Full textDysfunctional thoughts figure among the factors that contribute to the development and maintenance of gambling disorder. Cognitive restructuring, a treatment that targets dysfunctional thoughts, aims to identify and correct these thoughts. However, little information is available regarding the application of cognitive restructuring with gamblers, which makes it difficult for some counselors to apply it. Comprised of two articles, this thesis aims to document the evaluation and treatment of dysfunctional thoughts in individuals with gambling disorder. The first article is a systematic review of 39 treatment studies that applied cognitive restructuring. It documents: (1) cognitive restructuring intervention modalities; (2) objectives and outcome measures; (3) techniques/instruments/tools used; (4) the ways that cognitive restructuring is adapted for gamblers who play games that include a skill component and (5) terminology employed in studies to refer to gamblers’ thoughts. Results show that cognitive restructuring is mainly carried out face-to-face in an individual setting within five to 20 sessions. Few studies report a post-treatment measure of dysfunctional thoughts. Imaginal exposure is the most cited technique to facilitate cognitive restructuring. None of the studies that included skill games gamblers (e.g., poker) reported an adaptation of cognitive restructuring despite the fact that these gamblers endorse dysfunctional thoughts that are different from those of pure chance games gamblers. Results also show that the terms employed to refer to gamblers dysfunctional thoughts are used interchangeably while this may not be appropriate. To designate a thought that is somewhat false (e.g., “gambling helps me forget about my problems”) as erroneous would not be accurate. It also seems that thoughts documented in the field of addiction (e.g. relief-oriented thoughts) are not considered when applying cognitive restructuring with gamblers. Clinical and research recommendations are formulated regarding types of dysfunctional thoughts endorsed by gamblers during a gambling session and exposure as a technique to facilitate cognitive restructuring. The second article identifies dysfunctional thoughts verbalized by 29 individuals with gambling disorder during a session of gambling exposure (16 in virtual reality and 13 in imagination). It also compares imaginal exposure with virtual reality regarding quantity of gambling-specific thoughts (those that are erroneous based on the notion of chance) and the number of addiction-related thoughts (non-related to chance). It also compares the two exposure conditions on diversity of gambling-specific thoughts verbalized by gamblers. Results show that gamblers exposed in virtual reality verbalized a greater number and diversity of gambling-specific thoughts compared to gamblers in the imaginal exposure condition. Though apt to induce cravings and a feeling of being in a real gambling environment, virtual reality does not allow greater access to addiction-related thoughts compared to imaginal exposure. Addiction-related thoughts may be more automatic in gamblers (related to emotional expectations and management of gambling habits) and thus may not require an emotional context as induced by virtual reality to access them. However, exposure to gambling cues that induce cravings could bonify cognitive restructuring since it is known that such a context can increase therapeutic gains. In conclusion, this thesis focuses on exposure, a technique that is widely used to facilitate cognitive restructuring with gamblers, yet has seldom been described and evaluated for its efficacy. It also raises questions for the study of dysfunctional thoughts that are not related to the notion of chance in the gambling domain. Finally, it addresses the fact that addiction-related thoughts do not seem to be considered in cognitive restructuring with gamblers.

Full textThe aim of this research is to study the sensation seeking personality trait and alexithymia (and his link to depression) in gamblers who play different types of game (horse racing at the racetrack, fruit machine and casino games : roulette and cards). Our working hypothesis postulates that scores on sensation seeking (SSS-form V) and alexithymia (TAS-20) scales may vary depending on types of gamblers (pathological, regular and occasional) and types of game. Pathological gamblers who go to the racetrack (n=42), playing active games, have the highest scores on sensation seeking and are the most subject to alexithymia. Pathological gamblers of fruit machine (n=27), playing passive games, are low sensation seekers, they are alexithymic and have highest score of depression. Pathological gamblers of casino games (n=15), a mid class, are not performing well on these scales. These findings are consistent with the idea of identifying distinct subgroups of gamblers. Some people showing typical features could be attracted by some specific sort of games. Therefore treatment of pathological gamblers should take into account this typology to be more efficient

Full textIntroduction: Gambling Disorder is described by engaging in compulsive and excessive gambling behaviors that can lead to multiple negative consequences for the affected individual, his or her entourage and society. Gambling disorder is now included in the category of "Substance-Related Disorders and Addictive Disorders" in the Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5), based on shared characteristics with substance use disorders. For example, the use of advanced cerebral investigation methods such as functional magnetic resonance imaging (fMRI) has made it possible to demonstrate the presence of abnormal patterns of resting state functional connectivity, particularly in the fronto-striatal network in individuals with an addictive disorder. Moreover, magnetic resonance spectroscopy (MRS) studies have found abnormally low frontal lobe levels of neuronal metabolites including GABA, glutamate+glutamine (Glx) and N-acetyl-aspartate (NAA) in individuals with an addictive disorder. Finally, individuals with an addictive disorder are described by impaired decision-making as well as high levels of craving towards stimuli related to the desired substance or in this case, gambling. Interestingly, when applied over the prefrontal cortex, transcranial direct current stimulation (tDCS) stimulation has shown to modulate resting state functional connectivity and neuronal metabolites levels in several brain structures as well as improve decision-making in healthy volunteers and decrease levels of craving in substance use disorders but has never been used in gambling disorder. Objectives: Thus, the present doctoral thesis has two main objectives. 1) Evaluate the neurobiological effects of tDCS in subjects with gambling disorder: resting functional connectivity (study 1) and neuronal metabolites levels (study 2). 2) Evaluate the behavioral effects of tDCS in subjects with gambling disorder: craving (study 1). Methods: For both studies, we used a crossover, randomized and blinded at 2-levels study was conducted with subjects with gambling disorder. Each subject participated in two visits. During each of the two visits, a combined MRI/tDCS session took place during which measures of resting functional connectivity and MRS measurements were acquired. tDCS was applied bilaterally over the prefrontal cortex (anode right/cathode left) during 30 min at 1mA. In addition, craving was measured before and after each MRI/tDCS session. Study 1 revealed that active compared with sham tDCS induces a decrease in resting state functional connectivity between distinct frontal structures and over fronto-parietal networks. This study also showed that a single session of active compared with sham tDCS does not modulate craving levels. Study 2 revealed that active compared with sham tDCS increases prefrontal GABA levels but does not modulate prefrontal Glutamate and NAA levels nor striatal Glutamate, GABA or NAA levels. The results of this study also suggest that subjects with gambling disorder who display greater level of risk taking, impulsivity and craving are more likely to respond to tDCS. Conclusion: The two studies included in the doctoral thesis here presented have contributed to a deeper understanding of the neurobiological effects of tDCS in gambling disorder. Indeed, various neuronal substrates known to be associated with the symptoms of pathological gambling were modulated by tDCS, implying that this tool could have a therapeutic interest in this clinical population. Nevertheless, the beneficial effects of applying tDCS in order to reduce risk-taking and craving in this population remain to be demonstrated. Therefore, further studies have to be carried out to verify this hypothesis, as well as clinical trials employing protocols known to have beneficial results in several psychiatric diseases such as repeated tDCS sessions spread over a period of time. Full textPathological gambling is an addictive disorder characterized by loss of control over gambling behavior and its continuation despite negative consequences. As in other addictive disorders, gambling addiction is the result of the encounter between an object more or less addictive, a personality more or less predisposed, and a particular context that promotes more or less the gambling practice. The subject of this thesis has been developed to address a paradox between on the one hand a scientific gap in the literature focused on the subject of addiction, and on the other hand a strong clinical impression suggesting that gamblers playing different games have very different psychopathological profiles and gambling practices. The overall objective of this work was to explore the specificities of trajectories, gambling habits and psychopathology of gamblers of different types of gambling, to allow the development of best suited therapeutic and preventive approaches. The studies successively presented indicate that this is not simply the encounter between an individual and a gambling activity that guides the player's choice for his chosen game, but that it is a specific profile of gamblers who is attracted by a particular gambling activity. It is thus possible to identify specific gambler profiles according to their game of choice, with prospects of application in prevention, research and care. The works presented in this thesis demonstrate the importance to focus on the interaction gambler-game rather than separately to one or the other

Full textIntroduction: If gambling is a leisure activity, controlled and perceived as a source of entertainment for the majority of gamblers, for some others this behavior can become problematic. Currently, gambling motives, cognitive distortions, emotion regulation and emotional disorders (anxiety and depression) are mostly involved in the development and maintenance of gambling severity. However, the literature emphasizes the importance of distinguishing gamblers according to the type of game played (skill game and/or games of luck). This cross-sectional study aimed to determine the nature of the links between these different variables and their implications in gambling severity, both in skill and non-exclusively skill gamblers. Method: A total of 303 gamblers (229 skill gamblers and 62 mixed gamblers who practice both skill games and games of luck) were recruited and assessed for sociodemographic characteristics and gambling habits data, for gambling severity, gambling motives, cognitive distortions, emotional regulation strategies and negative emotional states (anxiety and depression). All participants were at least 18 years-old, fluent speaking French, and had a regular gambling activity (i.e. once per weak). Due to the small sub-sample of women (n=12), only male gamblers results are summarized. Results: Only male results are presented (N=291). The prevalence of at-risk and pathological gambling was respectively 17.5% and 16.2% in this sample. Although mixed players have significantly higher coping motives, illusion of control, anxiety and depression scores than skill gamblers, these differences are due to the greater severity of mixed players and not to the type of game practiced. Indeed, no difference is observed when skill and mixed gamblers are compared according to the gambling severity. However, gambling motives (coping, financial and enhancement), cognitive distortions (gambling expectancies, illusion of control, predictive control and inability to stop gambling) and anxiety and depressive symptoms increase with gambling severity in the total sample. A similar use of expressive suppression and cognitive reappraisal (emotion regulation strategies) appears between gamblers whatever the gambling severity (no significant differences). Although the gambling severity is mainly and positively linked to coping and financial motives, inability to stop gambling, and anxio-depressive symptomatology, the strength of these correlations is higher among mixed gamblers. In addition, gambling motives (coping and financial) are strongly involved in the development of cognitive distortions, which in turn are strongly involved in the development of gambling severity, and that in both skill and mixed gamblers. However, anxio-depressive symptomatology seems to play a greater role in mixed gamblers since it explains two to three times more gambling severity, coping motivation and (feeling of) the inability to stop gambling than in skill gamblers. Conclusion: Results indicate that mixed gamblers presented an emotional vulnerability that skill gamblers do not seem to express. Anxiety and depression closely linked with coping motives have probably conducted mixed gamblers to develop problem gambling due to their negative emotional states and the need to escape them. The type of game played should be taken into consideration in clinical interventions, especially because gamblers seem to present different profiles according to whether they practice exclusively skill games or have a mixed game practice. A systematic assessment of the type of game practiced, then gambling motives, cognitive distortions and emotional states could help health professionals to identify the most suitable clinical interventions for each patients

Full textIt is known from a long time that gambling disorder population suffers from a decision-making impairment, as evaluated by the "low Gambling Task" (IGT). Recently, the impact of motivation in gambling disorder has been mentioned. In thie context, the aim of the preser work was to clarity if decisional and motivational neuromarkers could constitute a reliable indicator to develop a gambling disorder. In first step, we inclued healthy volunteers in the aim to validate the motivational indicators of two Event Related Potentials (ERPs), the stimulus preceding negativity and the P300. thus, this first step led to define the link between motivation and decision-making in behavorial and neurophsysiological way. In addition, the P300 appeared to be an excellent indicator of motivation and reward sensitivity. We included video pokers players whose risk level to develop and excessive gambling was controlled. Preliminary data from thie gambler population conclude to a link between the inability to develop a successful strategy at the IGT and the risk to develop an excessiv gambling activity. All gamblers showed lesser motivation at the "Effort Expenditure for Reward Task". Whereas gamblers with a low risk to develop a gambling disorder showed a blunted P300, a neural marker of reduced interest about outcomes and a lesser motivation gamblers with high risk showed a P300 amplitude testifying of a great interest to immediate outcomes and a strong motivational level. It addition, the P300 amplitude was correlated to the risk level to develop a gambling disorder. At the end of this work, the P300 appears to be a reliable biomarker in the risk evaluation of gambling disorder. The pursuit of our investigations should confirm ou hypothesis

Full textThe aim of this research work is to investigate the psychological, emotional and motivational determinants of prosocial and antisocial behaviors in multiplayer online video games by combining several, but complementary, methodological approaches. A first systematic review article provides an overview of the links between the dimensions of the Five Factor Model of personality traits and behaviors associated with video games. The article highlighted the gaps in scientific knowledge about non-problematic gamers. We used a quantitative approach, through online questionnaires, to differentiate the psychological and motivational profiles of casual, regular and hardcore gamers and to propose explanatory models of social behaviors in multiplayer online games. A complementary study aimed at evaluating the impact of online games on feelings of isolation, emotions and emotional regulation during covid-19 quarantine. The observational experimental approach with physiological measurement (i.e., eye tracking, facial expressions and heart rate), provides information on potential emotional triggers during online games and on how gamers react in real time according to their psychological profile. The qualitative analysis of semi-structured interviews explores the links between gaming behavior and online and offline behaviors and provides information on how gamers perceive their behavior in different contexts. Full textIn this thesis, the author explores the link between virtual and reality as a means of demonstrating the stakes at play in the identifications teenagers establish with their avatars in videogames. The idea is that these identifications play a key role in the development of some psychopathologies. In particular, the phenomenon of the cyber-narcissistic object relation is presented. This mechanism seems to be involved in excessive video-gaming. The thesis is divided in three parts: first, the theoretical framework oriented on psychoanalytic theories is defined; second, an empirical study based on results from a multiple choice questionnaire (N = 67) is presented; and third, a clinical part is laid out on 12 case studies that emerged from the author’s practice, as well as on two cases of teenagers hospitalized in child psychiatry due to excessive video-gaming. Full textDecision-making, as well as addiction, are influence by a combination of three sets of factors: the situation, the object and individual differences (Appelt, Milch, Handgraaf, & Weber, 2011; Bonnaire, 2009; Einhorn, 1970; Griffiths, 2003). Each of these factors corresponds to a chapter of this thesis. In Chapter 1, the influence of the situation is investigated through the effect of Social Context. The influence of the Social Context is studied through, the Mere Presence of an Audience and through the presence of a Competitive Climate. In Chapter 2, the influence of the object is investigated through the effect of the knowledge of the object: risk-taking behavior is studied when participants are confronted to a known game and when they are confronted to an unknown game. In Chapter 3, the influence of individual differences is investigated through the effect of Self-Esteem on risk-taking behaviors. Two measures of Self-Esteem are used: the first one is a general measure of Self-Esteem while the second one is composed of two dimensions distinguishing Self-Liking and Self-Competence. The Social Representation Theory describes a relation between Social Representation and behavior. In this line, Chapter 4 investigates the Social Representation of risk in a gambling context with two studies. The first study is done via free association tasks and the second study is based on interviews with both gamblers who have gambling disorders and gamblers who do not have gambling disorder

Full textThis research aims at better understanding the role of the perceived risk in decision-making process to adopt recommended health behavior. Particularly, built on (i) existing gambling studies, the concept of perceived risk both in consumer psychology and in health research, (iii) Rothman and colleagues’ works on health framed-messages and (iv) models of health behavior change, this study contributes to address a specific issue: In what extent does the perceived risk of adopting responsible gambling behavior influence the evaluation and the adoption of such a health behavior? Whereas, to the best of our knowledge, there is no measurement instrument of the perceived risks of adopting responsible gambling behavior, this research aims at developing a measurement index measuring the perceived risks of adopting such a health behavior. Following Churchill’s, Rossiter's, Jarvis and colleagues’ and Mackenzie and colleagues’ recommendations regarding the development of measurement instruments, we conducted a quantitative study (n = 22 gamblers) in order to thoroughly conceptualize the concept of perceived risks of adopting a responsible gambling behavior. Three quantitative studies (n = 605 gamblers) allowed assessing the stability, the predictive and discriminant validity of the measure. Results show that the perceived risks of adopting responsible gambling behavior index is a valid and reliable measure formed of five dimensions: perceived risk of (i) living less excitement, (ii) having less socialization, earning less money, (iv) winning less money and, finally, (v) having less distraction from daily life. Results suggest that gamblers perceive responsible gambling as a risky alternative to gambling because they would not be able to fully satisfy their gambling motivations. Moreover, the perceived risk of adopting responsible gambling behavior play a key role in gamblers' decision to adopt such recommended health behavior

Full textSeveral studies such as the NESARC have demonstrated the comorbidity in pathological gamblers in the general population and in clinical samples. Pathological gambling often occurs with other comorbid disorders, such as addictions, mood disorders, anxiety disorders, and personality disorders. This study was conducted among 40 pathological gamblers admitted in treatment at Centre Dollard-Cormier, Institut universitaire sur les dépendances, in Montreal. The objectives were to assess the use of alcohol and drugs, the presence of psychological distress characterized by clinical symptoms and syndromes as well as by personality disorders, and the prevalence of PTSD in the gamblers of the sample. The CPGI was used to assess the severity of gambling. The other disorders were measured through the ASI, the AUDIT, the MCMI-III, the QÉT and the ÉMST. The results show that 65% of participants had a problem with alcohol (25% currently, 40% in the past), 27.5% presented a problematic drug use, 52.5% had a potential diagnosis of at least one clinical syndrome (mainly anxiety and depression), 55% had a potential diagnosis of at least one personality disorder, 30% presented symptoms of PTSD and 17.5% had a potential diagnosis of PTSD. In conclusion, comorbidity exists in the pathological gamblers of the sample. It is essential to identify it in order to better address the needs of the individual and to help him with the symptoms that worsen the gambling problem and increase the risk of recurrence.

Full textFor two decades, research has shown a rise in the population of pathological gamblers population, followed by an increase of gambling related problems. In recent years, a growing number of studies have demonstrated a link between pathological gambling and several deviant behaviors. However, its relationship with criminal offending is still largely unraveled, despite the fact that a significant number of gamblers do commit crimes during their career. Furthermore, the dynamic aspect surrounding gambling trajectories is still understudied, leaving numerous questions unanswered. We analyze retrospective data from 100 gamblers recruited in Montreal, Canada, between 2006 and 2008. Using a multilevel longitudinal model, we assess the impact of dynamic as well as static factors. Firstly, we aim to establish which factors distinguish criminal offending from non-criminal offending within pathological gamblers. Secondly, assuming that shared antecedent factors aren’t sufficient enough to explain all variations in the adherence to one or both observed trajectories over time, we assess the impact of conventional and deviant lifestyle circumstances on criminal offending. Moreover, we seek to uncover the sequential logic linking pathological gambling and criminal offending in order to determine which behavior precedes the other. Results reveal that gambling parameters are strongly associated with crime, at the static as well as at the dynamic level. Joint analysis of both static and dynamic factors related to gambling trajectories shows the impact of deviant lifestyle circumstances on criminal offending.

Full textModerate risk and problem gamblers in the Province of Quebec represent 1.4% and 0.4% of the population, respectively (Kairouz et al., 2015). Albeit prolific, the research on gamblers’ trajectories has been hampered by several methodological shortcomings leading to heterogeneity in results. Moreover, few have empirically investigated factors that were associated with change. However, it is known that life events or significant changes in people's life have an impact on the development of psychopathology (Brown & Harris, 1989). The purpose of this thesis is to explore the ways in which gamblers evolve over time depending on the severity of their gambling behaviors and harmful consequences, and to examine prospectively associations between life events and problem gambling severity. Using a two-year follow-up prospective design including three measurement waves, 179 gamblers selected from a representative survey divided into the four categories (non-problem, low-risk, moderate-risk, and problem gamblers) of the Problem Gambling Severity Index (Ferris et al., 2001) were examined. Occurrence of life events was also assessed annually. Our analyses reveal that the moderate-risk gamblers are highly heterogeneous, both in their composition and their evolution. Problem gamblers remain vulnerable and are at-risk of experiencing chronic problems over time. Cross-lagged analyses showed that some specific life events related to work, financial, social and lifestyle arenas, as well as the cumulative number of life events, were prospectively associated with an increase in severity of problem gambling 12-months later. Thus, this dissertation shows the significance of addressing sub-group specificities in secondary prevention programs, and the necessity for continuity of cares and relapse prevention in treatment programs. Moreover, social circumstances and stress affect gambling trajectories and need to be taken into account in secondary prevention and treatment.
