Abstract:

Purpose Collaboration facilitates competitive advantages. That’s the reason why every leader should be concerned with whether his or her part of the organization embraces collaboration or not. Collaboration in itself is an instrumental value, and it should be pursued as part of an organization’s enterprise-wide strategy, especially in merged organizations. Unfortunately, the natural inclination in merged organizations is for the merged entities to continue the past practice of independent, autonomous operations. When merged, the separate entities of the new organization naturally resist integration. The leader must infuse collaboration to enable knowledge sharing and the integration and employment of the new organization’s capabilities. The following discussion presents a roadmap to collaboration focused on the leader’s employment of seven critical practices. Design/methodology/approach Concept Paper Findings Scholarly study points to collaboration as an instrumental value. If it must become a value in the organization, then leaders must use seven practices as part of an enterprise-wide strategy and carefully follow the implementation of the seven practices until collaboration becomes a value and the natural instinct of the organization. Originality/value Leaders struggle with how to bring collaboration into an environment where inertial forces impede it. The following discussion presents a roadmap to infuse collaboration and grow an appreciation for it because this process will facilitate the development and implementation of goals, objectives and action plans for the organization. Seven practices are discussed and integrated into an operational design-like method just like the one commanders use to achieve victories in military campaign planning and execution.

As we all know UK Covid19 preparation was little clumsy and late to enforce, while people were getting locked down elsewhere in Europe, we were asked to do frequent hand washing. I worked and operated until 11th of March. Booked a basketball court on 14th March and played with my son, one to one, for a good hour. I saw another patient later in the afternoon and attended a birthday same evening. Walked back home and was absolutely fine. Next morning got up with dry cough, sore throat, fever and body aches and pain. There was no sinus issue, no running nose. Gradually and within a day developed loss of appetite and loss of taste. I knew I had the virus, called my manager cancelled all my appointments and operations for the next week.

Antiphospholipid antibody syndrome (APLA) is a non-inflammatory autoimmune disease characterised by spontaneous abortion, thrombocytopenia and thrombosis (arterial and venous). Intracardiac thrombosis is a rare complication of APLA, but coronary sinus thrombosis in APLA has hitherto not been reported. We recently treated a young woman with secondary APLA and systemic lupus erythematosus in whom coronary sinus thrombosis was detected in association with recurrent pulmonary embolism. Pharmacists who work in long-term care—or in any area of health care—are exposed to the health care system on a daily basis. While most of us recognize that the health care system has problems, especially at transitions of care, few of us appreciate fully how serious these problems can be. Certifying and accrediting agencies have encouraged health care providers to step out of their silos and work more collaboratively, and educators have emphasized understanding geriatric syndromes. A description of a personal journey through the health system—for both the patient and the caregiver—with two older individuals, this narrative discusses how difficult it is for patients to understand what's happening around them. It also discusses the clinical inertia in addressing geriatric issues and offers suggestions to health care providers facing similar situations.

Sinus of Valsalva Aneurysms are extremely rare cardiac anomalies.The sinus of Valsalva aneurysm (SVA) is a small dilatation caused by a separation between the aortic media and annulus fibrosus. Its may be either acquired or congenital. The right coronary sinus is most frequently affected. The most common complication being rupture.We present the case of a 12-year old girl with ruptured non-coronary sinus of valsalva aneurysm associated with ventricular septal defect , mitral regurgitation grade II and severe pulmonary hypertension. Echocardiography is sufficient to diagnose SVA, its complications, repercussions, and surgical options. SVA, even if asymptomatic, has potential risks of expansion,rupture, cardiac failure, endocarditis, embolism and sudden death. We report a rare case of Kartagener's Syndrome, congenital variety of Primary Ciliary Dyskinesia (PCD) with infertility. The patient exhibited the classical triad of which was elucidated by Manes Kartagener in 1933. The frequency of KS in the United States is 1 case per 32,000 live births. Situs inversus occurs randomly in half the patients with PCD; therefore, for every patient with KS, another patient has PCD but not situs inversus. Current nomenclature classifies all congenital ciliary disorders as PCDs in order to differentiate them from acquired types. KS is part of the larger group of disorders referred to as PCDs. Absence of frontal sinus is usually associated with various syndromes such as craniosynostosis, osteodysplasia, down syndrome etc. Geographically, absence of frontal sinus is seen usually in areas with cold climate. Background: Maxillary sinus and its close proximity to the oral cavity make it a common clinical concern for medical and dental practitioners. Due to anatomical complexities, it is difficult to evaluate the relation between the root apices of maxillary posterior teeth and the floor of maxillary sinus which is essential for diagnosis of sinus pathology, understanding the path of dental infection and planning of dental treatment.Aims and Objectives: To correlate the topographic relationship of the maxillary sinus floor to the maxillary posterior teeth roots as imaged by pairs of Orthopantomograph (OPG) and Digital Volumetric Tomography images (DVT).Materials and Methods: A total of 510 maxillary teeth from 85 patients were classified according to their topographic relationship to the maxillary sinus and measured according to their projection lengths on the sinus cavity using OPG and DVT modalities.Results: In cases of classification 0, 85% and in classification 1 , 55.3% cases shows similar classification in both OPG and DVT. 28.5% of cases in both imaging modalities show classification 2. Only 15.9% of teeth roots exhibits classification 3. 11.1% of cases showed classification 4.The panoramic radiograph showed a statistically significant 2.24 times longer root projection on the sinus cavity in OPG comparison to DVT images.Conclusion: Teeth roots projecting in to the sinus in OPG, shows no vertical protrusion in to the sinus in DVT images. Abstract Aim The purpose of this paper is to present a case involving the surgical management of a cutaneous sinus tract and a literature review. Background A cutaneous sinus tract of dental origin may easily be misdiagnosed. Exact diagnosis is necessary in the management of this pathological situation. Case Description A healthy 40-year-old man presented with a dimple in the skin of his right cheek. Upon further examination, the clinical crown of the mandibular right first molar was missing, leaving only the roots visible. Palpation of the affected area revealed a cord-like tract that was surgically excised. Summary A cutaneous sinus tract of dental origin is a canal that drains the infection from a dental source to the face or neck. A misdiagnosis of these lesions could lead to an ineffective and inappropriate treatment. We report a case of a cutaneous sinus tract of dental origin that we removed from the periapical zone of the causal teeth. The aim of this paper is to present a dental and medical literature review of cutaneous sinus tract that has a dental origin and to report a case where the sinus tract was surgically eliminated. Patients with a cutaneous facial sinus tract of dental origin often do not have obvious dental symptoms that can lead to misdiagnosing this pathological situation. Clinical Significance Elimination of the source of infection by endodontic treatment or tooth removal generally results in resolution of the sinus tract. The first robot rover to land on the Moon in nearly 40 years, China's Jade Rabbit, has begun sending back photos, with shots of its lunar lander. Jade Rabbit rolled down a ramp lowered by the lander and on to the volcanic plain known as Sinus Iridum at 04:35 Beijing time on Saturday (20:35 GMT). It moved to a spot a few metres away, its historic short journey recorded by the lander. On Sunday evening the two machines began photographing each other. A Chinese flag is clearly visible on the Jade Rabbit as it stands deployed on the Moon's surface.

Abstract:

Background: Mucosal abnormalities of the paranasal sinuses are frequently encountered as incidental findings during MRI evaluation of brain, however, little is known about their magnitude and spectrum in the Nepalese population. The purpose of this study was to analyze the spectrum of incidental mucosal abnormalities of paranasal sinuses in patients who had undergone MRI of brain for suspected intracranial pathologies. Methods: A retrospective cross-sectional study was conducted on 600 consecutive patients referred for brain MRI with suspicion of intracranial pathologies over a period of two years. The mucosal abnormalities of paranasal sinuses were evaluated and the findings were categorized according to the anatomic location and the imaging features of the abnormality seen on MRI. Results: Of total 600 cases, sinus abnormality was seen in 349(58.2%) patients. The spectrum of sinus abnormalities in 349 patients was as follows: mucosal thickening - 313(89.7%), polyp/retention cyst - 139(39.8%), sinus opacification - 114(32.7%), and fluid level - 23(6.6%). Maxillary sinus was involved in 291(83.4%) followed by ethmoid in 257(73.6%), frontal in 169(48.4%) and sphenoid sinus in 115(33.0%) cases. Conclusion: Incidental mucosal abnormalities of paranasal sinus are common findings on MRI performed for evaluation of intracranial pathologies. Mucosal thickening is the commonest abnormality and the maxillary sinus is the most commonly affected sinus. Objective: Urogenital Sinus is a rare congenital deformity which overture to preclude of normal embryonic vaginal progression. We report a case of urogenital sinus with ambiguous genitalia in a female adult. Case Report: An unmarried young female aged 26 years presented to our hospital with amenorrhea of 6 months and history of urinary incontinence from childhood. The diagnostic features of this case include normal haemogram with mildly elevated blood urea and creatinine, normal 17 ketosteroids, serum testosterone and DHEA; bilateral moderate hydronephrosis with uterus of size 6.8 x 4.6 x 5.3 cm; urogenital sinus with partial separation of bladder and vagina-vaginal type; bilateral hydrouretronephrosis with mild renal cortical scarring and bicornuate unicollis uterus cervix opening into the urogenital sinus. She was diagnosed with persistent urogenital sinus (Vaginal type) without congenital adrenal hyperplasia, but with associated mullerian anomaly. Treatment option was a U-shaped vaginoplasty with an aim of widening vaginal introitus and advancement of hypospadiac external urethral meatus to prevent micturition into vagina. Conclusion: Though the possible treatment options were discussed, this case study emphasizes the need of psycho social rehabilitation for the proper treatment of ambiguous genitalia. Rupture sinus of Valsalva (RSOV) is an uncommon condition with a wide spectrum of presentation, ranging from an asymptomatic murmur to cardiogenic shock or even sudden cardiac death. Our case presented at 62 yrs with progressive onset of dyspnea, palpitation with deterioration of exercise capacity. Diagnosis of ruptured sinus of Valsalva was made by echocardiography, it was aneurysmal and opened into right ventricular outflow tract. Coronary artery disease was excluded by coronary angiogram. Probable cause of rupture was atherosclerosis.We closed percutaneously with ADO I device. The procedure was completed uneventfully. Patient discharged with duel antiplatelet and is on follow up. Paranasal sinus mucocele commonly involved fronto-ethmoidal region rather than other due to its narrow anatomical drainage outflow, which put them at a higher tendency to get obstructed. Usually, it arises from an identifiable cause such as a history of endonasal surgery, facial trauma or background of nasal allergy or rhinosinusitis. Rarely patient presented with primary frontal mucocele, and its presentation depending on the mucocele location and extension with surrounding mass effect. Treatment is based on restoration of frontal sinus drainage and ventilation or towards a more radical and definitive approach which is sinus obliteration. Risk and benefits between these two need to be taken into consideration. In this essay I argue that predictions for the future of the philosophy of sport (as well as kinesiology as a whole) are complicated by at least three factors. These include the emergence of what I identify as “minority voices,” the fact that appearances deceive and that going “backwards” sometimes results in moving forwards, and the emerging realization that those of us in seemingly independent research silos are actually interrelated. Philokinesiologists cannot predict where they are going without knowing where physiokinesiologists, biomeckinesiologists, pedekinesiologists, and others are moving, and visa versa. I describe this uncertain journey as an exciting adventure, one that is made all the more interesting because we will be traveling together.

Background: Pilonidal sinus is the relatively common condition affecting men almost twice as often as the women we experienced. The estimated incidence is 0.26 per thousand populations in general. The management of pilonidal sinus disease remains controversial, and gold standard treatment modality has yet to be established. Limberg procedure is a safe and reliable technique in the treatment of the sacrococcygeal pilonidal sinus disease, with a low complication and recurrence rates if performed according to the appropriate surgical principles. Methods: This is a Prospective study on 24 patients between the period from July 2014 to Dec 2018 in the Department of Surgery in Bangabandhu Sheikh Mujib Medical University (BSMMU) as well as other different hospitals in Dhaka, Bangladesh. The patients having primary or recurrent pilonidal sinus disease underwent the foresaid surgical treatment. Results: Twenty four patients underwent this operation. Among them, the picture was that 20(83.3%) were males and 4(16.7%) were female. The mean age was 31, (Range: 17-45 years). 6(25%) patients were presented with recurrent sinus and 5 of them had one or more occasion previous surgery. Twenty one patients (87.5%) had full primary healing without any complication. But 1(4.2%) patient had minimal sarcoma, 1(4.2%) patient had superficial infection and the remaining other 1(4.2%) had partial flap necrosis. However, all three healed completely with conservative treatment. The mean length of hospital stay was 2.45 (Range: 1-5) days and the most patients returned to their work within 3 weeks. Conclusion: Limberg flap is very effective for sacrococcygeal pilonidal sinus disease with low complication rates, shorter hospital stay, low recurrence rates, earlier healing and reduced off-work period. The surgery can be easily mastered. We recommend Limberg flap as preferred surgery in the cases of Sacrococcygeal Pilonidal sinus. Bangladesh Journal of Medical Science Vol.19(1) 2020 p.105-109

There is growing evidence that arrhythmias are a major complication of COVID-19 infection. Inflammation of the myocardium, cytokine storm, hypoxia, changes of electrolyte levels, coronary vasospasm, microcloths may be factors that contribute to the development of arrhythmias and changes of the ECG. The aim of the study was to determine the sex and age characteristics of the prevalence and structure of cardiac arrhythmias in patients with severe COVID infection. In the study were examined 133 patients with severe COVID-19 infection, who were in the intensive care unit of the city clinical hospital №1 in Vinnytsia at the period from April to December 2020 and as a result of treatment with improved health were discharged from the hospital. Statistical processing of the obtained data was performed using the statistical software package SPSS 12.0 for Windows using parametric and non-parametric methods of statistical analysis. It was found that the most common comorbid pathologies of the patients with severe COVID-19 were – arterial hypertension 90 (67.7%), coronary heart disease 91 (68.4%), chronic heart failure 65 (48.9%), obesity 47 (35.3%) and diabetes mellitus 32 (24.1%). It was found that the prevalence of cardiac arrhythmias in patients with severe COVID-19 was 86 (64.7%) patients. Among all arrhythmias, tachyarrhythmias predominated – 68 (51.1%), in the structure of which sinus tachycardia was the most common in 45 (33.8%) and atrial fibrillation (AF) – in 19 (14.3%) patients, respectively. In turn, sinus tachycardia was significantly more common among women. The prevalence of bradyarrhythmias was 60 (45.1%), in the structure of which were dominated sinus bradycardia 16 (12%) and blockade of the right branch of the His bundle 30 (22.6%). Among men, there was a tendency to the predominance of complete left bundle branch block. The prevalence of ischemic changes and/or repolarization disorders according to the ECG was 119 (89.5%), in the structure of which were dominated the presence of inversion of the T wave 91 (68.4%) and depression of the ST segment 54 (40.6%). The mean age of the patients with ischemic ECG changes was significantly higher compared to patients without signs of repolarization disorders. Among men, there was a predominance of pancreatic hypertrophy and a tendency to predominance of left ventricular hypertrophy.

Cerebral Venous Thrombosis (CVT) is a rare form of Stroke characterized by thrombus formation in the cerebral veins. CVT is a result of various reasons among which the hyperthyroidism is not so frequently encountered. This is probably the first case report published from Nepal. The aim of this case report is to give the message that persistent severe headache in patient with hyperthyroidism can be the red flag and needs to be investigated further. We present a case of a 35 years old female who presented with complaints of severe headache and persistent in nature associated with vomiting since 5 days. She was a diagnosed subacute thryroiditis and under medicine from 1 month before presenting to us. Her Magnetic Resonance Venography (MRV) brain showed venous thrombosis within superior saggital sinus, left transverse sinus and sigmoid sinus. Her thyroid function test showed pretreatment T3 of 2.98 ng/ml T4 of 1.02 mg/ ml and TSH of 0.12 μIU/L. She was kept on anticoagulants and other supportive measures. Background The purpose of this study was to determine the efficacy of pediatric endoscopic sinus surgery based on a clinical symptom score outcome. We conducted a prospective analysis of established measures of clinical outcome based on a survey that was completed preoperatively and again 2 years after pediatric endoscopic sinus surgery. Methods Twenty-seven children underwent endoscopic sinus surgery. Twenty-three of the 27 children's surveys were completed both preoperatively and postoperatively by parents of the children. The main outcome measure was scores on parental survey and parental perception of surgical success. Results Results revealed that all children had an improved symptom score in all categories postoperatively as compared with preoperatively. Statistical analysis using the nonparametric sign test showed that a significant decrease (p < 0.05) in score occurred in 9 of the 15 categories (score lowered from preoperatively to postoperatively) for the children on an individual basis. These categories included a decrease in frequency of a cough (p = 0.013), nasal congestion (p = 0.031), visit to the doctor's office or hospital because of associated problems (p = 0.008), amount of days missing school or child care (p = 0.001), problems with routine activities such as eating and sleeping (p = 0.001), problems with attendance or conduct at school (p = 0.004), and a decrease in the child's sinus problems affecting the parent's performance at their job or at home (p = 0.035). Conclusions We concluded that parental perception of their child's clinical outcome after pediatric endoscopic sinus surgery appears favorable. (American Journal of Rhinology 16, 151–154, 2002)

The aim of this research was to know whether the Guided Discovery Instruction (GDI) method can improve conceptual understanding in learning mathematics. The research method used classroom action research in two cycles. The pre-cycle showed that students' conceptual understanding still was low. The research was conducted at Sekolah Lentera Harapan (SLH) in Ambon involving 14 students and the instruments used were post-test questionnaires, attainment sheet method, interviews, observations, and journal reflections. The results showed that using GDI improved students’ conceptual understanding in learning mathematics as evident by the increased number of students graduating KKM (71) from the first cycle (71.4%) to the second cycle (85.7%).BAHASA INDONESIA ABSTRAK: Penelitian bertujuan mengetahui peningkatan pemahaman konsep siswa melalui metode GDI dengan model penelitian tindakan kelas yang dilakukan dalam dua siklus. Penelitian ini dilakukan pada kelas XI-IPS SLH Ambon melibatkan 14 siswa dan menggunakan post-test, angket, lembar ketercapaian metode, wawancara, observasi, angket, dan jurnal refleksi sebagai instrumen. The form of research used in this study is Action Research. The population in this study amounted to 69 people consisting of 37 students in class VIII of SMP 1 Bandarlampung and 32 students in 32 students of class VIII A of SMP 25 Bandarlampung. To measure the students 'understanding of the concept, there were 3 cycles of tests with multiple choice questions 20 questions in each cycle. The results showed an increase in students' understanding of physics concepts seen from the percentage of learning outcomes completeness. Cycle I percentage of completeness in class VIII I is 40.54%, cycle II is 51.35 and in cycle III is 83.78%. Cycle I percentage of completeness in class VIII A is 18.75%, cycle II is 43.24% then in cycle III is 81.25%. Untuk mengukur pemahaman konsep siswa dilakukan test sebanyak 3 siklus dengan soal pilihan ganda 20 soal dimasing-masing siklus.Hasil penelitian menunjukkan peningkatan pemahaman konsep fisika siswa di lihat dari presentase ketuntasan hasil belajar. Siklus I presentase ketuntasan dikelas VIII I sebesar 40,54 %, siklus II sebesar 51,35 dan pada siklus III sebesar 83,78 %. Siklus I presentase ketuntasan dikelas VIII A adalah 18,75 %, siklus II adalah 43,24 % kemudian pada siklus III adalah 81,25 %. Melalui hasil analisis dan pembahasan dapat menyimpulkan untuk penggunaan program Crocodile Physics mampu dalam meningkatnya pemahaman konsep fisika.

OBJECTIVEIt is common for a medical disorder to be managed or researched by individuals who work within different specialties. It is known that both neurosurgeons and neurotologists manage vestibular schwannoma (VS) patients. While overlap in specialty focus has the potential to stimulate multidisciplinary collaboration and innovative thinking, there is a risk of specialties forming closed-communication loops, called knowledge silos, which may inhibit knowledge diffusion. This study quantitatively assessed knowledge sharing between neurosurgery and otolaryngology on the subject of VS.METHODSA broad Web of Science search was used to download details for 4439 articles related to VS through 2016. The publishing journal’s specialty and the authors’ specialties (based on author department) were determined for available articles. All 114,647 of the article references were categorized by journal specialty. The prevalence of several VS topics was assessed using keyword searches of titles.RESULTSFor articles written by neurosurgeons, 44.0% of citations were from neurosurgery journal articles and 23.4% were from otolaryngology journals. The citations of otolaryngology authors included 11.6% neurosurgery journals and 56.5% otolaryngology journals. Both author specialty and journal specialty led to more citations of the same specialty, though author specialty had the largest effect. Comparing the specialties’ literature, several VS topics had significantly different levels of coverage, including radiosurgery and hearing topics. Despite the availability of the Internet, there has been no change in the proportions of references for either specialty since 1997 (the year PubMed became publicly available).CONCLUSIONSPartial knowledge silos are observed between neurosurgery and otolaryngology on the topic of VS, based on the peer-reviewed literature. The increase in access provided by the Internet and searchable online databases has not decreased specialty reference bias. These findings offer lessons to improve cross-specialty collaboration, physician learning, and consensus building.

Abstract:

We report an extremely rare case of bilateral Non-Hodgkin lymphoma of maxillary sinus. Although most types of lymphoma have good response to chemotherapy especially in the early stages, our patient demonstrated resistance to the chemotherapy. Spinal metastases resulted in dismal end. The usual R-CHOP regimen was not effective prevent spread even after 4 cycles. This study aims to formulate a system of accounting cycle on trash bank. This studyused descriptive methods and qualitative data. This study’s subjects consisted of respondents and the data library that collected by interviews, documentation and literature. Analysis was done by analyze the data to understand the process of accounting cycle, preparing the system, the last summed up the results of analysis. The study found a system of financial records and financial statements based on SAK ETAP and some contract in PSAK Sharia. Accounting system has a preparation, recording and classification stage. The preparation stage such as chart of account, price list and customers list. The recording stage such as ledgers: trash inventory, savings cooperatives, list of net income cooperatives, list of savings, list of lending, list of electrical and pulse supplies and special journals: purchase of trash, sales of trash, saving of trash, cash receipts and cash payments. The classification stage such as general ledger, adjusting entries, trial balance. Then the system of financial statements consist of balance sheet, income statement, equity statement, cash flow statement and notes to the financial statements. Finally, for the preparation next period there are after closing journal and trial balance after closing.

Purpose – The purpose of this paper is to argue that recent attention has been focused on inpatient services at the expense of community mental health teams and that it is time to redress the balance. Design/methodology/approach – This is a personal viewpoint. Findings – In writing this piece it has enabled us to focus on just how widespread the issues are regarding the lack of focus on community services, and that the view and paradigm needs to change on all levels/structures. Services need to recognise the wide scope of community services and the part they inevitably play in someone’s recovery journey. It also throws the spotlight on services working too often in silos deeply affecting people in receipt of the services. Originality/value – To stimulate debate about the role of community mental health teams.

Abstract The monthly magazine Hochland was probably the most influential Catholic cultural periodical in Germany in the Weimar Period. According to Georg Cardinal von Kopp’s assessment in 1911, it was “unfortunately the most read periodical in all of the educated circles of Germany, Austria and German Switzerland”. Moving beyond the simple rejection of modern culture in Germany, the journal tried to follow a new program of mediatory engagement, although it did continue to hold to traditional positions in many regards. In this article the reception of modern, Enlightenment-affirmative philosophy of religion in the journal is introduced with reference to reviews and essays from the later 1910s to the early 1930s. The journal’s treatment of a few critical subject areas is given close interpretive analysis, including the journal’s treatment of Gertrud Simmel’s Über das Religiöse, individually conceptualized forms of personalist moral theory, and the general shift to phenomenological discourses and the individual in the philosophy of religion. The fundamental rejections of these ideas and these schools of thought in reviews and essays, which are also found in the journal at this time (as in most all German language Catholic cultural journals of the period), are not addressed in this article. The article thus sheds light on an often-forgotten and relatively small minority phenomenon in German Catholic intellectual circles of the Weimar Period, namely the positive embrace of Enlightenment-oriented modern thought. By promoting these ideas at this time, this group made themselves highly vulnerable to disciplinary measures by the Catholic Church. (The journal was put on the Index in 1911.)

Sinus histiocytosis with lymphadenopathy is a rare condition of unknown etiology that is characterized by the overproduction of histiocytes accumulating in the lymph node sinuses and in the lymphatics of extranodal sites if involved. Lymph nodes of the neck are the most common place of histiocyte accumulation, although other lymph nodes and organs may involve, as well. The common sites of the disease outside the lymph nodes are skin, soft tissues, upper respiratory tract, the sinuses, kidney, thyroid, small bowel, breast, and bone. Hepatosplenomegaly is uncommon. The symptoms of this disease vary with the site of involvement. It is often referred to as sinus histiocytosis with massive lymphadenopathy (SHML). In this article an adult male of 38 years, has been described who presented with bilateral cervical and axillary lymphadenopathy clinically suspected of infectious (tubercular-in the context of our country) or hematological malignant etiology. However on fine needle aspiration cytology, the smears showed many histiocytes, lymphocytes and plasma cells. Biopsy of cervical lymph node showed dilated sinuses filled with histiocytes and lymph node architecture showing inflammatory cells having lymphocytes and histiocytes. Osteoma is the most frequent benign tumor of paranasal sinuses. Usually osteomas are asymptomatic and discovered incidentally during radiological imaging for other reasons. A giant osteoma with intraorbital extension is rare. Plain radiograph may be useful; however CT scan is the best modality of investigation for diagnosing and treatment planning. In asymptomatic osteomas, serial follow-up can be done. Surgery is performed in the presence of symptoms and signs. We report a case of 60 years old woman with a large osteoma of right ethmoid sinus extending into the ipsilateral orbital cavity. Most arrhythmias that occur during sleep are detected incidentally on Holter monitoring, and are in fact benign. However, sometimes they may be an important clue to an underlying disorder requiring further investigation and treatment. Efforts should be given to recognize factors predisposing to sleeprelated arrhythmias; successful management of an underlying disorder can prevent and treat potentially dangerous cardiac rhythms. Sinus Histiocytosis/Rosai-Dorfman Disease (RDD) are benign, rare proliferative disorder caused by over production and accumulation of specific type of white blood cell (Phagocytic Histiocyte) in the lymph nodes of the body. Here, lymphadenopathy mostly painless and commonly found in the neck (cervical) but may occur in other areas of the body such as skin, lung, central nervous system, kidney (less than 5%). Predominantly it affects the young age group of children, adolescents or young adults. Kepulauan Karimunjawa merupakan salah satu Kawasan Strategis Pariwisata Nasional (KSPN) di Indonesia yang sedang dikembangkan. Keunggulan dari pariwisata di Karimunjawa adalah daya tarik wisata alamnya yang berupa wisata bahari, ekowisata, dan wisata petualangan. Potensi kekayaan bahari di Karimunjawa yang tidak kalah menarik salah satunya berupa peninggalan budaya bawah air. Tempat tenggelamnya kapal dan peninggalan bawah air yang berada di Karimunjawa membuat situs-situs ini berpotensi menjadi alternatif tujuan wisata bawah air khususnya daya tarik wisata minat khusus. Tujuan dari penelitian ini adalah menjelaskan profil produk wisata situs-situs bawah air di Karimunjawa. Secara administratif penelitian ini akan difokuskan berdasarkan batas geografis dari situs-situs bawah air yang berada di Perairan Pulau Karimunjawa (Situs Kapal Genteng dan Kapal Indonoor) dan Perairan Pulau Genting (Situs Seruni dan Situs Genting). Penelitian ini akan mengunakan metode deskriptif kualitatif. Pengamatan fisik akan dilakukan pada produk wisata (atraksi, aksesibilitas, akomodasi, fasilitas, service) serta lingkungan pada kawasan situs-situs peninggalan bawah air. Hasil penelitian menunjukkan bahwa situs-situs bawah air di Perairan Karimunjawa yang dapat dikembangkan sebagai daya tarik wisata minat khusus adalah Situs Kapal Indonoor dan Situs Kapal Genteng. Sedangkan untuk Situs Seruni dan Situs Genting masih harus dilakukan studi lebih lanjut karena dengan kondisi produk wisata saat ini kedua situs tersebut belum memungkinkan untuk dijadikan obyek wisata minat khusus.Kata-Kata Kunci: produk wisata, wisata minat khusus, situs bawah air, ANALYSIS OF TOURISM PRODUCTS OF UNDERWATER SITES AS ONE OF SPECIAL INTEREST TOURISM IN KARIMUNJAWA NATIONAL PARKKarimunjawa Islands are one of the National Tourism Strategic Areas in Indonesia and development of these areas are already underway. The research focus was divided based on the geographical boundaries of the underwater sites in the waters of Karimunjawa Island (Genteng and Indonoor Shipwreck Sites) and the waters of Genting Island (Seruni and Genting Sites). This research used a qualitative descriptive method grounded in relevant theories. The physical observation was made on tourism products (attractions, accessibility, accommodation, facilities, and service) and the environment around the sites of underwater relics. Findings of the research suggest that underwater sites which are potential for development as special interest tourism objects are Genteng and Indonoor Shipwreck Sites.
