Abstract:
New medications can provide substantial benefits, but high prescription drug prices have led to calls to contain costs. Even after accounting for discounts and rebates, average prices of leading brand-name drugs in the United States are two to four times higher than in other wealthy countries, raising questions about what these higher prices are buying us. With the advent of ever more targeted and powerful treatments, including cell- and gene-based therapies with multimillion dollar price tags, the need for sensible drug pricing policies will intensify. Price controls, common in other countries, seem appealing, but these measures can discourage innovation. Moreover, on what basis should policymakers develop such controls? This book argues that pricing prescription drugs to reflect the value they bring to patients, families, and society achieves the right balance. The book reviews the distinguishing features of the prescription drug market and explains why simple solutions like price controls and importing drugs from countries with lower drug prices are problematic without explicit assessments of value. It then describes how economists measure value, how value assessment for drugs is now being used in the United States, and what must happen going forward to overcome challenges.

Abstract:
This chapter examines three health determinants that lie inside of individuals’ own control: diet, physical activity, and substance use. The choices exhibited over these three classes of goods (or bads) have fundamental impacts on the health of the individual. Under each of them, three types of policy interventions are discussed: prices, regulations, and ‘nudges’. Generally, price interventions such as indirect taxation and subsidies are useful when demand is responsive to price changes, that is, on price-elastic goods. Regulations are required when consumers do not respond much to price changes, that is, for price-inelastic goods. The use of ‘nudges’ is based on the insight from behavioural economics that consumers are sensitive to how their choice sets are being presented. Through ‘choice architecture’ policymakers can help individuals make healthy choices rather than being victims of a short-sighted weak will.

This chapter is a general overview of the economic aspects of professional team sports leagues as well as the American economy. The NBA's turbulent birth as the BAA demonstrated that professional sports team owners' twin advantages of price-setting power over ticket prices and enhanced bargaining power over players were not sufficient conditions to ensure profitability. Price-setting power without sufficient demand could still lead to losses. The challenge was to increase demand, which would have led to higher ticket prices, more attendance, and greater revenues and profits. Greater profits would have enabled owners to pay higher salaries and to improve conditions, helping to erase any fly-by-night image. Thus the chapter looks at the issues surrounding profits, player salaries, technology, expansions, discrimination, and so on; as well as how the American economy performed during the years 1945–61 and how it affected attendance and demand for professional team sports leagues.

In 1494 King Charles VIII of France (1483–98) decided to extend French influence southwards and led a force of 30,000 men into Italy. The invasion was followed by interventions by his successors Louis XII (1498–1515) and Francis I (1515–47), as well as by the sporadic involvement of the Spanish King Ferdinand of Aragon (1479–1516) and by the emperor-elect Maximilian (1493–1519), and finally Emperor Charles V (1519–58), which drew parts of Italy into the Spanish empire. These monarchs clashed bloodily at battles such as Fornovo (1495), Agnadello (1509), Ravenna (1512), Novara (1513), Marignano (1515), La Biccoca (1522), and Pavia (1525). For these princes Italy offered a rich prize, for many Italian princes the alliances with the ultramontane powers provided opportunities to extend their power, but for many Italian civilians the wars were a disaster.

The great challenge posed by the construction of a state based around a noble and princely structure led first of all to confrontation with those republican ideals that continued to guide large and small communities in their day-to-day operation. Once again, the Visconti and the Sforza did not merely set about seeking a peaceful modus vivendi, but attempted to dismantle competing cultures through the development of an alternative model of social organization, based on distinctions among men. Communitary egalitarianism was opposed by the valorization of individual excellence, which the prince acknowledged on the basis of status, wealth, and local influence. This resulted in a frontal attack on the political subjectivity of the communities, which, however, reacted in different ways in the cities and in the countryside. This chapter focuses on these aspects.

This book shows how the work of a major post-war American poet has been centrally concerned with questions of national identity and intercultural poetic exchange, by reading crucial episodes in John Ashbery’s oeuvre in the context of an ‘other tradition’ of twentieth-century English poets he himself has defined. This line runs from the editor of Ashbery’s recent Collected Poems, Mark Ford, through Lee Harwood in the late 1960s, F. T. Prince in the 1950s, to ‘chronologically the first and therefore most important influence’ on his own work, W. H. Auden. Through detailed close readings of the poetry of Ashbery and these English poets, original interviews, and extensive archival research, a new account of Ashbery’s ‘minor’ aesthetic and a significant re-mapping of postwar English poetry are presented. The biographical slant of the book is highly significant, as it reads these writers’ poetry and correspondence together for the first time, suggesting how major poetic innovations arose from specific social contexts, from the particulars of relations between poets, and also from a broader climate of transatlantic exchange as registered by each poet. The result is that both Ashbery himself, and the landscape of post-war English poetry, are viewed in a significantly new light.

This chapter concerns the work of writers who proclaim their commitment to a readership of commoners: craftspeople, tradesfolk, domestic servants, and others below the rank of the gentry. In doing so, the chapter reveals the voracious appetite of the marketplace of print for copy. It draws attention to the competing interests of printers and considers the question of how to make a living by writing under these circumstances. Writers experimented with different methods of turning the copy they produced into a steady income, but many failed. However, the attempt led to the extraordinary variety of prose pamphlets (short, inexpensive books) printed in the 1580s and 1590s.

This chapter examines the relationship between print, popular political mobilization (crowd action, street politics, and mass petitioning), and the high politics of the Long Parliament. It examines the street demonstrations and crowd actions of winter 1641–2, focusing on the ways that print was used to organize, propagandize, or channel mass crowd activity and the ways those popular political mobilizations were inseparably linked to maneuver in parliament itself. The contested politics of the period—and in particular battles between the two houses of parliament over efforts to protect against a royal reaction—led to significant ideological escalation, as some backers of parliament began to question the function of the peerage, the negative voice of the king, and other constitutional conventions. The printed expression of these radical political impulses, coupled with the threat of violence from the crowds, fed a process of polarization, contributing to a growing royalist countermovement.

Au programme, Électre de Sophocle. La porte s’ouvre, Électre apparaît. C’est la fille de Clytemnestre et d’Agamemnon. Clytemnestre a assassiné Agamemnon à son retour de Troie, après dix ans de guerre. Elle règne aux côtés de son amant, Égisthe. Électre vient hurler sa peine devant le palais de Mycènes, face aux 12 000 spectateurs athéniens. Elle prend à témoin le jour qui se lève, rappelle le meurtre affreux de son père, invoque les déesses de la vengeance. Sophocle la représente ainsi, accablée par le deuil. Grâce à une tradition longue de 2 400 ans, nous connaissons les mots d’Électre prononcés ce jour-là : ceux de l’héroïne, ceux du poète, ceux de l’acteur derrière le masque. Nous avons le texte qui nous permet d’imaginer les gestes. Et de là les effets de scène, l’émotion des spectateurs, le spectacle vivant. Le présent ouvrage part à la recherche de ces gestes perdus. Entre les lignes des trois « Électre » d’Eschyle, de Sophocle et d’Euripide, il décèle des gestes de différentes natures : jeu et danse du comédien ; actes de parole d’Électre qui prie, se lamente, jure, maudit ; figures stylistiques par lesquelles le poète donne corps au texte… C’est une lecture qui mobilise aussi bien l’oreille que le regard. This chapter describes fiscal squeeze in an era of high political volatility and major economic challenges, including mass unemployment, a sharp increase in oil prices, double-digit inflation (i.e. a period of ‘stagflation’), and high levels of trade union militancy. The most dramatic period during the episode occurred in 1976, involving a split Labour Government under two different leaders, with a leadership election following a sudden prime ministerial resignation. That government pursued fiscal squeeze against the background of a deep currency crisis and bailout deals with outside lenders (the US Government and the IMF). The squeeze episode also led to some important institutional developments, producing the first major privatization since the 1950s and a new system of controlling public spending through ‘cash limits’.

This book offers a re-evaluation of the last year of the Fronde—the political upheaval between 1648 and 1652—in the making of seventeenth-century France. In late December 1651 cardinal Mazarin defied the order for his perpetual banishment, and re-entered France at the head of an army. The political and military crisis that followed convulsed the nation, and revived the ebbing fortunes of a revolt led by the cousin of the young Louis XIV, the prince de Condé. The book follows in detail the unfolding political and military events of this year, showing how military success and failure swung between the two sides through the campaign, driving both cardinal and prince into a progressive intensification of the conflict, while simultaneously fuelling a quest for compromise and settlement which nonetheless eluded all the negotiators’ efforts. The consequences were devastating for France, as civil war smashed into a fragile ecosystem that was already reeling under the impact of the global cooling of the ‘Little Ice Age’. 1652 raises questions about established interpretations of French state-building, the rule of cardinal Mazarin and his predecessor, Richelieu, and their contribution to creating the ‘absolutism’ of Louis XIV.

Abstract:
Ce deuxième tome de l’impensé numérique, qui vient compléter le premier tome paru en 2016, participe au développement d’une pensée critique du numérique que le directeur de cet ouvrage collectif a engagée voilà 25 ans maintenant. Il marque en quelque sorte un anniversaire, celui d’une réflexion au long cours sur l‘informatisation de la société. Ce temps long de la recherche se révèle, notamment, dans la première partie qui vise à élaborer, reprendre et affiner le cadre conceptuel de ce travail. On y revient, à nouveaux frais, sur les notions d’impensé informatique et numérique, on y présente les notions de « glissement de la prérogative politique » (GPP), qui souligne la prise en main par des acteurs privés de prérogatives jusque là dévolues au politique et de « gestionnarisation », qui désigne le processus qui met en avant la technique (gestion et numérique) et ses catégories au détriment des activités qui doivent s’y adapter. Ce travail de construction théorique, qui mobilise aussi les notions de confiance, d’usage, d’imaginaire et de reconnaissance, s’est déployé sur une bonne quinzaine d’année. Nous n’avons pas voulu supprimer cette épaisseur temporelle, qui fait pleinement partie du travail de recherche lui-même. Les deuxième et troisième parties font le point sur ce que l’on peut appeler une pragmatique de l’impensé : à savoir, comment il s’installe très concrètement aussi bien dans le mode de fonctionnement et d’architecturation d’internet, que dans nos plateformes et dans la manière dont elles transforment le jeu médiatique, à travers, également, l’instauration d’une nouvelle monnaie (le Bitcoin) et de son support technique (la blockchain) ou, enfin, par le biais du big data. L’impensé, en ce sens, n’est pas seulement un effet de discours, il est aussi un effet, pratique, de structuration du réel qui a pour conséquence de fermer des espaces de discutabilité. Ce qui est vrai à l’échelle stratégique de la deuxième partie l’est tout autant à l’échelle tactique, plus locale, qu’adopte la troisième partie : car l’impensé est tout autant au travail dans les discours performatifs de l’éducation, dans celui de la vulgarisation technique de l‘informatique que dans ceux qui structurent les espaces numériques de la culture. Un dernier texte ouvre sur une proposition technique qui s’appuie sur une réflexion critique, afin de montrer que celle-ci n’est pas seulement négative ou supposément technophobe, mais qu’elle peut également nourrir un dispositif technique innovant. La conclusion s’interroge sur la persistance des conditions de possibilité du développement d’une véritable posture critique face à ceux que l’on peut appeler les impenseurs. Elle offre également un petit kit pédagogique de présentation de l‘impensé, du GPP et de la gestionnarisation pour que la critique argumentée puisse, peut être, être mieux entendue. Abstract:
This chapter describes Bob Fosse’s first four Broadway shows, which he worked on with George Abbott or Jerome Robbins, both of whom proved important mentors to the young choreographer. This high-level apprenticeship served as a foundation for every Fosse show that followed. With The Pajama Game, his distinctive style was already fully developed as exemplified in “Steam Heat.” Damn Yankees introduced Fosse to Gwen Verdon, a former Jack Cole dancer whose performance of Fosse’s choreography for “Whatever Lola Wants” proved indelible and led to a professional and personal association that would last the rest of their lives. On New Girl in Town, a musicalization of Eugene O’Neill’s Anna Christie, Fosse fought with Abbott and producer Harold Prince over his “Red Light Ballet,” which they deemed censorable. The clash led Fosse to sever his ties with both men and determine that he would follow Robbins’s example and direct his next show.
