Abstract:
Objective: The Jirapa District in Ghana falls within the African meningitis belt where over 500 million people are at risk of epidemic meningitis. The district suffered an outbreak of Neisseria meningitides, W (NMW) in 2012 and a mixed outbreak of Streptococcus pneumonia and NMW in early 2016. We investigated the outbreak to identify the source, causative agents, and magnitude and assess health facility preparedness and propose control measures.Design and Setting: We conducted a descriptive study in all sub-districts of Jirapa, between 28th February to10th April 2016. We reviewed records at health facilities, assessed health facility preparedness, searched for cases, traced contacts of case to administer chemoprophylaxis and collect CSF for laboratory analysis. Data were entered in Microsoft excel cleaned, and exported to stata-13 for analysis by person place and time.Results: A total 233 meningitis cases were reported with mean age of 22.4years and standard deviation 21.6. Males were (57%), females (43%) and 60.8% were less than 19 years. Attack rate of meningitis was 214/100,000 with case fatality rate (CFR) of 12.4% (29/233). Causative agents were NMW (69.5%) and streptococcus pneumonia (27.1%), mainly serotype STN1 and H. influenza (3.4%). The index case had travel history to dollar power, close to Tain District which is the epicentre for the 2016 meningitis outbreak in Ghana.Conclusion: The Jirapa district experienced a mixed outbreak of streptococcal and meningococcal meningitis in early 2016, facilitated by migration. Abstract:
Background. Tramadol has gained popularity among the drugs of the most active population especially the respondents in Ghana abuse especially farmers who nicknamed as “farm and buy cow.” It has recently become a public health concern, and stakeholders are worried about tramadol abuse and its implications on health in the Upper West Region. The study sought to measure the prevalence of tramadol/related substance abuse and the associated factors. Methods. A community-based analytic cross-sectional study involving 420 respondents was conducted. The participants were selected using a multistage sampling technique. Semistructured questionnaire was used to generate the data. Results. About 77.6% of the respondents abuse tramadol while 83.9% of the participants take at least one other related substance or drug. Participants with history of any substance abuse were 5 times more likely to abuse tramadol [ AOR = 5.15 ; 95% CI (1.501-17.656); p = 0.009 ], compared to respondents with no history of any substance abuse. Respondents who take tramadol to enhance sex were 4 times more likely to abuse tramadol [ AOR = 3.776 ; 95% CI (1.352-10.545); p = 0.011 ]. Formal sector employment was protective against tramadol abuse [ AOR = 0.100 ; 95% CI (0.017-0.595); p = 0.011 ] compared to self-employment and the unemployed. In addition, use of nonopioid prescription drugs for posttraumatic/pain management reduced the risk of tramadol abuse [ AOR = 0.237 ; 95% CI (0088-0.640); p = 0.004 ] compared to the posttraumatic/pain management dependence on prescription of only opioid like tramadol. Conclusion. An infantile municipality like Jirapa is challenged with high level of tramadol and related substance which has serious repercussion on the health system in the Jirapa district. It is important that measures are taken by the stakeholders to stop tramadol and related substance and mitigate the impact of drug abuse in the district.

Though Ghana has made impressive strides in terms of economic growth, poverty reduction and democratic governance, there remains a substantial percentage of the population that still lives in poverty and are vulnerable to a range of economic, social, lifecycle and environmental shocks and risks. Therefore, Ghana rolled out a number of social protection interventions under the National Social Protection Strategy (NSPS) to mitigate the impact of extreme poor and vulnerabilities in society, especially among vulnerable children.The main objective of the study was to examine social protection and children vulnerability in Ghana by employing both qualitative and quantitative methods in achieving the objectives of the study in the Jirapa and Wa Municipalities of the Upper West Region of Ghana. Secondary literature and primary data were combined.The methods for data collection were questionnaire and interviews to generate information from government departments and institutions providing social protectionist services.Results suggest that there is increasing levels of vulnerability and orphanhood in children in the region. However, SP has some improvement in bene?ciaries household food consumption, income levels, saving levels, access to healthcare, and school attendance. However, there are institutional challenges that affect the implementation of the SP programme.The study concludes that SP programme contributed to poverty reduction as it enhanced bene?ciaries’ living conditions. It is therefore recommended that the Department of Social Welfare (DSW) be strengthened to properly target the real vulnerable children and households in their registration.

Abstract:
The study explored Head teachers’ instructional supervisory role among public Junior High Schools in the Jirapa Municipality of the Upper West Region of Ghana. It adopted psychological and system theory of supervision propounded by Planturrroot (2006) and Bertalanffy (2015) respectively. The mixed method sequential approach was followed in the conduct of the study where both quantitative and qualitative data were collected and analysed. A sample size of 200 respondents made up of 40 head teachers, 80 teachers and 80 pupils were used for the quantitative data, whilst 10 head teachers were selected for the qualitative data. The Statistical Product for Service Solutions (SPSS) version 23 was used to analyse the quantitative data while thematic approach was used for the qualitative data. The findings of the study revealed in-frequent orientation for newly engaged teachers. It further showed lower ratings in the provision of in-service training, time tabling design and setting of performance target for pupils and teachers. It was recommended that the Ministry of Education and Ghana Education Service (GES) should regularly organize workshops, seminars and in-service trainings on supervisory roles of head teachers and assistant head teachers to equip them with the needed knowledge and skills in the municipality. Lastly, Heads of schools and other supervisors should re-design schools` time tables to suit the use of instructional time in the lesson of teachers to enhance their monitoring.

Abstract:
Climate change is posing threat to agriculture in sub-Saharan Africa including Ghana. Climate-Smart Agriculture (CSA) is proposed to solve climate change impacts on agriculture. Smallholder farmers are adopting various strategies to be resilient to climate change effects. Empirical research is required to evaluate CSA utilisation in Ghana. Lawra, Jirapa and Nandom districts in the climate-risk areas of Guinea Savannah agro-ecological zone were chosen and Participatory Rapid Appraisal (PRA) tools were used. Farm budget analysis and market price methods were employed; key financial decision-making tools were net returns, profit margins and benefit-cost ratio. Soft systems content analysis, frequencies, means, ranking and data aggregation were employed to generate results. CSA use in the study districts was smallholder driven and male dominated. CSA was mainly used for staples including cereals and legumes and small ruminants under livestock. Crop-livestock integration and crop rotation were the common CSA practices with the highest costs (GH¢6,370.00) and highest revenues (GH¢9,460.00) respectively. Utilisation of CSA in the districts is beneficial and investments are profitable and financially viable. All actors and stakeholders must join forces to promote CSA in the districts. Rigorous promotional campaigns, capacity building and funding at all levels are crucial for CSA adoption in Ghana.

Abstract:
This thesis sheds light on differences in health insurance enrolment determinants and uptake barriers between urban and rural areas in the Jirapa district of Ghana. The National Health Insurance Scheme in Ghana has made significant progress in terms of enrolment, which has had a commensurate increase in utilization of health care services. However, there are challenges that pose a threat to the scheme’s transition to universal coverage; enrolment in the scheme has not progressed according to plan, and there are many barriers known to impede uptake of health care. Interestingly, these barriers vary in relation to locality, and rural residents appear to carry a disproportionate portion of the burden. A mixed method approach was employed to collect and analyse the data. On the basis of the primary qualitative and quantitative results, the thesis argues that the costs of enrolling and accessing health care is disproportionately higher for rural residents than it is their urban counterparts. It also highlights that the distribution of service benefits both in terms of the NHIS and health care in the Jirapa district favours urban residents. Lastly, the thesis found that whereas rural residents prefer health care provision to be social in nature, urban residents were more interested in the technical quality aspects of care. These findings suggest that rural residents are not benefitting from, or may not be accessing health services to the extent as their urban counterparts. Affordability, long distance to health facilities, availability and acceptability barriers were found to influence the resultant pro-urban distribution of the overall health care benefit.

Abstract:
The thesis empirically examines how microfinance products are designed and implemented, and the implications for clients’ households and sources of livelihood. The study argues that the design of products and implementation that reflect the livelihood needs and poverty context of clients is one of the effective ways to reduce poverty. It investigates the microfinance operations of three financial institutions: Sinapi Aba Trust (SAT), St Joseph’s Cooperative Credit Union (CCU) and Sonzele Rural Bank (SRB) in Jirapa, a municipality in Northern Ghana. The study deployed a mixed-methods approach to collect data from six rural and urban communities. Data was sought from secondary sources, 20 interviews, 10 focus group discussions and 120 questionnaires. The research adopted the Sustainable Livelihoods and the Making Markets Work for the Poor approaches as a guide in the framework of analysis. The study, using qualitative and quantitative analytical tools found that product designs of SAT and SRB did not reflect the needs and poverty context of the majority of their clients. Clients of SAT and SRB were found to be less involved in the product design processes, suggesting a top-down institutional approach that seldom incorporated the needs of the poor. The method of group formation has a substantial implication on members’ poverty outcomes. Groups involving only females had a significant and positive relationship with members’ household and business outcomes, while members of male-only groups had a negative relationship with their household outcomes. The thesis concludes that accessible interest on loans and incentives to encourage savings would make microfinance markets work more sustainably for the rural poor. The findings challenge a reconsideration of the design of microfinance products to integrate financial technology as an efficient approach to deliver financial services, especially in rural areas.

This dissertation thesis examines the environmental licensing of a hydroelectric power plant in the Amazon Basin: 'Usina Jirau.' This starting point for this case study was an allegation that the Brazilian environmental licensing process for hydroelectric power plants slows down the Brazilian development. As an initial matter, I assumed that such an argument is partially mistaken. On the one hand there may be problems in the administrative procedures and in the institutional design and functioning of the environmental agency, IBAMA; but on the other hand, the entrepreneurs did not properly consider legal environmental rules during the planning process. To test this hypothesis, I attended specific meetings on the subject, conducted interviews with key actors in the process, and researched the issues raised in different courts by a variety of litigators. I also analyzed the environmental licensing procedures extensively and some other to understand the economic argument, its variations and the proponents of such ideas. Additionally, I explain how and which hypotheses were tested in my research. In part II, I discuss the environmental licensing economic content, put the environmental documents from both state and non-state players. This paper is divided into four separate parts, and a conclusion. In part I, I explain the research problem, the context surrounding the construction of Usina Jirau as well, discuss some key players, and demonstrate how licensing rules together, and assess how the two aforementioned perspectives relate to each other. In part III, I describe the IBAMA bureaucracy functions as a means of assessing if, as a public administration, it was inefficient in licensing Usina Jirau. Thus, I describe the IBAMA authority to license such entrepreneurship, its structure and ability to provide this service, and list other administrative branches and entrepreneurs’ obligations. Lastly, I identify what were the sensitive points in the environmental licensing dynamics, which may have offered obstacles to the process. In park IV, I discuss the supposed failures in the control system of administrative duties. In conclusion, I summarize the research key points, offer some suggestions to improve the environmental licensing process and establish the distinction between inefficiency and enforcement of environmental conservational legal rules. Usina Jirau is an exemplary case of how a licensing process does not slow down the Brazilian development, and it is also a calling for Brazil to modernize and rationalize its environmental regulatory framework.
Esse é o caso do licenciamento ambiental do aproveitamento hidrelétrico Jirau, parte do Complexo Hidrelétrico do Rio Madeira. O ponto de partida da pesquisa foi a alegação de que o licenciamento ambiental de usinas hidrelétricas atrapalha o desenvolvimento brasileiro. Para enfrentar o argumento mencionado, levantei a suspeita de que o mesmo está parcialmente equivocado. Se por um lado, há problemas no procedimento administrativo do licenciamento ambiental e na estrutura e funcionamento do órgão licenciador; por outro lado, os interessados no sucesso da obra fizeram o planejamento do empreendimento sem levar em consideração exigências legais relativas ao ambiente. Para confirmar ou negar minha suspeita, fiz um levantamento dos atos do processo administrativo e das ações judiciais movidas por diferentes partes referentes ao licenciamento da UHE Jirau, conduzi entrevistas semiestruturadas com algumas pessoas envolvidas nesse processo, e consultei documentos dos órgãos estatais relacionadas ao caso, bem como documentos dos consórcios de empresas que concorreram ao leilão de geração de energia. O caminho metodológico adotado me fez dividir o trabalho em quatro capítulos e uma conclusão. No primeiro capítulo, apresento o problema da pesquisa, explico qual é o contexto da construção da UHE Jirau, e menciono os principais atores envolvidos. Também explico o funcionamento do argumento enfrentado. Por fim, explico como e quais são as hipóteses que serão testadas: hipótese da falha no sistema de controle e hipótese da ineficiência do procedimento. No segundo capítulo, trato do sentido jurídico-econômico do licenciamento ambiental, mapeio as regras que disciplinam esse instrumento de política ambiental e mostro o funcionamento da teoria e das regras na prática do licenciamento da usina Jirau. No terceiro capítulo, descrevo o funcionamento da burocracia do órgão licenciador para avaliar se o licenciamento ambiental da usina hidrelétrica Jirau foi ineficiente. Assim, descrevo as competências, a estrutura e a capacidade de atendimento do órgão licenciador, bem como as obrigações do empreendedor e demais órgãos. Por fim, identifico quais foram os pontos sensíveis da dinâmica do licenciamento ambiental que podem ter eventualmente impedido a administração de cumprir prazos legais. No quarto capítulo, busco identificar eventuais falhas no sistema de controle do processo do licenciamento ambiental da UHE Jirau a partir dos embates administrativos e judiciais que ocorreram a partir da alteração do projeto da usina após a realização do leilão, assim como das inovações processuais ocorridas no caso. O capítulo derradeiro retoma os pontos chave da pesquisa; aponta caminhos para a melhoria do licenciamento ambiental de usinas hidrelétricas no âmbito do órgão federal; e indica algumas exigências próprias da natureza das leis de conservação ambiental, as quais não devem ser confundidas com fatores de produção de ineficiência do procedimento. O caso de Jirau não pode ser acusado de criar embaraços temporais à expansão da oferta da energia necessária para o desenvolvimento brasileiro em razão de burocracia excessiva e fragilidades institucionais do órgão. O temor de criminalização dos técnicos aumentou o grau de exigências e o sistema de controle administrativo e judicial expôs à sociedade questões do licenciamento sem fazer com que o mesmo demorasse ainda mais. The Jovian Auroral Infrared Mapper (JIRAM) instrument aboard NASA’s Juno spacecraft was designed to monitor the atmosphere of Jupiter and its aurorae with an unprecedented spatial resolution. In this thesis, JIRAM data measured over the polar regions, have been used to derive quantitative information on the species CH4 and H3+ and on the variability of their spatial distribution. JIRAM spectra have been analysed in the spectral region 3-4 μm, that is particularly favourable for the study of the aurorae. Starting from the data previously analysed for the orbit JM0003, the dataset has been enlarged to include observations outside the auroral ovals with a lower signal and extending the analysis to successive orbits. The first Juno orbits, from JM0003 to JM0091, have been examined to find the most promising for the study of the aurorae. Along with the JM0003, the orbits JM0071 and JM0081 have been analysed for the south aurora. The selected spectra show H3+ and CH4 emissions in the auroral regions and have been analysed using an inversion technique based on a Bayesian approach. Preliminary tests have allowed to optimize for the new dataset the a-priori information vector and the corresponding error and limit the degrees of freedom of the information to just the abundances of the two species and H3+ temperature. The results have confirmed the presence of methane near both poles, within the auroral oval, and comparable abundances of H3+ in the two auroral regions, with values ranging between 2-2.8 10^12 cm^-2 e some peaks larger than 2.8∙10^12 cm^-2. The H3+ temperatures appear lower in the south aurora, where the values do not exceed 825 K, while in the north aurora the temperatures span between 800 K and 950 K. The comparison of these results with the images obtained from the JIRAM’s observations in the L band has also allowed to study the morphology of the Jovian aurorae and to highlight the displacement of a few degrees westward of the south aurora over the time.
