Abstract:
PURPOSE: Compare the new training model for vessel ligation with the traditional using rabbits in the assessment. METHODS: From 106 academics of the Division of Operative Technique and Experimental Surgery, 36 were divided into two groups and offered them two training models of ligature. The group was invited to participate to an experimental procedure with rabbits and reproduce the technique trained. The procedure consisted of performing ligatures in saphenous veins of the legs of rabbits. The students were evaluated by objective criteria by an observer evaluator and answered a questionnaire at the end of the procedure. RESULTS: The medical students who have had training with the new model had a higher number of hits compared to the other group performing ligatures without clamping with hemostatic forceps (p<0.05). There was also a higher number of hits for the new model group to perform ligation temporary and permanent ligation with the use of forceps (p>0.05). The questionnaires indicated that the new model develops more ability to work in groups (p=0.088). CONCLUSIONS: The project promotes the implementation of the new model for training ligature, indicating that the new model is better to the traditional surgical practice.

Abstract:
Abstract In the present study we compared the effects of the selective COX-2 inhibitor etoricoxib with those of the classical non-selective NSAID diclofenac on the inflammatory process and alveolar bone loss in an experimental model of periodontitis in rats. Ninety male Holtzman rats (250 g) were randomly sorted into four experimental groups: Sham+CMC and Ligature+CMC (control) groups which received 0.5% carboxymethylcellulose sodium (CMC) solution; Ligature+Diclofenac and Ligature+Etoricoxib groups which received Potassium Diclofenac and Etoricoxib, respectively, suspended in 0.5% CMC (10 mg/kg/day). At 7, 14 and 21 days after placing ligatures in the cervical region of both the lower right and left first molars, the animals were euthanized. At the end of each period, the mandibles were collected for radiographic examination of alveolar bone loss. In addition, alveolar bone and periodontal ligament tissue samples were collected for COX-2 expression analysis and gingival tissues were collected for measurement of PGE2 contents. Animals with ligature-induced periodontal disease showed significant increased COX-2 gene expression at days 7, 14 and 21 (p<0.05) on alveolar bone and periodontal ligament. However, both treatments resulted in significantly reduced alveolar bone loss when compared to the untreated Ligature group (p<0.05), with no statistical difference between Etoricoxib and Diclofenac Potassium groups. This study shows that both drugs were able to reduce alveolar bone loss after periodontal disease induction.

Abstract:
Background: Quercetin is a flavonoid which has potent anti-inflammatory, antibacterial, and antioxidant effect. Purpose of this study was to evaluate effects of quercetin on alveolar bone loss and histopathological changes in ligature-induced periodontitis in rats. Methods: Wistar rats were divided into four experimental groups: non-ligated control (C, n=8) group; periodontitis (P, n=8) group; ligature and low dose quercetin group (75 mg/kg/day quercetin, Q75 group, n=8); ligature and high dose quercetin group (150 mg/kg/day quercetin, Q150 group, n=8). Silk ligatures were placed at gingival margin of lower first molars of mandibular right quadrant. Study duration was 15 days, and animals were sacrificed end of this period. Changes in alveolar bone levels were clinically measured and tissues were immunohistochemically examined, matrix metalloproteinase 8 (MMP 8), inducible nitric oxide synthase (iNOS), tissue inhibitor of metalloproteinase 1 (TIMP 1), Cysteine-aspartic proteases 3 (Caspase 3), and tartrate-resistant acid phosphatase (TRAP) positive osteoclast cells, osteoblast, and neutrophil counts were also determined. Results and Discussion: Alveolar bone loss was highest in P group, and differences among P, Q75, and Q150 groups were significant. Both doses of quercetin decreased TRAP+ osteoclast cells and increased osteoblast cells. Inflammation in P group was also higher than those of C, Q75, and Q150 groups indicating anti-inflammatory effect of quercetin. iNOS, MMP-8, and caspase-3 levels were highest, and TIMP-1 expression was lowest in P group; differences were statistically significant. Conclusion: Within limits of this study, it can be suggested that quercetin administration may reduce alveolar bone loss by increasing osteoblastic activity, decreasing osteoclastic activity, apoptosis, and inflammation in an experimental model of periodontitis.

Abstract:
Background: Arteriovenous (AV) fistulas are considered the gold standard for ensuring safe and long-term vascular access in patients with haemodialysis-dependent end-stage renal disease. However, previous studies demonstrated that high-flow AV fistulas might add additional cardiovascular burden in the post-transplant setting, leading to frequent fistula closure in this population. Currently, there is no consensus regarding management of high-flow fistulas in post-transplant patients with stable kidney function. The present randomized controlled trial examines the effect of prophylactic AV fistula closure on high-output heart failure. Methods: Twenty-eight kidney transplant patients with stable graft function, absence of pre-existing severe cardiac failure, and brachial arterial flow rate of at least 1,500 mL/min were recruited and randomized in a 1:1 ratio to an intervention and control group, respectively. The intervention group was subject to immediate fistula ligature. Patients within the control group were referred to fistula ligature only if the main study endpoint high-output heart failure was reached. The latter was defined by the presence of at least 1 clinical sign (i.e., worsening NYHA score) and at least 2 of the following echocardiographic parameters: diameter of right atrium (major) >53 mm, right atrium (minor) >44 mm, inferior vena cava ≥21 mm, right pulmonary artery >20 mm, TAPSE <16 mm, systolic pulmonal artery pressure >40 mm Hg, and/or left ventricular eccentricity index <1. During a 24-month follow-up period, quarterly measurements of kidney function, NT-proBNP, and lactate dehydrogenase as well as a biannual echocardiographic check-up were performed. Results: High-output heart failure attributable to high-flow fistula was reported in 5 of 13 control patients (38.5%), whereas no patient in the intervention group presented with clinical and echocardiographic signs of high-output heart failure during the follow-up period. Thus, prophylactic ligature of high-flow fistulas avoided high-output heart failure in our patient population (p = 0.013). Three patients in the control group, however, had to undergo fistula ligature due to aneurysm formation (n = 2) and steal phenomenon (n = 1). Median NT-proBNP levels decreased from 317 ng/L pre-ligature to 223 ng/L post-ligature (p = 0.003). Serum creatinine levels did not significantly differ before and after AV fistula ligature (1.69 vs. 1.60 mg/dL, respectively, p = 0.059). Improvement of echocardiographic findings (e.g., a decrease in systolic pulmonary arterial pressure) was found in 7 of 8 ligature patients but did not reach statistical significance. Conclusion: Prophylactic ligature of high-flow AV fistulas after kidney transplantation can avoid high-output heart failure, and a more liberal approach to close AV fistulas might be justified.

Background: Hanging is one of the common methods of suicide in the world. The rate of suicide by hanging is increasing day by day in Bangladesh. Suicide by hanging is the act of intentionally killing oneself by suspension from an anchor point or ligature point (e.g., an over head beam or hook) by a ligature or jumping from a height with a noose around the neck. Hanging is a very simple method of suicide that does not require complicated techniques. The materials required are easily available, and a wide range of ligature can be used, so most of the people commit suicide by suspension from a height with a ligature around the neck.Objective: The objective of this study was to find out demography of hanging, its distribution according to age group, sex, common ligature materials used by victims, observed post mortem findings and in this way try to identify the causative factors and develop the preventive measures that are essential to reduce death due to hanging.Materials and method: A retrospective study was done on the basis of 3rd copies of postmortem reports preserved in the department of Forensic Medicine, Dhaka Medical College, Dhaka, Bangladesh for the period of 2 years, (from January 2008 to December 2009) with the permission of the autopsy surgeons.Results: Out of 574 cases, 159 (27.71%) were male and 415 (72.29%) female. Among them 304 (52.96%) were married and 270 (47.04%) were unmarried. Most of the victims (46.86%) were from the age group 21-30 years. Dopatta (orna) was the commonest (41.28%) ligature material.Conclusion: To reduce the number of suicide by hanging a well designed and comprehensive programme is needed, which will identify the causative factors and which might help in prevention of suicidal hanging.Delta Med Col J. Background In every anatomic lung resection, branches of the pulmonary artery have to be divided. In open surgery, this can be done with ligatures or staplers. In endoscopic surgery, only an endostapler can be used. By routing we ligate the vessels double. Bipolar sealing had yielded promising results, so we wanted to know if we can improve the bursting pressures especially in case of larger vessels by double sealing. Methods Experiments were performed on preparations of the left pulmonary artery extracted at the slaughterhouse. A pressure sensor was implanted at the central end to provide digital measurement of the pneumatic load on the vessel seal and thus establish bursting pressure in each case. Vessels were sealed with MARSEAL 5 (Gebrüder Martin GmbH & Co KG, Tuttlingen, Germany) and SealSafe G3 electric current. The vessels investigated were separated into three sizes: 1 to 6 mm, 7 to 12 mm, and >12 mm. The groups (n = 12 in each) were investigated for each vessel size—Group 1: ligature; Group 2: single seal; Group 3: double seals separated by gap of 0.5 cm; and Group 4: double seals separated by gap of 1.0 cm. Mean bursting pressure (mbar) was calculated for each group. Differences between groups were calculated with Mann–Whitney U test; differences with p < 0.05 were considered significant. Results The ligated vessels in the 1 to 6 mm group showed the highest bursting pressures (mean 515.7 ± 39.6 mbar). Mean bursting pressure in the single seal group was 231.6 ± 47.5 mbar. This was not significantly different from the group with double seals placed 0.5 cm apart. However, bursting pressures were significantly higher in the group with double seals placed 1 cm apart (p < 0.001). Mean value in this case was 308.5 ± 44.5 mbar. In the 7 to 12 mm vessels, mean bursting pressure was highest with ligation at 361 ± 67.1 mbar but was significantly higher in both groups with double bipolar seals (180.3 ± 52.1 mbar with 0.5-cm separation and 277.0 ± 64.5 with 1-cm separation) than in the single seal group (102.7 ± 16.1 mbar). In large vessels (>12 mm), mean bursting pressures were low (66.3 ± 12.7 mbar) with single seals but were significantly higher with double seals (162.3 ± 35.8 mbar [0.5-cm separation] and 137.3 ± 22.9 mbar [1-cm separation]). Conclusions In the ex vivo model of the pulmonary artery, double seals revealed significantly higher bursting pressures than single seals. If there is enough vessel length, the two seals should be placed 1 cm apart.

Abstract:
The purpose of this study was to evaluate the effects of systemic administration of the probiotic Bifidobacterium animalis subsp. lactis HN019 (HN019) on ligature-induced periodontitis in rats with experimental rheumatoid arthritis (RA). 28 rats were divided into four groups (n=7): RA (rheumatoid arthritis), RA/PROB (probiotic), RA/EP (experimental periodontitis) and RA/EP/PROB. From day zero, HN019 was added daily to the water of the PROB groups animals until the end of the experiment. From day seven, RA was induced. On day 28, in EP groups, ligatures were positioned around mandibular first molars and remained in position for 11 days, in order to induce periodontitis. The animals were euthanised on day 39. Microtomographic, histomorphometric, immunoenzymatic and microbiological analyses were performed. Data were statistically analysed (P<0.05). Group RA/EP/PROB presented reduced alveolar bone loss, tumour necrosis factor-α and interleukin (IL)-6 levels and increased IL-17 levels when compared with group RA/EP. There were no significant differences regarding connective tissue attachment level and IL-10 levels between groups RA/EP and RA/EP/PROB. Group RA/PROB showed decreased anti-citrullinated protein antibodies levels when compared with groups RA and RA/EP. Group RA/EP/PROB presented a higher rate of aerobic/anaerobic bacteria than group RA/EP. Systemic administration of HN019 promoted a protective effect against periodontal tissue destruction, decreasing both bone loss and inflammatory mediators and increasing the proportion of bacteria compatible with periodontal health, in rats with experimental RA and EP.

Abstract:
PURPOSE: To verify the development of blood vessels between the greater omentum and the liver in the presence of distinct liver blood intake blockages. METHODS: Two hundred and eighty conventional male Wistar rats were used, divided into 5 groups: control (n=35), laparotomy (n=35); hepatic artery ligature (n=70), ligature of the right-hand branch of the portal vein (n=70); and ligature of both blood vessels (n=70). The last three groups were divided into two subgroups each (n=35), according to the presence or absence of the transposition of the greater omentum onto the right hepatic lobe. The postoperative periods were 1, 3, 7, 15, 30, 60 and 90 days. At the end of each period, the greater omentum and right hepatic lobe were collected for histopathological examination. The presence of blood vessels between the referred tissues was verified by the administration of Indian ink as a marker of vascular lumen. RESULTS: Macroscopic and microscopic observation and the dye marker demonstrated the distribution of blood vessels between the greater omentum and liver tissues. CONCLUSION: The greater omentum was capable of developing blood vessels when fixed to the parenchyma of the liver after the suppression of hepatic blood flow.

Resumo: O objetivo deste estudo foi avaliar a influência do laser Er,Cr:YSGG no tratamento de doença periodontal induzida em ratos por meio das avaliações histológica e histométrica. Foram utilizados 60 ratos Holtzman (Rattus Norvegicus) submetidos a indução de doença periodontal através da inserção de ligadura na região subgengival do primeiro molar superior durante um período de 7 dias. Para o tratamento, as ligaduras foram removidas e os animais aleatoriamente divididos em 4 grupos. Grupo I - nenhum tratamento foi realizado, GRUPO II - raspagem e alisamento radicular, GRUPO III - aplicação do laser e GRUPO IV - raspagem e alisamento radicular associada à aplicação do laser. Após os períodos correspondentes a 7, 15 e 30 dias pós tratamento, os animais foram sacrificados e suas maxilas foram removidas para posterior processamento laboratorial. No período de 7 dias, os animais dos grupos III e IV apresentaram maior área de formação óssea em relação aos demais grupos (p<0.05). Aos 15 dias, a maior média de formação do tecido ósseo foi observada no grupo II quando comparado com os grupos I, III e IV. No período final (30 dias), o grupo IV demonstrou os melhores resultados quanto a área de tecido ósseo presente, em relação aos grupos I, II e III (p<0.05). Conclui-se que a raspagem associada ao laser de Er,Cr:YSGG foi capaz de promover a reparação dos tecidos periodontais a longo prazo<br>Abstract: The purpose of this study was to evaluate the influence of an Er,Cr:YSGG laser therapy in the treatment of induced periodontal disease in rats through histological analyses. Ligatures were placed on the first mandibular molar in a sub marginal position in 60 rats Holtzman (Rattus Norvegicus) during 7 days. After, ligatures were removed and the animals were divided randomly into 4 groups to followed treatments: Group I: None treatment, Group II: scaling and root planning, Group III: application of Er,Cr:YSGG laser and Group IV: laser in association with scaling and root planning. After 7, 15 and 30 days the animals were sacrificed for laboratorial procedures and subsequent histological analyses. After 7 days, the animals in groups III and IV showed a larger area of bone formation than the other groups (p< 0.05). At 15 days, less bone loss occurred in group II compared with groups I, III and IV. In the final period (30 days), group IV showed the best results on the area of bone tissue present, than groups I, II and III (p< 0.05). Resumo: Foram avaliados, clínica e radiograficamente, as reações de quatro diferentes superfícies de implante frente ao desenvolvimento e progressão da peri-implantite: Ticp: titânio comercialmente puro; TPS: titânio revestido com plasma spray de titânio; HA: hidroxiapatita; Ost. Superfícies tratadas com ácidos. Para tanto, foram utilizados seis cães, cujos pré-molares inferiores e superiores foram extraídos. Decorridos 90 dias, os implantes foram aleatoriamente colocados, e iniciou-se o controle químico e mecânico do biofilme bacteriano; após o período de osseointegração (90 dias) foram colocados os conectores, e após 45 dias da colocação dos conectores, iniciou-se a indução da peri-implantite por meio de ligaduras de fio de algodão (suspensão do controle do biofilme bacteriano), e os primeiros exames clínicos (profundidade de sondagem, sangramento à sondagem, índices de placa e sangramento, e mobilidade) e radiográficos (perda óssea) foram realizados. Esses parâmetros foram novamente avaliados após 20, 40 e 60 dias, quando da troca das lidaguras. A análise dos resultados não demonstrou diferença significante entre as superfícies, em nenhum dos parâmetros analisados, com exceção da mobilidade (avaliada pelo PeriotestÒ) e perda óssea, que foram menores nos implantes TPS. Concluiu-se que as superfícies apresentam comportamento semelhante durante a indução da peri-implantite, com exceção no parâmetros mobilidade e perda óssea, que apresentaram os implantes TPS com comportamento superior.<br>Abstract: The reactions of four different implant surfaces were evaluated in regard to the development and progression of peri-implantitis: titanium commercially pure (Ticp), titanium plasma spray (TPS) hidroxiapatite coated (HA) and acid attached surfaces (Ost). Six dogs had their premolars extracted. After 90 days, the implants were ramdonly placed and the dogs receive soft diet, chemical and mechanical plaque control; after the osseointegration period (90 days), the healing abutments were placed. After the healing period (45 days), the plaque control was suspended, and cotton ligatures were installed when the first clinical (probing depth, bleeding on probing, plaque index, bleeding index, and mobility) and radiographic (bone loss) exams were taken. These parameters were re-exanimate after 20, 40 and 60 days, when the ligatures were changed. The results did not show significant differences between the surfaces in any parameter observed except for the mobility (evaluated with PeriotestÒ), and bone loss. These two parameters had the TPS surface with the lower rates. Les processus physico-chimiques de reaction-diffusion ont ete proposes par turing comme une explication possible de l'auto-organisation et de la morphogenese biologique. La theorie prevoit que des systemes chimiques ou biochimiques loin de l'equilibre thermodynamique peuvent former des structures complexes a partir d'un etat initialement homogene. Les processus impliques sont non-lineaires, irreversibles, et consomment continuellement de l'energie. Dans certaines conditions, des solutions de microtubules in vitro se comportent ainsi. La prediction principale de cette theorie consiste en ce que les motifs doivent etre constitues de differences de concentration en reactifs. Dans cette these nous montrons que les motifs macroscopiques stries formes spontanement par les microtubules in vitro, contiennent des variations periodiques dans la concentration en microtubules. Les techniques employees sont la diffusion des neutrons aux petits angles et la fluorescence. C'est la premiere demonstration experimentale de la formation de structures de turing dans un systeme biologique in vitro. Reprenant l'idee initiale de turing, nous nous sommes demandes si ces processus peuvent se produire in vivo pendant l'embryogenese. Un motif microtubulaire strie, mis en evidence par immunofluorescence, apparait dans l'uf de la mouche drosophile au moment de la gastrulation. Une caracteristique des motifs formes par des processus de reaction-diffusion est leur sensibilite aux dimensions de l'echantillon. Nous avons trouve que le motif microtubulaire forme dans des ufs ligatures varie avec la longueur du fragment de la meme maniere que celui apparaissant in vitro dans des recipients de longueurs differentes. Ces resultats sont egalement en accord avec la theorie et apportent un element de preuve que les processus de reaction-diffusion peuvent se produire pendant l'embryogenese de la drosophile. Enfin, la comparaison du motif microtubulaire avec le motif d'expression d'un gene reconnu jouer un role dans la segmentation, suggere qu'il pourrait exister un lien entre les deux systemes.

Abstract:
This chapter examines ideological developments unfolding in the last months of the civil war. In particular, it examines the coalescence of a “propaganda collective,” centering on John Lilburne and his closest collaborators, which began to promulgate a distinctive vision of political reality, put forward as the need for a final settlement became evident. This “collective” established many of the ideological, organizational, and personal ligatures that would hold together the Leveller movement in 1647. However, this collective remained in 1645 an integral part of the wider “independent” coalition, sharing broader political and ideological objectives with other constituents of that coalition. Moreover, there were also important developments among other independent sympathizers, evident most notably in the emergence, for the first time, of overtly republican sentiments and arguments at the end of 1645.

Abstract:
Intrinsic to glaucoma surgery using a tube shunt is the management of early postoperative hypotony. This consideration is unavoidable in all cases when using a tube shunt without an intrinsic valve and must still be considered in tube shunts that contain a valve, as the valve may not function as anticipated. Unfortunately, in the attempt to avoid hypotony and its associated complications, we are faced with elevated intraocular pressure (IOP) and its associated difficulties. However, the attempt to control IOP is not the only consideration when anticipating intentional tube occlusion. Additional factors such as technical complexity of the procedure, predictability of IOP in the early postoperative period, potential to reverse occlusion either partially or completely, and the impact on the long-term function of the tube shunt must be considered. The desired endpoint when occluding a tube intentionally is the complete prevention of flow to the tube shunt reservoir. The standard ways to occlude the tube are an external encircling ligature or an internal occluding suture, otherwise known as an “obturator” or a “ripcord,” or some combination thereof. With the external suture technique, prior to placing the reservoir, a 7–0 or 8–0 polyglactin 910 (Vicryl™ , Ethicon, Inc., Somerville, New Jersey) suture is tightly tied around the tube approximately 4–6 mm from the reservoir (Figure 35.1). It is anticipated that this suture will dissolve in about one month, opening the tube. However, the timing of opening may be highly variable between individuals, and that variability may be problematic. Alternatively, a 9–0 polypropylene suture can be placed around the tip of an anterior chamber tube with release performed by laser lysis. To prevent the polypropylene suture from floating freely in the anterior chamber after release, a pass should be made through the wall of the tube during placement. Alternatively, with the “ripcord” technique, the end of a 3–0 polypropylene suture without the needle is threaded into the distal opening of the tube at the reservoir for a distance of 4–6 mm.

Abstract:
Defibrotide was able to prevent the hemodynamic and biochemical alterations caused by acute myocardial ischemia (AMI) induced by coronary occlusion in the cat when infused 3.5 h before and 5 h after left anterior descending coronary artery (LAD) occlusion. In the platelet perfused heart, Defibrotide was a selective stimulator of coronary vascular PGI^ but not of platelet thromboxane formation. The present study was designed both to investigate the effects of Defibrotide injected 30 min after the induction of acute myocardial ischemia (AMI) in the cat and to evaluate the ability of this drug to reduce infarct size. In the first set of experiments a permanent ligature (5 hours) was placed around LAD. ST segment from ECG, mean aortic pressure (MAP), heart rate (HR) and the pressure-heart rate index (PRI) were considered. Plasma and tissue creatine phosphckinase activity (CFK), tissue lactate and ATP were measured by enzymatic kits from Boehringer Biochemia. 30 min after coronary occlusion a loading dose of Defibrotide (32 ng Kg-1 ) was administered i.v. immediately followed by an infusion (32 ng Kg-1 h 4.5 h-1) MAP, HR and PRI were not modified either by AMI or by the infusion of Defibrotide. AMI-ST segment increases were reduced by Defibrotide from 0.5 h after the beginning of the treatment (—49% vs. AMI control) to the and of experiments (-83% vs. AMI control after 5 h occlusion period). Plasma CFK was reduced from 2.5 h after the beginning of the treatment (-29%) till the end of experiments (-52%). Ischemic tissue CFK, lactate and ATP were normalized by Defibrotide. In the second set of experiments the animals were infused with Defibrotide (50 or 200 mg Kg-1 h-1 , i.v.) starting 2 hours before coronary ligature. The infusion was maintained throughout the 5 h occlusion period. The risk and infarct areas were measured by Evans blu and nitroblue tetrazoliun staining. The 51 ± 3% of risk area was infarcted in AMI control cats. Defibrotide at the two tested doses significantly reduced these infarct areas to 42 ± 4% and 34 ± 2% of risk areas respectively. The beneficial effects of Defibrotide observed in AMI could be attributed both to its ability to enhance PGI2 release from vascular walls and to improved local tissue oxygenation and energy supplies. However it could be taken into account a direct cytoprotective action.
