adopted by a skilled team assigned for this purpose, and it was defined that, instead of restructuring, adjustments would be made to existing systems. There are also comments about weak points identified in the program of implementation of isolated systems that should be considered for the integration process, as well as the needs that drive the integration and expectations related to responsiveness, scope of work, collected results and also the outcome in terms of cultural change. The strategy is considered relevant when deciding for systems integration and for the methodology Six Sigma, which is the tool that allows costs measurement and the implementation of the philosophy of continuous improvement to exceed customer´s expectations.

Abstract:
This study explored the views and experiences of School Management Teams (SMTs) when implementing Integrated Quality Management System (IQMS) in three primary schools in the Nkangala Region of the Mpumalanga Province, South Africa. A qualitative research approach was adopted and the data were collected by means of semi-structured interviews and document retrieval in schools. The data were then analysed by the constant comparative method. All the participants in this study expressed the view that IQMS is a teacher appraisal system with a potential to develop educators in schools provided both evaluators and evaluatees are honest with the evaluation process and are able to identify their areas of development. However the experience of the principals and deputy principals with the implementation of IQMS in schools reveals that educators inflate their scores and do not identify areas where they need development. The Development Support Groups (DSGs) do not conduct authentic evaluation since they serve the purpose of helping educators to qualify for salary progression. Participants recommended external evaluation as a solution to subjective ratings since external evaluators have a potential to provide objective and credible evaluation because they are unfamiliar with educators and do not experience the pressure to maintain collegiality in schools. A key recommendation is that the Department of Education should conduct effective training for the stakeholders involved with the implementation of IQMS in schools.

Abstract:
AbstractThe challenges in setting up a document management system are both technical and organizational. Both need to be addressed to achieve success. Time is required to make a thorough inventory of system requirements, whether electronic or paper-based, and to organize the team that will maintain the system. It is crucial that key stakeholders are involved from the beginning and that they see the value of the quality system in improving clinical care. Routine use by all staff will prevent it from being seen as a burdensome additional imposition due to the need for JACIE accreditation. When fully implemented, the documents and policies become a readily accessible, clinically relevant, educational resource that improves patient care.

Abstract:
The Centre for Radiation Protection and Hygiene is the institution responsible for Radioactive Waste Management Service in Cuba. This Service comprises: centralized collection, transportation, treatment, conditioning, long term storage, and disposal of radioactive waste, as well as dismantling, decontamination and decommissioning of small nuclear facilities. Radioactive waste should be managed in such a way as to ensure adequate safety and protection of both human health and the environment. In order to fulfil this principle a Quality Management System has been implemented for the Radioactive Waste Management Service, based on the ISO 9000 Standards. The Quality System provides, as appropriate: • adequate assurance that the requirements specified by the Regulatory Authority relating to protection and safety are satisfied; • adequate assurance that the customer requirements are satisfied; • control of all kind and inventories of radioactive waste through the implementation of a comprehensive system for record keeping; • continuous improvement to guarantee the cost minimization related with the radioactive waste management; • and quality control mechanisms and procedures for reviewing and assessing the overall effectiveness of the system. In the year 2002, as a conclusion of a certification audit performed by the direction of the Centre for Radiation Protection and Hygiene, the Quality Management System implemented for the Radioactive Waste Management Service was successfully certified according to the requirements of the ISO 9001:2000 Standard.
