Abstract:
Background: Katsina State Government in Northern Nigeria introduced integrated supportive supervision (ISS) in primary health centers. The study was guided by the Primary Health Care Performance Initiative Conceptual Framework. The goal of the study was to measure the impact of ISS on the quality of primary health-care delivery. The outcome variables measured include infrastructure, basic equipment, human resources for health, essential drugs, the number of pregnant women screened for HIV, and the number of children receiving immunization. Methods: The study was a cross-sectional survey of 34 health facilities. Kruskal-Wallis nonparametric test followed by Dunn post hoc test and analysis of variance followed by Tukey post hoc test were both employed to compare the mean values of various indicators obtained over 6 visits of the ISS program from July 2018 to December 2018. Findings: The study shows the positive effect of the ISS on infrastructure, human resources for health, essential drugs, and the number of pregnant women screened for HIV ( P < .05). Human resources for health and the number of children receiving immunization were both not affected by ISS ( P > .05). Conclusion: Integrated supportive supervision need to be embedded into the 3 levels (primary, secondary, and tertiary) of health-care service delivery. Implications: Integrated supportive supervision will strengthen the Katsina State health system, ensure efficient use of the health facility assets and resources utilization, and improve patient/client satisfaction.

Abstract:
(1) Background: The Ministry of Health in Mali included the treatment of severe acute malnutrition (SAM) into the package of activities of the integrated community case management (iCCM). This paper evaluates the most effective model of supervision for treating SAM using community health workers (CHWs). Methods (2): This study was a prospective non-randomized community intervention trial with two intervention groups and one control group with different levels of supervision. It was conducted in three districts in rural areas of the Kayes Region. In the high supervision group, CHWs received supportive supervision for the iCCM package and nutrition-specific supervision. In the light supervision group, CHWs received supportive supervision based on the iCCM package. The control group had no specific supervision. (3) Results: A total of 6112 children aged 6–59 months with SAM without medical complications were included in the study. The proportion of cured children was 81.4% in those treated by CHWs in the high supervision group, 86.2% in the light supervision group, and 66.9% in the control group. Children treated by the CHWs who received some supervision had better outcomes than those treated by unsupervised CHWs (p < 0.001). There was no difference between areas with light and high supervision, although those with high supervision performed better in most of the tasks analyzed. (4) Conclusions: Public policies in low-income countries should be adapted, and their model of supervision of CHWs for SAM treatment in the community should be evaluated.

Abstract:
The use of online Integrated Supportive Supervision (ISS) is aimed to improve the quality of services provided by front line health workers. This work is aimed to document the effects of ISS on the performance of health workers in Zambia using selected key surveillance and immunization process indicators. ISS data on WHO ODK server of all Integrated Supportive Supervisory (ISS) visits that were conducted in Zambia between 1st January 2018 to 30th September 2018 were analysed to determine the Percentage point difference between the first and the most recent ISS visits in order to determine whether an observed gap during first ISS visit had persisted during the most recent ISS visit. Our study demonstrated that ISS has remarkable percentage point increase between the first and the most recent ISS visits on availability of an updated monitoring chart, health workers knowledge of AFP case definition and AFP case files. However, there exist variations in the frequency of ISS visits across the provinces of the country. Future research effort should consider assessing the quality of the ISS data through periodic data validation missions.

The increasing use of infusible biologic therapies, including the novel monoclonal antibody natalizumab for the treatment of relapsing forms of multiple sclerosis, has elicited much interest among neurologists in the provision of in-office infusions for their patients. An in-office infusion center may offer neurologists a means to provide integrated care for their patients in a familiar and supportive environment. This setting is especially convenient for chronically ill patients, allowing them to receive high-quality care under the direct supervision of their neurologist and facility staff. By administering infusible treatments in a neurology practice rather than referring patients to a hospital or oncology/hematology-based infusion center, the primary neurologist can more closely monitor clinical outcomes, treatment adherence, and the occurrence of adverse effects. In addition, there is greater opportunity for patient interaction and education, which can strengthen relationships with clinical caregivers. This model is also applicable to multispecialty or hospital-based neurology groups desiring to integrate neurology infusion services. In this article, we discuss overall management strategies; staffing and scheduling issues; general coding, billing, and reimbursement methodologies; and additional financial considerations.

Abstract:
PURPOSE Until human papillomavirus (HPV)–based cervical screening is more affordable and widely available, visual inspection with acetic acid (VIA) is recommended by the WHO for screening in lower-resource settings. Visual inspection will still be required to assess the cervix for women whose screening is positive for high-risk HPV. However, the quality of VIA can vary widely, and it is difficult to maintain a well-trained cadre of providers. We developed a smartphone-enhanced VIA platform (SEVIA) for real-time secure sharing of cervical images for remote supportive supervision, data monitoring, and evaluation. METHODS We assessed programmatic outcomes so that findings could be translated into routine care in the Tanzania National Cervical Cancer Prevention Program. We compared VIA positivity rates (for HIV-positive and HIV-negative women) before and after implementation. We collected demographic, diagnostic, treatment, and loss-to-follow-up data. RESULTS From July 2016 to June 2017, 10,545 women were screened using SEVIA at 24 health facilities across 5 regions of Tanzania. In the first 6 months of implementation, screening quality increased significantly from the baseline rate in the prior year, with a well-trained cadre of more than 50 health providers who “graduated” from the supportive-supervision training model. However, losses to follow-up for women referred for further evaluation or to a higher level of care were considerable. CONCLUSION The SEVIA platform is a feasible, quality improvement, mobile health intervention that can be integrated into a national cervical screening program. Our model demonstrates potential for scalability. As HPV screening becomes more affordable, the platform can be used for visual assessment of the cervix to determine amenability for same-day ablative therapy and/or as a secondary triage step, if needed.

Abstract:
BackgroundPneumonia remains the leading cause of child mortality in sub-Saharan Africa. The Integrated Management of Childhood Illness (IMCI) strategy was developed to standardise care in low-income and middle-income countries for major childhood illnesses and can effectively improve healthcare worker performance. Suboptimal clinical evaluation can result in missed diagnoses and excess morbidity and mortality. We estimate the sensitivity of pneumonia diagnosis and investigate its determinants among children in Malawi.MethodsData were obtained from the 2013–2014 Service Provision Assessment survey, a census of health facilities in Malawi that included direct observation of care and re-examination of children by trained observers. We calculated sensitivity of pneumonia diagnosis and used multilevel log-binomial regression to assess factors associated with diagnostic sensitivity.Results3136 clinical visits for children 2–59 months old were observed at 742 health facilities. Healthcare workers completed an average of 30% (SD 13%) of IMCI guidelines in each encounter. 573 children met the IMCI criteria for pneumonia; 118 (21%) were correctly diagnosed. Advanced practice clinicians were more likely than other providers to diagnose pneumonia correctly (adjusted relative risk 2.00, 95% CI 1.21 to 3.29). Clinical quality was strongly associated with correct diagnosis: sensitivity was 23% in providers at the 75th percentile for guideline adherence compared with 14% for those at the 25th percentile. Contextual factors, facility structural readiness, and training or supervision were not associated with sensitivity.ConclusionsCare quality for Malawian children is poor, with low guideline adherence and missed diagnosis for four of five children with pneumonia. Better sensitivity is associated with provider type and higher adherence to IMCI. Existing interventions such as training and supportive supervision are associated with higher guideline adherence, but are insufficient to meaningfully improve sensitivity. Innovative and scalable quality improvement interventions are needed to strengthen health systems and reduce avoidable child mortality.

Abstract:
The focus of this paper is the design and development of an innovative project-based work-integrated learning (WIL) course that enables undergraduate students from all faculties to work in interdisciplinary teams with partner organisations from the public, private, and community sectors. Projects are codesigned with partners interested in students from multiple disciplines bringing insights to problems that might not otherwise be resourced by their organisation. These ‘projects of consequence’ are unstructured, ambiguous and complex, requiring student teams to synthesise their different disciplinary knowledge. Student learning is supported by course curriculum design that includes proven pedagogical practices such as: ongoing feedback from partners; students working in autonomous small teams; academic supervision of teamwork; learning support before and during project work; debriefing; providing a safe, supportive space to experiment with new ideas, embracing the prospect of failure and to develop resilience; all underpinned by reflection so as to process, understand and integrate students’ experiences. The versatility of the course design was demonstrated by the rapid change from face-to-face delivery to an entirely online learning environment in response to the COVID-19 pandemic. The challenges and adaptations include changes to the way student teams worked, the additional support needed by students, and adjustment in the mode of interactions with partners.

Abstract:
Purpose Previous studies have demonstrated that an autonomy-supportive supervision style is associated with improved well-being and positive behaviours for supervisees. However, autonomy-supportive training (AST) has yet to be tailored to suit supervisors in low-skilled occupations for whom traditional pedagogical approaches may be inappropriate. The purpose of this paper is to describe the development and preliminary evaluation of AST for these supervisors, using self-determination theory (SDT) and andragogical principles of adult learning. Design/methodology/approach SDT and andragogical principles were systematically integrated to develop (a 3 h) AST programme. The training sessions were trialled with 11 first-line supervisors in New Zealand as a preliminary evaluation of AST. The evaluation used open-ended questions following Kirkpatrick’s evaluation model and incorporated the trainer’s reflections. Findings Supervisors found AST relevant, easy to understand and suited to their approach to learning. Trainer’s reflections also provided insight into the challenges in conducting such training for supervisors in low-skilled occupations and the article makes suggestions to address these challenges. Research limitations/implications AST can be successfully tailored to first-line supervisors, indicating that an autonomy-supportive style of leadership is relevant for those employed in low-skilled occupations. This initial evaluation provides a foundation for future studies to conduct higher-level assessment of AST. Practical implications AST can be utilised to provide first-line supervisors with access to improved leadership development opportunities. Challenges of conducting this kind of training programme in a context of low-skilled occupations are addressed and recommendations made for organisations and trainers. Originality/value This study is novel as it demonstrates the development of AST, a leadership skills training, tailored to suit the needs of an understudied group, supervisors in low-skilled occupations.

Background: Primary health care facilities constitute the first point of contacts of public with healthcare and form integral part of the country’s health system.Methods: A descriptive cross sectional study was conducted among 88 primary care facilities in the State. A simple random sampling technique was used to select the facilities. Federal Ministry of Health integrated supportive supervision tool was adapted for data collection and analysis done using SPSS Version 20.0. The variables were summarised with frequency and percentage and results presented in tables.Results: Almost two-thirds (65%) of the facilities provide 24 hours service coverage for both maternal and child care services. Only 16% of the facilities had medical officers, 12.5% had required number of nurse/midwife while 27% had no single nurse/midwife. With respect to trainings, one third of the facilities had personnel trained on medium and extended lifesaving skills, 20% had a trained staff on emergency obstetrics and newborn care while 61% had no single trained personnel on integrated management of childhood illnesses. A large proportion of the facilities provide maternal services such as focused ANC and delivery but none use partograph to monitor labour. A good number of facilities were lacking basic equipment and medicine supply with about two third of facilities lacking misoprostol and magnesium sulphate, and only 15% had functional DRF.Conclusions: Health resources and the level of service provision in its current form may not lead to a significant improvement in maternal and child health in the state to guarantee universal coverage.

Abstract:
Background. A complete and consistent use of integrated management of childhood illness (IMCI) protocol is a strategic implementation that has been used to promote the accurate assessment and classifications of childhood illnesses, ensures appropriate combined treatment, strengthens the counseling of caregiver, and speeds up the referrals to decrease child mortality and morbidity. However, there is limited evidence about the complete and consistent use of IMCI protocol during the assessment and classifications of childhood illness in Ethiopia. Therefore, this intervention was implemented to improve the assessment and classifications of childhood illness according to the IMCI protocol in Sanja primary hospital, northwest Ethiopia. Methods. A pre-post interventional study was used in Sanja primary hospital from January 01 to May 30, 2019. A total of 762 (381 for pre and 381 for postintervention) children from 2 months up to 5 years of age were involved in the study. Data were collected using a structured questionnaire prepared from the IMCI guideline, and a facility checklist was used. A five-month in-service training, weekly supportive supervision, daily morning session, and availing essential drugs and materials were done. Both the descriptive statistics and independent t -test were done. In the independent t -test, a p value of <0.05 and a mean difference with 95% CI were used to declare the significance of the interventions. Results. The findings revealed that the overall completeness of the assessment was improved from 37.8 to 79.8% (mean difference: 0.17; 95% CI: 0.10-0.22), consistency of assessment with classification from 47.5 to 76.9% (mean difference: 0.34; 95% CI: 0.27-0.39), classification with treatment from 42.3 to 75.4% (mean difference: 0.35; 95% CI: 0.28-0.47), and classification with follow-up from 32.8 to 73.0% (mean difference: 0.36; 95% CI: 0.29-0.42). Conclusion. The intervention has a significant improvement in the assessment and classification of childhood illness according to the IMCI protocol. Therefore, steps must be taken to ensure high quality of training, adequate supervision including the observation of health workers managing sick children during supervisory visits, and a constant supply of essential drugs and job aids for successful implementation of IMCI in the hospital and also to other facilities.

Abstract:
Improving nutrition contributes to productivity, economic development, and poverty reduction by getting better cognitive development, school performance, physical work capacity, and maintaining health status by reducing morbidity and mortality. Poor nutrition perpetuate the cycle of poverty. Community-Based Nutrition (CBN) is an important component of the National Nutrition Program, designed to build upon the Health Extension Program packages to improve nutritional status of under-five children and pregnant and lactating women. As part of this program shift, CBN training modules have been shortened and incorporated into the Integrated Refresher Training (IRT). The nutrition components of Integrated Refresher Training have not been assessed so far. This study aims to assess the quality of CBN component of integrated refresher training, stakeholder perceptions on the quality of training and change in the knowledge of HEWs. Institutional based cross-sectional study with both qualitative and quantitative data collection methods was used. Four Woredas were chosen purposively from a listing of all woredas receiving IRT module II in Amhararegion from June-July 2012. Many MTs and trainees mentioned difficulty of delivering the training as designed due to shortage of time allocated. This was alsoobserved in IRT sessions where MTs used additional than allocated time. Even though most trainees said the training on CBN component was adequate to give services to the community and significant knowledge change (p<0.05) was seen by participants after the training, it was observed that they failed to give all the appropriate advice related to CBN component during the field practice. Most of the HEWs reported that there was no supportive supervision for the last four months. In conclusion; the training that were given in four selected woredas of Amhara region were not quality wise and as designed. The IRT of nutrition component lack reporting and monitoring format. In all nutrition components of IRT and the allocated time for training is too short. The IRT of nutrition component is also not adequate for health extension workers to accomplish community based nutrition program. Thus, there should be additional training for the health extension workers.

IntroductionThis paper synthesises evidence on the organisation of primary health care (PHC) service delivery in low-income and middle-income countries (LMICs) in the Asia Pacific and identifies evidence of effective approaches and pathways of impact in this region.MethodsWe developed a conceptual framework describing key inputs and outcomes of PHC as the basis of a systematic review. We searched exclusively for intervention studies from LMICs of the Asia-Pacific region in an effort to identify ‘what works’ to improve the coverage, quality, efficiency, equity and responsiveness of PHC. We conducted a narrative synthesis to identify key characteristics of successful interventions.ResultsFrom an initial list of 3001 articles, we selected 153 for full-text review and included 111. We found evidence on the impact of non-physician health workers (NPHWs) on coverage and quality of care, though better integration with other PHC services is needed. Community-based services are most effective when well integrated through functional referral systems and supportive supervision arrangements, and have a reliable supply of medicines. Many studies point to the importance of community engagement in improving service demand. Few studies adopted a ‘systems’ lens or adequately considered long-term costs or implementation challenges.ConclusionBased on our findings, we suggest five areas where more practical knowledge and guidance is needed to support PHC systems strengthening: (1) NPHW workforce development; (2) integrating non-communicable disease prevention and control into the basic package of care; (3) building managerial capacity; (4) institutionalising community engagement; (5) modernising PHC information systems.

Background Integrated Community Case Management (iCCM) is a strategy for promoting access of under-served populations to lifesaving treatments through extending case management of common childhood illnesses to trained frontline health workers. In Ethiopia iCCM is provided by health extension workers (HEWs) deployed at health posts. We evaluated the association between the implementation of iCCM program in Assosa Zuria zone, Benishangul Gumuz region and changes in care-seeking for common childhood illnesses. Methods We conducted a pre-post study without control arm to evaluate the association of interest. The iCCM program that incorporated training, mentoring and supportive supervision of HEWs with community-based demand creation activities was implemented for two years (2017–18). Baseline, midline and endline surveys were completed approximately one year apart. Across the surveys, children aged 2–59 months (n = 1,848) who recently had cough, fever or diarrhea were included. Data were analysed using mixed-effects logistic regression model. Results Over the two-year period, care-seeking from any health facility and from health posts significantly increased by 10.7 and 17.4 percentage points (PP) from baseline levels of 64.5 and 34.1%, respectively (p<0.001). Care sought from health centres (p = 0.420) and public hospitals (p = 0.129) did not meaningfully change while proportion of caregivers who approached private (p = 0.003) and informal providers (p<0.001) declined. Caregivers who visited health posts for the treatment of diarrhea (19.2 PP, p<0.001), fever (15.5 PP, p<0.001), cough (17.8 PP, p<0.001) and cough with respiratory difficulty (17.3 PP, p = 0.038) significantly increased. After accounting for extraneous variables, we observed that care-seeking from iCCM providers was almost doubled (adjusted odds ratio = 2.32: 95% confidence interval; 1.88–2.86) over the period. Conclusion iCCM implementation was associated with a meaningful shift in care-seeking to health posts.

Building on the gains of the National Health Mission, India’s Reproductive, Maternal, Newborn, Child and Adolescent Health (RMNCH+A) Strategy, launched in 2013, was a milestone in the country’s health planning. The strategy recognised the interdependence of RMNCH+A Interventions across the life stages and adopted a comprehensive approach to address inequitable distribution of healthcare services for the vulnerable population groups and in poor-performing geographies of the country. Based on innovative approaches and management reforms, like selection of poor-performing districts, prioritisation of high-impact RMNCH+A healthcare interventions, engagement of development partners and institutionalising a concurrent monitoring system the strategy strived to improve efficiency and effectiveness within the public healthcare delivery system of the country. 184 High Priority Districts were identified across the country on a defined set of indicators for implementation of critical RMNCH+A Interventions and a dedicated institutional framework comprising National and State RMNCH+A Units and District Level Monitors supported by the development partners was established to provide technical support to the state and district health departments. Health facilities based on case load and available services across the High Priority Districts were prioritised for strengthening and were monitored by an RMNCH+A Supportive Supervision mechanism to track progress and generate evidence to facilitate actions for strengthening ongoing interventions. The strategy helped develop an integrated systems-based approach to address public health challenges through a comprehensive framework, defined priorities and robust partnerships with the partner agencies. However, lack of a robust monitoring and evaluation framework and sub-optimal focus on social determinants of health possibly limited its overall impact and ability to sustain improvements. Guided by the learnings and limitations, the Government of India has now designed the ‘Aspirational Districts Program’ to holistically address health challenges in poor-performing districts within the overall sociocultural domain to ensure inclusive and sustained improvements.

87 Background: Lay cancer patient navigators (LCPN) are volunteers from the community who assist cancer patients in overcoming barriers to receiving quality cancer care. LCPN can address issues such as. financial burden and psychosocial distress, and can reduce disparities in access to resources. There is little data regarding LCPN within academic medical centers. Methods: In 2013, 24 breast cancer patients at our center were surveyed to determine unmet needs. 82% reported feeling alone and 55% reported lack of awareness about available patient resources. These data supported the development of an LCPN program, in January 2015. Volunteers were provided with comprehensive training that focused on supportive communication, identification of barriers to care, and provision of available cancer resources to address unmet patient needs. Volunteers were integrated within clinical workflows to identify and interact with patients felt to potentially benefit from LCPN. Results: A total of 30 volunteer lay patient navigators were trained between January 2015 – June 2016. Of the 30, 14 elected to staff a weekly disease oriented clinic volunteering 3-4 hours per week. There have been 1,034 patient encounters with 484 encounters in new patients and 550 in repeat patients through June 2016. LCPN completed an encounter form after each patient visit. As a result of these encounters, referrals were provided to the following resources: patient and family resource center (74% ); community resource information (30%); financial counseling (27%); and social work (25%). LCPN report that they feel adequately trained to provide information and meet needs of cancer patients during these visits. Conclusions: We have developed an infrastructure to support an LCPN program within our academic medical center, and have found that this infrastructure facilitates provision of information to cancer patients that addresses their unmet needs. The model is cost-effective and requires few internal resources other than training and ongoing supervision. Future steps will include development of specific interventions to enhance the dissemination of this model throughout North Carolina.

Background Due to low care utilization, a complex intervention was done for two years to optimize the Ethiopian Health Extension Program. Improved quality of the integrated community case management services was an intermediate outcome of this intervention through community education and mobilization, capacity building of health workers, and strengthening of district ownership and accountability of sick child services. We evaluated the association between the intervention and the health extension workers’ ability to correctly classify common childhood illnesses in four regions of Ethiopia. Methods Baseline and endline assessments were done in 2016 and 2018 in intervention and comparison areas in four regions of Ethiopia. Ill children aged 2 to 59 months were mobilized to visit health posts for an assessment that was followed by re-examination. We analyzed sensitivity, specificity, and difference-in-difference of correct classification with multilevel mixed logistic regression in intervention and comparison areas at baseline and endline. Results Health extensions workers’ consultations with ill children were observed in intervention (n = 710) and comparison areas (n = 615). At baseline, re-examination of the children showed that in intervention areas, health extension workers’ sensitivity for fever or malaria was 54%, 68% for respiratory infections, 90% for diarrheal diseases, and 34% for malnutrition. At endline, it was 40% for fever or malaria, 49% for respiratory infections, 85% for diarrheal diseases, and 48% for malnutrition. Specificity was higher (89–100%) for all childhood illnesses. Difference-in-differences was 6% for correct classification of fever or malaria [aOR = 1.45 95% CI: 0.81–2.60], 4% for respiratory tract infection [aOR = 1.49 95% CI: 0.81–2.74], and 5% for diarrheal diseases [aOR = 1.74 95% CI: 0.77–3.92]. Conclusion This study revealed that the Optimization of Health Extension Program intervention, which included training, supportive supervision, and performance reviews of health extension workers, was not associated with an improved classification of childhood illnesses by these Ethiopian primary health care workers. This study uses a qualitative descriptive approach, data obtained through observation, documentation and in-depth interviews. Sources of data in this study were obtained through interviews on January 29 - February 7 2020 together with the Supervisior of the High School Dormitory, Musyrif, and board of the Santri Boarding Organization and SMA ICMBS Alumni as supporting data. The collected data were analyzed using descriptive analysis techniques. The results showed that in character building, the musyrif strategy is divided into two, namely (a) internal strategy includes six stages - planning, introduction, implementation, supervision, reward and punishment, and evaluation, and (b) external strategies namely introduction, supervision, and integrated evaluation through active communication with guardian parents in both formal and informal forms. So, it can be concluded that the musyrif strategy is very effective in shaping the character of leadership and student independence. Using semistructured interviews with 388 women under supervision, this study integrates criminal justice and communication theories by investigating gender responsivity and type of support in messages women receive about employment from supervision agents. Informational support was the most frequent form of supportive communication clients received from their agents, and was the only type of supportive communication clients perceived negatively. Women recalled agents’ messages that varied in their sensitivity to the range of women offenders’ needs (child and family care demands, human capital attainment, mental health issues, and substance abuse recovery). Supportive messages that took into account a variety of problems commonly shared by women on probation and parole had positive effects, whereas supportive messages that were relevant to employment, but failed to consider other needs, had negative effects. Results of this work have implications regarding effective support offered by community supervision agents as they discuss employment.

In the presence of increasing demands for manufacturing process optimization, the role of supervisory level of material handling systems is much more advanced today, ensuring not only data acquisition, visualization, monitoring, supervisory control, as well as synchronization with the higher control levels (FEM, ERP), but also providing functionality for supporting maintenance and decision-making processes to reduce downtimes, operations and maintenance costs. The article deals with the integration of control and maintenance functions in the hierarchical control system of a crane. The supervisory system for supporting control and proactive maintenance is prototyped at the laboratory overhead travelling crane. The article presents the control-measurement equipment and intelligent software tools implemented in the supervisory control and data acquisition (SCADA) system to aid decision-making process in proactive maintenance. The overview of the main components of the supervisory control and proactive maintenance subsystems is provided, and their respective role in control, supervision, and proactive maintenance is explained. The crane’s supervisory control includes the stereovision-based subsystem applied to identify the crane’s transportation workspace, determine the safety and time-optimal point-to-point trajectory of a payload. The proactive maintenance module consists of the human machine interface (HMI) supporting decision-making process, intelligent tools for upcoming downtime/failure prediction, and the crane's girder inspection using the metal magnetic memory technique.

This paper deals with the challenge of supervising PhD students. Any supervision is likely to constitute a challenging experience for the supervisor, even more so when they are a new academic staff member with little experience in PhD supervision in the Australasian context. This paper shows how one supervisor addressed the challenge by fostering a more collaborative research culture in her programme (Applied Linguistics) through peer group work, and can serve as a starting point for action for supervisors who are looking for possibilities to integrate their students into learning communities. The paper provides the theoretical rationale for peer learning in doctoral education and emphasizes the desirability for its implementation into supervisory practice from an educational perspective. The description of practice of one particular peer group allows for interesting insights into the genesis, activities, and self-evaluation of this group that emphasized the value of learning with and from each other through exchange, insight into the PhD process, feedback, moral support in a friendly, supportive environment, and research training. The paper concludes by discussing implications, and challenges of this study for practice, policy, and research, as well its limitations.

Background Epilepsy is a chronic disease characterized by periodic seizures that result from abnormal integrated firing impulses in the brain. It is one of the most common neurological disorders. Over the past few years, there has been increasing awareness about the effect that having a child with epilepsy has on parents and the reciprocal impact of parental knowledge and attitudes regarding epilepsy on the affected child. Objective This study aimed to assess parental knowledge, attitudes, and behavior toward their epileptic children. Methods A cross-sectional study was conducted in 2018 by the Pediatric Neurology Department of King Abdulaziz University Hospital, Jeddah, the Kingdom of Saudi Arabia. A sample size of 115 of 332 parents who have a child diagnosed with epilepsy and aged 18 years or younger were recruited for this study. Statistical analysis was performed using SPSS version 21. Data analysis was performed using an independent t test, a chi-square test, one-way analysis of variance, and correlation analysis. Results A total of 115 participants answered the questionnaire; of these, 65 (56.5%) were men, with an average age of 40.3 years, and the mean age of the children was 9.0 years. Overall, 85 (85/115, 73.9%) children were taken care of by both of their parents. The mean parental knowledge score was 7.49 (SD 2.08) out of 12, and it was significantly related to the educational level of the parent (P=.004). The knowledge question that was most frequently answered incorrectly was “Diagnosis of epilepsy is usually made based on at least two unprovoked seizures.” As only 28.7% (33/115) of participants chose the correct answer, mean parental attitude score was 26.51 (SD 4.284) out of 35, and there was no significant relation with the educational level of parents (P=.13); however, it was negatively correlated with the child’s age (P=.045). Mean parental behavioral score was 23.35 (SD 4.121) out of 35, and there was no significant relation with the educational level of the parents (P=.24). The most negatively answered question for the behavior section was “I can leave my child without supervision,” with a mean score of 2.25 (SD 1.09) out of 5. Gender did not play a significant role in parental knowledge, attitudes, or behavior (P=.44, P=.77, and P=.99, respectively). Conclusions Parental knowledge in our sample still needs improvement. Therefore, more awareness campaigns should be made for the community and for the parents of affected children to create a supportive environment for the children and help them thrive and develop.

Background and context: The Bridge of Support Program (BOS) is adapting a successful community based model of peer support to an acute setting, at the Sunshine Hospital Breast Clinic, to meet the cultural and socioeconomic diverse needs of women with breast cancer. This two year project was funded by LUCRF Community Partnership Trust. Aim: To improve the experience of women undergoing treatment of breast cancer at Western Health. To improve access to psych-social, emotional and practical support for women by extending the reach of CounterPart- a state-wide service of women´s Health Victoria, funded by the Victorian Department of Health and Human Services to provide peer support and information to people affected by breast or a gynecologic cancer. Program/Policy process: Peer support volunteers actively guided women to current and credible evidence-based information, support decision making and provide emotional support. Peer support volunteers are rostered once a week at Sunshine Hospital to coincide with breast clinic and include access to the day oncology unit and radiotherapy center. Detailed contact information is recorded and women receive follow-up contact (with consent) from the peer support volunteers at the CounterPart Resource Centre in Melbourne. A CounterPart staff member oversees the project and provides direct support and supervision to the volunteers on site at the hospital. Women can self refer. Outcomes: Between February and December 2016 the BOS program had 159 separate contacts with patients and their families. 82 contacts were with men and women who were new to CounterPart and 77 were follow-up contacts. 90 individuals treated for breast cancer at Western Health accessed the program, which represent 53% of the women seen by the breast service. 48% of the contacts were follow-up contacts with the CounterPart volunteers indicating that once engaged with the service many men and women continue to make contact. 38% of contacts were with women diagnosed with metastatic disease, a group who often have higher levels of unmet or more complex needs. 49% were born in a nonmain English speaking country (compared with the overall state of Victoria average of 19.6%) thus reflecting an accessible service to the non-English speaking community. What was learned: At a time when peer support is being increasingly recognized as a key part of effective supportive care in cancer services, the BOS program offers a model of integrated peer support that is respected, reliable, well supported and safe within the acute setting. This acute-community sector partnership demonstrates how the medical and social models of health care can work together to provide a connected and quality service for men and women diagnosed with breast cancer. An active research approach is enabling the project to be responsive to issues and challenges as they arise including the ongoing recruitment of women as volunteers from the local community to work within the acute setting.

The lack of supervision and the absence of a system in integrating the supporting data resulted in uneven distribution of support (both assignments and points obtained) and the absence of track records for lecturers. So that it requires a proposed business process design, blueprint document design, and the design of a user interface that is in accordance with the penunjang mechanism based on Tel-U Point in the Telkom Industrial Engineering Faculty with interesting and accessible visualization anywhere. With the design of this website-based information support mechanisms, it is expected that the control holders in each process can be clearly illustrated, supporting data can be well integrated, and data filling and storage can be automated in the system.

This study used a qualitative approach to techniques of data collection through interviews and in-depth study of the documentation. Formulation of the problem in this study were (i) How the system of supervision of the management of the fish market in Lappa Sinjai Regency? (ii) What is the determinant factor against the supervision management of fish market in Lappa Regency Sinjai? (iii) How the strategy of increased oversight of employee administration fish market in Sinjai Regency? The results of this study indicate that (i) the supervisory management system of fish market in Sinjai Regency, yet effective, this occurs because the implementation of the fish market service does not yet have an integrated surveillance system and its management do not involve private parties such as Village Units Economic Activities. The scrutiny has not shown optimal results due to the surveillance system management is not yet a professional, transparent and accountable so that efforts increased own-source revenue do not meet the expectations of the public. (ii) Factor determinants include the factor of human resources, the commitment of the apparatus, competence and leadership style. Salary/incentive factors, security, infrastructure and ecology as a factor in increasing the income supporting the own-source revenue through the levy management of fish market in Sinjai Regency. Background: Despite promising findings regarding the safety, fidelity, and effectiveness of peer-delivered behavioral health programs, there are training-related challenges to the integration of peers on health care teams. Specifically, there is a need to understand the elements of training and consultation that may be unique to peer-delivered interventions. Methods: As part of a pilot effectiveness-implementation study of an abbreviated version of Skills Training in Affective and Interpersonal Regulation (STAIR) for posttraumatic stress disorder (PTSD), we conducted a mixed-methods process evaluation utilizing multiple data sources (questionnaires and field notes) to characterize our approach to consultation and explore relations between fidelity, treatment outcome, and client satisfaction. Results: Peer interventionists exhibited high fidelity, defined by adherence ( M = 93.7%, SD = 12.3%) and competence ( M = 3.7 “competent,” SD = 0.5). Adherence, β = .69, t(1) = 3.69, p < .01, and competence, β = .585, t(1) = 2.88, p < .05, were each associated with trial participant’s satisfaction, but not associated with clinical outcomes. Our synthesis of fidelity-monitoring data and consultation field notes suggests that peer interventionists possess strengths in interpersonal effectiveness, such as rapport building, empathy, and appropriate self-disclosure. Peer interventionists evidenced minor challenges with key features of directive approaches, such as pacing, time efficiency, and providing strong theoretical rationale for homework and tracking. Conclusion: Due to promise of peers in expanding the behavioral health workforce and engaging individuals otherwise missed by the medical model, the current study aimed to characterize unique aspects of training and consultation. We found peer interventionists demonstrated high fidelity, supported through dynamic training and consultation with feedback. Research is needed to examine the impact of consultation approach on implementation and treatment outcomes. Plain Language Summary: Peers—paraprofessionals who use their lived experiences to engage and support the populations they serve—have been increasingly integrated into health care settings in the United States. Training peers to deliver interventions may provide cost savings by way of improving efficient utilization of professional services. Despite promising findings in regard to safety, intervention fidelity, and effectiveness of peer delivery, there are important challenges that need to be addressed if peers are to be more broadly integrated into the health care system as interventionists. These include challenges associated with highly variable training, inadequate supervision, and poor delineation of peer’s roles within the broader spectrum of care. Thus, there is a need to understand the unique components of training and consultation for peers. We report key findings from an evaluation of a pilot study of an abbreviated version of Skills Training in Affective and Interpersonal Regulation (STAIR) for posttraumatic stress disorder (PTSD), adapted for peer delivery. We characterize our approach to consultation with feedback and explore relations between fidelity, treatment outcome, and client satisfaction. Our study extends the small yet growing literature on training and consultation approaches to support fidelity (adherence and competence) among peer interventionists. Organizations hoping to integrate peers on health care teams could utilize our fidelity-monitoring approach to set benchmarks to ensure peer-delivered interventions are safe and effective.

The goal of this metasynthesis study was to employ qualitative research studies to explore the experiences of interpreters working in hospitals or mental healthcare facilities. Using Noblit and Hare’s meta-ethnographic strategy, findings from six qualitative articles were reduced to seven main themes: (1) technicalities of interpreting, (2) balancing objectivity versus supporting and identifying with the patient, (3) emotionally charged stories, (4) workplace stress, (5) interpreters’ recommendations, (6) coping strategies, and (7) integrating the past with existential growth. Practice recommendations include examining the workplace culture for attitudes that devalue interpreters. To reduce secondary trauma, it is recommended that interpreters become integrated team members with access to supervision and stress management, in order to build coping strategies. Interpreters suggested implementing preparation time and debriefing time, as well as ongoing training and professional development. However, these suggestions raise complicated administrative issues surrounding fee-for-service reimbursement.

Summary Objectives: A significant portion of care related to cardiorespiratory diseases is provided at home, usually but not exclusively, after the discharge of a patient from hospital. It is the purpose of the present study to present the technical means which we have developed, in order to support the adaptation of the continuity of care of cardiorespiratory diseases at home. Methods: We have developed an integrated system that includes: first, a prototype laptop-based portable monitoring system that comprises low-cost commercially available components, which enable the periodical or continuous monitoring of vital signs at home; second, software supporting medical decision-making related to tachycardia and ventricular fibrillation, as well as fuzzy-rules-based software supporting home-ventilation optimization; third, a typical continuity of care record (CCR) adapted to support also the creation of a homecare plan; and finally, a prototype ontology, based upon the HL7 clinical document architecture (CDA), serving as basis for the development of semantically annotated web services that allow for the exchange and retrieval of homecare information. Results: The flexible design and the adaptable data-exchange mechanism of the developed system result in a useful and standard-compliant tool, for cardiorespiratory disease-related homecare. Conclusions: The ongoing laboratory testing of the system shows that it is able to contribute to an effective and low-cost package solution, supporting patient supervision and treatment. Furthermore, semantic web technologies prove to be the perfect solution for both the conceptualization of a continuity of care data exchange procedure and for the integration of the structured medical data.

Aim/Purpose: This paper describes the context, development, implementation, and the potential transferability of an integrated online research environment that allows its users to conduct all aspects of research online. Background: While the content of most traditional courses can be delivered online and learning outcomes can be achieved by adopting equivalents to face-to-face pedagogic approaches, certain courses, such as those that require a substantial research component, present significant constraints for delivery online. To overcome these limitations, Australia’s largest university developed and implemented a Research Portal. Methodology: The development team conducted a functional requirements analysis, identified the components that would be necessary to meet user needs, and reviewed existing solutions. The Monash University Psychology Research Portal was designed, tested, developed, tested for user experience, implemented, and reviewed. The Research Portal is structured according to the usual research sequence and provides users with access to supporting information and integrated capacities including research supervision, participant acquisition, and data collection, analysis, and storage. Contribution: This integrated online research environment is equivalent to and, in some ways, superior to an on campus/non-online research capacity. Findings: The usage of the Research Portal has grown rapidly and has satisfied the requirements and met the research needs of students of an online course that includes a research project, providing a common, ubiquitously accessible, and integrated online research environment. Recommendations for Practitioners: Further refinement, personalization, and expanded implementation and dissemination of the Research Portal components are required. The formation of networks and working partnerships to operate, maintain, and promote Research Portal initiatives is recommended to reduce operational costs, increase access, and create an impact. Impact on Society: The Research Portal is an efficient resource that provides easy access to a standard interface at any time and from anywhere and can potentially be used by other online research courses, as well as by online and non-online researchers.

This article reports findings from an independent audit and evaluation of an innovative children's services programme (CSP) funded 2009-2011 by twelve Local Authorities under the auspices of the North-East of England's Improvement & Efficiency Partnership (NEIEP) to manage improvements and efficiencies across the children's services sector focusing upon messages for social workers. The overall purpose of the CSP was to make demonstrable progress in tackling the challenges of child poverty by delivering high quality support to the growing number of children with social care/health needs through regional improvement. The key objectives of the CSP included supporting workforce reform and integrated working; development of personalised services; family support to reduce the need for residential care; and provision of tools to aid commissioners with needs analysis. The audit identified key outputs, for example, improvements to best practice on Whole Family approaches, safeguarding and leadership training evolved through a skills framework; and included a regional model of social work supervision training along with a provision of options to increase the range and quality of foster care placements. The evaluation considered actions arising from the above findings, including demonstrated improvements to inter-disciplinary working and pooling resources to produce better outcomes for families; setting up a data-base to improve the balance between fostering, residential care and family support; and creating opportunities for social workers to explore the practical implementation of using personal budgets.

Design – Qualitative and quantitative text analysis. Setting – Learning Centre at Oslo University College and student work placements in Oslo, Norway. Subjects – Approximately 400 students enrolled in the undergraduate nursing degree programme. Methods – The author is a librarian and project manager of the Langerud project, an initiative wherein nursing students were jointly trained and supervised by nurse educators, nurse supervisors, and librarians in preparation for and during work placements over an eight-week period. In this role, the librarian author collected 36 student group assignments, 285 blog/wiki comments from students, nurse educators, nurse supervisors, and librarians, and 102 individual student logs written during six work placements between Spring 2010 and Spring 2012, which were posted in a learning management system (LMS), as well as in an evaluation form from Spring 2010. The unstructured text is analyzed according to how the students fulfilled the learning outcome of integrating steps zero to four of the seven-step EBP model: (1) Cultivate a spirit of inquiry; (2) Ask clinical questions in the PICO format; (3) Search for the best evidence; (4) Critically appraise the evidence; and (5) Integrate the evidence with clinical expertise and patient preferences and values. The logs are also analyzed quantitatively to measure if and how many students combined the three aspects of EBP - defined as being the practitioner’s individual expertise, best research evidence, and client values and expectations. Lastly, the author seeks to evaluate the role of the LMS as a mediating tool. Main Results – The author found that the majority (83%) of students successfully met the learning outcome, particularly for steps 1, 2, and 5. For step three, the author observed that some students did not apply PICO in the information-seeking process and were thus not sufficiently thorough in their searching. For step four, the author found that most students failed to demonstrate critical appraisal of the evidence and that many struggled to find up-to-date research findings. The author noted that the results for both steps three and four could be attributed to the students finding international databases and English-language research articles too challenging, given the language barrier. The author’s analysis of the logs reveals that two-thirds of the students combined the 3 aspects of EBP and that 39% described 1 or 2 aspects, of which most described user-based knowledge and experience-based knowledge. One department produced twice as many log entries as the other seven departments; in this department, students were able to choose what aspect of EBP to focus on and the librarian had a co-teaching role in that learning group. Overall, 60% of all students described research-based knowledge, which increased over time from 46% in Spring 2011, to 60% in Autumn 2011, and 83% in Spring 2012. On the evaluation form from Spring 2010, most students rated the supervision by and satisfaction with the nurse educator, nurse supervisor, and librarian as good, very good, or excellent, and many commented that the LMS was a useful learning platform. Conclusion – The author concludes that the project had a positive impact on students’ preparedness for work placements and that the early educational intervention improved IL and EBP competencies. Furthermore, the working relationship between the Nursing Department and Library was strengthened. After the Langerud project ended, the curriculum was revised to add more searching for research-based information in written assignments. Additionally, a lecture on EBP was developed based on real-life experiences from the project and delivered collaboratively by the project’s manager, a nurse educator, and a librarian.

Abstract We conducted an implementation research study to integrate a holistic package of physical health, mental health and psychosocial care for podoconiosis, lymphatic filariasis and leprosy into routine healthcare in Gusha cluster, Guagusa Shikudad district, northwest Ethiopia. The healthcare package included training patients in lower limb hygiene and skin care and provision of shoes, hygiene supplies and medication. The implementation activities included training events, workshops, awareness raising, self-help groups, supportive supervision, staff secondments and advisory board meetings. The cost of implementing the care package in Gusha cluster, with a population of 30 558 people, was 802 655 Ethiopian birr (ETB) (£48 159) and the cost of delivering care to 235 participants was 204 388 ETB (£12 263), or 870 ETB (£52) per person. There was a 35% decrease in the mean disability scores (measured using the World Health Organization Disability Assessment Schedule 2.0) and a 45% improvement in the dermatology-specific quality of life (measured using the Dermatology Life Quality Index) at the 3-month follow-up compared with baseline. There were reductions in the number of days with symptoms, days off usual activities/work and days with reduced activity due to illness, all of which were statistically significant. Our pilot suggests that integration of the care package into routine healthcare in Ethiopia may be effective in improving health-related quality of life and disability and reducing time out of economic activity due to illness.

INTRODUCTION: Māori perspectives should be genuinely represented and integrated into social work education to ensure Māori and non-Māori social work students alike are prepared for working effectively in Aotearoa New Zealand. In field education, Māori students may have particular needs and expectations that should be considered by academic staff and placement host organisations. Consequently, the placement experience for Māori students should reflect these needs and expectations.METHOD: As part of a wider research project which aimed to advance the quality of social work placements for Māori and Pasifika students, a hui was undertaken with a roopu (Māori branch) of the Aotearoa New Zealand Association of Social Work (ANZASW) in late 2014. This article focuses on their views of what constitutes a quality placement for Māori social work students. The project explored two key areas: what does a quality placement look like for Māori social work students and what can tertiary institutions do to better support Māori students to have a quality placement?FINDINGS AND CONCLUSIONS: Participants emphasised the usefulness of placement preparedness, clear expectations and open communication prior to, and during, placement. The placement should also be culturally safe and adequately challenge the student. Tertiary providers should support the student’s placement by being in regular face-to-face contact; preparing the student for the placement environment; supporting external cultural supervision; and by critically reviewing their curriculum.

In this article, I have adopted a holistic methodology to reflect my teaching and supporting learning experience (almost 35 years) in higher education as inspired from the SFHEA, UK. Evidence of my successful engagement across all five areas of activity will be provided that include firstly, design a series of teaching and learning activities to meet the learning outcomes of a module. Secondly, use a range of teaching approaches to enhancing student learning. Thirdly, use a variety of assessment for learning activities for students and providing feedback to promoting student engagement. Fourthly, develop effective learning environments and approaches to student support and guidance. Fifthly, engage in continuing professional development (CPD) in subjects /disciplines and their pedagogy, incorporating research, scholarship and the evaluation of professional practices. Moreover, I show my understanding of the appropriate core knowledge and demonstrating my commitment to the professional values. This paper contributes by providing and demonstrating a thorough understanding of effective approaches to learning and teaching support as a key contribution to high quality student learning. Relevance. Aging of the power equipment in railway power supply systems sharply raised a need for assessment of its states and degree of risk for operation outside rated service life. In critical conditions of technological processes and operational modes of the railways it is necessary to increase the equipment’s operational reliability. The scheduled maintenance and repair system whose main technical and economic criterion is the minimum of equipment’s downtimes on the basis of a rigid regulation of repair cycles, in the conditions of market regulations in the field of repair in many cases does not provide the optimal decisions due to insufficient financing. The solution of this problem is possible by improvement of the maintenance and repair system. Under these conditions the main direction for supporting the operational reliability of power electric equipment on TS is a development of the modern methods based on individual supervision over real changes of technical condition of power equipment. Scientific novelty. In this article the authors proposed an integrated approach, on the basis of which can be developed the effective maintenance and repair system for traction power supply systems. Proposed approach allowed to react quickly to changes of service conditions on traction substations, to control the technical condition of power electric equipment under the conditions of uncertainty, to establish interrelation between quality of service and operational reliability of the equipment, to choose a service strategy on traction substations. Practical importance. The validity of the developed method was confirmed by the results of calculations and practically by choosing the optimal maintenance's option for transformer TDTN-25000/150-70 U1 (ТДТН-25000/150-70 У1) on traction power supply substation.

Penelitian ini bertujuan untuk mengetahui bagaimana kreativitas guru pada pembelajaran tematik integratif, faktor-faktor dan upaya untuk meningkatkan kreativitas guru pada pembelajaran tematik integratif. Penelitian ini menggunakan pendekatan kualitatif dengan jenis studi kasus di Kelas 1 SD Anak Saleh Malang. Instrumen penelitian ini adalah observasi, wawancara, dan dokumentasi. Teknik analsis data yang digunakan adalah reduksi data, penyajian data, dan verifikasi. Hasil penelitian menemukan bahwa kreativitas guru dalam proses pembelajaran tematik integratif di kelas I SD Anak Saleh Malang cukup baik, karena pada umumnya guru kelas telah mampu membuat rencana pembelajaran yang kreatif, melaksanakan kegiatan pengajaran dengan baik, dan mengembangkan berbagai bentuk evaluasi pembelajan yang yang bervariasi. Faktor pendukung kreatifitas guru dalam pembelajaran adalah; keaktifan guru dalam komunitas profesi dan mengikuti kegiatan pelatihan, motivasi berprestasi, dan rekan kerja guru yang ramah dan peduli, dan kelengkapan sarana pembelajaran di sekolah. This study used a qualitative approach with the type of case study at first classroom of Elementary School Anak Saleh Malang. The instruments were observation, interviews, and documentation. The technique of data analysis used were reduction, display, and verification.The results of the study found that the teacher's creativity in the integrative thematic learning process at first class of Elementary School Anak Saleh Malang is quite good, because in general the classroom teacher has been able to make creative learning plans, carry out teaching activities well, and develop various forms of learning evaluation that vary. Factors supporting teacher creativity in learning are; teacher activity in the professional community and participating in training activities, achievement motivation, and friendly and caring teacher work colleagues, and complete learning facilities at school. The inhibiting factors of teacher creativity are: class teacher education that is not linear, lack of appreciation from superiors, and teacher welfare which is still minimal. The school's efforts to increase teacher creativity in thematic learning are: developing teacher competencies, encouraging teachers for further study, providing intensive supervision, and driving a conducive work atmosphere.Keywords: Teacher Creativity, Integrative Thematic Learning

Abstract:
Purpose – The purpose of this paper is to present the Early Childhood-Socio-Emotional and Behavior Regulation Intervention Specialist (EC-SEBRIS) Certificate Program model integrating knowledge and practice. Coursework, videotaping, on-site coaching, and the reflective process facilitate the application of knowledge through the integration of theory and practice supporting young children and families. It is designed to help teachers and early childhood professionals to internalize the skills and competencies needed to address challenging behaviors in their classrooms or at homes so that they can meet the critical social-emotional and behavioral needs of children. Design/methodology/approach – The paper will provide: research-based information to establish the need for such training programs; the rational to the conceptual framework of the EC-SEBRIS Certificate Program; the depiction of the wraparound training model, which uses triple coaching and mentoring methods: reflective supervision, videotaping, and on-site coaching for teachers; and a preliminary evaluation of the program, and future plans. Findings – The EC-SEBRIS Certificate Program is at its early implementation stage. The author have graduated four cohorts, for a total of 113, early childhood professionals who have been hired to enhance the behavioral health services provided to young children and families. Overall, results suggest that the students’ knowledge base and confidence improved from time 1 to time 2 for each of the knowledge-based courses. Research limitations/implications – This is a conceptual paper. Practical implications – The program bridges the early childhood education (ECE) and early childhood mental health fields, and recognizes the important role that early care and education professionals play in the socio-emotional development of young children. Coursework, videotaping, on-site coaching, and the reflective process facilitate the application of knowledge through the integration of theory and practice in the field supporting young children and families. Social implications – Challenging behaviors are one of the issues all early childhood educators have to deal within their classrooms. The model, as presented in the manuscript, can be implemented in other higher education institutions to enhance the skill sets of professionals who need to respond to this critical need and support healthy development of young children. Originality/value – This is an original model emphasizing the importance of training early childhood educators to support emotion and behavior regulation in young children. The paper presents a cutting edge teaching model which integrates knowledge, practice, and reflective practice. This training model focuses on the ECE workforce as the front line to the provision of early childhood mental health support.

In the context of the rapiddevelopment of the latest technologies, such as big data, artifi cial intelligence, cloud computing andblockchain, as well as the continuous development of the digital economy, digital data plays an important role in the development of enterprises. Data security issues affect the development of a countryand its security, are related to public interests, and are also closely related to the personal rights of its citizens. It is necessary to regulate data protection at the legal level.July 2, 2020 the website of the National People’s Congress published the Data Security Law of the People’s Republic of China (draft), which systematically refl ects the current national point of view on data security and development, noting the improvement of data security to the level of state security. Like the Cybersecurity Law, it is a component of the general concept of national security, belongs to the highest level of law and has the highest legal force. This law is of great strategic importance for building a data protection system in the PRC.As the main law in the fi eld of data security management ,the Data Security Law has made certain breakthroughs in the defi nition of the concept of data. The concept of data in the Data Security Law is not limited to network data, but includes information recorded in electronic form and in other ways; the necessary extraterritorial consequences are provided for in the draft Data Security Law, which is of great importance for the protection of national sovereignty and civil rights of the People’s Republic of China; the relationship between data security protection, data development and use is clarifi ed, and the status of data in the development of the digital economy is clarifi ed through legislation in order to fully demonstrate the economic value of data; the main body responsible for security, supervision and overall coordination has been created, the main responsibilities of various regions and departments have been increased, supervision responsibilities have been redistributed, which is more in line with the needs and current situation in data security supervision; a data classifi cation system has been established that promotes the relevant work of data security surveillance agencies; an appropriate mechanism has been created at the state level in the system of data security risk assessment, reporting, information exchange, monitoring and early warning; a data export control system and a mutual data protection system were created, as well as the construction of a cross-border data fl ow system of the PRC was standardized; data protection obligations in organizations and for individuals performing data operations and performing data protection duties were clarifi ed; adhere to the principles of security and development and provide measures to support and promote data security and development; institutional measures have been taken to ensure the security of government data.The draft law on data security contains a comprehensive concept of information security. The upcoming Law on Data Security will contain recommendations and basic principles for ensuring data security and development. All industries will have to pay attention to an integrated management system and various supporting provisions to ensure data security and development. Gradually, a favorable environment will be created for the effective use of data and safe development, which will signifi cantly contribute to the rapid development of informatization and digitization.The Data Security Law includes all types of information in the fi eld of protection. In the future, the Law on Data Security should pay attention to the Law on Cybersecurity, Law on State Security and Law on the Protection of Personal Information, which are under development, supplement and improve today’s relatively disparate legislation related to data security. The laws and regulations of the PRC on data security will be carefully developed on the basis of these three laws in order to fully implement the development, use of data and industrial development, thereby promoting new provisions in the creation of a legal data security system.

Abstract:
The decline in under-five mortality remains target of health development in Indonesia. One effort that can be done, among others, is to improve the skills of health workers in dealing with sick children through the Integrated management of Chilhood Illness (IMCI). This study aims to evaluate the implementation of IMCI in 10 selected districts/cities in Eastern Region of Indonesia with a sample of 20 puskesmas selected randomly. In total 40 under-five children were observed when receiving IMCI services at the puskesmas. In addition, an assessment of the completeness of filling out of 200 forms of IMCI under-five children who had come to the puskesmas a week before the survey was conducted. Information related to the availability of equipment to support IMCI services is collected through direct observation in 20 selected puskesmas assisted by a check list form. The results showed that 80% of puskesmas in the eastern region have implemented IMCI, but only 25% of puskesmas reaching all the under-five children. As many as 90% of puskesmas have been trained for IMCI, however only 15% have been monitored post training. Only 25% of puskesmas received supervision from the District Health Office in implementing IMCI. The observation results at the IMCI service for children under five showed that, the lowest score for compliance with IMCI was counseling (25.8%) and the highest was diarrhea assessment (73.8%). The results of observing the IMCI forms showed that the lowest score was feeding practice (30.4%) and repeat visits (30.8%). Meanwhile, oral rehydration facilities for diarrhea are reported to be inadequate, because they are only available at 50% of puskesmas. There needs to be monitoring and supervision of officer compliance and increasing the availability of supporting equipment and facilities/insfrastructure in the implementation of IMCI. Abstrak Penurunan angka kematian balita masih menjadi target pembangunan kesehatan di Indonesia. Salah satu upaya yang dapat dilakukan antara lain meningkatkan keterampilan tenaga kesehatan dalam menangani balita sakit, melalui pendekatan Manajemen Terpadu Balita Sakit (MTBS). Penelitian ini bertujuan untuk mengevaluasi pelaksanaan MTBS di 10 Kabupaten/Kota terpilih di regional timur, dengan jumlah sampel 20 puskesmas yang dipilih secara acak. Secara total, 40 pasien balita diobservasi pada saat mendapatkan pelayanan MTBS di puskesmas. Selain itu, dilakukan asesmen kelengkapan pengisian dari 200 formulir MTBS balita yang pernah datang ke puskesmas dalam kurun waktu seminggu sebelum survei. Infomasi terkait dengan ketersediaan peralatan untuk mendukung pelayanan MTBS dikumpulkan melalui observasi secara langsung di 20 puskesmas terpilih dibantu dengan formulir check list. Hasil analisis menunjukkan bahwa 80% puskesmas di regional timur telah melaksanakan MTBS, namun hanya 25% puskesmas yang menjangkau seluruh balita. Sebesar 90% puskesmas telah terlatih MTBS, namun hanya 15% yang dilakukan monitoring pasca pelatihan. Hanya 25% puskesmas yang mendapatkan supervisi dari Dinas Kesehatan Kabupaten/Kota dalam pelaksanaan MTBS. Hasil observasi pada saat pelayanan MTBS pada balita menunjukkan, skor kepatuhan pelaksanaan MTBS yang terendah adalah konseling (25,8%) dan tertinggi adalah asesmen diare (73,8%). Hasil observasi pengisian formulir MTBS menunjukkan, skor terendah pada pengisian pemberian makan (30,4%) dan kunjungan ulang (30,8%). Sementara itu, fasilitas rehidrasi oral untuk diare dilaporkan belum memadai, karena hanya tersedia di 50% puskesmas. Perlu adanya monitoring dan supervisi terhadap kepatuhan petugas serta peningkatan ketersediaan peralatan dan sarana/prasarana pendukung dalam pelaksanaan MTBS.

International medical graduate (IMG) specialists in anaesthesia need education to be part of the assessment process for pre-registration college fellowship. Fellowship of the anaesthetic college is required in Australia for registration as a specialist in this field. Marked differences exist between local trainees and IMG specialists in terms of training, stakes of the exam and isolation of practice. We have examined the reasons for the low pass rate for IMG specialists compared to the local trainees in the Australian and New Zealand College of Anaesthetists (ANZCA) final fellowship examinations. We also offer an IMG specialists’ view of this perceived problem. It highlights their difficulties in obtaining adequate supervision and education. What is known about the topic?There has been a worldwide shortage of doctors over the last decade. In Australia this shortage has been attributed to government policy in the 1990s limiting the number of medical school places. Other factors that may have contributed to this shortage are changes in the practice of medicine, increasing specialisation, growth in population and patterns of population settlement at the coastal fringes of Australia. The use of international medical graduates and reliance on them is associated with several problems and challenges. A key factor relates to their performance at a standard acceptable to the country. What does the paper add?This paper offers an examination of the issues that present to IMG specialists located at rural and remote areas of Australia. The global aim of this study is to understand the workforce education issues that present to IMG specialists as a basis for supporting this group, having migrated to Australia, to better prepare for assessment of their practice in this country. Results of a survey of IMG specialists in Anaesthesia are included to contribute to an overall view. It highlights their understanding of the issues that present when preparing for specialist assessments. What are the implications for practitioners?This information will be useful for policy practitioners who determine critical elements that influence workforce planning and education support. Decision makers will be able to make more informed decisions on the need to integrate education into planning for workforce efficiencies. There are currently no published data explaining why the pass rate for IMG specialist in anaesthesia is so different from local trainees and this paper also offers a viewpoint of present issues from those who are attempting these examinations.

In pace with the technical progress in controllability of mechatronic systems including machines and industrial equipment, the systems of industrial controllers (both PLC and microprocessor ones) more and more frequently use Ethernet-based networks for communication with supervising centres and surveillance systems. The Internet offers unsurpassed opportunities of remote programming as well as remote development, debugging and tuning the existing control software. Nowadays, supporting the remote tools and facilities is the essential requirement that is mandatory when decisions on purchase and implementation of industrial controllers are made. It is the underlying reason to launch more extensive research in that field. The presented paper describes dedicated software that has been developed to enable communication over the Internet within dispersed control systems. The system makes it possible to transmit and to receive short messages to and from the controlled actuators as well as to perform basic tasks related to management of data flow in networks that use TCD and UDP protocols. The special attention was paid to dynamic phenomena of the data exchange process. It is an issue of crucial importance within dispersed systems of industrial controllers and it assures efficient operation of the entire system owing to timely and quick respond to fast-changing control signals. Data exchange was carried out with the use of so-called primitives for Berkeley sockets that serve as primary structures within the network and are capable to perform basic operation such as creation and destruction, assigning network addresses to the sockets, establishing connections, transmission (broadcasting), receiving, etc. To measure time intervals of communication sessions the authors took advantage of functional features of contemporary motherboards of PC computers. In particular, the function of the API counter was used as it allows to readout the fast internal 64-bit counter which, in consequence, enabled measurement of time gaps with accuracy up to single microseconds. The described software performs tests of communication facilities in terms of their applicability to fast data exchange between field control modules of the control system and the CPU, whereas the entire communication is performed via Internet. Therefore the reaction time of a hypothetical field controller in respond to switchovers of the input signals or interrupt events can be measured. The communication and measurements were performed over local and national internet networks as well as for GPRS networks. Measurement results are presented in a compact form of tables that is suitable for further analysis. The presented system is able to transmit diagnostic information therefore it can be also used for integrated diagnostics of mechatronic systems as well as for location and analysis of possible failures within the in-field systems.

Abstract Tourism village is largely based in local wisdom is a form of integrated activities between attractions, accommodation, supporting facilities and additional facilities in the form of village management presented in the structure of community life that is bound by traditional procedures and norms that apply in the village. The development of sustainable tourism villages reguires the participation of local communities in all phases of development starting from the planning, implementation and supervision and control stages. Pujon Kidul Village in Pujon subdistrict, Malang regency is one of the tourist villages developed by the village community, which has tourism potential from the agriculture, livestock and small to mediun industries which is supported by the geographical and climatological conditions of the region. The purpose of this activity is to strengthen the promotion of village teourism based in local wisdom by identifying the potential of the Pujon Kidul village tourism area. Approach to the implementation of community service using the Rural Appraisal Approach (RAA) through a Group Discussion Forum (FGD) between the Pujon Kidul village apparatus, the Tourism Awarness Group (Pokdarwis) and the representatives of the village community. The results of this activity are strengthening promotion through a catalog of tourism village potentials and understanding of village afficials, the community and the private sector in planning the development of tourism villages through the utilization of the potential of geography, agriculture, animal husbandry and plantations in the tourism village of Pujon Kidul District of Pujon Malang Regency which begore socialization and counselling was 32% to 96%..
