Abstract:
Diabetes and periodontal diseases (PDs) exhibit a bidirectional relationship centered on an enhanced inflammatory response that manifests both locally and systemically. Diabetes is an established risk factor for PD, whereas the treatment of the latter has been shown to improve glycemic control in diabetic patients. Although compelling evidence fromin vitroand animal studies supports a plausible biological explanation for the relationship between the two conditions centered on systemic low-grade inflammation, the limited number of comparable large randomized clinical trials is reflected in the limited specific guidelines offered by the international organizations for diabetes and periodontitis regarding the management of the two diseases in an individual. Further understanding of the biological phenomena underlying PDs and diabetes is critical for individual therapeutic approaches to patients with both conditions by endocrinologists and periodontists.

Abstract:
Intense debate has focused on the use of statistical predictions of dangerousness in the criminal justice system. Two conflicting positions maintain wide support: that such predictions are never appropriate in criminal justice decision-making, and that they should be used far more often. Recognizing the fact that implicit and intuitive predictions are made every day in police, prosecutorial, sentencing, and other decisions, and explicit but unscientific predictions are common, this article suggests a theoretical framework justifying limited use of statistical predictions. Statistical predictions may present, in some instances, a morally preferable alternative to biased nonscientific and implicit judgments. Development of a sound jurisprudence of predictions faces major hurdles given the trend toward unscientific predictions in the law and the enormous judicial confusion in dealing with predictions. The concept has contributed to a string of notably poor Supreme Court decisions.

In the era of smaller and smaller biopsies submitted to pathology departments for diagnosis and the advent of personalized medicine, it has become imperative to efficiently and effectively use patient material to reach individualized, actionable diagnoses. The use of fine needle aspirates and core biopsies as acceptable methods for obtaining sufficient material for hematopoietic neoplasms under nonemergent conditions is debatable. There are, however, scenarios where only limited material is obtainable due to anatomic site, size of the lesion or condition of the patient. In these types of settings, thoughtful approaches and unconventional means are often necessary to reach a diagnosis. In this article, we describe three such scenarios and the unique tactics taken in each to obtain a personalized actionable diagnosis.

The hippocampus plays an important role in human memory and is known to be vulnerable to extreme hyperglycemia and hypoglycemia in animal models of type 1 diabetes. Within humans with type 1 diabetes, exposure to glycemic extremes has been associated with alterations in hippocampal structure and in memory function, but results are inconsistent. It has been hypothesized that the effects of hypoglycemia and hyperglycemia on the hippocampus may depend on when during neurodevelopment these extremes occur, possibly explaining some of these inconsistencies. However, data addressing this concept are limited. We review here the existing literature on this complex topic and suggest future avenues of required research.

Abstract:
The United States intervention in Haiti concludes another chapter in the development of the constitutional common law of presidential power. The Haiti experience further confirms the constitutional authority of the President to deploy armed forces into hostile foreign environments, and to initiate the use of force without prior, specific congressional authorization. The facts of the situation limit the “precedent” to small-scale interventions where the risk of major military engagements, either initially or upon escalation, is negligible. The cases of largescale hostilities, like Korea, Vietnam and Iraq, are quite different in fact and perhaps also in law. But the Haiti “precedent,” coupled with the recent interventions in Grenada and Panama and innumerable examples earlier in history, strongly supports an unqualified presidential power to carry out small-scale military operations in support of foreign policy goals.
