Abstract:
The Lao People’s Democratic Republic is nearly fifty years old, and one of the few surviving one-party socialist states. Nearly five decades on from its revolutionary birth, the Lao population continues to build futures in and around a political landscape that maintains socialist rhetoric on one hand and capitalist economics on the other. Contemporary Lao politics is marked by the use of cultural heritage as a source of political legitimacy. Researched through long term detailed ethnography in the former royal capital of Luang Prabang, itself a UNESCO recognised World Heritage Site since 1995, this book takes a fresh look at issues of legitimacy, heritage and national identity for different members of the Lao population. It argues that the political system has become sufficiently embedded to avoid imminent risk of collapse but suggests that it is facing new challenges primarily in the form of rising Chinese influence in Laos.

Abstract:
Interest in the global burden of critical illness and its sequelae are growing, but comprehensive data to describe the burden of acute and post-acute illness and the resources available to provide care are lacking. Challenges to obtaining population-based global estimates of critical illness include the syndrome-based definitions of critical illness, incorrect equating of ‘critical illness’ with ‘admission to an intensive care unit’, lack of reliable case ascertainment in administrative data, and short prodrome and high mortality of critical illness, limiting the number of prevalent cases. Estimates of the burden of post-critical illness morbidity are even less reliable, owing to the limited number of observational studies, inaccurate coding in administrative data, and the unclear attributable risk of these morbidities to critical illness. Modelling techniques will be required to estimate the burden of critical illness and disparities in access to critical care using existing data sources. Demands for critical care and post-discharge care for survivors are likely to increase because of urbanization, an ageing demographic, and ongoing wars, disasters, and pandemics, while the ability to assume the cost of increased critical care may be limited due to economic factors. Major public health questions remain unanswered regarding the worldwide burden of critical illness and its sequelae, variation in resources available for treatment, and strategies that are broadly effective and feasible to prevent and treat critical illness and its consequences.

This chapter provides a synthesis of some of the useful prison policies discussed throughout this volume. The sources of useful versus harmful policies in addition to the implications of the latter are discussed. Perhaps the most common source of harmful policies has been heavier emphases placed on punishment by politicians and court actors who are further removed from the prison experience. Common denominators of policies that have generally improved the welfare of prisoners and/or prison staff, on the other hand, include grounding in an increasingly humanitarian view of offenders, a growing awareness of both short- and long-term adverse effects of incarceration on offenders and the general population, greater reliance on empirically based strategies, and interagency collaborations to ensure long-term solutions while minimizing unanticipated ill effects. The greatest obstacles to overcoming harmful policies are also reviewed, highlighting the importance of cumulative knowledge and ongoing empirical research on best practices.

Ecology of the Shortgrass Steppe: A Long-Term Perspective summarizes and synthesizes more than sixty years of research that has been conducted throughout the shortgrass region in North America. The shortgrass steppe was an important focus of the International Biological Program's Grassland Biome project, which ran from the late 1960s until the mid-1970s. The work conducted by the Grassland Biome project was preceded by almost forty years of research by U.S. Department of Agriculture researchers-primarily from the Agricultural Research Service-and was followed by the Shortgrass Steppe Long-Term Ecological Research project. This volume is an enormously rich source of data and insight into the structure and function of a semiarid grassland.

Abstract:
This chapter examines the constitutional relationships between the executive and the legislature in Anglophone Africa, focusing on Ghana, Kenya, Namibia, and South Africa. The independence of Anglophone Africa led to the establishment of constitutional institutions that were mostly based on the Westminster system, which hardly ever migrated well. The chapter shows that constitutional supremacy has displaced Westminster parliamentary supremacy, but the constitutions usually establish parliaments in the Westminster tradition as the central institutional check on the executive. In practice, however, parliamentary checks are rarely effective, which reveals conceptual uncertainties regarding the foundational source and appropriately balanced allocation of constitutional power. The difficulty lies therein that both the American and British examples from which respectively the presidential and parliamentary constructions of the constitutions are drawn, presuppose regular shifts in electoral outcomes based on political performance, whereas the African pattern tends towards long-term incumbency of dominant political groupings.

This chapter looks at recent initiatives in the context of the European Commission's flagship plans for a Capital Markets Union (CMU) designed to encourage a pan-European private placement market. In reality, private placements are mainly available as a funding tool for medium-sized and larger companies. Nonetheless, private placements are regarded by CMU's policymakers as an alternative source of long-term funding, which is simple enough for smaller corporates and small and medium-sized enterprises (SME), and with benefits that they might find attractive. The Commission's initial intention in the design of a CMU was to enable a greater use by SMEs of private placements.

Significant advances have been made in the treatment of antisocial and aggressive behavior (conduct disorder), a severe source of impairment among children and adolescents. Several psychosocial interventions with strong evidence in their behalf with children and adolescents are highlighted in the chapter. They include parent management training, multisystemic therapy, multidimensional treatment foster care, cognitively based treatments, functional family therapy, brief strategic family therapy, and the Good Behavior Game. The treatments have been very well studied, with multiple replications. There remain questions about the long-term impact of treatments, the persons for whom one or more of these treatments are well suited, and how to optimize therapeutic change.

In this chapter I provide a methodological framework for the rest of the book to help explain Poland’s long-term economic backwardness, its unprecedented economic success since 1989, and its development prospects. I argue that the existing economic literature largely focuses on the proximate causes of economic growth, but underestimates the fundamental causes of growth—institutions, culture, ideologies, leaders, and luck—which drive economic policies and are ultimately the reasons why some countries are rich while others are poor. I then focus on the role of institutions and culture in explaining economic development. Following Acemoglu, Johnson and Robinson (ARJ), I distinguish between ‘extractive societies’ and ‘inclusive societies’, and argue that it is rare to move from one type of society equilibrium to another. I extend the ARJ institutional framework and show the conditions necessary for the shift from extractive to inclusive societies and vice versa.

Sacroiliac joint (SIJ) injections are valuable tools for diagnosing the source of low back pain and selecting patients for a radiofrequency ablation procedure, which tends to provide long-term relief for low back pain associated with SIJ dysfunction. Sacroiliac joint injections are generally safe and well-tolerated procedures. The most common complication is initial pain from distension of the joint capsule with contrast and local anesthetic. Despite adequate intra-articular needle placement, extravasation of local anesthetic may diffuse to lumbosacral nerve roots and/or the sciatic nerve, causing transient numbness and/or weakness. This chapter reviews the advantages of fluoroscopically guided SIJ injections as well as the step-by-step technique and how to avoid complications.

Abstract:
This chapter evaluates the large volume of creative scholarship that has reinterpreted and recast our understanding of the ‘heroic age’ of parliamentary reform before the early twentieth century. In doing so, it argues that this varied body of work in itself highlights the value of parliamentary reform as an area for historical research, not least because it has acted as a fertile source of new questions and approaches for political history more generally. Its centrality to accounts of Britain’s political past makes the conspicuous absence of historical accounts of parliamentary reform over the longue durée puzzling. The chapter ends by discussing whether a long-term analysis of parliamentary reform is desirable or possible and examining the potential for historical research into parliamentary reform after 1945.
