Full textThis PhD dissertation analyzes from a sociological point of view the evolutions in clinical care of atypical sex development, or intersex. It focuses mainly on the different ways specialized clinicians approach the controversy surrounding genitals normalization surgeries in pediatrics. From the observation of highly contrasted stances between, on one hand, movements of intersex people and national or international institutions defending human rights; and, on the other hand, dominant clinical discourses, this dissertation questions the conditions of the upholding or challenging of clinical practices, in a given time and location. The research is based on a qualitative inquiry and cross-analysis of hospital teams’ discourses and practices in France, the United-States and Switzerland, as well as on medical publications. It mainly shows that even though we tend towards a fair homogenization of medical discourses about “how” to proceed (protocols to use, care and techniques to offer, medical teams organization), the question of “whether” or not clinicians and parents should use the available techniques in order to change a child’s body, is subject to high internal tensions within the medical community, however often made invisible by consensus building. It shows how professional commitments, including on moral grounds, are tying clinicians to certain practices, as well as the specific dilemmas and uncertainties to which these practices confront them. This dissertation also shows that both supportive and critical views about those practices coexist most of the time in clinicians’ discourses. By comprehending clinical approaches of early genital surgeries from the point of view of their diversity and of the tensions that characterize them, this research highlights that the conceptions of what makes “good” care and of who is the patient are subject to variable interpretations, some of which question fundamentally the mandate granted exclusively to medicine until now

O nascimento de uma criança promove modificações relevantes no contexto da família. Na condição de intersexualidade as transformações se tornam ainda mais expressivas, desestabilizando a dinâmica familiar e a definição de papéis parentais. A maternidade se constrói em um ambiente de incertezas e especulações quanto ao sexo e gênero da criança. Este dissertação é composta de dois artigos. O primeiro intitula-se “O discurso biomédico e o da construção social na pesquisa sobre intersexualidade” e realiza uma revisão de literatura sobre as publicações de Medicina, Psicologia, Ciências Sociais e Direito/Ativismo Político que enfocam o tema da intersexualidade. As publicações foram analisadas segundo o contexto, área/disciplina, tipo de artigo, metodologia do estudo e conteúdo. O segundo artigo intitulado “Ambivalências na transição para maternidade: a chegada de uma criança intersexual” teve como objetivo analisar as especificidades do processo de construção da maternidade diante do nascimento de criança intersexual. Metodologia Estudo qualitativo selecionando mães de crianças com diagnóstico de Hiperplasia Adrenal Congênita, um das etiologias da intersexualidade, acompanhadas pelo Setor de Genética / HUPES/UFBA. Foram realizadas entrevistas narrativas com 12 (doze) mães e o levantamento de dados sócio-demográficos, história familiar e rede social de apoio mediante questionário. Resultados A dinâmica da ambivalência se apresentou como transversal a todo o processo de construção da maternidade, inscrita no próprio corpo da criança, nas crenças sobre o intersexo, atravessando a definição da identidade de gênero, os sentimentos maternos sobre a criança, as estratégias de cuidado, além da própria vivência da maternidade. Em relação aos sentimentos maternos sobre a criança, existiu uma oscilação da raiva à resignação. As crenças sobre a intersexualidade incluíram: cura espontânea, defeito corporal e doença. O lugar social do intersexual encontra-se em construção e envolve a percepção de normalidade e anormalidade, além da crença na existência dos dois sexos em um único ser. A rede social tanto apóia como limita a família. Algumas mães buscaram apoio de parentes mais próximos na realização de cuidados específicos à criança, outras, diante da incerteza em relação ao gênero da criança, priorizaram a proteção e o sigilo. Conclusão A transição para maternidade, apesar de envolver um evento não normativo, exige um alto grau de empoderamento materno no cuidado à criança. Essas mães são desafiadas a construir um lugar social para sua criança e utilizam tanto a revelação como o segredo sobre a situação de intersexualidade. A maternidade representa o eixo central da vida dessas mulheres. A maternidade, nesta condição, se constrói através da negociação de significados sobre o corpo e sobre o gênero proveniente do âmbito familiar e do discurso biomédico.

Full textInteractions among members of a social group have an important consequence in competition, cooperation and mating patterns, interfering in their fitness. Animals can associate preferentially and stablish social bonds. Bond maintenance can increase lifespan and reproductive success. Historically, there is a tendency in investigating male-female relationships and, consequently, mate selection, through male view, placing females in a passive position in this process. This research aimed to investigate the formation of intra and intersexual relationships in guinea pigs (Cavia porcellus) living in colony and verify if, in a controlled environment, where male interference were limited, females would prefer a specific male, indicating an active position in intersexual relationships. The guinea pig social structure was defined using the simple ratio association index and the social network analysis. Female preference was tested in a three partitions fenced , the central was empty and the laterals had a male restricted to it by guinea-pig collar. Females were free to move around all during 2 hours. Female preference analysis was made by comparisons between female time in each partition (Mann Whitney and Kruskal-Wallis, significance: 5%). Results showed that in colony females make intra and inter preferential associations, strongest among females-than among male-male or female-male in a higher number of individuals in the colony. In female-male associations, females had stronger relationship with two males from their colony. Results of preference tests reflect association preferences in colony: there were no difference in female time with the two prefered males of the colony. The third male was never chosen, and females prefered to spend more time in the empty partition. Results about social structure with association preferences, and in preference tests show the active role of females in intersexual associations and, possibly, in mate selection.
