Background: Many children in the world today grow up under very challenging circumstances. In Lesotho, the issue of HIV/AIDS, food-insecurity and poverty has caused the country several problems and statues a threat to the wellbeing of its young inhabitants. In those circumstances, adolescents’ capacity to face these challenges becomes a great part of the nurses’ work in the country. Aim: The aim of this study is to describe nurses’ experiences from strengthening capacity in vulnerable adolescents in a southern African nursing context. Method: Four qualitative, semi-structured interviews was conducted with nurses working at two different health care clinics for youth in Lesotho. The findings were analyzed through qualitative content analysis. Results: In the results, Resilience was identified as a core theme. This core theme was found to be encircled by four main themes: Nursing care, Strengthen capacity, Identify the whole and Challenges. Conclusion: Resilience was a concept used by all interviewed nurses and also the over-all goal of various nursing interventions. By using a holistic approach, the nurses viewed the whole person in his or her cultural and familial context. Thereafter different methods and techniques were used to build capacity in the adolescents and help them to face and overcome difficulties. Clinical relevance: Describing how nurses can help adolescents to build capacity can inspire nurses in all health care settings to implement nursing interventions and hence build resilience.<br><p>Röda Korsets sjuksköterskeförening stipendium 2015</p>

Abstract:
This thesis uses an ethnographic study to interrogate the policy discourse of capturing ‘child voice’ specifically in relation to a ‘looked after’ child. In recent years, attempts have been made to involve children who are ‘looked after’ in discussions and decisions about their care arrangements to ensure that their voice is heard. To ensure this happens, children ‘in care’ are asked about their care placement regularly as part of the care planning review process and their views are incorporated into decisions about their care plan. This study focuses on the lived experiences of a seven-year old female child, who I have referred to as ‘Keeva’, who is ‘in care’ under a Kinship Care arrangement. Over a period of a year, I was based in Keeva’s home one afternoon a week to gain insights about her lived experience as a ‘looked after’ child and how she represented herself. I also observed three care planning review meetings to see how her voice was captured by those charged with her care and how she was represented. I relate Keeva’s experience through seven narrative episodes to capture the rich complexity of the social world she inhabits. I explore aspects of her home and family, her interactions with others and her experience of exploring physical spaces both inside and outside the home. I suggest that these experiences underpin her sense of self and how she relates to others. Drawing on the ideas of Bourdieu, I suggest these experiences and her sense of place in the social order write themselves ‘onto her’ through her habitus and dispositions. Using a Foucauldian lens, I problematise the notion of voice as I contest that the child I observed engaged fully in the statutory processes that surround her. I suggest Keeva, a child who is ‘looked after’, will neither have nor feel she has the agentive properties to influence the care planning process. Instead, as her voice is irrevocably bound up in a bureaucratic process that is uncritically accepted as representative of her, she is obscured as a consequence. I also examine the multivocity in representations of Keeva highlighting the competing discourses of safeguarding, child protection and the ’rights-based’ agenda. I conclude that Keeva was not well represented in care planning reviews and had very little influence in decision-making about her care plan. Despite believing the opposite, those charged with her care failed to hear her or take note of what she said. Furthermore, there was an absence of criticality in representations of Keeva allowing Keeva to be constructed by those professionals involved with her care, in an unchallenged way. As a consequence she was silenced and less visible than the process itself.

<p> This study used a phenomenological research design to investigate how process features of child care quality are experienced in Hmong licensed family child care settings. Interviewing with open-ended questions was relied on as the data collection method. The results included eight major themes: playtime and learning activities are structured to promote individual child&rsquo;s cognitive and physical development; the child care program is founded on licensing standards of health, safety, and well-being; child care provider uses child-centered interactions in their involvement with the children; child care provider uses positive behavioral guidance in shaping children&rsquo;s behaviors; fostering a bi-cultural identity in Hmong children through different approaches; the relationships between the child care providers and children are strengthened by a kinship-based-relationship and a display of affection/love and care; feeding practice is adjusted to meet the needs of each child; and the child care provider seeks support to improve his/her child care practice. The findings from this study revealed that as Hmong, the child care providers appear to perceive themselves as a bridge that connects to the next generation, and it is their moral responsibility to instill a strong foundation in cultural identity, while providing the necessary academic preparations in the Hmong children in order for those children to be a bridge for another Hmong generation. The success of bridging from one generation to another will ensure the preservation of the Hmong culture, as well as the future success of the Hmong. These results can have important implications for early childhood professionals and organizations, as well as for the Hmong community.</p><p>

Abstract:
The increasing involvement of mothers in paid employment has brought attention to child care both as a critical social issue and as a pressing need for families. Nevertheless, child care in Canada continues to be framed as a private issue to be resolved by individual families. In the absence of policies and programs that ensure widespread access to affordable, high-quality care, women who combine motherhood with paid employment face considerable challenges in making decisions about child care. This study examines the processes by which women make child care decisions and sheds light on both how and why they make such decisions. The emphasis is on the meanings that women themselves give to motherhood, paid work, and child care and on how they resolve the competing interests that inevitably underlie work and family decisions. By drawing on women's accounts of their own lives, the research elucidates the multiple and interrelated factors that enter into women's decisions and thus offers insights into the reasoning behind complex patterns of decision making. In-depth interviews were conducted with 25 women who were intending to return to work or school following the birth of their first child. Women were interviewed at three points in time, encompassing a period from late pregnancy to several months after returning to work. The study furthers our understanding of the public and private dimensions of child care by revealing the dilemmas faced by women who frame their child care concerns in deeply moral terms, yet are called on to meet their child care needs within a public market oriented child care system. In particular, women's accounts of their experiences demonstrate the ways in which the intertwined and deeply privatised notions of 'dependent child' and 'good mother' underlie women's decisions about child care. Moreover, the research leaves no doubt that women's experiences of making child care decisions do not accord with the prevailing neo-classical economics version of rational and self-interested decision making. By examining women's decisions over time, the study illuminates the sequence of decision making about child care and adds to our understanding of what is entailed in looking for and deciding about child care. The study concludes with a discussion of implications of the findings for policy development and future research.

Nursing Science))--University of Stellenbosch, 2010.<br>ENGLISH ABSTRACT: HIV/AIDS is considered as a global problem with the number of people living with HIV infection continuing to increase. At the end of 2007 HIV/AIDS had already claimed 25 million lives. Of all new HIV infections 71% were diagnosed in the Sub-Saharan region in 2008, remaining the worst affected region globally. UNAIDS (2008:43) indicated that heterosexual intercourse remained the main origin for HIV infection in the Sub-Saharan region. Therefore the researcher is of the opinion that prevention strategies should focus mainly on sexual transmission of the disease. HIV/AIDS affects mainly people between the ages 15-24 years, notably the age group of most of the learners in Higher Education Institutions (HEIs). Lesotho, a country in the Sub- Saharan region, presents with the third highest HIV adult prevalence (23.2%) in the world and in the region. In an attempt to address the prevailing situation, Lesotho has a number of programmes geared towards addressing HIV/AIDS in the country. However, all these attempts exclude the learners in HEIs, yet the majority of learners are found within the most affected age group. It is also to be noted that Higher Education provides the bedrock for socio-economic and political development in Africa. Some studies have identified insufficient knowledge as being at the root of the increasing HIV infections among youth. However, other studies have shown that there is adequate knowledge among the young people, but still a challenge remains and that is to facilitate changes in behavioural patterns as a component to be linked to the knowledge. Studies conducted in other African countries have shown that there are anti-AIDS programmes and clubs for learners in HEIs where learners are involved in the fight against HIV/AIDS. No publication indicating the same for Lesotho’s HEIs could be found, except for the National University of Lesotho (NUL) that only launched its HIV/AIDS policy for learners in 2009. The researcher is of the opinion that HEIs in Lesotho are not doing enough to combat HIV/AIDS and hence intends to focus on HEIs in Lesotho. This study had two objectives namely:  To determine the knowledge of learners in a specific HEI in Lesotho regarding HIV/AIDS prevention and care.  To explore the needs of learners in a specific HEI in Lesotho regarding HIV/AIDS prevention and care. This mixed method study was conducted, comprising of both quantitative and qualitative designs. Quantitative phase used a questionnaire for determining the knowledge of learners. The questionnaire was adopted from a study that was performed to determine knowledge of South African educators in public schools with some modifications. The qualitative phase was used to explore the needs of the learners through the focus group discussions with the leaders of the learners. Sample was drawn from the entire population using stratified random sampling for the quantitative phase. The qualitative phase used the purposive sampling to obtain in-depth information concerning learners’ needs. Quantitative data was analysed through the use of statistical package for social sciences (SPSS) and qualitative data was analysed using the thematic analysis and open-coding. All ethical principles were adhered to especially the principle of respect for persons. The findings from the quantitative phase of the study showed that learners had adequate knowledge regarding HIV/AIDS prevention and care and the findings from the qualitative phase showed the various needs of the learners with regards to prevention and care of HIV/AIDS in a specific HEI in Lesotho. Recommendations have been proposed based on the findings from the two phases of the study. Limitations observed by the researcher have also been identified. In conclusion the objectives of the study were met and the research questions had been answered.<br>AFRIKAANSE OPSOMMING: MIV/Vigs word as ‘n internasionale probleem erken, siende dat daar ‘n verhoging in die toename van MIVgeïnfekteerde indiwidue tans is . Einde 2007 het MIV/Vigs het reeds 25 miljoen lewens ge-eis . In 2008 is 71% van al die nuwe MIV-infeksies in die Sub-Sahara streek gediagnoseer, wat aandui dat die streek die mees geaffekteerde streek tans is. UNAIDS (2008:43) het aangedui dat heteroseksuele omgang die hoofoorsaak van MIV-oordrag in die Sub-Sahara-streek is. Laasgenoemde het daartoe gelei dat die navorser van mening is dat voorkomende strategieë meestal op seksuele oordrag van die siekte moet fokus. MIV/Vigs affekteer meestal mense in die ouderdomsgroep 15-24, opmerklik is dit die ouderdomsgroep waarby meesste leerders in Hoëronderwysinstellings (HOI) is. Lesotho, ‘n land in die Sub-Sahara-streek, het tans die derde-hoogste MIV-voorkoms (23.2%) in die wêreld en in die streek. Lesotho het verskeie programme ontlont om MIV/Vigs te bekamp in ‘n poging om die huidige situasie te beredder . Nieteenstaande sluit al die programme leerders in HOI uit, alhoewel die leerders in die ouderdomsgroep van die mees-geaffekteerde groep val. Dit is ook duidelik dat Hoëronderwys die fondasie vir sosio-ekonomiese- en politieke ontwikkeling in Afrika verskaf. Sommige studies het onvoldoende kennis as die wortel van die verhoging van MIV-infeksies onder die jeug geïdentifiseer. Ander studies, daarenteen, wys dat kennis voldoende is onder jeug, alhoewel veranderinge in gedragspatrone om by die kennis aan te sluit ‘n uitdaging bly. Studies uit ander Afrikalande dui daarop dat daar anti-Vigs programme en klubs is waarby HO leerders betrokke is om teen die verspreiding van MIV/Vigs te veg. Geen publikasies in hierdie verband word in Lesotho aangetref nie, behalwe ‘n MIV/Vigs-beleid wat in 2009 deur “National University of Lesotho’ (NUL) gepubliseer is. Dus is die navorser van mening dat HOI nie genoeg doen om MIV/Vigs te beveg nie, daarom fokus sy op HOI in Lesotho. Hierdie studie het twee doelstellings ten doel gehad, naamlik om die leerders in ‘n sekere HOI in Lesotho se kennis aangaande MIV/Vigs voorkoming en sorg te bepaal en die behoeftes van die leerders aangaande MIV/Vigs voorkoming en sorg te verken. ‘n Studie met beide kwantitatiewe- en kwalitatiewe metodes is gebruik om die doelstellings te verwesenlik. In die kwantitatiewe fase is ‘n vraelys gebruik om leerders se kennis te bepaal. Die vraelys is verkry uit ‘n vorige studie wat in RSA gedoen is, maar aangepas om in die Lesotho-konteks te gebruik. Gedurende die kwalitatiewe fase is fokusgroep besprekings met die leiers van die leerders gehou om die behoeftes indiepte te verken. Die steekproef was uit die totale populasie getrek deur van gestratifiseerde streekproefneming gebruik te maak in die kwantitatiewe fase en ‘n doelgerigte steekproefneming is in die kwalitatiewe fase te gebruik. Die navorser het ‘n kwantitatiewe data-analise sagteware (SPSS)gebruik om kwantitatiewe data te ontleed en tematiese- oopkodering is gedurende die kwalitatiewe fase gebruik. Etiese kode is ten volle gerespekteer, veral die respek vir mense gedurende navorsing. Bevindinge van die kwantitatiewe fase het bewys dat leerders voldoende kennis aangaande die voorkoming en sorg van MIV/Vigs besit en die kwalitatiewe bevindinge het die behoeftes van leerders met betrekking tot die voorkoming en sorg van MIV/Vigs in ‘n spesifieke HOI in Lesotho geopenbaar. Die aanbevelings is gemaak, gebaseer op die bevindinge uit die twee fases. Beperkinge in die studie is uitgelig. Ter afsluiting is die doelstellings in die studie bereik en die navorsingsvrae beantwoord.

Abstract:
This case study investigated the quality of supervisor- employee communication and its impact on employee job satisfaction, self-esteem and trust, as well as the link to program quality in a center-based childcare setting. Three research questions guided the study with the purpose of examining communication as a means to improve job satisfaction, diminish staff turnover and strengthen program quality. Fifteen employees from a non-profit childcare center in San Joaquin County volunteered for the study. Data was collected from face to face and telephone interviews. Transcripts of the interviews revealed three themes- feedback, active listening and supervisor support. Responses were coded and categorized by theme, and then each theme was sorted into high quality supervisor communication responses and low quality supervisor communication responses. High quality supervisor communication was characterized by frequent and constructive feedback, self-motivation, openness and freedom to speak. The results of the study indicated high quality supervisor communication increased job satisfaction. Low quality supervisor communication was characterized by minimal feedback, the perception of favoritism, interference of work routine, low morale and increased absenteeism. The study found low quality supervisor communication hindered job satisfaction. The results of the study indicated that high quality communication between supervisor and employee had a positive impact on job performance, job satisfaction and organizational outcomes. Feedback, active listening and support were important communication behaviors sustaining job satisfaction. Passion for children proved to be intrinsic motivation to enhance program quality. Suggestions for developing program quality are provided.

Abstract:
This thesis explores adult-child relationships and the use of touch in residential child care. The discourses surrounding touch in residential child care are distinctly polarised: touch is identified as being both fundamental to child development and also feared as a result of increasing risk aversion resulting from the legacy of abuse scandals in the sector (Cooke, 2003; Steckley, 2012). There is currently a dearth of empirical research regarding how touch is used in residential child care, particularly with regards to observations of touch in practice. This research therefore contributes to filling this gap in research by utilising ethnographic methods of participant observation, semi-structured and ethnographic interviews to explore how staff members and children conceptualise, use and/or avoid physical touch in day-to-day practice. This research is examined through a lifespace framework and draws upon theories of intimacy in order to link the findings to wider sociological theories of relationships (Smith, 2005; Jamieson, 1998, 2011). By carrying out sustained observations of practice, this research examines how touch, as a facet of adult-child relationships in residential child care, is negotiated within the lifespace. It shows how intimacy is both cultivated and inhibited, thus contributing theoretically to wider sociological debates concerning adult-child relationships and touching practices, particularly in relation to professional relationships and intimacy (Morgan, 2009; Ferguson, 2011a). The fieldwork for this project took place over 6 months at Sunnydale House, a local authority children’s home in England. The findings suggest that touch is used much more regularly than has been previously suggested in literature (Ward, 1999; Cooke, 2003), that the majority of touch is instigated by children not workers and that recent organisational culture shifts have shaped the way that workers conceptualise touch. The findings also illustrate the complex range of factors which influence how touch is inhibited, including: risk aversion, emotional burn out, bureaucracy and the construction of children in care as ‘moral dirt’ (Ferguson, 2007). The thesis concludes by producing a typology of touching practices which synthesises the range of meanings behind the uses - and avoidance - of touch in residential child care practice. This typology is also used to re-conceptualise intimacy in professional relationships in a way that accounts for the messy ambiguity of adult-child relationships in the lifespace.

The problem of this study was to determine if employee child care difficulties were related to absenteeism. A case study was conducted among sixty-three employees at a north Dallas bank using a survey questionnaire. Descriptive statistics were used to analyze child care difficulties. A majority of employees experienced difficulty when co-workers had child care problems. A majority of the parent employees had difficulty finding sick or emergency/temporary child care, affording child care, and working overtime due to child care. The majority of parents had child care related absences and work interruptions and thirty-nine percent of them considered quitting their jobs due to child care problems. This study implied the need for employer-supported child care options for the bank employees.

Abstract:
The purpose of this study was to combine practice and research in a community agency. This study examined the extent and nature of the need for child care in Richmond, B.C. for parents who are employed or attending one of three institutions, located within close proximity of each other. The relationships between different variables were analyzed: age of child, type of care, hours of care and preferred location. The results of 237 families were compiled. Out of 237 families there was an indicated need for 217 children. The results indicated that there was a need for child care for children aged 0-13 years. The major types of care required were group care for children 0-5 years of age and out-of-school care for older children. Preferred location for care ranged from the vicinity of home and school to the parents' place of employment. Abstract:
The aim of this study was to explore care leavers' experiences to independency after being discharged from public care. The study explored the services provided for young adults who have left public care and how they perceived the support provided for them during their transition and after. The study has also an interest on the care leavers' rights according to the UN Convention on the Rights of a Child. A qualitative study approach was used to collect data, due to the focus on the interpretation of the care leavers' experiences and to get in-depth information and descriptions about their experiences and perspectives. Semi-structured interviews were conducted with four participants who have left care. Moreover, the findings show that the participants were disadvantaged in many areas during their transition and the support received were limited. From the findings, the care leavers were lacking stability, encouragement and both emotional and adequate practical support after being discharged from care.

Abstract:
The concept of service user participation in the delivery of services that affect them has gained momentum over the last thirty years. Children are no exception to this and those in care are subject to greater scrutiny of their lives than their peers. This study considered a key meeting for children in care – the Child in Care Review – and examined the extent to which children and young people are able to participate in these meetings and retain a level of control over their lives. The research, undertaken in one large local authority in England, explored the perspectives of children and young people, Social Workers, Independent Reviewing Officers and Senior Managers in individual qualitative interviews. The interview data was analysed thematically. The study found that young participants who reported a poor relationship with their Social Worker were more likely to feel negatively about their review and most young participants said that they found the review frustrating and stressful. The young participants were very aware of the workload pressures that Social Workers faced and how bureaucratic processes often seemed to translate in to them not receiving a good service. The Social Workers and Independent Reviewing Officers highlighted the importance of children’s participation, but in practice their commitment to the concept seemed minimal. Data would suggest some significant disconnection between Senior Managers’ views and all other participants’ perspectives on the challenges faced by social workers in terms of caseloads and workload pressures. Senior Managers reflected that little seemed to have changed in relation to children’s participation in their reviews over the last twenty-five years. The thesis concludes that as a vehicle for participation the Child in Care Review is still not working well, however the development of children chairing their own reviews offers some hope for the future. This practice could be built upon to ensure that children and young people leave Local Authority care with the best possible chance of becoming confident, stable and empowered adults.

The published research and personal practice experience from Sub-Saharan Africa indicate that up to 50% of child deaths occurring in hospital-based paediatric emergency care (PEC) take place within 24 hours of admission. This study contributes to the literature, by identifying important factors influencing mortality and other care outcomes among children admitted to a named children’s emergency room in Lagos, Nigeria (CHER) in this period of time. The study accepted the theoretical concept of a multifactorial causation of population health outcomes. Three explanatory frameworks are jointly considered in order to locate the chronological PEC pathway within a wider societal and environmental reality. A systemic review of the published literature on outcomes in developing country paediatric emergency rooms (PER), revealed that delayed progress through the PEC pathway was potentially causal of poor outcomes. Factors identified as contributing to delays included socioeconomic and cultural factors affecting parental healthcare seeking and functional features of individual PER. The literature also made a case for the role of the failures of wide-scale preventive interventions towards increasing the baseline burden for PEC in these contexts. The limitations of the previous research included the inability of the research approaches to allow an appreciation of the nature of the implicated factors and their roles relative to outcomes within the chronological PEC process. This study was conducted as a single-site case study due to the clear functional and conceptual boundaries offered by an examination of the first 24 hours in a named facility. The researcher’s social constructivist worldview emphasised the value of experiential information in examining PEC processes, as well as directing the collection analysis, and interpretation of the real-life, context-situated data. The data collection process involved 3 months of non-participant observation, 18 unstructured interviews, 4 focus groups, and the collation of 6 months’ worth of patient admission data. An inductive approach to analysis was followed by the triangulation of the emerging findings, and a final interpretation which patternmatched emergent themes against theoretical linkages towards PEC outcomes. This study identified prevailing sociocultural attitudes to illnesses in children, the use of alternative treatment modalities, public perceptions of available private care, and functional barriers in the CHER as contributing to the mortality risks and delayed progression through the in facility PEC. The study offers recommendations encouraging the public promotion of safer use of Home based self-medication HBSM, attendance at appropriate facilities, and improvements to available in-facility services, as well as the regulation of the emergency referral practice for private health providers in Lagos. It concludes by outlining the directions for the publication of the findings and suggesting possible future research.

In 2007-08, the Mexican-American border town of Tijuana showed an unusually high density of residential care settings for children and young people, typically founded and run by members of religious congregations based in the USA. Drawing on concepts from childhood and youth studies, international development, sociology of religion and urban sociology as analytical devices, this study centralises its view on one of these care homes; while at the same time exploring the broader geographical, political, economic and social context in which the high density of care homes occurs. Recurring themes are social order and the various initiatives to establish, maintain or change it, as well as interactions between members of different social groups that are shaped by and based in misrecognitions (‘non-encounter’). The areas of originality of this study are threefold. First of all, it integrates analysis at micro and macro levels. At the micro level it explores the institutional set up, staffing arrangements and the experiences of children and young people within the institution. Structures and mechanisms of governance, daily life in the institution and the social meanings ordering it are examined; allowing for a microscopic view that is embedded in a macro level analysis. The latter is concerned with recording factors such as migration into and through the border zone, the constitution of urban space that is specific to Tijuana, drug commerce and its consequences, poverty and inequality, conceptualizations of gender roles, principles of aid work, the structure of the religious field, including faith-based charitable activities taking place, and the formal and informal ramifications pertaining to the local economy. Secondly, this piece of work is a qualitative longitudinal study, employing an innovative multi-method research strategy consisting in real life and long-term online ethnography, child-led research methods, interviews and document analysis. Thirdly, the concept of the non-encounter is introduced to describe social interaction taking place under false premises. Key findings include the identification of peculiarities of the overall social and political situation, of the particular features of the urban agglomeration of Tijuana and of the religious field and the ways in which they relate to each other. Additionally, there is the insight that this unique combination is particularly conducive to the emergence and persistence of the kind of child- and youthcare under examination. The main argument is that residential child- and youthcare is a moral and political endeavour, undertaken in pursuit of specific aims and goals and informed by the norms and values of those carrying it out. Also, that residential child- and youthcare needs conditions conducive to its existence to spread and flourish. Located in time and space these vary in their specific characteristics across the different environments.

This study identified transition practices used by teachers and/or primary caregivers of infants and toddlers when entering child care programs across Dallas , TX . Participants completed the Program Entry Transition Practices Survey regarding their use of transition practices in fall 2003 and perceived barriers to entry transition practices. Results show frequency tallies, percentages of use for each transition practice, and the mean number of practices used for the entire sample and with the sample split according to participants from profit and non-profit programs. Results were also organized into four subscales based on the type of practice used. Results supported the hypothesis that teachers and caregivers would report more transition practices occurring after beginning care and directed towards a group than those occurring before beginning care and directed towards an individual. In response to the broader research question regarding barriers: Teachers and caregivers from profit and non-profit centers reported similar barriers to implementing transition practices.

Abstract:
This study examined maternal depression status from month 1 to 36 after birth using data from the NICHD Study of Early Child Care and Youth Development. Maternal depression was assessed with the Center of Epidemiological Studies Depression Scale. The prevalence of maternal depression was highest at 1 month, decreased at 6 months, and then kept fairly stable to 36 months. The prevalence was higher in blacks than other races, in 18-24 than 25-46 years old, and in single mothers than non-single mothers. Mothers with better physical health, social support, or employed had a lower prevalence than their counterparts. Mothers in poverty, receiving public assistance, or who had more parental stress had a higher prevalence. Social support and parental stress had a statistically significant relationship with maternal depression even after adjusting for other variables. In conclusion, this longitudinal study found that several maternal, child, and family factors were associated with maternal depression.

This research aims at discussed from the viewpoint of family day care providers to get their cognition and feeling of baby caring and proposes as the practical reference of improving service quality of baby caring. This research adopts semi-structural interview, and ten family day care providers are selected by purposive sampling for interview. Data analysis adopts open coding and through the supplement of comparative analysis of literature to characterize the working content of family day care providers and practical content of home supporting. The results revue’s family day care providers are more agree with their role as “caregiver”, which shall be different in some extent from the position and role of mother that entrusting them, and consider that they shall undertake their work as “supporter”. The interviewees confirmed their work by “express from heart”, “work value” and “affections connection”. Their working pressures are mainly from parents, including long caring time, difference in the ideal of baby caring, caring method and living custom, and the concern of baby appearance and safety, in which safety is the biggest working pressure. Furthermore, “family support” from the family day care providers is the major assistance to relive the working pressure of interviewees. Moreover, they interact with parents progressively, so that they could value, care and listen to each other, which could facilitate the entrance of social exchange. If parents consider themselves as “employer”, the relationship between them would limit to the level of employer and employee. According to the research it is suggested: (1)In terms of parents, they could take “family support” from the family day care provider as one consideration for the selection of family day care providers to facilitate the selection of family day care providers with good caring quality; (2)In terms of family day care providers, it is recommended that those who want to be family day care providers shall seek the agreement from their family first, and further provide instrumental and emotional labor division in the family; (3)In terms of cultivation unit, the application of family resource of the family day care providers shall list in the course to enhance the execution of caring work; (4) In terms of future researcher, it is recommended to increase number of interviewee to obtain data more comprehensively, and have further discussion of family support or family resource.

Relevant information on the diet of 6 to 36 months old infants and children, as well as their growth and development information were gathered. The results showed that: (a) In the food aspect, the child care centers and family child care provided about the same number of meals and diet, but the child care centers provided more amount of food, while family child care provided more variety. (b) In the growth aspect (children’s height, weight, and head circumference), 70 % of the chilren’s growth condition at the child care centers were above third-grade(50th percentile), and 60% of the chilren’s growth condition in family child care were above third-grade(50th percentile). But overall, the growth conditions of chilren at the child care centers were slightly better than those in family child care.(c) In the development aspect, the development conditions of children at the child care centers and the family child care were mostly within the normal development range of their ages. It transforms the key concept of professional image into a concrete analysis of actions that insure the child care quality. Also, it establishes reference to high service quality for family child care providers as a benchmark of child care in the workplace. Case study method and document analysis are used in the study. Eleven family child care providers with different backgrounds, experiences, and service models are selected through purposive sampling and classified through qualitative analysis after observation, interviews, and collection of relevant documents. The study’s findings are as follows. 1. The image of professional family child care providers includes professional capacity, executive capacity, and caring capacity, which are related to career abilities in several aspects: (1) Professional capacity consists of six career abilities, such as professional progress, parental interaction, and self-care. (2) Executive capacity consists of six career abilities, such as work administration, problem solving, and quality orientation. (3) Caring capacity consists of five career abilities, such as initiative, positive thinking, and empathy. 2. Actions corresponding to family child care provider's professional image and career abilities that insure the child care quality: (1) Professional capacity: professional progress consists of actions such as constant self-improvement; parental interaction consists of actions such as effective communication; self-care consists of actions such as doing outdoor exercises; development and game-based learning consist of actions such as sensory and movement exploration; environmental and routine planning consist of actions such as creating safe and adaptive environment; life and health care consist of actions such as satisfying physical needs. Child care stability affects child and family outcomes. Stability reflects the time dimension of a child care arrangement. Although stability does not guarantee positive outcomes, instability appears to decrease the likelihood of achieving them. Some level of stability is a necessary, although not sufficient, characteristic of care that meets children's needs. Child care stability is of special concern for children in low income families because child care impacts are greatest for these children, and current welfare policies result in more low-income children in nonparental care. This study increases understanding of child care stability through (a) an analysis of findings from stability studies over 30 years, (b) an examination of relationships of the four major stability measures, and (c) presentation of results from an analysis of the stability of subsidized child care arrangements in Oregon. The analysis of stability studies documented lack of consistency in conceptualization, measures, and methodology. These inconsistencies limit comparisons of reported stability findings and confidence in estimates of child care stability that have been reported. Examination of the four stability measures found that the three child-level measures appear to describe the same construct as they are highly correlated. The fourth stability measure is at the level of the arrangement and captures a distinctly different aspect of stability. Stability levels of subsidized arrangements in Oregon appear lower than those found in nationally representative samples but similar to levels found in populations participating in public assistance programs. About a third of children had very stable care but others had high levels of instability. Fifty percent of arrangement spells ended by 3 months, even when children were observed for 36 months. According to the result of this study, Japan’s school-age child care has five features: pay attention to children’s full development, orientation of public welfare system, legislate for specified duties of government, think highly of combining regional resources, encourage diversified service units to participate. At last, five implications of after-school child care are suggested: reflect on children’s needs and parents’ expectancies, clear up orientation and function, recognize the importance of government legislation, establish resource networks, develop multiple supply systems. The approach of the research was literature analysis and a questionnaire survey. Data were collected from 281 parents of kindergartens and infant centers and were used in analysis process of the descriptive statistics, t-test, one-way ANOVA, Pearson correlation, and stepwise multiple regression. The results of this study are: 1. Gender and monthly income show significant different with the part of parents’ child health cognition. 2. Job, level of education, number of the children and monthly income show significant different with the part of the child- care self-efficacy. 3. It is a significantly positive correlation between correlation between the parents’ child health cognition and child- care self-efficacy. 4. Parents’ child health cognition and level of education have high predicting for their child- care self-efficacy. Finally, according to research findings, the researcher proposed practical suggestions and implications.

Abstract:
This study explores both formal and informal arrangements for care of orphans in the Maseru district of Lesotho. The study adopted a qualitative approach using both in-depth interviews and documentary research as the primary methods of data collection. The research findings show that care of orphans in Lesotho is predominantly in the hands of non-governmental and church based organisations. The government’s role is limited to the provision of technical support, including the formulation of policies and laws and some welfare grant provision in the form of free primary education. Care of orphans in the communities is undertaken not only by the extended family members of orphans, but also non-relation community members. It is clear however that while such informal arrangements might provide care and protection to orphan, many are struggling in the face of extreme hardship and poverty. A crisis of social reproduction is therefore increasingly apparent in Lesotho.

This study was a quantitative research and the subjects were the child care providers registered in the Taipei city community child care scheme. There were 472 valid questionnaires. The results of this study were as follows: A. The expectations of child care providers on the service quality of the community child care scheme were all showed on every aspect and according to the degree of the expectations, five aspects in sequence were assurance, empathy, reliability, responsiveness, and tangibles. B. The actual feelings of the providers on the service quality of every aspect indicated some certain degree of satisfactions, and the five aspects in sequence were reliability, empathy, assurance, responsiveness, and tangibles. C. The satisfaction on the service items of community child care scheme were showed in every aspect and according to the degree of satisfaction, the service items in order were visitation counselors, on-the-job trainings, visitation counseling, telephone consultations, as well as job matches and referrals. D. The differences between the expectations and satisfactions on the service quality by examining the service quality of the five aspects, it was indicated that the child care providers had high expectations on the aspect of assurance but showed lower degree of satisfaction on the service quality,by viewing the results of the thirty-four questionnaire items on the service quality, it was found that “the positive attitudes of visitation counselors” was an important expectation for the child care providers as well as showed higher degree of satisfactions and regarding to“the referrals of child care”and“setting up the child care resource center”, the expectations were high, but showed lower degrees of satisfactions. E. The study indicated that the participation of on-the-job trainings was the main expectation for the child care service providers registered in the community child care scheme and they had higher satisfactions toward the trainings. At the same time, the active attitude of the child care providers was also an important crucial factor that affected the satisfaction of the community child care scheme. Based on the results of this study, suggestions on the government sectors, community child care scheme, child care providers and future research topics on facilitating the child care policy and services were addressed.

ABSTRACT The number of people in immediate need of anti-retroviral treatment (ART) in the southern African region continues to significantly exceed the capacity of health systems to provide it. Approaches to this complex rationing dilemma have evolved in different directions. The ethical concepts of fairness and equity have been suggested as a basis guiding rationing or patient selection processes for ART. The purpose of the study was to examine whether or not such concepts had relevance or operative value for a treatment team providing ART in rural Lesotho. Using an exploratory, single case study design the study found that while concepts of fairness and equity were relevant to the work of the treatment team, patient selection practices did not necessarily reflect what these concepts entail. The idea of fairness as a structured, formalized selection process did not figure in the approach to ART provision at St. Charles. A less formal, ‘first-come-first-served’ approach was adopted. While there was knowledge amongst some team members that social, economic or geographic conditions inhibit individuals and groups from gaining access to ART and that this was inequitable, it was felt that there was little they could do to try to mediate the impact of these conditions. The study findings pose importance questions about the approach to ART programming in resource constrained settings. The findings also question the relevance of trying to achieve fairness and equity when the gap between need for care and capacity to provide it remains so large.

Based on the literature analysis and interview results, it preliminarily builds the family childcare service inspection indicator system, which is reviewed by the experts to obtain the fuzzy Delphi questionnaire. Twelve fuzzy Delphi experts are invited to screen the indicator content. Then it uses AHP to build the related weights of family childcare service inspection indicators. According to the study findings and results, the study obtains the following conclusions:   I. The family childcare service inspection indicator system is composed of 2 layers, 8 dimensions and 63 sub items.   II. The family childcare service inspection should value the professional regulations and balanced development of childcare competency.   III. In the family childcare service indicator system, “Occupational Ethics” is the most important dimension in the professional regulations, and “Nutrition & Healthcare” is the most important dimension in the childcare competency.   Finally some specific suggestions are proposed based on the study results, which can service reference for the family childcare service and follow-up studies.

From the hermeneutic phenomenology point of view, this thesis thoroughly analyzes four interview cases on the caring experiences of four early child mothers with a view to entering the inner world of a mother who is in the stage of child nursing and , therefore, trying to develop a new perspective on child nursing. The following are the conclusions. I. The original family is the place where everyone grows up and is also the place where everyone’s love and emotions are originated. We live with it without noticing the importance of it. However, it does have a great influence on us. In the same way, it influences our decision on delivering and nursing babies. II. In the experiences of delivering a child, no matter the interviewed mothers use the term “wanting” or “ being able to” to describe their choices of having a baby, I find that their expectations on having a baby boy or baby girl is greatly influenced by the social attitudes which most people have in their society. These mothers even turn the expectation into their own great missions. Therefore, the situation of delivering a child they face actually has existed inside themselves long before they even get married. III. In the chapter of “nursing experiences”, I start from the early child mothers and look into other related environments. And I have the following findings. 1. Although it depends on the mother to be a full-time mother, the desire of being a full-time mother does relate greatly to how she comprehends her nursing situation. 2. Next, among the advantages and disadvantages of the nursing provided by the grandparents, I understand it is not really positively or negatively related. 3. Besides, in choosing the caretakers outside the family, while the reliable information of caretakers is not easy to gather, it is the guarantee from the relatives and friends and the issue of transportation convenience that these mothers care about the most. 4. From the experiences, I understand that the appreciation that the early child mothers show to their husbands’ participation in the nursing and the role of financial support are actually affected by the “normative vision”. Although it is not clear, it’s very influential to everyone in our society , certainly the early child parents included. 5. Finally, based on these mothers’ experiences, I come to realize what kind of nursing environment their government has provided. And meanwhile, despite the difficult situations they have encountered, I find most of these early child mothers don’t understand their problems of early child care are actually due to the improper policies their country has offered. In the conclusion, the researcher also provides the following opinions. I. To early child parents: All early child mothers should encourage contact between fathers and their kids, provide dads more chances to take care of their children, and tolerate the imperfect caring that may happen. Fathers should not use excuses to hinder the chances of taking care of their children. Furthermore, all wives and the whole society should encourage fathers to get involved in child care. Therefore, all children’s greatest well-being will be made. II. To those who want to be early child caretakers: Those who want to work in the field of childcare should improve their professional knowledge and ability, try more aggressive ways to show the differences between themselves and other caretakers who have never been trained. Therefore, the parents will be convinced that it is important to have professional personnel to do the job. III. To future researchers: Since this research didn’t distinguish the interviewed mothers’ roles as the first-time or second-time mothers, some differences, such as the mothers’ point of views or the things that they concern about, might not show up in the research. Therefore, the future researchers may try to tell the differences in their research and conduct some other interesting and important findings.
