Abstract:
There is a growing tendency in the application of the quality management system to the construction industry. Within this perspective, some quality management standards, like ISO 9001, are utilized to assure the quality in projects. Application of this system to water resources projects is also of importance since they are very large systems having various components for which quality management would improve the overall efficiency. The aim of this thesis is to examine the implementation of the quality management in the design and construction processes of a sample irrigation network in order to evaluate its benefits by the cost of quality, which is assumed to be the most effective performance measure. To this end, the prevention and appraisal costs and failure costs, which constitute the cost of quality,are identified separately and their interactions are investigated on a case study in which the causes of these failures are analyzed and quantified in the form of graphs.

Abstract:
In order to achieve accreditation for their services, pathology departments must meet international recognised standards and comply with national guidelines, issued by accrediting bodies such as Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency and the Human Tissue Authority in United Kingdom. Objectives: An evaluation of existing Quality Management System (QMS) and implementation of a quality standard and a centralised electronic document management software (Q-pulse) in a pathology department of a district hospital. Method: A service evaluation utilising an adapted version of the Context, Input, Process, and Product model, (with the addition of the implementation stage). This consisted of context evaluation (an evaluation of the department’s decentralised QMS), input evaluation (an assessment of the various types of electronic document management system (EDMS), implementation (the implementation of a centralised QMS and EDMS), progress evaluation (monitoring, documentation and assessment of the progress of the QMS) and finally impact evaluation (measuring the impact of the newly implemented system). Data collection included quality audits, non-participant observation and qualitative interviews. The implementation strategies involved technical support training and feedbacks. Results: The implementation of a continuous QMS and EDMS within the department, using the adapted CIPP model, increased the involvement of all staff in quality matters. It enabled the department to improve quality control processes, procedures and performance. Staff and quality teams now meet regularly and multidisciplinary workings groups have been put in place to continual improve the quality matters. The level of compliance and awareness to standards and guidelines has increased through education and training of staff in within the QMS, and there is an increase in top level management involvement in quality activities within the department. As a result of this evidence of compliance, the department’s Quality management system, was accorded, an unconditional accreditation, by the accrediting bodies. (Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency). Conclusion: The quality management system is functioning in all the departments. All staff involved in the evaluation processes, judged it to be useful for them and the department. Further research is required to establish the longevity of the implemented products and other modules within Q-Pulse.

Abstract:
A detailed framework is essential to facilitate quality system implementation. In this study, we have offered a cost effective do-it-yourself approach to quality management. We have proposed a quality system implementation framework for small-medium sized organizations to enable their transition from a no-quality system to an ISO 9001 quality management system. The proposed framework is validated using a case study of a small door manufacturing company. The findings reveal several setbacks experienced during quality system implementation and suggests means to overcome them using a proposed seven step framework. This study also advises an effective maintenance tool to facilitate continuous improvement in organizations after implementing a quality management system. The study results will be useful for quality practitioners, managers, consultants and engineers, especially in small companies and discloses several benefits that can be achieved by employing the proposed framework in any organization irrespective of its size and nature.

Abstract:

Active Pharmaceutical Ingredient (API) repackagers play a niche, yet critical role in the expansive pharmaceuticals industry. Many repackagers have sub-optimally implemented quality management systems (QMS) leading to sub-optimal performance and elevated risks. This study sets out to demonstrate a strategy to strengthen the case for quality for this industry sector. Dr. Deming's Plan-Do-Study-Act cycle serves as the study's framework, while Phil Crosby's Quality Is Free philosophy forms the theoretical basis. An archival measure of FDA Warning Letters concerning APIs has been performed to bring a considerable failure cost factor to the forefront. A statistical analysis has been performed on data available from the study's pilot firm to demonstrate relationships between quality-related and revenue performance indicators in order to present a bottom-line approach preferred by management. Results from the study indicate a linkage between QMS factors and business outcomes and call for improved understanding, increased support, and a push towards QMS maturation.

Esta dissertação propõe um processo de integração e implantação dos sistemas de gestão da qualidade, meio-ambiente e segurança e saúde ocupacional, os quais, atualmente, operam de forma isolada numa empresa. O negócio da empresa em questão é a produção de gases industriais e pretende que o processo de integração considere a cultura da organização e os esforços movidos nesta direção. A metodologia utilizada para a integração da gestão dos sistemas já é de uso e conhecimento das organizações e da academia e foi selecionada conforme equipe experiente e designada para este propósito, partindo do princípio que não se optaria pela reestruturação e sim pela adequação dos sistemas já existentes. São ainda comentados os pontos fracos identificados no programa de implantação dos sistemas isolados e que deveriam ser considerados quando da integração dos mesmos, assim como as necessidades que direcionam a integração e a expectativa com relação a responsividade, abrangência, resultados coletados, além da mudança cultural promovida. A estratégia é considerada diferencial na decisão da integração dos sistemas e a metodologia Seis Sigma a ferramenta que possibilita a mensuração de custos e a implantação da filosofia da melhoria contínua na busca em exceder as expectativas do cliente.
This dissertation proposes an integration process and the implementation of quality management systems, environmental and safety policies, and occupational health. These systems are presently operating separately in the company. The company´s core business is the production of industrial gases and its internal policies establish that the integration process should take into consideration the culture of the organization and also the efforts already taken to move in this direction. The methodology applied for the systems integration management is under knowledge domain and it is being used by organizations and by the academy. It was selected and adopted by a skilled team assigned for this purpose, and it was defined that, instead of restructuring, adjustments would be made to existing systems. There are also comments about weak points identified in the program of implementation of isolated systems that should be considered for the integration process, as well as the needs that drive the integration and expectations related to responsiveness, scope of work, collected results and also the outcome in terms of cultural change. The strategy is considered relevant when deciding for systems integration and for the methodology Six Sigma, which is the tool that allows costs measurement and the implementation of the philosophy of continuous improvement to exceed customer´s expectations.

Abstract:
This study explored the views and experiences of School Management Teams (SMTs) when implementing Integrated Quality Management System (IQMS) in three primary schools in the Nkangala Region of the Mpumalanga Province, South Africa. A qualitative research approach was adopted and the data were collected by means of semi-structured interviews and document retrieval in schools. The data were then analysed by the constant comparative method. All the participants in this study expressed the view that IQMS is a teacher appraisal system with a potential to develop educators in schools provided both evaluators and evaluatees are honest with the evaluation process and are able to identify their areas of development. However the experience of the principals and deputy principals with the implementation of IQMS in schools reveals that educators inflate their scores and do not identify areas where they need development. The Development Support Groups (DSGs) do not conduct authentic evaluation since they serve the purpose of helping educators to qualify for salary progression. Participants recommended external evaluation as a solution to subjective ratings since external evaluators have a potential to provide objective and credible evaluation because they are unfamiliar with educators and do not experience the pressure to maintain collegiality in schools. A key recommendation is that the Department of Education should conduct effective training for the stakeholders involved with the implementation of IQMS in schools.

The diffusion of a Quality Management System (QMS) to improve business performance depends upon the quality management strategy used. The aim of Eskom’s QMS strategy was to create a sustainable quality drive throughout the company to improve customer and stakeholder satisfaction. This research project’s objective is to determine how the diffusion of a QMS can improve the electricity supply industry in the Western Cape division of the electricity sector in South Africa. The key research objectives within the research study are:  to assess what effect a QMS has on a sustainable long-term solution for energy supply;  to assess how a QMS contributes to the establishment of world class processes and practices;  to determine how a QMS affects quality culture;  to determine what factors of a QMS play a role in a continual improvement process, and  to determine what barriers affect a sustainable QMS implementation. The research question is: “how can the diffusion of a QMS bring improvement to the electricity supply industry?” A quantitative research methodology was employed in the study. The responses to a survey questionnaire were analysed and conclusions were drawn. The research finding is that diffusion of a QMS improves the electricity industry by positively affecting attitudes towards the challenge of securing a sustainable long-term energy supply, improving processes and practices, engendering a quality culture and contributing towards continual improvement. At the same time, certain barriers to the implementation of a sustainable QMS were identified.

Abstract:
The need to improve on the quality and equity in education has been a direct challenge for the Department of Education in the South African post-apartheid era. The Department of Education, in agreement with the Education Labour Relation Council, implemented the Integrated Quality Management System (IQMS) as a quality management system, consisting of three programmes, aimed at enhancing and monitoring performance of the education system. The Integrated Quality Management System was the focus of this study and, in particular, the attitudes of educators towards Developmental Appraisal and Classroom Observation. The research was guided by the following three main research questions: What are the attitudes of educators towards Developmental Appraisal as an evaluation policy for accountability purposes? What are the attitudes of educators towards Classroom Observation as a practice for staff development with a developmental purpose? and To what extent do the attitudes of educators influence the implementation of Developmental Appraisal and Classroom Observation? Several factors influencing the implementation of Integrated Quality Management Systems were identified from literature and were selected for the study, namely the objectives, prevalence and frequency, benefits and difficulties of Developmental Appraisal and Classroom Observation. These were conceptualized in terms of three opposing dimensions using the Cube Model of Evaluation: internal and external, pull and push and bottom-up and top-down, all of which have varying degrees of tensions between them. Thus a point of balance on this three-dimensional continuum has to be delicately managed. The findings of this study, which resulted from analysing data collected from forty-four educators by means of questionnaires and semi-structured interviews, revealed the following: the overall conclusion regarding the educators' perceptions on the policy objectives of Developmental Appraisal is that the majority of the respondents tended to agree with the policy objective of Developmental Appraisal. However, one should take note of participants who did not agree, which illustrates the tension between policy and implementation and the possible resistance of educators. The findings of this study further revealed that initially educators were 'threatened' by Internal and External Evaluation and Developmental Appraisal, weighing heavily on the Performance Management aspect, which could be seen as manifesting a resistance to change. However, over time and through deeper understanding, there has been a shift in attitude once educators realised the developmental function of Developmental Appraisal in informing teaching and learning which consequently plays a major role in educators' professional development. In addition, this may have a positive effect on the implementation of Developmental Appraisal in future. However, the aspect of pressure, represented in the adapted model, is aimed at accountability of the various stakeholders within the schools to the Department of Education to ensure that quality education is provided. This suggests that there was a shift in the educators' attitudes from one of resistance to top-down directives to compliance and even a willingness to participate in the process of Developmental Appraisal and Classroom Observation. This could be a result of the monetary reward attached to performance. There was also a growing realisation of the potential effect on teaching and learning, which has resulted in the policy being viewed in a more positive light. However, there was concern about the lack of support, resources and facilities from the Department of Education, which resulted in the policy not being properly implemented. Finally, the findings of this study bring another dimension to the issue of the politics of resistance towards Integrated Quality Management Systems, in that rejection of evaluation is not against the system per se; educators as represented by the South African Democratic Teachers' Union (SADTU) want appraisal to be an essential part of their development and not a mechanism for enforcing control or imposing a 'police unit' on educators. Abstract:
The purpose of this study is to see how an organization can prepare themselves to overcome the barriers when implementing a quality management system. When introducing a new management system regardless of its intentions, there are several obstacles, or rather barriers, which an organization will face and ultimately have to deal with in order to assure a successful implementation of a QMS. Not knowing beforehand what problems may arise from adopting a new management system may halter the process of introducing the new management system and add costs, resources, and time to the introduction phase. Although there are general barriers and problems with implementing a quality management system, there are also specific barriers for each and every organization. It is of significant importance that organizations become aware of those areas of which they need to pay the most attention to when implementing a QMS. There will be forces for and against the change process of a QMS and the latter needs to be anticipated and dealt with for a successful implementation of a QMS. By using a case organization and the managers within, we were able to assess the pre-conditions at the organization and identify which barriers that were specific for them. As managers are the ones implementing the new system as well as controlling, maintaining and continuously improving it, it was necessary for us to acquire their opinions and ideas. A qualitative research approach was taken and semi-structured interviews were held at the case organization EDB Card Services AS in Mo i Rana, Norway, which included 6 managers ranging from administrative level to operations level. We found that the biggest barrier for the case organization was the fact that departments were not as inter-dependent, but rather independent from each other which is not compatible with a quality management system. It was evident that the departments could learn from each other and that spreading knowledge and proper documentation had to be improved. At the case organization one of their specific barriers were the sharing and handling of documents, this barrier could be eliminated by educating staff and managers of how to make proper documents with a clear purpose, and distribute documents to the right person at the right time. The managers were motivated and willing to spend time assuring that the quality requirements of the QMS is maintained however stressed the fact that more resources would be needed in order to achieve this. Our findings lead us to believe that general guidelines for how to implement a QMS are insufficient to successfully handle the inherent problems and barriers when changing from“what is”, to “what could be”. The specific barriers which arise when organization take a look at themselves and identify these are probably more likely to affect the implementation process more than those that are general. We came to the conclusion that EDB Card Services AS needs to deal with the barriers that we identified as forces against a QMS, namely: departmental barriers, documentation of processes and procedures, leadership, resources, and information sharing and documentation. To effectively prepare the organization for the implementation of a QMS these barriers needs to be dealt with. Furthermore, we recommend that the organization educate and train those responsible for the introduction of a QMS and that an independent steering group/committee is established.

Abstract:
The purpose of this research study was to explore how district officials monitor and provide support to teachers through the effective implementation of the Integrated Quality Management System (IQMS) in South African public schools. Through the utilization of a qualitative case study approach, and guided by a theoretical framework of “performative society”, the study attempted to provide a glimpse of how district officials carry out their responsibilities. Among others, by monitoring and providing necessary support to teachers in order to address various developmental needs as identified during the IQMS implementation cycles at institutional level. Data collection was a mix of semi-structured interviews, observations, field notes and a researcher journal. The major findings emanating from this study were six fold: First, district officials were very conversant with the policy imperatives of the ELRC Resolution 8 of 2003 that deals with the implementation of IQMS in public schools. Second, the study revealed a sense of despair and hopelessness on the side of educators because of evident absence of district officials’ provision of support, particularly during the IQMS cyclic implementation process. Third, while school based educators understood the principles, purposes and procedures of IQMS fully well, they were still struggling to translate theory into practice. Fourth, lack of proper planning in schools hindered the effective implementation of IQMS. Fifth, lack of special incentives for the SDTs. And sixth, is lack of resources in some public schools, as well as the attitude of teachers and school managers towards the implementation of IQMS in schools. These factors, the study revealed, were cited as reasons impeding the effective implementation of IQMS in public schools. Copyright

Abstract:
The purpose of this study was to investigate the extent to which Integrated Quality Management System (IQMS) is implemented in schools in the Libode education district. The study focused on the existence of IQMS documents as evidence or proof that IQMS is being implemented. In addition, the study looked at the existence and functionality of structures and processes for the implementation of IQMS in the selected schools. Lastly, the study focused on the deliverables and outcomes arising out of the implementation of IQMS in schools. The pragmatic paradigm and the case study design were used to frame and focus the research. Data was obtained from sixty (60) educators who were surveyed, principals of the three (3) selected schools, and three (3) Education District Officials (EDOs) selected on purpose for interviews. Data thus obtained was analysed both quantitatively using descriptive statistics, and qualitatively through extraction of themes. The study draws conclusions on findings based on the attendance of IQMS workshops meant to capacitate teachers on the conceptualisation and practice of IQMS; the availability and quality of IQMS workshops; understanding of IQMS processes by teachers and the officials; IQMS evaluation processes underwent by teachers; alignment between School Improvement Plans and the District Improvement Plan, and the connection between individual growth and school improvement; the constitution and functioning of IQMS structures; and the form and content of IQMS deliverables and outcomes. This research makes recommendations for IQMS implementation and further research.

ISO 9001: 2015 is the latest edition of the Quality Management System from the International Standard Organization. One of the most noticeable changes about the new edition is the emphasis on risk-based thinking. This study introduces a template for developing a Quality Manual and a systematic approach to Risk Assessment using a risk-based decision-making framework embedded in the Quality Management System. An extensive discussion on decision-making, risk and opportunity analysis is provided with the aim of developing a seamless integration between QMS, risk analysis, and decision-making. This study proposes a decision-making framework that aligns ISO 9001 requirements with the decision-making process. The proposed decision-making methodology is aimed specifically at product and service selection. A case study is used to demonstrate the methodology.

The study emerged out of an agreement reached by the Department of Education, Education Labour Relation Council, unions and stakeholders (ELRC, 2003) that Integrated Quality Management System (IQMS) be a developmental programme for quality performance in teaching and learning in public schools. The programme was based on the merging of three programmes, namely the Developmental Appraisal System (DAS), Performance Management System (PMS) and Whole School Evaluation (WSE) which were integrated in the implementation of IQMS (Resolution No.8 of 2003).The first three programmes, DAS, PMS and WSE, were less effective and had some flaws that caused implementers (educators) not to accept them as performance developmental programmes as they were punitive and judgmental. The researcher was interested in investigating those problems encountered by the educators in the implementation of IQMS. The study employed a qualitative research methodology with a qualitative survey design. Structured interviews were used to collect data. Five (5) schools out of three hundred and twenty five schools (325) in the Butterworth Education District were conveniently selected. From each school a sample of five (5) educators was purposively selected according to their duty lines on the IQMS structures, i.e. principal, school coordinator and three educators, for the purpose of getting relevant information from each level. Some of the main findings of the study were: the duration of the training was too short; unclear roles and responsibilities of role-players resulted in non-implementation of the programme; attachment of incentive to the programme made educators to focus on the incentives rather than on the programme and the heavy workload of educators hindered the implementation of IQMS. The researcher recommended that administrators/clerks be employed as a matter of urgency to all schools in order to lessen the educators’ workload especially the principals, for the benefit of accomplishing the objectives of IQMS programmes. Re-training was absolutely a necessity. Delinking of incentives to the development programme was a crucial issue. The researcher also recommended that the IQMS Departmental Officials should monitor the implementation of the programme timeuosly so as to tackle the problems in their premature stages. The improvement of working conditions in schools is essential. Key words: quality performance; quality management; quality performance measurement and quality implementation of IQMS programme.

This study evaluates the implementation of Integrated Quality Management System (IQMS) policy to improve the quality of teaching and learning at schools in the Cofimvaba Education District. IQMS involves the measurement and evaluation of teacher performance for developmental purposes and to reward the best performers in order to improve teaching. A qualitative research approach was adopted and data was collected using questionnaires distributed and retrieved from schools in order to determine educators’ experience of IQMS implementation. The data were then analysed using constant comparative methods. The findings indicate that most participants agreed with the objectives of the policy and felt that it should be retained to evaluate teacher performance for accountability purposes and improved teaching and learning results. The system has the potential to develop educators if is implemented honesty and as long as educators trust the evaluation processes to identify weaknesses and strengths. A key recommendation is that IQMS should be part of the school management team’s responsibilities, which must report quarterly on implementation.

The complex and lengthy implementation of the IQMS process influences the core duties of teachers and seems to contradict the stated aims. The research questions posed in this study specifically look at how the process is implemented and managed, and its effect on teacher's work is explored. Literature on school reform and systemic reform was reviewed to understand the context in which accountability has emerged as a key educational reform tool. The literature also provided both an international and local perspective of accountability. A critical review of Schrag's philosophical view of accountability was done in the literature review. The literature has given insight into following a qualitative research method to explore the effects of lQMS on the work of teachers.

The main purpose of this research was to evaluate the processes for IQMS implementation in the selected Eastern Cape schools so as to determine the model that can be postulated for improving the IQMS implementation in the South African schools. While mainly deriving from the fact that so far, empirical research reveals that the process for IQMS implementation has been marred with a significant number of challenges. The study star begins with the formulation of a thought-process in its overriding hypothesis. The hypothesis is that the application of a five constructs’ IQMS Implementation Model in Figure 1.1 would signficantly influence the successful IQMS implementation in South African schools. The five constructs that are outlined in the IQMS Implementation Model in Figure 1.1 include: (1) Considering the Notion of “Quality” and Whole School Evaluation, (2) Performance Management and Developmental Appraisals, (3) Considering Key Success Factors for IQMS Implementation, (4) the Use of Appropriate Implementation Processes, Systems and Methods, and (5) Constant Monitoring, and Evaluations and Applications of the Improvement Actions. Despite the fact that the integrated quality management theories strongly support this hypothesis, the study, in line with these five constructs which were also aligned with the five research questions and hypotheses that guided this research applies exploratory factor analysis. This has been done in order to further define the structure of the IQMS Implementation Model in Figure 1.1 prior to applying confirmatory factor analysis in order to determine whether the IQMS Implementation Model in Figure 1.1 would perfectly reproduce the 53 observed sample data which was used in the study. The results confirmed the overriding hypothesis in the IQMS Implementation Model in Figure 1.1 which is that effectiveness of the IQMS implementation processes in the South African schools can be significantly influenced by the application of the five constructs. These include: Considering the Notion of “Quality” and Whole School Evaluation, Performance Management and Developmental Appraisals, Considering Key Success Factors for IQMS Implementation, the Use of the Appropriate Implementation Processes, Systems and Methods, and Undertaking Constant Monitoring, Evaluations and Improvement Action. In effect, it was recommended that the Eastern Cape Department of Education must adopt the IQMS implementation Model in Figure 1.1 in order to effectively implement its integrated quality management systems. Despite examining the limitations of the study, the recommendations chapter also discussed the area for further research in which it was suggested that the area for further research would be: Evaluating the impact of appropriate standards’ setting on the successful IQMS implementation in the schools in the Eastern Cape schools.

In this report you can read about increasing demands from customers that want their suppliers to be certified by some type of standard, like ISO 9001 and ISO 14001. ISO 9001 is a standard of requirements for implementing a quality management system within an organisation and is built on seven main principles which the organization need to fulfil. ISO 14001 is a standard of requirements for implementing an environmental management system and is like ISO 9001 built on seven main principles. Implementation and the work with the standards are based of the PDCA-cycle. The thesis is made together with AGES Kulltorp AB, a company based in Kulltorp, Värnamo. They supply diecasted and machined products in aluminium and has realised that they are in need to get certified by ISO 14001 and needs an effectivization of their quality management system according to ISO 9001. To be able to implement a standard of this kind, it requires an involved, determined management team and an acceptance that work will be time and resource consuming. The organizations shall also be able to accept changes and the employees must be motivated and empowered to simplify the implementation. This report determines most of the theories from academic work by the practical work of updating the whole management system at AGES Kulltorp to fulfil all the requirements of ISO 9001 and ISO 14001. This report will also state what type of documents and routines that is required for the quality and environmental management system. The report will also describe how AGES Kulltorp made their journey against effectivisation of ISO 9001 and certification of ISO 14001. The survey that were performed at AGES Kulltorp expose most of the internal obstacles that the company must work with to be able to fully implement the management system to the organization.

This study examines the experiences of the school management teams and educators of the Isibane Circuit in the Queenstown District, regarding the implementation of an Integrated Quality Management System (IQMS) policy at school level. Following an agreement reached in 2003 between the South African Education Department and the major teacher organisations in this country, the IQMS was scheduled to be implemented in public schools from 2004. Two major challenges are identified and explained: accountability and development of human resources, as well as contradictions between internal and external evaluations. The study also elaborates on the changes in the educational history from the apartheid system to the democratic system of education. Nine schools participated in the study (two secondary schools and seven primary schools). These schools were purposely selected due to the fact that they had started implementing the IQMS policy, which was the main criterion for selection. Semi-structured interviews were used to gather data from principals, Heads of Departments as well as post-level one educators concerning their experiences of the IQMS implementation. The findings indicated that while the IQMS policy was good on paper, it was extremely difficult to implement. Issues of honesty and trust were at the root of difficulties that made the IQMS implementation challenging. The participants‟ understanding of IQMS was that it was intended for the provision of quality education and development.

The number of organisations with ISO 9000 certification over the world has increased dramatically in the last decade in developed and developing nations. The new standard ISO 9001:2000 is a way for an organisation to manage internal and external customer satisfaction and demonstrate continuous improvement. In Libya four Manufacturing Public Sector Organisations (LMPS0s) have got ISO 9000 certification, for exporting their products to reduce Libya's reliance on oil revenue. The low number of certified LMPSOs suggests that there are barriers affecting these organisations achieving ISO 9000 certification. In similar countries, for example, UAE they have more than nine hundred organisations registered with the certification. Therefore the aim of this research was "to identify and analyse the barriers that affect the implementation of a quality management system (QMS-ISO 9000 in Libyan Manufacturing Public Sector Organisations (LMPS0s))". From the literature review many barriers faced by many organisations, from different countries around the world, in their implementation process, were identified. Also, a conceptual framework was developed. There is a lack of empirical research on barriers affecting the new standard implementation, and the number of studies in Arabic countries is very limited with respect to the available literature in Western countries. To get an in-depth understanding of the barriers affecting LMPS0s, two case studies were carried out. A comprehensive list and in-depth understanding of barriers was identified by this research. Some of them (19) were identified as being unique to the Libyan culture in that they have not been reported before in the literature. Some of them are similar to barriers reported in Arabic (20), Islamic (19), Western (20) and other countries (32) such as Asian and South American countries. Therefore, this research makes a contribution to this area by adding to the limited literature. Another contribution of this research is that it has specifically filled the gap in knowledge in Libyan studies and in Arabic studies in general. Some recommendations for further research have been derived from this research.

As the importance of IT generally has increased, the usage and number of applications has grown as well. Applications are substantial assets for an organization that is heavily supported by IT. Consequently, a way of managing applications efficiently and sustaining the costs of applications rather low ? doing more with less ? is required. In addition it is also important, from a business perspective, that applications as part of IT are aligned with business strategies and goals. Application Portfolio Management (APM) is a practice and a business-centric activity aimed to allocate IT resources to support business objectives and strategies; it helps determining the impact of applications and the relative importance of each application in the portfolio to the business. APM attempts to justify and measure the financial benefits of each application relative to the costs of the applications? maintenance and operations. Performing APM successfully is nevertheless a complicated issue, and something that many businesses are challenged with on a daily basis. New applications are constantly and regularly added, downloaded, purchased or selfdeveloped ? too often without any further control. In order to regain the control of the software that automates the business, and ensuring that the applications adds value and reliability to the company, Scania CV AB requires a rigorous and comprehensive method for an APM approach. Não menosprezam toda a atividade relacionada com a inovação, mas consideram que é fundamental uma base com qualidade, permitindo assim atingir patamares superiores. O presente relatório visa abordar a metodologia e o processo de implementação das normas internacionais a nível geral. O conhecimento adquirido ao longo do percurso profissional no desenvolvimento de um software, bem como em outras competências intrínsecas, permitiram idealizar o desenvolvimento de uma plataforma capaz de gerir e implementar um sistema de gestão de qualidade. com o intuito de que este seja capaz de evoluir e de ser mantido de forma organizada. Com a implementação da ISO 27001, neste contexto, e após a análise de soluções existentes no mercado e da estrutura de implementação e certificação das normas internacionais, surgiu a necessidade de apresentar uma solução que vise facilitar este mesmo processo e que também ajude na manutenção de um sistema de gestão. Para este desenvolvimento foi necessário efetuar um levantamento de alguns sistemas que já existiam nesta área, sendo que as soluções encontradas no mercado não satisfaziam a necessidade de um sistema de gestão de qualidade. Apesar destas permitirem gerir este tipo de sistemas de qualidade, não estão focadas no processo de implementação e certificação das ISO. Deste modo, o QMS4.0 tem como objetivo minimizar esta lacuna identificada e facilitar o processo interno das empresas.
Technological developments in industry and in the methodologies used make it imperative for organizations to adopt new mechanisms and new technologies and to reflect on greater efficiency. In this context, organizations are increasingly focusing on quality management, seeing it as an effective tool to address national and international competitiveness. They do not underestimate all innovation-related activity, but considering that, a quality base is fundamental, thus allowing to reach higher levels. This report aims to address the methodology and process of implementation of international standards at the general level. The knowledge acquired along the professional career in software development as well as in other intrinsic skills allowed us to idealize the development of a platform capable of managing and implementing a quality management system in order to be able to evolve and to develop. be kept in an organized manner. With the implementation of ISO 27001 in this context, and after the analysis of existing solutions in the market and the structure of implementation and certification of international standards emerged the need to present a solution that aims to facilitate this same process and that also helps in maintaining a system management. For this development, it was necessary to make a survey of some systems that already existed in this area, and the solutions found in the market did not satisfy the need for a quality management system. Although these allow the management of such quality systems, they aren’t focused on the ISO implementation and certification process. This QMS4.0 aims to minimize this identified gap and facilitate this internal process of companies.

The South African government, through the Department of Education is continually seeking means and strategies to improve the quality of education in the country. Integrated Quality Management System (IQMS) is one of the systems adopted and implemented in all public schools to improve the quality of teaching and learning in schools. IQMS, as outlined in Collective Agreement No 8 of 2003, was informed by the Employment of Educators Act 76 of 1998 and the Constitution of the Republic of South Africa. IQMS consists of three programmes which need to be in place in order to enhance and monitor performance of the education system. Developmental Appraisal’s purpose is to appraise individual educators in a transparent manner with a view to determining areas of strengths and weaknesses and to draw up programmes for individual development; Performance Measurement’s purpose is to evaluate individual educators for salary and grade progression, affirmation of appointments and rewards and incentives, and Whole School Evaluation’s purpose is to evaluate the effectiveness of a school. The research focused on how IQMS is implemented in Mopani District secondary schools, in Limpopo Province, South Africa. Qualitative research methodology was used. Results indicated that some structures and individuals know their roles and responsibilities in IQMS implementation; however, they admit to encountering challenges in implementing it. Educators and structures generally perceive IQMS positively and believe that educator performance may improve if the challenges encountered are addressed by the Department of Education. Recommendations were also advanced on how to make IQMS serve the purpose for which it was intended.

The objective of this work is to develop a quality management system focused on the principles of TQM for cocoa micro and small enterprises (MSEs) in the province of La Convención, Cusco - Perú. The diagnosis for this investigation was made using in-depth interviews to the cocoa farmers that facilitated the identification of the current situation of cocoa MSEs. With the results obtained and based on the scientific articles studied, the critical elements in each activity related to the cocoa production process were determined, and it is focused on establishing a quality management system adaptable to the requirements of the plots studied. As a result of the analysis of the experts, the system in question is based on the following principles of TQM: documentation, customer focus, process-based approach and continuous improvement.

Since ISO 9000 was introduced in 1987 the number of organisations in developed and developing countries with ISO 9000 certification is increasing dramatically. Up to the end of 2006, the number of ISO 9001:2000 certified organisations reached 40,909 in the UK and 44,883 in the USA. Within Arab countries the number reached 1,928 in Egypt, 710 in Saudi Arabia and 1,040 in the United Arab Emirates. In Libya, however, this number up to the end of 2006 was only 46 and, according to the results of the conducted preliminary interviews of this study, the number of certified Libyan Manufacturing Companies (LMCs) up to the end of September 2007 was only 7 companies which is, relatively, a very low number. Accordingly, this thesis aims to "investigate the critical success factors (CSFs) affecting the implementation of ISO 9001:2000 in Libyan manufacturing companies'". A comprehensive review of literature was carried out to identify the critical success factors affecting the implementation of ISO 9001:2000 that were experienced by different organisations around the world, including its key aspects. The summarized factors were subsequently used in the empirical investigation of the relevant factors in LMCs. The employed research methodology involved the adoption of a quantitative approach, a survey strategy, utilising a self-administrated questionnaire method. All 7 certified companies were included in the survey. The Statistical Package for the Social Sciences (SPSS) was used to analysis the data using appropriate statistical techniques. The survey findings revealed that LMCs experienced many factors, these mostly being the same as those experienced by other companies in different countries. There was a relative degree of difference in the importance. These factors as identified in this study are presented in five groups. These groups in descending order are: management and people involvement, culture, training, communication, and support mechanism. The most critical success factor for ISO 9001:2000 implementation in LMCs is represented in the management and people involvement group which is the "top management commitment" factor. The study, therefore, contributes to the existing body of knowledge in the quality management discipline and fills the gap in knowledge in Libyan studies. Recommendations for further research also have been derived from this research.

Companies must respond and change if they wish to survive into the next century. The results and testing of the hypotheses in chapters four and five clearly indicate that there is a need for a structured Total Quality Management (TQM) system in the East London Depot, of South African Breweries (SAB). The vision of the company states that they aim to be the “benchmark of South African industry and the brewing world”. The mission of the company states that they wish to provide their consumers with the finest quality malt beverages, brewed and marketed by world - class people in a socially responsible and innovative manner. Two of the core values of the company are: • Customer service and consumer focus • Innovation and quality (http://Beernet) Taking the afore mentioned as a guide line, it is evident that although there is a need for a structured TQM system at the East London Depot, there are key focus areas to concentrate on for the program to be successful. The main focus area will be to change the perception of the staff with regards to doing things right the first time. Customer service is very important and should be understood by all. A competent person should be tasked with the implementation of the system. This person should then manage the system and ensure that the staff training and maintenance of documented procedures are adhered to. A TQM committee must also be established in order to perform the necessary audits. As mentioned in chapter one, the aim of the depot management is to improve on it’s current national ranking. A structured TQM program will make a huge contribution towards achieving the desired result. This will result in better results and achievement of goals. Abstract:
The aim of this research was to develop guidelines for improving the implementation of Quality Management Systems to provide quality service delivery in education. The research was done by means of both a literature review and an empirical research. The focus of the literature review was to determine the nature and purpose of existing Quality Management Systems (QMS) in education; and the role of District Officials and Schools Management Teams (SMTs) in the implementation of QMS to provide quality service delivery in education. Factors that contribute to the ineffectiveness of the implementation of the existing QMS in education were highlighted. The empirical research findings revealed that SMTs, being responsible for the implementation of QMS, lack knowledge and understanding of QMS implementation. Responses obtained from questionnaires revealed: limited support by District Officials, non-availability of support programmes; limited knowledge of the vision, mission and goals of the Department of Education; and limited resources provision. SMTs indicated dissatisfaction with the manner in which guidance and regulations pertaining to QMS implementation are provided. Curriculum and assessment support programmes were stated as limited, thus affecting learners’ and teachers’ performance. The impact of limited provision of resources; limited dedication to individual employees’ personal development by leadership and management; unclear communication means; and poor rewarding strategies to encourage employees and learners to perform were further highlighted. Based on the results obtained from the literature study and the empirical research, guidelines for improving the implementation of QMS to provide quality service delivery in education were developed and presented.

The objective of the reported research is to investigate the contribution which can be made to Quality Management System design and implementation through the application of discrete event simulation methods. Today's highly competitive world has forced manufacturing and service organisations to find ways to reduce costs while maintaining customer satisfaction and making continuous improvement to both products and/or services as well as processes. In approaching these demands the establishment of quality systems standards, such as ISO 9000, has been considered as one of the possible solutions. From another point of view, businesses have been making increasing use of simulation modelling as a powerful technique to reduce the risk in the process of decision-making by evaluating and analysing alternative strategies and solution in system design and improvement. Having considered the importance of quality management systems and standards as well as the capabilities of computer systems a generic simulation model for Quality Management Systems, as defined by the relevant standards, is defined and its use in a case study is demonstrated and evaluated in detail. The overall result of the research illustrates the feasibility of using computer simulation in the area of quality management and great benefits achieved by this application.

Abstract:
The purpose of the analysis was to realise potential areas for improvement within the Production Services department. Sources of inefficiency within the Reporting System were identified to support recommendations for the rationalisation, streamlining and alignment of processes. Lean philosophies and Total Quality Management techniques were reviewed to gain an understanding of applicability to Production Services. Evaluation through Value Stream Mapping determined a reporting process efficiency of 71% and two supplementary waste types were recognised in addition to the standard seven wastes of lean processes. The root cause of inefficiency and waste was investigated further with the 5 Whys Technique. A benchmark was established through reviewing industry based literature and conducting comparative studies of both like and unlike departments. Participant and non-participant naturalistic observation techniques were employed to collect a balanced data set for value stream analysis. Variable environmental factors were recognised to moderate results from direct analysis of the reporting system. Dissimilar political, economic, sociocultural, technological and legal factors were considered during comparison and benchmark. The findings in this report provide evidence of issues that should be addressed for the continuous improvement of both the reporting system, and the department. These have been provided for the consideration of Shell Todd Oil Services Limited Production Services and Logistics Manager.

The intended contribution and unifying purpose of this research was to identify the enablers and barriers affecting the implementation of quality management systems (QMS) within the Kuwaiti industrial context. This was achieved by assessing the current practices adopted by a number of manufacturing firms in Kuwait and then the gaps or areas that need to be filled were measured. QMS is defined as an organisation’s structure, responsibilities, processes, procedures and resources which guide and control an organisation with regard to quality in order to constantly improve the effectiveness and efficiency of its performance. QMS enables businesses to minimise undesired impacts on the processes while maximising the effectiveness and efficiency of those processes that deliver products or services to end-users. This PhD research focuses on identifying the general state of the manufacturing sector within Kuwaiti Small, Medium and Large Enterprises (KSMLEs) and analyses links, enablers and barriers related to the implementation of QMS by linking the possible effects of culture. The research was carried out using a case study as well as the mixed methods approach involving the use of both quantitative and qualitative research methods. In the first instance a single case study organisation involving a large sized enterprise within the Kuwaiti manufacturing sector was researched into as a pilot study. This helped in refining both the quantitative survey questionnaire and the qualitative interview questions. Thereafter, for the quantitative approach, a total of 192 valid responses out of 308 received data sets involving KSMLEs that included three different levels of management (top, middle and shop floor) were generated using physical and online-surveys, the target participants being managers working in the industrial sector in Kuwait. The qualitative research design included twenty-five interviews involving twenty-five KSMLEs, also at different management levels (six top, six middle and thirteen shop floor managers) in the manufacturing sector in Kuwait. Findings from this study confirmed that leaders and managers of the manufacturing sector in Kuwait are actively engaged with customers. However, they lack the process of delegating tasks and empowering of people. Findings further suggest that firms’ strategy must focus on appreciating cultural aspects such organisational culture, investing in employees, developing training programmes, and addressing community and social responsibility. This study contributes to the body of knowledge on QMS and provides a viable framework (model) that was over time developed through an iterative series of revisions, literature review, a case study and expert views. The new model designed and proposed in this thesis is called the Kuwait Quality Culture Model (KQCM). This model portrays the importance of culture along with other factors in the initiation and the implementation of QMS, particularly in Arabic culture. Overall findings confirm the researcher expectation that QMS performance (managing people, policy & strategy, partnerships & resources, and culture) is positively associated to commitment and leadership of top management and those managers at different levels stress on the importance and necessity of internal communication channels. Moreover, employees’ empowerment and relevant processes (people, policy & strategy, partnerships & resources, and culture) are crucial in implementing any quality practices. Furthermore, culture was found to be a vital factor as it plays an important role in the implementation of QMS in Kuwait. Finally, when implemented, the study confirmed that KQCM results (people, customers and society) would lead to better organisational performance.

Battery storage devices have been widely utilized for different applications. However, for high power applications, battery storage systems come with several challenges, such as the thermal issue, low power density, low life span and high cost. Compared with batteries, supercapacitors have a lower energy density but their power density is very high, and they offer higher cyclic life and efficiency even during fast charge and discharge processes. In this dissertation, new techniques for the control and energy management of the hybrid battery-supercapacitor storage system are developed to improve the performance of the system in terms of efficiency, power quality and reliability. To evaluate the findings of this dissertation, a laboratory-scale DC microgrid system is designed and implemented. The developed microgrid utilizes a hybrid lead-acid battery and supercapacitor energy storage system and is loaded under various grid conditions. The developed microgrid has also real-time monitoring, control and energy management capabilities. A new control scheme and real-time energy management algorithm for an actively controlled hybrid DC microgrid is developed to reduce the adverse impacts of pulsed power loads. The developed control scheme is an adaptive current-voltage controller that is based on the moving average measurement technique and an adaptive proportional compensator. Unlike conventional energy control methods, the developed controller has the advantages of controlling both current and voltage of the system. This development is experimentally tested and verified. The results show significant improvements achieved in terms of enhancing the system efficiency, reducing the AC grid voltage drop and mitigating frequency fluctuation. Moreover, a novel event-based protection scheme for a multi-terminal DC power system has been developed and evaluated. In this technique, fault identification and classifications are performed based on the current derivative method and employing an artificial inductive line impedance. The developed scheme does not require high speed communication and synchronization and it transfers much less data when compared with the traditional method such as the differential protection approach. Moreover, this scheme utilizes less measurement equipment since only the DC bus data is required.

Para a implementação do SGQ, a empresa teve de desenvolver vários procedimentos de acordo com os requisitos da Norma NP EN ISO 9001:2000, com vista à respectiva certificação por um Organismo Acreditado.

This paper, discusses the importance of implementing a QMS in a company on the construction industry. The study tries to frame the importance of quality in a modern organization, and the reasons for the company to implement a QMS, difficulties and benefits of implementation, and the changes already experienced by the organization through the process. For the QMS implementation, JAP had to develop several procedures in accordance with the requirements of the standard NP EN ISO 9001:2000, for the certification by an accredited body.

The purpose of this study was to investigate educator perceptions of the implementation of Integrated Quality Management Systems (IQMS) in Further Education and Training (FET) Colleges in South Africa. Quality in education is presently put more firmly on the agenda not withstanding the fact that a vast range of literature is available to support the process of improving the quality in education. The department of education has made many attempts to introduce an effective appraisal system in support of quality education only to find that the previous appraisal systems were rejected by the educator force for the mere fact that it was perceived as a form of inspection and supervision rather than for development purposes. This study also focuses on a review of the integrated quality management system (IQMS), of its purpose, guidelines and its subsequent implementation in FET colleges in South Africa in 2004. The primary focus of the discussion is on the processes, the methods, the guiding principles and the pitfalls of the implementation of quality management systems in educational organizations To guide this discussion literature reviews on quality, quality education, quality management and quality management systems were undertaken. The empirical study entailed gleaning information from a structured questionnaire which was distributed to a sample group of lecturers at Sedibeng College for FET in the Gauteng Province. The questions were structured in a manner to gain information about the understanding of the operational principles of quality management, the aims and guiding principles of IQMS, the importance of quality control and the external relationships of the college. The major finding emanating from this study was that there was a need for intensive training in IQMS, thus a management's strategy for ensuring that IQMS is effectively implemented was proposed. The overall recommendation arising from this study includes ensuring that management designs a quality manual to guide the process of quality assurance.

Water quality protection in Barbados is provided by means of a zoning policy defined on bacteriological travel-times, based on matrix groundwater flow; and without consideration of neither: surface/subsurface water karstic-hydrology-dynamics (which characterise the island of Barbados), nor chemical pollution-transport and -fate in surface water, which is the main source of groundwater recharge for the island. This thesis explores the institutional frameworks, as well as the physical data necessary to implement the use of a spatial, tightly-coupled Decision Support System for water quality protection and management in Barbados. While a technical DSS application is provided for demonstration purposes, focus is placed on the imminent importance of establishing a National Spatial Data Infrastructure, and on the necessary enhancement of institutional frameworks to meet the requirements for a DSS implementation at the national level. It is concluded that under current institutional and data quality, availability/accessibility conditions such an endeavour will not be possible.
La protection de la qualité de l'eau sur l'île de la Barbade est assurée par une politique de règlement de zonage définie par la mesure du temps de propagation bactériologique basée sur une matrice d'écoulement des eaux souterraines. Cette politique ne tient ni compte des dynamiques du système hydrologique-karstique des eaux souterraines/de surface (qui caractérisent la Barbade), ni du transport des polluants chimiques aboutissant en eaux de surface qui représentent la principale source de réalimentation des réserves d'eaux souterraines de l'île. Cette thèse explore les cadres institutionnels ainsi que les données physiques nécessaires pour mettre en application un système spatiale d'Appui à la prise de décision pour la protection de la qualité et la gestion de l'eau avec un système complexe de logiciels associés à l'aménagement du territoire de l'île de la Barbade. Tandis qu'une application technique SDD est fourni à des fins de démonstration, l'accent est mis sur l'importance imminente d'établir une Infrastructure Nationale de Données Géographiques, ainsi que la nécessité de renforcer le cadre institutionnel pour répondre aux exigences de le mise en pratique d'un SDD au niveau national. Il est conclu que due à la qualité des données et l'état actuel des cadres institutionnels que les conditions de disponibilité/accessibilité d'une telle entreprise ne sera pas possible.

O objetivo dessa dissertação é propor um modelo alternativo para a implementação de Sistema de Gestão da Qualidade em laboratórios de ensaios químicos que operam no ambiente universitário. O modelo proposto foi validado para o caso do Laboratório de Caracterização de Águas (LABAGUAS) do Departamento de Química da PUC-Rio. A motivação do trabalho para implementação de Sistema de Gestão da Qualidade no LABAGUAS propõe um padrão que possa ser utilizado como referência para laboratórios de ensaios químicos em ambiente universitário. A metodologia inclui revisão bibliográfica, a pesquisa documental e um estudo de caso para avaliar a implementação do Sistema Gestão da Qualidade em laboratórios de ensaios químicos em ambiente universitário, através do uso das normas gerais, de ferramentas da qualidade e de métodos de ensaios, propondo, assim, uma avaliação e melhoria dos ensaios gerenciada pelo ciclo PDCA (Planejar, Executar, Checar, Agir corretamente) na busca da melhoria contínua. Como resultados, têm-se o caso prático o ensaio determinação de pH em água a fim de avaliar as incertezas de medições analíticas, calculadas de acordo com o Guia EURACHEM e proporcionar evidências objetivas de um sistema para a garantia da qualidade. Procura-se evidenciar através de uma pesquisa, as universidades brasileiras e latinoamericanas que têm um Sistema de Gestão da Qualidade implementado em laboratórios de ensaios químicos. As principais conclusões do trabalho sinalizam que a implantação deste projeto cria oportunidades para o desenvolvimento de uma política de qualidade no Departamento de Química da PUC-RIO, estimulando o crescimento profissional e melhores condições de segurança e conforto ambiental do pessoal técnico envolvido, de forma que o laboratório possa implementar o Sistema de Gestão da Qualidade naturalmente na cultura local, sem que a liberdade acadêmica seja impactada.
This work has the objective of proposing an alternative model that serves as reference for the implementation of a Quality Management System in a chemical testing lab inside universities. The proposed model was validated for the case of the Characterization Laboratory Water - LABAGUAS, belonging to the chemistry department - PUC-RIO. The motivation of the work for an implementation of a Quality Management System at LABAGUAS proposes a standard which can be used as reference for chemical testing lab inside universities. Its methodology includes a bibliographic review, research documents and the study of a case in order to evaluate the implementation of a Quality Management System in a university environment, through the use of general standards of quality tools and methods of testing, proposing an evaluation and improvement of tests managed by the PDCA (Planning, Execution, Checking and Acting), in the search of a continuous improve. As results, we have the practical case for the determination of the pH, in order to evaluate the uncertainties of analytical measurements, calculated according to the EURACHEM guide and providing objectives evidences of a system for quality assurance. The dissertation seeks to demonstrate through a survey, that the Brazilians and Latin-Americans universities which have a Quality Management System implemented in a chemical testing lab inside a university environment. The main conclusions of the study show that the implementation of this project creates opportunities for the development of a quality policy in the chemistry department of PUC-RIO, encouraging professional growth and better standards of environmental safety and comfort of the technical staff involved so that the laboratory can implement the Quality Management System course in local culture, without wich the academic freedom is impacted.

Young generations in Taiwan are difficult to find a job nowadays, but there are several problems remain in domestic workforce, such as shortage of technical work force and lack of globalization in industrial workforce. Since 2007, Workforce Development Agency established the human resource quality management system entitled Talent Quality-management System (TTQS), to assist young generations acquire more capabilities to compete in the competitive market. To improve the implementation of TTQS, this study analyzed the core principles and proved needs of each phrase while adopting TTQS to training institutions by reviewing lectures, and by using I-P-O (Input→Process→Output) model, it tends to create a feasible and effective Check List. Then it defines proper process for implementation of TTQS and strategic approach by PDCA (Plan→Do→Check→ Act) circle operation. The case studies of the school of continuing education A university and B training and education center provide the data to this research, it is proven that the conclusions of the research and the strategic approach have been created as 1. If training institutions could correctly execute the specified strategic approach while adopting TTQS, and provide evidence of the consistency between what have been addressed or written with the actual actions, it could help the training institution pass the test and raise the grade; 2. The critical principle of each phrases Plan-Design-Do-Review-Outcome (PDDRO) such as the accuracy, system, and connection of planning , need to be fully controlled; functional needs analysis of design; systemized management of execution; examine of supervision of all process; and evaluation of outcomes, this evaluation system focus on the value create by training institutions which transfer to the target market and customers; 3. In the process of adopting TTQS, the support and the determination of managers exeute a vital role; 4. Findings among TTQS evaluation which has great positive influence to evaluation results if the training institution could demonstrate their innovations on training process or extraordinary outcome or contribution during the implementation when they are doing presentation to the evaluators.
