Abstract:

Recent advances in our understanding of HIV infection in patients with the acquired immunodeficiency syndrome (AIDS) are leading us to explore new treatment stragegies, including the use of combination antiretroviral therapy. In this review, we present information from recently completed clinical trials explore the use of combination therapy, including ACTG 175, the Delta studies, and the NUCA studies. In addition, we present preliminary about use of protease inhibitors, the newest class of antiretrovirals.

Abstract:

Introducción: en 2013, ONUSIDA reportó más de 62 millones de personas infectadas por el VIH desde el inicio de la pandemia; de estos, tres millones se infectaron durante el último año. El impacto del VIH/SIDA a nivel individual ha sido bien documentado, mas los efectos de la pandemia a nivel macroeconómico mundial han sido menos estudiados. Objetivo: determinar el impacto a nivel macroeconómico del VIH/SIDA. Materiales y métodos: estudio hermenéutico sobre fuentes secundarias de información que mediante la triangulación de esta con la teoría económica, se proyectaron posibles consecuencias en los países más afectados por la pandemia. Resultados: la pandemia del VIH/SIDA impacta la oferta de trabajo, mediante el aumento de la morbilidad y mortalidad. El SIDA reduce la productividad laboral, creando así crisis fiscales. El gasto público relacionado con el VIH/SIDA aumentará dentro del rango de 0,2% y 3,5% del presupuesto del gobierno. La baja productividad del sector primario nacional aumenta la importación de alimentos. En Sudáfrica para el año 2010 el PIB sería un 17% menor a un escenario sin VIH/SIDA. Conclusiones: se estima que el VIH/SIDA aumentará los costos de mantenimiento de la fuerza de trabajo, alterará la balanza comercial, induciendo déficit fiscal, así como una caída de la productividad cercana al 75% y del crecimiento económico de 5% del PIB en los próximos diez años. Aunque se realizó una búsqueda a nivel mundial, los resultados obtenidos se concentran en África.

Abstract:

Objective: The aim of this study is to describe the epidemiological and clinical characteristics of the women infected with the Human Inmunodefficiency Virus (HIV) who were seen at the Hospital Nacional Cayetano Heredia from January 1989 to December 1996. Material and Methods: We did a retrospective chart review of their demographic and medical registers. We used the Centres for Disease Control (CDC) classification for the HIV infection. (Stages I-IV). Results: We included 236 women, from 17 to 60 years old, with a mean age of 30.4 years. The number of diagnosed cases per year of infected women increased from 5 in 1989 to 63 in 1996. The probable means of acquisition of infection was heterosexual intercourse in 212 patients (90.2%). Only had one sexual partner in her life 113 (47.8%) of the patients seen. There were 4 (1.6%) sexual workers and there were no intravenous drug users. A percentage of 23.4% have had other Sexually Transmitted Diseases (STD), and these were more prevalent in women who had risky sexual practices (p=0.005). Out of the 236, 69 women had diagnosis of Acquired Inmunodefficiency Syndrome (AIDS) The most frequent AIDS-defining condition was the Wasting Syndrome in 23 women (33.3%). There were 19 deaths within the study period. The mean survival time after the AIDS diagnosis was 10.6 months and the most frequent death cause was Tuberculosis and Pneumocystis carinii Pneumonia. Conclusion: The fact of having a predominantly heterosexual transmission and that there is a high percentage of monogamous women and women with few sexual partners make us suppose that it is probably their partners risk sexual behavior what exposes women to HIV infection. We must underline the importance of an adequate information to women who start their sexual life and of the risks these bring, and the diffusion of the use of condoms to prevent this infection.

Abstract:

<strong>Introduction:</strong> Human Immunodeficiency Virus infected patients have an increased risk for developing different types of cancer. After the introduction of highly active antiretroviral therapy (HAART), and consequent increased survival, a shift has been seen in the spectrum and evolution of HIV infection related diseases, particularly oncological ones, with a tendency to increase non-AIDS-defining malignancies (NADM) as opposed to AIDS defining malignancies.<br /><strong>Material and Methods:</strong> Characterization of the Human Immunodeficiency Virus infected patients with a non-AIDS defining malignancy diagnosis, followed over 16 years at an outpatient clinic in Lisbon through the review of medical records and retrospective evaluation of demographic, epidemiological, clinical and laboratorial parameters, treatment and mortality.<br /><strong>Results:</strong> Of the 1042 patients evaluated, there were 34 Non- AIDS defining malignancy cases identified, mostly in men (78%), with a median age of 55 years. The most common cancers were: lung (20.6%), bladder (17.6%), prostate (8.8%), and anal carcinoma (5.9%). The mean time between Human Immunodeficiency Virus infection and non-AIDS-defining malignancy diagnosis was 6.8 ± 4 years. At the time of non-AIDS- defining malignancy diagnosis the majority of patients (78.8%) was receiving HAART for a mean period of 5.7 ± 3 years, most of whom were immune and virologically controlled (64%). There were 45.5% deaths, mainly in patients with lung cancer (20%).<br /><strong>Conclusion:</strong> Given the risk of developing a non-AIDS-defining malignancy in Human Immunodeficiency Virus-infected patients, it is essential to continue to invest in prevention strategies, promote smoking cessation as well as vaccination programs, as well as applying screening protocols adjusted to this population.

Abstract:

A partir de una experiencia personal (la muerte de un ser querido relacionada con el sida) y de un corpus de producciones artísticas que abordan la experiencia de vivir con VIH (de "vivir con virus", como marca un texto de Marta Dillon), en mi exposición plantearé una pregunta básica: ¿cómo pensamos hoy la vida? ¿Cuál es nuestra relación con lo vivo, con la bios? Esta exposición tendrá la forma de una galería de imágenes, una especie de álbum familiar extendido. El comentario de dichas imágenes (que incluyen los fotogramas de un documental, el registro de distintas piezas artísiticas, algunas fotografías de mi propio álbum familiar o los retratos de Michel Foucault, Luc Montagnier o Donna Haraway) articula y produce el discurso con el que me aproximaré a las respuestas que puede suscitar la pregunta de inicio: ¿cómo entendemos la vida hoy? ¿Qué efectos producen nuestras concepciones de lo vivo? ¿Qué formas de vida resultan o se ajustan a ellas? Más allá de la inercia que nos lleva a considerar al otro como a un extraño, un peligro, un objeto de consumo o un competidor, en mi presentación apuesto por nuestra capacidad para elegir una relación de riesgo, de confianza y de amor hacia las otras. Para ello, en cualquier caso, tendremos que pensar en los dispositivos (biopolíticos, biomédicos, securitarios) que dificultan o inhiben esta elección.

Abstract:

L’histiocytose langerhansienne est une maladie rare causée par la prolifération et l’infiltration d’un ou plusieurs organes, par des cellules dendritiques de type Langerhans. Elle s’exprime par des manifestations cliniques extrêmement polymorphes et peut toucher l’os, la peau, l’hypophyse, les poumons, le système nerveux central, et plus rarement le foie et le système digestif. Elle a été initialement décrite chez les enfants. L’histiocytose langerhansienne de l’adulte présente une entité particulière tant par ses manifestations cliniques que par sa prise en charge. Le cas présenté est celui d’un patient âgé de 53ans, en bon état de santé apparent et non fumeur, qui s’est présenté à la consultation pour des lésions nécrotiques et douloureuses des muqueuses gingivales mandibulaires et maxillaires, associées à des mobilités dentaires sévères. L’examen exobuccal ne révélait aucune asymétrie faciale ni adénopathies. L’examen endobuccal confirmait la présence de lésions nécrotiques gingivales et une parodontite sévère au niveau mandibulaire antérieur et maxillaire postérieur. Le secteur maxillaire antérieur présentait un parodonte sain. L’examen radiologique panoramique et Cone Beam CT , révélaient des lyses osseuses moyennes à terminales s’étendant de la 44 à la 38 et au niveau des molaires maxillaires droites et gauches. Les dents antérieures ne présentaient quant à elles pas de lyse. Par ailleurs des images lacunaires à l’emporte pièce siégeaient au niveau du secteur mandibulaire édenté. Ces manifestations évoquaient un lymphome ou une manifestation orale d’une infection virale type VIH. L’examen biologique révélait une légère hyperleucocytose et une augmentation de la vitesse de sédimentation à la première heure. L’examen anatomopathologique des lésions muqueuses, a rapporté la présence d’éléments histiocytaires se regroupant en nodules, concluant en une histiocytose langerhasienne de l’adulte. Le bilan d’extension ne révélait aucune atteinte associée, concluant en une histiocytose localisée. L’étude moléculaire a montré la présence d’une mutation V600E du gène BRAF (Facchetti et al, European journal of pathology, 471(4); 2017). Ce dernier est situé sur le chromosome 7, et il est impliqué dans l’envoi des signaux qui déterminent la croissance cellulaire. L’évolution et le pronostic de cette maladie sont étroitement liés à l’âge et aux organes atteints. Les régressions spontanées ont été rapportées, et peuvent être induites par un curetage ou par une simple biopsie. (Goncalves et al, Biomedical research notes,9(19); 2016) Pour notre cas, les extractions des dents mobiles avec curetage des lésions ont permis une cicatrisation complète. La thérapeutique médicamenteuse peut comprendre une corticothérapie locale ou systémique, des bisphosphonates, voire même une radiothérapie. Par ailleurs la confirmation de la mutation BRAF permet d’oser un traitement spécifique par inhibition de l’enzyme produite par ce gène.
