Abstract:
Intradural disc herniation is a rare entity, representing between 0.26% and 0.30% of all herniated discs. 92% of cases occur in the lumbar spine, more frequent at the L4-L5 level. The diagnostic and therapeutic conduct performed in a 61-year-old male patient who came to the office with a one-year and six-month history of dorsolumbar pain is described, accompanied by sensory alterations in the lower limbs. In view of the exacerbation of symptoms, a simple magnetic resonance study was performed, observing an intraspinal, intradural, extramedullary lesion at the D12-L1 space. An intradural disc herniation is suspected. After being evaluated the case in the group of Neurosurgeons (Group of Spinal Surgery) of the Institute of Neurology and Neurosurgery of Havana, it is proposed to carry out surgical treatment, which consisted of the extraction with microsurgical technique of the Herniated disc, preserving the vertebral anatomy through the laminoplasty technique, an open book variant. The patient evolved satisfactorily after three months of follow-up. It is concluded that intradural disc herniation should be included among intradural, extraxial lesions of the spine. By means of the magnetic resonance study, a group of imaging signs that support the preoperative diagnosis of this lesion can be described. Surgery is the definitive therapeutic method and allows an accurate diagnosis of spinal herniation to be established.

Abstract:
Purpose: Describe an unusual development of fungal keratitis caused by Acremonium sp in six patients who underwent cataract surgery at an ophthalmology service in Brazil, as well as to report the origin of these infections. Methods: Swabs from the affected corneas were collected to perform culture for bacteria and fungi. These materials were sown in Blood agar (Difco/ USA), Macconkey agar (Difco/USA) and Thioglycolate broth (Difco/USA) for bacterial research and Sabouraud agar (Difco/ USA) for fungi research. A microbiological study was carried out to analyze surgical instruments, the environment and other materials used in the surgeries. Results: Case reports occurred with six patients, all aged over 71 and 85 years, who underwent a surgical procedure for cataract correction by an Ophthalmology service. After surgery, these patients presented loss of unilateral visual capacity, with the formation of a white mass on the cornea. Swabs from the affected corneas were collected to perform culture for bacteria and fungi, in specific culture media. The bacterial cultures showed negative results. Fungal cultures revealed the presence of Acremonium spp. Conclusions: It is likely that Acremonium spores found in the wardrobe were deposited on all the sterile material stored there. At the time of cataract surgeries, this material was unpacked and fungal structures became detached and found in the surgical field of these patients' eyes a gateway. Thus, cleaning and sanitizing measures for surfaces must be implemented and monitored, especially in critical areas such as in hospital areas, in order to avoid damage to patients' health

Abstract:
We present the case of a 13-year-old male patient with a two-month clinical picture of global headache and progressive decrease in visual acuity. Neuroimaging showed a well-defined giant left frontal lesion, hypointense in T1, with heterogenous signal in T2 and intense enhancement after gadolinium. The patient was treated with surgical resection and the lesion confirmed pathologically as Cavernous Hemangioma with focal component of aneurysmal bone cyst. The aneurysmal bone cyst is a benign lesion of rapid growth and very destructive, which commonly occurs in metaphysics of long bones or axial skeleton, requiring radical treatment. It was first described in 1942, as an injury that occurred mostly in people under 20 years of age with a peak between 12 and 13 years, but very rarely occurs in the head and neck, mostly involving the occipital bone. The case is presented below together with a review on the subject of Aneurysmal Bone Cyst, through databases such as EMBASE, PubMed and COCHRANE, selecting 70 articles that covered relevant aspects of its physiopathology, clinical and radiological manifestations and surgical approach.

Abstract:
The gallbladder hypoplasia is a rare congenital anomaly. We present a case of a 38-year-old man who was referred to us for surgical management of gallbladder disease. The patient presented abdominal pain compatible with repeating biliary colic as the first clinical manifestation. Preoperatively, the magnetic resonance cholangiopancreatography imaging was useful to study thoroughly the biliary tract in order to avoid any iatrogenic injury. Intraoperatively, laparoscopy revealed a rudimentary gallbladder and the histopathological examination confirmed the diagnosis of hypoplasic gallbladder. Laparoscopic cholecystectomy was successful and the patient did not present abdominal pain again.
