Despite the growing need for long-term care, the United States does not have a coherent set of long-term care policies. The existing patchwork of programs and services can be difficult for patients and their families to understand and fails to adequately support many of those in need of care. This chapter traces the historical background of long-term care policy and assesses the three formal channels through which individuals currently navigate long-term care. It addresses the strengths and weaknesses of long-term care coverage briefly through Medicare and private long-term care insurance, and much more fully through Medicaid. The chapter concludes by focusing on families, particularly women, who continue to provide extensive care through informal care work. It is the most vulnerable older and disabled Americans, particularly those who are women, black and Hispanic, and single, and their families who face the greatest difficulties under the current system and who will be most affected by future policy changes.

Sciatic neuropathy presents with buttock pain worsened by sitting on the affected side and associated with ankle and knee extension weakness. Electrodiagnostic evaluation will help to distinguish it from peroneal or tibial mononeuropathies, lumbosacral plexopathy, or lumbosacral radiculopathy. It can be difficult to distinguish from a peroneal mononeuropathy due to the preferential involvement of the peroneal division of the sciatic nerve. EMG study of the short head of the biceps femoris allows for distinction between these entities. Long-term outcome and prognosis studies are sparse although Preservation of distal lower-extremity strength may be a significant predictor earlier and/or better clinical recovery.

Abstract:
A study of conflicting passions in Hume’s theory reveals that several psychological principles explain how these passions interact, often making the dominant passion even stronger. Hume’s distinction between violent passions and causally strong ones, and between calm passions and causally weak ones, is essential to his theory of motivation; however, it introduces questions about our ability to moderate emotional upheaval. The person most likely to find true happiness has “strength of mind”: the prevalence of calm passions over violent ones, such as concern for long-term good over intensely-felt interest in short-term good. Several principles emerge from Hume’s discussion of passionate dynamics to explain how a person deficient in this virtue might develop it. Self-moderation of the passions is possible, contrary to the warnings of the early modern rationalists, although within certain limits.

Osteoporotic fractures are major causes of suffering and death. Dual-energy x-ray absorptiometry (DEXA) is the standard of care for diagnosis (T-score ≤ –2.5) of osteoporosis. Prevention of fractures requires addressing bone and muscle strength and balance. Physical exercise, good nutrition (fruits, vegetables, adequate calcium), adequate vitamin intake (C, D, and K), tobacco cessation, and no more than moderate alcohol intake enhance bone health and decrease fracture risk. Long-term treatment with glucocorticoids, certain drugs used in breast or prostate cancer treatment, and proton pump inhibitors used for gastroesophageal reflux disease may increase the risk for osteoporosis. Pharmacologically, bisphosphonates are the mainstay of osteoporosis treatment.

This chapter explores the interactions between economic growth and human development, as measured by the Human Development Index, theoretically and empirically. Drawing on many studies it explores the links in two chains, from economic growth to human development, and from human development to growth. Econometric analysis establishes strong links between economic growth and human development, and intervening variables influencing the strength of the chains. Because of the complementary relationship, putting emphasis on economic growth alone is not a long-term viable strategy, as growth is likely to be impeded by failure on human development. The chapter classifies country performance in four ways: virtuous cycles where both growth and human development are successful; vicious cycles where both are weak; and lopsided ones where the economy is strong but human development is weak, or conversely ones where human development is strong but the economy is weak.

This chapter compares the leadership capital of two long-serving UK prime ministers: Tony Blair and Margaret Thatcher, treble election winners who held office for a decade. Mapping their capital over time reveals two very different patterns. Thatcher began with low levels of capital, building to a mid-term high and final fragile dominance, though her capital fell between elections. Blair possessed very high levels from the outset that gradually declined in a more conventional pattern. Both benefited from electoral dominance and a divided opposition, Thatcher’s strength lay in her policy vision while Blair’s stemmed from his popularity and communication skills. The LCI reveals that both prime ministers were successful without being popular, sustained in office by the electoral system. Towards the end of their tenures, both leaders’ continued dominance masked fragility, ousted when unrest in their parties and policy unpopularity eroded their capital.

Abstract:
In this chapter, we present an overview of some major topics and themes explored in the research on team creativity. We discuss the strengths and limitations of some primary methodological approaches to the study of creativity in teams, including short-term team settings, long-term team settings, and case studies. We also explore some of the major theories of collaborative creativity, which to varying degrees focus on contextual and organizational factors, as well as motivational, cognitive, and social processes involved in enhancing innovation in teams. We evaluate the sometimes conflicting findings from research on team size, participative safety, conflict, affective processes, and supportive versus constrained environments. At a broader level, we discuss interteam and network dynamics as they impact team innovation. Finally, we summarize some areas of research that seem to have conflicting or paradoxical effects, suggesting areas of future research focus.
