Abstract:
<strong>Background:&nbsp;</strong>Meckel&rsquo;s Diverticulum is a congenital anomaly of the gastrointestinal tract that can present with a wide range of clinical manifestations. This study aims to compare incidental and symptomatic cases of Meckel&rsquo;s Diverticulum to better understand the clinical characteristics and outcomes associated with this condition.&nbsp;<strong>Materials and Methods:&nbsp;</strong>We conducted a retrospective analysis of patients diagnosed with Meckel&rsquo;s Diverticulum at MGM Medical College and Hospital, Jamshedpur. over a five year period. Patient demographics, clinical presentation, diagnostic modalities, and surgical outcomes were analyzed and compared between incidental and symptomatic cases.&nbsp;<strong>Results:</strong>&nbsp;A total of 30 patients with Meckel&rsquo;s Diverticulum were included in the study, with 60% of cases being incidental findings during other surgical procedures. The remaining 40% presented with symptomatic complications, including gastrointestinal bleeding, obstruction, and inflammation. The average age of incidental cases was 32 years, while symptomatic cases had an average age of 28 years. Diagnostic modalities such as abdominal ultrasound, computed tomography (CT) scans, and technetium-99m pertechnetate scintigraphy were utilized in both groups, with CT scans being the most commonly employed. Surgical resection was performed in all symptomatic cases, leading to a resolution of symptoms and a low rate of postoperative complications.&nbsp;<strong>Conclusion:</strong>&nbsp;Meckel&rsquo;s Diverticulum is commonly diagnosed incidentally during other surgical procedures and tends to present at a slightly older age in incidental cases compared to symptomatic cases. Accurate preoperative diagnosis through imaging studies is crucial for appropriate surgical management in symptomatic cases. Surgical resection remains the standard treatment for symptomatic Meckel&rsquo;s Diverticulum, resulting in favorable outcomes and low postoperative morbidity. &nbsp; &nbsp;

ABSTRACTINTRODUCTIONIn 2013, the United States Preventive Services Task Force recommended annual low-dose computed tomography (CT) screening for lung cancer in high-risk adults with a significant smoking history. These screenings result in large numbers of incidental findings, and although most of these do not warrant further investigation, there have been reported cases of incidental findings identified on CT screening that led to successful treatment of a previously undiagnosed comorbidity.CASE HISTORYHere, we report a case of papillary renal cell carcinoma that was detected incidentally on low-dose CT in an asymptomatic individual, a rare diagnosis considering that renal neoplasms account for &amp;lt;1% of incidental findings on these screenings.CONCLUSIONThis case highlights the value of investigating these incidental findings, with the goal of detecting underlying disease in some cases before it would have presented symptomatically.

Abstract:
OBJECTIVEOccasionally, diffusely infiltrating low-grade gliomas (LGGs) are identified as incidental findings in patients who have no signs or symptoms that can be ascribed to the tumors. The diagnosis of incidental, asymptomatic LGGs has become more frequent due to the vast increase in access to medical imaging technology. While management of these lesions remains controversial, early surgery has been suggested to improve outcome. The authors set out to identify and review the characteristics and surgical outcomes of patients who underwent surgical intervention for incidental LGG.METHODSAll cases of LGG surgically treated between 2004 and 2016 at the authors’ institution were analyzed to identify those that were discovered incidentally. Patients with incidentally discovered LGGs were identified, and their cases were retrospectively reviewed. An “incidental” finding was defined as an abnormality on imaging that was obtained for a reason not attributable to the glioma, such as trauma, headache, screening, or research participation. Kaplan-Meier analysis was performed to determine actuarial rates of overall survival, progression-free survival, and malignant progression–free survival.RESULTSIn 34 (6.8%) of 501 adult patients who underwent surgery for LGG, the tumors were discovered incidentally. Headache (26%, n = 9) and screening (21%, n = 7) were the most common indications for brain imaging in this group. Four of these 34 patients had initial biopsy after the tumor was identified on imaging. In 5 cases, the patients opted for immediate resection; the remaining cases were managed with a “watch-and-wait” approach, with intervention undertaken only after radiological or clinical evidence of disease progression. The mean duration of follow-up for all 34 cases was 5 years. Twelve patients (35.3%) had disease progression, with an average time to progression of 43.8 months (range 3–105 months). There were 5 cases (14.7%) of malignant progression and 4 deaths (11.8%). Oligodendroglioma was diagnosed in 16 cases (47%) and astrocytoma in 15 (44%). Twenty-five patients (74%) had IDH1 mutation and demonstrated prolonged survival. Only 2 patients had mild surgery-related complications, and 16 patients (47%) developed epilepsy during the course of the disease.CONCLUSIONSIn this retrospective analysis of cases of incidentally discovered LGGs, the tumors were surgically removed with minimal surgical risk. In patients with incidental LGGs there is improved overall survival relative to median survival for patients with symptomatic LGGS, which is likely attributable to the underlying favorable biology of the disease indicated by the presence of IDH1 mutation in 74% of the cases.

context: Medico-legal autopsy is performed to find out the cause, time of death and identify diseases which could have contributed to the death. Gross and microscopic examination, toxicological analysis and laboratory investigations will aid in establishing the cause of death. Histopathological analysis is considered as the gold standard in finding out a cause of death in cases of sudden death or when there is no history of any previous illness. Methods and MaterIal: Four infant autopsies were conducted and their respective organs which included lungs, liver and brain and were received in our department. statIstIcal analysIs used: Results were tabulated, analyzed, and subjected to statistical analysis using SPSS (Statistical Package for Social Sciences) Software (Trial version). results: First case showed fat emboli which was an incidental finding. Second was a case of hyaline membrane disease in a newborn. Third one was a case of steatohepatitis in a 9 month old child and the last one was an incidental finding of gemistocytic astrocytoma which was undiagnosed antemortem. conclusIon: The precise cause of death must be determined with reference to clinical, autopsy, and forensic findings before reporting histopathological finding. Histopathology in autopsy plays a vital role in the study of some of the rare lesions contributing to the knowledge of pathology. This study highlights the various incidental and rare cases in autopsies, which are of critical value in academic and research purposes.

We performed a retrospective chart review to categorize a group of petrous apex findings that were noted incidentally on magnetic resonance imaging (MRI) in 88 patients. These patients were among those who had been seen at a tertiary care center between July 1988 and July 1998. These incidental findings, which were unrelated to the presenting clinical manifestations, included asymmetric fatty bone marrow (n = 41), inflammation (19), cholesterol granulomas (14), cholesteatomas (9), and neoplasms (5). Followup imaging and clinical surveillance of these patients has not demonstrated any significant change in the incidentally detected lesions. In all cases, the incidental MRI findings represented benign pathology.

BACKGROUND Gallbladder adenocarcinoma is the most common malignant tumor of the biliary tract. Most of gall bladder cancers are detected incidentally only after pathological examination of the surgical specimens. In this study we investigated the characteristics of incidental gallbladder cancers in our center and also reviewed the current literature regarding the diagnosis and treatment of such incidentally detected tumors in the gall bladder. METHODS We retrospectively reviewed all of the cholecystectomy specimens in the archives of Pathology Department in the hospitals affiliated to Shiraz University of Medical Sciences in the study period (2010-2016). Clinicopathological characteristics were extracted from the patients’ clinical charts, which included symptoms, radiological findings, laboratory data, and surgical procedures as well as outcome. RESULTS During these 7 years we identified 18 cases of incidental gall bladder cancer, consisted of 13 women and 5 men with the age range of 32 to 85 (62.5 ± 14.2) years detected after pathological study of the resected gall bladders among more than 4800 resected gall bladders. During the period, only two patients were operated on with the impression of gall bladder adenocarcinoma, which was not included in the study. Ten cases were T1 and eight were T2 at the time of surgery. They have been followed up for 1-7 years, during which, six cases of T2 died. All of the T1 cases are alive and symptom free. CONCLUSION By increasing laparoscopic cholecystectomies in our center we observed 0.37% incidental cases of gall bladder adenocarcinoma. Preoperative diagnosis of this cancer in early stages is very difficult and needs high degree of suspicion. The most important predictor of prognosis is the stage of the cancer at the time of surgery. To the best of our knowledge, this is the first report of incidental gall bladder cancer from Iran.

Abstract:
Objective: To determine the frequency of gall bladder carcinoma incidentally diagnosed in patients undergoing routine laparoscopic cholecystectomy. Material and Methods: Histopathology section of Pathology Department, Azra Naheed Medical College Raiwind Road, Lahore from January 2018 to July 2021. A total of 214 simple or laparoscopic cholecystectomy specimens were included. Detailed gross &amp; microscopic examination was done. All diagnoses including benign histopathological findings and incidental gall bladder carcinoma were analyzed. The important findings like resection margin involvement, maximum wall thickness of gallbladder, type of adenocarcinoma and TNM staging were noted. Results: Out of 214 simple or laparoscopic cholecystectomy specimens, there were three (1.4%) cases of incidental gallbladder carcinoma exclusively found in females. Mean age recorded was 45.7 years. Maximum thickness of gall bladder wall was 4.7 cm and maximum stage was pT2. Conclusion: According to our study there were three cases of incidental gallbladder carcinomas out of 214 cholecystectomy specimens. Considering this, histopathological study of gall bladder is essential not to miss any carcinoma, keeping in view the grim prognosis. Abstract:
We studied the rate of incidental detection of lung carcinomas and its effect on long-term survival in a nationwide cohort of patients operated for nonsmall cell lung cancer (NSCLC).All patients operated for NSCLC in Iceland during 1991–2010 were included. Demographic and clinicopathological features were compared in patients diagnosed incidentally using chest radiography or computed tomography (CT), and in those with symptomatic presentation. Multivariate analysis was used to evaluate prognostic factors.Out of 508 patients, 174 (34%) were diagnosed incidentally; in 26% of cases by chest radiography and in 8% by CT. The CT-detected tumours were significantly smaller than symptomatic tumours, diagnosed at earlier TNM (tumour, node and metastasis) stages and more often of adenocarcinoma histology. 5-year cancer-specific survival for symptomatic versus incidentally diagnosed patients detected by chest radiography and CT was 41%, 57% and 68%, respectively (p=0.003). After adjusting for stage, the hazard ratio (HR) for NSCLC mortality was significantly lower for incidental diagnosis by CT (HR 0.55, 95% CI 0.31‒0.98; p=0.04) compared to incidental diagnosis by chest radiography (HR 0.95, 95% CI 0.70‒1.27; p=0.71) or symptomatic diagnosis (HR 1.0).One-third of surgically treated NSCLCs were detected incidentally, with an increasing rate of incidental CT diagnosis. NSCLC patients diagnosed incidentally by CT appear to have better survival than those diagnosed incidentally by chest radiography, and particularly those who present with symptoms.

ObjectivesCancer can be diagnosed in the absence of tumour-related symptoms, but little is known about the frequency and circumstances preceding such diagnoses which occur outside participation in screening programmes. We aimed to examine incidentally diagnosed cancer among a cohort of cancer patients diagnosed in England.DesignCross-sectional study of national primary care audit data on an incident cancer patient population.SettingWe analysed free-text information on the presenting features of cancer patients aged 15 or older included in the English National Audit of Cancer Diagnosis in Primary Care (2009–2010). Patients with screen-detected cancers or prostate cancer were excluded. We examined the odds of incidental cancer diagnosis by patient characteristics and cancer site using logistic regression, and described clinical scenarios leading to incidental diagnosis.ResultsAmong the studied cancer patient population (n=13 810), 520 (4%) patients were diagnosed incidentally. The odds of incidental cancer diagnosis increased with age (p&lt;0.001), with no difference between men and women after adjustment. Incidental diagnosis was most common among patients with leukaemia (23%), renal (13%) and thyroid cancer (12%), and least common among patients with brain (0.9%), oesophageal (0.5%) and cervical cancer (no cases diagnosed incidentally). Variation in odds of incidental diagnosis by cancer site remained after adjusting for age group and sex.There was a range of clinical scenarios preceding incidental diagnoses in primary or secondary care. These included the monitoring or management of pre-existing conditions, routine testing before or after elective surgery, and the investigation of unrelated acute or new conditions.ConclusionsOne in 25 patients with cancer in our population-based cohort were diagnosed incidentally, through different mechanisms across primary and secondary care settings. The epidemiological, clinical, psychological and economic implications of this phenomenon merit further investigation.

Abstract:
Introduction and aim. Most gallbladder cancers (GBCs) are discovered incidentally after routine cholecystectomy. The clinicopathological characteristics and prognostic implications of incidental gallbladder cancer (IGBC) versus non-incidental gallbladder cancer (NIGBC) is not known. Material and methods. During this study, clinicopathological details compared between incidental and non-incidental GBC groups included age, sex, clinical presentation, preoperative radiological diagnosis, surgical management, and macroscopic and microscopic features. The primary outcome of the study was difference in overall survival (OS) between IGBC and NIGBC. Results. Among 348 surgically treated patients, 56.6% weren’t preoperatively suspected of GBC. Macroscopic examination showed characteristic thickened gallbladder wall without mass lesion (IGBC) vs. clear mass lesion (NIGBC) on imaging. Interestingly, NIGBC had higher LVI (27% vs. 14%) and T stage (68% T2b/T3 vs. 47% T1b/T2a) despite lower margin involvement (p &lt; 0.001). The OS for all patients was 12.2 months (median). Among patients who underwent surgery with curative intent, the median survival time was 21.4 months. However, within this group, NIGBC cases had a worse median survival (17 months) compared to IGBC cases (21 months). Conclusion. Rising incidental GBC necessitates routine microscopic examination of all gallbladder specimens. Surgeons in high-risk areas should remain vigilant for GBC in patients with atypical clinical and ultrasound findings. Early detection and curative resection are paramount for long-term survival in gallbladder carcinoma, with IGBC potentially offering a survival benefit regardless of stage or tumor characteristics. Prospective studies including detailed pathology and molecular analysis are needed to confirm this observation.

This project focuses on the Incident Management System (IMS) and how it was used to manage the Oklahoma City, World Trade Center, and Lafayette, IN terrorism incidents. The Incident Management System is used by emergency response agencies to manage the scene of a disaster and consists of eight management components which are: Modular Organization, Integrated Communications, Common Terminology, Unified Command Structure, Consolidated Action Plans, Manageable Span of Control, Designated Incident Facilities, and Comprehensive Resource Management. Through journal articles and interviews, this project assesses observations that occurred during the response phase of each terrorism incident. These observations are then categorized under each of the IMS components to determine whether unique, or more complex procedures occurred with terrorism, as compared to other types of emergency disasters. This study further explores why the IMS is important to communities, and why a community's comprehensive plan should include goals and risk management studies that affect a community's ability to effectively respond to a terrorism incident.<br>Department of Urban Planning

This thesis focuses on the topics of Business Continuity Management and Disaster Recov-ery Management in the context of small and medium sized businesses which offer or use IT services in the cloud. The aim of this thesis is to carry out a theoretical research of BCM and DRM in the aforementioned context and to analyze situation in a specific company based on the results of the research. This includes analysis of specific incidents that hap-pened in the company, analysis of how the company reacts to the incidents and how are the incidents logged and reported. The analysis identifies weak spots in the company and their potentials of improvement. The most serious weak spot discovered is that BCM and DRM are not implemented in the company. In the last part this thesis suggests measures to im-prove the situation in the specific company. That includes both specifying general goals and procedures and also defining specific policies, plans and reaction schemes. Specifically those are politics handling the incidents categorization, warning and communication, inci-dent reporting and performing maintenance.

Abstract:
This study examines an English for Academic Purposes (EAP) teacher's speech throughout one curricular unit of an intermediate grammar and writing course in order to better understand which high-value vocabulary students might acquire through attending to the teacher and noticing words that are used. Vocabulary acquisition is important for English for Academic Purposes students, given the vocabulary demands of academic language. The Academic Word List (Coxhead, 2000) has been shown to include important vocabulary in written academic texts, and has become a standard part of English for Academic Purposes curricula and pedagogical materials. Although explicit vocabulary instruction is important, research has shown that large amounts of vocabulary may be acquired incidentally by attending to meaning. Classroom instruction provides a great deal of input, and could potentially offer a chance for students to encounter and begin to learn academic vocabulary through incidental acquisition. However, existing research on incidental vocabulary acquisition in classrooms has focused on adult instruction and English as a Foreign Language settings, resulting in a lack of evidence about English for Academic Purposes classrooms. To respond to these needs, this study analyzes the occurrence and repetition of Academic Word List items in the teacher's speech throughout two weeks of a course in an intensive academic English program in the United States. Two weeks of naturalistic class recordings from the Multimedia Adult Learner Corpus were transcribed and analyzed using the RANGE program to find the number of academic vocabulary types in the teacher's speech and how often they were repeated. Additionally, I derived categories of classroom topics and coded the transcribed speech in order to investigate the connection between topics and academic word use. Academic Word List items are present in the teacher's speech, although they do not constitute a large proportion overall, only 2.8% of the running words. Most of the AWL types relate to specific classroom topics or routines. There are 13 AWL types repeated to a high degree, and 26 AWL types repeated to a moderate degree. These items are the most likely candidates for incidental vocabulary acquisition, though there is evidence from the videos that most of the students already understand their general meanings. It is unlikely that students could learn a great deal about AWL items that they were not already familiar with. However, it is possible that the teacher's speech provides incremental gains in AWL word knowledge. These findings show that there may be a substantial number of AWL items that students learn about even before explicitly studying academic vocabulary. Teachers should try to draw out students' familiarity with these forms when explicitly teaching AWL vocabulary in order to connect familiar words with their academic meanings and uses.

I decided to write this master thesis in relation with my work experience at CSC within the service desk. Working at the service desk is a very complete mission that has to be well understood from analysts in order to reach high performance and thus satisfy users. Because it is the goal of CSC, satisfying customers, I decided to focus on this principle and with the results of my research and work experiences I would like to bring in this thesis some findings and recommendations I came up with in order to optimize the service desk quality and to improve the customer satisfaction. In the first chapter, I will mainly describe the work of an analyst at the service desk, and I will define the incident management process. It will help understanding how a service desk works. The incident management process is monitored by many metrics. Some of these metrics are documented and some of them are not and their impacts are different. We will see on what we should focus our efforts. We will see in the second chapter the impacts of these metrics on performance. Measuring the performance of the service desk is important in order to be aware of what should be improved to reach the highest standard of services provided. We will see in the end of this chapter that the most important performance indicator is the customer satisfaction. From this fact, I will bring some findings from seven months of experience within the service desk in order to improve the quality of the services and the customer satisfactions. I will focus on what can be implemented from the service desk analysts not from the management. I will also focus on the effects of the steps already taken to improve customer satisfaction.

Abstract:
The present research explores the process of coming to terms with the decision to divorce. The research was conducted with 20 women of North-American culture, divorced or separated a minimum of six months with no thought of reconciliation. The methodology employed retrospective accounts. A semi-structured interview using the critical incident technique pioneered by Flanagan (1954) was used to gather data. The subjects were asked to describe specific incidents which prompted them to reassess their marriage and eventually decide to separate or divorce. They were also asked to describe incidents which made it more difficult to come to that decision. A total of 175 incidents were collected illustrating a range of experiences which either precipitated or hindered the decision to separate or divorce. Using an inductive method of analysis, the data was organized in a classification schema consisting of three superordinate categories - feelings, cognitions, behaviours - and 33 subcategories. In addition, a summary of the marital problems highlighted in the critical incidents is provided, with examples of the marital dynamics involved. Finally, a four-stage model outlining the process of coming to terms with the decision to divorce was derived from the category system. The model focuses on the intrapsychic dynamics of the subjects in the decision-making process. The labels given to these stages are disillusionment, ambivalence, cognitive restructuring, and resolution. The findings of the present research are compared and contrasted to those of social exchange theorists (Albrecht & Kunz, 1980; Levinger, 1965), stage theorists (Duck, 1982; Kaslow, 1981; Ponzetti & Cate, 1988; Vaughan, 1979), and grief theorists (Crosby, Gage & Raymond, 1983, 1986; Wiseman, 1975). The issues raised in the present research are discussed from a gender role perspective, in light of the theories of Attanucci (1988), Eichenbaum and Orbach (1983), Gilligan (1982), Goodrich, Rampage, Ellman and Halstead (1988), Herman (1977), Lerner (1977), Miller (1976; 1983; 1984; 1986) and Rubin (1983). Part of installing new infrastructure is that work is commenced around existing and live services. During installation, live services are occasionally struck which have the potential to cause injury and/or death along with subsequent economic costs of having to repair the damaged services. David Hopper and Shane Soulsby have investigated some of the root causes for service strikes to occur with a distinct focus on process, communication, culture and costs. From that, key findings have been established along with recommendations and an implementation plan in an effort to lower the number of service strikes on projects.

Abstract:
Academic institutes' networks, like commercial networks, have confidential and valuable information that attracts hackers. From 6 October 2000 to 29 March 2001, the authors collected data on possible attacks and probes against East Tennessee State University's campus network. The number of suspicious activities detected daily varied from 200,000 to more than 2,000,000, with ICMP-based attacks accounting for more than 81% of all attacks. While ICMP-based attacks were reasonably harmless, these activities as a whole depleted network bandwidth significantly. Severe attacks were detected daily. Port scans and host scans that involving 2 or more /24 subnets were detected every week. Attacks and probes were distributed throughout a typical day and week. Our research results suggested policy makers in academic institutions like ETSU should adopt standard measures to secure campus networks, including firewalls, intrusion detection systems, server management, and risk assessment.

AbstractChapter two constructs a comprehensive ethical framework to facilitate the analysis of intricate bioethical issues like incidental findings (IFs) in genomics. Drawing from both secular and Islamic traditions, it synthesizes Robert Veatch's multi-layered approach to bioethics and the recommendation by Muslim ethicists to engage diverse scholarly disciplines. The “Theoretical Level” section explores Islamic metaethics rooted in theology and legal theory, centering on aligning human actions with God’s will to achieve benefit and avert harm. It examines the process of religio-ethical reasoning (ijtihād) employed by Muslim scholars to discern divine guidance on novel issues. The “Practical Level” section outlines the fivefold classification scheme for categorizing human acts based on their moral value within the Islamic tradition: prohibited, obligatory, reprehensible, recommended, and permissible. Distinct from secular schemes, this classification’s theological foundations, definitions, and moral dimensions are elucidated. Bridging theory and practice, the chapter proposes utilizing this fivefold scheme as a nuanced tool to evaluate the ethical management of IFs. It advocates a dynamic approach, acknowledging how evolving scientific understanding may shift the categorization of specific IFs over time. The chapter lays the groundwork for the subsequent analysis, where representative cases illustrating each ethical category are examined through the synthesized Islamic ethical lens, fostering constructive dialogue between religious and secular bioethical discourses on this complex issue.

AbstractImpacted canines are one of the common problems encountered by the oral surgeon. Patients may present at different ages and many cases will be incidental findings. Close interaction with the paedodontist and orthodontist is required to get an optimal outcome. Surgical removal may not be the best treatment in all the cases and particular treatment plan will have to be tailored for the needs of the patient. Localizing the impacted canine seems not a challenge any more with the advent of CBCT, in indicated cases. This chapter elaborates on canine impaction, keeping in mind the basic principles mentioned in the chapter on third molar impactions. Premolars, incisors and other teeth may be impacted but most of the surgical principles and approaches mentioned for canine can be applied to them as well.

AbstractThe advent of flow diverter (FD) stents has apparently reduced the role of microneurosurgery for paraclinoid aneurysms despite sparse high-quality evidence about their long-term effects.The present study critically reviews the overall results of a microneurosurgical series of 57 paraclinoid aneurysms.Of these aneurysms, 47.4% were regular in size while 19.3 were giant. Barami type I was predominant. In 21 aneurysms a hemorrhagic onset occurred. Pterional approach with intradural anterior clinoidectomy was preferred by far. Clipping was possible in 91.2% of aneurysms and a high-flow bypass was the choice in five cases. An mRS of 0–2 was achieved in 77.3% of patients, typically &lt;50 years old.Visual field appeared improved or unchanged in 36.3% and 63.6% of the symptomatic patients, respectively. In 76.1% of incidental aneurysms, campimetry was unaffected by surgery.A complete aneurysm exclusion was achieved in 93% of cases using a single procedure. No recurrences were documented on an average follow-up of 54.1 ± 34 months.Microneurosurgery is still a valuable, definitive, and durable option for Barami type Ia, Ib, or II paraclinoid aneurysm, especially in patients &lt;50 years old and visually symptomatic. Conditions other than these are ideal candidates for FD stents.

Abstract Statistics maintained by the Pipeline and Hazardous Materials Safety Administration (PHMSA)(1) suggest increasing trends in internal corrosion incidents on hazardous liquid and natural gas transmission pipelines in the United States. In spite of improvements in pipeline integrity and risk management, pipeline degradation due to internal corrosion continues to cause leaks and failures. In some cases, pipeline operators may be missing opportunities to learn from these incidents, and thereby develop improved approaches to mitigation and prevention. This paper demonstrates an analytical methodology to learn from internal corrosion incidents that consists of three levels of analysis; determining the immediate (mechanistic) cause, identifying the basic or contributing causes, and analyzing the root causes from a management systems perspective. A hypothetical example case is used to demonstrate how each of these levels of analysis (techniques) is conducted. Pipeline operators can use one or more of these analyses to learn from internal corrosion incidents and identify steps for continuous improvement of internal corrosion management programs. The methodology presented in this paper is equally applicable to any type of integrity threat. To fully implement lessons learned and realize the value of the analysis, the need for planning and follow-through is also discussed.

Abstract Aqueous alkanolamine regenerative solvent systems have been widely used to remove CO2 and H2S in refineries, gas processing plants, chemical plants, and liquefied natural gas (LNG) production facilities for over five decades. Industry advances in amine chemistry and equipment design have greatly reduced the corrosion risks in amine systems compared to the early monoethanolamine (MEA) systems employed in the 1950’s, however, some corrosion risks remain. Corrosion threatens the integrity of plant equipment and places plant personnel at risk of chemical exposure and injury. Downtime, lost production, and equipment repairs resulting from corrosion cost the oil and gas industry over 1 billion dollars annually. Corrosion in amine systems due to high acid gas loadings and oxygen degradation products have been well documented in industry literature, but there are other causes of corrosion in amine systems which can have similarly destructive results. This paper will present five case studies of corrosion related failures in alkanolamine acid gas removal systems including examples of the following types of corrosion: Uniform, Pitting, Chloride Induced Crevice, Cavitation, and Microbiologically Induced Corrosion (MIC). Identifying and correcting the causes of corrosion requires an interdisciplinary approach using multiple scientific tools including: chemistry, engineering design, metallurgy, and computer simulation. Each case study will explain the types of corrosion observed and the scientific concepts used to identify and resolve each type of corrosion.

Introduction: Intracranial arterial dolichoectasia (IADE) is a diameter increase and/ or a long and tortuous path in one or more Intracranial arterial. Most patients keep asymptomatic and eventually present neurological complications (ischemia, bleeding or compression of adjacent structures). At around 12% of the patients with stroke present this kind of intracranial dilated, which 80% are in the posterior cerebral circulation, mainly in basilar artery. Objective: Report an unusual case of IADE refractory to the clinical treatment. Method: The information was obtained by reviewing the medical record, after the patient’s consent. Result: Patient, 51, male, hypertensive and dyslipidemic, with abdominal aortic aneurysm surgery and an incidental diagnosis in 2015 of fusiform basilar artery aneurysm, presented in March 2020 after cervical flexion, dysarthria, horizontal diplopia when looking to the right and ptosis to the left. The computed cranial angiotomography showed a basilar artery of 1.8 cm diameter fusiform aneurysm, compressing the pons and medulla oblongata, with no signs of ischemia or bleeding. He was discharged from hospital with clopidogrel without deficits, however, after 30 days, manifested a new focal neurological deficit with spontaneous remission. During the investigation a skull resonance was presented without ischemia, transcranial doppler with circulatory delay and without embolization. In view of the recurrence, an exchange in clopidogrel for ticagrelor was decided, which was kept stable for 30 days. Conclusion: IADE may be an incidental finding or even a life-threatening illness. Thus, these cases remain a major challenge in clinical practice and in interventional radiology.

In 2008, Pipeline Research Council International (PRCI) took the initiative to identify the main causes of reportable incidents in compressor and pump stations. Data was gathered from several sources including the United States� Department of Transportation Pipeline and Hazardous Materials Safety Administration, Canada�s National Energy Board, and PRCI member companies. More than 1600 incidents were reviewed over an 18 year period (1990 to 2008). The incidents were evaluated based on their frequency of occurrence and the consequences of the incidents (injury, ignition, environmental impact, etc�). In pump stations, pump seals, valves out of sequence due to operator error, and gasket and bolting were identified as the highest impact incidents types. In compressor stations, the three highest impact incident types were found to be pipe components, natural forces (hurricanes and lightning strikes), and gaskets and bolting. During the 2008 project, research roadmaps were developed based on the results of the incident data review. In the process of defining the research projects, a brief review into the available technology for the incidents types was conducted. It was quickly found that a more detailed state-of-the-art review was needed to accurately identify the research required for several of the incident areas. Therefore, a state-of-the-art review of the three highest impact incidents in pump and compressor stations was proposed. The work documented in this paper is the state-of-the-art review of these incidents. In the PRCI CPS 9-1 (2008) project, it was found that more information was needed on several of the incidents in order to fully define the root cause. Therefore, the first task of the PRCI CPS 9-1 (2009) effort was to attempt to gather more information on the top three impact incident types. Thirty-two pipeline companies were contacted and additional information was provided for approximately 25% of the incidents. From the review of this additional and past data, several focus areas were identified for the state-of-the-art reviews. The state-of-the-art studies included a survey of the current technology, identification of common failure mechanisms, and review of strategies to reduce incident occurrences. These studies are reviewed in detail in the appendices of this document. From the state-of-the-art studies and the incident review, technology gaps were identified. Technology gaps are areas where new innovative technologies or applications are required to address current inspection/maintenance strategies for a particular piece of equipment or task. Technology gaps were only identified for pump seals. These gaps included the inability for pump seals to survive process upset conditions, inability to correctly identify and model expected loads and operating conditions for pump seal selection, and lack of installed seal inspection or life prediction methods except through leakage detection. All other incident types (valves out of sequence due to operator error, gaskets and bolting, pipe components, and natural forces) have adequate technology to address the incident occurrences. In the majority of the incidents, even though the technologies exist, it may not be used or applied correctly. Several recommendations were made for future work. These included work that a company may consider conducting internally to reduce the occurrence of incidents and future research. The recommendations for future work for operators and research for industry are summarized in a list below. Research items included on the research roadmaps are indicated with an asterisk.

Incidents due to outside forces have been the leading cause of significant failures on transmission pipelines in the USA since formal record keeping and trend analysis of incidents began in the 1970's. This trend remains currently, with these results reflecting incident experience for 2002. While this situation is not expected to change in the near term, the recent data from 2002 indicate corrosion is overtaking outside force as the main failure cause. The objectives of this report are to evaluate current state of knowledge concerning assessment and management of mechanical damage to pipelines, and identify outstanding issues or gaps to be addressed by the industry. Motivation for such an evaluation and gaps analysis lies in several factors, including much work done has been recently done worldwide. Third-party damage is typically the largest incident category, with the largest consequences, and pending regulatory actions are consequence based, and will require a management strategy to deal with mechanical damage.

This project details the methodologies used to collect and review crack failures, near misses and false positives, and how the available technologies are used in consideration of the CM-SRP pillars of crack (1) susceptibility, (2) inspection, (3) assessment and remediation, and (4) management. This project outcome enhances crack management by providing an independent review and understanding of the research gaps as they relate to cracking in pipelines, based on historic incidents, operator interviews, and subject matter expert opinions. The scope of work consisted of developing future research guideline suggestions through the following tasks: * Task 1 - Collection and review of publicly available reports, * Task 2 - Collection and review of PRCI member incident reports and operator interviews, * Task 3 - Compilation of root causes for historic crack-related pipeline incidents, * Task 4 - Categorization of root causes within the CM-SRP, and * Task 5 - Identification of research gaps in the CM-SRP. A seminal piece in our understanding of how high-profle cases of police violence can affect communities, Desmond, Papachristos, and Kirk (2016), found that resident calls to the police via 911 significantly declined after the beating of Frank Jude. These effects were especially prevalent in primarily Black neighborhoods. In this study, we used an interrupted time series design to replicate the original results in a different city using a fatal incident of police violence. We also extended the methods of original study by further disaggregating the follow-up efects to include officer-initiated events, which capture more discretionary activity for patrol officers. Our results confirm the original findings, with resident calls to 911 declining in majority-Black neighborhoods after a deadly incident of police violence, signifying a decay in community trust and legitimacy. Importantly, we also fnd an immediate and striking decline in officer-initiated activity after the same incident in majority-Black neighborhoods. Conversely, White neighborhoods experienced a slight increase. This study reinforces and adds further context to a growing body of research that explains how incidents of police violence can affect the actions of community residents and the police, including how we conceptualize and measure the concept of “de-policing.”

This factsheet provides key findings and insights from an ammunition profiling case study conducted in Luton and Bedfordshire in the United Kingdom. Mesenteric pseudocyst (MP) is a rare heterogeneous group of intra-abdominal benign cystic lesions with different etiopathogenesis and clinically silent behaviours. These lesions are introduced as one of the entities based on the histological features of thick fibrous cyst walls, barren of the epithelial lining. Often, they present as expanding abdominal masses or are diagnosed incidentally in conventional radiological studies, exploratory laparotomies, or with symptoms of complications such as infection, torsion, or rupture. Surgical removal of the cyst, with or without resection of the affected intestinal segment, is the treatment of choice. Depending upon the size and location of the lesion and related complications, it can be managed by open surgical procedures or laparoscopic approach. Only a handful of 7 cases of traumatic mesenteric cysts have been reported yet in the vicinity of blunt abdominal trauma. We report a rare incidentally detected case of mesenteric pseudocyst (traumatic) in a male of early 20s with a history of blunt abdominal trauma 13 months back and for which serial abdominal exploratory laparotomies were performed. A brief review of the literature is provided, conforming to the rarity of the case. This case highlights the role of histomorphology in diagnosing a benign cystic entity with accuracy, that could be misdiagnosed as infectious granulomatous lesion.
