Abstract:
The biomechanical contribution of the interfragmentary Kirschner wire as a component of composite fixation for acetabular fracture repair was subjectively and objectively evaluated. Acetabular osteotomies were repaired using the screw/wire/polymethylmethacrylate (SWP) composite fixation with or without one of three configurations of Kirschner wire in 32 hemipelves obtained from 16 dogs. Reduction, assessed objectively and subjectively, was unaffected by Kirschner wire placement. Hemipelves repaired with Kirschner wire(s) were subjectively more stable prior to application of polymethylmethacrylate when manually assessed in multiple planes. Consistent incremental increases in stiffness, yield load, and maximum load sustained that were observed during biomechanical testing were not significant, with the exception that hemipelves repaired with two Kirschner wires had significantly greater yield loads than hemipelves repaired without Kirschner wires. The subjective results of this study support the use of at least one interfragmentary Kirschner wire to maintain reduction prior to polymethylmethacrylate application; however, fracture configuration and location may dictate the number and pattern of interfragmentary Kirschner wires used to maintain reduction prior to application of the polymethylmethacrylate. The objective results and observations made during biomechanical testing suggest that use of one or more interfragmentary Kirschner wires may enhance stability after polymethylmethacrylate application.

Introduction: Fractures of the patella constitute almost 1% of all skeletal injuries, resulting from either direct or indirect trauma. Many forms of internal fixation for patellar fractures have been described in the literature but perfect anatomical reduction during surgery has an excellent outcome irrespective of the method of fixation used. The conventional method of patellar tension band wiring is always done with the help of two parallel Kirschner wires. Here, in our study, we intended to see whether the use of 3 parallel Kirschner wires in comparison to conventional one results in a superior functional outcome. Methods: A prospective observational study was carried out in ANIIMS and GB Pant hospital Port Blair from November 2016 to November 2019 on 44 patients of patellar fracture operated by tension band wiring with 22 patients in 2 parallel Kirschner wires and 3 parallel Kirschner wires groups respectively. Results: 44 patients were followed up postoperatively for 1 year and assessed by Lysholm knee score for pain status and working status and other variables. The mean age of the participants was 40.27 years (41.86 in two Kirschner wire group and 38.68 in three Kirschner wire group) in our study. Lysholm knee score, pain status, or working status was not significant throughout the mean follow-up periods for both groups. Conclusions: We found no significant difference between patellar TBW in 3 Kirschner wire and 2 Kirschner wire groups. However small sample size limits our study.

SummaryA Kirschner-Ehmer device was used to stabilize caudal lumbar fractures/luxations in three dogs weighing 12 kg or less. A through and through Kirschner-Ehmer device maintained alignment during the healing process using the appropriate sized rods and clamps. Postoperative management included strict cage confinement and oral broad spectrum systemic antibiotics for up to two weeks after removal of the Kirschner-Ehmer device. In all three cases, the fractures/luxations were healed within six to eight weeks. The Kirschner-Ehmer device could then be removed with sedation or general anaesthesia.

Kirschner wire is frequently used in orthopedic surgery, and migration is not exceptional. Intrathoracic migration is well-known, especially after surgery on the shoulder girdle, however, intratracheal migration is extremely rare. We describe a case of intratracheal migration of two Kirschner wires in a 41-year-old man who had them placed two years previously for a right clavicle fracture. He experienced chest pain followed by hemoptysis. Thoracic computed tomography revealed intratracheal migration. Remove of the Kirschner wires was performed by a cervical-sternotomy approach. Prompt removal of migrated Kirschner wires must be carried out urgently to avoid fatal complications.

Abstract:
Fusion of an interphalangeal joint of a lesser toe is a commonly used procedure for addressing interphalangeal joint deformities such as a hammer or a claw toe. Fusion can be achieved by insertion of an intramedullary Kirschner wire in a retrograde manner. Deviation of the Kirschner wire from the intramedullary canal into the surrounding soft tissues is common. This can render the fusion unstable and can cause painful soft-tissue irritation and early Kirschner wire loosening, resulting in an unstable nonunion with recurrence of deformity. We describe a simple and reproducible technique to assist with optimal intramedullary placement of the Kirschner wire, thereby reducing the risk of complications after interphalangeal joint fusion of a toe.

An intramedullary Kirschner wire was combined with an intra-osseous wire loop for fixing finger fractures. The mechanical stability of this technique to resist a bending moment in the sagittal plane was compared with four other conventional methods: single loop, loop and an oblique Kirschner wire, crossed Kirschner wires and dorsal plating. It was found that when the applied force was small, the technique was much more stable than the crossed Kirschner wires and was similar to the others. The ability to withstand mechanical failure was much better than most of the methods except the dorsal plate. This technique is recommended for replantations, transverse fractures and short oblique fractures with little comminution.

Kontrollröntgen során fedezték fel, hogy a tűződrótok a jobb mellüregbe vándoroltak. Videothoracoscopos úton technikailag nem lehetett őket eltávolítani, ezért jobb oldali thoracotomia történt. Pneumolysist követően a 2-es tüdősegmentumból és a kupolánál, medialisan a gerinccsatornába fúródó Kirschner-drótot távolítottak el. A posztoperatív időszakban sebészeti komplikáció nem volt. A szerzők áttekintik ennek a ritkán előforduló szövődménynek az irodalmát és felhívják a figyelmet, hogy a mellüregbe vándorolt tűződrótok eltávolítása szükséges a fenyegető súlyos szövődmények megelőzésére.

Abstract:
This study advances design and manufacturing of the tip geometry of the Kirschner wire (K-wire) — a popular orthopedic drilling tool with a sharp, three-sided trocar point — for improved bone drilling efficiency. Since there is no active cutting edge as there is for twist drill bits, bone drilling forces can be so large as to cause excessive heat generation and tool wear. Our modified K-wire consists of three slots at the tip to enhance cutting and chip (bone debris) evacuation. A finite element model was developed to simulate bone debris generation, thus predicting the improvement by the slot design. An experimental study was conducted to validate the model and ascertain actual drilling performance. During 5 mm drilling the maximum thrust force and torque were both reduced on average by 30–40% based on the experimental data. The overall trends of both force and torque agree with the model-predicted results.

Abstract:
Seligson [1] describes how Hoffmann and Jaquet, a medical doctor and an engineer, respectively, developed the original Hoffmann fixator as a tool to stabilize human fractures with minimal invasiveness. Whether being utilized in mass trauma injury situations such as the 2010 Haitian earthquake, within our emerging geriatric population, or in veterinary applications, external fixation is widely used [1–4]. In this investigation, a rod-to-wire coupling, shown in Figure 1, and hereafter referred to as the R2W clamp, has been designed and validation tested for Stryker Orthopaedic’s Hoffmann II (HII) External Fixation System. As the name implies, this clamp has the purpose of connecting 8mm rods to 1.5mm or 2mm Kirschner (k-) wires or olive wires to stabilize bony fragments in the lower extremity, thus expediting healing in a trauma case. This paper summarizes the results of the validation tests conducted on prototype clamps. This clamp effectively allows placement of a wire to further stabilize a frame [3] by allowing wire placement without the addition of an intermediate ring, as shown in Figure 2. The wire could be added to any configuration with two parallel rods extending in plane with the bone. As shown in Figure 3, the R2W clamp can be positioned “outboard” with the rod between it and the bone, or “inboard” between the rod and the bone, allowing the surgeon geometric flexibility. The use of two k-wires is recommended to stabilize each bone fragment [5]. One of the goals of the validation testing was to determine the effectiveness and functional safety of the clamp as related to surgically applied k-wire tensions of either 50 kg or 100 kg. Since it is feasible that surgeons may tighten, loosen, then retighten the clamp while positioning it during surgery, the effects of clamp retightenings on the performance of the R2W clamp were also evaluated [4].

Abstract:
Introdução - O conceito de “osteossíntese biológica” surgiu atrelada a evolução da maneira como se enxergava o tratamento cirúrgico das fraturas, priorizando não apenas os fatores mecânicos, mas promovendo também as condições biológicas necessárias para a regeneração óssea. Objetivo - Devido a uma menor taxa de recuperação, e um número relativamente alto de complicações, técnicas do tipo ORIF (Open Reduction Internal Fixation) passaram a ser repensadas, quando possível, no que se diz respeito a conservação e integridade dos vasos que nutrem o osso, preservação do periósteo e dos tecidos moles adjacentes, tendo em vista que estes são fundamentais para o processo de regeneração óssea e consolidação das fraturas. Desta maneira, se torna fundamental a elaboração de novas pesquisas relacionadas ao assunto, para que tal conhecimento seja fundamentado na rotina de mais profissionais que atuam na ortopedia. Métodos - Diversas técnicas de osteossíntese minimamente invasivas são empregadas na rotina de muitos hospitais e clinicas no mundo inteiro, dentre elas destacam-se; a MIPO (Minimally Invasive Plate Osteosynthesis) que consiste na colocação de placa e parafusos sobre o osso, de forma epiperiosteal, sem expor o foco da fratura por meio de dois acessos (um proximal e um distal ao foco da fratura), e a MINO (Minimally Invasive Nail Osteosynthesis) onde é feita a colocação de uma haste bloqueada por de meio de um único acesso proximal, além de dois pequenos acessos para a colocação dos parafusos ou bolts. Além disso, uma das técnicas mais utilizadas é a fixação esquelética externa, que consiste na transfixação dos fragmentos ósseos por pinos de steinmann, ou por fios de kirschner, presos por uma, ou mais, hastes que promovem a estabilidade sem a necessidade de expor o foco da fratura. Resultados - A tomada de decisão, no que se diz respeito a escolha por uma técnica de osteossíntese minimante invasiva ou uma técnica aberta do tipo ORIF, deve sempre levar em consideração fatores importantes como: raça, peso, porte, doenças preexistentes, cuidados do tutor, assim como o tipo de implante e a habilidade do cirurgião para realizar tal técnica, entre outros. As técnicas minimamente invasivas visam reduzir o trauma cirúrgico, causando o menor distúrbio possível ao suprimento sanguíneo, o que melhora o potencial de consolidação, por alcançar um equilíbrio entre fatores mecânicos e biológicos. Por conta de o acesso ser fechado, as cirurgias minimante invasivas proporcionam menor dano aos tecidos moles adjacentes, preservando o potencial osteogênico do hematoma causado pela fratura, reduzindo o tempo cirúrgico, além de reduzir as chances de infeção pós-operatória. Deve-se sempre buscar o comprimento e alinhamento corretos do membro, para que não haja prejuízos no retorno a função normal. Conclusão - Sendo assim, conclui-se que a atualização de técnicas de osteossíntese minimamente invasivas oferece vantagens no que se diz respeito a consolidação de fraturas, sendo necessária a difusão em larga escala dessas técnicas, visando melhores resultados dentro das cirurgias ortopédicas. Abstract:
Fraturas de tarso e metatarso em gatos constituem uma das importantes urgências da rotina cirúrgica ortopédica veterinária, sendo as de tarso mais raras. O maior problema causado por lesões dessas regiões é a perda da capacidade de sustentação do peso corporal e consequente claudicação. A maioria dos casos derivam de traumas diretos resultantes da ação da força sobre o osso, ou de lesões resultantes de hiperextensões, nos casos de luxações. Fraturas metatársicas são classificadas de acordo com sua localização. O objetivo deste relato é abordar um caso de artrodese realizada em um felino atendido no Hospital Veterinário Universitário Prof. Ivon Macêdo Tabosa da Universidade Federal de Campina Grande, campus de Patos/PB. Para a metodologia de elaboração deste trabalho foram analisados dados das fichas clínicas, exames laboratoriais, laudos radiográficos, fichas do exame ortopédico e da descrição cirúrgica. O relato em questão trata-se de um paciente felino de 1 ano, SRD, macho, com 3,7 kg, que chegou para o atendimento clínico com o histórico de que há cerca de uma semana havia saído de casa e ao retornar se apresentava “mancando”. Durante o exame físico o animal se apresentou ativo, em estação e alerta, os parâmetros cardiorrespiratórios sem alterações. O exame ortopédico do paciente mostrou desvio no membro pélvico direito e uma pequena fístula, e no exame de locomoção verificou-se claudicação do mesmo membro. Após o exame físico foram solicitados exames de hemograma e imagem (radiografia simples) nas projeções médiolateral e craniocaudal. No laudo radiográfico: fratura completa transversa em diáfise proximal dos ossos metatársicos dos dígitos II ao V do membro pélvico direito, com o aumento da radiopacidade em torno das articulações metatársicas dos respectivos dígitos. Para a cirurgia realizou-se: incisão cutânea lateral entre tarso e metatarso, seguida pela divulsão romba subcutânea e dissecação muscular para exposição do calcâneo, dos ossos do tarso e metatarso. Após a exposição a fratura foi reduzida pela compressão manual e instalação de pinos de Kirschner de 1 mm cruzados.Para estabilização temporária. Após a implantação dos pinos a placa de bloqueio cônico por acoplamento micro Fixin 1.7 mm foi instalada escolhida por ser um implante versátil, pois permitiu a utilização de 3 parafusos de 1,7 mm no calcâneo e 3 de 1,3 mm nos metatarsos. Antes da implantação da placa houve a drilagem da articulação com broca para fazer o desgaste das articulações que prosseguiu com a colocação de enxerto ósseo oriundo da tíbia proximal, coletado por meio da perfuração com broca de 2.0 mm. Posteriormente, enxertia, miorrafia, redução do espaço subcutâneo ambos com fio ácido poliglicólico 3.0, padrão “x” e “vai e vem”, respectivamente, e dermorrafia com nylon 3.0. Após a realização da artrodese por meio da placa de bloqueio cônico por acoplamento empregados para o tratamento do paciente, foram obtidos resultados satisfatórios, com boa consolidação e aceitação biológica do enxerto, com a locomoção anatômica devolvida ao paciente. Abstract:
Az elmúlt évtizedben már számos kutatás bizonyította, hogy a pedagógusok megváltozott feladatai és szerepei, a módszertani kultúra és a komplex szemlélet a digitális kompetencia fejlesztésével párhuzamosan megjelenik (Fehér, 1999; Kirschner-Woperies, 2003; Law, Chow és Yuen, 2005). A tanítási-tanulási folyamat résztvevőiként a pedagógusok is részesei a rendkívüli iramban változó lehetőségeknek, melyek az iskola különböző „valós és virtuális” terein egyre inkább biztosítottá válnak (UNESCO, 2011). 2020 márciusában 6 a koronavírus-járvány miatt bevezetett digitális munkarend által – korábban még nem tapasztalt módon – aktuálissá vált mindaz, amit a „távoktatás”, „digitális oktatás”, „otthontanulás” és szinonimáik képviseltek. A pedagógusok, diákok és szülők egyik napról a másikra kerültek át az osztálytermi környezetből a digitális platformok világába, megváltoztatva szerepeiket, feladataikat és kompetenciahatáraikat. Felmérésünk azzal a céllal készült, hogy feltárja a tanítók hogyan igyekeztek megtalálni a tantárgyi specialitásnak, informatikai tudásuknak, rendelkezésre álló eszközeiknek, a diákoknak és a szülőknek leginkább adekvát megoldásokat. Kutatásunkban kvantitatív módszert alkalmazva, online kérdőíves vizsgálattal mértük fel az 1-6. osztályban tanító pedagógusok véleményét és gyakorlatát a digitális munkarendben történő oktatással kapcsolatban (N=71). Előadásunk a tanítók által alkalmazott karanténpedagógiára vonatkozó öszszefüggésekre világít rá a következő aspektusokból: eszközök, internet a távoktatásban, a pedagógusok digitális kompetenciája, a tananyagok küldésének felülete, kapcsolattartás a tanulókkal és szülőkkel. Fókuszba kerültek az alkalmazott digitális platformok és eszközök, azzal a céllal, hogy feltárjuk az elmúlt időszakban aktuális alsó tagozatos oktatás és nevelés főbb aspektusait. A digitális munkarend, valamint az online eszközök kiválasztásának és kipróbálásának tanítói tapasztalatait is vizsgáltuk az önértékelésen alapuló tanítói digitális kompetenciák feltárása mellett. A kapott leíró statisztikai elemzések megerősítik, hogy a digitális munkarenddel kialakuló távolléti oktatási struktúra rendszerezettségében, illetve következetes használatában plasztikus eltérések jelentkeztek. Példaként említhető a kapcsolattartási mintázatok egyenetlensége és a formális (Kréta)/informális kapcsolattartási struktúra (Messenger/Facebook csoport) használatának eltérő megoszlása. Konkrét számadatokkal mérve a szülőkkel való kapcsolattartásban kiemelkedett az egyéb lehetőségek mellett a Facebook csoport (59%), és a Messenger (53%) használata a formális platformként említett Krétával (22%) szemben. Összefoglalva, az eredmények hozzájárulhatnak az általános iskolákban működő online oktatás gyakorlatának megismeréséhez, a pedagógiai praxis lehetőségeinek bővítéséhez, ugyanakkor megerősítjük Lévai (2016) felvetését is, miszerint a pedagógusok felelőssége számottevő és árulkodó abból a szempontból is, hogy mit mutatnak róluk digitális lábnyomaik, illetve az online elérhető felületek. ---- Lower-primary-school teachers’ digital educational experience: possibilities in lockdown induced pedagogy ----- The past couple decades have seen a parallel change in teachers’ tasks and roles, methodologies, educational approaches and the development of their digital competences (Fehér, 2009; KirschnerWoperies, 2003; Law, Chow and Yuen, 2005). Also teachers as participants of the teaching-learning process experience the rapid evolution of the possibilities in the real-life and virtual platforms of school (UNESCO, 2011). In March, 2020 unprecedentedly digital education was launched due to the coronavirus pandemic and “distance education”, “digital education”, “home schooling” (at present regarded as synonyms) had become reality. All at once, teachers, students and parents were transmitted from real-life classrooms into the world of virtual platforms, which modified their roles, tasks and competence boundaries. Our survey was conducted to explore how lower-primary-school teachers experimented to find the most adequate solutions related to the issues of their teaching areas and digital knowledge. Also, we aimed to detect the most characteristic challenges that the respondent teachers had to face during the lockdown period online education processes. Our quantitative research was conducted to collect data via an online questionnaire to elicit beliefs and practices of lower-primary-school teachers of 1-6-forms regarding their work during lockdown digital education (N=71). The main focus of our research was the interrelationships of lockdown induced pedagogy from the following aspects: tools in digital education, teachers’ digital competence (based on self-assessment), the platforms of sharing teaching contents and of contacting students and parents. The focal points of our study were the applied digital platforms and tools and to gain insights into the teachers’ digital competences based on their self-evaluation. We also explored the respondent teachers’ choices and experience regarding the online tools of digital education. The results of descriptive statistics showed that there were differences in the structure and consistent use of digital education during the lockdown. As an example, the distribution of the formal Kréta platform and other informal ones (Messenger/Facebook group) is uneven in terms of contact between teachers and parents. Also, the Facebook group (59%) and Messenger (53%) platforms outnumber the use of Kréta (22%) during teacher-parent communication. In sum, our results can contribute to a better understanding of the practice of online education at primary-schools, and also to the extension of the teaching repertoire in 1-6 forms. These findings are also compatible with Lévai’s (2016) results, that is teachers’ digital footsteps and the online platforms that they use can reveal a lot about them, which is a great professional responsibility.
