Abstract:
Tyrimo tikslas: įvertinti 8 savaičių kineziterapijos namų programos efektyvumą leukemija sergančių vaikų fizinei būklei. Tyrimo metodai: iš viso tyrime dalyvavo 16 leukemija sergančių vaikų, tiriamųjų amžiaus vidurkis - 10,81±3,83 metai. Tyrimas buvo atliekamas Vilniaus universiteto Vaikų ligoninės onkohematologijos skyriuje nuo 2014 m. lapkričio iki 2015 m. balandžio mėnesio. Tyrimui atlikti buvo panaudota centimetrinė juostelė, kuria išmatuotos kūno apimtys (šlaunų, liemens, dubens) bei krūtinės ląstos ekskursija. Tiriamųjų pėdos dorzalinės fleksijos ir šlaunies vidinės bei išorinės rotacijų amplitudės matuotos goniometru. Pėdos dorzalinę fleksiją ir blauzdos tiesimą atliekančių raumenų jėga vertinta pagal R. Lovett testą. Taip pat tiriamieji atliko 3 metrų „Stotis ir Eiti“ bei modifikuotą funkcinį judėjimo testus. Duomenų analizė atlikta naudojant statistinės analizės Excel 2007 bei „R-3.2.0 for Windows“ programas. Rezultatai: kineziterapijos namų programos taikymas 8 savaites leukemija sergantiems vaikams yra efektyvi priemonė, siekiant koreguoti kūno antropometrinius duomenis, didinti sumažėjusias judesių amplitudes ir raumenų jėgą bei gerinti „Stotis ir Eiti“ testo ir modifikuoto Keitel indekso rezultatus. Išvados. Po 8 savaičių kineziterapijos namų programos taikymo leukemija sergantiems vaikams statistiškai reikšmingai padidėjo krūtinės ląstos ekskursija, sumažėjo šlaunų, liemens ir dubens apimtys, padidėjo vertintų sąnarių judesių amplitudės, raumenų jėga, pagerėjo “Stotis ir Eiti” testo bei modifikuoto Keitel indekso rezultatai. Gauti rezultatai leidžia daryti prielaidą apie pagerėjusią tiriamųjų pusiausvyrą bei funkcinę būklę.

Abstract:
Background and Purpose The purpose of this study was to translate the German Keitel Functional Test (KFT) into Danish and test it for reliability, concurrent and predictive validity, and responsiveness in patients with rheumatoid arthritis (RA). Methods Translation of the KFT was performed according to international recommendations, and the translated version was tested twice by 2 observers for intraobserver and interobserver reliability, with a 1-week interval between assessments, in 20 patients with RA with stable disease activity. Validity was investigated by studying 2 patient groups: (1) 15 patients with long-lasting (median=6 years) active RA, tested before and after 2, 6, and 14 weeks of anti-tumor necrosis factor alpha (TNF-α) inhibitor therapy, and (2) 35 patients with early (median=0.25 year) RA, tested at years 0, 0.5, 1, and 2. Twenty-three patients in the early RA group also were tested at year 7. KFT, conventional clinical and biochemical markers of disease activity, and Health Assessment Questionnaire (HAQ) were used. Results The translated KFT showed good intraobserver reliability (intraclass correlation coefficients [ICC]=.90 and .95, coefficient of variation [CV]=3.5%) and interobserver reliability (ICC=.99 and .92, CV=3.5%), and the KFT correlated with several measures of disease activity and, most closely, with the HAQ. The KFT was, in contrast to clinical disease activity measures, not sensitive to changes over time. Only baseline KFT data were significantly related to functional changes over a long period of time as measured by the KFT, and only in the early RA group. Discussion and Conclusion The Danish translation of the KFT showed good reliability, acceptable concurrent validity, very poor responsiveness, and inconclusive results concerning predictive validity. The results of this study do not support the use of the KFT for monitoring function in clinical practice, as an outcome measure in clinical trials, or as a predictor of functional changes.

Abstract:
Zeng and Keitel proposed an arbitrated quantum signature scheme in 2002. Recently, Curty and Lütkenhaus pointed out that the protocol is not operationally specified. In a reply, Zeng gave more details of the scheme. The author also claimed that the scheme is suitable for unknown messages. In this letter, we remark that the invented scenario in the original scheme is artificial. This is because its security entirely depends on the presence of a trustworthy arbitrator. Moreover, the claim that the original scheme is suitable for unknown messages is not sound.

Abstract:
Niels Bohr, around 1930, when he tried to describe some processes in quantum physics, created the term ‘complementarity’. Bohr believed in the general epistemological and metaphysical significance of this principle. Since then several authors have used this term to capture the essential aspects of the cognitive and epistemological development of scientific and mathematical concepts (Otte and Steinbring, 1977; Kuyk, 1977; Otte, Keitel and Seeger, 1980; Otte, 1984, 1990, 1994; Douady, 1991; Sfard, 1991; Jahnke, 1992). However, what is complementarity? It is an idea or a concept! Being an idea, complementarity finds different expressions within different contexts. Mathematicians do not know what to do with ideas and they want definitions instead. In this article, we discuss different aspects of complementarity trying to clear the matter.Keywords: Mathematics Education. Philosophy of Mathematics. Bohr acreditava na significância metafísica e epistemológica desse princípio. Objective: The objective of the current investigation was to explore performance-based predictors of decline in the performance quality of everyday tasks as a first step for early identification, screening, and referral to minimize disability in community-dwelling older adults. Method: This was a secondary analysis of data from 256 community-based older women. Mobility, activities of daily living (ADLs), and instrumental activities of daily living (IADLs) were measured using the Performance Assessment of Self-Care Skills (PASS). Logistic regression models explored cognitive and motor predictors of performance quality while controlling for demographics and diagnoses. Results: Functional reach ( p = .049) and cognition ( p = .012) were predictive of mobility quality, whereas balance ( p = .007) and the Keitel Function ( p = .005) were predictive of ADL quality. Manipulation and cognitive measures were predictive of cognitive and physical IADL quality. Discussion: Cognitive and physical screens are both important to identify older adults at risk for disability.

Abstract:
Objective.To investigate disease course, outcome, and predictors of outcome in an unselected population-based cohort of individuals diagnosed with juvenile chronic arthritis (JCA) followed for 17 years.Methods.The cohort consisted of 132 incidence JCA cases identified 1984–1986 according to EULAR criteria. At 5-year followup, 129 individuals underwent joint assessment, laboratory measurements, radiographic examination, and medication and functional assessment. At 17-year followup, 86 were examined with joint assessment, laboratory measurements, medication assessment, Health Assessment Questionnaire (HAQ), Keitel functional test (KFT), and Medical Outcomes Study Short Form-36 (SF-36).Results.At 17-year followup, 40% were in remission, 44% changed subgroups, median HAQ score was 0.0 (range 0.0–1.5), and median KFT was 100 (range 54–100). SF-36 scores were significantly lower compared to a reference group. Thirty-nine percent of those in remission at 5-year followup were not in remission at 17-year followup. In multivariate analyses of variables from the 17-year followup: remission was predicted by remission at 5-year followup (OR 4.8); HAQ > 0 by rheumatoid factor (RF)-positivity at 5-year followup (OR 3.6); KFT < 100 by nonremission (OR 11.3); and RF-positivity (OR 5.6) at 5-year followup; and the SF-36 physical component summary score above average of the reference group by remission at 5-year followup (OR 5.8).Conclusion.This longterm study of 86 individuals with JCA showed large variability of disease courses and of impaired health-related quality of life. Sixty percent were not in remission at 17-year followup. Longterm outcome was best predicted by and associated with characteristics at 5-year followup rather than those at onset.

Abstract:
ZusammenfassungBeinbewegungen während des Schlafs werden in periodische Beinbewegungen und nicht periodische Beinbewegungen eingeteilt. Sie sind quasi nicht subjektiv und daher am ehesten durch eine Fremdanamnese erfassbar. Die kardiorespiratorische Polysomnografie mit Videometrie hat sich als Goldstandard zur differenzialdiagnostischen Einordnung nächtlicher Beinbewegungen erwiesen. Nach allgemein anerkannten Kriterien kann hiermit zwischen periodischen und nicht periodischen Beinbewegungen unterschieden werden. In der Regel werden periodische Beinbewegungen mit dopaminergen Substanzen behandelt, wenn sie im Rahmen eines Restless-legs-Syndroms oder einer Periodic Limb Movement Disorder auftreten. In vielen anderen Fällen besitzen nächtliche Beinbewegungen keinen Krankheitswert und bedürfen keiner Therapie. Treten nächtliche Beinbewegungen symptomatisch im Rahmen anderer Erkrankungen z. B. Schlafapnoe-Syndrom oder idiopathisches Parkinson-Syndrom auf, gilt es, die zugrunde liegende Erkrankung primär zu behandeln.

Abstract:
Die Hypothyreose ist einer der häufigsten endokrinen Störungen. Die Herausforderung in der Diagnostik ist - neben des initialen Verdachts - diejenigen Fälle zu erfassen, die eine transiente Form der Hypothyreose aufweisen und daher in der Regel keiner Therapie bedürfen. Dazu bedarf es neben dem TSH bzw. TSH und fT4 keiner weiteren Laboruntersuchung, sondern einer guten Anamnese. Bei der subklinischen Hypothyreose gibt es einen breiten Konsens darüber, Patienten mit TSH über 10 mU/l zu substituieren. Bei Werten zwischen 4 und 10 mU/l ist eine genaue klinische Evaluation zur Therapieentscheidung hilfreich. Die Beurteilung der Klinik zusätzlich zur Bestimmung des fT4 ist auch sinnvoll beim Monitoring der Therapie von Patienten mit sekundärer Hypothyreose. Die Therapie der Hypothyreose erfolgt mit L-T4, die Gabe eines Kombinationspräparates mit T3 bietet keinen zusätzlichen Nutzen. Die erforderliche Substitutionsdosis liegt bei ca. 1,6 µg/kg täglich. Bei gesunden jungen Patienten kann mit einer Dosis von 75 - 100 µg L-T4/Tag gestartet werden, bei älteren Menschen und Patienten mit kardiovaskulären Erkrankungen ist ein vorsichtiger Beginn sinnvoll (25 - 50 µg). Frauen, die unter Substitution mit L-T4 schwanger werden sollten die Dosis um 25 % erhöhen sobald die Schwangerschaft diagnostiziert wird. Zum Überwachen der Substitution reicht in der Regel das TSH, das im Bereich von 0.5 - 2.5 liegen sollte.

Kurzfassung: Der Artikel befasst sich mit der Kritik an dem NATO Einsatz Unified Protector in Libyen im Sommer 2011. Er setzt sich mit den fünf hauptsächlichen Argumenten der Kritiker auseinander. Diese sind: (1) Der Konflikt zwischen Bürgern und Regierung sei von außen induziert worden, (2) das Gaddafi Regime hätte den Konflikt politisch oder militärisch lösen können, wenn es nicht zur Intervention der NATO gekommen wäre, (3) das Gaddafi-Regime habe keine Verbrechen gegen die Menschlichkeit begangen, tatsächlich sei es das Ziel der NATO gewesen Gaddafi zu stürzen; (4) die NATO habe mit den Luftangriffe das Land zerstört und Tausende Bürger getötet, und (5) die NATO habe keinen Plan für die Zeit nach Gaddafi gehabt und habe daher das Land lediglich ins Chaos gestürzt. Keines dieser Argumente hält einer kritischen Prüfung stand.

Zusammenfassung: Gegenstand und Ziel: Dieser Artikel gibt eine Literaturübersicht über Vorratsmilben und ihre Bedeutung in der Tiermedizin. Weiterhin präsentiert er Ergebnisse aus einer eigenen Studie, deren Ziel es war herauszufinden, ob Vorratsmilben im Trockenfutter oder in der direkten Umgebung von Hunden vorkommen. Material und Methode: Im ersten Teil der Studie wurden 23 Hundefuttersäcke über einen Zeitraum von sechs Wochen mittels mikroskopischer Untersuchung auf eine Kontamination mit Vorratsmilben überprüft. Im zweiten Teil der Studie erfolgte eine Untersuchung von Staubproben aus 20 unterschiedlichen Haushalten mit gesunden Hunden auf eine Kontamination mit Vorratsmilben. Ergebnisse: In keiner Futterprobe fanden sich Vorratsmilben. In fünf der insgesamt 40 untersuchten Staubproben waren Milben verschiedener Spezies nachweisbar: Dermatophagoides pteronyssinus (4/40), kurzschwänzige Demodex-Milbe (1/40) und Vorratsmilbe (1/40). Schlussfolgerung: Die Ergebnisse lassen vermuten, dass Vorratsmilben kommerzielles Hundetrockenfutter nicht kontaminieren, aber im Hausstaub vorkommen können. Klinische Relevanz: Bei den meisten Hunden mit positiven Serum- oder Hauttestreaktionen gegen Futtermilben wird eine Änderung der Fütterung keinen Einfluss auf die klinische Symptomatik haben.

Zusammenfassung Hintergrund Bei etwa jedem dritten Patienten mit einem penilen Plattenepithelkarzinom (PSCC) gilt die Infektion mit Humanen Papillomaviren (HPV) als entscheidender Faktor der Karzinogenese. Erste Daten zur optimierten Risikostratifizierung und Therapie anhand des HPV-Status existieren; somit hat der Nachweis einer HPV-Genese möglicherweise mehr als lediglich eine epidemiologische Dimension. Referenz zur Detektion von HPV ist die aufwendige Polymerase-Kettenreaktion (PCR). Unklar ist, ob Alternativen wie die In-situ-Hybridisierung (ISH) oder verschiedene Surrogatmarker (immunhistochemischer Nachweis einer p16INK4a-Expression, histologischer Subtyp, Tumorinvasionsfront, Koilozytose), zur Bestimmung des HPV-Status ausreichend sind. Methoden In dieser unizentrischen Studie an 34 Patienten mit PSCC wurden eine Multiplex nested-PCR zur Festlegung des HPV-Status und Genotypisierung der HPV-DNA sowie eine ISH durchgeführt. Diverse histologische Kriterien und p16INK4a wurden durch zentralen Review bestimmt. Der Einfluss verschiedener Kriterien auf die karzinomspezifische Mortalität (CSM) wurde mit Cox-Regressionsmodellen geprüft (FU: 92 Mon.). Ferner erfolgte der Vergleich der diskriminativen Qualitäten verschiedener Tumorinvasionsmuster (u. a. TNM-Klassifikation 7. vs. 8. Auflage) hinsichtlich der CSM-Prädiktion. Ergebnisse Gemäß PCR-Analyse wiesen 26 % der Patienten HR-HPV auf (n = 9). ISH und die untersuchten histologischen Kriterien zeigten eine unzureichende Vorhersagegenauigkeit des HPV-Status (p > 0,3). p16INK4a erreichte in den Testkriterien Sensitivität, Spezifität, positiv-prädiktiver und negativ-prädiktiver Wert sowie Gesamtübereinstimmung 66,7 %, 84 %, 60 %, 87,5 % bzw. 79,4 % (AUC: 0,753; p = 0,026). Keines der Untersuchungskriterien (inkl. PCR) korrelierte signifikant mit der CSM. Im Vergleich zur 7. wies die 8. Auflage der pT-Klassifikation eine bessere CSM-Prädiktion auf (C-Indizes 70,2 % vs. 72,9 %). In Addition zur Schwellkörper-Invasion besaß die Urethra-Infiltration keinen eigenständigen prädiktiven Wert. Eine durch uns vorgeschlagene Neu-Gruppierung der Schwellkörper-Invasionsmuster erzielte eine Diskriminationssteigerung bzgl. der CSM (C-Index 77,9 %). Schlussfolgerung p16INK4a erlaubt – im Gegensatz zur ISH und den untersuchten histologischen Kriterien – eine akzeptable Vorhersage des HPV-Status. Der Nachweis von HPV-DNA inkl. seiner Surrogatmarker hatte keinen Einfluss auf die CSM. Die 8. Auflage der pT-Klassifikation scheint prognostisch sinnvoll, da die urethrale Tumorinvasion keinen eigenständigen Einfluss auf die CSM aufweist.
