Abstract The concept of solidarity has received increasing attention in discussions about public health interventions, both as a possible justification for such interventions and as a possible motivating factors for individual action. This chapter provides an analysis of whether thinking through a lens of solidarity is likely to be helpful in devising strategies and policies to combat antimicrobial resistance. It first provides a critical overview of recent accounts of solidarity and argues that solidarity must be understood as a group based concept. It then applies this conception of solidarity to individual use of antibiotics through a case study of the antibiotic treatment of moderate and severe acne where it is argued that solidarity based thinking is valuable within a context of shared decision-making. Issues of policy making are then discussed and it is argued that basing a policy on solidarity on the one hand constrains the methods chosen to pursue public health goals, but that on the other hand solidarity may provide a strong and durable motivation to comply with such a policy. The limits of solidarity are explored in the final section and it is concluded that 1) the concept of solidarity does have an important role to play in thinking about public health, 2) considerations of solidarity can help us shape the goals and methods of public health policies in the area of antibiotics, and 3) that it is likely that solidarity may also be helpful in thinking through other contentious issues in public health.

Abstract Restrictive policies that limit antimicrobial consumption, including therapeutically justified use, might be necessary to tackle the problem of antimicrobial resistance. We argue that such policies would be ethically justified when forgoing antimicrobials constitutes a form of easy rescue for an individual. These are cases of mild and self-limiting infections in otherwise healthy patients whose overall health is not significantly compromised by the infection. In such cases, restrictive policies would be ethically justified because they would coerce individuals into fulfilling a moral obligation they independently have. However, to ensure that such justification is the strongest possible, states also have the responsibility to ensure that forgoing antimicrobials is as easy as possible for patients by implementing adequate compensation measures.

Not so in South Africa, where pension policies did not undergo any significant reforms during the entire apartheid era (1948–1990). This chapter takes a constructivist approach to unpacking the marginal reform efforts in social security in an otherwise reform-fervent apartheid regime. Through in-depth analyses of historical accounts and parliamentary debates, I investigate the ideational underpinnings of and justifications for pension policy developments. The reasons for the state’s limited pension reforms are the ideas of “separate development” and different levels of “civilisation” whereby the white regime could justify taking primary responsibility for only their “own people”.

AbstractThis chapter examines how Taiwan’s written media justified the state’s introduction of funeral reforms in the second half of the twentieth century. Situating this case study within the broader sociopolitical context of contemporary Taiwan, it illustrates how discourse analysis can be used as a tool for studying change. The state-led reforms induced changes in a field in which religious rituals play an important role, as state authorities operated with priorities differing from ritual practice. Instead, they were concerned with measures for land saving and popularized practices such as cremation or natural burials. The discourse analysis reveals that the justifications brought forward for reforms appeared with a high degree of consistency starting from the late 1970s in Taiwanese press articles. Following Michel Foucault’s understanding of discursive formations, four sub-formations can be distinguished, which all have in common that they are aimed at problematizing ritual practices prevalent at funerals. These sub-formations consisted of considerations concerning the quantitative limits of available cemetery land for graves, arguments referring to the economic advantages of cremation, articulations of the ideal of green cemeteries designed in a park-like fashion, and a critique of geomancy in labeling it superstitious. The discursive voices emerging in the sub-formations were state and local authorities, as well as experts and journalists commenting on reform measures. These priorities and justifications for reforms appeared to be incompatible with religious funeral rituals and are analyzed as changes in terms of a secularization process of Taiwan’s funerary practice. An important finding is that the secular reform measures were, to a large extent, inspired by similar reforms in other regions in the world, and are as such part of a global pattern.

Often, this limit may be selected based on examination of stress levels in the flanges.
