The book contains a detailed description of various laboratory tests used for the etiological diagnosis of diseases caused by viruses, bacteria, fungi, protozoa. The advantages and disadvantages of direct and indirect methods for determining more than 50 pathogens are noted, their diagnostic sensitivity and specificity are compared. Information about the choice of the optimal type of specimens and the time of its collection for testing will undoubtedly help the clinician to choose an effective algorithm for laboratory diagnosis of a certain infection and will avoid unnecessary expenditures of staff time and material resources for performing uninformative studies. Along with information about modern trends in laboratory diagnosis of infections, this book provides brief information on the main types of current laboratory research. Knowledge about the physiological properties of analytes used in clinical practice and the diagnostic significance of their changes is extremely necessary for detection of the manifestations of infectious diseases, their complications and treatment monitoring. Due to the completeness and diversity of the material presented, the wide range of studies involved, the book will be interesting and useful to a wide range of readers: infectious disease specialists and clinicians of other specialties, including specialists in clinical diagnostic laboratories and laboratories of hygiene and epidemiology centers, as well as to undergraduate and graduate students – biologists and doctors.

Abstract:
At present, there is the highest number of displaced persons since World War II. Add to that the approximately one billion people living in so-designated fragile states, as well as multiple large-scale humanitarian crises. These vulnerable populations are subject direct and indirect health effects. Unfortunately, delivering effective interventions to them is fraught with difficulty, and the evidence base is weak. The chapter examines the conditions that give rise to acute and prevalent health risks among vulnerable populations, and the present challenges to research. These issues are particularly pertinent to inform our understanding of the ecology of infectious disease. Research contributions are also essential to address infectious diseases and to weigh the relative benefits and risks of different control options. Effective research in crises provides critical information for our understanding of infections among the most vulnerable.

Infectious diseases is a large topic; however, the Royal College of Emergency Medicine (RCEM) curriculum focuses on the key areas which this chapter covers. One of the most important areas in emergency medicine is sepsis and its early recognition. The management of sepsis is currently changing in line with current research. This chapter provides an up-to-date overview of the diagnosis and management of sepsis, with particular respect to early goal-directed therapy and the Surviving Sepsis Campaign, knowledge of which is required for the Intermediate Fellowship of the Royal College of Emergency Medicine short-answer question (FRCEM SAQ) paper. In addition, this chapter also covers the pathophysiology and management of fever, as well as neutropenic sepsis and central nervous system infections. This chapter also covers the public health aspect of infections, as well as the management of needlestick injuries.
