ZusammenfassungDie Embolia cutis medicamentosa ist eine seltene Komplikation von intramuskulären Injektionen. Dabei entwickelt sich eine livedoartige Dermatitis, die häufig in hämorrhagische Nekrosen übergeht. Die Pathogenese ist nicht geklärt. Diskutiert wird eine intraarterielle Injektion des Arzneimittels. Wir berichten über einen Fall von Embolia cutis medicamentosa nach Varizensklerosierung mit Polidocanol.

ZusammenfassungAuch wenn die intramuskuläre Injektion in der Notfallmedizin eher eine untergeordnete Rolle spielt, stellt sie doch einen effektiven, sicheren und schnellen Applikationsweg zur Verabreichung bestimmter Notfallmedikamente dar. So findet sie sich in verschiedenen Leitlinien als First-Line-Zugangsweg, so z. B. bei der Anaphylaxie und dem Status epilepticus. Auch bei aggressiven bzw. agitierten Patienten und Kindern bestehen gute Erfahrungen hinsichtlich der intramuskulären Verabreichung (analgo-)sedierender Medikamente. In Notfallsituationen gut zugängliche Applikationsorte sind der laterale Oberschenkel, die Wade und die Schulter. Bei sorgfältiger und antiseptischer Vorgehensweise sowie korrekter Wahl von Punktionsort und Nadellänge sind Komplikationen selten.

Abstract:
ZusammenfassungEine Patientin wurde vom Hausarzt aufgrund einer Rhinitis allergica mittels einer Injektion mit Triamcinolon intramuskulär im Bereich des Oberarms behandelt. Im Injektionsbereich wurde 9 Monate später eine 5 cm im Durchmesser große Lipathrophie nachgewiesen. Die Schlichtungsstelle bejahte einen ärztlichen Behandlungsfehler. Die parenterale Verabreichung von Kortikosteroiden vom Typ Triamcinolon-Depot sollte Situationen vorbehalten bleiben, in denen eine lokale bzw. orale Therapie nicht durchführbar oder kein ausreichender Therapieeffekt zu erzielen ist; es war jedoch weder die Schwere der allergischen Rhinitis noch eine vorherige Behandlung mit laut Fachstandard zunächst empfohlenen lokalen bzw. systemischen Antiallergika dokumentiert. Zudem war maßgeblich für die Verursachung der Komplikation einer aseptischen Lipatrophie die fehlerhafte Injektionstechnik sowohl bez. der Tiefe als auch des Injektionsortes zu beanstanden.Die intra- und/oder subkutane Injektion einer Triamcinolon-Kristallsuspension hat ein hohes Risikopotenzial, eine langanhaltende Atrophie der Dermis oder des subkutanen Fettgewebes zu verursachen. Die gluteal tief intramuskuläre Anwendung von Triamcinolonacetonid für die Therapie der allergischen Rhinitis ist nur zugelassen, wenn diese einen schweren Verlauf zeigt und eine leitliniengerechte Lokaltherapie versagt hat. Diese Indikationseinschränkungen sollten streng beachtet und Patienten über das Nebenwirkungspotenzial der Therapie rechtswirksam aufgeklärt werden.

Abstract:
Syfte: Syftet med litteraturstudien är att presentera kunskapsläget över vad litteratur föreslår om omvårdnad i samband intramuskulär injektioner. Metod: Vi har skrivit en litteraturstudie där en systematisk litteratursökning genomförts, och där åtta artiklar inkluderats i vår uppsats. Goodmans (SBU, 1993) metod användes. Sökningarna utfördes i följande databaser: Cinahl, Medline, PubMed samt Swemed+. Artiklarna är kritiskt granskade enligt Polit et al (2001) samt en poängsättningstabell (Carlsson & Eiman, 2003). Resultat: Med enkla medel och med en väl utvärderad injektionsteknik minskar risken och incidensen för smärta och komplikationer.
Aim: The aim of this study is to present the level of knowledge of what literature suggest about the aspects of nursing and intramuscular injections. Methods: According to Goodmans (SBU, 1993) method on how to do a literature review we have done a systematic literature search and review. Searches were made in databases; Cinahl, Medline, PubMed and Swemed+. Eight articles of quantitative design was included in our study. The articles are scrutinized according to Polit et al (2001) and Carlsson & Eiman (2003). Results: With simple methods and by using validated technique for injections the possible pain, risks and complications can be minimized.

Abstract:
Wir nahmen an, dass nach intramuskulärer Injektion von Koffein und Halothan in Schweinen mit Veranlagung (MHS) und ohne Verlangung (MHN) für eine maligne Hyperthermie, es zu einem dosisabhängigen intramuskulären Laktatanstieg kommt. Des weiteren ist die Ausdehnung des dadurch ausgelösten Hypermetabolismus nur auf ein kleines Areral um die Injektionsstelle begrenzt. Eine systemische MH-Krise wird nicht ausgelöst. Mikrodialysesonden wurden in die Hinterläufe von 7 MHN und 7 MHS Schweinen platziert und mit Ringerlösung perfundiert. Nach Einstellen eines Gleichgewichtes wurden Boli von Halothan und Koffein in aufsteigenden Konzentrationen in den Muskel appliziert. Für den zweiten Versuchsansatz, die regionale Ausbreitung betreffend, wurden wiederum Mikrodialysesonden im Abstand von 10 mm und 25 mm zum Injektionsort platziert. Das Laktat wurde photospektrometrisch im Dialysat gemessen. Durch intramuskuläre Halothan- und Koffeinapplikation kommt es zu einer dosisabhänigen Erhöhung der intramuskulären Laktatkonzentration mehr beim MHS als MHN-Tieren mit signifikantem Unterschied. Laktaterhöhungen fanden sich darüber hinaus nur an der Injektionsstelle der Trigger- substanzen, nicht jedoch in 10 und 25 mm Entfernung davon. Somit ist der Anstieg der lokalen Laktatkonzentration im Muskel nur auf ein kleines Areal um die Injektionsstelle begrenzt
We hypothesized that IM halothan and caffein injection increases lokal lactat concentration dose-dependently in malignant hyperthermia susceptible (MHS) und non-susceptible (MHN) pigs and that the hypermetabolic reaction measured by regional distribution of lactat is limited to a small area around the injection side. Microdialysis probes were placed in the hindlimbs of 7 MHS and 7 MHN pigs and perfused with Ringers solution. After equilibration, boluses of increasing halothan and caffein concentrations were injected. For the second hypothesis regarding regional distribution, microdialysis probes were positioned in 7 MHS and 6 MHN pigs at the injection site for halothan and caffein and at a distance of 10 mm and 25 mm. Lactat were measured in the dialysate by spectrophotometry.Halothane and caffeine increased IM lactat dose-dependently in MHS significantly more than in MHN pigs. Lactat was increased only at the infection site but not at 10 mm and 25 mm distance. The increase of lactat after lokal MH trigger injection is limited to a small area around the probe
