Abstract:
Mechanical testing based on ISO 14801 standard is generally used to evaluate the performance of the dental implant system according to material and design changes. However, the test method is difficult to reflect on the clinical environment because the ISO 14801 standard does not take into account the various loads from different directions during chewing motion. In addition, the fracture pattern of the implant system can occur both in the horizontal and the vertical directions. Therefore, the purpose of this study was to compare fatigue characteristics and fracture patterns between single directional loading conditions based on the ISO 14801 standard and multi-directional loading condition. Firstly, the static test was performed on five specimens to derive the fatigue load, and the fatigue load was chosen as 40% of the maximum load measured in the static test. Subsequently, the fatigue test was performed considering the single axial/occlusal (AO), AO with facial/lingual (AOFL) and AO with mesial/distal (AOMD) directions, and five specimens were used for each fatigue loading modes. In order to analyze the fatigue characteristics, the fatigue cycle at the time of specimen fracture and displacement change of the specimen every 500 cycles were measured. Field emission scanning electron microscopy (FE-SEM) was used to analyze the fracture patterns and the fracture surface. Compared to the AO group, the fatigue cycle of the AOFL and AOMD groups showed lower about five times, while the displacement gradually increased with every 500 cycles. From FE-SEM results, there were no different surface morphology characteristics among three groups. However, the AOMD group showed a vertical slip band. Therefore, our results suggest that the multi-directional loading mode under the worst-case environment can reproduce the vertical fracture pattern in the clinical situation and may be essential to reflect on the dental implant design including connection types and surface treatments.

Abstract:
Objective: To propose a methodology based on virtual simulation to assist in the design proposals of dental implants. Methods: The finite element method (FEM) was used to analyze the biomechanical dental implant system behavior, determining von Mises stress distribution induced by functional loads, varying parameter as load direction and geometric characteristic of the implant (taper, length, abutment angulation, thread pitch and width pitch). A final design was obtained by considering the parameters that showed improved performance. The estimated lifetime of the final design was calculated by reproducing in a virtual way the experimental fatigue test required by the ISO:14801 standards. Results: For all the studied cases, the maximum stresses were obtained in the connecting screw under oblique loads (OLs). The estimated lifetime for this critical part is at least 5 × 106 cycles, which meets the requirement of the ISO:14801. In bone tissue, the largest stresses were concentrated in cortical bone, in the zone surrounding the implant, in good agreement with previous reports. Conclusions: A dental implant design was obtained and validated through a simple and efficient methodology based on the application of numerical methods and computer simulations.

Fatigue is the most common mechanical failure type in dental implants. ISO 14801 standardizes fatigue testing of dental implants, providing the load-life curve which is most useful for comparing the fatigue behavior of different dental implant designs. Based on it, many works were published in the dental implant literature, comparing different materials, component geometries, connection types, surface treatments, etc. These works are useful for clinicians in order to identify the best options available in the market. The present work is intended not for clinicians but for dental implant manufacturers, developing a design tool that combines Finite Element Analysis, fatigue formulation and ISO 14801 experimental tests. For that purpose, 46 experimental tests were performed on BTI INTERNA® IIPSCA4513 implants joined with INPPTU44 abutments by means of INTTUH prosthetic screws under three different tightening torque magnitudes. Then, the load case was reproduced in a FE model from where the nominal stress state in the fatigue critical section was worked out. Finally, Walker criterion was used to represent accurately the effects of mean stress and predict fatigue life of the studied dental implant assembly, which can be extended to most of the products of BTI manufacturer. By means of this tool, dental implant manufacturers will be able to identify the critical design and assembly parameters in terms of fatigue behavior, evaluate their influence in preliminary design stages and consequently design dental implants with significantly better fatigue response which in turn will reduce future clinical incidences.

(1) Background: In today's dentistry, implantology has become a therapeutic resource of choice in certain clinical situations. The design of implants has evolved in several aspects since their inception. Dental implants were initially designed with an external hex connection, although due to force transmission and security in the adjustment of the prosthesis, later implants featured an internal hex connection. This study aims to analyse the mechanical properties of two types of implants (an internal connection and an external connection) from the same manufacturer and their different prosthetic components (union screw between implant and prosthetic abutment, and the abutment itself) when subjected to different types of load. (2) Materials and methods: Intraosseous dental implants of similar shape, design and size, although different in type of connection (external vs. internal), were studied. The specifications of the UNI EN ISO 14801 test standard were used, with all determinations being carried out three times. Finally, the dimensional characterisation of the samples analysed after the dynamic load study was carried out, and the values of both study groups were compared by means of the non-parametric Mann–Whitney U test to find statistically significant differences (p < 0.05). (3) Results: For the static characterisation test, we found between 610.9 N and 986.1 N for the external connection and between 1263.6 N and 1324 N for the internal connection (p = 0.011). All of the dynamic load tests were positive and there was no failure in any of the components studied. (4) Conclusions: After the analysis of the samples studied in vitro, satisfactory results were obtained, demonstrating that both connections can support considerable mechanical loads according to international standards (UNI EN ISO 14801).

Mechanical complications in implant-supported fixed dental prostheses are often related to implant and prosthetic design. Although the current ISO 14801 provides a framework for the evaluation of dental implant mechanical reliability, strict adherence to it may be difficult to achieve due to the large number of test specimens which it requires as well as the fact that it does not offer any probabilistic reference for determining the endurance limit. In order to address these issues, a new software program called ProFatigue is presented as a potentially powerful tool to optimize fatigue testing of implant-supported prostheses. The present work provides a brief description of some concepts such as load, fatigue and stress-number of cycles to failure curves (S-N curves), before subsequently describing the current regulatory situation. After analyzing the two most recent versions of the ISO recommendation (from 2008 and 2016), some limitations inherent to the experimental methods which they propose are highlighted. Finally, the main advantages and instructions for the correct implementation of the ProFatigue free software are given. This software will contribute to improving the performance of fatigue testing in a more accurate and optimized way, helping researchers to gain a better understanding of the behavior of dental implants in this type of mechanical test.

The market share of zirconia (ZrO2) dental implants is steadily increasing. This material comprises a polymorphous character with three temperature-dependent crystalline structures, namely monoclinic (m), tetragonal (t) and cubic (c) phases. Special attention is given to the tetragonal phase when maintained in a metastable state at room temperature. Metastable tetragonal grains allow for the beneficial phenomenon of Phase Transformation Toughening (PTT), resulting in a high fracture resistance, but may lead to an undesired surface transformation to the monoclinic phase in a humid environment (low-temperature degradation, LTD, often referred to as ‘ageing’). Today, the clinical safety of zirconia dental implants by means of long-term stability is being addressed by two international ISO standards. These standards impose different experimental setups concerning the dynamic fatigue resistance of the final product (ISO 14801) or the ageing behavior of a standardized sample (ISO 13356) separately. However, when evaluating zirconia dental implants pre-clinically, oral environmental conditions should be simulated to the extent possible by combining a hydrothermal treatment and dynamic fatigue. For failure analysis, phase transformation might be quantified by non-destructive techniques, such as X-Ray Diffraction (XRD) or Raman spectroscopy, whereas Scanning Electron Microscopy (SEM) of cross-sections or Focused Ion Beam (FIB) sections might be used for visualization of the monoclinic layer growth in depth. Finally, a minimum load should be defined for static loading to fracture. The purpose of this communication is to contribute to the current discussion on how to optimize the aforementioned standards in order to guarantee clinical safety for the patients.

Most modern dental implants differentiate in regard to the fixation of the abutment into two main categories - the external or internal hexagon or octagon. We performed mechanical tests according to the ČSN EN ISO 14801 standard on a dental implant variant using the external hex. We found that failure of all implant specimens occured below the screw head. To improve the current geometry, we performed numerical analysis of an alternative variant (internal hex) and compared it with analysis of the current geometry (external hex). It was found that the stress distribution of the variant with internal hex is preferable to the old variant. Although extreme values of shear stress in the corresponding plane of loading are higher, they do not concentrate below the screw head, where the screw itself is thinner and more prone to breaking. Therefore, it seems that the new variant of the dental implant is stronger, which is still to be proven by mechanical tests.

Powder metallurgy (PM) commercial purity titanium (CP Ti) was fabricated and studied, with an aim of utilization for dental application. PM CP Ti was manufactured using a cost effective approach, where affordable hydrogenation–dehydrogenation (HDH) process Ti 99.4 wt.% powder was consolidated via the following sequence of PM techniques: cold isostatic pressing, warm vacuum pressing at 420 °C and warm direct extrusion at 500 °C. The paper presents the first studies on processing, microstructure, testing of mechanical properties, fatigue performance and bonding strength with different veneer coatings. By employed consolidation process sound material with low porosity (1.5%) and sustained oxygen content (0.21 wt.%) was attained. The tensile properties obtained for PM CP Ti (UTS = 701 MPa, YS0.2 = 512 MPa, ε = 13 %) were improved over to those for cast / milled CP Ti Grade 4 reference, the material commonly used in dentistry. Tested using the ISO 14801 standard for dental implants, the samples machined from PM CP Ti showed fatigue performance similar to CP Ti Grade 4. PM CP Ti used as a metal base material in restoration metal – ceramic systems showed very good bond strength with three commercially available veneering ceramics and complied with the ISO 9693 standard. Within the limitations of this paper, the preliminary results demonstrated that performance of economic PM CP Ti is equal or superior to CP Ti Grade 4 reference material and it can be used in prosthodontics.

Ce travail de recherche vise à développer des méthodologies d’évaluation mécanique des implants dentaires, de leurs composants prothétiques et de l’interaction avec les tissus osseux de la mâchoire. D’après l’Organisation Mondiale de la Santé (OMS), 15 à 20% de la population âgée de 35 à 44 ans est atteint de problèmes d’édentation partielle et près de 30% de la population âgée de 65 à 74 d’édentation totale. Cela représente donc approximativement 7 millions de personnes en France ayant subi des pertes de dents durant leur vie. Les traitements proposés par l’implantologie dentaire sont devenus des solutions efficaces pour les problèmes d’édentation avec des taux de réussite cliniques supérieurs à 90%. Cependant, des complications cliniques existent, caractérisées par des inflammations ou des pertes partielles des tissus environnants. Les dentistes étant responsables du choix du traitement thérapeutique pour leurs patients, la prise d’une décision appropriée est capitale et exige une compréhension des risques et des bénéfices. C’est dans ce contexte qu’une première méthodologie d’évaluation a été développée pour améliorer le design des implants dentaires de type endosseux, dans les conditions de la norme de certification ISO 14801. La méthodologie est basée sur des approches numériques par Analyse d’Eléments Finis. Ensuite, des approches théoriques et des données cliniques ont été utilisées pour évaluer le comportement mécanique des tissus osseux de la mâchoire face aux différentes configurations par implants dentaires et leurs composants prothétiques
The research purpose in this study is to develop of mechanical evaluation methods to dental implants, their prosthetic components and the interactions with bone tissues in mandible zone. According to the World Health Organization (WHO), 15 to 20% of population from 35 to 44 years old is reached of partial edentulism problems and about 30% of population from 65 to 74 years old is reached of complete edentulism problems. This represents approximately 7 million people in France who have lost teeth during their lifetime. Treatments proposed by dental implantology have become effective solutions for edentulous patients with clinical success rates greater than 90%. However, clinical complications exist, characterized by inflammation or partial losses of the surrounding tissues. Dentists are responsible for the treatment choice for their patients; an appropriate decision is crucial and requires an understanding of the benefits and risks. In this context, a first methodology was developed for evaluated the endosseous dental implants under the conditions of the ISO 14801 certification standard. This methodology is based with an approach numerical by Finite Element Analysis (FEA). Furthermore, theoretical approaches and clinical data were used to evaluate the mechanical resistance of the bone tissues in the jaw with different dental implant configurations and their prosthetic components

Today the ISO 14001 system is used within a rather wide spectrum of firms and industries, even if the early adopters of the standard mainly where organizations in the heavy industry and manufacturing sectors (Peglaue and Baxter, 2007). Many of those early adopters have now been certified for over 10 years, and during the years several surveys and case studies have been conducted to pinpoint the motives, benefits and challenges for why organizations chose to implement and maintain the system (Massoud, 2010; Jiang and Bansal, 2003). However, one issue with research about ISO 14001 up until now is that most studies have focused upon a managerial perspective where the managers´ attitudes have been in focus rather than the employees (Boiral, 2007). Therefore, this case study set out to explore how the employees, the production operators, inside one certified organization are involved in ISO 14001 through a Grounded Theory approach. The results of the study showed that the production operators relied upon different strategies in order to legitimize their involvement in ISO 14001. Further on, the study shows that the production operators where involved in ISO 14001 at a level which was required in order to keep the certification. However, they could not obtain enough legitimacy to motivate an involvement in ISO 14001 which extended what was required in order to keep the certification. Grounded in the discovery of the different strategies used by the employees, this study argues that environmental awareness could be seen as an internal barrier and as a source for unsatisfied employees. Something which risks emphasizes a feeling of being involved in a rational myth (Boiral, 2007).

Arbetet med miljöfrågorna är svåra att strukturera och därför har olika internationella miljöstandarder skapats. SS-EN ISO 14000 är en serie internationella miljöstandarder, som i dagligt tal kallas ISO 14000 och i denna standard ingår ISO 14001 vilken kan användas som grund vid uppbyggandet av ett miljöledningssystem och det är införandet av denna som analysen vilar på. Syftet med uppsatsen är att undersöka vilka hinder som kan uppstå vid införandet av ISO 14001-standarden. Även att identifiera vilka lösningar som finns att tillgå gällande hindren. Dessutom utforskas vilka de olika incitamenten är som driver en verksamhet till att vilja integrera miljöfrågor bland övriga beslut. Slutligen granskas fördelar med att införa miljöledningssystemet ISO 14001.

Företagens tillämpning av miljöledningssystemet ISO 14001 har sedan etableringen år 1994 fram till idag gradvist stegrat. Miljöfrågor får allt mer uppmärksamhet av allmänheten, samtidigt ställs det krav på företag om att de ska driva sin verksamhet utifrån ett miljövänligt perspektiv.

Miljöledningssystemet ISO 14001 är en internationellt känd standard och förekommer oftare bland företag som innehar en certifiering. Investering av ett miljöledningssystem för med sig stora kostnader och företaget förväntas arbeta mycket och tillsammans för att få det att fungera väl. Det finns företag som driver sin verksamhet samtidigt som de tar hänsyn till miljön, men det är först när företag använder sig av ett miljöledningssystem som deras miljöarbete får betydelse. Det kan förklaras med den extra post som tillkommer; miljörevision. Det innebär att företagets miljöarbete granskas av externa revisorer. De ser till att ständiga förbättringar genomförs och att miljötänkandet regelbundet finns i företagets processer. Miljöledningssystemet hjälper företaget att få bättre struktur på sin verksamhet genom dokumentation. Denna dokumentation leder till ökad kontroll, effektivare resursanvändning och underlättar för den nyanställda att komma in i arbetet.

Föreliggande undersökning fokuserar på företag som är etablerade i Karlstad och som har miljöledningssystemet ISO 14001. Undersökningen inkluderar sammanlagt sju intervjuer med miljösamordnare på företagen. Resultaten av undersökning visar att företag miljöcertifierar sig främst i syfte att tillfredställa intressenternas krav och för deras verksamhets fortsatta existens, den egna viljan har alltså väldigt liten inverkan. Trots att investeringen kräver mycket av personalen och kostar en del är det lönsamt eftersom intressenterna är viktiga för företagets fortlevnad. Om miljöledningssystemet bidragit till förändrad kundtillströmning, resultat och ställning är det enligt företagen svårt att mäta. Dock spekuleras det om att miljöledningssystemet har varit positivt för företagets helhetsbild vilket också har påverkan på kundtillströmningen. Samhället förändras mer eller mindre ständigt vilket i många fall har inverkan på företagen, en förändring såsom exempelvis lågkonjunkturen idag påstår de deltagande företagen att det inte kommer att påverka nuvarande miljöarbetet negativt. Miljöarbetet är något som får kosta trots sämre tider.

Vi har gjort en fallstudie som utforskar hur ledarskapet påverkar och påverkas vid entillämpning av ISO 14001, samt hur tillämpningen av standarden är utformad påstudiens företag. Analysen har genomförts med hjälp av institutionell teori somteoretisk referensram. Vi kom fram till att ISO 14001 inte leder till eninterorganisatorisk homogenisering mellan olika företag, inomsamhällsplaneringsbranschen som vårt fallföretag agerar i. Vidare fastslog vi även enparadox i utformningen av ISO 14001-systemet hos företaget; otydliga istället förtydliga krav ökar chansen för att behålla certifieringen. Till sists kom vi också framtill att ledarskapets svårighet att kommunicera aktörernas rätt att översätta ISO 14001i sin praktik, beror på att ISO 9001 och ISO 14001 hos vårt fallföretag är utformatsom ett gemensamt system. Där ISO 9001 ligger i fokus. Uppsatsen syfte är att bidratill existerande institutionell teori.

I och med lanseringen av det nya ledningssystemet för trafiksäkerhet, ISO 39001, vill Trafikverket förstå vilka framgångsfaktorer som finns för ett väletablerat ledningssystem. Detta görs genom att utvärdera ett ledningssystem som har kommit att få god global spridning och etablering, miljöledningssystemet ISO 14001. Syftet med denna uppsats är att utreda vilka framgångsfaktorerna är för etableringen och spridningen av miljöledningssystemet, ISO 14001, för att sedan se om dessa går att applicera på trafiksäkerhetsledningssystemet, ISO 39001. För att besvara detta genomfördes en litteraturstudie som kompletterades med en empirisk undersökning. Den empiriska undersökningen utgjordes av semistrukturerade intervjuer som genomfördes med fyra aktörer med god kännedom om den nya trafiksäkerhetsstandarden. Uppsatsen visade att de framgångsfaktorerna som kan relateras starkast till ISO 14001 är dess koppling till ISO 9001, den rådande tidsandan samt fördelar som ökade marknadsandelar genom god publicitet vid certifiering. De krav som ställdes på underleverantörer från redan certifierade företag är också en betydande faktor. Även om trafiksäkerhet fått större utrymme på världsagendan under senare år var inte trafiksäkerhetsengagemanget lika stort vid lanseringen av ISO 39001 som miljöengagemanget var vid lanseringen av ISO 14001. Spridningen av ISO 39001 beror även på regeringsstöd, med förhoppning om att den införs i lagstiftningen då nollvisionen är ett riksdagsbeslut. Den offentliga sektorn tog initiativ till arbetet med ISO 39001 och efterfrågan kan bidra till att ISO 39001 når stora framgångar inom den offentliga sektorn. ISO 39001 har föregåtts av en nollvision och en nationell branschspecifik säkerhetsstandard som har banat vägen för standarden inom vissa sektorer. De verksamheter som inför ISO 39001 kommer troligtvis göra detta för att uppfylla marknadskrav och myndighetskrav. En viktig framgångsfaktor för ISO 39001 är att den ger förutsättningar för företag att ställa krav på sina underleverantörer, något som bekräftades vid intervjuerna.

O objetivo principal desta pesquisa foi realizar um estudo para a implementação do Sistema de Gestão Ambiental - ISO 14001, na Indústria de Móveis Rudnick S. A..Trata-se, portanto, de um estudo de caso e, no decorrer do processo, a abordagem utilizada foi a qualitativa. Para a realização da pesquisa, os meios utilizados foram a pesquisa bibliográfica e a pesquisa de campo. Com relação aos fins, este trabalho foi caracterizado como descritivo e explicativo. Em relação aos dados, esses foram coletados através de fontes primárias, utilizando-se de entrevistas direcionadas e tendo, como informantes, pessoas das diversas áreas afins e com observações in loco. As informações secundárias foram obtidas através de documentos legais da empresa e de diversas publicações pertinentes à pesquisa. O estudo, além de demonstrar a importância da gestão ambiental, apresentou o levantamento dos aspectos e impactos ambientais provocados pelas atividades da indústria e ações que a empresa já vem adotando em prol do meio ambiente. Assim, com base na análise de dados da pesquisa, foi possível elaborar os fluxogramas de todas as etapas do processo de fabricação dos móveis, bem como, a identificação e avaliação dos aspectos e impactos ambientais decorrentes do processo e sua disposição final e, por fim, foi proposto um modelo de Programa de Gestão Ambiental e um Manual de Gestão Ambiental para a empresa, com base na ISO 14001. Neste contexto, constatou-se que a empresa tem grande interesse pela implementação do SGA - ISO 14001 no seu parque fabril, pois a mesma demonstra certa facilidade em adequar-se à Norma, já que vem operando com um sistema de gerenciamento de resíduos industriais, seja para atender as exigências normativas ou por pressões do mercado internacional, ressaltando o compromisso da empresa com o meio ambiente, sendo este um processo irreversível no mundo dos negócios.

Our research is based on data triangulation methodology by which we are going to answer the question with a combination of two elements: the design and development in combination with life cycle perspective according to ISO 14001:2015 and organization consider the life cycle perspective when they design and develop their products, in a modified form introducing many new aspects of life-cycle thinking. This Master’sthesis aims to discuss the Sustainability approach through the use of Environmental Management Standards (EMS), the results achieved by organizations that implement and certify those EMS, and a special focus on the current process of ISO 14001:2015 revision and the logic behind it. Revisiting the concept of Sustainability, the status of the International Organization for Standardization 14001, requirements that related to that life cycle perspective in ISO 14001:2015, eco-design, circular economy and its expected outcomes are discussed. The ISO 14001:2015 revision will have major impacts on the more than 300,000 worldwide certified organizations and on the many professionals that work with it. Analysis of the development of a sustainability portfolio within a globally-operating manufacturing company, we came different illustrate the kinds of life cycle work involved in dealing with activities and interests, connecting activities and interests into action-nets, performing life cycle practices, and spreading the life cycle idea. Finally, we discuss implications of life cycle work for research in the field of organization and management studies and questions related to the topic with quality engineers within the organization.

Allt fler företag väljer att föra ett aktivt miljöarbete och marknadsföra sig som miljöanpassade. Många företag väljer att följa olika miljöstandarder som är ett verktyg för att miljöarbetet utförs på strukturerat och effektivt sätt. Enligt miljöstandarden ISO 14001 kan företag införa och certifiera ett miljöledningssystem, som är en arbetsmodell som företaget skall följa i sitt miljöarbete. Rapporten syftar till att genom intervjuer kartlägga de förändringar införandet av ett miljöledningssystem innebär för mindre företag. Rapporten kartlägger både fördelar och nackdelar systemet medfört. Rapporten tar även upp följande frågor: Vilka förväntningar hade företagen innan implementationen av miljöledningssystemet och har dessa förväntningar uppfyllts? Har den kostnad miljöledningssystemet skapat uppvägts av de ekonomiska fördelar miljöledningssystemet skapat. Hur har företagens miljöpåverkan förändrats efter införandet av miljöledningssystemet? Undersökningen visar att miljöledningssystemet medfört ett förändringar för företagen till exempel bättre miljötänkande och rutiner för miljöarbete, kostnadsbesparingar, konkurrensfördelar och positivt inställda medarbetare.
