Abstract:
Oxycodone (oxy) is a semi-synthetic opioid commonly used as a pain medication that is also a widely abused prescription drug. While very limited studies have examined the effect of in utero oxy (IUO) exposure on neurodevelopment, a significant gap in knowledge is the effect of IUO compared with postnatal oxy (PNO) exposure on synaptogenesis—a key process in the formation of synapses during brain development—in the exposed offspring. One relatively unexplored form of cell–cell communication associated with brain development in response to IUO and PNO exposure are extracellular vesicles (EVs). EVs are membrane-bound vesicles that serve as carriers of cargo, such as microRNAs (miRNAs). Using RNA-Seq analysis, we identified distinct brain-derived extracellular vesicle (BDEs) miRNA signatures associated with IUO and PNO exposure, including their gene targets, regulating key functional pathways associated with brain development to be more impacted in the IUO offspring. Further treatment of primary 14-day in vitro (DIV) neurons with IUO BDEs caused a significant reduction in spine density compared to treatment with BDEs from PNO and saline groups. In summary, our studies identified for the first time, key BDE miRNA signatures in IUO- and PNO-exposed offspring, which could impact their brain development as well as synaptic function.

Abstract:
BackgroundFever of unknown origin (FUO) and inflammation of unknown origin (IUO) are diagnostically challenging conditions. Diagnosis of underlying disease may be improved by 18F-fluorodesoxyglucose positron emission tomography (18F-FDG-PET).MethodsProspective study to test diagnostic utility of 18F-FDG-PET/CT in a large cohort of patients with FUO or IUO and to define parameters that increase the likelihood of diagnostic 18F-FDG-PET/CT. Patients with FUO or IUO received 18F-FDG-PET/CT scanning in addition to standard diagnostic work-up. 18F-FDG-PET/CT results were classified as helpful or non-helpful in establishing final diagnosis. Binary logistic regression was used to identify clinical parameters associated with a diagnostic 18F-FDG-PET/CT.Results240 patients were enrolled, 72 with FUO, 142 with IUO and 26 had FUO or IUO previously (exFUO/IUO). Diagnosis was established in 190 patients (79.2%). The leading diagnoses were adult-onset Still’s disease (15.3%) in the FUO group, large vessel vasculitis (21.1%) and polymyalgia rheumatica (18.3%) in the IUO group and IgG4-related disease (15.4%) in the exFUO/IUO group. In 136 patients (56.7% of all patients and 71.6% of patients with a diagnosis), 18F-FDG-PET/CT was positive and helpful in finding the diagnosis. Predictive markers for a diagnostic 18F-FDG-PET/CT were age over 50 years (p=0.019), C-reactive protein (CRP) level over 30 mg/L (p=0.002) and absence of fever (p=0.001).Conclusion18F-FDG-PET/CT scanning is helpful in ascertaining the correct diagnosis in more than 50% of the cases presenting with FUO and IUO. Absence of intermittent fever, higher age and elevated CRP level increase the likelihood for a diagnostic 18F-FDG-PET/CT.

Abstract:
Menstrual pain is consequent to intense uterine contraction aimed to expel menstrual flow through downstream uterine cervix. Herein it was evaluated whether characteristics of uterine cervix are associated with intensity of menstrual pain. Ultrasound elastography was used to analyze cervix elasticity of 75 consecutive outpatient women. Elasticity was related to intensity of menstrual pain defined by a Visual Analogue Scale (VAS). Four regions of interest (ROI) were considered: internal uterine orifice (IUO), anterior (ACC) and posterior cervical (PCC) compartment and middle cervical canal (MCC). Tissue elasticity, evaluated by color score (from 0.5 = blue/violet (low elasticity) to 3.0 = red (high elasticity), and percent tissue deformation was analyzed. Elasticity of IUO was lower (p = 0.0001) than that of MCC or ACC, and it was negatively related (R2 = 0.428; p = 0.0001) to menstrual VAS (CR −2.17; 95%CI −3.80, −0.54; p = 0.01). Presence of adenomyosis (CR 3.24; 95% CI 1.94, 4.54; p = 0.0001) and cervix tenderness at clinical examination (CR 2.74; 95% CI 1.29, 4.20; p = 0.0004), were also independently related to menstrual VAS. At post hoc analysis, women with vs. without menstrual pain had lower IUO elasticity, expressed as color score (0.72 ± 0.40 vs. 0.92 ± 0.42; p = 0.059), lower percent tissue deformation at IUO (0.09 ± 0.05 vs. 0.13 ± 0.08; p = 0.025), a higher prevalence of cervical tenderness at bimanual examination (36.2% vs. 9.5%; p = 0.022) and a higher prevalence of adenomyosis (46.5% vs. 19.9%; p = 0.04). These preliminary data indicate that IUO elasticity is associated with the presence and the intensity of menstrual pain. Mechanisms determining IUO elasticity are useful to be explored.

Abstract:
This review focuses on the diagnostic value of hybrid F18-FDG Positron Emission Tomography/Computerized tomography (PET/CT) in fever of unknown origin (FUO) and inflammation of unknown origin (IUO). Due to the wide range of possible causes both FUO and IUO remain a clinical challenge for both patients and physicians. In addition, the aetiology of IUO shows the same variation in diseases as the FUO spectrum and probably requires the same diagnostic approach as FUO. There are numerous historically used diagnostic approaches incorporating invasive and non-invasive, and imaging techniques, all with relative high specificity but limited sensitivity. This hampers the generalization of these diagnostic approaches. However, recently published reports show that F18-FDG PET/CT in FUO and IUO has a high sensitivity and a relative non-specificity for malignancy, infection and inflammation. This makes F18-FDG PET/CT an ideal diagnostic tool to start the diagnostic process and to guide subsequent focused diagnostic approaches with higher specificity. In addition, F18-FDG PET/CT has a relative high negative predictive value. Therefore F18 FDG PET/CT should be incorporated in the routine diagnostic work-up of patients with FUO and IUO, preferably at an early stage in the diagnostic process.

Abstract:
Background Fever of unknown origin (FUO) and inflammation of unknown origin (IUO) are challenging medical problems. Previous studies have shown that 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) plays an important role in the diagnosis of FUO, but few studies have explored this diagnostic technique in relation to IUO. Purpose To systematically review and perform a meta-analysis of published data on the diagnostic performance of PET/CT in the diagnosis of FUO and IUO. Material and Methods A comprehensive literature search was performed in accordance with the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines published in March 2018. Meta-analysis of diagnostic performance was performed using STATA 12.0 software. Subgroup analysis was performed by stratification based on study design, number of patients, geographic area, and final diagnosis based on 18F-FDG PET/CT. Meta-regression analyses were performed to recognize heterogeneity. Results Our meta-analysis included 23 studies, comprising a total sample size of 1927 patients. The pooled diagnosis performance was calculated with a per-patient-based analysis: sensitivity = 0.84 (95% confidence interval [CI] = 0.79–0.89), specificity = 0.63 (95% CI = 0.49–0.75), positive likelihood ratio = 2.3 (95% CI = 1.5–3.4), negative likelihood ratio = 0.25 (95% CI = 0.16–0.38), diagnostic odds ratio = 9 (95% CI = 4.0–20), and AUC = 0.84 (95% CI = 0.81–0.87). Conclusion In patients with non-specific symptoms and signs, 18F-FDG PET/CT is very helpful for recognizing and excluding diseases, directing further diagnostic decisions, and avoiding unnecessary invasive examinations. We recommend that 18F-FDG PET/CT should be considered among the first-line diagnostic tools for patients with FUO and IUO.

O objetivo deste trabalho é a análise da variação temporal da cobertura de uso e ocupação do solo de uma região estuarina no litoral centro-norte do Estado de Santa Catarina, reconstruindo as alterações na dinâmica da paisagem em um período de 75 anos (1938 - 2013), além de calcular as variações do Índice de Uso e Ocupação do Solo (IUO) e do Índice de Cobertura Vegetal (ICV). Para a classificação digital de uso e ocupação do solo foi realizada uma classificação visual, em ambiente SIG. Os resultados indicaram que nas décadas de 30, 50 e 70 ocorreram as maiores transformações na área de estudo, sendo que a variação do IUO apontou um crescimento das áreas antropizadas e o ICV demonstrou uma redução das áreas com vegetação. Estudos desta natureza constituem-se ferramentas importantes para a tomada de decisão dos gestores municipais, na busca de um crescimento mais harmônico das cidades, compatibilizando as necessidades dos moradores por moradias e serviços públicos com os remanescentes naturais, que tem igual importância para a qualidade de vida da população.

Abstract:
2071 Background: EGFRvIII is a mutant version of EGF receptor resulting from the genomic deletion of exons 2 through 7 and is expressed only in certain tumors. AMG 595 is an experimental therapeutic specifically targeting EGFRvIII and consists of an EGFRvIII-specific antibody conjugated to the maytansinoid antimicrotubule agent DM-1. Due to its specificity, mechanism of action, and pre-clinical activity, AMG 595 is expected to have clinical effect only in tumors expressing EGFRvIII. Since the reported prevalence of EGFRvIII in glioblastoma multiforme (GBM) is ~30%, prospective selection of patients with EGFRvIII positive tumors was desired for clinical development. An immunohistochemical (IHC) assay developed with Dako using a novel EGFRvIII antibody is currently being employed for patient selection for the AMG 595 phase I study in recurrent GBM (NCT01475006). Methods: An appropriate IHC reagent for human tissue was created using the variable region of a novel EGFRvIII-specific antibody developed using Xenomouse technology. Staining conditions were optimized using Dako pharmDx reagents, the Dako Link 48 Autostainer, and FFPE tissue sections. Confirmatory transcript analyses of adjacent sections were conducted using the NanoString platform. Results: Robust and reproducible staining for EGFRvIII was observed using archived GBM resections. Percentage of stained cells correlated with levels of EGFRvIII transcripts, and tumors without staining did not express EGFRvIII transcripts. Although some tumors exhibited homogenous staining, most were heterogeneous with varying distribution and percentages of stained tumor cells similar to literature reports of IHC analysis utilizing other EGFRvIII-specific antibodies. Tumor samples from patients entering into the AMG 595 Phase 1 study analyzed with this IHC test have displayed the predicted staining prevalence. Conclusions: The developed EGFRvIII IHC assay, approved under an Investigational Device Exemption, is currently being successfully employed to prospectively select patients in an ongoing phase I trial. Use of this well characterized IHC test will enable correlation of clinical outcome and staining characteristics to inform subsequent studies.

Die Temperung mittels Xenon-Blitzlampen (Flash Lamp Annealing - FLA) ist ein Kurzzeittemperverfahren mit Pulszeiten im Bereich von Millisekunden, bei dem nur die Oberfläche eines Substrats erhitzt wird. Durch die Blitzlampentemperung kann der Schichtwiderstand von tansparenten leitfähigen Oxidschichten reduziert und die Transmission im sichtbaren Licht erhöht werden. In dieser Arbeit wurde dies am Beispiel von Indium-Zinn-Oxid (ITO), Indium-Zink-Oxid (IZO) und aluminiumdotiertem Zinkoxid (AZO) gezeigt. Es wurde untersucht, welchen Einfluss verschiedene Prozessparameter wie Energiedichte und Pulszeit des Xenonlichtblitzes, die Absorption der zu behandelnden Schicht, die Gasatmosphäre während der Temperung, die Reflexion im Prozessraum und das Substratmaterial auf die optischen und elektrischen Eigenschaften der Schichten haben. Für ITO-Schichten mit einer Schichtdicke von 150 nm kann der Widerstand von 45 auf <14 Ohm verbessert werden, was vergleichbar mit einer konventionellen Temperung im Umluftofen ist.

The thesis aims to apply the theoretical approach of IPOs and financial services firm valuation on the Czech subsidiary of KBC: ČSOB. The theoretical part reviews the process of IPO with typical features of IPOs, and the pricing of IPO is discussed with the focus on theory of banks valuation. The empirical part of the thesis provides a financial analysis of the ČSOB, business plan and the valuation of the bank as of 30 September 2010 with the price of the actual IPO. This part aims at practical application of the theory of IPO and valuation. Al in all, the aim of the thesis is to apply theoretical knowledge of the IPOs on the practical case of ČSOB. Various analyses such as of KBC, ČSOB, Czech banking market, development of IPOs in CEE are used in order to apply and determine the reasons for the IPO, its timing, price and potential structure of investors.

The main theme of this diploma thesis is Information Security Management System (ISMS) which is based on security standard ISO 27001 and ISO 27002. This thesis is one part of the project of integration ISMS to company RWE. First goal is analysis of actual documentation of RWE. Second goal is proposal of ideal structure of ISMS documentation. Third goal is assignment the parts of RWE documentation to ideal structure of ISMS documentation. Analysis of actual documentation used knowledge about RWE documentation to create overview table with all documents and their relations. Ideal structure of ISMS documentation was based on selected parts of ISO 27001 and multicriterial analysis. Third goal of this thesis was reached by assignment parts of RWE documentation to selected parts of ISO 27001 from the second goal. Contribution of this diploma thesis is the ideal structure of ISMS documentation and form of old RWE documentation assignment, because these goals are usual steps of PDCA cycle of ISMS but they are described briefly and sparsely in security standards and works related to ISMS.

Den här rapporten handlar om Individuella utvecklingsplaner (IUP). Från 1/1-06 gäller en förordning om att alla barn i grundskolan, särskolan, sameskolan och specialskolan ska ha IUP. Syftet med rapporten är att undersöka vilken information lärare och elever fått om IUP, men också ta reda på deras inställning till och uppfattning om IUP.

För att få reda på detta har jag intervjuat fem lärare och fem elever. Lärarna har mellan 3-20 års erfarenhet av skolarbete. Eleverna går i skolår 1-6. Jag har utifrån litteraturen studerat synen på lärande, hur lärare ställer sig till nya utvecklingsplaner och hur det sedan faller ut i praktiken.

Resultatet av intervjuerna blev något nedslående med tanke på att förordningen om IUP redan trätt i kraft. Endast en av de fem lärare jag intervjuat hade kommit igång med sitt IUP arbete. Därför kunde de andra fyra inte svara annat än att de inte märker av IUP i skolans vardagliga arbete. Eleverna gav ungefär samma svar.

Trots det har jag dragit slutsatsen att samtliga lärare och elever har en positiv inställning till IUP och dess införande. Lärarna uppfattar IUP som ett hjälpmedel och en garanti för måluppfyllelse.

Däremot har IUP inte implementerats i det vardagliga arbetet i skolan.

Man kan också se att det saknats gemensamma diskussioner om synen på kunskap och kunskapsutveckling.


This report is about Individual Development planes (IUP). From 1/1-06 a new law requires all schools to draw an IUP for all pupils in primary schools; schools for dysfunction, ethnic schools and special schools. The purpose of this study is to examine what information teachers and pupils have about the IUP and find out what their adjustment and comprehension are to the IUP.

I decided to interview five teachers and five pupils. The teachers have 3-20 years of experience in teaching. The pupils are seven, ten and twelve years old and go to the regular Swedish school.

In the literature I studied different kind of cognitive approaches, how teachers think about new development planes and how it all turns out in reality.

The result of my interwjues was rather negative. Only one of the teachers I interviewed has been working IUP inspired for some years. All the others haven’t started yet. Therefore they couldn’t assert that IUP did any marks in the daily schoolwork. The pupils gave the same answers.

The IUP has not been realized in the daily schoolwork and you can also see that the teachers miss common discussions about cognitive approaches.

Although I have made the conclusion that all the teachers and pupils are positive to the IUP. The teachers think about the IUP as a working tool and a guarantee to attain good results in school.

Abstract:
Today the ISO 14001 system is used within a rather wide spectrum of firms and industries, even if the early adopters of the standard mainly where organizations in the heavy industry and manufacturing sectors (Peglaue and Baxter, 2007). Many of those early adopters have now been certified for over 10 years, and during the years several surveys and case studies have been conducted to pinpoint the motives, benefits and challenges for why organizations chose to implement and maintain the system (Massoud, 2010; Jiang and Bansal, 2003). However, one issue with research about ISO 14001 up until now is that most studies have focused upon a managerial perspective where the managers´ attitudes have been in focus rather than the employees (Boiral, 2007). Therefore, this case study set out to explore how the employees, the production operators, inside one certified organization are involved in ISO 14001 through a Grounded Theory approach. The results of the study showed that the production operators relied upon different strategies in order to legitimize their involvement in ISO 14001. Further on, the study shows that the production operators where involved in ISO 14001 at a level which was required in order to keep the certification. However, they could not obtain enough legitimacy to motivate an involvement in ISO 14001 which extended what was required in order to keep the certification. Grounded in the discovery of the different strategies used by the employees, this study argues that environmental awareness could be seen as an internal barrier and as a source for unsatisfied employees. Something which risks emphasizes a feeling of being involved in a rational myth (Boiral, 2007).

Abstract:
Standarder för ledningssystem är ett hjälpmedel för företag och myndigheter att bedriva arbete i en viss fråga. Spridningen och etableringen av ledningssystemstandarder är beroende av flera faktorer. I och med lanseringen av det nya ledningssystemet för trafiksäkerhet, ISO 39001, vill Trafikverket förstå vilka framgångsfaktorer som finns för ett väletablerat ledningssystem. Detta görs genom att utvärdera ett ledningssystem som har kommit att få god global spridning och etablering, miljöledningssystemet ISO 14001. Syftet med denna uppsats är att utreda vilka framgångsfaktorerna är för etableringen och spridningen av miljöledningssystemet, ISO 14001, för att sedan se om dessa går att applicera på trafiksäkerhetsledningssystemet, ISO 39001. För att besvara detta genomfördes en litteraturstudie som kompletterades med en empirisk undersökning. Den empiriska undersökningen utgjordes av semistrukturerade intervjuer som genomfördes med fyra aktörer med god kännedom om den nya trafiksäkerhetsstandarden. Uppsatsen visade att de framgångsfaktorerna som kan relateras starkast till ISO 14001 är dess koppling till ISO 9001, den rådande tidsandan samt fördelar som ökade marknadsandelar genom god publicitet vid certifiering. De krav som ställdes på underleverantörer från redan certifierade företag är också en betydande faktor. Även om trafiksäkerhet fått större utrymme på världsagendan under senare år var inte trafiksäkerhetsengagemanget lika stort vid lanseringen av ISO 39001 som miljöengagemanget var vid lanseringen av ISO 14001. Spridningen av ISO 39001 beror även på regeringsstöd, med förhoppning om att den införs i lagstiftningen då nollvisionen är ett riksdagsbeslut. Den offentliga sektorn tog initiativ till arbetet med ISO 39001 och efterfrågan kan bidra till att ISO 39001 når stora framgångar inom den offentliga sektorn. ISO 39001 har föregåtts av en nollvision och en nationell branschspecifik säkerhetsstandard som har banat vägen för standarden inom vissa sektorer. De verksamheter som inför ISO 39001 kommer troligtvis göra detta för att uppfylla marknadskrav och myndighetskrav. En viktig framgångsfaktor för ISO 39001 är att den ger förutsättningar för företag att ställa krav på sina underleverantörer, något som bekräftades vid intervjuerna.

Arbetet med miljöfrågorna är svåra att strukturera och därför har olika internationella miljöstandarder skapats. SS-EN ISO 14000 är en serie internationella miljöstandarder, som i dagligt tal kallas ISO 14000 och i denna standard ingår ISO 14001 vilken kan användas som grund vid uppbyggandet av ett miljöledningssystem och det är införandet av denna som analysen vilar på. Syftet med uppsatsen är att undersöka vilka hinder som kan uppstå vid införandet av ISO 14001-standarden. Även att identifiera vilka lösningar som finns att tillgå gällande hindren. Dessutom utforskas vilka de olika incitamenten är som driver en verksamhet till att vilja integrera miljöfrågor bland övriga beslut. Slutligen granskas fördelar med att införa miljöledningssystemet ISO 14001.
