Abstract:
Some mothers are unable, or choose not, to breastfeed; bottle-feeding carries certain risks that can be minimized by following simple rules. Formula must satisfy all the nutritional needs of an infant and recommendations for the composition of infant formula have been adopted in the Codex Alimentarius. The two main types of formula differ in protein composition (whey or casein predominant). Follow-on formula are designed for infants from 6–12 months of age. Soy protein is an alternative to cow milk protein, but because of its high phytoestrogen content, should not be used before 6 months. Unmodified cow’s milk as the main drink before 12 months of age is associated with iron deficiency. ‘Complementary feeding’ embraces all solid and liquid feeds other than breast milk and infant formula. There is considerable international variation in practice with regard to introduction of complementary feeds, but in general this should not be before 17 weeks, and not after 26 weeks.

Abstract:
Food refusal is common in early life for a variety of reasons that include parental anxiety and forced feeding. From about 6 weeks of age, infants regulate their energy intake according to energy needs. Newborns prefer sweet-tasting feed and at 3–5 months readily accept new tastes. At 12–18 months fear of new foods (neophobia) develops. Self-feeding by the end of the first year should be encouraged and the child allowed to be messy and enjoy meal times. Repeated exposure to a food is the best way of it becoming accepted. Faddy/picky eating is most likely to occur around 18 months of age. There are some simple rules for modifying eating behaviour, as well as increasing energy intake when appropriate.

Breastfeeding provides almost all of the nutrient requirements for the growing infant to the age of 6 months. Complementary foods rich in iron should be introduced by 6 months, but continued breastfeeding past this point is still beneficial to the infant. It is also beneficial for the mother, reducing postpartum weight retention and lowering the risk of breast and other gynaecological cancers. Breastfeeding women have increased energy requirements to meet the energy costs of milk production. Some specific nutrient requirements also increase during lactation. Excess alcohol consumption, smoking, illicit drugs, and acute exposures to known toxic substances should be avoided. Some prescription and over-the-counter drugs are suitable for use during lactation, including some pain medications, anti-infectives, anti-hypertensives, and drugs to treat postpartum depression. These should be taken under the advice of a physician.

Abstract:
Spinal muscular atrophy (SMA) is a common, inherited, pediatric motor neuron disorder caused by insufficient SMN protein. As of yet, there is no good treatment for the disease. SMA has an incidence of ~1 in 10,000 newborns carrier frequency of 1 in 50, making it the most common inherited cause of infant mortality. Patients with severe SMA, or Werdnig-Hoffman disease, typically manifest weakness during the first 6 months of life. Such patients are so debilitated that they never sit independently, frequently succumbing to the disease before age 2 years. A much milder form of SMA, Kugelberg-Welander disease, with onset after 18 months of age, often during childhood and characterized by prolonged ambulation and a normal life expectancy, was described in 1956. In 1995 mutations in a novel gene, Survival of Motor Neuron 1 (SMN1), were determined to be the specific cause of SMA.
