Abstract:
Ksharsutra is a Medicated seton which is made by coating the Barbour thread with 21 coatings of Kshara i.e. an herbal alkaline powder, turmeric and latex of Euphorbia nerrifolia. Ksharsutra application is a minimal invasive well established procedure in management of fistula-in-ano. Research on Ksharsutra started since 1968 and it is being used till date with high success rate. In this case report the patient was asymptomatic before 15 years but then a boil developed at perianal region which spontaneously bursted and pus discharge was seen from the boil. Patient was diagnosed with Grade 5 St. James’s university hospital classification of perianal fistulae. This was a case of high anal horse shoe fistula. In this case Fistulectomy or Fistulotomy would cause incontinence. So in this case the Ksharsutra was used but with a modification of classical technique called as IFTAK (Interception of fistulous track with application of Ksharsutra) technique. In which a window was created at 6o’ clock and intersphincteric tract was identified and in that tract Ksharsutra was placed. This technique cures such types of complex Fistula in ano with minimal tissue damage and duration of healing is also reduced.

Abstract:
<strong>Background:</strong>&nbsp;Fistula in ano presents a significant clinical challenge due to its recurrent nature and impact on quality of life. Traditional surgical treatments include fistulectomy and less invasive procedures such as the Ksharsutra technique, but recurrence and postoperative complications remain concerns.&nbsp;<strong>Objective:</strong>&nbsp;This study aims to evaluate and compare the efficacy, safety, and patient satisfaction of three surgical techniques for treating fistula in ano: fistulectomy, Ksharsutra, and LIFT (ligation of intersphincteric fistulous tract).&nbsp;<strong>Method:</strong>&nbsp;Ninety patients diagnosed with fistula in ano were randomized into three groups: Group A (fistulectomy), Group B (LIFT), and Group C (Ksharsutra). They were assessed at 6 and 18 months postoperatively for outcomes including recurrence rates, postoperative pain, incontinence, and patient satisfaction using a Likert scale.&nbsp;<strong>Results:</strong>&nbsp;The LIFT procedure (Group B) demonstrated superior outcomes across all measured parameters. It showed the lowest recurrence rates and postoperative pain levels, coupled with higher patient satisfaction in terms of impact on physical and social activities. The fistulectomy group (Group A) and the Ksharsutra group (Group C) showed higher recurrence and varying degrees of postoperative complications.&nbsp;<strong>Conclusion:</strong>&nbsp;The LIFT procedure offers a significant advantage over fistulectomy and Ksharsutra in the treatment of fistula in ano, providing better clinical outcomes with fewer complications and enhanced patient satisfaction. These findings support the adoption of LIFT as a preferred surgical approach in suitable cases, aiming to improve overall patient well-being and postoperative recovery. &nbsp; &nbsp;

Abstract:
Management of fistula is one the most notorious job for the surgeon because of its ill site, chronicity, and recurrences. Treatment modalities like anal fistula plug, mucosal flap advancement, fibrin glue, and VAAFT (Video Assisted Anal Fistula Treatment) are there but ksharsutra has edge on them for its success rate and non-recurrence. Even after its better results, weekly sitting and pain during changing of ksharsutra and discomfort of ksharsutra being in situ makes it difficult for patient to pursue the treatment. Hence, a new approach was mandatory. Considering fistula to be a dushtavrana, shashti upakrama like lekhan, shodhan and ropan were applied in a patient of high-anal transphincteric fistula. Partial fistulectomy, excessive curettage (lekhan) from its internal opening was done. Internal opening was closed and wound was allowed to heal by second intension with the help of shodhan and ropan tail. Post-operative antibiotics and anti-inflammatory were given. It was observed that wound healed at good rate without discomfort of patient in 25 days. It underlines importance of shashtiupkrama in modern age of surgery also.

Abstract:
There are several anorectal disorders facing by large number of population with diverse group of pathologic conditions. Among them anorectal disorders like Arsha (haemorrhoids), Parikartika (fissure) and Bhagandar (fistulas) are among the most common anorectal conditions. These conditions are the cause of significant discomfort and disability to the patient and can impair a person’s quality of life. Ayurveda is playing important role and contributing to modern technology with its own originality with authenticity. Advantage of Ksharsutra therapy in the management of Arsha is that it is a minimally invasive surgical procedure and can be performed under local anaesthesia and have good results in wound healing. In the presented study, Ksharsutra ligation was performed on a patient aged 23 years suffering from Arsha on 3 ‘o’ clock position prolapsed without bleeding but with irritation, pruritis ani and mild mucous discharge. Ksharsutra has been found to be effective, economic, easy to prepare and use. Moreover the daily routine of the patient is not disturbed. As this study performed on a single patient, more such trials are required to reach concrete conclusions.

Abstract:
Fistula in Ano is a chronic inflammatory condition having a tubular structure with an opening in the Anorectal canal at one end and the surface of the perineum or perianal skin on the other end. Fistula-in ano is one such condition that is prone to recurrence even after repeated surgery and can be effectively treated by Ayurvedic medicines. Ksharasutra is one of the chief modalities in the treatment of Bhagandara in Ayurvedic science. Here is a case report of 31 years male patient with chief complaints of pain and pus discharge from the perianal region presented to Shalya OPD on Oct 14, 2022. After his consent, Ksharsutra treatment was planned with Saptaparna Snuhi Ksharsutra, along with some oral medications. The treatment continued for a period of 1 month, and the patient was kept on follow-up for 6 months having no reoccurrence or any other complaints.

Fistula in ano is an anorectal disorder in which two openings are present. One is located near perianal re-gion and another is at anal canol or rectum. Fistula in ano is itself a challenging disease for patients because of mortality is not associated but daily routine life of patients become crushed. Ayurveda also said about Bhagandar (fistula in ano) that it is Kashta Shadhya Vyadhi i.e. difficult to treat because its take time and recurrence rate is also associated. But Ayurveda has described the Kshar sutra therapy for the treatment of fistula in ano which have high success rate, but time taken. With time, revolution has also come in Ayurve-dic treatment modalities for anorectal fistulas. A modified Ksharsutra technique is new technique for treatment of complex fistula in ano. The name of this technique is interception of fistulous tract and appli-cation of Ksharsutra (IFTAK). This technique has high success rate with minimal complication, early re-covery and there is no recurrence is associated. Cosmetically, this technique is good.

Ulcer and wound managements have prime importance in the eld of surgical practice, it continuous as a challenge from ancient to modern era due to its diversity in onset and management hurdles. Healing is a mechanism where the body attempts to regain the integrity of injured part. Many factors like systemic illness, malnutrition, vascular insuffciency may delay the process of healing. In Ayurveda many procedures and different formulations are enumerated for Vrana management by our Acharyas. In Sushruta Samhita Acharya elaborated about Vrana, Dushtavrana and its 60 types of treatment modalities. Use of ksharsutra is a unique parasurgical procedure one among them and it works as potent debriding agent enhances formation of healthy granulation tissue and proper healing. Ksharsutra application is one of the most effective treatments in ayurveda. The numerous qualities of kshar, such as chedan, bhedan, lekhana, ropan, and pachan, helps in simultaneous track cutting and healing.A case report of 68 year old man presenting with non- healing ulcers at 2nd finger of left toe and greater toe with pus discharge ,unpleasant smell ,inflammation has been presented here.
