Abstract:
Environmental change is a ubiquitous feature of the conditions faced by species, so they must either evolve, move to avoid threats, or perish. Species require genetic diversity to evolve to cope with environmental change through natural selection (adaptive evolution). The ability of populations to undergo adaptive evolution depends upon the strength of selection, genetic diversity, effective population size, mutation rates and number of generations. Loss of genetic diversity in small populations reduces their ability to evolve to cope with environmental change, thus increasing their extinction risk. Adaptive evolution in the short to medium term predominantly utilizes pre-existing genetic diversity, but new mutations make increasing contributions in later generations. Evolutionary potential can be estimated from the heritability of fitness in the environment of interest, or by extrapolation from genomic diversity.

The contributors to Long Term use the tension between the popular embrace and legalization of same-sex marriage and the queer critique of homonormativity as an opportunity to examine the myriad forms of queer commitments and their durational aspect. They consider commitment in all its guises, particularly relationships beyond and aside from monogamous partnering. These include chosen and involuntary long-term commitments to families, friends, pets, and coworkers; to the care of others and care of self; and to financial, psychiatric, and carceral institutions. Whether considering the enduring challenges of chronic illnesses and disability, including HIV and chronic fatigue syndrome; theorizing the queer family as a scene of racialized commitment; or relating the grief and loss that comes with caring for pets, the contributors demonstrate that attending to the long term offers a fuller understanding of queer engagements with intimacy, mortality, change, dependence, and care. Abstract:
Intensive care unit-acquired weakness (ICUAW) is a significant and common complication with major implications for survivors of critical illness. ICUAW is a clinical diagnosis made in the presence of generalized muscle weakness that occurs in the setting of critical illness when other causes of muscle weakness have been excluded. Critical illness polyneuropathy and myopathy are the most common causes of ICUAW. Short-term implications of ICUAW include alveolar hypoventilation and an increased risk of pulmonary aspiration, atelectasis, and pneumonia—factors which may contribute to acute respiratory failure and ICU re-admission. In the long term, ICUAW has been associated with physical disturbances, including unsteady gait, sensory loss, foot drop, and, in more severe cases, persistent quadriparesis and ventilator dependency. ICUAW appears to heavily influence the failure of ICU patients to return to baseline health status post-discharge. There is a paucity of evidenced-based therapeutic strategies to reduce the incidence of ICUAW; however, early rehabilitative therapy might represent an effective measure in improving functional status.

Abstract:
Distal arm weakness may be caused by involvement of the intrinsic hand muscles (interossi, thenar and hypothenar muscles, lumbricals) or extrinsic hand muscles (long finger flexors and extensors). ALS is typical for the former type, and IBM is typical for the latter type. Incoordination of skilled finger movement due to cerebellar disease is associated with normal strength. Poor mobility due to joints pain and swelling should not be confused with muscle weakness. Mononeuropathies such as ulnar, radial, median, and AIN,lesions usually cause differential loss of function. Myasthenia sometimes causes weakness of the wrist and finger extensors. A small but distinct group of distal hereditary myopathies should always be borne in mind. Progressive sensorimotor neuropathies are usually associated with sensory symptoms. Multifocal motor neuropathy can be a diagnostic challenge.
