Canada is in the midst of a staggered implementation of the International Statistical Classification of Diseases and Related Health Problems, Tenth Revision, Canada (ICD-10-CA) and the Canadian Classification of Health Interventions (CCI). These classifications are more comprehensive than historical standards and their capacity extends beyond the scope of their predecessor classifications. Canada is the first country to produce the new classifications in a database, with the final product in an infobase format. The Canadian Institute for Health Information (CIHI) was responsible for the enhancement of ICD-10-CA, the development of CCI, the education of coders, and the provision of post-implementation support.

The disposal and recycling of chlorohydrocarbons waste from organic synthesis which is annually produced at least 1,5 million tons is one of the global problems and is under the great UN's attention. Organochlorine residues are xenobiotics and adversely affect the environment. Thermodynamic modeling of interactions in the systems ZnO-CHxCly (x + y= 4)-O2 for determination of the theoretical content of chlorohydrocarbons as chlorinating agents for extracting nonferrous metals from very concentrated ore is presented in this article. The studies were conducted using the "Astra" software based on the principle of maximum entropy. Infobase of the "Astra" complex contains information on 5547 compounds, 79 items codified in the National Bureau of Standards and the Joint Institute for High Temperatures, Russian Academy of Sciences.

Los pacientes con trastorno mental grave constituyen un reto en su atención, por las necesidades a nivel social, psiquiátrico, laboral, orgánico y, fundamentalmente, humano. El objetivo de este trabajo es analizar la bibliografía disponible acerca de las intervenciones recomendadas desde diferentes perspectivas: enfermería, psiquiatría, atención primaria e institucional. Se recoge información a partir de bases de datos (Pubmed, NHSEvidence, Cochrane, Tripdatabase, UpToDate, National Guideline Clearinghouse, Guidelines International Network, CMA infobase), entre 2000 y 2016. Se emplean los descriptores “schizophrenia”, “bipolar disorder”, “older people”, “aripiprazole”, “paliperidone”, “elderly”, “severe mental disorder”, “primary care”, “nurs*” y sus equivalentes en español. La evidencia disponible tanto a nivel de los programas de atención como la medicación en este grupo de pacientes es baja. Los estudios encontrados analizan programas con escaso número de participantes y en muchos casos sin comparativos ni grupos control, incluyendo recomendaciones en base a subanálisis de estudios en población general y extrapolaciones en función de las necesidades de este grupo. A pesar de la escasa evidencia disponible y la necesidad de desarrollar estudios de mayor calidad, podemos establecer la recomendación de integrar abordajes de diferentes perspectivas en la atención y el cuidado de estos pacientes.

SummaryUsing recent pooled data from the World Health Organization Global Infobase and the World Factbook compiled by the Central Intelligence Agency of the United States, this study assesses the relation between income inequality and obesity prevalence among 31 OECD countries through a series of bivariate and multivariate linear regressions. The United States and Mexico well lead OECD countries in both obesity prevalence and income inequality. A sensitivity analysis suggests that the inclusion or exclusion of these two extreme cases can fundamentally change the findings. When the two countries are included, the results reveal a positive correlation between income inequality and obesity prevalence. This correlation is more salient among females than among males. Income inequality alone is associated with 16% and 35% of the variations in male and female obesity rates, respectively, across OECD countries in 2010. Higher levels of income inequality in the 2005–2010 period were associated with a more rapid increase in obesity prevalence from 2002 to 2010. These associations, however, virtually disappear when the US and Mexico have been excluded from the analysis. Findings from this study underscore the importance of assessing the impact of extreme cases on the relation between income inequality and health outcomes. The potential pathways from income inequality to the alarmingly high rates of obesity in the cases of the US and Mexico warrant further research.

AbstractBackgroundAnimal studies and a few clinical trials lend credibility to the hypothesis that not all types of fatty acids carry the same potential for weight gain. Only a few epidemiological studies concerning this issue are currently available and results are conflicting.AimThe purpose of the present ecological study was to test the existence of an association between obesity prevalence and the types of fat available in 168 countries.MethodsData on the prevalence of obesity (BMI ≥ 30 kg/m2) for women over 15 years of age were obtained from the WHO Global InfoBase. Food balance sheets for the years 1998 to 2002 were obtained from the FAOSTAT database. Five-year means for energy, total fat, MUFA, PUFA, SFA and ‘other fat’ per capita were calculated, with their standard deviations, for each country. Bivariate correlations and a multiple linear regression model were used to test for the association between prevalence of obesity and types of fat available in these countries.ResultsNot surprisingly, dietary energy supply, SFA, PUFA and ‘other fat’ were positively associated with the prevalence of obesity. We also found, however, a strong negative association between MUFA availability and obesity prevalence (β= −0·68,P< 0·0001).ConclusionPopulations with a lower prevalence of obesity seem to consume a greater amount of MUFA. Considering the partial correlations between variables, our results suggest that in countries with higher obesity prevalence, it is the shift from MUFA to PUFA that particularly appears to be associated with the risk of obesity.

Introduction The Public Health Agency of Canada’s Canadian Chronic Disease Surveillance System (CCDSS) produces population-based estimates of chronic disease prevalence and incidence using administrative health data. Our aim was to assess trends in incidence rates over time, trends are essential to understand changes in population risk and to inform policy development. Methods Incident cases of diagnosed asthma, chronic obstructive pulmonary disease (COPD), diabetes, hypertension, ischemic heart disease (IHD), and stroke were obtained from the CCDSS online infobase for 1999 to 2012. Trends in national and regional incidence estimates were tested using a negative binomial regression model with year as a linear predictor. Subsequently, models with year as a restricted cubic spline were used to test for departures from linearity using the likelihood ratio test. Age and sex were covariates in all models. Results Based on the models with year as a linear predictor, national incidence rates were estimated to have decreased over time for all diseases, except diabetes; regional incidence rates for most diseases and regions were also estimated to have decreased. However, likelihood ratio tests revealed statistically significant departures from a linear year effect for many diseases and regions, particularly for hypertension. Conclusion Chronic disease incidence estimates based on CCDSS data are decreasing over time, but not at a constant rate. Further investigations are needed to assess if this decrease is associated with changes in health status, data quality, or physician practices. As well, population characteristics that may influence changing incidence trends also require exploration.

BACKGROUND: Cholangiocarcinoma and gallbladder cancer are difficult to treat curatively. The treatment of choice is surgery, dependent on detection at a resectable stage. No chemotherapy or radiotherapy options have shown substantial activity. Gemcitabine has demonstrated response in similar cancers. Considering the lack of treatment options for cholangiocarcinoma and gallbladder cancer, a systematic review of the evidence on gemcitabine use for these indications was performed.OBJECTIVE: To perform a systematic review to evaluate the role of gemcitabine in the treatment of cholangiocarcinoma and gallbladder cancer.METHODS: The MEDLINE database was searched (1996 to March 2005) using the medical subject headings 'gemcitabine' and 'gallbladder neoplasms' with results limited to English only. Proceedings from the 1998 to 2004 meetings of the American Society of Clinical Oncology, including the 2004 Gastrointestinal Cancers Symposium, were searched for relevant abstracts. The Canadian Medical Association infobase and the National Guidelines Clearinghouse were also searched for practice guideline reports. Reports were selected and reviewed by two reviewers, and the reference lists from those were searched for additional trials.RESULTS: A total of 13 single-arm phase II trial reports were obtained.CONCLUSIONS: In appropriate patients with gallbladder cancer or cholangiocarcinoma, surgery offers the best chance for survival and should remain the first treatment of choice. For patients not considered candidates for surgery, but willing and able to tolerate chemotherapy alone or in combination with a fluoropyrimidine (such as 5-fluorouracil or capecitabine), gemcitabine appears to be a reasonable alternative to best supportive care, although this conclusion has not been confirmed with a randomized controlled trial.

Introdução: A fimose primária é um problema frequente na consulta de saúde infantil, com prevalência de 11% aos 3 anos de idade. Objetivo: O objetivo desta revisão é avaliar a evidência da eficácia do uso de corticoides tópicos (CCT) na redução da limitação da retratilidade do prepúcio em crianças e adolescentes. Métodos: Pesquisa efetuada nas bases de dados National Guideline Clearing House, Canadian Medical Association Pratice Guidelines Infobase, Cochrane Library, DARE, Bandolier, Evidence Based Medicine Online, Trip Database Online e Pubmed, utilizando os descritores “Phimosis and steroids”. Pesquisaram-se normas de orientação clínica (NOC), meta-análises, revisões sistemáticas e ensaios clínicos aleatorizados e controlados (ECAC), nas línguas portuguesa, inglesa e espanhola. Foi usada a escala Levels of Evidence of Oxford Centre for Evidence Based Medicine. Resultados: Foram encontrados 148 artigos, dos quais 6 cumpriram os critérios de inclusão: 1 NOC e 5 ECAC. A orientação da Sociedade Europeia de Urologia Pediátrica publicada em 2013 recomenda o uso de CCT na fimose primária. Os ECAC incluíram no total 624 crianças e adolescentes entre os zero e os 15 anos. Foram testados diferentes CCT e esquemas terapêuticos. Verificou-se uma melhoria da retratilidade do prepúcio comparativamente com o placebo, com uma eficácia terapêutica dos CCT descrita entre 66% e 90%. Conclusão: Os estudos suportam a eficácia dos CCT no tratamento da fimose primária em crianças e adolescentes. No futuro é pertinente o desenho de estudos metodologicamente homogêneos e de maior dimensão para clarificar qual o melhor CCT e esquema terapêutico.

Material e métodos: Pesquisa de guidelines (GL), ensaios clínicos aleatorizados e controlados (ECA), revisões sistemáticas (RS) e meta-análises (MA), publicadas entre 2009 e 2019, nas bases de dados e sítios de medicina baseada na evidência da PubMed, da Cochrane Library, DARE, Bandolier, TRIP Database, BMJ Evidence-Based Medicine, National Guideline Clearinghouse, NICE, Canadian Medical Association Practice Guidelines Infobase e Primary Care Clinical Practice Guidelines, utilizando-se os descritores MeSH: ‘carpal tunnel syndrome’ e ‘corticosteroids’. Foi aplicada a escala Strength of Recommendation Taxonomy (SORT), da American Academy of Family Physicians, para atribuição dos níveis de evidência (NE) e das forças de recomendação (FR). Resultados: Dos 172 artigos devolvidos da pesquisa efetuada foram selecionados seis: um ECA, quatro RS e uma norma de orientação clínica (NOC). A eficácia dos corticoides orais a curto-médio prazo no alívio sintomático da STC é evidenciada por vários ECA e RS metodologicamente bem concebidos. Na maioria dos estudos a administração oral de 20mg de prednisolona por duas semanas conduz ao alívio temporário dos sintomas. Conclusões: A corticoterapia oral pode ser usada no alívio temporário dos sintomas do STC a curto prazo, num regime de 20mg diários de prednisolona durante duas semanas (SORT A). O seu uso no tratamento a médio e a longo prazo apresenta um nível de evidência mais baixo (SORT B).

Abstract:
The paper presents the data obtained in the study of the health of the ecosystems in a number of recreational areas of the Black Sea and the Eastern Gulf of Finland. Prompt assessment of the health of the ecosystems was carried out by testing the health of adult bivalve mollusks inhabiting them, using the functional load method based on the analysis of their heart rate measured by a fiber-optic bioelectronic system. The goal of the study is to test the innovative bioindication technology developed earlier by the authors on the basis of the above-mentioned method in the regional programs for environmental monitoring of the ecosystems of coastal marine and freshwater recreational water areas (using the example of a number of water areas of Sevastopol, as well as several freshwater areas of the Kurortniy District of St. Petersburg. It has been established that the functional state of mollusks may serve as an indicator of excessive pollution of coastal waters by the objects that discharge insufficiently treated wastewater from their local treatment facilities, including household wastewater. The technology applied in this study for the assessment of functional state of local species of molluscs may be effectively used to solve the problems of early diagnostics of alterations in the health status of coastal aquatic ecosystems and contribute to ensuring the ecological safety of recreational water areas, serving as an infobase for the development of science-based environmental management decisions. Taking into account a high expressiveness and easy-to-use technology applied in the study (which does not require expertise for its practical implementation), this technology may be considered as the best available technology for biomonitoring the water quality of recreational waters in Russia.

Abstract:
Abdominal pain represents one of the most important diagnostic challenges for any physician and its correct interpretation and management require a proper systematic approach and sometimes an urgent action. Moreover the guidelines that can be referred to for indications about the most adequate management procedures are few and often focused only on radiologic management. Consequently, the approach to abdominal pain is often empirical. Therefore, we propose a review of the literature on the diagnosis of abdominal pain, which may contribute to improve the diagnosis and treatment of this complex condition through a systematic review of the evidences available in this field. As to our methodology, we conducted an extensive search in the main guideline databases (<em>SIGN, ICSI, NICE, National Guideline Cleringhouse, CMA Infobase, NZ Guidelines Group, National System Guidelines, Clinical Practice Guidelines Portal, eGuidelines</em>), using as key words abdominal pain and abdominalgia. The guidelines were assessed according to the 2010 Italian version of the AGREE (Appraisal of Guidelines, Research and Evaluation II) methodology. Afterwards we formulated our main recommendations associated with the corresponding levels of evidence and focused our attention on some grey areas, which we investigated with further research using <em>Medline</em> and the main systematic review databases (<em>Cochrane database</em>). The four main grey areas investigated were: hospital admission criteria, prognostic stratification, need for analgesic treatment and possibility of attributing abdominalgia to an abdominal pain syndrome. We then formulated our consesus-based recommendations on the grey areas. Abdominal pain management remains a complex issue for internists. As with other diagnostic challenges, it would be advisable to develop additional guidelines based on a multidisciplinary approach and not only focused on radiological management.

OBJECTIVESpinal metastases that recur after conventional palliative radiotherapy have historically been difficult to manage due to concerns of spinal cord toxicity in the retreatment setting. Spine stereotactic body radiation therapy (SBRT), also known as stereotactic radiosurgery, is emerging as an effective and safe means of delivering ablative doses to these recurrent tumors. The authors performed a systematic review of the literature to determine the clinical efficacy and safety of spine SBRT specific to previously irradiated spinal metastases.METHODSA systematic literature review was conducted, which was specific to SBRT to the spine, using MEDLINE, Embase, Cochrane Evidence-Based Medicine Database, National Guideline Clearinghouse, and CMA Infobase, with further bibliographic review of appropriate articles. Research questions included: 1) Is retreatment spine SBRT efficacious with respect to local control and symptom control? 2) Is retreatment spine SBRT safe?RESULTSThe initial literature search retrieved 2263 articles. Of these articles, 160 were potentially relevant, 105 were selected for in-depth review, and 9 studies met all inclusion criteria for analysis. All studies were single-institution series, including 4 retrospective, 3 retrospective series of prospective databases, 1 prospective, and 1 Phase I/II prospective study (low- or very low–quality data). The results indicated that spine SBRT is effective, with a median 1-year local control rate of 76% (range 66%–90%). Improvement in patients’ pain scores post-SBRT ranged from 65% to 81%. Treatment delivery was safe, with crude rates of vertebral body fracture of 12% (range 0%–22%) and radiation-induced myelopathy of 1.2%.CONCLUSIONSThis systematic literature review suggests that SBRT to previously irradiated spinal metastases is safe and effective with respect to both local control and pain relief. Although the evidence is limited to low-quality data, SBRT can be a recommended treatment option for reirradiation.

Objectives:To catalogue and comparatively assess the quality of Clinical Practice Guidelines (CPG) for ischemic stroke taking into account format and development methodology.Methods:We performed a comprehensive, systematic bibliographic search of CPGs addressing the management of ischemic stroke. We designed a sensitive strategy, using methodological filters in the following databases: Medline, IME and Lilacs, National Guidelines Clearinghouse, National electronic Library for Health, NICE, Guidelines International Network (GIN), Canadian Medical Association Infobase, development groups such as Scottish Intercollegiate Guidelines Network (SIGN), New Zealand Guidelines Group (NZGG), Agency for Healthcare Research and Quality (AHRQ), Ministry of Health Singapore, Institute for Clinical Systems Improvement (ICSI); and scientific societies: American Heart Association, American Medical Association, Royal College of Physicians London. We included all CPGs published in English, French, Italian, Portuguese, or Spanish from 1999 to 2005 and excluded those CPGs whose scope was primary prevention and rehabilitation from ischemic stroke. Four researchers independently assessed the structure and methodologies followed in drafting the CPGs using the Changing Professional Practice (CPP) and Appraisal of Guidelines Research &amp; Evaluation (AGREE) instruments.Results:We retrieved 117 documents; following application of exclusion criteria, twenty-seven CPGs were appraised. With regard to methodological quality (using the AGREE instrument), the domains that scored highest were “Scope and purpose” and “Clarity and presentation.” The lowest scoring domains were “Stakeholder involvement,” “Rigor of development,” and “Applicability.” Most guidelines received an overall score of “would not recommend” (77.8 percent). Finally, based on the CPP instrument, most of the CPGs evaluated were aimed at secondary care and did not provide updating procedures.Conclusions:The overall quality of the CPGs published for ischemic stroke management did not have minimum methodological quality. Quality improvement has been observed in more recent CPGs and may be due to the publication of new tools such as the AGREE or CPP instruments, as well as international initiatives for CPG improvement.

La investigación fue de tipo básica, con un diseño de nivel descriptivo – explicativo, ya que buscó la generación de conocimiento evaluando el nivel de cumplimiento normativo del registro de obras públicas en el sistema INFObras en la EPS Tacna S.A. durante el periodo 2013 – 2020, fortaleciendo el control y la transparencia de las obras públicas con el fin de favorecer el control social por parte de la ciudadanía. La población estuvo constituida por las 86 obras públicas registradas en el sistema INFObras y los 17 proyectos de inversión registrados en el sistema de seguimiento de la ejecución presupuestal del Ministerio de Economía y Finanzas (MEF). Se concluyó que la aplicación del control social en la evaluación de la información del sistema INFObras de la Contraloría General de la República del Perú (CGR) y del sistema de ejecución presupuestal del MEF permitió evidenciar un alto nivel de incumplimiento normativo en el registro de obras públicas del sistema INFObras, por parte de la EPS Tacna S.A.; limitando el derecho a la participación ciudadana de la población de la región Tacna, es decir, tener la posibilidad de conocer si las obras en su región se estén desarrollando dentro de los plazos establecidos, con la calidad requerida y cumpliendo su finalidad pública.

The article aims to find the probability effects of bank’s health level using CAMEL ratio analysis. The statistic method used to test on the research hypothesis was logit regression. The dependent variable used in this research was bank’s health level and independent variables were CAMEL financial ratios consisting of CAR, NPL, ROA, ROE, LDR, and NIM. The report data were extracted from bank’s financial from financial report, which had been published and accumulated by Infobank research bureau with valuation, based on bank Indonesia policy. The sample consisted of 60 healthy banks and 14 unhealthy banks in 2005 and 2006. The empirical result of this research indicates that the Non Performing Loan is the significant variable affecting bank health level.
