Abstract:
Background: Emergency nurses (ENs) often care for patients nearing the end of their lives or with life-limiting illnesses. However, ENs are hesitant to initiate palliative or end-of-life (PEOL) discussions because of a lack of comfort with these topics. Many ENs have no formal PEOL communication training which contributes to the lack of comfort with PEOL discussions in the emergency department (ED). Thus, the purpose of this quality improvement project was to determine how PEOL communication training affected rural ENs perceived comfort level during PEOL conversations.

It holds an important aspect in law enforcement since it sees what will benefit people the most as the main discussion. Abstract:
AbstractIn this chapter, I evaluate from a philosophical perspective the ongoing discussions in the Belgian civil society triggered by the euthanasia law and its implementation in end-of-life care since 2002. I challenge the idea that the so-called normalization of euthanasia is an established fact and I contend that the ongoing discussions on the possible abuses of the law and the practice of euthanasia are unavoidable. I see three reasons for this. First, in contrast with what some think, euthanasia can never become a “normal” therapeutic option, that could be integrated in standard medical practice. Euthanasia is, by its very nature, a transgression of a fundamental moral taboo and will thus always, however liberal the law might be, challenge the conscience of some physicians. Secondly, because of its transgressive meaning, every act of euthanasia can always be contested by family members or the larger society. This is the case when euthanasia is given to patients who are not terminally ill (such as psychiatric patients), but also when a physician performs euthanasia in an undignified and negligent way. Thirdly, the very existence of the law puts pressure not only on patients and physicians but also the larger society. By making euthanasia into a symbol of the good death, a whole society loses its sensitivity for the intrinsic transgressive nature of euthanasia and creates all sorts of strategies to mask the wavering of conscience that results from this collective negligence. There is no way, so I conclude, to avoid this. The wavering of conscience will continue to haunt the end-of-life care in Belgium as long as the law on euthanasia remains as it is.

Abstract:
This chapter discusses the planning for end-of-life care. Results of a 2017 survey show that older adults are likelier than younger people to have documented their end-of-life wishes. Yet few adults with or without written documents have discussed end-of-life care with their doctors. Age, education, and discussions about death were predictors of having such a plan. Those with a college education or higher were most likely to have a written document, compared to those with some college, high school, or less education. Also, talking about death while growing up correlated with having written documents about wishes for medical care. Care settings encouraging or mandating doctors to use an advanced directive toolkit, such as the one created by the National Physician Orders for Life-Sustaining Treatment Paradigm, can improve the communication gap with providers and help seriously ill patients better express their wishes for end-of-life care.

This chapter ties up many of the themes of the book. It looks back to the discussions of luck by discussing the various uses of ‘odd’ or ‘odds’ in the book, where it especially figures outside the luck chapters in the chapter on the messenger as the third party, the odd man. This leads to a discussion of the collocation of ‘odds and ends’ and then to three stories that are odds and ends. The first deals with two six-year-old girls on their first day of school who engage in a verbal fight to the death that rather brilliantly deals with Austin’s performatives better than most academic literature. The second is a tale of the author as a father to a woebegone nine-year-old son who discerned that day that life has no point as he sobbed uncontrollably, which leads to a brief discussion of Gloucester in Lear and a powerful Middle English lyric and the pointlessness of everything. The third story is short and is a tale of what the author is left to at his end: rereading and being discovered by his daughter to be both knave and fool.

Abstract:
In Chapter 3 I enter the classroom with the teachers and students. This chapter presents an ‘ethnography’ of two different classes, one a Qur’an class and the other a class of Qur’anic exegesis for advanced students. We also hear discussions about the importance of taharat or ritual purity. We see how students and teachers interact, and how adab guides their relationship. The chapter shows how teachers skillfully present the material in a way that students find meaningful. It also discusses the role of memorization and peer learning in madrasa education. An appendix of the madrasa syllabus at the end of the chapter allows me to highlight the commonalities between ‘traditionalist’ Barelwis and Deobandis/Tablighis.
