Abstract:
The loss of the papilla can lead to cosmetic deformities, phonetic problems and lateral food impaction. Several reasons contribute to the loss of interdental papillae and the establishment of "black triangles" between teeth. In 1996, Han and Takei proposed a novel approach based on the use of a semi-lunar incision placed in the alveolar mucosa of the interdental area. The present case report, utilizing their approach, showed that within limits papillary reconstruction is achievable.

Abstract:
Introduction. The interdental papilla may be lost or reduced in height, forming black triangles due to various reasons, which gives an unaesthetic appearance when the patient smiles. Various noninvasive and invasive techniques have been used to augment/reconstruct the interdental papilla, to reclaim the pink triangle. The most satisfactory and natural appearance can be obtained by augmenting or reconstructing the lost papilla as the pink esthetics is as important as the white esthetics. Cases. Two female patients and 1 male patient reported to the dental department with the complaint of a small black gap in the gum area between their upper front teeth since 6 months and 1 year, respectively. On examination, the interdental papillae in all three cases were classified as class 1 (Nordland and Tarnow’s). The interdental papilla was augmented surgically by using three different approaches in each case. Result and Conclusion. Postoperatively and after 1-month follow-up, there was a complete fill of the interdental area by the interdental papilla normal (Nordland and Tarnow’s). As a result, the black triangle was successfully converted into a natural pink triangle in all three cases.

ABSTRACT Increasing esthetic demands of the modern era has converted modern dentistry to pink and white esthetic dentistry. One of the most difficult goals in the regeneration of the soft tissues is the reconstruction of interdental papilla. A number of techniques have been tried, but the results still lack predictability. Nonsurgical techniques include repeated curettage of the interdental papilla, orthodontic and restorative correction, and hyaluronic acid application, while surgical techniques include pedicle and free gingival graft, connective tissue grafts, and subepithelial connective tissue graft. This article presents a report of two cases where a combination of platelet-rich fibrin (PRF) and pedicle graft was utilized to retain maximum vascularity and minimize scar tissue formation. The atraumatic management of the tissues with a pouch-like design avoids tension and pressure and is critical for the success of the procedure. Abstract Aim The aim of these case reports was to evaluate the effectiveness of a subepithelial connective tissue graft with a coronally advanced flap to reconstruct an interdental papilla. Background In the last decade, esthetics has become a major concern in periodontal therapy. One of the most difficult and elusive goals for the periodontist in the esthetic aspect of periodontal therapy is the reconstruction of the interdental papilla. Consequently, the absence or loss of the interdental papilla for a variety of reasons may create significant esthetic, functional, and phonetic challenges for the patient. Case Description Five systemically healthy patients between the ages of 23 and 52 years with absence of the periodontal pocket were selected. Loss of interdental papillary height was classified using the Nordland and Tarnow classification system as class 1 to 4. All five patients received a subepithelial connective tissue graft (SCTG) with a coronally advanced flap (CAF). Four parameters were measured at baseline and at six months post-surgery: (1) vertical distance from contact point to gingival margin, (2) soft tissue (papilla) height in the interdental area, (3) horizontal distance at the gingival margin, and (4) width of keratinized gingiva. The changes noted at six months post-surgery were (1) the vertical distance decreased from 3.2±0.44 mm to 0.4±0.54 mm, (2) the mean gain in soft tissue (papilla) height increased from 3.2±0.44 mm to 5.8±0.44 mm, (3) the horizontal distance at the gingival margin was reduced from 2.6±0.54 mm to 0.4±0.54 mm and the width of the keratinized gingiva increased 1.4±0.89 mm. Conclusion The periodontal surgical technique used for the five reported cases successfully reconstructed the interdental papilla in just one type of papilla loss, the class I situation. Therefore, it should not be concluded that the technique shown would be equally as successful for every type of papilla loss. Furthermore, the postoperative time interval was short, at only six months. Clinical Significance A subepithelial connective tissue graft supported by a coronally advanced flap should be considered to surgically correct the loss of an interdental papilla in class I cases. Abstract:
BACKGROUND: Various techniques have been implemented to reconstruct the deficient interdental papilla.AIM: The present trial was conducted to assess the effect of injection of hyaluronic acid gel for the reconstruction of deficient interdental papilla.MATERIAL AND METHODS: Thirty-six deficient interdental papilla sites in ten patients were randomly allocated into two equal groups; intervention group who received the injection of hyaluronic acid (HA) gel and control group who received the injection of saline solution as a placebo. Each deficient papilla received three injections. The first injection was given one week following the re-evaluation period (four weeks after performing full mouth supra-gingival scaling and sub-gingival debridement). The second and third injections were given after three and six weeks, respectively. The height and surface area of black triangles were recorded at baseline before the injection procedures. The participants were recalled after three and six months from the first injection for re-measuring the recorded parameters. At 6 months, patients' satisfaction was also assessed.RESULTS: After three and six months from baseline, the results revealed a statistically significant higher mean decrease in height and surface area of black triangles in favour of the HA group. From three to six months, there was no statistically significant difference between the two groups in both parameters. At 6 months, the HA group showed a statistically significant higher mean satisfaction score than the saline group.CONCLUSION: The use of hyaluronic acid gel for the reconstruction of interdental papillary deficiency was effective with promising levels of patients' satisfaction.

O terço inferior da face assume extrema importância, no que diz respeito à percepção da estética facial de um indivíduo, estando o seu sorriso, em íntima relação com o conceito de beleza pessoal, assim como, com o bem-estar psicológico e interação social diária. A sociedade em geral vem registando uma preocupação, cada vez maior, pela percepção estética individual, o que também acontece, no âmbito da Medicina Dentária, com a cavidade oral. E nesta área, as papilas interdentárias são, actualmente, um dos maiores dilemas, dadas as repercussões físicas da sua ausência, com efeitos nefastos na estética do sorriso, e a dificuldade de restituição das mesmas. Deste modo, em virtude de uma sociedade que atribui maior importância à estética, inúmeras técnicas e procedimentos têm sido apresentados para a reconstrução, de diferentes casos e situações, de perda papilar, permanecendo este tema em contínuo desenvolvimento. As opções terapêuticas para a reconstrução papilar podem, por sua vez, ser mais ou menos invasivas, consoante a sua causa e o seu grau de destruição, sendo indispensável o conhecimento, por parte do médico dentista, dos factores que condicionam as papilas interdentárias, para assim, fazer o correcto diagnóstico e eleger o tratamento mais adequado.
The face lower third is extremely important regarding the perception of facial aesthetics of an individual, with his smile being in close relation with the concept of personal beauty, as well as the well-being and everyday social interaction. Society at large is increasingly recording a major concern with aesthetic perception by the individual, which also occurs within Dentistry, with the oral cavity. And in this area, the interdental papillae are currently one of the biggest dilemmas, given the physical repercussions of their absence, with adverse effects on smile aesthetics, and the difficulty to replace them. Thus, by virtue of a society that places more focus on aesthetics, numerous techniques and procedures have been presented for the reconstruction of different cases and situations of papillary loss, remaining this topic in continuous development. Therapeutic options for papillary coating can, in turn, be more or less invasive depending on their cause and the extent of destruction, so, the knowledge of the dentist about the factors that influence the interdental papillae is essential, to make a correct diagnosis and to choose the most appropriate treatment.

Introdução: Os procedimentos de reconstrução da papila interdentária revestem-se de uma tal complexidade técnica que representam um verdadeiro desafio na área da Periodontia. O ácido hialurónico é uma substância que vem sendo cada vez mais estudada, tanto no tratamento de diversas patologias, como em situações de melhoria estética. Muito recentemente, a reconstrução mediante a injeção de ácido hialurónico na papila interdentária vem sendo desenvolvida como uma promissora alternativa às técnicas cirúrgicas de reconstrução da papila interdentária. Objectivo: O objetivo deste trabalho de revisão bibliográfica foi avaliar o potencial da utilização do ácido hialurónico na reconstrução da papila interdentária. Materiais e Métodos: Foi realizada uma pesquisa bibliográfica, entre outubro de 2013 e junho de 2014, nas bases de dados MEDLINE, B-on, Scopus e Google Académico. Dessa pesquisa resultaram 48 artigos. Foram ainda consultados dois casos clínicos gentilmente cedidos pela empresa CPMPHARMA. Resultados: Os resultados dos escassos estudos disponíveis até à data permitem afirmar que a utilização de ácido hialurónico na reconstrução da papila interdentária constitui uma importante alternativa com vantagens significativas. Conclusões: São necessários mais estudos para confirmar os resultados encontrados.
Introduction: The procedures for reconstruction of interdental papilla are of such technical complexity that represent a real challenge in the field of Periodontics. Hyaluronic acid is a substance that has been increasingly studied, both in the treatment of various diseases, and situations of aesthetic improvement. Very recently, the reconstruction by injection of hyaluronic acid in the interdental papilla has been developed as a promising alternative to surgical techniques for reconstruction of interdental papilla. Aim: The aim of this work of literature review was to evaluate the potential use of hyaluronic acid in the reconstruction of the interdental papilla. Methods: A survey was conducted between October 2013 and June 2014 in MEDLINE, B-on, Scopus and Google Scholar data. This research resulted on 48 articles. Two cases were consulted and kindly provided by the company CPMPHARMA, who has been devoting to research this issue. Results: The results of the few studies available to date allow us to affirm that the use of hyaluronic acid in the reconstruction of the interdental papilla is an important alternative with significant advantages. Conclusions: Further studies are needed to confirm the results.
