Full textBackground : Nursing workload assessment tools are widely used to determine nurse staffing requirements in the neonatal intensive care unit (NICU). However, several tools exist and it is unclear if they are equivalent. Objective : We aimed to compare three existing workload assessment tools and assess their association with mortality or morbidity among very preterm infants. Methods : Single-center retrospective cohort study of infants born <33 weeks and admitted to a 52-bed level 3 NICU in 2017 to 2018. Required nurse staffing was estimated for each shift using the Winnipeg Assessment of Neonatal Nursing Needs Tool (WANNNT) used as reference tool, the Quebec Provincial NICU Nursing Ratio (QPNNR), and the Canadian NICU Resource Utilization (CNRU). We evaluated correlation between tools using Pearson R. The association between NICU nursing provision ratio (actual number of nurses / recommended number of nurses per shift according to the tools used) during the first 24h, 7 days of hospitalization and whole hospital stay with mortality / morbidity was assessed using logistic regression models.Results : Median number of nurses required per shift using the WANNNT was 25.0 (interquartile range [IQR]:23.1–26.7). Correlation between WANNNT and QPNNR was high (r = 0.92, p < 0.0001), but the QPNNR underestimated the number of nurses per shift by 4.8 (IQR: 4.1–5.4). Correlation between WANNNT and CNRUwas moderate (r = 0.45, p < 0.0001). Nursing provision ratio during the first seven days of admission calculated using both WANNNT and QPNNR was associated with mortality/morbidity (adjusted odds ratio [aOR] (95% CI):0.92 (0.86-0.99); 0.94 (0.89-0.98), respectively). The association between nursing provision ratios for the first 24h and whole hospital stay and mortality/morbidity was not statistically significant. Conclusion : Lower nursing provision ratio during the first seven days of admission is associated with anincreased risk of mortality / morbidity in very preterm infants.

Full textNurse shortage in the province of Quebec is a growing concern for stakeholders and policy makers in the health care and social services domain. Remote areas are particularly affected by this workforce imbalance because of specific challenges they face relatively to their geographical isolation and recruitment problems. Newly graduates from nursing programs in Quebec universities’ represent a recruitment pool of choice for recruiting nurses that are willing to go rural. The main objective of this project is to explore factors that influence final-year nursing students’ intention to choose a rural area as first employment location. A secondary objective of this study is to explore the links between the intention to work in remote area and the availability of information and communications technology (ICT) in these areas. To meet our objectives, a transversal observational study was conducted. A survey based on the theory of planned behavior to which we add the two fallowing constructs: personal identity and telehealth was build and validated for data collection. Different analyses were performed according to the respondent’s background (rural or urban) and this survey allowed the identification of factors linked to the intention to settle in rural area after graduation. For rural background respondents, personal identity, perceived behavioral control and social norm were identified as determinants underlying the intention to settle down in a rural setting. For urban background respondents, personal identity, perceived behavioral control and affective attitude were significantly associated with their intention to settle down in a rural setting. This project contributes to a greater understanding of the elements that could influence the choice of a place of practice for newly graduated nurses.

Full textInformation and communication technologies are increasingly seen as potential solutions to major challenges of healthcare. As such, an Electronic Health Record (EHR) is being developed in the Province of Quebec and it is considered the key to the integration of various tools that will enable a safer and more efficient healthcare system for every Quebec citizen. However, the expected benefits can only materialise if healthcare professionals adopt this new technology. This study looked at the factors that influence nurses’ intention to adopt the EHR. A questionnaire was distributed to 199 primary care nurses. Multiple hierarchical regression indicates that the variables explained 58% of the variance in nurses’ intention to adopt an EHR. These findings suggest that in order to improve nurses’ intention to adopt EHR, interventions should target the attitudinal belief that using an EHR will improve the quality of care and should also focus on the perceived behavioral beliefs of nurses to use the EHR.

Full textIn Quebec, the aging of the population is leading to a significant increase in nursing needs, particularly in the community. Indeed, seniors living in the community and whose health status is fragile regularly see their care needs change, particularly because they spend many hours in hospital, which contributes to their functional decline. This is particularly true in the context of intermediate resources, where people living in them already deal with disabilities and complex health problems. Failure to provide the necessary nursing care can lead to a rapid deterioration in the senior's level of autonomy and health status, which compromises community maintenance and leads to an increase in the use of institutionalized long-term care housing. It is therefore imperative to facilitate, through the provision of appropriate nursing care, the delicate transition period experienced by seniors following hospitalization. Indeed, the presence of adapted transitional care significantly reduces hospital readmissions and emergency room visits, in addition to having a positive effect on the overall health status of seniors and their quality of life. The objectives of this research project were therefore to develop a nursing intervention protocol to facilitate the transition of seniors returning to intermediate resources after a hospital stay in Quebec, and to assess the feasibility and acceptability of the latter as part of a pilot project in a clinical setting. The definition of nursing care recommended for this research project is that proposed by the McGill model. This definition underlies the nurse's intervention, and involves mobilizing the strengths, resources and motivation of seniors and their families to improve their health or recovery through their ability to cope and develop, here in the context of the post-hospital transition. On the other hand, the theory of self-determination (Deci & Ryan, 1985; Ryan & Deci, 2017f), the mini-theory of basic psychological needs (Ryan & Deci, 2017a), the middle-range theory of self-care of chronic illness (Riegel, Jaarsma, & Strömberg, 2012) and the theory of uncertainty (Mishel, 1988, 1990) also provide a very significant theoretical perspective in this project. The intervention research method used is that of Sidani and Braden (2011). This method involves both a deductive approach, through the use of middle-range theories and the consultation of empirical data, and an inductive approach, through the collection of experiential data. In this three-phase project, a descriptive qualitative approach was used for the experiential data collected from eleven (n=11) senior participants and health professionals. A triangulation of experiential, empirical and theoretical data then provided an in-depth understanding of the transition experienced by seniors returning to intermediate resources following hospitalization, and a list of unmet care needs (Phase 1, Step 1). The experiential data collected were again used to identify possible interventions and then triangulated with theoretical and empirical data to design the nursing interventions needed to adequately address the identified needs (Phase 1, Step 2) and then theorize these interventions (Phase 1, Step 3). Phase 2 of the project then consisted of operationalizing the results obtained in Phase 1 into a written intervention protocol. Finally, in Phase 3, a pilot project was conducted with three seniors (n=3). These seniors were provided with the intervention protocol, and its acceptability and feasibility were then studied, as well as its immediate effects. The results obtained suggest the acceptability and feasibility of the protocol developed and illustrate positive effects related to supporting people's autonomy, improving their self-care capacities and reducing the uncertainty experienced during the transition. Finally, in addition to revealing precisely how nurses can intervene to facilitate this transition, explaining the mechanisms underlying the effectiveness of proposed nursing interventions helps to illustrate why these interventions are beneficial to individuals and how they produce their effects, which is not currently available in the literature on nursing transitional care models.

Full textEutrophication and contaminated sediments (e.g.: nutrients, metals, etc.) are preoccupying issues in aquatic ecosystems and water resource protection. Saint-Augustin Lake (Quebec, Canada) is affected by both problems in addition to the presence of de-icing salts. This small shallow lake has low renewal time and receives an intensive phosphorus transfer from bottom sediments to the water column. Since 2001, the lake presents cyanobacteria blooms every summer (P > 70 μg/L). Galvez and collaborators has proposed a remediation method for this lake that consists of (i) the precipitation of phosphorus present in the water column by the addition of alum, in association with the deposition of (ii) an active capping layer (limestone + sand) to isolated P, Al and other contaminants (e.g., metals) within bottom sediments and allow recolonisation by benthos. The objective of this study is to assess the ecotoxicological impact of that remediation method. Two ecotoxicological approaches are used to assess the impact of this method: (I) single-species bioassays (algae Pseudokirchneriella subcapitata and duckweed Lemna minor growth; daphnid Daphnia magna and amphipod Hyalella azteca survival and reproduction, invertebrate Chironomes riparius survival, growth and emergence, fathead minnow P. promelas and rainbow trouts O. mykiss survival) and (II) a 2-L microcosm as adapted by Triffault-Bouchet. Alum toxicity and limestone impact were studied separately in laboratory: first with artificial water and sediments and then using actual lake overlying water and sediments. The results show that algae and daphnids are sensitive to alum treatment. No significant effects were observed on the other organisms. These organisms had the ability to recolonize the treated media either with alum treatment or with complete treatment. These results suggest that the restoration method is promising. The environmental compatibility of the technique, would be demonstrated after testing in situ with a close monitoring of phytoplankton and zooplankton in the lake. However, the results of this study showed that the treatments dephosphatation efficacity was not enought to reach the MDDEP quality criteria (20 µg/L). Also, the concentration of residual aluminum after treatments was still over the MDDEP quality criteria after treatments. Full textThe objective was to evaluate the agronomic potential, namely establishment, winter survival, biomass production, and nutritive value of novel forage species with a high content of condensed tanins or sesquiterpene lactones grown in pure stand or in mixture with two forage grasses, timothy (Phleum pratense L.) and meadow fescue (Festuca pratense Huds.), under pasture management in four climatic regions of Quebec. The field experiment took place in Sainte-Anne-de-Bellevue (Montreal region), Saint-Augustin-de- Desmaures (Capitale-Nationale region), La Pocatière (Bas-Saint-Laurent region), and Normandin (Saguenay-Lac-Saint-Jean region), over two years (2016 and 2017). At all study sites in 2017, chicory (Cichorium intybus L.) and birdsfoot trefoil (Lotus corniculatus L.) had satisfactory yields compared to alfalfa at all sites, while sainfoin (Onobrychis vicifolia Scop.) did not perform well. Sainfoin had the highest content of condensed tanins (average of 10.7 g kg-1 dry matter) and moderately and slowly degradable proteins.

Full textThis study aims to verify the existence of positive impacts in terms of human capital, such as the acquisition of knowledge and entrepreneurship, among Mexican workers enrolled in the Seasonal Agricultural Worker Program in the province of Quebec. To do this, 30 semi-structured interviews were conducted with individuals working on Île d'Orléans and in the municipalities of Saint -Augustin-de-Desmaures, Saint-Apollinaire and L’Assomption. A categorization of the experiences according to the degree of capacity building was carried out considering the state of origin, the number of trips to Canada with the SAWP, the acquisition of knowledge and its application, land ownership, the pursuit of projects and the interest to remain in the SAWP. Categories that have resulted are the survival experience, the supplementary experience, the wished entrepreneurial experience and the established entrepreneurial experience. Investigations have shown that there are benefits in several cases with these seasonal migrations.

Full textAfforesting abandoned agricultural lands (fallow lands) is a method authorized by the Kyoto Protocol to create CO₂ sinks and thus reduce atmospheric CO₂. It turns out that Abitibi-Ouest offers the greatest potential for afforestation of fallow lands in Quebec with its 51 000 hectares of fallow land. However, before starting any afforestation project, it is essential to assess whether afforestation provides real benefits in terms of carbon sequestration and therefore contribute to climate change mitigation. The purpose of this study is therefore to document these benefits. To do this, soil and biomass measurements were carried out from June to September 2019 in the Abitibi-Ouest region in 26 fallow lands with natural plant succession, and 23 afforested fallow lands in white spruce. The first objective was to reconstruct the chronosequence of plant succession on fallow lands left to natural vegetation. The second objective was to compare the carbon accumulation over time in the main pools (vegetation, soil) of natural and afforested fallow lands. It has been observed that over a 50-year horizon, afforested fallow lands stored twice as much carbon in vegetation, while natural fallow lands stored twice as much carbon in the soil. Overall, the total amount of carbon stored (including both vegetation and soil) was similar between afforested fallow lands and those left to natural succession. This study will guide decisionmaking on the use of afforestation of fallow lands in Abitibi and highlights the fact that other management objectives than carbon sequestration should be considered. The results obtained will also fuel reflection on the relevance of afforestation as a tool to fight climate change, whether at the provincial, national or global level.

Full textMany economic and climatic disruptions have influenced the Quebec dairy farms between 2000 and 2010. However, the incidence of these disruptions did not have the same effect on the financial health of the farms, as demonstrated by the disparity shown in their financial results. In order to obtain a better understanding of the presence of heterogeneity, the strategic behavior of the most efficient and the less efficient producers has been evaluated from two series of analysis, performed on eleven management strategies. The results showed that the producers of the head subgroup obtained better security margin, labour organization, dairy productivity and valorization of the fodder than the producers of the tail subgroup, regardless of the economic and climatic situation. It appears then, that their production structure and their strategic planning are more economically efficient and more capable of restraining the negative effects of disruptions.

Full text$8373052\u$aMining in Canada is of undeniable economic and social importance. The mining industry produces tailings thatmay contaminate the soil and water ecosystems, causing long-term land degradation. This study focuses on the ecological restoration of gold mining wastes in the Canadian north, specifically in northern Quebec. The objective of this master thesis is to assess the potential of different implantation systems for the revegetation of a mine degraded land by assessing the survival of plants, the physicochemical parameters, and the microbial communities of the soil after four years. The field trials were established on gold post-mining sites on fine tailings, using a mixture of tree and herbaceous species, the amendment of symbiotic microorganisms, and biochar. This study has provided insight into the complexity of soil-plant-microorganism relationships. The results showed apositive effect of inoculation on the tree survival rate, especially for Alnus viridis ssp. Crispa and Picea glauca and their survival rate of over 90%. Moreover, the characterization of fungi and bacteria in the soil withthe Illumina Miseq technology made it possible to assess the diversity of microorganisms and their interactionin the environment. Also, the presence of bacteria of the genus Pseudomonas and ectomycorrhizal fungi of the genera Amphinema and Cadophora suggest that these beneficial soil microorganisms are well adapted to thisenvironment and can be used as inoculum. Biochar affects soil chemical parameters, especially pH, total carbon,and total nitrogen. These parameters ultimately affected the soil microbial communities. Thus, our results suggest that revegetation of degraded mine sites is possible with a combination of plants, amendments of biochar and, microsymbionts, adapted to edapho-climatic conditions.

Full textDepuis 2007, les infirmières praticiennes spécialisées en soins de première ligne (IPSPL) conjuguent une formation infirmière et médicale afin de mieux répondre aux besoins de santé de la population québécoise. Plusieurs études conduites hors du Québec ont démontré les impacts positifs de la pratique des IPSPL sur la qualité des soins offerts aux personnes atteintes de maladies chroniques. La présente étude qualitative descriptive exploratoire a documenté les représentations d'informateurs clés quant au rôle, aux pratiques et aux défis des IPSPL dans le domaine des maladies chroniques au Québec. Des entrevues individuelles semi-dirigées ont été conduites auprès de 20 informateurs clés. Ces informateurs clés sont des professionnels de la santé et des gestionnaires ayant une expérience significative concernant la formation, le développement et l'implantation du rôle, la gestion administrative et la pratique clinique des IPSPL. Une analyse thématique des entrevues intégralement retranscrites a été réalisée. Les résultats de cette étude révèlent trois principaux aspects de la pratique des IPSPL dans le domaine des maladies chroniques: 1- une large reconnaissance de la valeur ajoutée du rôle et des pratiques des IPSPL dans les milieux de soins de première ligne, 2- des défis parfois persistants associés à la méconnaissance de leur rôle, à la rigidité de la réglementation entourant leur pratique et à la complexité de prise en charge de la multimorbidité, et 3- la rareté, au sein des équipes des soins de première ligne, d'une offre de services planifiée, coordonnée et interprofessionnelle à destination des personnes atteintes de maladies chroniques. Il apparaît ainsi que les IPSPL rencontrent d'importantes barrières dans leur pratique nécessitant entre autres des ajustements législatifs et organisationnels. L'optimisation de leur rôle passerait également par le déploiement systématique dans les équipes de soins de première ligne d'un plan mieux défini de prise en charge de cette clientèle.

Full textInternationally educated nurses (IEN) face a variety of challenges throughout their integration journey into the nursing profession. The participation to an integration program influences their progress. The purpose of the research was to explore the experiences of IEN in their professional transition when implementing an integration program. The transitions theory of Meleis, Sawyer, Im, Messias, & Schumacher (2000) served as a guide for this research. Using a grounded theory approach described by Strauss and Corbin (2004), data were collected from 14 IEN, including 5 in individual interviews and 9 in group interviews. The results show that IEN experiment professional differences. Those related to nursing activities differ according to the IEN working place in the country of origin. In fact, IEN who have worked in hospitals move from the role of technical care executant to a health professional with diverse and autonomous activities. While community-based IEN are moving from diversified, self-sustaining activities to a role that is focused on nursing care. Other professional differences also emerged from the analysis and refer to the clinical context. The elements identified relate to the legislative framework, interpersonal verbal and nonverbal communication, the type of clientele, the health situations experienced by the clientele and the equipment used. Thereafter, personals conditions, conditions related to the integration program and community conditions influence the way the professional transition is lived by the IEN. The emerging theoretical model provides a deeper understanding of the professional transition of IEN and the relationships between each of the concepts during an integration program.

Full textIn northern Quebec and Labrador, the Rivière-George and Rivière-aux-Feuilles caribou herds have been declining since the 1990s and 2000s, respectively. In particular, marked decline of the Rivière-aux-Feuilles caribou herd coinciding with the demographic growth of muskoxen, an introduced species in northern Quebec, has led local Inuit communities to question the potential for interspecific competition. In response to these concerns we used three methods to evaluate the potential for interspecific competition at different scales: (1) we quantified spatial overlap, (2) we compared both species habitat selection patterns in their overlapping range and (3) we evaluated diet overlap. We used telemetric data and collected fecal samples of both species for DNA barcoding analysis. At the annual scale, spatial overlap was low and limited in time, mostly in spring on the coast of Hudson Bay and in summer on the coast of Ungava Bay. Within their overlapping range, migratory caribou and muskoxen had mostly different habitat selection patterns, particularly in regard to habitat types and proximity to the coast. At a finer spatial scale, this led to low probability of co-occurrence across both coastal areas. Our dietary analysis of fecal samples suggested that both species had a similar diet based on the frequency of the most detected plant families (Ericaceae, Salicaceae et Betulaceae) in summer and winter. Combined, our results suggest that the potential for competition between migratory caribou and muskoxen is low in our study area. However, several factors such as the possible expansion of the muskoxen range and interactions with predators, in the context of climate change, could potentially influence the interactions between these two ungulate species

Full textIn order to respond to an increasing demand for re-using bituminous materials, this study, carried out under the Research Chair i3C, has evaluated the effect of recycled asphalt pavement (RAP) on the mechanical behavior of a recycled base materials for five different RAP content (0 , 30, 50, 60 and 70% RAP). The main objectives of this project were to characterize the resilient and permanent deformation of these materials as well as assessing their variability on site and provide innovative solutions for the different issues. Laboratory tests were conducted on the five mixtures and field trials were carried out on three sites in the greater Quebec area. Some aspects were also tested in the road simulator apparatus of Laval University (SimUL). The data collected in the field with the light weight deflectometer (LWD) and the dynamic cone penetrometer (DCP) and also the triaxial cell has documented the characteristics and mechanical behavior of recycled base material. The project showed that materials with a higher RAP content are more likely to accumulate permanent deformation. Using base materials with more than 50% RAP can increase rutting problems. However, in order to overcome this prejudice, a model that relates the rate of deformation (B), the RAP content and the deviatoric stress is proposed. This new approach propose to evaluate the enhancement of the asphalt thickness needed to reduce the sensitivity to rutting of a recycled base layer.

Full textSome of the United Nations Sustainable Development Goals are related to land and natural resources, they are so connected to surveyors profession. However, current knowledge does not allow us to identify whether the professional actions of land surveyors are consistent with these objectives. In this context, we raise the problem of knowing whether land surveyors contribute to sustainable development. In fact, we want to know if the Quebec land surveyors in their daily tasks take actions that contribute to sustainable development. We hypothesize that Quebec land surveyors take little actions that promote sustainable development, specifically the environmental issues defined in the BNQ 21000 standard and this is in part because they don’t know the definition of sustainable development and are not sensitive to it. To answer our questions, we performed a survey among land surveyors of Quebec. According to the OAGQ's 2016-2017 annual report, there are 1059 land surveyors as of March 31, 2017. Almost 20% of them responded to the survey and the majority seems very sensitive to sustainable development. In addition, their knowledge of sustainable development is medium but the environmental actions taken are low. Sensitivity and knowledge are therefore not the factors that justify the low level of the environmental actions.

Full textIn recent years, in Quebec, the nursing profession has continued to change and redefine it self. As a general nurse practicing mainly in hospitals, various extended roles associated with specialized care sectors have been created, such as that of specialized nurse practitioner. The deployment of these roles is linked, among other things, to the increased and complex care/service needs of an aging population living with chronic diseases and to the exploding costs associated with these requests. The emerging role of nurse practitioner is predominantly played by women. This gendered characteristic is certainly not foreign to the multiple constraints experienced by these professionals in environments where entrepreneurial-type governance is increasingly felt. Researchers, theorists and activists have taken an interest in this phenomenon and have notably denounced the fact that care activities are little recognized. The repercussions of the lack of recognition on nurses practicing in specialized care sectors are not unrelated to this phenomenon, but they remain largely unknown in the eyes of the scientific literature. The aim of this descriptive qualitative research is to explore the recognition practices experienced on a daily basis by Quebec nurses working in different sectors of specialized care, more particularly nurse practitioner, and the way in which these experiences have repercussions on the latter and on their conditions of care. The individual and collective actions, actions of a political nature, that these nurses mobilize or would like to mobilize are also investigated with in the framework of this project, and of the larger research project in which it is part, and this, in order to promote taking into account their socio-political demands aimed at obtaining the recognition they are entitled to obtain. Epistemologically, our study falls within the critical theory allowing us to use theoretical reflections offeminist and postmodernist type. At the end of this study, the results indicate that these nurses mainly receive recognition from patients, and this, more particularly according to the time they give them. Lack of awareness of their role, by the population and by the professionals with whom they work, has a direct impact on there cognition they receive on a daily basis. Undue control of their professional practice has the effect of considerably degrading their working conditions. The latter are aware of the importance of acting politically to take more place in the social sphere, but are constrained, at several levels, to the deployment of their political actions.

Full textBased mainly on industrial, specialized and increasingly concentrated agriculture, the food system of the industrialized countries is now highly criticized because of its negative externalities on the environment, land use and employment. In order to reconnect food and agriculture and to reduce the distances traveled by food, many initiatives seek to reduce the number of intermediaries in the food system and bring geographically closer production and consumption. In Canada, large cities have programs that promote local food. We developed a methodology and a model to answer these questions: Would we be able to meet all our needs through local production? If not, in which proportion? From available statistics, we evaluated the potential production of the regions under study (Capitale-Nationale and Chaudière-Appalaches) and then analyzed the adequacy between production and food consumption. Results show that the studied territory could produce up to 155% of the calories needed by its population, but livestock farming pulls the theoretical coverage of food needs down to 38%. The actual coverage rate of local needs by local products varies from less than 5% to more than 400%, depending on the food item. This percentage is unique to the region, but our model could be applied at different scales, and in any region. A field survey allowed us to seek the representation of agri-food stakeholders in the local food issue. All stakeholders are in favor of relocating food. On the other hand, they all believe that the price of local food is higher, which does not play in favor of local food. Full textLa thèse a deux objectifs. Elle vise, dans un premier temps, à améliorer les connaissances sur l’expérience de la grossesse au travail. Dans un deuxième temps, la thèse propose un bagage conceptuel permettant d’opérationnaliser l’analyse des rapports sociaux de sexe de manière à favoriser le renouvellement théorique en relations industrielles. La recherche actuelle en relations industrielles interroge peu les raisons sous-jacentes aux difficultés pour le marché du travail de tenir compte des aspects « dits » plus privés de la vie des femmes, comme la grossesse. Ces lacunes posent la nécessité d’intégrer le regard théorique féministe et d'incorporer une analyse tenant compte des rapports sociaux de sexe dans l'analyse en relations industrielles. Pour faciliter l’opérationnalisation du cadre théorique retenu, nous intégrons les concepts de la démocratie, du pouvoir et des ressources tels que définis par la théorie de la structuration. Nos résultats qui s’appuient sur le récit d’expérience de 28 infirmières ayant bénéficié d’un retrait préventif pour 10 centres hospitaliers québécois ainsi que sur le récit des 10 gestionnaires et des 9 représentantes syndicales qui sont impliquées dans la gestion du retrait préventif pour ces mêmes hôpitaux ont permis, sous la loupe des cadres théoriques et opérationnels retenus, de rendre visibles les manifestations des rapports sociaux de sexe dans les lieux de travail. Ainsi, les travailleuses enceintes, leurs représentantes syndicales et les gestionnaires (influencées par les représentations de la grossesse au travail et les règles (pratiques) entourant la gestion du retrait préventif en organisation) agissent de façon paradoxale à l’égard de la conciliation travail/grossesse. Elles peuvent agir sur les représentations et les règles et ainsi abaisser les effets des rapports sociaux de sexe en facilitant le maintien en emploi des travailleuses enceintes dans des conditions respectant leurs compétences, leur santé et celle de leurs enfants à naître. Au contraire, leurs actions peuvent contribuer à maintenir, ou parfois même, réaffirmer les structures de domination lorsqu’elles ne favorisent pas la mise en place de conditions favorables pour la conciliation travail/grossesse et qu’elles conduisent les travailleuses enceintes à se retirer du travail.
Analysis of gender relations as a way of renewal of the theoretical and practical industrial relations field. Study on the impact of the access to preventive withdrawal for pregnant workers in Quebec hospitals. The thesis has two objectives. First, improve knowledge about pregnancy at work. Second, propose conceptual knowledge allowing for the operationalization of the analysis of gender relations in such a way as to foster the theoretical renewal of industrial relations. Current research in industrial relations fails to thoroughly examine the underlying reasons for the labour market’s difficulty in taking into account aspects that are “said” to be more private about women’s lives, such as pregnancy. These gaps emphasize that the feminist theoretical perspective should be considered and an analysis that takes into account gender relations incorporated in the analysis of industrial relations. To facilitate the operationalization of the theoretical framework selected, we integrate the concepts of democracy, power and resources as defined by the structuring theory. Our results, which are based on the account of the experience of 28 nurses who benefitted from preventive withdrawal among 10 Quebec hospitals and the account of 10 managers and 9 union representatives who are involved in the management of preventive withdrawal for these same hospitals, helped, through the theoretical and operational frameworks selected, to identify demonstrations of gender relations in the workplace. Thus, pregnant workers, their union representatives and the managers (influenced by the representations of pregnancy at work and the rules (practices) regarding the management of preventive withdrawal within an organization) act paradoxically toward the work-pregnancy balance. They can act on the representations and rules and therefore reduce the effects of gender relations by facilitating the continued employment of pregnant workers in a manner that respects their skills and their health and that of their unborn children. On the contrary, their actions can help maintain, or even sometimes, reinforce the structures of domination when they fail to foster the implementation of conditions conducive to the work-pregnancy balance and lead pregnant workers to withdraw from work.

Full textL’Entente multisectorielle est un mécanisme de concertation qui vise à garantir une meilleure protection et à apporter l’aide nécessaire aux enfants victimes d’abus sexuels, de mauvais traitements physiques ou d’une absence de soins menaçant leur santé physique. Son application au Québec relève principalement des Centres jeunesse, des instances policières et des procureurs. Quoique cette entente soit en vigueur depuis plus de dix ans, on ignore toujours si elle répond à son objectif initial. Cette recherche questionne les capacités d’une telle Entente de parvenir à ses fins de protection, pour les enfants et leurs familles. Elle entend démontrer qu’à certains égards, son application peut au contraire donner lieu à des risques de victimisation secondaire. Une démarche quantitative à visée exploratoire et descriptive a permis de dresser un portrait des situations récentes d’abus physiques et sexuels soumises à l’Entente multisectorielle (N= 222) au Québec, et d’identifier quelles situations ont pu mener à des accusations criminelles. Les résultats ont démontré que seulement une situation sur cinq conduisait à des accusations, les autres cas donnant lieu à un abandon des procédures ou à un manque de suivi, le tout engendrant potentiellement des conséquences néfastes pour la clientèle. Ces résultats soulignent l’importance de reconsidérer les besoins des enfants exposés à la trajectoire de criminalisation.
The Multi-sectoral Agreement is a consultation mechanism developed for the purpose of ensuring better protection and providing necessary help to children victims of sexual abuse and physical ill-treatment, or whose physical health is threatened by a lack of appropriate care. In Quebec, its implementation depends primarily on youth centres, prosecutors and the police. Although this Agreement has been in effect for more than ten years, it remains unclear as to whether it accomplishes its intended purpose. This study examines the capacity of such an Agreement to fulfill its protective purpose, for children and their family. The study also intends to show that in certain cases, the implementation of the Agreement can lead to risks of secondary victimization. A descriptive and exploratory quantitative approach served to develop a profile of recent sexual and physical abuse situations addressed by the Multi-sectoral Agreement (N= 222) in Quebec, and to identify the situations that led to criminal charges. The results show that only one fifth of the situations led to charges. The other cases led to the abandonment of litigation or a lack of follow-up, potentially resulting in adverse consequences for clients. These results highlight the importance of reconsidering the needs of children exposed to criminal proceedings.

Full textThe popularity of selective special school projects prejudices equality of opportunity in Quebecer secondary schools. This study focuses on a non-selective special school project that offers both homework supervision and guidance for all and the possibility of participating in extracurricular activities for every pupil who wishes to do so. The study aims to describe the difference in pupils’ perception of the efficiency of the project on homework guidance and supervision according to their characteristics (gender, secondary cycle and type of education) and their work habits (time spent daily on homework and where homework are done). This study uses secondary data from a previous collaborative study which had shown students to have a positive perception of the project’s efficiency (Larouche, Amboulé-Abath et Savard, 2016a). Data interrogated stems from a questionnaire answered by 596 secondary school pupils from special needs education and regular classes. Results show many differences in pupils’ perception of the efficiency of the project according mainly on their work habits, but also on gender, secondary cycle, and type of education. Pupils who have a most positive perception of the efficiency of the project are those who do their homework both at home and at school and those who spend at least 30 minutes daily on their homework. Data also show that girls have a more positive perception of the efficiency of the project than boys. The latter tend to perceive more the disadvantages of the project. Pupils form cycle one (junior) and those from cycle two (senior) have a different perception of the project’s efficiency. Pupils enrolled in special needs classes have a more negative perception of the efficiency of the project than their peers.

Full textMedication reconciliation (MedRec) is a formal process. While doctors’ and pharmacists’ contribution is clearly spelled out, nurses’ involvement remains to be clarified. Goal: Describe nurses’ involvement in implementing MedRec among cardiorespiratory patients admitted to a hospital centre’s cardiology department. Framework: Meleis theory, Reason model. Method: Single case study. Three data sources: interviews (N = 6) (nurses, doctors and pharmacists); a qualitative review of patient files (N = 22), and documentation (N =15). Results: While nurses are indeed expected to manage forms in the various phases of transition of care, their involvement extends well beyond this task and falls into four main categories: distinct involvement; nuanced mobilization; safety net, and; contributive activities. Results show that the accompaniment of patients and their loved ones is learned through teaching, screening high-risk patients and collaborating in a way that supports the mobilization of the other professionals involved to ensure patient safety during transitions.

Full textProblem: Assistant head nurses have a primary role. However this role is not very popular and has received little attention from researchers. Questions: What are the easy and difficult activities and skills? How to live the work entry of the AIC of a Quebec hospital? Method: A qualitative descriptive type of approach using semi-structured interview was preferred. Our sample is formed of six AIC and three CUSI. Results: The results show that the activities related to clinical work and communication emerge as the most easy and activities related to the management seem more difficult. The identified skills are: leadership, technical and clinical skills, interpersonal relationship, planning and organization, ability to delegate and critical thinking. Their work entry is not made without clash. Conclusion: An image related to the recognition comes from the research of "HEROES" in the health system.

Full textPeople suffering from Alzheimer’s disease suffer from cognitive declines which deteriorate their general and oral health. We frequently observe dental cavities, periodontal disease and tooth loss among these people. In that respect, we wanted to establish a health and oral evolutionary curve throughout Alzheimer’s disease. To that end, we conducted a prospective observational study that includes a transverse and longitudinal component. The participants had to have Alzheimer’s disease at different stages, have teeth and live in the province of Quebec. Combative, bedridden people, and those who were diagnosed with bacterial endocarditis or who had heart valve prostheses were excluded from this research. The data corresponding to the variable responses (number of visible and missing teeth, obturations, cavities, retained dental roots and loose teeth, plaque index and gingival bleeding) and explanatory variables (age, gender, education level, resulting score to the Folstein Mini-Mental State Examination, co-morbid conditions, quantity of saliva and the use of dental care) have been provided through a medical examination form. We recruited participants (n = 46) from six Quebec health facilities between May 2016 and October 2017. This study showed that as Alzheimer’s disease progresses, dental plaque (p = 0,05) and gingival bleeding (p = 0,01) increase and the number of teeth (p < 0,0001) and fillings (p = 0,04) decreases. Also, the number of cavities, retained dental roots and loose teeth, seem to increase with the progression of the disease. The probability of having a cavity drastically increases as the cognitive decline progresses (p = 0,023). In conclusion, oral health deteriorates with the development of Alzheimer’s disease, and this from the moderate stage of the disease.
