Abstract A vast staff served the asylums: doctors, nurses, attendants, artisans, clergy, kitchen and laundry workers, and other who maintained buildings, farm, gardens and cemetery. Ward work was particularly demanding, with long hours, and poor conditions of employment. A regimented and punitive culture and distrust between management and lower ranks of staff, contributed to “a general feeling of insecurity” among them. They had high rates of sickness and a high turnover, sometimes over 75 per cent annually, resulting in an inexperienced workforce. The asylum leadership contributed to creating a dysfunctional system in which the patients, whom the asylum was meant to serve, were far from central to it.

AbstractAlthough the social structure of Turkish society has changed from a broad family order to a nuclear family, family relations still hold an important place, where traditional elements dominate. Still, elderly people are cared for by their family in their home environment. Thus, the role of family members is crucial in taking care of elderly individuals. In Turkey, the responsibility of care is largely on women; the elderly’s wife, daughter, or daughter-in-law most often provides the care. Family members who provide care need support so that they can maintain their physical, psychological and mental health. At this point, Antonovsky’s salutogenic health model represents a positive and holistic approach to support individual’s health and coping. The salutogenic understanding of health emphasizes both physical, psychological, social, spiritual and cultural resources which can be utilized not only to avoid illness, but to promote health.With the rapidly increasing ageing population globally, health expenditures and the need for care are increasing accordingly. This increase reveals the importance of health-promoting practices in elderly care, which are important for the well-being and quality of life of older individuals and their families, as well as cost effectiveness. In Turkey, the emphasis on health-promoting practices is mostly focused in home-care services including examination, treatment, nursing care, medical care, medical equipment and device services, psychological support, physiotherapy, follow-up, rehabilitation services, housework (laundry, shopping, cleaning, food), personal care (dressing, bathroom, and personal hygiene help), 24-h emergency service, transportation, financial advice and training services within the scope of the social state policy for the elderly 65 years and older, whereas medical management of diseases serves elderly over the age of 85. In the Turkish health care system, salutogenesis can be used in principle for two aims: to guide health-promotion interventions in health care practice, and to (re)orient health care practice and research. The salutogenic orientation encompasses all elderly people independently of their position on the ease-/dis-ease continuum. This chapter presents health-promotion practices in the care of elderly home-dwelling people living in Turkey.

One way to show our understanding of ancient Greek management of water is to follow an ordinary person in her daily patterns, observing when and how she uses water. This schedule ignores differences that derive from local geology, climate, or customs, but rather tries to set out the common patterns. 1. At daylight. Wake up. Go to room or alcove set aside for excreting and do that. Rinse with previously used water. Then go to courtyard, pull up bucket of water from cistern, pour into louter, and wash face and hands. Save water for re-use (Fig. 13.3). 2. First meal. Fix breakfast, using water from cistern for any cooking. Water donkey, dog, house plants, with water from cistern or re-usable water from cooking or bathing. 3. Work. Morning and mid-afternoon to late day: A. Do family laundry—use giant pithos or scrub-board at edge of courtyard, filled from downspout from roof or with buckets of water from the cistern; hang clothes to dry on poles or rope strung between posts (columns) supporting roofs around courtyard. Alternate: laundry might be done communally at a large tank that received the overflow from a fountain near the agora, and the wet clothes carried home and spread out to dry, as above (Fig. 17.5). B. Or do craft activity such as making pottery, using courtyard and water from cistern. C. Or go out to farm. Excrement and garbage were probably carried daily to the farm for fertilizer. An important farming task was to monitor the irrigation of timber lots, fields, orchards, and vineyards with waste water from the town or with spring or river water or dispersed rainwater. D. Or do shopping and/or selling. Periodically carry craft items to Agora to sell them. If need be, rinse items such as vases in public fountains to show off their best colors. In Athens, women participated in the markets, selling lettuce and other farm or craft products, but in some Greek cities shopping and selling were solely masculine activities. 4. Recreation. A. Talk with cousin from the country who waters his donkey at the public trough in the Agora. B. On the way home stop at neighborhood fountain to chat with other people fetching water to drink (Fig. 21.1). C. On special occasions (marriage, birth) go to a sanctuary for a ritual bath. (Fig. 6.1).

In response to government and public pressures, the healthcare industry has in the past few years directed a significant effort toward the proper and safe management of its medical waste streams. Medical waste is classified as a biohazardous waste, which according to a study published by the United States Agency for Toxic Substances and Disease Registry (1990), may result in human infection and transfer of disease. This includes injury and infection with the Hepatitis B Virus (HVB) and the Human Immunodeficiency Virus (HIV), by janitorial and laundry workers, nurses, emergency medical personnel, and refuse workers who may come into contact with medical waste. In a recent survey conducted in the United States and Japan, and reported by the World Heath Organization (WHO) (1994), it was found that injuries by sharps constitute about 1% to 2% per annum for nurses and maintenance workers and 18% per annum for outside waste management workers. In Japan, the survey indicated that injuries by sharps constitute about 67% for in-hospital waste handlers and 44% for outside waste management workers. In order to reduce the risks associated with medical waste, proper management mechanisms should be adopted by healthcare facilities to protect the health of the staff within the medical facility, waste collectors/workers, and the public once the waste has left the facility for final disposal. These mechanisms include waste identification, segregation, storage, and treatment. However, and as a first step in the implementation of a waste management system, the management of a medical facility should conduct an audit of the generated waste streams.

This chapter begins by tracing the origins of the anti-kleptocracy cause in the United States, starting with the harsh Cold War environment and the Foreign Corrupt Practices Act of 1977. It explores the status quo ante of dictators being able to launder their funds in the US financial system with impunity immediately before and after the turn of the century. At this time, there was no law prohibiting American banks and other institutions receiving the proceeds of foreign corruption. The USA Patriot Act closed this legal loophole, yet practice lagged, and laws at first failed to have much of an impact. More recent cases indicate at least partial effectiveness, however, with instances of successful prevention and some looted wealth confiscated and returned.
