Abstract:
The landmark paper discussed in this chapter, published in 2002 by Dolin et al., examines the incidence of moderate-to-severe pain and severe pain after major surgery with three analgesic techniques: intramuscular analgesia, patient-controlled analgesia, and epidural analgesia. Up until 1990, intramuscular morphine was the main form of post-operative pain control, with patient-controlled analgesia and epidural analgesia as relatively new techniques. The authors found that the mean incidence of moderate-to-severe pain was more common with intramuscular analgesia (67%) than with patient-controll

Abstract:
The use of painkillers ranging from simple analgesics to strong opioids is a common feature in the acute pain management of haematological conditions. However, each disease also has its own specific aetiological factors for pain, requiring specific treatment. Haematological patients with chronic pain on long-term opioid therapy may require multidisciplinary pain management to improve quality of life and prevent chronic escalation of opioid doses. Intramuscular injections should be avoided in all patients. The use of pethidine (meperidine) is not recommended.

Tularemia, caused by the gram-negative coccobacillus Francisella tularensis, is an extremely infectious bacterial zoonosis. Symptoms depend on site of exposure; they can be nonspecific and may include fever, lymphadenopathy, ulcer or papule, and nausea/vomiting. Natural transmission occurs via small mammals, such as rabbits, or arthropod bites. IV or IM antibiotics are preferred over oral forms. Supportive care is also critical; some patients may require respiratory support. If used as a biological weapon, aerosolized F. tularensis would be the most likely route of transmission. Vitamin K is involved in blood clotting, and lack of this nutrient results in a hypocoagulable state, associated with a high risk of bleeding events. Deficiency is rare among adults, because vitamin K is widely available in foods. However, the placenta transmits vitamin K relatively poorly, leading to a general deficiency in full-term neonates, who are also particularly vulnerable to the effects of low vitamin K levels because of their immature clotting systems. Deficiency can lead to intracranial haemorrhage following birth trauma, or classic vitamin K deficiency bleeding. It is important to

Piriformis syndrome is an entrapment neuropathy caused by compression or irritation of the sciatic nerve as it courses in proximity to the piriformis muscle. Conservative treatment modalities for piriformis syndrome include the use of anti-inflammatory analgesic medications or muscle relaxants. Physical therapy is often employed to correct the abnormal pelvic biomechanics and focus on stretching the piriformis muscle. Prior to proceeding with invasive surgical approaches, this chapter advocates the use of piriformis muscle injection. The technique both confirms the diagnosis and offers therape

A new diagnosis of SLE can be frightening for patients, and the importance of education and reassurance must be remembered—clinical nurse specialists can play a key role in this. Equally, lifestyle advice regarding sun-protection and smoking cessation should not be neglected. Symptomatic treatment, topical corticosteroids, antimalarials, and non-steroidal anti-inflammatory drugs are generally sufficient to manage these cases, but acute symptomatic flares may require short-term

Anaphylaxis is a severe life-threatening systemic hypersensitivity reaction, which occurs in approximately 10–20 per 100,000 population per annum, and accounts for 0.3% of adult critical care admissions. Anaphylaxis most commonly results from an exaggerated immune reaction to foreign antigens, prompting release of vasoactive substances from mast cells. A broad range of agents including foods, insect stings, latex, and drugs can trigger anaphylaxis. Common food allergens include nuts, shellfish, milk, and eggs. The most frequently implicated drugs include neuromuscular blocking agents (NMBAs) a
