Abstract The purpose of this paper is to present some existence results for coupled fixed point of a (φ,ψ) —contractive condition for mixed monotone operators in metric spaces endowed with a directed graph. Our results generalize the results obtained by Jain et al. in (International Journal of Analysis, Volume 2014, Article ID 586096, 9 pages). Moreover, we have an application to some integral system to support the results.

Gold nanoparticles (GNPs) have proven to be good nano-sources of heat in the presence of specific electromagnetic radiation. This process, in fact, becomes strongly enhanced under plasmon resonance. In particular, the amount of generated heat and the consequent temperature increase depend on the number of GNPs that are collectively excited and on their relative distance. Abstract:
Introduction: Non-union of intertrochanteric and supracondylar fractures of femur are uncommon which results in pain and functional disability, presenting major surgical challenge. We wish to report on two cases regarding endoprosthesis as management for non-union. Case Presentation: Patient A is 54 years old lady sustained closed comminuted intertochanteric fracture of right femur (31A2.3). She underwent open reduction, intramedullary nail with cerclage wire on day 3 post trauma. At 6 months, there is sign of avascular necrosis of femur head with non-union fracture site. Patient still complaining of pain, on non-weight bearing ambulation. At 1 year, patient underwent right proximal femur replacement. At 3 months post replacement, she is able to full weight bear with walking cane without pain. Patient B is 56 years old gentleman sustained Grade IIIa open comminuted fracture of right supracondylar with intercondylar split (33C2.2). He underwent emergency wound debridement, screw fixation with high tibial pin insertion. At 4 months, he underwent distal locking plate of right femur with synthetic bone grafting. At 8 months post fixation, there is non-union comminuted fracture of supracondylar, still unable to weight bear. At 1.5 year post trauma, patient underwent right distal femur replacement. At 3 weeks post replacement, he is able to full weight bear without aid. Discussion: Most failures of treatment occur in unstable fracture patterns of hip. (1) Endoprosthesis may provide as alternative treatment if internal fixation are recognized to be suboptimal, amount and quality of remaining distal bone stock and level of the non-union. (2,3) In both our cases, patient experienced marked improvement in functional capacity and pain free, which are the hallmarks of a successful salvage procedure. (4) Conclusion: Endoprosthesis can be consider as alternative treatment for non-union intertrochanteric and supracondylar fracture of femur, hence providing stable joint, pain free and early return to daily activities. OCT-A is becoming more popular in recent years and there is a high demand to improve the quality of angiograms as well as to extract quantitative information. We applied various processing methods for microvasculature enhancement like Hessian-Frangi to a data set obtained with Bessel and Gaussian OCT systems. Background: “Mishra distribution" of B. K. Sah (2015) has been obtained in honor of Professor A. Mishra, Department of Statistics, Patna University, Patna (Sah, 2015). A one parameter Poisson-Mishra distribution (PMD) of B. K. Sah (2017) has been obtained by compounding Poisson distribution with Mishra distribution. It has been found that this distribution gives better fit to all the discrete data sets which are negative binomial in nature used by Sankarn (1970) and others. A generalisation of PMD has been obtained by mixing the generalised Poisson distribution of Consul and Jain (1973) with the Mishra distribution.Materials and Methods: It is based on the concept of the generalisations of some continuous mixtures of Poisson distribution.Results: Probability density function and the first four moments about origin of the proposed distribution have been obtained. The estimation of parameters of this distribution has been discussed by using the first moment about origin and the probability mass function at x = 0 . This distribution has been fitted to a number of discrete data-sets to which earlier Poisson-Lindley distribution (PLD) and PMD have been fitted.Conclusion: P-value of generalised Poisson-Mishra distribution is greater than PLD and PMD. Background: Ibrutinib, a small molecule inhibitor of Bruton's Tyrosine Kinase (BTK) is approved for use in a variety of lymphomas. Priced at $130,000/year, Ibrutinib imposes a significant financial burden on patients and society[1]. The study serving as the basis for the currently approved dose [2] demonstrated >95% BTK receptor occupancy at a dose of 2.5 mg/kg. Data suggests that lower doses of Ibrutinib are equally effective[3] and dose reductions[4, 5] do not compromise outcome. Objective: To evaluate patient outcomes and cost savings with clinically indicated low dose (LD) of Ibrutinib in a community practice in hematological malignancies. Method: All patients treated with standard and LD Ibrutinib between January 2014 and July 2020 were identified. Reason for dose modification and best responses were abstracted. Patients with inadequate follow up or less than a week of treatment were excluded from the analysis. Responses were defined based on the iwCLL response criteria for Chronic Lymphocytic Leukemia (CLL), Lugano criteria for Non-Hodgkin's Lymphoma and International Working Group on Waldenström's Macroglobulinemia (WM), as applicable. To calculate drug cost at lower doses of Ibrutinib, cumulative number of patient-months on different dose levels of ibrutinib was calculated by adding the number of months each patient had remained at the dose level at the time of data cut-off. Drug cost at LD was calculated by multiplying monthly wholesale acquisition price for different dose levels of ibrutinib by the cumulative number of patient-months at that dose level. Cost differential between actual drug cost and projected drug cost at full dose was calculated. Results: 98 patients were identified. 10 were excluded from the analysis based on drug not started (3), inadequate follow-up (3), other (4). Median length of follow up for all patients was 20 months (4-70 months) and on LD Ibrutinib 12.5 months (1-60 months). 10 and 12 patients received 140 mg and 280 mg of Ibrutinib respectively due to side effects. 61 patients had CLL, 9 WM, 15 mantle cell lymphoma (MCL), and 2 marginal zone lymphoma (MZL). Response rates were similar across diagnoses and dose levels (TABLE 1 and FIGURE 1). Progressive disease (PD) at low dose was seen in 2 CLL patients with complex cytogenetics, deletion 17p and extensive prior therapy. The one WM patient with PD had been extensively pretreated. Cumulative patient-months at the 140 mg and 280 mg dose levels of Ibrutinib was 177 and 123 months respectively. Drug cost for the 140 mg and 280 mg Ibrutinib cohorts were $712,276 and $989,943 respectively, for a total cost of $1,702,219. Potential drug cost for the 420 or 560 mg dose of Ibrutinib for the same duration of therapy was $3,621,828. Cumulative cost avoidance on LD Ibrutinib was $1,919, 608. Seiring dengan adanya transformasi teknologi saat ini, media sosial sebagai tempat berekspresi diri mendorong munculnya fast fashion yang mengacu pada praktik industri di mana pelaku industri fashion menawarkan berbagai macam produk baru dengan konsep pergantian mode yang cepat dan paling disukai pasar. Penelitian ini bertujuan untuk mengetahui bagaimana pengaruh store attribute terhadap customer experience, dan brand loyalty pada toko Hijup. Pengumpulan data dilakukan dengan kuesioner yang melibatkan 150 responden yang pernah mengunjungi dan berbelanja di toko Hijup dalam 12 bulan terakhir. Metode Path Analysis dilakukan untuk mengetahui bagaimana store attributes mempengaruhi customer experience dan brand loyalty dalam industri ritel fashion. Hasil penelitian menunjukkan bahwa store attribute memiliki pengaruh positif signifikan terhadap customer experience di dalam toko Hijup. Kemudian, ditemukan juga pengaruh positif signifikan antara customer experience terhadap brand loyalty. Adanya penelitian ini diharapkan dapat membantu toko Hijup sebagai pelaku retail khususnya di industri fashion muslim untuk meningkatkan store attribute mereka sehingga para peretail dapat menciptakan loyalitas bagi konsumen mereka. Data were collected using questionnaires involving 150 respondents who had visited and purchased at Hijup stores in the last 12 months. The path Analysis method was conducted to find out how store attributes affect customer experience and brand loyalty in the fashion retail industry. The results showed that store attributes had a significant positive effect on customer experience in Hijup stores. Then, it was also found that there was a significant positive effect between customer experience on brand loyalty. This research is expected to help Hijup stores as retail players, especially in the Muslim fashion industry to increase their store attributes so that retailers can create loyalty for their consumers. The multiple factors that define and relate service quality and customer or consumer satisfaction have served as the basis for the development of several multidimensional models that have allowed the study of this important issue. The level of research that will be used in this work is the explanatory levelbecause it is intended to evaluate the role of innovation in the design of the service quality improvement plan and its impact on customer satisfaction. As a case study, the M7D model was applied to two national banks. Among other results, national banks obtained 83.5% compliance with the M7D model, the dimensions with the best performance are customer satisfaction, organization, processes and social responsibility. While, medium-sized banks have a compliance level of 62.7% with the M7D model, the dimensions with the best performance are customer satisfaction, Social responsibility and leadership. It is concluded that banks must innovate at the level of preparation and training of human resources and adequacy. Anemia pada ibu hamil merupakan suatu keadaan kekurangan zat besi dalam darah saat masa kehamilan. Dampak buruk anemia pada ibu hamil terhadap janin adalah pertumbuhan janin terhambat, kelahiran prematur, bayi mengalami cacat bawaan, berat badan lahir rendah (BBLR), dan kematian janin dalam kandungan. Sedangkan dampak anemia pada ibu hamil adalah kesulitan bernapas, mudah lelah, jantung berdetak kencang, tekanan darah tinggi, insomnia, dan meningkatkan risiko perdarahan. Penelitian ini dilakukan menggunakan metode studi literatur review. Pencarian data dilakukan dengan menggunakan penelusuran internet dengan referensi yang sesuai dengan kriteria akan dijadikan sumber untuk diteliti lebih lanjut. Untuk menekan angka penurunan anemia pada ibu hamil dan mengurangi dampak buruk pada ibu dan bayi akibat anemia, dapat diatasi dengan patuh mengonsumsi tablet Fe. Kepatuhan ibu hamil dalam mengonsumsi tablet Fe berupa ketepatan dosis, ketepatan cara, dan ketepatan frekuensi. Mengonsumsi tablet zat besi tidak akan memberikan dampak penurunan anemia bila kepatuhan konsumsi tablet Fe masih rendah. Mengonsumsi suplementasi tablet Fe berdasarkan waktu dan cara yang tepat dapat meningkatkan jumlah hemoglobin.

Background:Stress is a risk factor of various diseases including autoimmune diseases. Autoimmune diseases are one of the leading causes of morbidity in young adults.(1)Examination stress is a main concern nowadays due to the study style, lack of preparation, doctor- student relationship and family pressure.(2)The previous studies declared that stress may causes neuroendocrinal changes leading to immune dysregulations and cytokines production.(3)Objectives:The aim of study is to scope the light on the importance of stress as a predisposing factor in autoimmune disease flares particularly Examination stress.Methods:A three-year (2017-2019) cross-sectional prospective study conducted on 1365 students who presented to the Alexandria University rheumatology clinic during examinations. Clinical assessments, routine investigations, activity markers, activity indices, stress and anxiety questionnaires and perceived stress scale (PSS) were applied to all patients during consecutive visits.Results:Through 5800 visits in three years during examination sessions, patients age ranged from (17 -25) years with 76% females and 24% males. They grouped into SLE (31.35%), Rheumatoid arthritis (RA) (37.28%), Fibromyalgia (13.91%), FMF (2.63%), Ankylosing Spondylitis (1.75%), Psoriatic arthritis (0.73%), systemic sclerosis (0.58%), and undifferentiated connective tissue (11.73%). According to SLE patients, 43.92% were newly diagnosed whilst 54.16% of previously diagnosed SLE presented with Flare in particular lupus nephritis (56.33%), arthritis (43.22%), hematological (49.76%) and serositis (21.36%). Interestingly, RA patients who newly diagnosed were 35.16% of total RA patients while 42.42% of previously diagnosed RA patients presented with moderate and high DAS-28 due to incompliance with treatment in (64.37%) of patients, (11.53% on biological, 88.47% on conventional treatment). In addition, (49.36%) of FMF presented in recent attacks. It was also found that Arthralgia, bone aches and sleep deprivation are the main complaints. Concerning, A High perceived stress scale (PSS) was associated with High DAS28 and SLEDI-2K scores. (rs= 0.723, 0.865) (P<0.001)Conclusion:Examination stress is one of triggering factor for autoimmune disease flares. ABSTRACT We report a rare case of sessile polypoidal xanthoma of vocal cord in a 38-year-old female, presented with complaint of hoarseness of voice for 3 months. Hopkins telescopic examination showed a sessile polyp arising from the anterior aspect of laryngeal vocal cord. With clinical diagnosis of vocal cord polyp, microlaryngeal surgery was done and tissue was sent for histopathological and culture examination. Background:Spondyloarthropathies (SpA) including ankylosing spondylitis are characterized by inflammatory arthritis involving the spine and peripheral joints. Bone SPECT/CT is in the spotlight as it can reflect the current level of inflammation.Objectives:We aimed to investigate the diagnostic performance of bone SPECT/CT for axial SpA (axSpA) at the level of sacroiliac joints.Methods:Patients with low back pain who had undergone SPECT/CT of the SI joints were selected for inclusion in this study through a retrospective review of medical records from August 2016 and July 2018. We used semi-quantitative scoring methods for SPECT/CT. For visual scoring, a score of 0 was assigned when tracer uptake in the sacroiliac joint was less than that in the sacrum; a score of 1, when equal to that in the sacrum; and a score of 2, when greater than that in the sacrum. A score of 2 was considered positive for the diagnosis of sacroiliitis on SPECT/CT (Figure 1). The diagnosis of axSpA was retained when patients fulfilled the Assessment of SpA International Society criteria.Results:A total of 164 patients were enrolled (34 patients with axSpA). The remaining 130 patients had non-axSpA rheumatic inflammatory disease (n=24), vertebral disk herniation (n=13), avascular necrosis (n=11), and others such as bursitis, and fracture (n=85). The mean age of aSpA (37.8±15.6 years) was lower than controls (49.8±16.4 years) (p<0.001), and axSpA (64.5 %) had more male than others (42.1 %) (p=0.024). The sensitivity, specificity, positive and negative predictive values of bone SPECT/CT for axSpA were 83.9%, 63.2%, 34.7%, and 94.4%, respectively. The bone SPECT/CT maximal score and BASDAI score has positive correlation (r=0.481, p=0.007). The bone SPECT/CT compared with MRI is marginal correlation (k=0.369, p<0.001).Conclusion:In patients with low back pain, the bone SPECT/CT has a high negative predictive value that can exclude AS. ABSTRACT Episiotomy scar endometriosis is characterized by presence of endometrial tissue (glands and stroma) and is a rare condition. It may also extend and involve the anal sphincter. The ideal treatment is wide excision to prevent recurrence but it may cause fecal incontinence, if the anal sphincter is involved. We describe here a case with much deeper extension into ischiorectal fossa. She was treated with wide local excision and primary sphincteroplasty. These nanorobots allow precision interactions with nanoscale objects or can manipulate with nanoscale resolution. Treatment opportunities in dentistry may include local anesthesia, dentition renaturalization, and permanent hypersensitivity cure, complete orthodontic realignments during single office visit, and continuous oral health maintenance using mechanical dentifrobots. Dental nanorobots could be constructed to destroy cariescausing bacteria or to repair tooth blemishes where decay has set in, by using a computer to direct these tiny workers in their tasks. The achievement of family well - being is an aspiration of all families including families of migrant workers, to achieve the well - being of one of the ways that the families of migrant workers with the departure of the wife work as domestic servants in various countries. The purpose of this study was to analyze social capital, economic coping, sress symptom’s husband and family subjektive well - being of women migrant workers. This research use cross sectional studies. The location was chosen purposively in Tanggeung Village, Pagermaneuh Village, Margaluyu Village, Karangtengah Village, Tanggeung District and Pasirdalam Village Kadupandak District, Cianjur, West Java, Indonesia. Seventy five families were selected purposively among the families of women migrant workers. The finding indicates that social capital is in the moderate category, the coping economy is in the moderate category, the sress symptom’s husband is in the low category and the family well-being is in the low category. Finding in this study family subjective well-being is influenced by income per capita, sress symptom’s husband and economic coping. Keywords: economic coping, family subjective well-being, social capital, stress symptom Abstrak Kesejahteraan keluarga merupakan sesuatu yang ingin dicapai seluruh keluarga, termasuk keluarga Tenaga Kerja Wanita (TKW), untuk mencapai kesejahteraan tersebut salah satu cara yang dilakukan oleh keluarga TKW adalah mengirim istri sebagai pembantu rumah tangga di berbagai negara. Tujuan dari penelitian ini adalah untuk menganalisis modal sosial, coping ekonomi, gejala stres suami, dan kesejahteraan subjektif keluarga TKW. Penelitian ini menggunakan cross sectional studies. Lokasi dipilih dengan metode purposive di Provinsi Jawa Barat, Kabupaten Cianjur, Kecamatan Tanggeung, Desa Pagermaneuh, Desa Marguluyu, Desa Karangtengah, Desa Tanggeung, Kecamatan Kadupandak, Desa Pasirdalam. Jumlah sampel sebanyak 75 keluarga TKW dengan metode purposive sampling. Penelitian ini menemukan modal sosial berada pada kategori sedang, coping ekonomi berada pada kategori sedang, gelaja stres suami berada pada kategori rendah, dan kesejahteraan subjektif berada pada kategori rendah. Penelitian ini juga menemukan kesejahteraan keluarga berpengaruh terhadap pendapatan perkapita, gelaja stres suami, dan coping ekonomi. ABSTRACT Introduction A comparative study of a failed and a successful merger in health care was conducted in an attempt to explore the reasons for success or failure of mergers. Materials and methods This was a retrospective exploratory study of two mergers with the primary hospital being common. A detailed study of the existing policies and procedures, hospital records, financial data, and hospital statistics was conducted. Trends were analyzed and compared with the observed value. Results Reasons for the successful mergers can be attributed to managerial commitment and coordination, communication, as well as proximity of consultants of the two merging entities. Adequate strategic planning, a professional approach, and incremental implementation of necessary changes are also essential. No redressal of staff and stakeholder apprehension and poor communication are reasons for failure of mergers. Conclusion A merger in the Indian hospital scenario is a fairly recent phenomenon. Factors, and their interplay that result in successful or failed mergers in Indian context, are an unexplored field of research. Mergers are capital and resource-intensive, and the consequences of a failed alliance are huge. Thus, a careful, holistic feasibility study is essential before embarking on a merger. One of the problems faced by working mothers is caring for children, especially children who are still at an early age. This is because their workplace does not allow them to bring small children or does not provide daycare. This study aims to find out the types of child care that working mothers in Indonesia choose and their considerations in choosing that type. This research method uses a mixed-method design by combining two types of quantitative and qualitative data. Data collection was carried out through surveys with questionnaire instruments and interviews with working mothers. The number of respondents surveyed was 500 working mothers and 100 of them were interviewed. Interviews were conducted in person or via telephone and video calls using the Zoom meeting and WhatsApp applications. The study was conducted in 2020 and 2021. Results The total DMFT score was 180 and the average DMFT was 1.37 for the total prison population. A significant association was found between adverse habits and different age groups (χ2 = 16.77, p = 0.033) and between duration of adverse habits and different age groups of prisoners (χ2 = 16.58, p -value = 0.034). Significant correlation was found between adverse habit and duration of adverse habit (p < 0.05) in subjects having dental caries. Conclusion Though the prevalence of dental caries was so high, treatment for the same was not available due to lack of a dental unit in the jail. ABSTRACT Patients usually under-report symptoms of incontinence. Therefore, improved physician-patient communication is vital. Most urinary incontinence can be evaluated and treated after careful history and simple clinical assessment. Initial treatment, for both urge urinary incontinence and stress urinary incontinence, is lifestyle modification and pelvic floor muscle exercises. Urgency responds to bladder training and drug therapy with anticholinergic medication. Pharmacotherapy has a limited place in stress incontinence.
