Abstract:
This contribution is an interpellation to the churches in the City of Libreville to take concrete action in response to acute present urban problems that face the city. Currently, the Church pays little or no attention to urban issues that affect the City of Libreville. Many evangelicals in Libreville perceive giving attention to these urban challenges such as pollution, absence of waste management, heat waves, etc to be a distraction to the core business of ministry which is the salvation of souls. Against this backfrop, this contribution argue that this attitute amount to neglecting God’s mandate to be stewards of creation. These problems pose serious challenges to the City of Libreville including churches and they therefore deserve theological/missiological attention. The Church should participate towards identification of root-causes for these urban challenges and in the process open the Church to flourish in socio-political, socioeconomic and socio-cultural spaces as she works towards solving these issues.

Abstract:
SMMEs provide an indispensable framework for addressing unemployment, poverty and boosting the economy of a country. This report was impelled by the need to create and sustain enterprises within the hotels and restaurant sub-sector of the tourism industry in Libreville, Gabon. A quantitative methodology was embraced to disperse seventy self-administered questionnaires to owners and managers of hotels and restaurants. The results demonstrate that the key components which lead to the failure or non-sustainability of businesses include: limited access to start-up finance required to cover start-up and growth cost, the low return on investment, as well as the mismanagement of businesses. Given that job creation remains a dependable method for diminishing unemployment and poverty, establishing and sustaining businesses in Libreville would be a step in the right direction in Gabon. <p class="Pa5"><strong>Objectives: </strong>Many researchers continue to believe that urbanization is a major contributor to diabetes. We seek to demon­strate that the social status associated with urbanization has an impact on the preva­lence of diabetes in Libreville, Gabon in sub-Saharan Africa.</p><p class="Pa5"><strong>Methods: </strong>Our study was conducted in Li­breville, the capital of Gabon; the city has a population of 397,000. Our study analyzed data from the registries of patients hospital­ized in 2013 in the main diabetes center in Libreville.</p><p class="Pa5"><strong>Result: </strong>The results revealed that, for 2013, 798 patients were hospitalized with diabetes at a prevalence of .2%. We found differences (<em>P</em>&lt;.05) between women (423) and men (375). Mean age for women was 52.02 years and 48.88 years for men. The number of existing cases hospitalized was significantly more than new cases. All levels of society were represented in our study: students (42); military (36); administratives (99); technicians (180); unemployed (295); and retired (146). The results showed that the unemployed (36%), particularly women (29.40%) are most affected by diabetes.</p><p><strong>Conclusion: </strong>Our results show the impact of social status on the increase of diabetes in Libreville. We found that urbanization, associated with insecurity especially in women, had an effect on the prevalence of diabetes in Libreville. Abstract:
Background: Exclusive breastfeeding from birth to six months of age has an unparalleled benefit on a child's growth and development. This survey aimed to assess the practice of exclusive breastfeeding (EBF) in Libreville and Lambaréné. Method: This is a prospective study, including mother-child pairs of infants taken to vaccination centers in Libreville (urban area) and Lambaréné (semi-rural area). The data collected included sociodemographic aspects of families, the children's characteristics, and the duration of EBF in months. Result: We included 552 mother-child pairs, 58% of whom were recruited (n = 320) in Libreville. The average age of mothers was 26.5 ± 6.4 years, while fathers were 33.3 ± 7.8 years. The mean age of children was 4.2 ± 3.1 months. Male participants were accounted for 52.2% (n = 288), while females 47.8% (n = 264). The mean duration of exclusive breastfeeding in the whole was 0.9 months. Six-month EBF was generally performed at 9.2% (n = 51, 95% CI = 7.1%-11.9%). The factors influencing the EBF were father’s level of education (all classes combined) (p = 0.025), marital status (p = 0.011), and residential area (OR = 3.40, p <0.001) Conclusion: The duration of exclusive breastfeeding in the two studied is lower than the WHO recommendations. The associated factors found are areas of work to be explored to encourage this important practice for our infants.

Dans la partie Nord du Grand Libreville, les quartiers Angondjé, Okala et Mikolongo connaissent une croissance démographique accompagnée d’une extension urbaine soutenue, et ayant des impacts sur les zones humides à mangroves. Considérée comme un danger écologique, la dégradation des mangroves dans ces milieux est due principalement aux activités anthropiques socio-économiques. Dans ces forêts à mangroves, les activités et les aménagements exercent des pressions humaines. La méthode de collecte des données et autres informations a fait appel respectivement, d’une part, sur le terrain à un guide d’observation directe et de caractérisation des activités socio-économiques, des prises de vues et l’utilisation d’un GPS et, d’autre part, la consultation des rapports divers et des documents cartographiques. Les données recueillies montrent que les activités tels que le bâti, le maraîchage, la pêche, la chasse et les manifestations de la pollution sont les facteurs de dégradation et déforestation des mangroves. Cette anthropisation problématique des milieux humides à mangroves se déroule dans un contexte d’étalement urbain favorisé par une urbanisation non maîtrisée, dans le nord du Grand Libreville. In the northern part of Greater Libreville, the Angondjé, Okala and Mikolongo districts are experiencing demographic growth accompanied by sustained urban expansion, and having impacts on wetlands with mangroves. Considered an ecological hazard, the degradation of mangroves in these environments is mainly due to anthropogenic socio-economic activities. In these mangrove forests, activities and developments exert human pressures. The method of collecting data and other information called respectively, on the one hand, on the ground for a guide of direct observation and characterization of socio-economic activities, shots and the use of a GPS and, on the other hand, the consultation of various reports and cartographic documents. The data collected show that activities such as construction, market gardening, fishing, hunting and manifestations of pollution are the factors of degradation and deforestation of mangroves. This problematic anthropization of wetlands with mangroves takes place in a context of urban sprawl favored by uncontrolled urbanization, in the north of Greater Libreville.

Cet article examine l’impact de la transition numérique sur l’entrepreneuriat informel des femmes commerçantes à Libreville, au Gabon, et sur leur autonomisation économique. Utilisant une approche qualitative exploratoire et s’appuyant sur des entretiens semi-directifs avec des commerçantes de Libreville ainsi que divers acteurs (ONG, etc.), l’étude révèle que la transition numérique présente des opportunités, notamment en matière d’accès à l’information et aux services financiers ainsi que de renforcement des réseaux sociaux et du capital humain. Néanmoins, elle met également en évidence des défis significatifs, tels que l’accès limité aux technologies, des compétences numériques insuffisantes ainsi que l’existence de normes sociales et culturelles contraignantes. Des stratégies d’adaptation et de résilience élaborées par les femmes commerçantes pour faire face à ces obstacles sont identifiées. Ces constatations suggèrent des directions pour les décideurs politiques, les gestionnaires et les organisations désireux de soutenir l’entrepreneuriat féminin dans le cadre d’une numérisation en expansion.

La diminution drastique de terres arables à Libreville pousse de plus en plus les agriculteurs à s’installer dans les localités proches de la capitale gabonaise pour y pratiquer l’agriculture et ravitailler Libreville en produits agricoles.Objectif. La présente étude qui se déroule à quatre-vingt kilomètres de Libreville, a pour objectifs de : (1) déterminer certaines propriétés physico-chimiques et biologiques des sols de Kango, et de comparer les valeurs trouvées avec les valeurs seuil agronomiques, (2) relever les contraintes liées à la production agricole sur ces sols, et (3) proposer des solutions adéquates pour une exploitation agricole respectueuse de l’environnement.Méthodes. Des échantillons de sols ont été prélevés, et les analyses physico-chimiques et biologiques y ont été réalisées : le pH eau, le pH KCl, le carbone organique total, l’azote total, l’azote assimilable, le phosphore total, le phosphore assimilable, l’aluminium, les cations basiques, la capacité d’échange cationique, le taux de saturation en bases échangeables, la biomasse microbienne, le quotient microbien, les rapports azote microbien/azote total et carbone microbien/azote microbien.Résultats. Les sols prélevés sont argileux, ils ont des teneurs en carbone organique total, en azote total et en phosphore total agronomiquement convenables. Mais la très forte acidité, les teneurs en phosphore et azote assimilables, en cations basiques, le taux de saturation très bas et l’activité biologique constituent les principales contraintes à la production agricole.Conclusion. Le chaulage, l’apport de biochar et des amendements organiques sont des solutions à appliquer pour obtenir de bons rendements dans ces rendements dans ces sols dans le respect de l’environnement. Description of the subject. Faced with the drastic reduction of arable land in Libreville following the development of the city, more and more farmers and agro-industries are settling in localities close to the Gabonese capital where they can find land to exploit in to supply Libreville with agricultural products, such as the locality of Kango, located 82 kilometers from Libreville.Goals. The objectives of this study are (1) to determine certain physico-chemical and biological properties of the soils of Kango and to compare the values found with the agronomic threshold values, (2) to identify the constraints likely to limit agricultural production on these soils, and (3) to propose adequate solutions for an efficient and environmentally friendly agricultural exploitation.Methods. Soil samples were taken, and physico-chemical and biological analyzes were carried out: water pH, KCl pH, total organic carbon, total nitrogen, assimilable nitrogen, total phosphorus, phosphorus assimilable, aluminum, basic cations, cation exchange capacity, exchangeable base saturation rate, microbial biomass, microbial quotient, microbial nitrogen/total nitrogen and microbial carbon/microbial nitrogen ratios.Results. The soils sampled are clayey, they have agronomically suitable total organic carbon, total nitrogen and total phosphorus contents. But the very high acidity, the assimilable phosphorus and nitrogen, basic cations, the very low saturation rate and the biological activity constitute the main constraints to agricultural production.Conclusion. Liming, the addition of biochar and organic amendments are solutions to be applied to obtain good yields in these soils while respecting the environment.ls dans le respect de l’environnement.

Abstract:
Background: Physical activity and healthy diet can prevent or reduce risk of developing type 2 diabetes, otherwise may reduce the disease complications among diabetics. The objective of this study was to evaluate the physical activity and diet of type 2 diabetics in Libreville. Methods: It was a case-control study conducted at the Endocrinology Department of the University Hospital Centre of Libreville and at the Biochemistry Laboratory of the University of Health Sciences. Patients (n=272) recruited were divided in type 2 diabetics (n=136) and controls (n=136). Physical activity was assessed using WHO Global Physical Activity Questionnaire (GPAQ). Diet was assessed using FAO Food Consumption and Diversity Scores (FCS and DDS). Results: Diabetics had higher overall physical activity than controls (19.1% &lt;i&gt;vs.&lt;/i&gt; 10.3% respectively; p= 0.0033). Then, low activity was obtained in more than half of diabetics (80.8%). Acceptable dietary intake was more frequent in controls than in cases (78.7% vs. 51.4%, p= 0.0000). Similarly, poor dietary diversity was observed in 34.6% of diabetics versus 8.8% of controls (p= 0.0000). Several variables were associated with type 2 diabetes. These were age (p= 0.0000), TyG (p= 0.0000), triglyceridemia (p=0.0000), first and second-degree relative family history of diabetes (p=0.0030 and p=0.0020 respectively) and alcohol consumption (p= 0.0060). When stepwise linear regression was performed, dietary intake and dietary diversity scores were negatively associated with diabetes. Conclusion: This study revealed that diabetics in Libreville are intensely physically active, but that this activity needs to be reinforced and accompanied by greater dietary diversity.

Abstract:
Background: The objectives of this work were to inventory the different morphological entities, to alert the medical class to the need for multidisciplinary care; to sensitize the health authorities on this pathology having a considerable influence on the fertility of the woman and a possible evolution towards cancerous disease.Methods: A retrospective study was carried out over 20 years from January 1983 to December 2002 at the laboratory of anatomy pathology of the faculty of health sciences in Libreville. The revealing clinical signs were the couple's bleeding and infertility. Curettage, endometrial biopsy, and subtotal or total hysterectomy were fixed with 10% buffered formalin or Bouin's fluid. After staining, histological study was carried out by the pathologist.Results: The histological images of the endometrial polyps were the most representative (73 cases or 17.68%), followed by prolonged proliferative endometers (70 cases, or 16.95%), glandulo-cystic hyperplasia (66 cases, or 15.98%) and persistent proliferative endometers (58 cases, or 14.04%). More than half of the numbers concerned women under 35 (225/413 cases); in those over 45 years of age, the predominantly endometrial carcinoma was found. The most affected province was Woleu-Ntem (24%), followed by Haut-Ogooué (18%).Conclusions: The different dysfunctional endometers are also described in the different regions of the continent. The management of these aims to be multidisciplinary and urgently requires a frank collaboration between clinician and morphologist, especially for country where a birth policy is clearly expressed by governing bodies.

Abstract:
Introduction: As kidney function declines towards the more severe stages of chronic kidney disease (CKD), the interactions between kidney, intestine and bone become increasingly unstable. CKD with mineral and bone disorders and secondary hyperparathyroidism would be developing. The aim of this study was to determine the phosphocalcic profile of CKD patients in Libreville. Materials and Methods: This was a cross sectional study with 89 CKD patients recruited. A blood sample was taken to measure PTH, vitamin D, FGF-23 by ELISA method; calcium, magnesium, fasting blood glucose, phosphate and creatinine by spectrophotometer. Results: Mean phosphorus levels were 1.3 ± 0.5 mmol/L and hormone levels 81.8 ± 26.2 pg/mL and 27.5 ± 5.0 ng/mL for PTH and vitamin D respectively. Significant hyperphosphatemia was found among 43 (48.3%; p=0.0135) patients. There were 59 (66.3%) subjects with hypovitaminosis D p=0.0000. Less than 50% of patients had normal blood glucose levels (p=0.0034). PTH was 99.4 ± 16.4 pg/mL in dialysis patients and 61.7 ± 20.3 pg/mL in non-dialysis patients, with a p=0.0000. Vitamin D levels were significantly higher in patients without calcium supplementation (29.5 ± 5.0 ng/mL) than in those with supplementation (25.1 ± 4.0 ng/mL, p= 0.0000). Conclusion: Phosphate levels remained high in our study population. Vitamin D deficiency was found in the majority of our patients. It would be advisable to readjust the management of these patients in order to minimize the effects of hyperphosphatemia and improve life quality.

In Africa and Gabon particularly, rampant urbanization, the increase in female professional activity, the break-up of the classic family model have induced new eating behaviors and consumption patterns. The development of the cold chain promotes the reduction of food losses, the conformity of products to quality requirements, market safety and the improvement of production. The cold chain is therefore an essential factor in food safety. In Gabon, national consumption needs are more or less covered by local production with regard to plantain, cassava, taro, yam and fish products. The coverage rate is zero for rice, wheat, fruit ballast, quite low for vegetables, meat and poultry. So much so that it is common to hear people say in Libreville: “I am going to buy the embalmed, how are we going to do it again?”. FAO 2008 defines food security as a state where "all human beings have, at all times, physical and economic access to sufficient, safe and nutritious food, enabling them to satisfy their energy needs and food preferences to carry out a healthy and active life » Our non-stratified sample includes teachers and ATOS staff from the ENS Libreville campus. A total of 100 questionnaires are distributed, only 88 are collected for the study. The survey conducted within the premises of the Superior Normal School of Libreville is not statistically representative of the whole country. The limitations of the study are related to its geographical coverage. The approach is based on the place of purchase and the type of purchase. This study shows that respondents determine the quality of what they buy by the eyes, the color, the smell and the place of purchase. As places of purchase they prefer supermarkets (Mbolo, CKDO). Many of them believe that the authorities do not pay enough attention to the quality of food. The objective of this work is to understand the processes that populations bring into play in the development of food quality.

Following the observed increase of malaria prevalence among older children in Gabon, a descriptive observational study was carried out in 2012 to determine the prevalence of malaria in adults presenting with fever in two health centres of Libreville, the capital city of Gabon. Thick- and thin-blood smears for malaria diagnosis were performed in febrile individuals aged more than 15 years old. Age, use of bed nets, previous antimalarial drug treatment, clinical symptoms, chest radiography results, and available haemoglobin data were also recorded. Among the 304 patients screened, the global malaria frequency was of 42.1%(n=128/34).Plasmodium (P). falciparumwas the only species identified. The proportion of patients with a clinical malaria requiring parenteral treatment was 38.5%, whereas 47.5% of outpatients had uncomplicated malaria. According to WHO classification, 14 (19.7%) infected patients had severe malaria; neurological and respiratory symptoms tended to be more frequent in case ofP. falciparuminfection. Anaemia was found in 51.5% adults and none had severe anaemia. Almost half of adults consulting for fever in two health centres of the urban city of Libreville have malaria. The use of insecticide-treated bed nets, the screening, and the treatment of individuals withP. falciparummicroscopic and submicroscopic asymptomatic infection or clinical malaria should be emphasized to reduce the transmission.

Abstract:
The aim of this article is to contribute to our understanding of how local rap worlds evolve by describing the successive processes of the decline and revitalization of rap music in Gabon between 2009 and 2020, and by questioning the technologies and mediations employed for that purpose. This article considers how these transformations overlap with a history of complex relationships between music and politics, arguing that the revitalization of the rap world was related to a broader attempt at social healing and reconciliation after a violent political conflict. It stems from a long-term ethnography study conducted in Libreville and the Gabonese diaspora from 2008 to 2016 and on the analysis of two recent rap projects launched in Libreville: ‘Bwiti Gang Cypher’ and ‘Catalogue Challenge’. Through the analysis of these two performances, I highlight how the attempt at revitalization was relying on a complex mix of mediations and technologies, including original hip hop conventions and local healing rituals and how it has allowed for the transformation of divisive conflicts into a cathartic moment of collective listening. This article finally proves the double dimension of musical revitalization, one where music rebirth and social healing overlap, and it shows how the embeddedness of music and politics can be permanently transformed through the agency of social actors who develop a creative play between different technologies and mediations.

Introduction: Non-traumatic digestive perforations are digestive surgical emergencies. They are usually considered perforations in free peritoneum. The objective of this work is to study the epidemiological, diagnostic and therapeutic aspects of non-traumatic digestive perforations at the Visceral Surgery Department of the Libreville University Hospital. Patients and method: Retrospective study of patients hospitalized and operated on at the Visceral Surgery Department of the Libreville University Hospital from January 2016 to December 2020 for non-traumatic digestive perforations. Epidemiological, etiological and therapeutic data were studied Results: They represented 14.1% of digestive surgical emergencies. The average age of the patients was 34±13.7 years [16-70 years]. the sex ratio was equal to 2.3. Intraoperatively, duodenal perforations were found in 48.1% of patients (n=37). Among the perforations of the small intestine, 4 were probably of typhoid origin (5.1%). The only colonic perforation was of tumor origin. In case of perforation of duodenal ulcer, suture associated with omental plasty was performed. An appendectomy was performed in all patients with appendiceal perforation. Overall operative morbidity was 3.9%. Mortality was 1.3% (n=1). The average length of hospitalization was 8.3 days. Conclusion: Non-traumatic digestive perforations are frequent digestive surgical emergencies, occurring at all ages, especially in men. The etiologies are numerous. The surgical procedure depends on the etiology.
