Abstract:
Introduction: Rural emergency departments (EDs) are important safety nets for 20% of Canadian citizens. In Quebec, the province’s 26 rural EDs treat an average of 19,000 patients/year and are on average 300 km from levels 1 and 2 trauma centers. These distances signify that Emergency Medical Services (EMS) play a considerable role in the care of rural patients. EMS in Quebec province are private local services. There are no published reports on EMS in rural Quebec. As part of a larger study on rural emergency care, this descriptive study aimed at offering a comprehensive portrait of EMS. Methods: We conducted semi-structured interviews with managers of all paramedic services in rural Quebec. Interview questions focused on number of transports, training, availability of telemetry, GPS technologies, and work schedules. Results: Fifty managers of the 51 private companies serving the 26 rural EDs in Quebec were interviewed (response rate 98%). All were primary care paramedics (PCP). In 2010, EMS transported 40,671 patients, with 10,228 emergency transports to the rural EDs. A total of 7956 inter-facility transfers were conducted, 1499 of them emergency. Each ED required between 88 and 700 inter-facility transfers. A total of 60% (n=31/51) had GPS technology, only 25% (n=13/51) had telemetry features. Work schedules varied with 13% (n=7/51) of companies offering shifts of less than 12 hours, 28% (n=14/51) 24/7 weekly shifts, and 56% (n=29/51) a combination. Conclusion: This is the first study to describe rural EMS in Quebec. The finding that Quebec’s rural EDs are served by 51 privately-owned companies is unique in Canada. The considerable number of EMS transports, including inter-facility transfers, may reflect lack of local resources in rural EDs, the vulnerable population served, or the increased trauma risk in rural areas. Future studies should examine inter-facility transport reasons, costs, times and adequate training/scope of EMS practice.

Abstract:
The study aimed to determine whether the incidence and duration of certified sick leave (CSL) among nurses had increased during major restructuring of the health care system in the province of Québec, and to determine whether nurses exposed to adverse psychosocial factors at work showed an increased incidence of CSL. It involved nurses working in 13 health facilities. Sickness absence data were retrieved from administrative files (n = 1454). Incidence of CSL for all diagnoses and for mental health problems was examined. Telephone interviews were conducted to measure psychosocial factors at work with validated instruments. There was an increase in CSL among nurses during the restructuring, particularly for mental health problems. Modifiable adverse psychosocial work factors were identified and provide basis for interventions. Since human resources are the mainstay and primary resource of the health network, it is essential that people be able to perform their work under optimal conditions.

Purpose.The purpose of this paper was to examine leadership practices in effective schools located in economically disadvantaged areas of three Canadian provinces: Ontario, Québec, and New Brunswick.Research Design.Our study was conducted in five successful schools selected on the basis of student outcomes on province-wide standardized exams, as well as on some risk factors such as rural area, low socioeconomic level, and proportion of Francophones (Ontario and New Brunswick). To increase the study’s validity, we used triangulation and various data sources: (1) individual interviews; (2) observation of school principals; (3) field documentation; (4) student essays; (5) internal school documents such as mission statement, rules, and directives.Participants.Participants included Department of Education heads and school board administrators, school principals and vice principals, teachers, school counsellors, educational psychologists, parent school board members, and students.Findings.Results show that leadership practices in effective schools can be grouped together around five dimensions: establishing goals and expectations; strategic resourcing; curriculum planning, coordination, and evaluation; promoting and participating in teacher supervision and development; ensuring order and support.

Abstract:
This article presents a retrospective analysis of enteric disease outbreak investigations led by or conducted in collaboration with provincial health authorities in the Province of Quebec from 2002 through 2012. Objectives were to characterize enteric disease outbreaks, quantify and describe those for which a source was identified (including the control measures implemented), identify factors that contributed to or impeded identification of the source, and recommend areas for improvement in outbreak investigations (including establishment of criteria to initiate investigations). A descriptive analysis of enteric disease outbreak summaries recorded in a provincial database since 2002 was conducted, and corresponding outbreak reports were reviewed. Among 61 enteric disease outbreaks investigated, primary pathogens involved were Salmonella (46%), Escherichia coli O157:H7 (25%), and Listeria monocytogenes (13%). Sources were identified for 37 (61%) of 61 of the outbreaks, and descriptive studies were sufficient to identify the source for 26 (70%) of these. During the descriptive phase of the investigation, the causes of 21 (81%) of 26 outbreaks were identified by promptly collecting samples of suspected foods based on case interviews. Causes of outbreaks were more likely to be detected by weekly surveillance or alert systems (odds ratio = 6.0, P = 0.04) than by serotyping or molecular typing surveillance and were more likely to be associated with a common event or location (odds ratio = 11.0, P = 0.023). Among the 37 outbreaks for which causes were identified, 24 (65%) were associated with contaminated food, and recalls were the primary control measure implemented (54%). Review of enteric outbreaks investigated at the provincial level in Québec has increased the province's ability to quantify success and identify factors that can promote success. Multiple criteria should be taken into account to identify case clusters that are more likely to be resolved.

Abstract:
Purpose The purpose of this paper is to examine multi-stakeholder cooperatives (MSCs), a relatively new and understudied type of cooperative, by focusing on the impact of a new member status: the “supporting member.” Supporting members are included in the ownership structure, participate in the decision-making process and contribute to the share capital without being formally defined as users of the cooperative’s services, an important disruption to the traditional cooperative venture. Design/methodology/approach This paper is based on a qualitative study using in-depth interviews with 30 members (i.e. founders, managers, board members and employees) of 14 MSCs located in the Canadian province of Québec. Findings This study suggests that including supporting members in the cooperative venture impacts the three core features of cooperatives, which are traditionally user-owned, user-controlled and user-benefiting. Despite supporting members’ positive contributions to an MSC’s development and success, the inclusion of such members generates management challenges and organizational paradoxes. Social implications The inclusion of supporting members allows MSCs to become an experiment in “stakeholder democracy” and a space of negotiation between organizations, citizens and institutions, as MSCs represent and embody some of the community’s needs and desires. Originality/value This study constitutes an original contribution to paradox literature, as it describes the specific upward and downward spirals related to the inclusion of supporting members, highlights innovative responses to these paradoxes and extends understandings of cooperatives as hybrid organizations entangled in bundles of paradoxes.

AbstractObjective:The aim of this paper is to describe the source and the scope of social inequalities in infant feeding practices. It examines the extent to which different recommendations are followed in different social groups and highlights the main factors influencing the total adherence to three recommendations at the population level.Design, setting and subjects:The study follows a representative sample (n= 2103) of the children born in 1998 in the province of Québec (Canada). Detailed information on breast-feeding and complementary feeding was collected at 5 and 17 months by face-to-face interviews with the mother. The independent variables were mother's age, mother's education level, poverty level, family type, socio-economic status (SES) and living area. Odds ratios (adjusted for baby's rank in the family, birth weight and premature birth) are presented for breast-feeding, and for formula and cows' milk consumption, at different ages. The adherence to a combined indicator cumulating three recommendations (breast-fed at birth, complementary food at 4 months or later and cows' milk at 9 months or later) is also presented.Results:The analysis indicates that adherence to the recommendations is low in Québec. Breast-feeding initiation, duration and its exclusivity improved with mother's age and education level and SES. Adherence to the different recommendations was interrelated, indicating an accumulation of bad nutritional circumstances for children in low-SES families. The odds of being fed in accordance with the three studied recommendations, when living in a family with the highest SES, was 2.3 times higher than when living in a family with the lowest SES. When living with a highly educated mother, the odds ratio was 2.7 times higher than when living with a low-educated mother. For mother's age, the odds ratio reached 3.7 for children from mothers aged ≥35 years, in comparison with children from mothers ≤24 years old. When SES or mother's education level was combined with mother's age, the children in the best situation were >8 more times likely than the least privileged children to be fed in accordance with these recommendations. Living area was not related with infant feeding during the first year of lifeConclusions:Breast-feeding and nutrition could be related with different health and cognitive outcomes in childhood and later in life. Consequently, social disparities in diet during infancy could play a role in the development of social and health inequalities more broadly observed at the population level. Intervention to improve adherence to breast-feeding and nutrition recommendations in infancy should be prioritised and evaluated for its impact on the reduction on infant diet inequalities over time

Abstract:
Background The persistence of cannabis use disorder (CUD) in young adults with first-episode psychosis (FEP) is associated with poor clinical and functional outcomes. Face-to-face psychological interventions are effective in treating CUD. However, their use in early intervention services (EISs) for psychosis is inconsistent because of barriers, including high workload and heterogeneity in training of clinicians and lack of motivation for treatment among patients. Tailoring new technology-based psychological interventions (TBPIs) to overcome these barriers is necessary to ensure their optimal acceptability. Objective The aim of this study is twofold: to explore psychological intervention practices and intervention targets that are relevant for treating CUD in individuals with early psychosis and to explore factors related to the development and implementation of a technology-assisted psychological intervention. Methods A total of 10 patients undergoing treatment for FEP and CUD in EISs participated in a focus group in June 2019. Semistructured individual interviews were conducted with 10 clinicians working in first-episode clinics in the province of Québec, Canada. A hybrid inductive-deductive approach was used to analyze data. For the deductive analysis, we used categories of promoting strategies found in the literature shown to increase adherence to web-based interventions for substance use (ie, tailoring, reminders, delivery strategies, social support, and incentives). For the inductive analysis, we identified new themes through an iterative process of reviewing the data multiple times by two independent reviewers. Results Data were synthesized into five categories of factors that emerged from data collection, and a narrative synthesis of commonalities and differences between patient and clinician perspectives was produced. The categories included attitudes and beliefs related to psychological interventions (eg, behavioral stage of change), strategies for psychological interventions (eg, motivational interviewing, cognitive behavioral therapy, psychoeducation, stress management), incentives (eg, contingency management), general interest in TBPIs (eg, facilitators and barriers of TBPIs), and tailoring of TBPIs (eg, application needs and preferences, outcome measures of interest for clinicians). Conclusions This study provides a comprehensive portrait of the multifaceted needs and preferences of patients and clinicians related to TBPIs. Our results can inform the development of smartphone- or web-based psychological interventions for CUD in young adults with early psychosis.

Abstract:
This text presents the results of the first research conducted on “green” actions and strikes for climate in high schools across Québec, a Canadian province that witnessed in 2019 the larger street protests of the international youth movement. Based on 20 semistructured interviews with students from 12 high schools, letters from school principals addressed to parents, and research in the media, this text reaffirms that schools are a place of political conflicts and struggles not only between students and adults but also between students in opposite currents of the movement. It is also a reminder of the involvement of young people in autonomous direct action groups (Extinction Rebellion). The discussion then focuses on potential implications of the movement for future elections, the legitimacy of these collective actions in relation to the philosophical debate about civil disobedience (John Rawls, Ronald Dworkin, Manuel Cerveza-Marzal, and Alan Carter), and the hope for a renewal of the student movement in Québec in the face of a disaster of unprecedented scale.

Over the past five years, the interactive whiteboard (IWB) has been massively introduced into schools across the province of Quebec, Canada. This study explores how the IWB is being used, and the associated benefits and challenges. Data on 11,683 students (from 4th year elementary to grade 12) and 1,131 teachers were collected with five instruments: 1) a survey questionnaire for all students, 2) a survey questionnaire for all teachers, 3) individual interviews with teachers, 4) group interviews with teachers, and 5) group interviews with students. Far from calling into question the need to integrate technology into education, the results reveal that certain tools, such as the IWB, may be more complicated and time-consuming to integrate than others. However, despite teachers’ reports of technical problems, the IWB appears to have real educational potential.Au cours des dernières années les tableaux blancs interactifs (TBI) ont été massivement introduits dans les écoles du Québec (Canada). Cette étude explore comment cet outil numérique est utilisé, de même que les avantages et les défis qui y sont associés. Les données ont été recueillies auprès de 11 683 élèves (de la 4e à la 12e année) et 1 131 enseignants avec cinq outils de collecte de données: 1) un questionnaire d’enquête pour tous les élèves; 2) un questionnaire d’enquête pour tous les enseignants; 3) des entrevues individuelles avec des enseignants; 4) des entrevues de groupe avec des enseignants; 5) des entrevues de groupe avec des élèves. Loin de remettre en question l’intégration des technologies en éducation, les résultats de cette étude révèlent plutôt que certains outils, comme le TBI, peuvent parfois être plus complexes et chronophages à utiliser que d’autres. Néanmoins, même si plusieurs enseignants ont souligné les nombreux problèmes techniques, le TBI comporte un grand potentiel éducatif.

Abstract:
This qualitative study examines the perceptions of mothers, fathers, and adolescents on the relations they see between intimate partner violence, mental health and substances use parental problems that co-occur in their family, and the mothers’ and fathers’ associated parenting challenges. The sample was composed of 43 people (15 mothers, 16 fathers, and 12 adolescents) who were directly affected by the co-occurrence of intimate partner violence and mental health or substances use parental problems. The participants were recruited with the collaboration of public and community organizations in the Province of Québec (Canada). The data were collected through semi-structured interviews and combined with a table detailing the specific problems identified by the participants. A thematic content analysis method was employed to do the coding, and a summary of the results was returned to some participants who agreed to collaborate with the validation of the results. The analysis revealed numerous and diverse relations between the interviewees’ co-occurring problems, but two principal patterns emerged. The first one was chiefly observed in the comments made by mothers and adolescents who saw the co-occurring problems as being the consequence of the intimate partner violence on the mother. The second pattern was largely observed in the fathers’ comments, who explained that the substance use problems came before the intimate partner violence and acted as an aggravating factor. The co-occurrence of intimate partner violence with other parental problems strongly affected the mothers’ and fathers’ parenting, making it more difficult to meet the children’s needs. Considering the complexity and diversity of the experiences and needs of the mothers, fathers, and children in co-occurring situations, future studies should evaluate the services provided to these families and the coordination between the different concerned organizations.

Comment les politiciens adaptent-ils leurs stratégies de communication aux différentes émissions de télévision dans lesquelles ils sont amenés à accorder des entrevues? On constate aisément que l'espace médiatique n'est pas homogène, qu'il est constitué d'une diversité de pratiques et que la maîtrise de cet espace est un élément stratégique de la pratique politique, puisque les médias constituent le principal canal par lequel les politiciens entrent en communication avec l'électorat. Dans la littérature sur les genres télévisuels, nous avons constaté que ces derniers sont la plupart du temps décrits et catégorisés en fonction de la nature informative ou divertissante de leur contenu. Toutefois, il apparaît que ces catégories sont à la fois difficiles à associer à des caractéristiques uniformes et qu'elles perdent en capacité explicative dans un paysage médiatique profondément marqué par la transformation et l'hybridation des pratiques. Pour surmonter les limites associées aux descriptions des genres télévisuels en termes d'information et de divertissement, nous avons adopté une approche sociale des pratiques médiatiques, alors conçues comme des activités de communication plutôt que comme des produits médiatiques. Nous avons en l'occurrence mobilisé des apports théoriques issus de l'interactionnisme de Goffman et Blumer ainsi que des genres du discours de Bakhtine pour désigner les interactions entre les acteurs politiques et médiatiques à la fois comme lieu de construction et de définition de l'espace médiatique et comme source de données pour comprendre comment ils y organisent leur action. À la lumière de ce positionnement théorique et méthodologique, nous avons formulé deux questions de recherche : 1) quelles caractéristiques permettent de saisir et de décrire la variété des émissions d'entrevue dans l'espace télévisuel; et 2) quels sont les critères qui permettent le mieux d'expliquer l'organisation des stratégies de communication des politiciens dans l'espace médiatique? Notre stratégie de recherche repose sur la description de la production verbale des acteurs politiques et médiatiques en situation d'entrevue, en vue de reconstituer les cadres interprétatifs qui sous-tendent leurs interactions. Principalement inductive, notre démarche met à l'avant-plan l'observation d'une variété de formats d'entrevues afin de faire ressortir des critères de description des genres ancrés dans les pratiques des acteurs. Nous avons d'abord construit une grille d'analyse de la performance des acteurs et l'avons ensuite utilisée pour proposer un découpage du corpus cohérent avec les catégories qu'ils utilisent pour organiser leurs pratiques. Comme méthodes de traitement des données, nous avons utilisé le modèle modulaire d'analyse du discours de Roulet et Moeschler, ainsi que certains outils d'analyse conversationnelle empruntés notamment à Schegloff, Greatbatch, Hutchby, Heritage et Clayman. Nous avons rassemblé un corpus constitué de 25 entrevues télévisées, tirées de 9 émissions diffusées sur les chaînes québécoises de langue française, présentant des entrevues avec 3 chefs de partis politiques québécois : Pauline Marois, François Legault et Régis Labeaume. Sur le plan descriptif, l'analyse des données montre que l'organisation des pratiques d'entrevue avec des politiciens repose principalement sur deux critères : 1) la répartition des rôles; et 2) l'orientation de la relation. La combinaison de ces deux critères permet de décrire les différentes configurations interactionnelles qui peuvent caractériser une entrevue médiatique. Sur le plan théorique, les observations permettent de revisiter le concept de genre médiatique et de montrer que l'organisation des pratiques de communication publique relève à la fois de la récupération d'éléments typiques et de l'adaptation de ces éléments aux objectifs et aux besoins circonstanciels des acteurs.

Les loteries télévisées sont des émissions très populaires au Québec : plus d'un million de personnes regardent au moins une émission chaque semaine. Mais, jusqu'à maintenant, aucune recherche n'a cherché à analyser le contenu présenté aux téléspectateurs. La présente étude porte sur le contenu des émissions La Poule aux œufs d 'or, La Lampe magique et Déci$ion qui sont présentées au public québécois. Les résultats révèlent que des éléments associés à l'illusion de contrôle et à la croyance erronée quant à la possibilité réelle de gain ont été observés dans toutes les émissions. En outre, les animateurs véhiculent plusieurs fois par émission des biais cognitifs qui relèvent de l'illusion de contrôle, de la superstition et de l'évaluation biaisée des résultats. Des recherches futures devront être menées pour évaluer l'effet sur le téléspectateur de l'exposition à des mécanismes de pensées erronées.

Le double doublage francophone donne lieu à deux doublages différents pour un même film américain : un doublage français et un doublage québécois. Issue de la législation des deux aires géographiques, cette pratique donne l'occasion inégalable d'analyser les deux doublages produits d'un point de vue sociolinguistique. Au Québec, la langue du doublage est souvent nommée « français international », mais en vérité, il n'existe pas de source décrivant officiellement les normes prescriptives ou objectives de ce français, qui est davantage une idéologie (Sanders, 1996, p. 113) qui découle des représentations que les doubleurs ont de la langue française. Il a déjà été démontré que la langue du doublage québécois est particulière en raison du désir des doubleurs d'employer un français à la fois ancré au Québec et délocalisé, qui sont deux objectifs difficilement conciliables (Reinke et Ostiguy, 2012, Reinke et al.,2017 et 2019). En effet, il semble avoir dans le doublage québécois peu de traits de la variété québécoise, et ce, même si plusieurs de ces traits sont aujourd'hui acceptés en variété soutenue. Afin de connaître quel type de norme est suivi par les doubleurs, endogène ou exogène, l'étude du e variable est pertinente, principalement à cause de sa variation diatopique (Côté, 2012, p. 260) et diaphasique (Lucci, 1978, p. 46). Afin de nous éclairer sur les normes objectives suivies par les doubleurs, nous analyserons les usages des doubleurs quant à cette variable linguistique dans un corpus de transcriptions de 11 films et séries télévisées. La réalisation du e variable sera analysée selon la provenance géographique du doublage, la formalité de la situation de communication et le genre cinématographique.
The French language "double dubbing" phenomen on gives rise to two different dubbing tracks for the same American movie: a French dubbed version from France and one from Québec. As a result of legislation in the two geographical regions, this practice provides an unparalleled opportunity to study the two dubbings from a sociolinguistic point of view. In Québec, the language of dubbing is often called "international French", but in reality, there exists no source officially describing the prescriptive or objective norms of this particular French, which is more of an ideology (Sanders, 1996: 113) originating from the dubber's representations of the French language. It has already been shown that the language of the Quebec dubbing is peculiar, due to the dubbers' desire to use a language both rooted in Québec and delocalized, which are two objectives that are difficult to reconcile (Reinke et Ostiguy, 2012, Reinke et al., 2017 et 2019). Indeed, Québec's dubbing seems to offer few features typical of Québec's French, even if several of these traits are now accepted in formal speech. In order to elucidate what type of norm is followed by the dubbers, endogenous orexogenous, the study of variable e is relevant, mainly because of its diatopic (Côté, 2012,p.260) and diaphasic variation (Lucci, 1978, p. 46). In order to shed light on the objective norms followed by dubbers, we will study how they use the variable e in a corpus composed of the transcriptions of 11 films and television series. The usage of the variable e will be analyzed according to geographical origin of the dubbing, register and film genre.

Abstract:
L’infodivertissement, genre télévisuel qui allie le divertissement et l’information, n’est pas un phénomène nouveau, mais sa popularité grandissante soulève plusieurs questions. Cette recherche se concentre sur le discours politique diffusé dans ces émissions en abordant le recours aux émotions comme un élément de ce discours. Nous proposons une analyse de discours qui vise à décrire la fréquence et la nature des recours aux émotions dans certains discours politiques. En adoptant une méthode comparative, nous souhaitons mettre en évidence les différences entre les discours présents dans les émissions d’information et dans les talk-shows québécois. Le corpus analysé est formé d’entrevues avec les chefs de partis québécois lors des élections de 2007 et 2008. Les entrevues sont issues d’émissions d’information (Le Téléjournal 22h, Les Coulisses du pouvoir), d’une émission d’infodivertissement (Tout le monde en parle) et d’une émission de divertissement (Deux filles le matin). Les entretiens ont été sélectionnés en s’assurant une diversité des chefs et des genres d’émissions. Les données analysées ont permis de mettre en lumière certaines constantes propres à chacun des genres télévisuel. Cependant, les résultats montrent que l’influence du genre télévisuel sur le recours aux émotions dans le discours politique n’est pas aussi importante que certains auteurs le prétendent. Notre objectif n’est pas de régler le débat sur la place de la politique dans l’infodivertissement, mais plutôt d’approfondir les connaissances sur le discours politique dans les émissions d’infodivertissement en éclairant le débat présent dans la littérature.

Ce mémoire porte sur les modalités de l’apprentissage et de la critique des règles de la communication publique par les représentants étudiants lors de la grève étudiante de 2012 au Québec. En tenant compte des caractéristiques particulières des organisations étudiantes et des conditions dans lesquelles les représentants étudiants apprennent les rudiments des rôles d’attaché de presse et de porte-parole, ce mémoire propose d’étudier les modalités de l’apprentissage et de la critique des règles de la communication publique. Des acteurs nouveaux qui ne bénéficient pas des mêmes ressources humaines et financières que les acteurs gouvernementaux et médiatiques, mais avec qui ils ont débattu publiquement à l’occasion de la grève étudiante de 2012. Comment ont-ils appris les règles de la communication publique dans un débat public intense et animé qui a par la suite pris des allures de crise sociale? Suite à l’expérience de 2012, ces personnes sont devenues beaucoup plus connues et reconnues. Nous voulons comprendre comment les différentes façons d’apprendre des règles de la communication publique sont tributaires de l’environnement particulier et du contexte dans lequel se trouvaient alors les représentants étudiants. Nous voulons également nous attarder au contenu de l’apprentissage et de la critique pour mieux comprendre quelles sortes de règles sont apprises et quelles en sont les fonctions stratégiques. La recherche prend appui sur une analyse qualitative d’entrevues télévisées diffusées à l’émission 24 heures en 60 minutes et sur une série d’entretiens semi-dirigés avec des représentants étudiants. This thesis deals with the methods of learning and criticizing the rules of public communication by student representatives during the student strike in 2012 in Quebec. Taking into account the particular characteristics of student organizations and the conditions in which student representatives learn the rudiments of the roles of press officer and spokesperson, this paper proposes to study the modalities of learning and criticism of rules of public communication. New actors who do not benefit from the same human and financial resources as government and media actors, but with whom they publicly debated during the 2012 student strike. How did they learn the rules of public communication in an intense and animated public debate which subsequently degenerated into a social crisis? As a result of the 2012 experience, these individuals became much more known and recognized, and gained notoriety. We want to understand how the different ways of learning about the rules of public communication depend on the particular environment and the context in which the student representatives were at that time. We also want to focus on the content of learning and criticism to better understand what kinds of rules are learned and what are their strategic functions. The research is based on a qualitative interview analysis of the public affair program 24 heures en 60 minutes, on RDI, and a series of semi-conducted interviews with student representatives.
