Full textBACKGROUND: Widespread in the use of communication technologies and growing information and knowledge requirements for nursing care and management have emerged the need for controlled vocabularies oriented to the electronic health records systems, which have to be evaluated for validity and reliability. GOAL: To evaluate the validity of the nursing interface terminology termed ATIC. METHODS: Mixed-method evaluation study: (1) the structure of the terminology is assessed for inductive validity by means of an interpretative conceptualization of the metaparadigmatic constructs and the nursing process and (2) evaluation of the metric properties using descriptive designs to determine face validity (experts’ survey), content validity (measuring the presence of its concepts in the scientific literature), criterion validity (mapping its diagnosis axis to the NANDA International taxonomy) and reliability (measuring its usability in the acute care setting). FINDINGS: The structure of this vocabulary is held on an eclectic philosophical and theoretical foundation, giving a specific meaning to the interpreted constructs. According to the experts’ opinion its face validity scores 8.1/10. The scientific production of the discipline covers 98.7 % of the concepts within its diagnosis axis. More than 60% of these concepts match to those in the NANDA_I taxonomy. Nurses in the acute care setting use 92.3% of the diagnostic concepts of this vocabulary. CONCLUSIONS: In the light of the results, the ATIC nursing interface terminology is a valid and reliable disciplinary tool.

Full textThe features and relations of the geomorphological, climatic, biogeographic and anthropic subsystems and of the environmental impacts in the coastal sector of Chile between latitudes 32º and 33º South are analyzed, using the conceptual processes of general systems theory. Through a chronological focus, the various contributions to knowledge are studied and the ever-evolving aspects of the shapes, modes and processes identified in each subsystem are analyzed. Special attention is placed on the anthropic alterations to the biosphere subsystem set and the forms of occupation and use of the coast. The interest in this research arises from observation of the settlement, use and anthropic impacts without a defined regulation to which most of the coast of central Chile has been subjected. This provides the opportunity to study a coast on which these alterations have not yet occurred and to propose measures, from the perspective of sustainable development, that would make it possible to prevent these errors from being committed in other places. The evolution of institutions in Chile has enabled a great variety of regulatory systems to form with the aim of controlling the use of the land and of the activities there. Nevertheless, there are very few rules that directly have as their purpose the territorial structure of the coastland, a space of variable width and with active physical and anthropic processes. National policy for coastal Chile considers this stretch of land as an edge, not leaving possibility for a harmonious management nor being sufficient to deal with the existing biophysical ongoing processes. Considering another angle, the regulations for general planning are limited almost entirely to urban areas, providing few possibilities for regulating the rural spaces that are between the towns while facilitating the development of conurbations and with it the growth of urban spread, continuously and parallel to the coastline, as has already happened along much of the coastal part of central Chile. The coast between 32º and 33º South is a zone of transition between a Mediterranean coast and another that is more strictly semiarid. Here one can clearly see the land forms of marine origin resulting from a changing process over the Quaternary period in which the influences of climatic change, varying sea levels and tectonic processes combined. Within this setting, the first groups of humans began to settle, staying seasonally at sites necessary for their subsistence, being close to the biological resources and the places usable as protection. In the coastal strip studied, the currently populated areas are located on the coastal plains, co-existing with agricultural, forestry, port and docking uses located on other geomorphological units, each one of which undergoes various kinds of impacts and alterations associated with human settlement and the activities that go on in them. The dual organization of the relationships of these features at the mesosystemic level allows the way in which these elements are organized and the alterations are caused to be identified, with four interfaces being defined, that is, spaces defined by the relations between the bio-physical and anthropic environments and also the rules set in the institutional sphere. Thus, at the interfaces of use, impact, protection and restriction, qualitative levels of degradation are identified, as are the bodies of regulations that enable them to be structured and controlled. On the basis of these interface spaces, specific measures and actions are proposed with the aim of putting into effect a sustainable management of the coast.

Regenerative electrodes are designed to interface a high number of nerve fibers by using an array of holes, with electrodes built around them, implanted between the severed stumps of a peripheral nerve. Applicability of regenerative electrodes is dependent on the success of axonal regeneration through the perforations or holes, the possibility of nerve damage from the mechanical load imposed by the electrode or from constrictive forces within the holes, and the biocompatibility of the components.
In this work, we have evaluated the axonal regeneration through the regenerative electrodes implanted chronically between the severed stumps of the rat sciatic nerve. Results show that all the rats implanted with the sieve electrode had regenerated and reinnervated target organs. However, in a few cases decline of targer reinnervation and loss of regeneration nerve fibers was found from 6 to 12 months postimplantation, probably due to a compressive axonopathy. Motor axons regenerated scattered within minifascicles and the number of motor axons was markedly lower distally than proximally to the sieve electrode. These results are in relation with those obtained in the quantification of sensory neurons and motor neurons that regenerated through the sieve electrode.
The next objective was the obtention of neural signals through the sieve electrode after functional and electrical stimulation of distal targets. Recordings were obtained from a low proportion of electrodes on the sieve in response to functional stimulation of the paw. Failures are mainly attributed to increase of the electrodes impedance and to breaks due to mechanical traction affecting the whole system during chronic implants.
Next study was done to evaluated the long-term capabilities to maintain axonal regeneration in an experimental amputee model. Moreovere, we evaluated if it is possible to enhance axonal regeneration with Schwann cells (SCs) transplantation. Results showed that axons can sustain distal regenerated branches without degenerating for a long time and express functional features. A transplant of primary Schwann cells in the amputee model has positive effects, by stimulating the axonal growth response and the number of regenerating axons. SCs of an immortalized line are not able to induce axonal growth in a permisive environment such as the degenerated peripheral nerve. GDNF production by these cells favorably influences axonal regeneration.
In the last work we evaluated motor nerve regeneration after peripheral nerve injury and strategies to improve selective nerve regeneration. Results showed that the normal fascicular architecture and fascicular pattern of motor axons are maintained after a sciatic nerve crush, but there are lost after nerve section and repair, when motor axons are scattered within small regenerated fascicles throughout the nerve. The loss of the fascicular organization signficantly correlates with the deficient recovery of motor function. Transplantation of a small predegeneraed nerve segment inside a silicone guide enhances axonal regeneration, leading to slightly faster and higher levels of target reinnervation. Regeneration of motor axons is promoted at early times by the motor graft, whereas reinnervation of sensory pathways is increased in the presence of the sensory graft.
Full textRATIONALE: One of the first-line treatments of diabetic macular edema (DME) are intravitreal injections of vascular endothelial growth factor inhibitor (antiVEGF) drugs. The high clinical, social and economic impact of this pathology has increased interest in the detection of prognostic biomarkers of response to treatment in order to improve the individualization of treatment, reducing the burden of care in ophthalmology departments. Posterior vitreous detachment (PVD) in patients diagnosed with DME is a poorly studied event, however, published studies agree on a lower rate of DVP in patients with DME than in patients without DME. OBJECTIVES: To demonstrate that intravitreal injections of antiVEGF drugs in patients with DME induce PVD, to demonstrate that the presence of PVD in patients with DME improves the efficacy of antivEGF injection therapy, and to demonstrate that the presence of PVD reduces the number of injections of Anti-VEGF necessary DME treatment. MATERIAL AND METHODS: An observational, prospective and longitudinal study was performed comparing two groups based on presence (Group 1: 18 eyes) or absence (Group 2: 20 eyes) of vitreomacular adhesion (VMA) at 12 months follow-up. The study protocol consisted of visits every 2 months for 1 year, evaluating visual acuity (VA), foveal thickness (CFT) and PVD grade in each visit. The treatment regimen consisted of a fixed regimen of 3 intravitreal injections of Ranibizumab every 2 months, followed by a bimonthly pro re nata regimen. We also included a control group of 10 eyes of diabetic patients without EMD to analyze the incidence of PVD at one year of follow-up. RESULTS: At baseline, 76.32% of the patients had VMA, and at month twelve, a PVD event rate of 37.9% was observed in patients who had received intravitreal injections of antiVEGF while no cases were seen in the control group (P <0.001). The VA improvement was 6.94 letters in Group 1 (p=0.03) and 4.9 letters in Group 2 (p=0.08), this difference being not statistically significant (p = 1.00). CFT reduction was 259±239.62μm in Group 1 (p = 0.0002) and 118±148.33μm in Group 2 (p = 0.003), this difference being not statistically significant (p = 0.06). Patients in Group 1 received an average of 4,83±1,79 injections of anti-VEGF, and those in Group 2, 5.4±1.76 injections, this difference being not statistically significant (p=0.332). CONCLUSIONS: Repeated intravitreal injections of antiVEGF in patients with DME induce PVD in 37.9% of cases with VMA. The presence of PVD does not improve the efficacy of antiVEGF treatment; on the contrary, we found a tendency towards a greater reduction of CFT in patients with VMA. Patients with DME and PVD do not require fewer injections of anti-VEGF for one year.
Acurana, Flores Yasett Gisela.

In this Thesis we have analyzed different preliminary aspects related to the use of complex oxides, such as manganes perovskites, for implementation of functional devices with potential application on magnetoelectronics. To be precise, we have faced in deep the different aspects related with the fabrication of magnetic tunnel junctions based on the La2/3Ca1/3MnO3 (LCMO)/SrTiO3 (STO) system.
Our work makes evident the suitability of RF sputtering technique for growing high quality epitaxial layers of LCMO onto several single-crystalline substrates (STO, LaAlO3 (LAO)). The heterostructures LCMO/STO and LCMO/LAO have been object of a detailed analysis showing up that structural misfit plays a significant role both on the heterostructures growth process and on the physical properties. The system LCMO/STO (δ=1.2%) is under in-plane tensile strain but, thin films up to 100 nm thick can be grown without any partial relieve of the structural strain. In contrast, the system LCMO/LAO (δ=-1.8%) grows under in-plane compressive strain and shows clear traces of partial relaxation of structural strain even for thicknesses around 10nm.
The analysis of the magnetic and transport properties indicates that both systems have an outstanding depression on both the temperature transition, TC, and the saturation magnetization, MS, when decreasing the thin film thickness, being this phenomenon more pronounced in the case of LCMO/STO. High temperature annealing processes in air promote a remarkable improvement of the magnetic and transport properties in both systems. This improvement is faster and more evident in the LCMO/LAO system.
In addition, we have verified that the increment of the film thicknesses also promotes an improvement of the physical properties, approaching that of bulk samples. This evolution has been correlated with the existence of a non magnetic layer at the topmost layers of the LCMO thin films, whose effect becomes more relevant as the film thickness decreases and being dominant for ultra-thin films in the LCMO/LAO system. It has been observed that this dead layer can be reduced up to one order of magnitude by high temperature annealing processes.
Our work has demonstrated that structural strain plays a fundamental role on the magnetic and transport properties of these systems and the improvements of TC and MS with annealing processes is strongly linked to a partial relaxation of the structural strain. Other possibilities, such as oxygen uptake or modifications of the microstructure of the samples have been ruled out in base of Hall Effect measurements and high resolution transmission electron microscopy analysis.
To study transport properties at the interfaces we have developed an experimental procedure based on the use of an atomic forces microscope (AFM) working in current sensing mode, permiting the electrical characterization of insulating barriers and the interfaces between the precious metal and the complex oxide. This method allows characterizing transport properties of barriers and interfaces avoiding the problems related to the uncertainty of the exact nature of the contact between the AFM tip an the analyzed surface. Measurements were realized by defining nanocontacts, with different geometries and sizes, using different nanofabrication techinques.
