Abstract:
Atlanto-axial (AA) joint injections can be helpful in the diagnosis and management of occipital headaches stemming from the AA joint. Because of the complicated anatomic landscape, imaging guidance is vital in addition to a sound understanding of the anatomy. Several different image modalities are available for use in AA joint blocks. Fluoroscopy is the most widely used for its familiarity, relative low cost, and wide availability. The addition of digital subtraction angiography to the conventional fluoroscope offers an additional safety benefit when performing injections near critical blood vessels. Computed tomography guidance offers a better view that allows for visualization of the soft-tissue structures; however, its risk/benefit ratio limits its use in the everyday pain practice. Flat detector CT may offer a promising compromise, incorporating delayed CT images into the conventional fluoroscopy procedure. Safety is the highest priority when performing AA joint blocks to prevent potentially devastating outcomes.

Abstract:
The lung is a relatively new site for the application of ultrasound. Beyond the more established assessment of pleural effusion, this organ has been traditionally considered off limits for sonographic investigation, since air is a well-known foe of the ultrasound beam. However, in recent years it has been shown that this apparent physical limitation can be overcome when the air content decreases, as happens in a diseased pulmonary parenchyma. The most useful lung ultrasound sign for cardiologists is the presence of B-lines, the sonographic hallmark of pulmonary interstitial syndrome, including interstitial pulmonary oedema. Bilateral multiple B-lines are present in patients with pulmonary congestion and may help assess and semiquantify the extent of extravascular lung water in patients with heart failure. This sign is low cost, easy to perform, can be repeated at bedside, and does not employ ionizing radiation. Lung ultrasound is also useful for detecting other pulmonary conditions such as pneumothorax, and lung consolidations such as pneumonia or pulmonary infarction.

Abstract:
Este estudo objetiva avaliar o desenvolvimento motor, estado nutricional, nível de mercúrio e perfil socioeconômico de crianças residentes na Ilha de Cotijuba, Pará, Brasil, na faixa etária de 5 a 10 anos. Metodologicamente, o desenvolvimento motor foi avaliado através do protocolo TGMD – 2 (Test of Gross Motor Development, Secunde Edition). A avaliação do estado nutricional da amostra, utilizou o software WHO AntroPlus. A análise de mercúrio foi realizada através da metodologia de redução e espectrometria de absorção atômica por vapor frio (CVAAS) (sistema aberto de circulação do fluxo de ar), utilizando um detector de mercúrio automático, o Automatic Mercury analyzer Model HG-201 da Sanso Seisakusho Co. Ltd., Tokyo, Japan. Para a avaliação socioeconômica da amostra, foi utilizado o instrumento CCEB 2014 (Critério de Classificação Econômica Brasil 2014) da ABEP. A análise dos dados foi realizada pelo programa SPSS 22.0, em que se adotou a estatística descritiva de tendência central, não central e dispersão e para a estatística inferencial primeiramente foi realizado o teste de Kolmogorov-Smirnov para normalidade da amostra, e o Teste t independente para comparar as médias da amostra de acordo com o sexo e grupo etário. O teste de correlação de Pearson foi utilizado para analisar o grau de associação entre as variáveis. Foi adotado um nível de significância p≤0,05. O desenvolvimento motor encontrou-se na média em sua maioria, seguido por abaixo da média e, posteriormente, acima da média. Quanto ao estado nutricional, a maioria apresentou peso adequado para a idade, estatura adequada para idade e eutrofia com o aparecimento de alguns casos de sobrepeso e obesidade com o avançar da idade. O nível de exposição ao mercúrio encontrou-se abaixo do limite de tolerância biológico, com diferença significativa entre sexo. O perfil socioeconômico das famílias, na sua maioria, perteceu às classes C2 e D. A renda média bruta das famílias, majoritariamente, apresentou-se entre R$ 1277,00 e R$ 895,00. Conclui-se que a avaliação do desenvolvimento motor e os fatores intervenientes a este estão na normalidade. Análises similares devem ocorrer periodicamente para planejamento legislativo e projetos de intervenção em educação e saúde.

Myocardial infarction is defined pathologically as myocyte necrosis due to prolonged ischaemia. These conditions are met when there is a detection of a rise and/or fall of cardiac biomarkers, preferably troponins, with at least one value above the 99th percentile of the upper reference limit, together with evidence of myocardial ischaemia, as recognized by at least one of the following: symptoms of ischaemia, electrocardiographic changes of new ischaemia, the development of pathological Q waves, imaging evidence of a new loss of viable myocardium or new regional wall motion abnormality, or the identification of an intracoronary thrombus by angiography or autopsy.

Abstract:
This chapter examines solutions to the Maxwell equations in a vacuum: monochromatic plane waves and their polarizations, plane waves, and the motion of a charge in the field of a wave (which is the principle upon which particle detection is based). A plane wave is a solution of the vacuum Maxwell equations which depends on only one of the Cartesian spatial coordinates. The monochromatic plane waves form a basis (in the sense of distributions, because they are not square-integrable) in which any solution of the vacuum Maxwell equations can be expanded. The chapter concludes by giving the conditions for the geometrical optics limit. It also establishes the connection between electromagnetic waves and the kinematic description of light discussed in Book 1.

The first order of business in crisis management is figuring out that there is a crisis. Once a brewing crisis is recognized, health officials can organize a coherent response, limit its impact, and even make an early pivot to achieve long-lasting change. Unfortunately, spotting a crisis early is far easier said than done. It’s the rare crisis that announces itself with a phone call 12 hours in advance. Most crises go unnoticed even as clues emerge, lost in the stream of the daily activity of an agency or hidden by biases, assumptions, and wishful thinking. To be successful, officials and their agencies should pursue a proactive strategy to identify crises early. There are three elements of effective crisis detection: spotting signals, pulling in data and assessing the situation, and developing a space and culture to put the pieces of the puzzle together.
