Abstract:
Objective The aim of this study was to evaluate the angiosome of the superior and inferior labial arteries within the superior and inferior labia and to describe superior and inferior labial musculomucosal axial pattern flaps that can be used for intra-oral wound reconstruction. This study also presents the clinical use of a superior labial musculomucosal flap in a dog. Materials and Methods The common carotid arteries of six canine cadavers were injected with barium sulphate. The skin of the face and labial mucosa was removed and radiographed to study the vascular supply of the superior and inferior labia. Results The angiograms in all dogs demonstrated that the superior and inferior labial arteries were located within the musculomucosal layer of the labia. At the junction of the rostral and caudal half of the upper lip, extensive choke anastomoses joined the angiosome of the infra-orbital artery. The inferior labial artery perfused the caudal half of the lower labium and had extensive choke anastomoses with the middle and rostral mental arteries. Clinical Significance The musculomucosal flaps of the superior and inferior labia contain a rich arterial blood supply, which suggests that these flaps may survive in live dogs. The superior labial musculomucosal flap was successfully used to reconstruct a large cleft palate in a single clinical case.

Abstract:
Labial fusion is defined as either partial or complete adherence of the labia minora. It is also known as vulvar fusion, labial adhesion, labial agglutination or synechia of the vulva. This condition is common in pre-pubertal girls when estrogen levels are low and commonly resolves spontaneously post-puberty. This condition is usually asymptomatic and can be treated with topical application of estrogen or betamethasone cream or by manual or surgical separation of adhesions. We present a case of labial fusion in post pubertal girl which was managed surgically.

Abstract:
Complete labial fusion with retention of urine is a rare clinical entity. It occurs when the labia are fused in midline, forming a raphe. It usually develops in postmenopausal women with hypoestrogenism. The usual treatment is with topical estrogen and surgical separation followed by some dilatation. Here we present a case of a 58 years’ postmenopausal,unmarried, nullipara woman who came to our emergency ward with retention of urine.She had history of incomplete voiding and dribbling of urine since six months. General examination and investigations were normal. Her clitoris was normal in size, and the labia majora and anus were visualized well; however, the urethral meatus and vagina were not seen because of fused labia minora.Patient required surgical excision of fused labial folds to relieve the retention.Keywords: Labial fusion; postmenopausal; urinary retention.

Labial fusion is sealing of labia minora in midline, also known as labial adhesion or labial agglutination or synechia vulvae. This condition is common in pre-pubertal females usually asymptomatic when oestrogen levels are low and commonly resolves spontaneously post-puberty if unresolved medical treatment includes use of estrogen cream or betamethasone cream application, very rarely surgical treatment required, if not responding to medical treatment due to dense adhesions. This case report is unusual as it has presented in a post-pubertal female requiring surgical management.

Abstract:
Abstract Background Penile inversion vaginoplasty (PIV) has become the most commonly performed genital surgery for transfeminine patients. However, patients undergoing this procedure may still require revisions to achieve a satisfactory result. Objectives The authors report on the utilization of autologous fat grafting to the labia majora to improve results after PIV and complications that may predict the need for grafting. Methods A retrospective chart review was conducted of a single surgeon’s patients who underwent PIV between July 2014 and December 2019. Demographic information, operative information, and postoperative outcomes were abstracted from the health records. Wilcoxon rank sum tests and chi-squared test were employed for continuous variables and categorical, respectively. Results A total of 182 transfeminine and gender-diverse patients underwent PIV, with 6 patients (3.3%) eventually undergoing labial fat grafting. The most common indication for labial fat grafting was flattened labia majora (83%). All fat grafting procedures were performed concurrently with other revisions of the vaginoplasty. There were no demographic or medical history differences detected between the fat grafting and non-fat grafting groups. Patients who underwent labial fat grafting were more likely to suffer from introital stenosis (33% vs 6%, P = 0.007) and prolonged granulation tissue greater than 6 weeks after initial vaginoplasty (83% vs 32%; P = 0.01). Conclusions Labial fat grafting is a safe and effective method to address defects in the labia majora after PIV. Prolonged granulation tissue and introital stenosis may predict the need for labial fat grafting, possibly due to increased scarring and contracture at the surgical site. Level of Evidence: 4

Abstract:
Labial tooth row variation was assessed in Dendropsophus labialis tadpoles from the Cordillera Oriental in Colombia. The presence of a second anterior labial tooth row was detected, showing thus a labial tooth row formula of 2/2. This second anterior tooth row was present in a substantial number of individuals, suggesting that the observed variation is not unusual. This variation contradicts earlier reports about the number of labial tooth rows in this species, whose labial tooth row formula has been reported as 1/2 without variation. A novel ontogenetic pattern of tooth row addition was detected, differing from earlier observations made on Pseudacris regilla tadpoles in terms of tooth row appearance order. The taxonomic implications for these data are discussed along with the evolution of oral morphologies in the genus.

On examination, she had well developed secondary sex characters but fused labia minora with a single pinpoint opening through which she was passing both urine and menstrual blood. Case. A 25-year-old woman presented with acute urine retention with overflow 6 months after an inadequate treatment of severe monilia infections. Examination revealed complete adhesion between both labia majora. Division of adhesion was done with reconstruction by labial mucocutaneous flap. Complete recovery was achieved with good cosmetic outcome.Conclusion. Labial adhesions whatever their severity is can be surgically divided with complete correction by locally designed flap to reconstruct the introuitus with rapid recovery, good healing, and good cosmetic outcome.

Abstract:
This article describes and analyses the frication of the voiced labial consonants (/b/ and /v/) in Changana, a Bantu language (S53, in Guthrie’s 1967-1971 classification). In the light of the autosegmental phonology (Leben 1973, 1978, 2006; 1973, Goldsmith 1976, 2004; Odden 1986) combined with the Feature Geometry theory, the article discusses phonological processes that turn voiced labials into labial-alveolar affricate [bz]. In this study, we assume that the process of hiatus resolution by gliding is the trigger of the alteration under analysis. That is, when derivative suffixes with low vowel (/a/) and the high front vowel (/i/) in the initial position are attached to words with rounded vowels (/o, u/) in final position in some morphological processes such as diminutivisation and locativisation, the results are undesirable sequences (hiatus). In order to resolve such hiatus, a series of phonological processes such as the turning of the rounded vowel in the word final position into labial-velar glide allowing the adjacency of voiced labials with labial glide which violates the Obligatory Contour Principle (OCP) takes place. The present study analyses the OCP using empirical Changana data collected both in the fieldwork supplemented by data from other sources including bibliographical and introspective data. The article is organised as follows. Firstly, it discusses the theoretical framework; secondly it analyses the Hiatus Resolution in Changana; thirdly, it analyses the data and lastly, it presents the main conclusions of the study.

Novas interfaces homem-computador têm sido pesquisadas a fim de se tornarem mais naturais e flexíveis. O rastreamento labial, foco deste trabalho, é parte deste contexto, podendo ser utilizado na detecção de emoções, bem como no auxílio ao reconhecimento de voz. Pode assim tornar-se um módulo inicial para a leitura labial, na criação de interfaces voltadas a deficientes auditivos. Os algoritmos disponíveis na literatura foram analisados e comparados, mostrando os prós e contras de cada método. Finalmente foi escolhido desenvolver uma técnica baseada em Active Appearance Model (AAM). O AAM gera um modelo a partir de um conjunto de imagens de treinamento, que pode ser utilizado no rastreamento labial de novas imagens. A técnica proposta baseia-se no uso de algoritmos genéticos para o ajuste do modelo, diferente, portanto, da técnica proposta originalmente pelo AAM. A convergência da técnica proposta foi extensivamente analisada, com a variação de parâmetros, buscando a análise de erro residual da função custo e sua relação com o tempo de convergência e erro de posição.
New human-computer interfaces have been researched to make them more natural and flexible. The lip tracking, emphasis of this work, is part of this setting it can be used for detecting emotions and to aid the speech recognition. It may well become an initial module for lip-reading, in the creation of interfaces directed to hearing impaired people. Algorithms available on literature are analyzed and compared, explaining the upsides and downsides of each method. Finally was chose to develop a technique based on Active Appearance Model (AAM). The AAM generates a model from a set of training images, which can be used in tracking of new lip images. The proposed technique is based on the use of genetic algorithms for fitting the model, different therefore of the technique originally proposed by AAM. The convergence of the proposed technique has been extensively analyzed with the variation of parameters, searching the error analysis of the cost function residual error and its relation to the execution time and position error.

La anatomía de la fisura labial es diferente para cada paciente y varios autores han descrito modificaciones de técnicas tradicionales en la búsqueda de obtener un diseño más individual con mejores resultados. La técnica de Millard es probablemente la técnica quirúrgica más usada por los cirujanos alrededor del mundo en el manejo de la fisura labial unilateral, sin embargo existen algunas limitaciones en esta técnica en fisuras completas con deficiencia de tejidos en el segmento lateral. En ese sentido, se diseño la presente técnica que busca corregir de manera paralela la deficiencia en el segmento lateral de la fisura labial unilateral completa. Esta técnica se basa en una doble rotación y avance localizando las cicatrices sobre las líneas naturales del labio entre las unidades estéticas del mismo. El presente estudio evalúa la simetría labial obtenida con el uso de esta técnica en el tratamiento quirúrgico de las fisuras labiales unilaterales completas. METODOS El presente es un estudio observacional analítico de cohorte prospectivo. Se evalúa la utilidad de una nueva técnica en el tratamiento quirúrgico en 30 pacientes consecutivos con diagnóstico de fisura labial unilateral completa luego de un año de la operación realizando las medidas antropométricas pre y postoperatorias. El test Wilcoxon, la t de student y un nivel de confidencia de 95 %, fueron utilizados para establecer la significancia de las diferencias entre el lado sano y el lado fisurado (p < 0.05). RESULTADOS El estudio no encontró diferencias significativas entre el lado fisurado y el no fisurado (sano) en la altura labial (p: 0.310), ancho labial (p: 0.425) y altura del bermellón (p: 0.283) después de la cirugía labial utilizando esta técnica. Sin embargo hubo diferencias significativas entre el lado sano y el fisurado en el ancho de la base alar (p: 0.01). CONCLUSIONES Se observó la obtención simetría labial en casos de fisura labial unilateral completa con el uso de la técnica de doble rotación y avance superior. ---The anatomy of the cleft lip is different for each patient and many authors have been described modifications of traditional techniques in order to obtain a more individual design and better results. Millard’s technique is probably the most common surgical technique used by surgeons around the world, however there are some short-comings to repair complete clefts with deficiency of tissues of the lateral segment. In that way, we have developed a new technique which corrects the medial and lateral lip segment’s deficiency in complete unilateral cleft lip. This technique is based on a double rotation advancement concept placing the scars over the natural lines between the aesthetic subunits of the upper lip. This study evaluates the symmetry obtained with the use of this technique in the surgical treatment of the complete unilateral cleft lips. METHODS. This is a prospective analytic observational cohort study. It evaluates the utility of a new technique for surgical treatment in 30 consecutive patients with complete unilateral cleft lip after one year of the surgery making the pre and postoperatory anthropometric measurements. The Wilcoxon and t-student tests and level of confidence of 95 % were performed to assess the statistical significance of differences between the cleft side and non-cleft side (p < 0.05). RESULTS This study found no statistically significant differences between cleft side and non-cleft side lip height (p=0.310), lip width (p=0.425) and vermilion height (p=0.283) after lip repair using this technique. However there were significant differences between cleft side and non-cleft side nasal base width (p: 0.01). CONCLUSIONS. We observed the lip symmetry obtained in cases of complete unilateral cleft lip with the use of the upper double rotation and advancement technique. Key words: Unilateral cleft lip Surgical technique

A Reconstrução Facial Forense (RFF) é uma área da Antropologia Forense que busca aproximar a aparência de um indivíduo desconhecido por meio de reconstrução dos tecidos moles sobre seu crânio, após seu estudo antropológico. A RFF divulgada nos meios de comunicação visa a um reconhecimento, que pode desencadear um processo de identificação humana. Conhecer as relações entre tecidos duros e moles é essencial para se aumentar a precisão das RFFs. Neste estudo, buscou-se reunir informações antropométricas sobre a boca e suas proporções. Foram constituídas duas amostras a partir de um acervo de tomografias computadorizadas. A amostra 1 (n=327) consistiu em homens (n=127) e mulheres (n=185) entre 11 e 81 anos, divididos em seis faixas etárias e buscou verificar as distâncias lineares delimitadas por pontos anatômicos em tecido mole: altura da zona vermelha da boca, largura da boca, proporção entre os mesmos e possibilidade de se estimar a largura da boca a partir da distância intercanina. A amostra 2 (n=108) consistiu em homens (n=40) e mulheres (n=68) entre 20 e 81 anos, divididos em três faixas etárias e buscou verificar as proporções entre altura da zonas vermelha da boca, largura da boca e distâncias lineares entre pontos craniométricos em tecidos duros. As medições foram feitas com o programa OsiriX e os resultados foram analisados através de através de estatística descritiva para todas as variáveis estudadas, por meio da comparação entre as médias, os desvios padrão e as diferenças entre as médias, com intervalo de confiança de 95% (IC95%) e aceitando nível de significância de 5% (p<0,05). Para a amostra 1, a altura da zona vermelha correspondeu a aproximadamente 26% da largura da boca, em ambos os sexos, com tendência a diminuir ao longo do tempo. A largura da boca aumentou ao longo do tempo em homens e manteve-se estável em mulheres. À distância intercanina se atribuiu o valor médio de 75% da largura da boca, em homens, e 80% da largura da boca em mulheres. Para a amostra 2, à distância entre os pontos infradentale e supramentale se atribuiu em média 55% da altura da zona vermelha para ambos os sexos, e entre os pontos philtrum medium e supradentale, em média 85% da altura da zona vermelha em homens, e 88% da mesma medida em mulheres. À distância entre os forames mentuais, se atribuiu em média 97% da largura da boca em mulheres, e em homens 93% deste valor. A reunião de dados antropométricos da boca para a população brasileira pode contribuir para aumentar a precisão das RFFs e auxiliar a identificação humana.
Forensic Facial Reconstruction (FFR) is an area of Forensic Anthropology that seeks to approximate the appearance of an unknown individual by reconstructing soft tissues over ones skull after an anthropological analysis. The FFR published in the media aims at a recognition, which can trigger a process of human identification. Knowing the relationship between hard and soft tissues is crucial to increase the accuracy of FFRs. In this study, we sought to gather anthropometric information on the mouth and its proportions. Two samples were taken from a collection of CBCT scans. Sample 1 (n=327) consisted of men (n=127) and women (n=185) aged between 11 and 81 years old, divided into six age groups and verified the linear distances defined by anatomical landmarks in soft tissue: vermillion height, mouth width, the proportion between these and the possibility of estimating mouth width from the intercanine distance. Sample 2 (n=108) consisted of men (n=40) and women (n=68) aged between 20 and 81 years old, divided into three age groups, and verified the proportions between the vermilllions of the mouth, mouth width and linear distances marked in bony craniometrical landmarks. The measurements were made in OsiriX software and the results were analyzed by averages, standard deviations and differences between the averages. For sample 1, vermillion height corresponded to approximately 26% of mouth width, in both genders, with a tendency to decrease over time. Mouth width increased over time in men, and kept stable values in women. The intercanine distance corresponded to about 75% of mouth width in men, and 80% of the mouth width in women. For sample 2, the distance between the landmarks infradentale and supramentale and was on average 55% of the vermillion height in both genders, and the distance between the landmarks philtrum medium and supradentale was about 85% of the vermillion height in men and about 88% of the same measure, in women. The distance between the mental foramina was on average 97% of the mouth width in women, and 93% of this distance in men. Gathering anthropometric information on the mouth for the Brazilian population can contribute to increase the accuracy of FFRs and assist human identification.

Abstract:
Determina las características mucogingivales del frenillo labial superior en pacientes adultos atendidos en la Clínica de la Facultad de Odontología de la Universidad Nacional Mayor de San Marcos. La muestra estuvo constituida por 138 pacientes de 18 a 55 años. Se evaluó los tipos de adherencia, las variantes anatómicas y la longitud de inserción del frenillo labial superior; así como también, el fenotipo gingival y posición de la papila interdental de los incisivos centrales superiores. Los datos fueron sometidos a las pruebas de correlación de Pearson, Kruskal Wallis y Mann-Whitney. Se encontró que el tipo de adherencia y las variantes anatómicas del frenillo labial superior presentan asociación estadísticamente significativa con la posición de la papila interdental de los incisivos centrales superiores (p=0,038) y (p=0,015) respectivamente; sin embargo, en relación al fenotipo gingival de los incisivos centrales superiores no se encontró asociación estadísticamente significativa con las características del frenillo labial superior. Abstract:
Este trabajo busca estudiar la materialidad del lápiz labial para analizar su rol en la creación de la apariencia femenina. Es una continuación de la Tesis de Licenciatura del mismo nombre; sin embargo, en esta fase el análisis está enfocado en el aspecto material del labial, así como en la dimensión corporal y en los discursos que atraviesan saberes y prácticas vividas a través del labial. El trabajo de campo principal se realizó dentro de la exposición museal “LÁPIZ LABIAL: IDENTIDAD, PRESENTACIÓN Y EXPERIENCIAS DE LA FEMINIDAD”, que se llevó a cabo del 4 al 24 de marzo de 2013 en la Galería de Arte Domingo Pantigoso. En consecuencia, la metodología estuvo centrada en la información recogida en la muestra. Además, se empleó datos provenientes del trabajo de campo realizado para la Tesis de Licenciatura sobre el mismo tema. Dicha información consta de entrevistas individuales y grupales, auto etnografía, aparte de análisis de imágenes provenientes de la publicidad y de registro fotográfico ejecutado durante los encuentros con los informantes. Luego de analizar es aspecto material del lápiz labial como objeto cultural, y la corporalidad de la usuaria femenina, se concluye que el lápiz labial no representa ser mujer, ES ser mujer: éste produce, y al mismo tiempo es identidad femenina. Es una herramienta para poner en un plano sensorial representaciones de la feminidad, que se diferencian a través de discursos transversales. En la aplicación del labial es que se concreta la producción de una nueva materialidad. El cuerpo no es un lienzo en blanco, sino que a su vez es un objeto transformador y que se transforma para incorporar discursos. Los saberes que representan a la industria cosmética están entonces en constante relación con la transformación del rostro para proveer de una apariencia acomodada al modelo de belleza imperante, y a los ideales de realización y belleza natural. El labial delinea las fronteras de lo que consideramos ser mujer en nuestra sociedad, y demuestra qué prácticas son permitidas y cuáles no.

Abstract:
Le vitiligo est une leucodermie affectant 0,5 à 1% de la population mondiale. Il n’y a pas de différence de prévalence concernant l’âge, le sexe ou le type de peau. La disparition des mélanocytes entraine une hypopigmentation localisée en plaques symétriques blanches ivoire, à bords nets souvent hyperpigmentés. Les lésions sont le plus souvent retrouvées au niveau des zones découvertes, de frottement et des extrémités. L’évolution des lésions est imprévisible, alternant des phases de développement et de quiescence. Le mécanisme physiopathologique est mal connu, il s’agirait d’une pathologie auto immune avec une prédisposition héréditaire sur un terrain psychologique affaibli. On le retrouve associé dans d’autres affections auto immunes : l’insuffisance surrénalienne, les pathologies thyroïdiennes et la maladie de Biermer (Nagarajan et al (2015)). Le cas clinique rapporté est celui d’une jeune femme de 17 ans, qui présentait une dépigmentation de la lèvre supérieure apparue en octobre 2016. Initialement, la lésion affectait l’hémi lèvre supérieure gauche. Le dermatologue avait posé le diagnostic de vitiligo et instauré un traitement par vitamine C et acide folique, suivi pendant un mois, sans résultat. En juillet 2017 la patiente a consulté en pathologie de la muqueuse buccale car la lésion s’était étendue à l’ensemble de la lèvre supérieure avec atteintes de la commissure labiale gauche et cutanée en regard. L’interrogatoire a révélé un mordillement chronique des lèvres. L’examen de la muqueuse buccale a mis en évidence une dépigmentation linéaire de du bord vermillon de la lèvre supérieure avec renforcement pigmentaire périphérique. L’examen cutané a révélé une plage dépigmentée centimétrique de l’auriculaire de la main droite. Un bilan biologique incluant thyréostimuline, anticorps anti thyroglobuline et anticorps antithyroperoxydase a été prescrit de façon systématique. Il n’a pas révélé d’anomalie. Le traitement prescrit était : tacrolimus à 0,1% en application locale biquotidienne et arrêt de la pathomimie. Le vitiligo des muqueuses buccales est rare. Il a essentiellement été décrit en Inde où la maladie est endémique (Nagarajan et al (2015)). L’atteinte des muqueuses orales concernerait 55% des patients et la lèvre serait touchée dans près d’un cas sur 2 dans cette population. Le cas présenté concernait une patiente originaire d’Afrique du nord. Les études concernant le traitement du vitiligo labial ont été menées uniquement sur le traitement chirurgical : micropigmentation et greffes de mélanocytes semblent avoir le plus fort taux de succès (Gupta et al (2006)). Rodrigues et al (2017) ont conseillé une application locale biquotidienne de tacrolimus 0,1% pour les affections de la face et des zones intertrigineuses. De par son mode d’action, le tacrolimus a une activité immunosuppressive et pourrait favoriser la pigmentation de la muqueuse orale (Fricain et al 2005). Dans le cas présenté, un traitement par tacrolimus a été instauré dans un premier temps. Un traitement chirurgical par greffe de mélanocytes ou un traitement cosmétique sera proposé en cas d’échec du traitement local. Bien que rare, le vitiligo de la muqueuse buccale ne doit pas être ignoré du chirurgien oral qui devra collaborer avec le dermatologue pour définir le traitement adapté.
