Ventilator-associated pneumonia (VAP) is pneumonia developing 48-hours or more after tracheal tube insertion and mechanical ventilation. Nos objectifs ont été d'évaluer l'utilisation de plusieurs dispositifs dans le cadre de la prise en charge des situations d'intubation difficile imprévue survenant au bloc opératoire et dans un contexte d’urgence extra hospitalière puis de valider leur efficacité lorsqu'ils furent intégrés dans des algorithmes précis de prise en charge. Notre objectif était d’optimiser la prise en charge des voies aériennes en urgence pour aboutir à une diminution de la morbi-mortalité accompagnant l’intubation. Notre travail a permis de fixer l’incidence de l’intubation difficile en urgence à 7,4% et d’identifier les facteurs associés à cette intubation difficile. Introduktion: Populationen som lider av obesitas ökar och därmed förekommer allt oftare i antalet anestesier i Sverige. Obesitas är en riskfaktor för svår luftväg och det är därmed av vikt att förstå dess inverkan på klinisk luftvägshantering. Uppemot 30% av morbiditet och mortalitet vid anestesi är relaterad till luftvägshantering och är därmed den största orsaken till komplikationer inom anestesisjukvården. Introduction: Les douleurs de gorge post opératoires sont fréquentes. Nous avons étudié l'influence de la pression du ballonnet de la sonde d'intubation sur leur incidence. Méthodes: Etude prospective randomisée double aveugle. 50 patients. Ballonnet gonflé à l'AIR ou au SERUM pour une pression à 20cmH20. Anesthésie sous protoxyde d'azote (N2O). Lésions trachéales évaluées par une fibroscopie. Douleur de gorge évaluée en post opératoire. Résultats : Les pressions du groupe SERUM étaient stables contrairement à celles du groupe AIR. Tous les patients du groupe AIR avaient des lésions trachéales
