Full textBackground: Person-centred care (PCC) is when the person seeking care get’s involved in their care and are seen as more then their illness or ailment. PCC as a concept began in dementia care, but is now the approach that most healthcare institutions strive after. To achieve the improved care that PCC can bring, there is need for a well- planned and clear implementation plan from the management. Aim: To describe the nursing staffs experience of implementing person-centred care as a work procedure in inpatient care. Method: An inductive qualitative study where eleven interviews was completed. Eight interviews were strategically chosen and a content analysis was carried out. Result: There is hope among the participants that implementation will bring a better and safer care, but also concerns about fear of change among the healthcare staff and increased workloads. Participants experience that PCC will place higher demans on the healthcare staff, especially nurses, regarding time, knowledge and communication skills. Conclusion: To provide the best care, there has to be dedicated and competent staff who also have the support and understanding from the management. Despite the obstacles that seem to exist, there is a positive spirit among the nurses in relation to the implementation of person-centred care. There is hope that person-centred care, among other things, will increase participation that could lead to shorter hospital stays and risk of needing to seek care again short after being discharged from the hospital. Ultimately it provides a better care in the whole for the individual person seeking care.

Full textBakgrund: Sfinkterskada är en allvarlig komplikation i samband med vaginal förlossning. Önskan om att undvika vaginal förlossning igen ses hos kvinnor som tidigare drabbats av sfinkterskada. Syfte: Syftet med studien var att ta reda på om det fanns riktlinjer gällande sfinkterskada i samband med förlossning vid Sveriges förlossningskliniker samt sammanställa och granska innehållet i riktlinjerna. Metod: Innehållsanalys med en kombination av deduktiv och induktiv ansats. Resultat: Av de 39 kliniker som svarade hade 31 riktlinjer gällande sfinkterskada. I riktlinjerna beskrevs vanligast förekommande riskfaktorer samt prevention med hjälp av perinealskydd. Läkaren ställde diagnos och utförde reparation av sfinkterskadan på operationsavdelningen. Komplikationer som samlagssmärta och anal inkontinens fanns beskrivet i riktlinjerna. Paracetamol och diklofenak var den vanligast förekommande smärtlindringen. Profylax som antibiotikabehandling och laxantia gavs av de flesta förlossningskliniker. Information till den nyförlösta kvinnan beskrevs. Uppföljning hos läkare, barnmorska och sjukgymnast rekommenderades efter fyra veckor till sex månader. Vid kommande graviditet och förlossning föredrogs kejsarsnitt. Slutsats: Riktlinjerna var uppbyggda på samma sätt och hade till stor del liknande innehåll. Författarna till föreliggande fördjupningsarbete efterfrågar en nationell riktlinje.

Full textObstetric care is in constant change. Currently one of them is the rise in frequency of induced labor. Several studies point towards a continuation of that trend. The midwife must have the knowledge and ability to speak to, inform and support women during pregnancy and childbirth also when there is an increased risk for complication, as with induced labour. To be able to give proper support before, during and after induced labor and delivery, the midwife needs knowledge about women’s experiences. The purpose of this study is to describe those experiences using qualitative method with an inductive approach where the data material is women’s written stories. The analysis was produced using qualitative content analysis, as per Elo and Kyngnäs (2007). Eleven women who have given birth using induced labor have shared their experiences. In the material three main categories stand out; Reflecting upon one's delivery in light of the idea of a normal labour and birth, Many emotions are generated by giving birth and The encounter is central for the experience of giving birth. The results show that the women had positive expectations ahead of the induction but also that they felt it was not the real way to give birth and it did not seem as natural as they would have wished. They expressed concerns about the effects the induction might have on their child and a feeling they could not fully trust their own bodies as the labor had been forced upon them. The women did not feel they had received as much information as they would have wished. They emphasized the reception they received from the personnel and that the support from the midwife was important. A relationship built on mutuality and respect could compensate for the discomforts of examinations and interventions.
