Abstract:
ObjectivesCommunity members are often the first to witness and respond to medical and traumatic emergencies, making them an essential first link to emergency care systems. The Emergency First Aid Responder (EFAR) programme is short course originally developed to help South Africans manage emergencies at the community level, pending arrival of formal care providers. EFAR was implemented in two rural regions of Zambia in 2015, but no changes were originally made to tailor the course to the new setting. We undertook this study to identify potential refinements in the original EFAR curriculum, and to adapt it to the local context in Zambia.DesignThe EFAR curriculum was mapped against available chief complaint data. An expert group used information from the map, in tandem with personal knowledge, to rank each course topic for potential impact on patient outcomes and frequency of use in practice. Individual blueprints were compiled to generate a refined EFAR curriculum, the time breakdown of which reflects the relative weight of each topic.SettingThis study was conducted based on data collected in Kasama, a rural region of Zambia’s Northern Province.ParticipantsAn expert group of five physicians practising emergency medicine was selected; all reviewers have expertise in the Zambian context, EFAR programme and/or curriculum development.ResultsThe range of emergencies that Zambian EFARs encounter indicates that the course must be broad in scope. The refined curriculum covers 54 topics (seven new) and 25 practical skills (five new). Practical and didactic time devoted to general patient care and scene management increased significantly, while time devoted to most other clinical, presentation-based categories (eg, trauma care) decreased.ConclusionsDiscrepancies between original and refined curricula highlight a mismatch between the external curriculum and local context. Even with limited data and resources, curriculum mapping and blueprinting are possible means of resolving these contextual issues.

Art and Healing: The Rituals of Possession Mahamba Related to Fertility Among the Luvale of Zambia This paper deals with a number of symbolic practices and art forms that have developed within rituals of possession known as mahamba in the upper Zambezi and Kasai area. I refer mostly to the Luvale of Zambia, among whom there are numerous mahamba, variously related to serious illness, sterility, madness, failure, social alienation or other ills. Presenting alternatively aspects of ad hoc therapeutic rituals, and “social drama, to borrow V. Turner’s expression”, or of religious cults, they are performed by both men and women. Mahamba can lead to initiation into a professional cast or simply serve to restore a possessed patient's physical health or social status. As rituals that are both 'religious' and 'therapeutic', they must be considered within the broad context of cosmology and medical knowledge. Here I will discuss the transforming role of certain artefacts and performances in mahamba rituals that aim at restoring female fertility.

Abstract:
This study examines long-term spatial and temporal trends of drought characteristics based on the Standardized Precipitation Index at three different time scales (3, 6, and 12 months) over Zambia from 1981 to 2017. Drought characteristic conditions such as duration, severity, and intensity at monthly, seasonal, and annual levels were analyzed to investigate the drought patterns over Zambia. The results show a significant increase in drought events over the southwestern regions and a decrease over the northeastern regions. It is in this regard that two stations from different locations (southwest and northeast) were analyzed. The results show increasing trends of drought over Sesheke (southwest) and decreasing trends over Kasama (northeast). More drought impacts are felt over southern compared to northern areas, which poses a serious concern to both agriculture and hydrological industries over the drought-prone areas of Southern Zambia. However, the analysis further shows that droughts were more intense, persistent, and severe over the southwest, while moderate droughts were found in some few areas of Northeast Zambia. The Mann–Kendall test trend and slope indicated that both annual and seasonal drought have increased. However, drought increment at an annual level shows a low magnitude as compared to the seasonal level. This suggested the importance of evaluating drought at an interannual and seasonal time scale over Zambia. Specifically, the drought increased determinedly before 2010 and became erratic between 2010 and 2017 with considerable regional variation. Zambia experienced moderate to severe droughts during 1991–1992, 1994–1995, 2006–2005, and 2015–2016, which resulted in serious damages to the environment and society. According to the findings of this study, it is suggested that the implications of drought can be managed by creating strategies and adaptation measures.

Opening ParagraphThe research upon which this article is based was carried out over a period of four months in the Northern and Luapula Provinces of Zambia. Before that, I spent two years working in the area as an agricultural officer. One of the villages described in the article is situated on the western shore of Lake Bangweulu, the other 20 km south of Kasama, the major city of Northern Province, the homeland of the Bemba (Fig. 1). The area is almost exclusively devoted to chitemene shifting cultivation, an agricultural system where crops are grown in the ash from burning the collected, stacked branches that have been lopped and chopped from an pressure, vegetation is now chopped before it is fully regenerated, and the system seems to be starting to break down. A major task during my field stay was to describe the ecological (Stromgaard, 1984a) and economic (Stromgaard, 1984b) aspects of this change, but it soon became apparent that subsistence activity and social structure deeply influenced the village's economic activities.

Abstract:
A human-porcine reassortant strain, RVA/Human-wt/ZMB/UFS-NGS-MRC-DPRU4723/2014/G5P[6], was identified in a sample collected in 2014 from an unvaccinated 12 month old male hospitalised for gastroenteritis in Zambia. We sequenced and characterised the complete genome of this strain which presented the constellation: G5-P[6]-I1-R1-C1-M1-A8-N1-T1-E1-H1. The genotype A8 is often observed in porcine strains. Phylogenetic analyses showed that VP6, VP7, NSP2, NSP4, and NSP5 genes were closely related to cognate gene sequences of porcine strains (e.g., RVA/Pig-wt/CHN/DZ-2/2013/G5P[X] for VP7) from the NCBI database, while VP1, VP3, VP4, and NSP3 were closely related to porcine-like human strains (e.g., RVA/Human-wt/CHN/E931/2008/G4P[6] for VP1, and VP3). On the other hand, the origin of the VP2 was not clear from our analyses, as it was not only close to both porcine (e.g., RVA/Pig-tc/CHN/SWU-1C/2018/G9P[13]) and porcine-like human strains (e.g., RVA/Human-wt/LKA/R1207/2009/G4P[6]) but also to three human strains (e.g., RVA/Human-wt/USA/1476/1974/G1P[8]). The VP7 gene was located in lineage II that comprised only porcine strains, which suggests the occurrence of independent porcine-to-human reassortment events. The study strain may have collectively been derived through interspecies transmission, or through reassortment event(s) involving strains of porcine and porcine-like human origin. The results of this study underline the importance of whole-genome characterisation of rotavirus strains and provide insights into interspecies transmissions from porcine to humans.

Abstract:
Background Pteropine orthoreovirus (PRV) is an emerging bat-borne zoonotic virus that causes severe respiratory illness in humans. Although PRVs have been identified in fruit bats and humans in Australia and Asia, little is known about the prevalence of PRV infection in Africa. Therefore, this study performed an PRV surveillance in fruit bats in Zambia. Methods Egyptian fruit bats (Rousettus aegyptiacus, n = 47) and straw-colored fruit bats (Eidolon helvum, n = 33) captured in Zambia in 2017–2018 were screened for PRV infection using RT-PCR and serum neutralization tests. The complete genome sequence of an isolated PRV strain was determined by next generation sequencing and subjected to BLAST and phylogenetic analyses. Replication capacity and pathogenicity of the strain were investigated using Vero E6 cell cultures and BALB/c mice, respectively. Results An PRV strain, tentatively named Nachunsulwe-57, was isolated from one Egyptian fruit bat. Serological assays demonstrated that 98% of sera (69/70) collected from Egyptian fruit bats (n = 37) and straw-colored fruit bats (n = 33) had neutralizing antibodies against PRV. Genetic analyses revealed that all 10 genome segments of Nachunsulwe-57 were closely related to a bat-derived Kasama strain found in Uganda. Nachunsulwe-57 showed less efficiency in viral growth and lower pathogenicity in mice than another PRV strain, Miyazaki-Bali/2007, isolated from a patient. Conclusions A high proportion of Egyptian fruit bats and straw-colored fruit bats were found to be seropositive to PRV in Zambia. Importantly, a new PRV strain (Nachunsulwe-57) was isolated from an Egyptian fruit bat in Zambia, which had relatively weak pathogenicity in mice. Taken together, our findings provide new epidemiological insights about PRV infection in bats and indicate the first isolation of an PRV strain that may have low pathogenicity to humans.

Bu çalışma Şubat 1990 ve Kasım 2019 döneminde petrol fiyat şoklarının küresel finansal aktiviteye olan zaman-değişimli etkilerini Zamana Göre Değişen Parametreli VAR (TVP-VAR) modeli uygulayarak incelemektedir. Bu bağlamda aylık spot WTI ham petrol fiyatları, dünya ham petrol üretimi ve Kansas Şehri Finansal Stres Endeksi (KCFSI) verileri ampirik analizde kullanılmıştır. Çalışmanın ampirik sonuçları petrol fiyatlarındaki kalıcı bir artışın finansal koşulları olumsuz olarak etkilediğini göstermektedir. Bununla birlikte, pozitif bir petrol arz şoku petrol fiyatlarında düşüşe neden olmaktadır. Çalışmanın bulguları literatürde elde edilen sonuçlarla uyumludur ve TVP-VAR modelinin yapısal petrol fiyat şoklarının zaman-değişimli yapısını yakalamadaki tutarlılığını göstermektedir.
