Abstract:
We characterized the response to intravenous S-isopropyl isothiourea (IPTU), a novel potent nitric oxide synthase (NOS) inhibitor, in rodent and porcine models of hemorrhagic shock (HS). IPTU (at 300 micrograms/kg, administered as 3 subsequent bolus injections), in anesthetized rats hemorrhaged to a mean arterial blood pressure (MAP) of 35 mmHg, increased MAP and improved survival over 120 min. In anesthetized pigs hemorrhaged to a MAP of 45 mmHg, IPTU (0.3 mg/kg plus 1 mg.kg-1.h-1) increased MAP and systemic vascular resistance. IPTU did not alter the cardiac index, renal blood flow, arterial and portal oxygen content, or splanchnic oxygen consumption or extraction. In contrast, infusion of norepinephrine (100 micrograms.kg-1.h-1) did not alter MAP and increased mortality in the rat model, whereas it caused a transient increase in MAP and a tachycardia in the porcine model of HS without significantly affecting the other parameters studied. Inhibition of the endothelial NOS in early severe HS may have beneficial effects on blood pressure and survival without altering cardiac output and splanchnic and renal perfusion.

Abstract:
Abstract Introduction Portal and/or splenic vein thrombosis (PVST), stemming from immune thrombocytopenia (ITP), warm autoimmune hemolytic anemia (wAIHA) or other splenic diseases, is highly associated with splenectomy complications. Although symptomatic PVST is a rare event (incidence rate 1-3%) reportedly associated to mortality, it has been shown through systematic computed tomography (CT) scans that PSVT events exist in up to 50% of splenectomized patients. The clinical significance of this finding remains unclear. Indications of anticoagulation depend on the site and extent of PSVT. The relevance of postoperative CT scanning is discussed, specifically in the context of immune cytopenia, where PSVT seems to be less frequent and less extended. We conducted a retrospective study to assess the incidence, the sites and the outcome of PSVT in ITP and wAIHA patients. Patients and methods The study was conducted between 2009 and April 2015 in a tertiary-care center for immune cytopenias (Henri Mondor Hospital, France) where postoperative CT scans are systematically performed. We analyzed splenectomized ITP or wAIHA (without underlying malignancy) adult patients undergoing a postoperative enhanced abdominal CT scan. Clinical and biological data were collected from medical records. All CT scans were reviewed by a senior radiologist using a standardized form. Results Thirty nine patients (19 men, 20 women), with a mean age of 45 years (range, 19-83), were splenectomized mostly by laparoscopic surgery (n=38), for ITP (n=30) or wAIHA (n=9). Thirty six patients (92%) had received a preventive anticoagulation treatment for a median duration of 24 post-operative days (range, 7-36). Among ITP patients, the mean preoperative platelet count was 95.6 G/L (SD: 97). ITP patients had received corticosteroids (75%), intravenous immunoglobins (43%), or thrombopoietin receptor agonists (41%) in preparation for splenectomy). The overall initial response rate at 2 months from splenectomy was 69% (ITP: 63%, wAIHA: 89 %). The CT scan was performed within 5 days on average (range: 3-12) of splenectomy. The incidence rate of PSVT was 74% (29/39), without discrimination between ITP (22/30, 73%) and wAIHA (7/9, 78%, p = 0.79) (Table 1). There were mostly distal splenic vein (dSVT) (n=14), or dSVT associated with intrahepatic portal vein thrombosis (iPVT) (n=12). There were no mesenteric vein thromboses, 1 extrahepatic portal vein (ePVT) and 1 proximal splenic vein thrombosis (pSVT). Most of the patients were asymptomatic (n=18). No significant association between PSVT and any clinical or biological characteristics such as platelet count, preoperative treatments, thrombocytosis, or efficacy of splenectomy was observed. However, PSVT patients tended to have higher spleen weights (206 g vs 102 g, p = 0.14). Thirteen of the 29 patients with a PVST were treated with a curative anticoagulation treatment for a mean duration of 11 weeks (range, 4-37). Among the 26 patients evaluable in follow-up, the rate of complete thrombosis resolution was 84% (21/26); specifically, 62% (8/13) in anticoagulated patients and 100% in the 13 non anticoagulated patients (9 dSVT, 4 iPVT+dSVT). A portal cavernoma occurred after iPVT+dSVT despite anticoagulation in the context of wAIHA. Anticoagulation complications included abdominal wall hematoma requiring transfusions (n=1) and iron deficiency anemia (n=1). Conclusion In our study, post-splenectomy PSVT was frequent and often asymptomatic, involving mostly intrahepatic portal or distal splenic veins and was resolved in half of the cases without any curative anticoagulative therapy. Our results suggest that a systematic screening of PSVT by CT scan should not be performed in absence of risk factors for thrombosis. Table 1. incidence rate and sites of PSVT. Abstract:
A 71-year-old man with obstructive jaundice was referred to our department. He underwent cholangiojejunostomy 15 years ago for palliative drainage. At that time, he had obstructive jaundice caused by an unresectable pancreatic head tumor. Contrast-enhanced computed tomography (CE-CT) now revealed a mass with low enhancement in the hepatic hilum that occluded the hilar bile duct and infiltrated extensively along the portal vein and hepatic artery. CE-CT also showed marked atrophy of the left hepatic lobe. No swelling or tumors were observed in the pancreas. Serum immunoglobulin G4 (IgG4) levels were as high as 465 mg/dL. Endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) was performed targeting the hepatic hilar lesion. Immunohistological results of the biopsy specimens suggested that the lesion was an IgG4-related hepatic inflammatory pseudotumor (IPT) with no atypical cells. Steroid treatment resulted in rapid clinical improvement. This case suggested the usefulness of EUS-FNB for diagnosing IgG4-related hepatic hilar IPT.

Abstract:
The involvement of nitric oxide (NO) in the vascular escape from norepinephrine (NE)-induced vasoconstriction was investigated in the hepatic arterial vasculature of anesthetized cats. The hepatic artery was perfused by free blood flow or pump-controlled constant-flow, and NE (0.15 and 0.3 μg ⋅ kg−1 ⋅ min−1, respectively) was infused through the portal vein. In the free-flow perfusion model, the NE-induced hepatic vasoconstriction recovered from the maximum point of the constriction, resulting in 36.6 ± 5.9% vascular escape. Blockade of NO formation with N ω-nitro-l-arginine methyl ester (l-NAME, 2.5 mg/kg ipv) potentiated NE-induced maximum vasoconstriction, and the potentiation was reversed byl-arginine (75 mg/kg ipv). Furthermore, NE-induced vasoconstriction became more stable after l-NAME, resulting in an inhibition of vascular escape (7.5 ± 3.3%), and the inhibition was reversed by l-arginine (23.0 ± 6.4%). Similar potentiation of NE-induced vasoconstriction and inhibition of hepatic vascular escape by l-NAME (40.4 ± 4.3% control vs. 10.2 ± 3.7% post-l-NAME escape) and the reversal byl-arginine were also observed in the constant-flow perfusion model. The data suggest that NO is the major endogenous mediator involved in the hepatic vascular escape from NE-induced vasoconstriction.

Abstract:
IntroductionIntimate partner violence (IPV) considerably harms the health, safety and well-being of women. In response, public health systems around the globe have been gradually implementing strategies. In particular, low-income and middle-income countries (LMIC) have been developing innovative interventions in primary healthcare (PHC) addressing the problem. This paper describes a protocol for a systematic review of studies addressing the impacts and outcomes of PHC centre interventions addressing IPV against women from LMIC.Methods and analysisA systematic search for studies will be conducted in African Index Medicus, Africa Portal Digital Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase, Index Medicus for the Southeast Asia Region, IndMed, Latin American and Caribbean Health Science Literature Database (LILACS), Medecins Sans Frontieres, MEDLINE, Minority Health and Health Equity Archive, ProQuest, PsycINFO, Scientific Electronic Library Online, (SciELO) and Social Policy and Practice. Studies will be in English, Spanish and Portuguese, published between 2007 and 2017, addressing IPV against women from LMIC, whose data quantitatively report on the impacts and outcomes for survivors and/or workers and/or public health systems preintervention and postintervention. Two trilingual reviewers will independently screen for study eligibility and data extraction, and a librarian will cross-check for compliance. Risk of bias and quality assessment of studies will be measured according to: (1) the Cochrane Collaboration’s tool for assessing risk of bias for randomised controlled trials and (2) the Methodological Index for Non-Randomised Studies (MINORS). Data will be analysed and summarised using meta-analysis and narrative description of the evidence across studies. This systematic review will be reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Protocols(PRISMA P) guidelines.Ethics and disseminationThis systematic review will be based on published studies, thus not requiring ethical approval. Findings will be presented in conferences and published in a peer-reviewed journal.PROSPERO registration numberCRD42017069261.

Abstract:

Abstract:
A Portopulmonary Venous Anastomosis (PPVA) is a direct vascular connection between the portal venous system and the pulmonary veins, close to the left atrium. We describe a patient with advanced cirrhosis and moderately severe Hepatopulmonary Syndrome (HPS) in whom (PPVA) was demonstrated by a high resolution computed tomographic scan. In order to reduce the symptoms of hypoxia in our patient, we sequentially performed Transjugular Intrahepatic Portosystemic Shunt (TIPS) followed by a catheter based obliteration of the PPVA. The case raised three questions which were addressed through an extensive literature review. Among patients with advanced cirrhosis, what is the frequency of PPVA? Our literature review suggests that the frequency of finding a PPVA in patients with portal hypertension depends on the imaging technique used. Transhepatic or transvenous portal venography may demonstrate PPVA in about 20% of patients with cirrhosis and varices. Limited published experience with contrast enhanced (bubble) echocardiography suggests that PPVA may be present in about 30% of such patients. An increasing number of case reports of PPVA have been published in recent years. This observation indicates that routine use of High Resolution Computed Tomography (HRCT) and magnetic resonance imaging is able to identify PPVA in cirrhotic patients. The visualization of PPVA with HRCT depends on the timing of the contrast injection and the expertise of the viewer. The present report describes three patients with portopulmonary venous anastomosis (PPVA) in whom balloon-occluded retrograde transvenous obliteration (B-RTO) of gastric varices was attempted. No patients had a gastrorenal shunt. In one patient, after an approach from the inferior phrenic vein (IPV), the PPVA was embolized with the use of microcoils. Portopulmonary Venous Anastomosis Detected at Balloon-occluded Retrograde Transvenous Obliteration for Gastric Varices among cirrhotic patients with PPVA, what is the clinical significance of the PPVA? There are multiple causes of clinically significant hypoxia in patients with cirrhosis. A PPVA is a right to left shunt that, theoretically, could be associated with clinically significant systemic arterial hypoxemia or emboli. There is not definitive evidence in the published literature that PPVA alone causes clinically significant hypoxia in cirrhotic patients. PPVA is, however, a documented important risk factor for systemic emboli when needle or catheter techniques are used to treat or prevent bleeding from gastroesophageal varices.

Abstract:
The South African Institute for Aquatic Biodiversity (SAIAB) operates several research platforms, which may be used by the broader South African research community (e.g. a marine research vessel and a remotely operated underwater vehicle). SAIAB’s Enterprise-grade data centre, along with expertise in systems administration and biodiversity information management, allow the institute to offer a Biodiversity Information Management Platform. Data hosted by SAIAB is replicated across three data centres, with each centre being at least 250m apart and operating independently. Infrastructure at two data centres replicates in real time, forming a high availability cluster. The third datacentre is dedicated to storing backups. High-capacity tape backup will be added in the near future. As an additional measure, cloud storage is used to store daily extracts of Specify databases, which are retained for one year. In the first instance, the Platform aims to provide SAIAB researchers and associates with biodiversity data curation services. This begins with support for the SAIAB Collections Division, to ensure that voucher specimens, tissue samples and associated media are accurately catalogued and can be easily retrieved. Biodiversity data curation is broader than this. It also means that any biodiversity data/metadata (records of species, events, occurrences/observations and traits) can potentially be curated using Specify Software, and standardised and published (subject to relevant policies) to the GBIF Data Portal using the GBIF Integrated Publishing Toolkit. The use of Specify Software to curate biodiveristy data that do not represent voucher specimens (e.g. underwater images and video) is a new research project within SAIAB, which has the potential to be extended beyond SAIAB. A new national initiative, the Natural Science Collections Facility (NSCF), was launched in 2017 to reinvigorate natural science museums across the country, to halt deterioration of specimens and improve capacity for specimen and data curation. In support of the NSCF, the SAIAB platform is offered to natural science museums in South Africa (excluding herbaria, which are all part of or affiliated with SANBI, and therefore accommodated by a different system). Each museum will be provided with a webserver, Specify 7 database, Specify web portal and IPT server. In offering this platform to the broader South African Biodiversity Science community, SAIAB is primarily motivated by the potential for collaborative research in capacity development for biodiversity data curation / information management, using Specify Software. The first research project will examine participating museums’ capacity to use the Specify Workbench sustainably, to import new voucher/occurrence records generated by fieldwork. The requisite training to enhance this potential will be provided. The Natural Science Collections Facility (NSCF) is an important collaborator in the context of enhancing the general state of South Africa’s specimen collections, and the Specify Collections Consortium is an important collaborator, specifically for support.

Abstract:

Abstract:
In this thesis I investigate intimate partner violence (IPV) against women in heterosexual relationships by analysing the accounts of women and men in the Anglophone Caribbean country of St. Vincent and the Grenadines. Since IPV occurs in the context of a range of abusive practices (Dobash and Dobash 2004) participants' talk on the use and experiences of violent acts, violent threats, as well as other controlling and coercive tactics are examined as part of this study. Analytically, I focus on the points at which discourses of gender converge with narratives of violence. In other words, the current work examines the ways in which participants construct, (re)produce, disturb and/or negotiate gender in their accounts of IPV, and the kinds of power dynamics that are implicated in these verbal performances. I apply a feminist poststructuralist framework to the study of IPV against women. Synthesising feminist theories of gender and power, and poststructuralist insights on language, subjectivity, social processes and institutions, feminist poststructuralism holds that hegemonic discourses of gender are used to subjugate women (Weedon 1997; Gavey 1990). The points at which individuals complicate dominant discursive practices will also be assessed as part of this approach. In-depth interviews conducted with 34 participants - 19 women and 15 men - between 2007 and 2008 are analysed by using a version of discourse analysis (DA) compatible with the feminist poststructuralist framework outlined in the thesis. My analysis begins by highlighting the ways in which narratives of gender inscribe asymmetrical relations of power. The focus then shifts to a comparison of women's and men's accounts on a range of abusive acts. Traditional scripts on gender are often used to police the boundaries of femininities and masculinities, tying these to female and male bodies respectively. This is the context in which control, coercion, violence and violent threats are discussed in these accounts. Understandings of manhood and womanhood also emerge in the analysis of the strategies used to explain violence. I conclude with a summary and discussion of the analysis, and I suggest possible areas for further research on IPV in the Anglophone Caribbean.

Abstract:
Abstract The tracer-based production logging technology can be used to obtain the well production data continuously for several years without the need for risky well interventions and expensive equipment. The paper examines the case of placing polymer-coated tracers dopped proppant in a horizontal well with ten multi-stage frac intervals and using two different tracers dopped proppant codes for two frac ports (the first and the last ones) to identify the performance of the far and near zones of a hydraulic fracture. Upon the completion of the hydraulic fracturing operations, the collected reservoir fluid samples were studied in the laboratory. Chemical tracers contained in the samples were detected by flow cytofluorometry using custom-tailored machine learning-based software. The studies helped identify the productivity of each frac port, calculate the contribution of each port in percentage points, and also evaluate the productivity of the near and far hydraulic fracture zones in the first and the last intervals. The analysis provided data on the exact content of oil and water in the production profile for each frac interval. The results of tracer-based logging in the well in question revealed that the interval productivity is changing in the course of several months of surveillance. The most productive ports and those showing increasing oil flow rate were identified during quantitative analysis. The use of tracer dopped proppant with different codes within one multi-stage frac interval enabled detecting a peak release of chemical tracers from the far fracture zone in the initial periods of well operation followed by a consistent smoothing of the far and near zones’ production profiles. Laboratory analysis of reservoir fluid samples and hydraulic fracturing simulations proved the uniform distribution of proppant across the entire reservoir pay zone and laid the foundation for further research required to better understand the fracture geometry and reduce uncertainties in production optimization operations.

Abstract:
Abstract Sustained casing pressure (SCP) is a very costly event for any operator either at production phase or at the end of a well’s lifecycle. SCP is a result of incomplete hydraulic isolation across hydrocarbon bearing zone. In one of the gas fields in Malaysia, notoriously known for shallow gas hazard, drilled development wells which have reportedly been suffering SCP. In the past, various improvements in cement slurry design and placement methods were deployed in order to provide complete zonal isolation, especially at the shallow gas sand, yet SCP issue was encountered occasionally. In the current development campaign, different strategy to providing annulus sealing was adopted. This paper discusses proactive steps taken in the slurry design, fit together with the dual stage cementing approach, as a primary means of placing cement above the shallow hazard interval. During the design phase, essential key parameters that would lead to successful placement of cement in the annulus as well as unique slurry design that suits for two stage cementing methods were studied. Risk involved in first stage cementing is one of the most important steps that should be analyzed in detail and put mitigation measures in place to ensure the second stage cement job can be performed as planned. In addition to the slurry properties, such as fluid-loss value, gas-tightness, etc., thickening time and top of cement (TOC) of the lead slurry in the first stage cement job has become enormously critical in designing dual stage cementing job in order to assure cement ports in the stage collar are not covered with hard cement forcing the termination of second stage job prematurely. Besides cementing design, careful selection of the stage collar location and casing annulus packer in the string is also of significant importance in leading to successful two stage cement job. Two development wells with above approached has been delivered and no sustained casing pressure has been experienced. This proactive approach to use two stage cementing as primary plan has proven to successfully eliminate the risk of SCP, which was a frequent struggle in their sister wells drilled with primary cementing in the past in the same field. The risk analysis combined with careful considerations of critical cementing design parameters and selection of stage tool location have become a novel approach to combat against SCP in this gas field.
