Full textDysfunctional thoughts figure among the factors that contribute to the development and maintenance of gambling disorder. Cognitive restructuring, a treatment that targets dysfunctional thoughts, aims to identify and correct these thoughts. However, little information is available regarding the application of cognitive restructuring with gamblers, which makes it difficult for some counselors to apply it. Comprised of two articles, this thesis aims to document the evaluation and treatment of dysfunctional thoughts in individuals with gambling disorder. The first article is a systematic review of 39 treatment studies that applied cognitive restructuring. It documents: (1) cognitive restructuring intervention modalities; (2) objectives and outcome measures; (3) techniques/instruments/tools used; (4) the ways that cognitive restructuring is adapted for gamblers who play games that include a skill component and (5) terminology employed in studies to refer to gamblers’ thoughts. Results show that cognitive restructuring is mainly carried out face-to-face in an individual setting within five to 20 sessions. Few studies report a post-treatment measure of dysfunctional thoughts. Imaginal exposure is the most cited technique to facilitate cognitive restructuring. None of the studies that included skill games gamblers (e.g., poker) reported an adaptation of cognitive restructuring despite the fact that these gamblers endorse dysfunctional thoughts that are different from those of pure chance games gamblers. Results also show that the terms employed to refer to gamblers dysfunctional thoughts are used interchangeably while this may not be appropriate. To designate a thought that is somewhat false (e.g., “gambling helps me forget about my problems”) as erroneous would not be accurate. It also seems that thoughts documented in the field of addiction (e.g. relief-oriented thoughts) are not considered when applying cognitive restructuring with gamblers. Clinical and research recommendations are formulated regarding types of dysfunctional thoughts endorsed by gamblers during a gambling session and exposure as a technique to facilitate cognitive restructuring. The second article identifies dysfunctional thoughts verbalized by 29 individuals with gambling disorder during a session of gambling exposure (16 in virtual reality and 13 in imagination). It also compares imaginal exposure with virtual reality regarding quantity of gambling-specific thoughts (those that are erroneous based on the notion of chance) and the number of addiction-related thoughts (non-related to chance). It also compares the two exposure conditions on diversity of gambling-specific thoughts verbalized by gamblers. Results show that gamblers exposed in virtual reality verbalized a greater number and diversity of gambling-specific thoughts compared to gamblers in the imaginal exposure condition. Though apt to induce cravings and a feeling of being in a real gambling environment, virtual reality does not allow greater access to addiction-related thoughts compared to imaginal exposure. Addiction-related thoughts may be more automatic in gamblers (related to emotional expectations and management of gambling habits) and thus may not require an emotional context as induced by virtual reality to access them. However, exposure to gambling cues that induce cravings could bonify cognitive restructuring since it is known that such a context can increase therapeutic gains. In conclusion, this thesis focuses on exposure, a technique that is widely used to facilitate cognitive restructuring with gamblers, yet has seldom been described and evaluated for its efficacy. It also raises questions for the study of dysfunctional thoughts that are not related to the notion of chance in the gambling domain. Finally, it addresses the fact that addiction-related thoughts do not seem to be considered in cognitive restructuring with gamblers.

Full textThe aim of this research is to study the sensation seeking personality trait and alexithymia (and his link to depression) in gamblers who play different types of game (horse racing at the racetrack, fruit machine and casino games : roulette and cards). Our working hypothesis postulates that scores on sensation seeking (SSS-form V) and alexithymia (TAS-20) scales may vary depending on types of gamblers (pathological, regular and occasional) and types of game. Pathological gamblers who go to the racetrack (n=42), playing active games, have the highest scores on sensation seeking and are the most subject to alexithymia. Pathological gamblers of fruit machine (n=27), playing passive games, are low sensation seekers, they are alexithymic and have highest score of depression. Pathological gamblers of casino games (n=15), a mid class, are not performing well on these scales. These findings are consistent with the idea of identifying distinct subgroups of gamblers. Some people showing typical features could be attracted by some specific sort of games. Therefore treatment of pathological gamblers should take into account this typology to be more efficient

Full textIntroduction: Gambling Disorder is described by engaging in compulsive and excessive gambling behaviors that can lead to multiple negative consequences for the affected individual, his or her entourage and society. Gambling disorder is now included in the category of "Substance-Related Disorders and Addictive Disorders" in the Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5), based on shared characteristics with substance use disorders. For example, the use of advanced cerebral investigation methods such as functional magnetic resonance imaging (fMRI) has made it possible to demonstrate the presence of abnormal patterns of resting state functional connectivity, particularly in the fronto-striatal network in individuals with an addictive disorder. Moreover, magnetic resonance spectroscopy (MRS) studies have found abnormally low frontal lobe levels of neuronal metabolites including GABA, glutamate+glutamine (Glx) and N-acetyl-aspartate (NAA) in individuals with an addictive disorder. Finally, individuals with an addictive disorder are described by impaired decision-making as well as high levels of craving towards stimuli related to the desired substance or in this case, gambling. Interestingly, when applied over the prefrontal cortex, transcranial direct current stimulation (tDCS) stimulation has shown to modulate resting state functional connectivity and neuronal metabolites levels in several brain structures as well as improve decision-making in healthy volunteers and decrease levels of craving in substance use disorders but has never been used in gambling disorder. Objectives: Thus, the present doctoral thesis has two main objectives. 1) Evaluate the neurobiological effects of tDCS in subjects with gambling disorder: resting functional connectivity (study 1) and neuronal metabolites levels (study 2). 2) Evaluate the behavioral effects of tDCS in subjects with gambling disorder: craving (study 1). Methods: For both studies, we used a crossover, randomized and blinded at 2-levels study was conducted with subjects with gambling disorder. Each subject participated in two visits. During each of the two visits, a combined MRI/tDCS session took place during which measures of resting functional connectivity and MRS measurements were acquired. tDCS was applied bilaterally over the prefrontal cortex (anode right/cathode left) during 30 min at 1mA. In addition, craving was measured before and after each MRI/tDCS session. Study 1 revealed that active compared with sham tDCS induces a decrease in resting state functional connectivity between distinct frontal structures and over fronto-parietal networks. This study also showed that a single session of active compared with sham tDCS does not modulate craving levels. Study 2 revealed that active compared with sham tDCS increases prefrontal GABA levels but does not modulate prefrontal Glutamate and NAA levels nor striatal Glutamate, GABA or NAA levels. The results of this study also suggest that subjects with gambling disorder who display greater level of risk taking, impulsivity and craving are more likely to respond to tDCS. Conclusion: The two studies included in the doctoral thesis here presented have contributed to a deeper understanding of the neurobiological effects of tDCS in gambling disorder. Indeed, various neuronal substrates known to be associated with the symptoms of pathological gambling were modulated by tDCS, implying that this tool could have a therapeutic interest in this clinical population. Nevertheless, the beneficial effects of applying tDCS in order to reduce risk-taking and craving in this population remain to be demonstrated. Therefore, further studies have to be carried out to verify this hypothesis, as well as clinical trials employing protocols known to have beneficial results in several psychiatric diseases such as repeated tDCS sessions spread over a period of time. This Ph.D. used intracranial EEG (iEEG) and combined Functional Magnetic Resonance Imaging (fMRI) and endocrinology to perform four studies investigating reward processing in healthy subjects, patients with epilepsy implanted with depth electrodes and individuals with gambling disorder.
