Abstract:
Background: Acute STEMI patients constitute a large proportion of admissions in coronary care unit and their management and prognostic implification is of immense importance. Prolonged QRS duration on electrocardiogram (ECG) has been associated with cardiac structural and functional abnormalitiesVery few studies were performed correlating QRS duration and LV systolic function in acute STEMI patients in our country. This study assessed whether QRS duration on ECG is correlated with LV systolic function measured in patients with acute STEMI.
 Aim of the Study: The aim of the study is to evaluate the relationship between QRS duration on ECG with left ventricular systolic function by echocardiography in patients with acute ST elevation myocardial infarction. Methods: A cross sectional observational study was conducted in the department of Cardiology in Mymensingh Medical College Hospital from November, 2016 to February, 2018 among purposively selected 235 patients with STEMI following inclusion & exclusion criteria. After detailed history, physical examination and investigations the selected patients underwent transthoracic Echocardiography to asses left ventricular systolic function applying Teichholz method.
 Results: Mean age of the patients 51.84±11.74 years. 135(57.45%) patients had acute anterior MI, while 98(41.70%) had Inferior MI and only 2(0.85%) had lateral MI. Mean QRS duration was 93.3 ±10.18 ms, with maximum value 125ms and minimum value 66ms. Mean left ventricular ejection fraction was 49.71%±9.87%, with maximum value 74% and minimum value 23%. QRS duration and left ventricular ejection fraction were moderate negative correlation with r= -0.611, <P = 0.001.
 Conclusion: The study concluded that QRS duration is negatively correlated with Left ventricular systolic function in patients with acute ST elevation myocardial infarction.

Abstract:
La reactivation de l'expression des genes de globine ftale est une voie therapeutique importante dans la recherche de traitements des hemoglobinopathies severes. Le travail presente dans cette these porte sur le role des polymorphismes du complexe -globine dans la comprehension des mecanismes qui interviennent dans le controle de l'expression de l'hb f. Ce type de marqueurs est egalement d'interet majeur dans les etudes de genetique des populations. La production d'hbf est controlee genetiquement. Parmi les determinants associes a une expression elevee de l'hemoglobine f, on distingue une association entre le taux d'hb f et le polymorphisme ct situe -158 pb en amont du gene g et/ou le site hs2 du lcr. Les recherches ont le plus souvent ete menees chez des sujets anemiques. L'interpretation est, dans ce cas, perturbee par l'expansion de l'erythropoiese et par l'hemolyse. Nous avons donc etudie l'association de ces deux polymorphismes avec un pourcentage de cellules f eleve, chez 48 sujets as siciliens, non apparentes et non soumis au stress anemique, qui ont permis d'etudier specifiquement l'effet du chromosome #a. Les rares cas de recombinants presents dans nos series ont permis de montrer une association plus forte du pourcentage eleve de cellules f au lcr. Les differences observees au niveau du lcr resident dans une sequence repetee de type (at)xn12(at)y. L'etude d'une large famille a plusieurs generations presentant un pourcentage eleve d'hb f a permis de mettre en evidence un nouveau mutant naturel localise dans la boite ccaat distale. L'environnement genique du gene -globine a ete caracterise par l'etude des haplotypes #a avec definition non ambigue des haplotypes, grace a l'etude de familles, sur la population sicilienne etudiee precedemment. Contrairement a la stricte homogeneite de l'haplotype lie aux mutations deleteres (telles que la mutation #s), il existe une grande variabilite des haplotypes non soumis a une pression de selection revelee par l'ajout du marqueur lcr-hs2. Cette diversite serait due a une plus grande frequence des evenements de recombinaison et de conversion genique au niveau de ce locus, lorsqu'il n'est pas soumis a une pression de selection. La difference de variabilite entre les deux types de chromosomes pourrait aussi s'expliquer par le fait que les chromosomes #s ont ete selectionnes de facon recente et qu'ils n'ont donc pas eu le temps de diverger. L'acces privilegie a la population du mzab nous amene a etudier les variants hemoglobiniques dans cette population. L'observation chez les mozabites de l'hb d ouled rabah, consideree comme marqueur prive des touaregs kel kummer etait inattendue. Cela nous a amene a caracteriser le contexte genique de la mutation sur 100 kb dans les deux populations. Les groupes erythrocytaires abo, rh et kell ont aussi ete analyses. Les donnees observees permettent d'emettre l'hypothese que ce marqueur puisse etre considere comme specifique des populations berberophones.

A Generalização Cartográfica é uma área de Cartografia utilizada para se obter versões de mapas cartográficos. Em mundos virtuais este processo pode ser implementado com a geração de níveis de detalhamento (Level of Detail – LOD), mas não existem regras precisas para construir esses níveis de forma automática; grande parte dos trabalhos encontrados na literatura foca apenas na simplificação de polígonos. Para se obter versões generalizadas de mapas, a cartografia faz uso de doze operadores, aplicados pelo Cartógrafo segundo o seu conhecimento do problema. Estes operadores são: simplificar, suavizar, agregar, amalgamar, “merging”, colapsar, refinar, exagerar, aumentar, deslocar, classificação e simbolização. É realizada uma análise destes operadores verificando a viabilidade de sua aplicação em mundos virtuais. O conhecimento de um especialista na área de Cartografia é modelado através de regras de conhecimento, as quais alimentam um sistema especialista utilizado para selecionar os objetos de acordo com o tema, que nesta dissertação é o turismo, e para a aplicação dos operadores sobre o mundo virtual. Três dos operadores cartográficos foram escolhidos para serem implementados para mundos virtuais, pois já eram utilizados de forma isolada: simplificar, suavizar e simbolização. A partir destas informações, o SisGen3D, Sistema de Generalização para mundos virtuais construídos em VRML (Virtual Reality Modelling Language), é apresentado e validado com um mundo virtual, o Recife Antigo Virtual. Este sistema foi desenvolvido em JAVA, modelado com UML (Unified Modelling Language) e utiliza o gerenciador de banco de dados MySQL.<br>This work deals with the transposition of cartographic concepts and tools to virtual reality applications, using cartographic generalization in virtual urban worlds for tourism applications. Cartographic Generalization is an area in Cartography concerned with the generation of versions of cartographic maps. In virtual worlds, it can be implemented using Levels of Detail - LODs, but there are no known rules for constructing these levels automatically; most of the literature concentrates in polygon simplification. To get versions of maps, Cartography uses twelve operators, applied by the cartographer using domain-specific knowledge: simplification, smoothing, aggregation, amalgamation, merging, collapse, refinement, exaggeration, enhancement, displacement, classification and symbolization. An analysis of these operators is performed in the context of virtual reality. The cartographer domain-specific knowledge is modelled in knowledge rules in an expert system. This expert system is responsible for the selection of important objects and of the application of cartographic operators in virtual worlds. Three operators were implemented to virtual worlds: simplification, smoothing and symbolization. Finally, SisGen3D, a system for generalization virtual VRML worlds is presented. This system, developed in Java, modelled in UML and using the MySQL database, is validated using the Recife Antigo Virtual as a testbed.

博士<br>國立臺灣科技大學<br>電子工程系<br>94<br>Owing to the support by most rendering systems, the polygonal mesh representation has been the dominant representation scheme of 3D objects for many years. However, to represent an object in high resolution, it usually requires enormous amount of polygons. Consequently, the polygonal mesh simplification as a very useful scheme to prevent from overly use of polygons was intensively studied. Traditional iterative-edge-collapse-based in-core simplification algorithms usually create very good quality approximations. Somehow, this class of algorithms has relatively low runtime efficiency and large main memory cost that is sensitive to its input mesh size. On the other hand, the out-of-core grid-based algorithms capable of dealing with very large meshes often create very low quality approximations at low resolution grids. In this dissertation, we will propose a linear-time high efficiency and low main memory cost simplification algorithm for high quality discrete LOD generations, a novel cache-based approach to dealing with large meshes, and a storage independent simplification system allowing in-core, out-of-core, hybrid operations, and multistage integration of simplification algorithms of various types.
