Abstract:
Objectives Dry socket and post-extraction pain are typical discomforts experienced by patients after tooth extraction. In this study, we inserted gauze coated with oxytetracycline-hydrocortisone ointment into the extraction socket immediately after lower third molar extraction and then evaluated the occurrence of dry socket and post-extraction pain compared with gauze non-insertion. Methods This retrospective study was carried out on patients undergoing lower third molar extraction in the Department of Oral Surgery at Shizuoka Prefectural General Hospital in Shizuoka, Japan from November 2018 to October 2019. A comparison was carried out between a gauze-insertion group and a non-insertion group. The occurrence versus non-occurrence of dry socket was determined, and degree of pain was assessed based on a visual analogue scale (VAS) and on patients reporting the number of loxoprofen sodium oral analgesic tablets (60mg/tablet) that they had taken. Dry socket was defined as patient-reported spontaneous pain that did not subside 1 to 3 days postoperatively. Spontaneous post-extraction pain was recorded four times: on the operative day, on the first postoperative day (POD1), on POD3, and during suture removal (POD7). Results The occurrence of dry socket was lower in the gauze-insertion group than in the non-insertion group (0.9%, 2/215 vs. 19.6%, 9/46, p<0.001). The results also showed that both VAS-defined pain level and the number of analgesic tablets taken were lower in the gauze-insertion group than in the non-insertion group on POD3 and POD7. Conclusions and clinical relevance Inserting gauze coated with oxytetracycline-hydrocortisone ointment into the extraction socket immediately after third molar extraction reduces the occurrence of both dry socket and post-extraction pain.

The evolution of the optical network to meshed architecture led to the need to design and deploy reconfigurable optical add/drop multiplexers (ROADM) as routing nodes. The design complexity of such devices has a direct impact on their manufacturing cost. The aim of this thesis is to propose a low cost fully reconfigurable solution. We have chosen liquid crystal technology associated with polarization filters to achieve a tunable wavelength blocker (WB) used as a ROADM subsystem. This technology is simple and mature, and tuning voltages needed are relatively small (a few volts). As the filtering function is a key feature that characterizes any WB subsystem, we have demonstrated that with liquid crystal cells and birefringent plates we are able to tune the central wavelength, the bandwidth and the transmission function shape of these birefringent filters. All of these features have enabled us to design optical devices dedicated to WDM dynamic applications. We then experimentally tested the feasibility of a tunable WB and a 1x2 wavelength switch based on hybrid liquid crystal polarization filters.

L'apparition des dernières applications de l'internet à haut débit engendre une augmentation importante des échanges d'informations à travers le monde. Pour éviter une saturation inexorable du réseau de télécommunications, les liaisons par fibre optique avec multiplexage en longueur d'onde (DWDM) se généralisent pour permettre la transmission de données avec un débit actuel maximum de 1Tbit/s. L'aiguillage en longueur d'onde des signaux entre les différentes boucles de ce type de réseau est réalisé à l'aide de Multiplexeurs Insertion- Extraction (MIE). Ce mémoire est consacré à l'étude et à la réalisation de deux MIE réalisés par photoinscription de réseaux de Bragg (=filtres sélectifs en longueur d'onde photoinscrits par un champ d'interférence de lumière laser UV) dans des guides optiques à coeur dopé en germanium. Le premier MIE étudié lors de cette thèse est réalisé sur la base d'un coupleur directif en optique intégrée SiO2/Si dans la zone de couplage duquel deux réseaux de Bragg ont été photoinscrits. Le travail a consisté à améliorer à la fois la fabrication des guides optiques (diminution de leur biréfringence) ainsi que leur photoinscription (meilleure apodisation de la réponse spectrale du spectre réfléchi). La conception et la réalisation de premiers prototypes permettant l'extraction de canaux espacés de 200 GHz ont alors pu être effectuées. La deuxième étude se concentre, quant à elle, sur l'optimisation d'un MIE plus mature basé sur un interféromètre de Mach-Zehnder à réseaux de Bragg réalisé en fibre bicoeur. L'influence néfaste du déphasage engendré par les réseaux de Bragg sur la réponse spectrale de l'interféromètre a été discutée et minimisée. La reconstruction de la dispersion chromatique de ces réseaux via la réponse spectrale de l'interféromètre a aussi été étudiée. Des composants industrialisables pour réseaux d'espacements intercanaux de 100 GHz et 50 GHz ont alors pu être réalisés.

La proliferation et le grand succes des applications incorporant des reseaux de neurones artificiels (rna) dans plusieurs domaines montrent l'utilite du paradigme rna. Neanmoins, ce paradigme a une limite : son incapacite inherente a fournir une explication des resultats obtenus. C'est essentiellement pour vaincre cette limite que plusieurs chercheurs se sont interesses a combiner les rna et les systemes symboliques de maniere a profiter de leurs avantages et eviter leurs faiblesses. Dans cette these, nous decrivons notre contribution a ce domaine de recherche. Nos travaux s'articulent autour de trois axes : l'insertion de connaissances dans un rna, l'extraction de regles a partir d'un rna et l'utilisation d'un rna pour raffiner une base de regles existante. Dans le premier axe : apres une etude critique des principales techniques developpees, nous avons propose deux techniques d'extraction de regles. La premiere, miter, associe a chaque neurone un calibre de poids. Ce dernier est traduit sous forme d'une regle symbolique de la forme m-parmi-n. La seconde, emire, extrait des regles a partir d'un rna sans tenir compte de sa structure interne et en utilisant uniquement ses entrees pertinentes. Dans le deuxieme axe : apres une presentation des principales techniques existantes, nous avons propose deux techniques d'insertion de regles symboliques dans un rna. La premiere, ruleneur, associe a chaque regle ecrite sous une forme normale disjonctive un neurone. La seconde, opneur, associe a chaque operateur logique (et ou ou) un neurone. Dans le troisieme axe, apres une presentation des differents systemes hybrides, nous avons propose le systeme ranni qui combine les deux axes precedents en utilisant un module probabiliste pour leur mise-a-jour.

The development of algorithms that effectively modify all-hexahedral finite element mesh is currently an active area of research. Mesh modification can be used to improve mesh quality reduce the time required to mesh a model, and improve the finite element analysis results. However, general modification of all-hexahedral meshes has proven difficult because of the global effects of local modifications. This thesis explains the global constraints on modifying all-hexahedral meshes and then presents three mesh modification techniques that make it possible to do local modifications while accounting for the global effects. These techniques are sheet insertion, sheet extraction, and mesh cutting. Sheet insertion is used to refine a mesh by inserting sheets of hexahedral elements into existing meshes. Sheet extraction coarsens existing meshes by deleting sheets of elements from the mesh. Mesh cutting is used to modify a simple mesh to fit it to complex geometric feature. The mesh modification techniques are covered in detail with representative graphics. Examples are given that demonstrate the application of each technique to the mesh generation process.

L'essor des télécommunications a entraîné une augmentation du débit d'informations. Pour pallier ce problème le multiplexage en longueur d'onde possible en optique semble une bonne solution : il permet le transport simultané grâce à une même fibre optique de plusieurs signaux de longueurs d'onde différentes. La solution choisie ici utilise l'échange d'ions dans un substrat de verre et combine un interféromètre Mach Zehnder réalisé à l'aide de guides d'ondes enterrés et un réseau de diffraction gravé sur des guides d'ondes de surface. Avant la réalisation du composant global, nous avons développé un procédé particulier de l'échange d'ions : l'enterrage différentiel. Il permet de réaliser des guides d'ondes dont certaines parties sont enterrées tandis que d'autres restent en surface. Ce procédé a abouti à des guides d'onde avec une différence de profondeur d'enterrage de 10 μm sans que la transition entre zone enterrée et zone de surface ne crée de pertes excédentaires ou ne modifie la polarisation. Ce qui correspond aux modélisations. Ensuite, nous avons vérifié la validité du procédé de gravure RIE de réseau en surface du substrat. Pour cela, des filtres de Bragg associant un guide d'ondes par enterrage différentiel et un réseau gravé par dessus ont été réalisés. Nous obtenons alors des filtres avec un taux de réjection de 21 dB et une bande de réjection de 0,45 nm. Enfin, nous avons réalisé le composant global associant des coupleurs réalisés par échange d'ions dans du verre et les filtres de Bragg précédents. Cet assemblage à permis d'aboutir à un extracteur de longueur d'ondes de taux de réjection de 30 dB et de largeur de bande de réjection de 0,45 nm vers 1548 nm
Ln the last few years, the development of telecommunications has led to an increase of informations rate. To solve this problem wavelength multiplexing possible in optic seems to be a good solution: it allows the simultaneous transmission in a same fibre of several signais of different wavelengths. The solution chosen here use ion exchange on glass substrate and combine a Mach Zehnder interferometer realised with buried waveguides and a diffraction grating etched upon surface waveguides. Before the realisation of the global component, we have developed a particular process of ion-exchange: selectively buried waveguides. It allows to realise 3 dimensional waveguides namely waveguides with some parts buried inside the glass substrate and some parts at the surface of the glass substrate. The process we developed allows to realise waveguides with a burying depth difference of 10 μm unless the transition between the buried parts and the surface parts creates excess losses or change the polarisation. This corresponds to the simulations. Then, we have checked the validity of the fabrication process of etched diffraction gratings at the surface of a glass substrate: we have realised Bragg Filters by the association of a selectively buried waveguides upon which a grating has been etched. We obtained filters with a rejection rate of 21 dB and a rejection band width of 0,45 nm. Finally, we have realised the global component, association couplers made by ion exchange in glass and the previously described Bragg filters. This assembly led to the realisation of a drop filter with a rejection rate close to 30 dB and a rejection band of 0,45 nm around 1548 nm

Abstract:
Le développement du multiplexage en longueur d'onde dans les télécommunications optiques nécessite l'utilisation de filtres rapidement accordables et ayant une bonne sélectivité. La technique proposée dans cette thèse consiste en l'association d'un réseau de diffraction et d'un réseau de Bragg intracavité. Ce principe a été implémenté en deux montages expérimentaux. Le premier utilise un réseau intracavité inscrit sur cristaux liquides, le second un réseau de Bragg acousto-optique. Le dispositif à cristaux liquides permet d'obtenir un filtre avec une bande passante de 0,09nm. Cette sélectivité est satisfaisante cependant la méthode utilisée ne permet pas de faire varier sa période et donc d'accorder le filtre. L'acousto-optique, en revanche, est une technologie déjà bien maîtrisée et qui permet d'incrire des réseaux d'indice dont la période peut être modifiée très rapidement (~µs). Le dispositif que nous avons réalisé avec une cellule acousto-optique permet d'obtenir une bande passante de 0,076nm avec un rapport signal à bruit de 20dB. Ce filtre est accordable sur une gamme de 2,2nm, ce qui permet de distinguer 30 longueurs d'ondes. La bande passante du filtre ainsi que sa gamme d'accord sont très facilement adaptables en changeant l'orientation ou le pas du réseau de diffraction. Afin de modéliser le principe de filtrage, une simulation en faisceau gaussien a été développée. Elle est en bon accord avec les résultats expérimentaux et nous permet de prévoir les performances qu'il est possible d'obtenir avec d'autres réseaux de Bragg. Par exemple, en remplaçant la cellule acousto-optique, il est possible d'obtenir un filtre capable de séparer 500 longueurs d'ondes
The constant growth of the needs in data transmission rates has led to the development of wavelength division multiplexing (WDM). The demultiplexing associated with WDM requires very selective filters with fast tuning possibilities. In this thesis, we propose a filtering technique associating a diffraction grating with an intracavity Bragg grating. Two filters have been implemented experimentally using this principle: one using a liquid crystal intracavity grating and one using an acousto-optic Bragg grating. The liquid crystal device has a bandwidth of 0. 09nm, which is very interesting but this technique does not allow to tune the filter. A tunable Bragg grating can be implemented using the acousto-optic effect. The device we made using an acousto-optic cell has a bandwidth of 0. 076nm with a signal to noise ratio of 20dB. This filter is tunable on a range of 2. 2nm, which allows to separate 30 wavelengths. The bandwidth and the tuning range can be easily adapted by changing the diffraction grating's orientation or its period. A numerical simulation of the response of the filter for a gaussian beam has been developped and it fits the experimental results. It allows us to calculate the performances that could be obtained with filters designed differently. For example, replacing our acoust-optic cell by one with a wider active medium (commercially available) would allow us to separate 500 frequencies. It would then be possible to have a tuning range of 100nm with a bandwidth of 0. 2nm

The needle-based procedures are usually considered minimally invasive. However, in insertion into soft tissues such as brain and liver, the tissue damage caused by needle insertion can be very significant. From the literature, it has been known that reduction in needle insertion and extraction forces as well as tissue deformation during the insertion results in less invasive procedure. This work aims to design and develop a new bioinspired design for surgical needles which reduce the insertion and extraction forces of the needle, and its damage to the tissue. Barbs in honeybee stinger decrease its insertion force significantly. Inspired by that finding, a new honeybee-inspired needle was designed and developed, and its insertion mechanics was studied. To study the insertion mechanics of honeybee-inspired needle, insertion tests into artificial and biological tissues were performed using both honeybee-inspired and conventional needles. The barb design parameters effects on needle forces were studied through multiple insertion and extraction tests into PVC gels. The design parameters values of the barbs were experimentally modified to further reduce the ultimate insertion and extraction forces of the needle. Bioinspired needle with modified barb design parameters values reduces the insertion force by 35%, and the extraction force by 20%. To show the relevance, the insertion tests into bovine liver and brain tissue were performed. Our results show that there was a 10-25% decrease in the insertion force for insertions into bovine brain, and a 35-45% reduction in the insertion force for insertions into the bovine liver using the proposed bioinspired needles. The bioinspired and conventional needles were manufactured in different scales and then used to study the size scale effect on our results. To do so, the insertion tests into tissue-mimicking PVC gels and liver tissues were performed. The results obtained for different sizes of the needle showed 25-46% decrease in the insertion force. The tissue deformations study was conducted to measure tissue deformation during the insertion using digital image correlation. The tissue deformation results showed 17% decrease in tissue deformation using barbed needles. A histological study was performed to accurately measure the damage caused by needle insertion. Our results showed 33% less tissue damage using bioinspired needles. The results of the histological study are in agreement with our hypothesis that reducing needle forces and tissue deformation lead to less invasive percutaneous procedures.

An example of this technology is represented by the generation of a drilling template, to assist the surgeon in identifying the optimal direction of insertion of the pedicle screws, capable of significantly reduce intervention times, in addition to the inevitable exposure of the patient to ionizing radiation, to which he is subjected during a normal arthrodesis intervention procedure. The design of a drilling guide requires, however, a particular attention in identifying the undercuts present on the vertebral surface, those areas of the spinous process which, reported inside the cavity of the template, involve complications at the time of extraction. In parallel, it is vitally important to carry out an evaluation of its stability during its use. In this article, starting from the analysis of the interferences present during the insertion of the template, a semi-automatic correction model is proposed for the generation of a new profile of the same, which facilitates its extraction without causing injury to the vertebral regions involved from the contact with the mask.

AbstractThe use of stable isotope-labeled standards (SIS) is an analytically valid means of quantifying proteins in biological samples. The nature of the labeled standards and their point of insertion in a bottom-up proteomic workflow can vary, with quantification methods utilizing curves in analytically sound practices. A promising quantification strategy for low sample amounts is external standard addition (ExSTA). In ExSTA, multipoint calibration curves are generated in buffer using serially diluted natural (NAT) peptides and a fixed concentration of SIS peptides. Equal concentrations of SIS peptides are spiked into experimental sample digests, with all digests (control and experimental) subjected to solid-phase extraction prior to liquid chromatography tandem mass spectrometry (LC-MS/MS) analysis. Endogenous peptide concentrations are then determined using the regression equation of the standard curves. Given the benefits of ExSTA in large-scale analysis, a detailed protocol is provided herein for quantifying a multiplexed panel of 125 high-to-moderate abundance proteins in undepleted and non-enriched human plasma samples. The procedural details and recommendations for successfully executing all phases of this quantification approach are described. As the proteins have been putatively correlated with various noncommunicable diseases, quantifying these by ExSTA in large-scale studies should help rapidly and precisely assess their true biomarker efficacy.

In order to address poor bandwidth-utilization in circuit-switched WRONs, various techniques for optical packet-switching (OPS) have been explored, but needing complex technologies, such as real-time header extraction/insertion, packet alignment, etc. An intermediate solution between the WRONs and OPS networks – the optical burst-switched (OBS) network – has been explored, where several packets are clubbed together at ingress nodes to form optical bursts, which are transmitted with the headers sent as control packets ahead of each bursts. With this prior resource-reservation scheme at en-route nodes before burst arrivals, OBS networks overcome the challenges of OPS networks, while improving bandwidth utilization as compared to WRONs. We first present the node architectures, followed by header-processing schemes and switch designs for OPS networks. Next we present the basic concepts of OBS networking and describe the necessary network protocols, including burst assembly scheme, just enough time (JET) signaling, resource-reservation and routing schemes. Tube misdirection and inadequate tube length are 2 problems commonly seen by glaucoma surgeons. These complications can occur at the time of surgery or develop years later. Understanding the underlying causes for these problems and techniques to prevent and manage these issues is critical for successful outcomes. Tube placed too anteriorly. During anterior chamber tube shunt implantation, a 23-gauge needle is commonly used to make entry into the anterior chamber. The tube is then inserted through this track into the space between the corneal and iris planes. For most surgeons the anterior chamber depth has been altered prior to tube insertion from shallowing caused by loss of aqueous volume during creation of a paracentesis or deepening caused by filling the chamber with viscoelastic to create enough room for placement. As the intraocular pressure (IOP) becomes lower, the postoperative anterior chamber depth is not always equal to the preoperative depth. As a result, the iris may move slightly forward, or if the shunt is to be implanted in combination with lens extraction, the iris may move more posteriorly. The unpredictability of postoperative chamber depth prevents consistent intraoperative determination of the appropriate plane of entry. Minimizing steps that affect the anterior chamber depth prior to tube insertion may help avoid these issues. However, despite limiting anterior chamber depth alterations, it is not uncommon for the tube to not end up in the desired anterior chamber plane. The correct plane is in front of the iris with just enough space to prevent iris contact. If the tube ends up too anteriorly, it may rub against the corneal endothelium and result in endothelial cell loss with chronic corneal edema. Alternatively, if the tube is placed too posteriorly, it could rub against the iris and cause chronic low-grade inflammation, worsening glaucoma along with other iritis-related issues. In either case, a new track may be made to direct the tube into the anterior chamber. Tube in iris stroma or in the posterior chamber. Occasionally the tube embeds in the iris stroma during its insertion. Careful preoperative evaluation is important for prevention of this scenario.

For an example, the insertion of Business Intelligence (BI) tools in healthcare environments has been recently used to improve healthcare delivery. It is based on the analysis of data in order to provide useful information. BI tools assist managers and health professionals through decision-making, since they allow the manipulation and analysis of data in order to extract knowledge. This work aims to study and analyze the time that physicians take to prescribe medical exams in Centro Hospitalar do Porto (CHP), though BI tools. The main concern is to identify the physicians who take more time than average to prescribe complementary means of diagnosis and treatment, making it possible to identify and understand the reason why it occurs. To discover these outliners, a BI platform was developed using the Pentaho Community. This platform presents means to represent information through tables and graphs that facilitate the analysis of information and the knowledge extraction. This information will be useful to represent knowledge concerning not only the prescription system (auditing it) but also its users. The platform evaluates the time prescription, by specialty and physician, which can afterwards be applied in the decision-making process. This platform enables the identification of measures to unravel the time differences that some physicians exhibit, in order to, subsequently, improve the whole process of electronic medical prescription.

This paper describes the derivation and experimental validation of geometric equations that govern insertion and extraction of a robotic inspection system that operates in gaps around vertical dry storage casks. During insertion, a robotic system may become jammed due to unbalanced forces acting on the robot, or wedged due to over-sized robot geometry. The robot must be removable by a tether in the event of power loss. Assuming simplified geometry and a quasi-static approach, the problem is modeled using a two-dimensional representation in which the robot is assumed to be rigid with equal weight distribution and a constant friction coefficient between surfaces. Equilibrium equations are derived from a modified peg-insertion formulation, allowing calculation of the maximum size of the robot and angle of insertion as a function of inspection gap geometry and friction. Experimentation tested the derived relationships using varying robot dimensions in a 1:1 scale mock-up of the overpack-to-canister gap space of a nuclear dry storage container. Experimental data confirmed that the modifications of the typical peg-insertion predicted successful insertion and extraction better than unmodified equations. The error between the model and experimentation had a mean and standard deviation of 4.4 and +/− 0.53 degrees.
