A combination of IR spectroscopy, scanning electron microscopy (SEM) and powder X-ray diffraction has been used to analyze the compositional and architectural variation across the different parts (core, middle and outer layers) of five human urinary calculi (KS1–KS5) from eastern India. Rietveld quantitative phase analysis using X-ray powder diffraction revealed that the composition of the core regions in KS1–KS3 and KS5 is exclusively whewellite, whereas in KS4 it is a mixture of whewellite (84.5 wt%) and carbonated hydroxyapatite (15.5 wt%). While one of the renal stones, KS1, is composed of only whewellite in all three regions, a distinct variation in phase composition from the core towards the periphery has been observed in KS2–KS5. A drastic change in phase composition has been noted in KS5, with the major constituent phases in the core, middle and outer layers as whewellite (100.0 wt%), anhydrous uric acid (60.7 wt%) and carbonated hydroxyapatite (69.6 wt%), respectively. The crystallite size of whewellite in different parts of the kidney stones varies between 91 (1) and 167 (1) nm, while the corresponding sizes of the anhydrous uric acid in KS5 and carbonated hydroxyapatite in KS3 are 107 (1) and 18 (1)–20 (1) nm, respectively. SEM images of the kidney stones showed different levels of organization, resulting from an agglomeration of crystallites with diverse shapes and sizes.

Karsinoma sel skuamosa (KSS) adalah tumor ganas yang berasal dari sel epitel skuamosa dan merupakansalah satu jenis kanker ditemukan pada mukosa rongga mulut. Insidensi KSS sekitar 90% dari seluruh jeniskeganasan yang terdapat pada rongga mulut dan frekuensi KSS rongga mulut di Indonesia mencapai 3-5%dari seluruh kanker organ tubuh lainnya. KSS dapat diklasifikasi berdasarkan morfologi karakteristikjaringannya, dimana dibagi kepada tiga jenis yaitu KSS berdiferensiasi baik, sedang dan buruk. Tujuanpenelitian ini adalah untuk melihat karakteristik morfologi KSS rongga mulut berdasarkan jenisdiferensiasinya. Rancangan penelitian ini merupakan deskriptif dengan cara cross sectional terhadap 30sampel blok parafin yang terdiagnosa sebagai KSS rongga mulut yang diperoleh dari Laboratorium PatologiAnatomi FK USU/RSUP Haji Adam Malik Medan tahun 2009-2013, dan dilakukan pewarnaanHematoxylin-Eosin (HE). Preparat diamati di bawah mikroskop cahaya Olympus CX21. Hasil penelitianmenunjukkan KSS berdiferensiasi baik (63%), KSS berdiferensiasi sedang (37%), dan tidak ditemukan KSSberdiferensiasi buruk. Kesimpulan penelitian ini menunjukkan KSS berdiferensiasi baik lebih banyakdibandingkan KSS berdiferensiasi sedang, dan tidak ditemukan KSS berdiferensiasi buruk.

The Initiative of establishing Information Technology (IT) and Community Service Centers, later renamed Knowledge Stations (KSs) was launched in 2001. The KSs initiative is intended to implement IT in local communities (LCs) and remote areas in preparation for the E-Government process. This study develops a model that explores KSs’ role as a partnership in E-Government readiness in Jordan through answering the following two questions: why is a clearly comprehensive role of KSs needed for the readiness of E-Government in Jordan? How does this role take place practically? The research methodology is the case study that was applied to six KSs as a purposive sample in Amman, the capital of Jordan. Nine semi-structured interviews have been conducted with the director of KSs project, trainers, trainees and volunteers in the KSs project. The findings of the study showed that the role of KSs in E-Government readiness has four pillars: enhancement of community awareness in social and economic fields, development of Information and Communications Technology (ICT) capabilities, lessening computer illiteracy fulfillment of comprehensive development.

The apparent conductance (Kss, in W.m-2.degrees C-1) of a given region of superficial shell (on the thigh, fat + skin) was determined on four nonsweating and nonshivering subjects, resting and exercising (200 W) in water [water temperature (Tw) 22-23 degrees C] Kss = Hss/(Tsf-Tsk) where Hss is the skin-to-water heat flow directly measured by heat flow transducers and Tsf and Tsk are the temperatures of the subcutaneous fat at a known depth below the skin surface and of the skin surface, respectively. The convective heat flow (qc) through the superficial shell was then estimated as qc = (Tsf - Tsk).(Kss - Kss,min), assuming that at rest Kss was minimal (Kss,min) and resting qc = 0. The duration of immersion was set to allow rectal temperature (Tre) to reach approximately 37 degrees C at the end of rest and approximately 38 degrees C at the end of exercise. Except at the highest Tw used, Kss at the start of exercise was always Kss,min and averaged 51 W.m-2.degrees C-1 (range 33-57 W.m-2.degrees C-1) across subjects, and qc was zero. At the end of exercise at the highest Tw used for each subject, Kss averaged 97 W.m-2.degrees C-1 (range 77-108 W.m-2.degrees C-1) and qc averaged 53% (range 48-61%) of Hss (mean Hss = 233 W.m-2).(ABSTRACT TRUNCATED AT 250 WORDS)

In this tribological study, we highlight the effect of lubricating oil viscosity in the Multi-pads hydrodynamic journal bearings generate important improvement in characteristics of stiffness and stability in the high speed turbomachines. Depending on viscosity of oil film (three values) variation for five tilted pads bearing, each pad is pivoted and is facilitated to be tilted with small angles, by using Matlab program, we calculate the oil film thickness for convergence layer. We applied Reynold’s equation and solved it’s numerically by using finite difference method with 5 nodes technique to find the pressure distributed on each node in the mesh of tilted pad, then calculate stiffness coefficients. Results show that there is clear effect on stiffens with viscosity change. The increase in value of Krr (for n = 0.3) between viscosity (0.04 Pas. s) and viscosity (0.058 Pas. s) is14.33 MN/m, while the increase in Krr value between viscosity (0.058 Pas. s) and viscosity (0.087 Pas. s) is 11.37 MN/m. the increase in value the of Kss (for n = 0.3) between viscosity (0.04 Pas. s) and viscosity (0.058 Pas. s) is5.921 MN/m, while increase in Kss value between viscosity (0.058 Pas. s) and viscosity (0.087 Pas. s) is9.55 MN/m respectively. the increase in value of Ksr (for n = 0.3) between viscosity (0.04 Pas. s) and viscosity (0.058 Pas. s) is 8.95 MN/m, while the increase in Ksr value between viscosity (0.058 Pas. s) and viscosity (0.087 Pas. s) is 14.41 MN/m respectively. the increase in value of Krs (for n = 0.3) between viscosity (0.04 Pas. s) and viscosity (0.058 Pas. s) are 5.08 MN/m, while the increase in Krs value between viscosity (0.058 Pas. s) and viscosity (0.087 Pas. s) is8.19 MN/m respectively. The values of the dominate principal coefficients Krr is greater than that of Ksr, also The values of the principal coefficients Kss is greater than that of cross coupling Krs for all values of viscosity that studied. From this result, we can conclude the side effect of cross coupling coefficients (Ksr ,Krs) can be overcome by great values for principal coefficient (Krr, Kss) respectively, so we can get good improvement instability for this bearing by variation the viscosity. After that, we regarded to use high viscosity lubricant in multi-pad journal bearing to improve the performance and stability by controlling the stiffness coefficients.

In this work a plain (conventional) and a tilted bad bearings are analyzed and studied (both have the same dimensions and work conditions). Three, four and five pads models are studied. The pads are fixed circumferentially in a manner each two close pads are separated with an angle that is the same for all the close pads. Each pad is pivoted in its leading edge and is facilitated to be tilted around this pivot with small angles. This study was achieved for three values of tilting angles. These angles are, 0.01o, 0.011o and 0.012o. Two dimensions’ form of Reynolds equation is used and solved numerically then the stiffness coefficients had been calculated. A comparison had been done between the stiffness coefficients values for the two models (tilted pads and conventional bearings). The results of this comparison was found that (for n = 0.4), are, +28%, +275%, +270 % and -100% for Ksr, Krr, Kss and Krs respectively. It was noticed that the values of the principal coefficients Krr and Kss are increased significantly while the cross coupling coefficient, (Krs) is decreased significantly and (Ksr) is increased slightly. And this is a positive sign for stability improvement. Also it was found that increasing the tilt angle (delta) cause an increase of the stiffness coefficients values. These increases (for n = 0.4 and changing the tilt angle from 0.010 to 0.011 then to 0.012 respectively) are, Krr=+50%, +47%, Kss = +40%, +32%, Ksr = +33%, +42%, and Krs = +15%, +20%. And since increasing the values of the cross coupling coefficients make the bearing stability worse, so increasing the tilt angle is not a preferable method for improving stability. Changing the number of pads is a better solution and it was found that three pads bearing is the best among them.

Loss of ovarian function by the treatment for gynecological malignancy results in a drastic decrease of estrogen causing physical and mental symptoms. The purpose of this study is to evaluate the effect of Japanese Kampo Kamikihito (KKT) and Kamishoyosan (KSS) on menopausal symptoms in gynecological cancer patients. Patients who had menopausal symptoms after gynecologic malignancy treatment were enrolled and randomly divided into a KKT or a KSS group. Kupperman Menopausal Index (KI) questionnaires were obtained before tumor treatment, at baseline, and at 4 and 8 weeks. Changes in KI scores and severity of each symptom were evaluated. A total of 33 patients were enrolled: 18 in the KKT group and 15 in the KSS group. The KI scores significantly decreased at 4 and 8 weeks compared with baseline in both groups. Although no significant difference was found in change in KI scores between the KKT and KSS groups, efficacy showed some differences. Both KKT and KSS were effective for insomnia, vertigo, and palpitation. KSS was also effective for vasomotor symptoms and arthralgia/myalgia. In conclusion, both KKT and KSS were effective for menopausal symptoms in patients after gynecological tumor treatment. Tailor-made Kampo therapy may contribute to improve patients’ physical and mental symptoms.

Dolayısıyla hayırseverlik anlayışından farklı olarak Kurumsal Sosyal Sorumluluk faaliyetlerinde kurumsal ve toplumsal olmak üzere iki taraflı yarar gözetilmektedir. Bu nedenle özellikle kurum yararının sağlanabilmesi için sosyal sorumluluk çalışmalarından kamuoyunun haberdar edilebilmesi önemlidir. Günümüzde kurumsal web sitelerinin bu çalışmaların duyurulmasında etkin bir şekilde kullanıldığı görülmektedir. Bu çalışma öncelikle, Türkiye’de özel sektörün Kurumsal Sosyal Sorumluluk anlayışına ilişkin toplumsal ve kurumsal yarar algısını ortaya koymayı amaçlamaktadır. Ayrıca bu çalışmada, kurumsal yararın sağlanabilmesi için KSS faaliyetlerinin duyurulmasında web sitelerinin kullanımı araştırılmaktadır. Çalışmanın ilk bölümünde, KSS kavramının içeriği, tarihsel gelişimi ve hayırseverlik anlayışından farklı yönleri açıklanmaktadır. İkinci bölümde, KSS kavramı ile ilişkili ekonomik yaklaşımlar ele alınmaktadır. Her iki bölümde de literatür taraması yapılarak, çalışma konusu bilimsel bir çerçevede incelenmektedir. Araştırmaya dayalı üçüncü bölümde ise, Capital dergisi tarafından gerçekleştirilen “Türkiye’nin En Büyük 500 Özel Şirketi” araştırmasının 2011 yılında açıklanan sonuçlarına göre, genel merkezi İstanbul’da bulunan şirketlere yönelik bir anket uygulaması yapılarak elde edilen verilerin analizi sunulmaktadır. Ayrıca araştırmaya katılan şirketlerin kurumsal web siteleri incelenerek KSS faaliyetlerinin bu web sitelerinde yer alıp almadığı belirtilmektedir. Araştırmanın sonucuna göre, şirketlerin büyük bir çoğunluğu KSS faaliyetlerinin kurumsal yararları olduğunu kabul etse de toplumsal yararlarının kurumsal yararlarından fazla olduğu görüşündedir. KSS faaliyetlerinden sadece toplumsal yarar gözettiklerini belirten şirketlerin % 67,6’sının kurumsal web sitelerinde bu tür faaliyetlerini açıklamadıkları bununla birlikte, KSS faaliyetlerinden hem toplum hem kurum yararı gözettiklerini belirten şirketlerin ise % 79,1’inin kurumsal web sitelerinde bu tür faaliyetlerini duyurdukları görülmektedir.

BACKGROUND: The Korean Shoulder Scoring System (KSS) is a reliable and valid procedure for discriminative assessment of the clinical status of patients with rotator cuff tears. This study evaluates the correlation between the preoperative KSS and factors in patients with rotator cuff tears.METHODS: From November 2009 to June 2016, 970 patients who underwent arthroscopic rotator cuff repair were retrospectively evaluated. A total of 490 patients met the study criteria. Preoperative factors included age, sex, symptom duration, mediolateral (ML) and anteroposterior (AP) tear size, acromiohumeral distance (AHD), tangent sign, tendon involvement (type I, supraspinatus; type II, supraspinatus and subscapularis; type III, supraspinatus and infraspinatus; type IV, all 3 tendons), fatty infiltration of rotator cuff muscles (group I, Goutallier stages 0 and 1; group II, Goutallier stages 2, 3, and 4), and KSS.RESULTS: Old age, ML tear size, and AP tear size negatively correlated with the preoperative KSS (p < 0.001). AHD showed a positive correlation with the preoperative KSS (p < 0.001). A significantly inferior preoperative KSS was found in females and type III tendon involvement (p < 0.001). For supraspinatus and infraspinatus, the preoperative KSS of group II fatty infiltration showed a significantly lower score than group I fatty infiltration (p < 0.05).CONCLUSIONS: A relatively lower preoperative KSS was associated with old age, large tear size, narrow AHD, female, type III tendon involvement, and group II fatty infiltration of the supraspinatus and infraspinatus. Our study indicates that preoperative KSS can be a good measurement for the preoperative status of patients with rotator cuff tears.

Kajian dalam artikel ini diarahkan pada sikap Komunitas Sedulur Sikep (KSS) terhadap alam. Berdasarkan hasil analisis terhadap beberapa literatur yang membahas KSS, serta wawancara dan observasi peneliti terhadap sikap dan perilaku KSS di wilayah Kudus, Pati, dan Blora maka dapat disimpulkan bahwa KSS memiliki sikap yang sangat mendukung pelestarian alam. Kesimpulan tersebut juga didasarkan pada gagasan kosmologis dalam prinsip-prinsip pengajaran yang dimiliki KSS sebagaimana tercermin dalam teknik pertanian yang memberi kesempatan tanah untuk beristirahat dan sikap menghindari konflik dengan hama. Sikap ramah terhadap lingkungan ini dapat menjadi contoh bagi gereja dan masyarakat dalam bersikap terhadap alam terlebih adanya fakta bahwa kekristenan telah lama dituding terlibat dalam kerusakan lingkungan berdasarkan penafsiran teks-teks Alkitab. Dengan kesadaran terkait masalah-masalah lingkungan hidup maka diperlukan juga sebuah paradigma penafsiran teks-teks Alkitab yang benar-benar mempertimbangkan pelestarian alam.

Abstract Introduction The Karolinska Sleepiness Scale (KSS) and the Profile of Mood States Fatigue and Vigor subscales (POMS-F and POMS-V) are commonly used to assess subjective sleepiness, fatigue and vigor in response to sleep loss. However, the detailed time course of relationships between these measures across sleep loss and recovery remains unknown yet is critical for assessing varying changes in perception of subjective states. Repeated measures correlation (rmcorr) examined within-individual association between the measures throughout a highly controlled sleep deprivation study. Methods Forty-one healthy adults (ages 21-49; mean±SD, 33.9±8.9y; 18 females) participated in a 13-night experiment consisting of two baseline nights (10h-12h time-in-bed, TIB) followed by 5 sleep restriction (SR) nights (4h TIB), 4 recovery nights (12h TIB), and 36h total sleep deprivation (TSD). A neurobehavioral test battery, including the KSS, POMS-F, and POMS-V, was administered every 2h during wakefulness. Rmcorr compared KSS, POMS-F, and POMS-V scores by examining correlations by study day (e.g., Baseline day 2) and by time point (e.g., 1000h-2000h). Rmcorr cutoffs were as follows: r=0.1:small, 0.3:moderate, 0.5:large. Results KSS and POMS-F maintained positive correlations throughout the study, whereas POMS-F and POMS-V and KSS and POMS-V were inversely correlated. All correlations were significant except those for POMS-F and POMS-V across recovery day 1 and KSS and POMS-F across recovery day 4. All measure pairs showed moderate to large correlations across baseline and SR1-5, but only small to moderate correlations across recovery. KSS and POMS-F and KSS and POMS-V showed moderate to large correlations across TSD; however, POMS-F and POMS-V only showed a small correlation. All three pairs showed consistent moderate (POMS-F and POMS-V) or large (KSS and POMS-F, KSS and POMS-V [moderate at 2000h]) correlations when analyzed by time point across the study. Conclusion Overall, the strength of relationships between KSS, POMS-F, and POMS-V scores varied as a function of type of sleep loss (SR or TSD) and by fully rested states, but not by time of day. This demonstrates the importance of determining perceptions of sleepiness, fatigue, and vigor in relation to each other, especially during recovery for all three constructs. Longan (<em>Dimocarpus longan</em> L.) is not an indigenous fruit from Indonesia. Thus, the demands for this fruit were supplied by importing them from nearby countries such as Vietnam, Thailand, and China. Longan has been cultivated and developed in Indonesia in order to supply the local needs and to reduce the rapid flows of importing Longan. Recently, a brand new breed of local Longan has been engineered from Sleman District of D.I.Yogyakarta Province, known as <em>Kelengkeng Super Sleman</em> (KSS). The KSS well adapted to medium-high above sea level- terrain, the most typical terrain in Sleman. Some prosperous features of this Longan cultivar were had a short harvesting time (2 years), thicky flesh, low water content, and able to be induced twice per year for flowering (to produce fruits). All of this goodness thus been introduced to rural communities of Padukuhan Gejayan, Desa Condongcatur, Depok, Sleman, D.I.Yogyakarta by a well designed community service programme. The kind of activites that was going on in this programme were workshop, training, and assistance. Workshop was emphasized on the prospectous consequences of planting Longan trees (KSS) for a long term until could be developed into local industrial product (One Village One Product/OVOP concept), basic knowledge on Longan diversity, KSS cultivation technology, basic knowledge on plant physiology, environmental development based on Education for Sustainable Development (EfSD), and potential local market for KSS. Training was mainly focused on KSS seedling production by both apical stem grafting method and medium preparation method, KSS plantation in some villager houses for a pilot project on how to maintain KSS such as health evaluation, health recovery for infested or unhealthy plant, method on lateral branching development, method on hormone-induced flowering, and application of resting period for after-harvesting plant. Assistance has been actively conducted during training to motivates villager to be get involved. Some positive indicators for implementation of this programme are both the great enthusiasm on KSS Longan plantation in communities and “the shifting paradigm” on KSS Longan maintenance for which has been long considered to be both impractical and arduous.

Zusammenfassung Kühlschmierstoff (KSS)-strömungen werden in Schleifprozessen eingesetzt, um Schleifbrand am Werkstück zu verhindern. Bislang wurde die KSS-Zufuhr heuristisch optimiert. Um eine zielgerichtete KSS-Strömungsoptimierung zu ermöglichen, wird erstmals die optische schleifscheibennahe Messung von KSS-Strömungsfeldern mittels Particle Image Velocimetry (PIV) und durch Nachverfolgung von Mustern in Schattenwurfbildern untersucht. Als erster Schritt wird an der rotierenden Schleifscheibe, jedoch ohne die Bearbeitung eines Werkstücks, gemessen. Beide Messmethoden liefern vergleichbare Ergebnisse, wobei nahe der Schleifscheibenoberfläche mittels PIV Messwerte mit geringerer Messunsicherheit erzielt werden. Mit dem verwendeten Messaufbau lassen sich mittels PIV Geschwindigkeiten in einem Abstand zwischen 0 cm bis 2 cm und mittels Schattenwurf zwischen 0 cm bis 5 cm zur Schleifscheibenoberfläche messen. Im Ergebnis konnte erstmals der für die Kühlleistung relevante Parameter des Geschwindigkeitsverhältnisses zwischen KSS-Anströmung und Schleifscheibenoberflächengeschwindigkeit in situ gemessen werden. Es wurde festgestellt, dass eine aus der Literatur bekannte Verringerung der Kühleffizienz ab einem Geschwindigkeitsverhältnis von 0,8 mit einem Abprallen des KSS-Strahls beim Auftreffen auf die Schleifscheibe korreliert. In zukünftigen Untersuchungen ist zu klären, wie sich die hier gewonnenen Erkenntnisse zur Messfähigkeit und zum Strömungsverhalten auf den Schleifprozess mit Werkstück übertragen lassen.

Background. Knee arthroplasty surgeries are in ever-increasing demand. With unicompartmental knee arthroplasty (UKA), patients may benefit from a higher range of flexion and a better Knee Society Score (KSS). Aim. In this study, we compared the short-term clinical outcomes of total knee arthroplasty (TKA) and UKA using the patient-specific templating (PST) technique. Methods. Two groups of 20 knees each were divided into UKA and TKA groups depending on the Oxford criteria of UKA. Only patients with medial compartmental osteoarthritis were included. KSS, functional knee score (FKS), and ROF were assessed preoperatively and at 6 months postoperatively. Results. The TKA group has shown a significant improvement compared to the UKA group in KSS (MD = 39.35 vs. 31.2, respectively, p = 0.003 ). Both TKA and UKA have shown no significant difference concerning both the FKS (MD = 32 and 31.75, respectively, p = 0.926 ) and ROF (MD = 10.25 and 7.25, respectively, p = 0.072 ). Discussion. The higher improvement of KSS in the TKA group can be attributed to the fact that patients in the TKA had significantly worse KSS preoperatively. Also, the small improvement in ROF in the UKA group might be related to their wider preoperative ROF. Conclusion. Preoperatively, the TKA group had lower KSS and ROF compared to UKA. The improvement of KSS from preoperative to postoperative was more significant in TKA. However, the TKA group has shown less range of flexion postoperatively.

Females are well known to suffer disproportionately more than males from stress-related neuropsychiatric disorders, especially during perimenopausal and postmenopausal periods. In addition to a decline in serum estradiol levels, environmental stress and social stress likely contribute to the development of neuropsychiatric symptoms in perimenopausal and postmenopausal women. Kamishoyosan (KSS) is a traditional Japanese Kampo medicine, composed of a specified mixture of 10 crude compounds derived from plant sources, widely used for various neuropsychiatric symptoms in perimenopausal and postmenopausal women. However, the molecular mechanisms underlying KSS-mediated attenuation of neuropsychological symptoms and stress-response behaviors in perimenopausal and postmenopausal women remain unknown. In the present study, we first established a mouse model for postmenopausal depression-like signs using chronic water-immersion and restraint-stressed ovariectomized (OVX) mice to investigate the underlying molecular mechanism of KSS. We found that continuous administration of KSS to these mice normalized the activation of the hypothalamic-pituitary-adrenal (HPA) axis, ameliorated stress-induced depressive behavior, and prevented a decrease of neurogenesis in the hippocampus. As previous studies have implicated dysfunction of the hippocampal 5-HT1A receptor (5-HT1AR) in depressive disorders, we also evaluated the effect of KSS on 5-HT1AR expression and the protein kinase A- (PKA-) cAMP response element-binding- (CREB-) brain-derived neurotrophic factor (BDNF) signaling pathway in the hippocampus in this model. The level of 5-HT1AR in the hippocampus decreased in chronic stress-exposed OVX mice, while KSS treatment normalized the stress-induced decrease in 5-HT1AR expression in the hippocampus of chronic stress-exposed OVX mice. Furthermore, we found that KSS treatment upregulated the expression levels of phosphorylated PKA (p-PKA), phosphorylated CREB (p-CREB), and BDNF in the hippocampus in chronic stress-exposed OVX mice. These results suggest that KSS improves neuropsychiatric symptoms through 5-HT1AR and PKA-CREB-BDNF signaling in the hippocampus in postmenopausal women.

Tujuan: Mengetahui profil klinikopatologi KSS kepala dan leher berdasarkan kelompok usia, jenis kelamin, lokasi tumor, stadium, derajat diferensiasi dan Invasi Perineural (IPN).Metode: Penelitian ini merupakan studi deskriptif terhadap 133 kasus KSS kepala dan leher di RSUP Dr M Djamil Padang periode 2016 sampai 2018. Sampel diperoleh dari blok parafin yang berasal dari jaringan hasil biopsi kemudian dilakukan reevaluasi terhadap derajat histopatologi dan IPN berdasarkan klasifikasi The World Health Organization (WHO). Hasil: Kasus KSS kepala dan leher pada penelitian ditemukan pada usia 51-60 tahun (30%), dengan lokasi hipofaring dan laring (50,4%), laki-laki (71,4%), stadium IV (64%), sebagian besar dengan KSS berdiferensiasi baik (42,8%) dan IPN hanya ditemukan sebanyak 10,5% kasus. Simpulan: Profil klinikopatologi KSS kepala dan leher di RSUP Dr. M. Djamil Padang sebagian besar didapatkan pada usia dekade ke-5, lokasi di hipofaring dan laring, laki-laki, sebagian besar pada stadium lanjut, derajat histopatologi baik dan sebagian kecil mengandung IPN. Protein 16 (p16) merupakan salah satu protein supresor tumor yang memiliki peran dalam patogenesis Karsinoma Sel Skuamosa (KSS) kepala dan leher. Peran protein ini berbeda sesuai dengan faktor risiko paparan karsinogen dan memiliki implikasi terhadap faktor prognostik patologik. Tujuan: Menentukan hubungan protein 16 dengan derajat histopatologik dan Invasi Perineural (IPN) pada KSS kepala dan leher. Metode: Penelitian ini merupakan cross-sectional study dengan sampel sebanyak 60 kasus KSS kepala dan leher yang telah didiagnosis di Laboratorium Patologi Anatomik RSUP Dr. M. Djamil Padang periode Januari 2016 sampai Desember 2018. Sampel diperoleh secara simple random sampling dari blok parafin yang berasal dari jaringan tumor. Dilakukan penilaian ulang pada slaid histopatologik untuk menilai derajat histopatologik dan invasi perineural. Ekspresi p16 pada sel tumor dilihat dengan pulasan imunohistokimia. Korelasi dilakukan analisis statistik bivariat menggunakan uji Chi-square dengan tingkat kemaknaan p<0,05. Hasil: Ekspresi p16 positif didapatkan pada 24 kasus (40%) dengan kasus terbanyak pada KSS kepala dan leher derajat tinggi, sedangkan pada 36 kasus (60%) tidak terdapat ekspresi p16. Ekspresi p16 positif memiliki hubungan yang bermakna dengan derajat histopatologik (p = 0,002) namun tidak terdapat hubungan antara ekspresi p16 dengan IPN (p = 1,000). Simpulan: Ekspresi p16 positif berhubungan dengan derajat histopatologik tumor yang lebih tinggi namun p16 tidak tidak berhubungan dengan kejadian invasi perineural pada KSS kepala dan leher.Kata kunci: derajat histopatologik, ekspresi p16, invasi perineural, KSS kepala dan leher

Alignment data in patient-specific instrumented (PSI) knee arthroplasty have been examined in several meta-analyses, with demonstration of comparative but not improved restitution of a neutral mechanical axis and favorable outcomes in femoral implant axial alignment. To date, no comprehensive synthesis of data has been conducted specifically for functional outcomes. Relevant databases were searched according to Preferred Reporting Items for Systematic reviews and Meta-analyses guidelines during the period 2000 to 2015 of Levels 1 and 2 studies comparing functional outcomes of patient-specific instrumentation (PSI) versus conventional instrumentation. Outcomes of interest included postoperative Knee Society score (KSS) (function), KSS (knee), range of movement (ROM), Oxford knee scores (OKS), and Western Ontario and McMaster Universities Arthritis Index (WOMAC) scores. Five randomized controlled trials and three prospective-comparative studies were included reporting on a total of 828 knees. All eight studies reported postoperative KSS (function) and seven studies reported KSS (knee). Three studies reported postoperative OKS, four documented ROM, and two reported WOMAC scores. Meta-analysis demonstrated equivalent KSS (function) scores 3 months postsurgery (Z = 1.71, p = 0.09). In addition, no significant differences were found for KSS (function) at 6 months (Z = 0.51, p = 0.61) or 12 to 24 months postsurgery (Z = 0.05, p = 0.96). In addition, no significant differences were found between PSI and conventional instrumentation for postoperative KSS (knee), ROM, or OKS. In summary, this systematic review and meta-analysis demonstrates no conclusive evidence for or against PSI when considering short-term functional outcomes. Further high-quality studies are required to investigate both mid- and long-term outcomes as well as survivorship data.

Aktuelle Forschungsarbeiten zeigen, dass sich durch eine effektive KSS (Kühlschmierstoff)-Versorgung verbesserte Zerspanbedingungen erreichen lassen. Folglich reduziert sich die thermo-mechanische Belastung und die Prozessleistung kann erhöht werden. Darüber hinaus sinkt aufgrund strömungsoptimierter KSS-Düsen und einer geringeren KSS-Fördermenge der Energieverbrauch. Infolgedessen sind die KSS-Anlagen kleiner dimensionierbar. Somit lässt sich die Energieeffizienz und Wirtschaftlichkeit signifikant steigern. Current research activities have shown that an effective fluid supply improves the chip removal conditions. In consequence, thermo-mechanical loads affecting the workpiece material can be decreased at increasing productivity. Furthermore, the energy consumption can be reduced due to flow optimized fluid supply nozzles and minimized fluid flow rates. As a result fluid supply systems can be downsized and significant increase of energy and economic efficiency can be obtained.

Kearns Sayre syndrome (KSS) is mitochondrial multisystem disorder with no proven effective treatment. The underlying cause for multisystem involvement is the energy deficit resulting from the load of mutant mitochondrial DNA (mtDNA), which manifests as loss of cells and tissue dysfunction. Therefore, functional organ or cellular replacement provides a promising avenue as a therapeutic option. Patient-specific induced pluripotent stem cells (iPSC) have become a handy tool to create personalized cell -based therapies. iPSC are capable of self-renewal, differentiation into all types of body cells including cardiomyocytes (CM) and neural progenitor cells (NPC). In KSS patients, mutations in mtDNA are largely found in the muscle tissue and are predominantly absent in the blood cells. Therefore, we conceptualized that peripheral blood mononuclear cells (PBMNC) from KSS patients can be reprogrammed to generate mutation free, patient specific iPSC lines that can be used as isogenic source of cell replacement therapies to treat affected organs. In the current study we generated iPSC lines from two female patients with clinical diagnosis of classic KSS. Our data demonstrate that iPSC from these KSS patients showed normal differentiation potential toward CM, NPC and fibroblasts without any mtDNA deletions over passages. Next, we also found that functional studies including ATP production, reactive oxygen species generation, lactate accumulation and mitochondrial membrane potential in iPSC, CM, NPC and fibroblasts of these KSS patients were not different from respective cells from healthy controls. PBMNCs from these KSS patients in the current study did not reproduce mtDNA mutations which were present in muscle biopsies. Furthermore, we demonstrate for the first time that this phenomenon provides opportunities to create isogenic mutation free iPSC with absent or very low level of expression of mtDNA deletion which can be banked for future cell replacement therapies in these patients as the disease progresses.

Penanaman tanaman buah seperti Kelengkeng Super Sleman (KSS) menjadi alternatif dalam memecahkan permasalahan terkait mengurangi dampak pemanasan global melalui program penghijauan. Selain itu memiliki nilai tinggi baik dari sisi pemenuhan gizi maupun produk pemasaran di bidang ekonomi bagi masyarakat sekitar. Pengenalan teknik budidaya KSS berbasis lingkungan di Dusun Gejayan merupakan salah satu bentuk dukungan pembangunan yang berkesinambungan melalui program setiap daerah dalam menghasilkan suatu produk (one village one product). Kegiatan dilakukan melalui tiga tahapan, yaitu; penyuluhan, pelatihan, dan pendampingan secara berkala. Hasil yang diperoleh dalam penyuluhan sebagai kegiatan awal adalah terjadi peningkatan persentase pengetahuan peserta dengan nilai sebelum dan sesudah kegiatan adalah sebesar 69,72%. Hasil juga menunjukkan bahwa warga Padukuhan Gejayan secara umum telah mengalami perubahan paradigma terkait budidaya KSS dan memahami penerapan teknik budidaya KSS berbasis lingkungan. Dampak positif tidak hanya berapa dukungan penghijauan dan pemenuhan gizi warga, tetapi juga menambah perekonomian warga melalui penjualan buah dan bibit ke konsumen. Dengan dukungan penuh dari Kepala Padukuhan Gejayan, penanaman pohon KSS diharapkan terus berkembang dan menjadi suatu budaya di dalam lingkungan Padukuhan Gejayan.

Kawasaki shock syndrome (KSS), on the other hand, is less well recognized and has been reported in small single-center international studies and case reports. We report a case in the United States of an 11-year-old male with multiorgan failure and shock, presumed to be secondary to toxic shock but later diagnosed with KSS, an underrecognized entity in the US and review the literature. KSS should be considered in a critically ill child with unexplained shock.

ABSTRACT What knowledge and skills (KSs) are necessary for complex Agreed-Upon Procedures (AUP) engagements, such as those that include eXtensible Business Reporting Language (XBRL) tagged data? This study contributes by investigating these issues in two studies: (1) a survey and (2) structured interviews of accounting professionals with XBRL expertise. Survey results suggest commonality in perceptions of the KSs needed in two knowledge categories, “Accounting and Financial Reporting” and “Undertaking AUP on XBRL Instance Documents,” and, variability in ratings of two categories: “XBRL technology” and “audit procedures.” Interview results indicate that knowledge of the U.S. GAAP Taxonomy is most important to AUP engagements followed by the nature of AUP engagements. The results suggest: (1) a need for clarification of XBRL-relevant KSs and (2) an important but unfilled role for universities and scholars in discerning, supporting, and developing the KSs needed to support complex assurance engagements.

<p><strong>Introducción</strong></p><p>El reemplazo total de rodilla en secuela de trauma, secundaria a fracturas periarticulares de fémur distal o tibia proximal, implica un tratamiento complejo pero es una opción válida para el manejo de estos pacientes. El ojetivo de este trabajo es evaluar en forma retrospectiva los resultados obtenidos en RTR en pacientes con secuela de fracturas peri articulares de la rodilla.</p><p><strong>Material y Métodos</strong></p><p><strong></strong>En un estudio retrospectivo, multicéntrico donde se evaluaron 29 pacientes, con artrosis secundaria a fracturas a los que se les realizó RTR entre 2008-2015. 19 mujeres y 10 varones. Edad promedio 59 años. 23 fracturas en tibia proximal y 6 en fémur distal: 6 extraarticulares y 23 intraarticulares. Tratamiento inicial: ortopédico en 8 pacientes; reducción abierta y fijación interna (RAFI) en 21. Promedio de cirugías hasta la artroplastia 2,14 (1 a 7). Se empleó el Knee Society Score (KSS) y radiografías para la evaluación pre y postquirúrgica. Rango de movimiento (ROM) prequirúrgico: flexión 60° (45°-90°), extensión 15° (0°-30°). KSS dolor 30 (10-45) y para KSS funcional 25 (20-45). KSS total 30 (20-60)</p><p><strong>Resultados</strong></p><p><strong></strong>El seguimiento promedio 45 meses (6-104). Se restauró el eje mecánico en todos los casos. ROM en el último control: flexión 112° (60°-140°); extensión 5° (0°-30°). KSS dolor 40 (30-50) y KSS funcional 35 (30-50). KSS total 81 (50-95) Resultados buenos (KSS ≥80 puntos) 22 pacientes (75,8%), regulares 6 (20,6%) y pobres 1 caso (3,44 %).. 13% de complicaciones mayores.</p><p><strong>Discusión</strong></p><p><strong></strong>Si bien el RTR para el tratamiento de secuelas de fracturas periarticulares de la rodilla, presenta mayor porcentaje de complicaciones, siendo los resultados inferiores a los publicados en RTR en artrosis no traumaticade rodilla, el RTR es una opcion valida para el rescate postraumatico, obteniendo una mejoria significativa tanto en el KSS como en el rango de movimiento. El mismo implica un manejo de partes blandas minucioso, y una mayor complejidad técnica.</p>

To encourage more qualified and integrated business processes, BPS should optimize the knowledge sharing process (KSP) among government employees in statistical areas. This study designed a Knowledge Sharing System (KSS) to facilitate KSP in BPS towards knowledge sharing improvement. The KSS manifested a hypothesis that the design of qualified knowledge management can facilitate an organization to overcome the lack of integration among business processes. Hence, BPS can avoid repetitive mistakes, improve work efficiency, and reduce the risk of failure. This study generated a business process-oriented KSS by combining soft system methodology with the B-KIDE (Business process-oriented Knowledge Infrastructure Development) Framework. It delivered research artifacts (a rich picture, CATWOE analysis (costumer, actor, transformation, weltanschauung, owner, environment), and conceptual model) to capture eight mechanisms of knowledge, map them into the knowledge process, and define the applicable technology. The KSS model has perceived a score of 0.40 using the Kappa formula that indicates the stakeholders’ acceptance. Therefore, BPS can leverage a qualified KSS towards the integrated business processes statistically while the hypothesis was accepted.

Objective Radiographic changes in knee osteoarthritis (OA) are not always associated with symptoms, especially in its early stages. Ultrasonography (US) can detect early changes in the knee joint, but the changes that reflect symptoms have not been fully elucidated. This study aimed to identify US-detectable changes in the knee that are often associated with knee symptoms and demonstrate the feasibility of early diagnosis in symptomatic knee OA using US. Design In this cross-sectional community-based study, 1,667 participants aged ≥60 years (1,103 women [66%]) were included. All participants concurrently underwent US and radiography of the knee and completed the Knee Society Knee Scoring System (KSS) questionnaire. Simple and multiple regression analyses were used to examine the associations between US findings and KSS symptom subscales. Results Among all participants, medial meniscus protrusion and medial osteophytes, age, and body mass index showed significant associations with KSS symptom scores. Among 894 participants with Kellgren-Lawrence (KL) grade ≤1, medial osteophytes and age were significantly associated with KSS symptom score. US measures were more related to KSS symptoms than KL grades. Conclusions Among the knee US-detectable changes, medial osteophytes were strongly associated with knee symptoms. Osteophytes are reliable predictors of symptomatic early knee OA, even in participants with few radiographic OA changes.

Abstract Introduction The 10-minute and 3-minute versions of the Psychomotor Vigilance Test (PVT10 and PVT3) and the Karolinska Sleepiness Scale (KSS) are commonly used to assess objective behavioral attention deficits and subjective sleepiness in response to sleep loss, respectively. However, the precise time course of relationships between behavioral attention and subjective sleepiness across sleep loss and recovery remains unknown but is critical for determining whether objective and subjective measures track each other. Repeated measures correlation (rmcorr) examined within-individual association between these measures throughout a highly controlled sleep deprivation study. Methods Forty-one healthy adults (ages 21-49;mean±SD, 33.9±8.9y;18 females) participated in a 13-night experiment consisting of two baseline nights (10h-12h time-in-bed, TIB) followed by 5 sleep restriction (SR) nights (4h TIB), 4 recovery nights (12h TIB), and 36h total sleep deprivation (TSD). A neurobehavioral test battery, including the PVT10, PVT3, and KSS, was administered every 2h during wakefulness. Rmcorr compared PVT10 [lapses (reaction time [RT] >500ms) and 1/RT (response speed)], PVT3 (lapses [RT>355ms], 1/RT), and KSS scores by examining correlations by day (e.g., Baseline day 2) and time point (e.g., 1000h-2000h). Rmcorr ranges: r=0.1:small; r=0.3:moderate; r=0.5:large. Results Generally, the correlations between the PVT10 and KSS and the PVT3 and KSS showed a similar pattern for lapses and 1/RT. PVT lapses and KSS scores showed small or non-significant correlations during baseline and recovery, whereas SR and TSD showed moderate correlations. PVT 1/RT and KSS scores showed moderate correlations during baseline, moderate to large correlations during SR and TSD, but small correlations during recovery. PVT10 and PVT3 1/RT showed stronger correlations with KSS scores than lapses. Additionally, all relationships showed moderate to large correlations by time point across the study. Conclusion Overall, the relationship between behavioral attention and sleepiness was stronger across sleep loss (SR or TSD) relative to fully rested states while it was consistently relatively strong at specific times of day throughout the study. In contrast to published literature, there is a remarkably stable relationship between an individual’s objective behavioral attention performance and perceptions of sleepiness during sleep loss, which is not evident during recovery or at baseline. ABSTRACT The Middle Permian (Guadalupian), Upper Permian (Lopingian) and Lower Triassic Khuff and correlative formations in the Arabian Plate consist of six “third-order” sequences, from oldest to youngest KS6 to KS1, and at least 45 “fourth-order” sequences. They are here dated using biostratigraphic constraints and correlated to two independent sequence-stratigraphic time scales: (1) global sequences calibrated in the Geological Time Scale GTS 2012; and (2) orbital-forcing glacio-eustatic sequences that track the 0.405 million year (Myr) orbital eccentricity signal in the M&H-2010 scale (Matthews and Al-Husseini, 2010). The chronostratigraphic calibration of the Khuff sequences provides a reference section and common nomenclature that can be used for regional and global correlations. It permits positioning the hydrocarbon reservoirs of the Khuff and equivalent formations in a sequence-stratigraphic framework that can be used in exploration and reservoir characterization. The lower sequence boundary of the Khuff Formation (Khuff SB) is correlated to global Wordian SB Wor1 near the Roadian/Wordian Boundary at 268.8 ± 0.5 Ma, and correlative SB 19C at 268.9 Ma in the M&H-2010 scale. The upper sequence boundary of the Khuff Formation with the overlying Sudair Formation (Sudair SB) is correlated to Olenekian SB Ole1 near the Induan/Olenekian Boundary at 250.0 ± 0.5 Ma, and correlative SB 17 at 249.5 Ma in the M&H-2010 scale. These calibrations imply the Khuff was deposited in about 19.4 Myr, and consists of 48 “stratons”; i.e. transgressive-regressive (T-R) depositional subsequences with an average duration of 0.405 Myr corresponding to long-eccentricity orbital cycles 664 to 617. The 48 stratons are predicted to form four “dozons” (19C, 18A, 18B and 18C), each consisting of 12 stratons. Individual dozons lasted 4.86 Myr and are separated by regional sequence boundaries (SB 19C to SB 17A). In Oman, Khuff Sequence KS6 on the Saiq Plateau is correlated to the subsurface Lower Khuff Member, and both are interpreted to consist of 12 subsequences that are correlated to stratons 664–653 forming Dozon 19C between 268.9–264.0 Ma. KS6 is correlated to the four global sequences Wordian Wor1 to Capitanian Cap1 dated between 268.8–264.0 Ma in GTS 2012. Khuff Sequence KS5 corresponds to the Middle Khuff Member up to the top of Middle Khuff Anhydrite in subsurface Oman. On the Saiq Plateau, KS5 potentially consists of 12 cycle sets (Koehrer et al., 2010) that are correlated to stratons 652–641 of Dozon 18A, between 264.0–259.2 Ma. It is correlated to global sequences Capitanian Cap2 and Cap3 dated between 264.0–259.8 Ma in GTS 2012. Khuff Sequence KS4 consists of 11 cycle sets on the Saiq Plateau and other localities in Al Jabal al-Akhdar in Oman (Koehrer et al., 2010, 2012). Introducción: El tratamiento del valgo severo es complejo. El abordaje externo descrito por Keblish es ventajoso para tratar estas deformidades. Evaluamos nuestros resultados utilizando un abordaje externo en artroplastias de rodilla con genu valgo severo.
Materiales y Métodos: De 795 artroplastias primarias realizadas entre enero de 2012 y marzo de 2020, analizamos 40 que tenían un abordaje externo. Todos los pacientes tenían una deformidad en valgo >20° y un ligamento colateral medial suficiente. Se incluyó a 33 mujeres (3 bilaterales) y 4 hombres, el promedio de edad era de 71 años. La causa fue fundamentalmente osteoartrosis (82%). La deformidad prequirúrgica era de 27°. Las mediciones preoperatorias eran: KSS 15 (rango 5-42) y KFS 17 (rango 0-40). El tiempo promedio de cirugía fue de 91 minutos y el seguimiento, de 37 meses.
Resultados: El ángulo femorotibial posoperatorio fue de 6,2° (rango 4-40), el KSS posoperatorio fue de 79 (rango 46-95) y el KFS, de 82 (rango 60-100). Hubo tres complicaciones (7%): una infección, un mal posicionamiento de componentes y uno de neuropraxia peronea.
Conclusiones: En la artroplastia de rodilla por genu valgo, este abordaje permite restituir el eje femorotibial, lograr una adecuada estabilidad de la prótesis, aun con implantes sin mayor grado de constreñimiento, con una tasa de complicaciones equiparable a la de otras técnicas.

Aims: With increasing life expectancy and ageing population, more octogenarians would benefit from total knee arthroplasty (TKA). The aim of this study is to evaluate the survivorship of TKA in octogenarian and their long-term outcomes. Methods: This was a retrospective review of 67 patients aged above 80 years who had primary TKA performed between 2005 and 2013 at a single centre. A sex-matched younger cohort of 67 primary TKAs during the same time-period was recruited for control. Five and 10-year survival was calculated. Pre- and peri-operative factors were collected and compared. Post-operative range of motion, Knee Society Score (KSS) and Function Score (KFS) were collected and compared. Rate of revision, infection, aseptic loosening and mortality was collected. Results: The mean follow-up period was 10.39 ± 2.29 years. The 5- and 10-year survival of octogenarian group was 85.1% and 54.6%, respectively. Pre- and peri-operative factors between the octogenarian and control groups were comparable ( p > 0.05). Both groups showed improvement in range of motion, KSS and KFS post-operatively. Rate of revision, aseptic loosening and deep infection was 0% in octogenarian and 1.5% in control group. Conclusion: TKA can be performed in patients with age above 80 years old with comparable post-op knee range, complication rate and long-term functional score. Advanced age should not be a limiting factor for TKA.

Objective. To describe unusual course and unusual phenotypic features in an adult patient with Kearns–Sayre syndrome (KSS). Case Report. The patient is a 49-year-old male with KSS, diagnosed clinically upon the core features, namely, onset before the age 20 of years, pigmentary retinopathy, and ophthalmoparesis, and the complementary features, namely, elevated CSF protein, cardiac conduction defects, and cerebellar ataxia. The patient presented also with other previously described features, such as diabetes, short stature, white matter lesions, hypoacusis, migraine, hepatopathy, steatosis hepatis, hypocorticism (hyponatremia), and cataract. Unusual features the patient presented with were congenital anisocoria, severe caries, liver cysts, pituitary enlargement, desquamation of hands and feet, bone chondroma, aortic ectasia, dermoidal cyst, and sinusoidal polyposis. The course was untypical since most of the core phenotypic features developed not earlier than in adulthood. Conclusions. KSS is a multisystem disease, but the number of tissues affected is higher than so far anticipated. KSS should be considered even if core features develop not earlier than in adulthood and if unusual features accompany the presentation.

Objective Fatigue in road traffic can occur after long driving times or during night-time driving and can lead to a dangerous decrease in vigilance or microsleep. As a countermeasure, the effectiveness of brain stimulation by means of frequency-modified music is investigated. Small frequency shifts between two different sounds simultaneously perceived by both ears (e.g. 400 and 418 Hz) stimulate the brain to increase activity in the stimulated range (e.g. 18 Hz). Methods The effects of acoustic brain entrainment (ABE) in the EEG beta range (12–20 Hz) were compared to placebo during day and night driving (n = 40 each) in 80 subjects. The effects were examined at the subjective (Karolinska Sleepiness Scale, KSS), physiological (EEG) and performance level (test battery for attention testing, TAP). The test drive took place on the motorway. Sequence of events with measurement times: Preparation in the laboratory (60 min; TAP & KSS), driving (90 min day/ 180 min night; KSS every 30 min), beta/placebo stimulation while driving (20 min), rest (20 min; TAP & KSS), driving (60 min; KSS every 30 min), end of driving (40 min; TAP & KSS). EEG was recorded continuously (Fz, Cz, Pz, C3, C4) and analyzed for 10 min time windows. Results The subjective fatigue rating in the KSS decreased significantly after ABE compared to placebo. This was still significant about 100 minutes after stimulation. The ABE led to a significant increase in EEG beta activity compared to placebo. This was still significant about 80 minutes after stimulation. Furthermore, the ABE led to a significant decrease in theta activity compared to placebo. This was still significant about 70 minutes after stimulation and was more pronounced during daytime driving. Faster reaction times were observed for the ABE compared to the placebo condition during day and night driving. The faster reaction times were partly significant for the ABE still 80 minutes after stimulation. Conclusion Positive effects of ABE during driving could be demonstrated on the subjective, physiological and performance level. The effect was more pronounced during the day than at night. No negative side effects of Beta Stimulation were observed. The stimulation in the beta frequency range led to an increase of beta activity in the EEG.

Abstract Purpose Spinal cord involvement in Kearns-Sayre (KSS) syndrome could be more frequent than commonly thought. Our aims were to evaluate the involvement of the spinal cord in patients with KSS by means of MRI and to investigate possible correlations of spinal and brain disease with patient disability. Methods Eleven patients with KSS disease and spinal cord MRI were retrospectively recruited. The severity of spinal disease was defined as follows: grade 0 (none), grade 1 (focal), and grade 2 (extensive). We calculated a radiologic score of brain involvement based on typical features. We performed a chi-square test to correlate spinal cord and brain MRI involvement to patient disability. For significant variables, a contingency coefficient, phi factor, and Cramer’s V were also computed. Results Spinal cord lesions were detected in 6/11 patients, showing four patterns: involvement of gray matter, gray matter and posterior columns, posterior columns, and anterior columns. The severity of spinal disease was grade 1 in two and grade 2 in four patients. All patients showed brain involvement (9-point average for patients with spinal involvement and 10 for the others). A significant correlation was found between disability score and spinal cord involvement (χ2 = 7.64; p = 0.022) or brain score (χ2 = 26.85; p = 0.043). Significance for brain score-disability correlation increased with the spinal cord as a cofactor (χ2 = 24.51; p = 0.017, phi factor = 1.201, Cramer’s V = 0.849, contingency effect = 0.767; p = 0.017). Conclusion Spinal cord lesions are common in KSS. Patients with spinal disease show higher disability than patients without spinal cord lesions, supporting the inclusion of dedicated acquisitions to routine MRI of the brain in patients with KSS.

Introduction: This study compared the immediate and early postoperative outcomes of medial compartment knee osteoarthritis patients receiving unicompartmental knee replacement (UKR) with a matched cohort of total knee replacement (TKR). Methods: 26 UKR patients were matched with 26 TKR patients based on age, body mass index, pre-operative radiographic severity, range of motion (ROM), Knee Society score (KSS) and Feller patella score. Immediate postoperative outcomes were reflected by postoperative pain, blood loss, length of stay and the number of physiotherapy sessions attended. Early postoperative outcomes (ROM and KSS) were measured at 3 months and 1 year post-operatively. Results: UKR patients had less hemoglobin drop (UKR: 1.2 g/dL, TKR: 1.6 g/dL, p = 0.04), shorter length of stay (UKR: 4.3 days, TKR: 6.0 days, p < 0.001) and required less physiotherapy sessions for recovery (UKR: 6.9 sessions, TKR: 9.3 sessions, p < 0.05). There were no statistically significant differences in early post-operative pain score and postoperative analgesia use ( p > 0.05) between the two groups. Patients receiving UKR had significantly higher post-operative KSS (UKR: 155.9, TKR: 142.4, p = 0.005) and ROM (UKR: 115.8o, TKR: 98.8o, p < 0.001) at 3 months. The KSS and ROM of UKR group at 3 months was better than TKR group at 1-year follow-up. Conclusion: In patients with medial knee osteoarthritis, UKR showed less postoperative analgesic use and blood loss, shorter length of stay, shorter course of rehabilitation and faster recovery with better early KSS and ROM than TKR. Follow up is necessary for comparison in long term outcome and survivorship between the two groups.

Excessive triglyceride accumulation in lipid-metabolizing tissues is associated with an increased risk of a variety of metabolic diseases. Kamishoyosan (KSS) is a Kampo composed of 10 constituent herbs, and contains moutan cortex (MC) and paeonol (PN) as the major ingredient of MC. Here, we demonstrate the molecular mechanism underlying the effect of KSS on the differentiation of mouse preadipocytes (3T3-L1 cells). KSS inhibited the accumulation of triglycerides in a dose-dependent manner in 3T3-L1 cells that were induced to differentiate into adipocytes. We also found that MC and PN were responsible for the anti-adipogenetic effect of KSS and significantly suppressed the expression of CCAAT/enhancer-binding proteins-δ (C/EBP-δ) mRNA 3 days after the induction of differentiation. Thus, PN may contribute to the anti-adipogenetic property of MC in 3T3-L1 cells. In addition, PN inhibited dexamethasone (Dex)-induced glucocorticoid receptor (GR) promoter activity. Taken together, these results suggest that PN suppresses C/EBP-δ expression by inhibiting Dex-induced GR promoter activity at the early stage of differentiation and, consequently, delays differentiation into mature adipocytes. Our results suggest that the habitual intake of Kampo-containing PN contributes to the prevention of the onset of metabolic diseases by decreasing the excessive accumulation of triglycerides in lipid-metabolizing tissues.

Purpose To review the outcome of unilateral total knee arthroplasty (TKA) in patients with bilateral knee osteoarthritis (OA) to determine whether unilateral TKA can achieve good health-related quality of life (HRQoL). Methods Records of 68 women and 25 men with severe bilateral knee OA who underwent unilateral TKA on the more symptomatic knee by a single surgeon and did not return for the second TKA within 2 years were reviewed. Outcome at 6 months and 2 years was assessed using the mental and physical component summary (MCS and PCS) of the Short-Form 36 (SF-36) for HRQoL as well as the Oxford Knee Score (OKS) and Knee Society Score (KSS) for knee-specific outcome. Results The SF-36, OKS, and KSS improved significantly after unilateral TKA. Respectively for MCS and PCS of SF-36, OKS, and KSS knee and function scores, 65%, 89%, 98%, 98%, and 98% of patients reported absolute improvement at 2 years. Respectively for PCS, OKS, and KSS function score that a minimal clinically important difference (MCID) was established, 77%, 96%, and 80% of patients achieved MCID at 2 years. Conclusion Unilateral TKA achieves good outcome in HRQoL at 2 years in patients with bilateral knee OA.

Aims and Objectives: The purpose was to evaluate the clinical outcomes and patient satisfaction of patients with knee osteoarthritis treated within a selective arthroplasty algorithm, using focal metal implants (FMI), unicompartmental (UKA), patellofemoral (PFA), bicompartimental (BKA) and total knee arthroplasty (TKA) in a single center. Materials and Methods: This is a prospective study analyzing a consecutive series of 613 patients treated by two surgeons in a single center. In 283 men and 330 women either partial or total knee replacement were performed. Their mean age at surgery was 63±6,85 years and mean BMI 29,55±5,00 kg/m2. 357 UKA, 178 TKA, 30 PFA, 23 BKA and 25 FMI were performed. Implants were cemented or cementless (UKA) and made of cobalt chrome in partial knee and zirconium oxide in total knee replacement. Demographics and patient reported outcomes (VAS, KOOS, Oxford Knee Score (OKS) and Knee Society Score (KSS Expectations and satisfaction)) were collected preoperatively and 3,6 and 12 months postoperatively. A total of 442 of 457 eligible patients (Compliance = 96,7%) have thus far completed the 12 months follow-up time point. Results: All mean KOOS, OKS, KSS and VAS scores improved significantly 1 year after surgery (p<0.05). Mean preoperative aggregated KOOS improved from 49.8±13,3 to 74,8±16.9 in UKA, from 43,9±13,3 to 65,2±12,4 in PFA, from 46.6±13,1 to 73,8±14,4 in TKA, from 45,2±11,2 to 73,0±9,8 in BKA and 40.9 ± 23.0 to 63.2 in FMI (p<0.05). Mean preoperative aggregated OKS improved from 25,3±7,6 to 38,7±8,4 in UKA, from 22,9±7,6 to 33,3±8,6 in PFA, from 23.3±7.3 to 37,5±7.7 in TKA, from 22,0±7,5 to 39,0±4.0 in BKA and from 22.9 ± 10.0 to 33.4 ± 11.3 in FMI (p<0.05). The mean pain level VAS decreased from pre-treatment to 12 months after surgery in UKA from 5.6 to 1.6, in PFA from 6.2 to 2.8, in TKA from 6.2 to 1.7, in BKA from 6.8 to 1.6 and VAS 5.5 to 2.4. The Mean KSS Expectation/Satisfaction improved at 1 y FU in UKA in expectation 13.7 ± 1.6 and mean KSS satisfaction: 31.1 ± 8.4, in FMI in expectation: 13.5 ± 1.6 and mean KSS satisfaction 26.0 ± 11.4 , in TKA: expectation 13.5 ± 1.8 and mean KSS satisfaction 30.0 ± 7.1, in PFA: expectation 13.7 ± 1.7 and mean KSS satisfaction 26.4 ± 9.2 and BKA: expectation 13.6 ± 1.9 and mean KSS satisfaction 31.2 ± 4.2. Two patients (0.4%) underwent revision (at 3 month for inlay dislocation (UKA) and at 12 Month for desease progression (FMI)). Conclusion: This study shows excellent clinical results and patient satisfaction of patients treated within a selective arthroplasty algorithm. Adherence to strict indications for partial as well as for total knee replacement will lead to a comparable significant improvement of patient reported outcomes, patient satisfaction and a low revision rate one year postoperatively.

The aim of the study was to determine the effects of air relative humidity (RH: 60 and 80%) during the drying process of “krakowska sucha staropolska” (KSS) sausages on selected quality characteristics. The composition and production process of KSS sausages complied the requirements of traditional specialities guaranteed. It was found that the use of lower RH of drying air allowed a time reduction of 20%. Lowering the RH of air during the drying process did not affect the cross-sectional colour and odour of the sausages, colour components, TBARS values, protein, fat and salt content. However, the acceleration of the drying process resulted in very dried outer layers and less dry interior of KSS sausages. As a result, the sausages had lower scores in the sensory evaluation of hardness and overall acceptability. For this reason, shortening the drying process of sausages by using air with lower relative humidity is not recommended.

<p class="abstract"><strong>Background:</strong> Osteoarthritis of knee is a chronic degenerative joint disease which leads to pain, swelling, stiffness and decreased quality of life. The purpose of this study was to assess the effect of intra-articular steroid injection for osteoarthritis knee by using knee society score (KSS) and visual analog scale (VAS).</p><p class="abstract"><strong>Methods:</strong> A prospective study of 38 patients (44 knees) diagnosed with osteoarthritis of knee were given 80mg of intra-articular methylprednisolone injection. The outcome was assessed using KSS and VAS before the injection and 3 months post injection.<strong></strong></p><p class="abstract"><strong>Results:</strong> The mean VAS improved 3 months post injection whereas there was no improvement in the mean KSS. The scores of VAS 3 months post injection were statistically significant whereas the KSS scores were statistically insignificant compared with the pre injection values.</p><p class="abstract"><strong>Conclusions:</strong> Intra-articular steroid injection for osteoarthritis of knee produces significant pain relief for most patients even in severe cases but this not translate to better functional outcomes.</p>

Objectives: Ball velocity is generated during the overhead baseball pitch via efficient force transmission up the kinetic chain, from the lower body up and outward to the throwing hand. The kinematic sequence, or the sequential timing pattern of peak angular velocities of body segments during a pitch, provides insight to segment position and motion control that drives the kinetic chain (Putnam CA, 1993). Previous publications report an ideal kinematic sequence (KS) where the timing of each body segment’s peak angular velocity occurs in a proximal-to-distal (PDS) pattern resulting in greater ball velocity and reduction in throwing arm injury risk (Fortenbaugh D, et.al, 2009). A recent study revealed that baseball pitchers perform a variety of KSs (Scarborough DM et.al, 2018). There is no known investigation of the relationship of kinematic sequences and throwing arm joint torques. The purpose of this study was to 1) identify the number of different KSs performed by each pitcher and 2) compare elbow valgus and shoulder external rotation (ER) and extension (Ext) torques between the 3 primary KSs performed during the fastball pitch. Methods: Fourteen collegiate baseball pitchers (20.57 ± 1.91 yr) underwent 3D biomechanical pitch analysis using 20 motion-capture Vicon MX™ cameras (360 Hz). A total of 119 fastball pitches with an average of 8.5 ± 2.71 pitches per player were analyzed. Elbow valgus and shoulder external rotation and extension torques were calculated. The timing of peak angular velocities for the pelvis, trunk, arm, forearm and hand body segments were recorded to generate each pitch’s KS. KSs were then divided into groups based on similarities to the ideal PDS pattern. ANCOVA statistical analyses were performed to compare joint torques across these KS groups with ball velocity as a covariate. Results: A total of 13 different KSs were observed across the 14 pitchers resulting in an average of 3 ± 1.41 different KSs per pitcher. Three different primary KS groups were identified: (1) PDS group: with a KS closest to the ideal PDS pattern (2) the Altered Distal Upper Extremity segment: with the forearm peaking after the hand (the most common group) and (3) Altered Proximal Upper Extremity segment order with the arm segment peaking after the hand (2nd most common). Across these three primary KS patterns, statistically significant differences were noted for elbow valgus torque [F(62,2) = 8.785, ɳ2 = .221, p < 0.00], shoulder external rotation (ER) torque [F(62,2) = 14.127, ɳ2 = .313, p < 0.00] and shoulder extension (Ext) torque [F(62,2) = 13.237, ɳ2 = .299, p < 0.00] (Figure 1). Conclusion: Our findings demonstrate that collegiate baseball pitchers performed an average of 3 different kinematic sequence patterns during fastball pitching. This is the first study to demonstrate a relationship between KSs and elbow and shoulder torque production. As anticipated, the PDS KSs produced the least torque across the elbow and shoulder joints. Alterations in Distal Upper Extremity KS was most common and generated the greatest shoulder Ext torques. Alterations in the Proximal Upper Extremity KS demonstrated the greatest elbow valgus and shoulder ER. Further study of the influence of kinematic sequence on joint torques in the baseball pitch may provide insight into pitching injuries and injury avoidance programs.

Fine KSbOSiO4 (KSS) powders have been prepared by a sol-gel method starting from the system of TEOS, KSb(OH)6, H2O, C2H5OH, or mechanochemical mixing method with a stoichiometric mixture of amorphous silica and KSb(OH)6. The KSS crystallized sluggishly into a tetragonal symmetry in low temperature and transformed into an orthorhombic one in higher temperature, indicating the amorphous-like powder patterns in the intermediate step. This amorphization process could be related to some redistribution of the polysilicate network structures. The mechanochemical activation energy stored in the powder surface, as well as the coexisting matrix (KSbO3), retards the transformation. It was concluded that the KSS powders composed of high molecular weight siloxane polymers, which were formed by hydrolysis in the presence of a small amount of NH3 aqueous solution, showed higher sinterability. The sintering, where the calcining process was omitted, enabled the dense tetragonal-type ceramics stabilized at lower temperature, rather than higher temperature, to fabricate.

Objectives: Medial open wedge high tibial osteotomy is a widely used treatment method for early stages of medial compartment osteoarthritis, particularly in the middle aged patients. Correction of the malalignment of the knee and even distribution of the body weight through knee joint is considered to facilitate the regeneration of the damaged cartilage. Addition of a regenerative procedure to tibial osteotomies may enhance its potential effects. In this study we analyzed the effect of, if any, simultaneous use of BMAC with high tibial osteotomy. Methods: We have evaluated the clinical results of 46 knees of 38 patients that underwent open wedge high tibial osteotomy for early stage medial compartment arthrosis between 2017-2020. Patients were recruited into two groups as HTO only group (Group 1) and HTO+BMAC group (Group 2). Mean follow up time was 12.6 months. Preoperative International Knee Documentation Committee (IKDC) subjective score and Knee Society Score (KSS) of the patients were compared with those at the 1st, 3rd, 6th and 12th post-operative months. Wilcoxon Ranked Signed test was used to detect the significant difference. Results: Group 1 consisted of 18 female and 4 male patients, Group 2 consisted of 14 female and 2 male patients. Two patients in Group 1 received bilateral HTO and 6 patients in Group 2 received bilateral HTO+BMAC. Twenty four knees in Group 1 and 22 knees in Group 2 were included for evaluation. There was no significant improvement in the KSS and IKDC scores of the patients in Group 1 at the first follow-up, however at the 3rd month follow-up, KSS scores showed significant improvement (p<0,001) whereas IKDC scores did not show any improvement (p>0,005). Both KSS and IKDC scores showed significant improvement at the 6th and 12th months. (p<0,001). In Group 2, only KSS scores showed improvement at the 1st month follow-up (p<0,001). Both KSS and IKDC scores showed significant improvement at the 3rd, 6th and 12th month follow-up compared to preoperative scores. Conclusion: We believe that simultaneous use of BMAC with open wedge high tibial osteotomy for early stage medial compartment arthrosis may relieve pain at the early period and improve patient compliance to rehabilitation. Increase in range of motion promote the recovery period and provide rapid return to daily activities, thereby positively affecting cartilage regeneration.
